Abstract Streptococcus ruminantium is the causative agent of several bovine and ovine diseases, however reports are uncommon and application of whole genome sequencing to identify is rare. We report for the first time, a severe ovine mastitis outbreak caused by S. ruminantium in Italy, 2022. S. ruminantium was isolated from 12 adult lactating ewes with diffuse nodules in the mammary parenchyma and predominantly serous and clotted milk. All outbreak isolates, along with five additional historical Italian isolates (between 2011 and 2017), were genomically characterised and then analysed in the context of all publicly available S. ruminantium genomes. Antimicrobial susceptibility testing was performed to determine the MICs of 16 antibiotics. The results showed that all isolates were susceptible to all antimicrobials tested except kanamycin. Single Nucleotide Variant analysis confirmed this as a clonal outbreak across 10 sheep (≤ 15 SNVs), while the two others were colonised by more distantly related clones (≤ 53 pairwise SNVs), indicating the presence of multiple infecting lineages. The five historical S. ruminantium isolates were comprised of genetically-distant singletons (between 1259 and 5430 pairwise SNVs to 2022 outbreak isolates). Ovine isolates were found to be genetically distinct to bovine isolates, forming monophyletic groups. Bovine isolates were similarly made up of singleton clones in all but two isolates. Taken together, our genomic analysis using all globally available genomes is consistent with general opportunistic pathogenesis of S. ruminantium. We encourage future genomic surveillance efforts to facilitate outbreak detection, as well as improve our understanding of this poorly-understood, multi-host, zoonotic pathogen.

Background Allergic asthma is an inflammatory disease characterized by a type 2 immune response. Pollens are a common cause of seasonal asthma. Allergic rhinitis (AR) frequently associates with asthma. The treatment usually aims at controlling inflammation and relieving symptoms. Inhaled corticosteroids are the most effective controller and short-acting b2-agonists (SABA) as a reliever for asthma. Oral antihistamines and nasal corticosteroids (NC) are the mainstays for AR. A multicomponent nutraceutical containing perilla, quercetin, and vitamin D3 significantly prevented AR exacerbations in children. Thus, the current study explored the add-on use in adult patients with mild persistent asthma and AR due to grass pollen allergy. Methods The treatment lasted three months. Asthma and AR symptoms, asthma control test, spirometry, nasal eosinophils, and use of rescue medications (SABA and NC) were evaluated in the previous grass season and throughout the treatment. All patients were treated with ciclesonide (320 mcg/day) and cetirizine (10 mg/day). Unobtrusive sensing (device-free sensing) aims to embed sensing into our daily lives. This is achievable by re-purposing communication technologies already used in our environments. Wireless Fidelity (Wi-Fi) sensing, using Channel State Information (CSI) measurement data, seems to be a perfect fit for this purpose since Wi-Fi networks are already omnipresent. However, a big challenge in this regard is CSI data being sensitive to ‘domain factors’ such as the position and orientation of a subject performing an activity or gesture. Due to these factors, CSI signal disturbances vary, causing domain shifts. Shifts lead to the lack of inference generalization, i.e., the model does not always perform well on unseen data during testing. We present a domain factor-independent feature-extraction pipeline called ‘mini-batch alignment’. Mini-batch alignment steers a feature-extraction model’s training process such that it is unable to separate intermediate feature-probability density functions of input data batches seen previously from the current input data batch. By means of this steering technique, we hypothesize that mini-batch alignment (i) absolves the need for providing a domain label, (ii) reduces pipeline re-building and re-training likelihood when encountering latent domain factors, and (iii) absolves the need for extra model storage and training time. We test this hypothesis via a vast number of performance-evaluation experiments. The experiments involve both one- and two-domain-factor leave-out cross-validation, two open-source gesture-recognition datasets called SignFi and Widar3, two pre-processed input types called Doppler Frequency Spectrum (DFS) and Gramian Angular Difference Field (GADF), and several existing domain-shift mitigation techniques. We show that mini-batch alignment performs on a par with other domain-shift mitigation techniques in both position and orientation one-domain leave-out cross-validation using the Widar3 dataset and DFS as input type. When considering a memory-complexity-reduced version of the GADF as input type, mini-batch alignment shows hints of recuperating performance regarding a standard baseline model to the extent that no additional performance due to weight steering is lost in both one-domain-factor leave-out and two-orientation-domain-factor leave-out cross-validation scenarios. However, this is not enough evidence that the mini-batch alignment hypothesis is valid. We identified pitfalls leading up to the hypothesis invalidation: (i) lack of good-quality benchmark datasets, (ii) invalid probability distribution assumptions, and (iii) non-linear distribution scaling issues.

