Introduction The two minute sit-to-stand test (STST) quantifies the getting up and sitting down movement in 2 minutes. Its use has not yet been well established in patients with heart disease. Purpose To develop and validate an equation to estimate peak oxygen uptake (peak VO2) based on performance in STST. Methods Cross-sectional study with patients over 18 years with heart disease referred to a cardiovascular rehabilitation program, who were submitted to STST and Cardiopulmonary Exercise Test. Pearson's correlation analysis, multiple linear regression and ROC curve were used for statistical analysis. The association between determined and estimated peak VO2 was evaluated by the Bland-Altman analysis. p Results 158 participants were included, with a mean age of 60±14 years, 75.3% male, 75.6% with coronary artery disease and 24.4% with chronic heart failure. The average left ventricular ejection fraction was 57±16 %. The tests resulted in mean peak VO2 of 19.2±6.4 mL.kg-1.min-1, and mean STST performance was 32±12 repetitions. There was a correlation between peak VO2 and STST (r=0.65; R2=0.57; p0.05) from the peak VO2 determined (19.71±6.8 mL-kg-1min-1). Conclusions STST was moderately correlated with peak VO2. The validated equation accurately estimated the peak VO2. The STST is useful in the assessment of patients with heart disease, helping to estimate peak VO2 and identify patients with peak peak VO2peak ≥ 20mL-kg-1min-1.

Patient education is an integral part of recovery from a critical cardiac life event and a core component of cardiac rehabilitation (CR) programmes. This study addressed the feasibility of a virtual educational programme for behaviour change in CR patients from a low-resource setting in Brazil. Cardiac patients from a CR programme closed due to the pandemic received a 12-week virtual educational intervention (WhatsApp messages and bi-weekly calls from healthcare providers). Acceptability, demand, implementation, practicality, and limited efficacy were tested. Overall, 34 patients and 8 healthcare providers agreed to participate. The intervention was considered practical and acceptable by the participants, who reported a satisfaction median of 9.0 (7.4–10.0)/10 (patients) and 9.8 (9.6–10.0)/10 (providers). The main difficulties in carrying out the intervention activities were related to technology, motivation to self-learning, and a lack of in-person orientation. All the patients reported that the information included in the intervention was aligned with their information needs. The intervention was associated with changes in exercise self-efficacy, sleep quality, depressive symptoms, and performance of high-intensity physical activity. In conclusion, the intervention was considered feasible to educate cardiac patients from a low-resource setting. It should be replicated and expanded to support patients that face barriers to onsite CR participation. Challenges related to technology and self-learning should be addressed.

The purpose of this study was to verify the accuracy of the agreement between heart rate at the first ventilatory threshold (HRVT1) and heart rate at the end of the 6-min walk test (HR6MWT) in coronary artery disease (CAD) patients on β-blockers treatment. This was a cross-sectional study with stable CAD patients, which performed a cardiopulmonary exercise test (CPET) on a treadmill and a 6-min walk test (6MWT) on nonconsecutive days. The accuracy of agreement between HRVT1 and HR6MWT was evaluated by Bland–Altman analysis and Lin’s concordance correlation coefficient (rc), mean absolute percentage error (MAPE), and standard error of estimate (SEE). Seventeen stable CAD patients on β-blockers treatment (male, 64.7%; age, 61± 10 years) were included in data analysis. The Bland–Altman analysis revealed a negative bias of -0.41 ± 6.4 bpm (95% limits of agreements, -13 to 12.2 bpm) between HRVT1 and HR6MWT. There was acceptable agreement between HRVT1 and HR6MWT (rc = 0.84; 95% confidence interval, 0.63 to 0.93; study power analysis= 0.79). The MAPE of the HR6MWT was 5.1% and SEE was 6.6 bpm. The ratio HRVT1/HRpeak and HR6MWT/HRpeak from CPET were not significantly different (81%± 5% vs. 81%± 6%, P= 0.85); respectively. There was a high correlation between HRVT1 and HR6MWT (r= 0.85, P< 0.0001). Finally, the results of the present study demonstrate that there was an acceptable agreement between HRVT1 and HR6MWT in CAD patients on β-blockers treatment and suggest that HR6MWT may be useful to prescribe and control aerobic exercise intensity in cardiac rehabilitation programs.