Introduction: Working with dreams in groups allows understanding of unconscious phenomena that characterize it as unity and totality. The dreamer becomes the vehicle of emotions, fantasies,and anxieties that dominate the group at a given moment, allowing them to be understood and processed (Friedman et al., 2002). Recognizing a projected theme in the group generates a change of perspective for the group, which becomes a specific psychic space (Kaës, 2001), a shared psychic field. The analysis of the shared dreams helps the understanding of the emotional concerns related to the Covid 19 and its psychological repercussions. Methods: Six matrices of social dreaming of different groups for analytical purposes were conducted during the Covid pandemic period. The sessions were transcribed and the texts were analyzed by performing a thematic analysis (Braun & Clarke, 2006) with the support of Atlas.ti software. Results: From the analysis of the results, three main themes were identified: a) “Lately I have more nightmares when I am awake than when I am asleep": Nightmares during the Covid-19 pandemic; b) “Losing one’s senses and losing the sense”: Helplessness and environmental mastery across time and space; c) Do play like a child: Recalling the child-past in the uncertain present. Discussion and conclusions: Through dreams and dream narratives we witness the evolution of group thinking through a progressive growth of the links between individuals and the common and shared field. The members of the group are thus able to identify within dreams the meanings that are useful for the entire group to process the painful experience that unites them.

Abstract Background Worldwide, veterinary practitioners and students are reported to be at higher risk of suicide, burnout, and depression compared to other occupational groups. The aim of the current study was to apply text mining and topic modelling analysis on scientific literature regarding suicide, burnout, and depression among veterinary practitioners and students to extract meaningful and synthetic information. These statistical approaches can be used to comprehend more in deep the phenomena involving veterinarians and veterinary students and to suggest the potential changes needed in admission to veterinary school, veterinary curricula, and post-graduation initiatives as preventive actions. Results A systematic search protocol was set up to identify scientific literature that published on the topic from 1985 to 2019. Two-hundred-eleven records were selected with abstracts/texts submitted to text mining and topic modelling analysis. Student, stress, work, anim*, and euthanasia resulted the most frequent terms. Topics modelling allowed to differentiate groups of words and papers in 3 areas of interest: 1) students’ difficulties encountered during their studies that increase stress and anxiety impairing their psychological health; 2) exposure to death and euthanasia as risk factor for mental health; and 3) need of support among those providing medical and health care, and of supportive group work to cope with such profession. Conclusion Based on the most frequent words included in the clouds and on the contents of the papers clusterised in them, some suggestions are interfered. It is emphasized that the veterinary curricula should include courses that prepare them early to deal with animal death and post-death grief of pet owners, to handle ethical dilemmas and moral stressors, to communicate with clients and staff members, to work in team, to balance work-family life and to promote individual and team resources. Specific courses for veterinary practitioners could keep them updated on their new roles and ways to handle them among functioning as potential feedbacks to monitor their psychological wellbeing.

The emerging field of interpersonal emotion regulation (IER) is drawing attention to forms of emotion regulation which involve communication and social interaction as part of the regulation process. The availability of instruments to measure IER in different languages represents significant promise for future work in this field. The goal of the present study was to validate an Italian adaptation of a self-report instrument for the assessment of IER, the Interpersonal Emotion Regulation Questionnaire (IERQ; Hofmann et al., 2016). In an Italian sample (N=448), exploratory and confirmatory factor analyses confirmed the original structure comprising four factors (Enhancing Positive Affect, Perspective Taking, Soothing and Social Modelling). Correlations with other measures of emotion regulation showed good convergent validity of the questionnaire.

One of the main challenges in group therapy with drug-addicted patients is collective pseudomentalization, i.e., a group discourse consisting of words and clichés that are decoupled from any inner emotional life and are poorly related to external reality. In this study, we aimed to explore the phenomenology of pseudomentalization and how it was addressed by the therapist in an outpatient group for drug-addicted patients. The group was composed of seven members, and the transcripts of eight audio-recorded sessions (one per month) were rated and studied. The interventions of the therapist were measured with the mentalization-based group therapy (MBT-G) adherence and quality scale by independent raters. Two sessions, one with the highest and one with the lowest adherence, were selected, and the clinical sequences of pseudomentalization were analyzed in a comparative way. The findings revealed that pseudomentalization does occur as a collective phenomenon, akin to “basic assumptions” of Wilfred Bion, which we reconceptualized in this study. Any pseudomentalization seemed to be reinforced by the therapist when she was presenting frequent and long interventions, when abstaining from the management of group boundaries, when providing questions focused more on content than on the mental states of the group members, and when not focusing on emotions. However, the ultimate source of collective pseudomentalization seemed to be the fear of the group members of being overwhelmed by painful emotions, mental confusion, and a loss of identity. The findings also indicated that the principles of MBT-G may be a good antidote to pseudomentalization.

In Italy, dairy sheep farming represents a vital agro-industry sector, but it is still challenged by contagious agalactia (CA), which is endemic there, and vaccination is the most economical and sustainable tool for control. This study aimed to evaluate the combined Mycoplasma agalactiae (Ma)-Staphylococcus aureus (Sa) vaccine (Ma–Sa) against the Ma monovalent vaccine in ewes. Twelve primiparous Ma-free ewes were randomly grouped into three equal groups: first, the control group injected with placebo, second, the group vaccinated with the Ma monovalent vaccine, and third, the group vaccinated with Ma–Sa combined vaccine, with two S/C doses at 45-day intervals. The animals were examined for serological, hematological, and somatic cell count (SCC) changes for 17 successive weeks. A significant increase in anti-Ma antibody mean titers, leukocytes, and platelets was observed in the vaccinated animals, with the highest values in those who received the combined vaccine. Neutrophils were high only in the animals who received the combined vaccine. SCC was lower in the vaccinated animals during the first six weeks. This study concludes that the combined Ma–Sa vaccines enhance immune response and potentiate its efficacy against Ma. This improvement might be attributed to the sensitization/activation effect of S. aureus on platelets, which are recoded to act as a key regulator for the coordination of all components of the innate immune system. Even though this study included a small number of animals, its findings about the potentialities of this inactivated vaccine in the control of CA are strongly encouraging. Further confirmation might be needed through additional replicates and a challenge study is needed before proceeding with widespread use.

On March 11, 2020, the World Health Organization (WHO) declared a state of health emergency affecting the entire world population. Given the serious practical and psychological difficulties and complications that have been experienced during this period, many scholars have created hypothesis, as a consequence, an increased possibility of developing post-traumatic stress disorder (PTSD) within the general population with significant implications for one’s dream activity. Participants in the study were recruited via the instant messaging application ‘WhatsApp’ for a period of 14 days. The study consisted of three phases: the first phase provided information on the purpose of the research and how to carry it out; in the second phase, each participant, using the Bionian model of dream experience as a focus, was asked to write down dreams, emotions and free connections/associations related to the dream. At the end of the collection, the texts obtained were analysed by means of a qualitative analysis performed with the aid of the MAXQDA software. The study confirms the computational and exploratory analysis of the text carried out in the research of Pesonen et al. (2020), finding also in our sample the presence of the hypothesized clusters going to explain the manifestation of imagery related to COVID-19 also within the dream activity. To confirm this, the nightmare of participant number 6 of the study is reported. The following qualitative research has offered an insight into the traumatic nature of the COVID-19 pandemic, showing how many unmetabolized ‘daytime elements’ have been reproposed in the dream scenario, recalling the symptomatology of PTSD through the presence of distressing content that affect the quality of sleep and the daily life of the individual.

The concept of interpersonal emotion regulation (IER) refers to a variety of processes in which emotion regulation occurs as part of live social interactions and includes, among others, also those interpersonal interactions in which individuals turn to others to be helped or to help the others in managing emotions. Although IER may be a concept of interest in group therapy, specific theoretical insights in this field appear to be missed. In this article, we firstly provide a review of IER definitions, of classifications of IER strategies, and of IER clinical conceptualizations. Afterwards, we discuss the relevance of considering IER for group therapy, both in terms of non-specific group therapeutic factors and of group therapy techniques promoting adaptive emotion regulation, underlining the potentially relevant role of IER behaviors as intrinsically involved in group experience.

Background: The COVID-19 pandemic is causing major social changes to which significant psychological effects are linked. During the first phase of the pandemic wave in Italy, whilst there was insufficient information about the phenomenon and the strategies to safeguard the population against it, many categories of people, whose professions required constant contact with the public, were affected by the contagion.Aims: The literature has shown how religiousness can support the management of stress due to diseases and health risks. In relation to this, the current study wanted to investigate how priests managed the early stages of the pandemic. This work, therefore, aimed to investigate the psychological experiences related to the contagion and the eventual death of colleagues as well as the resilience strategies activated by the priests during the process.Participants: The research involved 12 Catholic priests, all male and aged between 42 and 63 years. They came from the same pastoral community in one of the regions in Northern Italy that were most affected during the first phase of the pandemic. Those ministers had been constantly in contact with the faithful of their parishes since the breakout of the virus.Methodology: A qualitative research design was adopted, and in-depth interviews were conducted. The dialogues aimed at investigating the deep, personal and relational experiences of the priests, together with their concerns and the tools they adopted to manage anxiety. The texts obtained from the interviews were subjected to thematic analysis.Results: The areas studied concerned the experiences of the participants during the lockdown, the implications of social distancing and lack of funeral rituality and, finally, the importance of prayer as a resilience factor.Conclusions: In the current scenario dominated by the pandemic, it is significant and stimulating to understand and reflect on the functions and roles of the experiences of faith, particularly the act of elaborating the process of mourning due to COVID-19.

Many psychological factors influence the onset and prognosis of heart disease, among the most studied, there are personality traits, depressive and anxious experiences. Type D personality, characterized by high levels of social inhibition and negative affectivity, is associated with unfavourable prognosis for cardiac patients. Depressive and anxious symptoms influence the onset of cardiac disorders, the management of the disease and their impact on quality of life. Aim of this study is to evaluate the impact of cardiac rehabilitation on social inhibition, negative affectivity, anxious and depressive symptoms in cardiac patients. Patients were recruited at the Rehabilitation Cardiology Unit of Cittadella Hospital, after an acute cardiac event, they were enrolled to attend a cardiac rehabilitation program. Patients were administered the Hospital Anxiety and Depression Scale (HADS), and the Type D Scale (DS-14). Results showed significant differences between the pre- and post-test evaluation: a decrease in the Negative Affectivity subscale (DS-14) and in the Anxiety subscale and emotional distress (HADS). Other differences resulted with respect to the sociodemographic variables, particularly unmarried patients showed higher levels of anxiety at the post-test with respect to married patients, also working patients showed lower levels of depression than those who were unemployed or retired because having a job helps patients to feel less depressed because they can concentrate their resources and thoughts on other things that are not just the disease. The study demonstrated that Cardiac Rehabilitation has beneficial effects on the psychological state of patients to stimulate awareness of the disease, modification of its perception and exchange of information valuable to reorganize private and professional lives.

Social media are widely held to have played an important role in the 2017 UK general elections. But it is not altogether clear how exactly they contributed to the communication battle between Labour and the Conservatives. This article analyses the posts and comments on the official Facebook pages of the Labour Party and the Conservative Party and their respective leaders, Jeremy Corbyn and Theresa May. We look at the relationship between topics, emotions, and user engagement. Labour clearly outperformed the Tories, with Corbyn’s personal page having 10 times the interactions of May’s. We retrieve part of the reason for this success in the “positive posting” strategy adopted by Labour and the way it helped to attract user engagement. While the Conservative Party focused on negative issues such as Brexit, terrorism, and national security, Labour focused on positive issues, such as the promise of higher social spending and appeals to the grassroots, generating far higher levels of engagement. Overall, positive topic tended to fare better than more negative and controversial issues, such as security and Brexit. Our findings thus suggest the need for a more balanced understanding of the relationship between content, emotions, and user engagement on social media, moving beyond simplistic views of social media politics as necessarily biased in favor of aggressive and negative campaigning.

Cardiac rehabilitation is the sum of psychological, physical and social treatments that are offered to cardiac patients to maintain or regain an active position in society. This study wants to evaluate changes in the perception of the illness and in the self-efficacy of the management of positive and negative emotions in patients who went through cardiac rehabilitation. Sixty-seven patients (20 females, 47 males) were selected within the cardiac rehabilitation unit in the Hospital of Cittadella (Italy). Illness Perception Questionnaire - revised version and the Scale for the self-efficacy of the management of positive and negative emotions were submitted at the beginning and at the end of the rehabilitation program. One-way analyses-of-variance were performed to evaluate different answers in questionnaires between pre- and post-evaluation, and to explore gender differences. A significant change was found in the perception of duration of illness, perceived as permanent and longer after the cardiac rehabilitation program. Furthermore, at the end of the cardiac rehabilitation program men perceived the illness more chronic than women, even if they are less worried and anxious. Intensive cardiac rehabilitation has a great emotional impact on cardiac patients, influencing their perception and management of the illness. Working on emotions, through psychological groups, helps patients change their beliefs by offering them a different perspective to approach the illness.

Research on heart disease have found a strong and consistent evidence of association between some psychosocial risk factors, including depression, anxiety, self-efficacy, lack of social support and outcome of disease. Depression increases the risk of cardiac death and is highly predictive of reduced adherence to recommended treatments; anxiety appears to be linked to adverse cardiac outcomes. It was demonstrated that Cardiac Rehabilitation (CR) leads to substantial improvements and positive outcomes because combines the prescription of physical activity with the modification of risk factors and aims to reduce symptoms related to the disease and the risk of new cardiovascular events. The main objective of this study is to determine if a short and intense CR program can produce a positive impact on anxious and depressive symptoms revealed in cardiac patients, confirming results of previous researches. The protocol was proposed to all patients referred for an outpatient CR after an acute event who attended the short 2-week intensive rehabilitation program. A total of 157 patients recruited at the operating unit of Cardiology, in the Hospital of Cittadella (Italy), was included in the analysis. The Beck Depression Inventory-II and the State-Trait Anxiety Inventory-Y were administered to the patients. SPSS 17.0 was used for statistical analysis. T-tests for paired samples were used to evaluate differences between the beginning and the end of the CR program. There was a statistically significant difference between the beginning and the end of the CR program. Results for paired samples showed significant differences in all factors of the BDI-II and in the total score. In addition, a statistically significant difference was found even in the state - anxiety subscale. No significant difference was detected for the trait anxiety. According to recent studies, this research shows that the CR program has a significant impact on levels of anxiety and depression, because all activities focus their commitment on changing the patient’s personal beliefs and perception of illness, promoting the exchange of information and sharing of concerns and fears, increasing the patient’s resilience with the aim of enabling him/her to reorganize positively his/her personal, family and professional sphere.

Il contributo che proponiamo riflette su alcuni aspetti dell’esperienza di migrazione, spesso traumatica, da una prospettiva specifica: il vissuto dei bambini. L'aspetto traumatico dell'esperienza migratoria e le conseguenze che questa può portare all'individuo migrante vengono delineate secondo la prospettiva psicoanalitica nella quale il concetto di perturbante e spaesamento descrivono bene come l'esperienza migratoria possa trasformarsi in un fattore pericoloso per il sentimento d’identità e per la stabilità psichica ed emozionale della persona, che comporta, in molti casi, una regressione emozionale. I bambini, più resilienti al cambiamento, vivono in modo meno difensivo l'esperienza della mescolanza. La capacità degli adulti di accompagnare i bambini, dal punto di vista emotivo, nell'esperienza del cambiamento è determinante, giacché tale esperienza impatta su di loro in maniera ancor più complessa che sugli adulti, poiché devono elaborare il processo migratorio assieme a tutte le trasformazioni legate all’età e allo stadio evolutivo. L'esperienza del gruppo di bambini migranti, che in questo lavoro è descritta, ha consentito di individuare alcune chiavi di lettura del processo migratorio che è stato possibile comprendere attraverso la lente d’ingrandimento delle dinamiche di gruppo.

Background Allergen-specific immunotherapy (AIT) still remains the only causal treatment for IgE mediated respiratory diseases (rhinitis/asthma) In addition to the observed clinical decrease in symptoms, AIT can provide a long-lasting and preventive effect. In particular it can modify the progression from rhinitis to asthma.Methods The study was observational, open, non randomized, controlled, prospective and performed in a real-life setting. Patients with pure mite-induced allergic rhinitis were followed-up, receiving adjuvanted SLIT (aSLIT), standard SLIT (sSLIT) or drug treatment alone, according to their preference starting between 2008 and 2009. The possible onset of asthma, changes in pulmonary function and bronchial hyperreactivity (BHR) were assessed over a 5-year horizon. Also the onset of new sensitizations and symptoms-medication score (SMS) were evaluated.Results One hundred forty two patients fulfilling the inclusion criteria were assessed at baseline, and 124 had the 5-year evaluation (age range 8–57, 69 male). After 5 years of treatment, new sensitizations appeared differentially among treatments with 58.1% of new sensitizations in the drug treatment group, 13.2% in the sSLIT patients, and 8.1% in the aSLIT patients. At the end of 5 years, SMS significantly changed (P?0.001) in all groups, with a negative trend for controls, as compared to the SLIT treatments. The SMS decreased in both SLIT groups at 5 years, with no change in patients on drug treatment alone. The use of salbutamol (absent at baseline), showed an overall increase only in the group receiving drugs alone with a significant difference at 5 years (P?

This research considers the observation of certain parameters within groups formed by patients affected by multiple sclerosis. A music/art therapist and a psychologist/psychotherapist at a neuro-rehabilitation structure in Venice led the groups. Our goal is to assess how the perception of the history of disease can change in patients by using myth and expressive-narrative workshops as a means of trans-formation. The following instruments were administered at the beginning, during, and at the end of the treatment: SCL–90–R, TAS–20, FAT.A.S.–G, and CGQ. Re-sults show a change in patients? perceptions of themselves, both with respect to their disease and to other people. Therefore, myths and their narration appear to be a possible resource that can promote mental transformation processes in an institu-tional neuro-rehabilitative context.

In all group therapeutic processes, there are interacting and interdependent mechanisms and changing conditions: the therapeutic factors (Corsini & Rosenberg, 1955; Yalom, 1995). These factors are intrinsic to the therapeutic process and unrelated to the type of group (Rorhbaugh & Bartels, 1975). The present study examines the factor structure of the questionnaire Factors Aspecific and Specific in the Group Therapy (FAT.A.S.-G.; Marogna, 2009), designed to investigate specific and non-specific therapeutic factors. The questionnaire was administered to 167 patients involved in a short-term psychotherapy group. The factor analysis identified two main dimensions: interdependence and the group as Object-Self. The Cronbach Alpha coefficients range from .88 to .93, showing high internal consistency between items.

This paper evaluates process and outcome in a 2 years supportive psychotherapy psychodynamic-oriented of a young adult self-referred for concerns about University choice. The diagnosis was Panic Attack Disorder (DSM-IV) with features of obsessive-compulsive and narcissistic personality disorder (PDM). Twenty-eight verbatim transcripts of the sessions were coded with the Psychodynamic Intervention Rating Scales, the Collaborative Interactions Scale and Defense Mechanism Rating Scale. A log linear analysis model showed the trends of process variables during the treatment. A hierarchical regression analysis evaluated the importance of tailoring the clinician interventions in respect to the average level of defenses. Outcome results showed how patient's diagnosis changed and symptoms decreased.

Il moltiplicarsi di proposte di intervento psicologico e la crescente richiesta da parte degli utenti e delle istituzioni di cura di misurare efficacy ed effectiveness delle psicoterapie, rende necessaria una valutazione empirica dei modelli di intervento applicati, attraverso lo studio di ciò che avviene in psicoterapia e dei relativi meccanismi di cambiamento e azione. Scopo del presente lavoro è introdurre il lettore ai diversi disegni di ricerca per la valutazione empirica della psicoterapia nel complesso contesto di alcuni servizi Universitari dell’Ateneo Patavino. L'interesse sarà posto sui diversi progetti e strumenti per la valutazione dell’esito, dei macro e dei micro processi di cambiamento di terapie ad indirizzo psicodinamico, caso singolo e di gruppo, anche in relazione alle neuroscienze.

Neutrophil extracellular traps (NETs) are structures composed of DNA, histones, and antimicrobial proteins that are released extracellularly by neutrophils and other immune cells as a means for trapping and killing invading pathogens. Here, we describe NET formation in milk and in mammary alveoli of mastitic sheep, and provide a dataset of proteins found in association to these structures. Nucleic acid staining, immunomicroscopy and fluorescent in-situ hybridization of mastitic mammary tissue from sheep infected with Streptococcus uberis demonstrated the presence of extranuclear DNA colocalizing with antimicrobial proteins, histones, and bacteria. Then, proteomic analysis by LTQ-Orbitrap Velos mass spectrometry provided detailed information on protein abundance changes occurring in milk upon infection. As a result, 1095 unique proteins were identified, of which 287 being significantly more abundant in mastitic milk. Upon protein ontology classification, the most represented localization classes for upregulated proteins were the cytoplasmic granule, the nucleus, and the mitochondrion, while function classes were mostly related to immune defence and inflammation pathways. All known NET markers were massively increased, including histones, granule proteases, and antimicrobial proteins. Of note was the detection of protein arginine deiminases (PAD3 and PAD4). These enzymes are responsible for citrullination, the post-translational modification that is known to trigger NET formation by inducing chromatin decondensation and extracellular release of NETs. As a further observation, citrullinated residues were detected by tandem mass spectrometry in histones of samples from mastitic animals. In conclusion, this work provides novel microscopic and proteomic information on NETs formed in vivo in the mammary gland, and reports the most complete database of proteins increased in milk upon bacterial mastitis. Summary: Background: We evaluated whether obstructive sleep apnoea (OSA) and continuous positive airway pressure (CPAP) treatment influence left ventricular (LV) remodelling independently of abdominal obesity and metabolic syndrome (MetS). Methods: Cardiorespiratory examination, 24-h BP monitoring and echocardiogram were performed in overweight/obese patients with increased abdominal adiposity and symptoms suggesting OSA : OSA/MetS (n.50), OSA/noMetS (n.22), noOSA/MetS (n.29), noOSA/noMets (n.16). The evaluation was repeated in 41 patients after ≥18 months of CPAP. Results: Despite similar age, gender, BMI and 24-h BP, the 2 groups with MetS had greater LV remodelling (LV hypertrophy and diastolic dysfunction) than the 2 groups without MetS. From multiple regression analysis independent determinants for LV mass were MetS, 24-h systolic BP and age, for LV diastolic function were LV mass index, MetS and age. After CPAP, the 20 patients with decreased body weight showed diastolic BP decrease, LV hypertrophy regression and diastolic function improvement, whereas, despite similar respiratory improvement, BP and LV parameters were unchanged in the 21 patients with body weight unchanged/increased. Conclusion: In patients with increased abdominal adiposity, LV remodelling is not associated to OSA per se; chronic CPAP treatment does not influence LV remodelling whose regression is mainly linked to body weight decrease. Background: Data on the long-term effects of sublingual immunotherapy (SLIT) are sparse, and the optimal duration of treatment is a matter of debate. Objective: We sought to prospectively evaluate the long-term effect of SLIT given for 3, 4, or 5 years and to compare the effect of those different durations. Methods: In this prospective open controlled study we followed up patients with respiratory allergy who were monosensitized to mites for 15 years. The subjects were divided in 4 groups receiving drug therapy alone or SLIT for 3, 4, or 5 years. Clinical scores, skin sensitizations, methacholine reactivity, and nasal eosinophil counts were evaluated every year during the winter months. The clinical effect was considered to persist until clinical scores remained at less than 50% of the baseline value, and then patients underwent another course of SLIT. Results: Seventy-eight patients were enrolled, and 59 completed the study. In the 12 control subjects no relevant change in clinical scores was seen throughout the study. In the patients receiving SLIT for 3 years, the clinical benefit persisted for 7 years. In those receiving immunotherapy for 4 or 5 years, the clinical benefit persisted for 8 years. New sensitizations occurred in all the control subjects over 15 years and in less than a quarter of the patients receiving SLIT (21%, 12%, and 11%, respectively). The second course of vaccination induced a benefit more rapidly than the first course. The behavior of bronchial hyperreactivity and nasal eosinophils paralleled the clinical score. Conclusion: Under the present conditions, it can be suggested that a 4-year duration of SLIT is the optimal choice because it induces a long-lasting clinical improvement similar to that seen with a 5-year course and greater than that of a 3-year vaccination. Background: Sublingual immunotherapy (SLIT) has been proved effective in allergic rhinitis, but there are few studies assessing its effects on inflammation and on the lower airways. Objective: We sought to evaluate at the same time the effects of SLIT on rhinitis symptoms, nasal inflammation, and lower airways function in patients with birch pollinosis. Methods: Adult patients with rhinitis and asthma monosensitized to birch were evaluated during a run-in pollen season and then randomized to receive openly either drugs alone or drugs plus SLIT and reevaluated in the subsequent 4 pollen seasons. Rhinitis symptoms and consumption of bronchodilators were assessed by means of diary card. A nasal smear for eosinophil count was carried out in and out of pollen seasons, and pulmonary function tests with methacholine challenge were performed at each season. Results: Of 79 enrolled patients, 27 dropped out, with a significantly higher rate of dropouts in the control group. There was a decrease in symptoms and bronchodilator use in the SLIT group versus the control group, becoming significant at the second and third pollen seasons, respectively (P < .01 at all times). Nasal eosinophils decreased significantly in the active group, starting from the third pollen season (P < .01). In the SLIT group a significant increase in FEV1, specific airways conductance, and maximal expiratory flow at 25% of forced vital capacity was seen starting from the second year and was associated with an increase in the methacholine threshold dose (P < .01). The differences were significant also at the intragroup comparison over time. Conclusion: SLIT achieved a significant clinical benefit in birch pollinosis, reduced the eosinophil infiltration in nasal mucosa, and significantly improved pulmonary function during the pollen seasons.