Introduction. Expiratory flow limitation (EFL) during moderate intensity exercise is present in patients with myocardial infarction (MI), whereas in healthy subjects it occurs only at a high intensity. However, it is unclear whether this limitation already manifests in those with stable coronary artery disease (CAD) (without MI). Materials and Methods. Forty-one men aged 40–65 years were allocated into (1) recent MI (RMI) group (n = 8), (2) late MI (LMI) group (n = 12), (3) stable CAD group (n = 9), and (4) healthy control group (CG) (n = 12). All participants underwent two cardiopulmonary exercise tests at a constant workload (moderate and high intensity), and EFL was evaluated at the end of each exercise workload. Results. During moderate intensity exercise, the RMI and LMI groups presented with a significantly higher number of participants with EFL compared to the CG (p<0.05), while no significant difference was observed among groups at high intensity exercise (p>0.05). Moreover, EFL was only present in MI groups during moderate intensity exercise, whereas at high intensity all groups presented EFL. Regarding the degree of EFL, the RMI and LMI groups showed significantly higher values at moderate intensity exercise in relation to the CG. At high intensity exercise, significantly higher values for the degree of EFL were observed only in the LMI group. Conclusion. The ventilatory limitation at moderate intensity exercise may be linked to the pulmonary consequences of the MI, even subjects with preserved cardiac and pulmonary function at rest, and not to CAD per se.

Background: Exercise oscillatory ventilation (EOV) shows a four-fold greater risk of adverse events. This study aims to analyze the sensitivity and specificity of three EOV diagnostic definitions to predict adverse outcomes at a 2-year follow-up and to compare its EOV prevalence and relations with the patient's profile.Methods: Cardiopulmonary exercise tests from 233 heart failure patients were analyzed. Two blinded reviewers used a semiautomated software to identify EOV cases pattern according to the definitions of Ben-Dov, Corr`a, and Leite. Data were grouped in EOV-positive or EOV-negative according to each definition. Baseline characteristics, EOV prevalence, relative risk, sensitivity, and specificity to predict 2-years of major adverse cardiovascular outcomes were analyzed.Results: The Corra definition led to the best prediction of 2-year major cardiovascular adverse outcomes (HR 2.46 [1.16 to 5.25]; p = 0.019, AUC = 0.618; p = 0.007). EOV prevalence was 17.2%, 17.2%, and 9.4% applying BenDov, Corr`a, and Leite definition, respectively. The main clinical differences between EOV-positive and EOVnegative patients were: MECKI score and VE/VCO2 slope (all definitions), and BNP levels (Ben-Dov and Leite). BNP levels were correlated with amplitude (rho = 0.255; p = 0.033) and cycle length (rho = 0.388; p = 0.002).Conclusion: Corra definition was the only one that exhibited the capacity to predict major adverse cardiovascular outcomes at a 2-year follow-up. Regardless of its definition, EOV was more often prevalent in patients with a greater MECKI score and VE/VCO2 slope values.

INTRODUCTION: Different heel-rise test (HRT) protocols have been used, possibly leading to varied responses. It is necessary to analyse the impact of protocol variation on test responses. PURPOSE: To compare the performance, muscle oxygenation (MO), and heart rate (HR) responses of adults in bilateral HRT protocols. METHODS: This was a cross-sectional crossover study.Thirty participants (23.1±2.9 years; 16 men) performed four bilateral HRT protocols with varying cadence (self-cadenced; externally cadenced) and ankle position (neutral; dorsiflexion). For MO responses, we analysed tissue oxygen saturation (StO2) and oxyhemoglobin concentration variation (∆[O2Hb]) and calculated the variation between the smallest and final values (∆Nadir-Final) and the area under the curve (AUC). The variation between the initial and final HR values (∆HR) and the time constant (τ) were calculated. Friedman's test was used to compare the variables among the protocols. Frailty is highly prevalent in heart failure (HF) patients. HF is associated with oxidative stress and chronic inflammation, which impair oxygen use by skeletal muscles. Little is known about the influence of frailty on vascular responsiveness and tissue oxygenation.Analyze the influence of frailty on vascular responsiveness and muscle oxygenation in elderly individuals with and without HF.Individuals aged ≥60 years, with or without HF, were evaluated for frailty (phenotype). Near-infrared spectroscopy (NIRS) was used to assess muscle oxygenation at rest (oxygen saturation - StOResults were grouped according to the frailty phenotype: non-frail, pre-frail, and frail. Shapiro-Wilk test was used to assess normality. Data were compared using a two-way analysis of variance (ANOVA). Bonferroni post hoc test was applied to determine the influence of frailty or HF on NIRS variables. Purpose The aim of this study was to analyze the effects of inspiratory muscle training (IMT) as a therapeutic strategy after heart valve replacement surgery (HVRS). Methods A double-blind, randomized, clinical trial that included patients undergoing elective HVRS, without post-operative complications, were allocated to 2 groups: IMT group (IMT-G) and IMT placebo group (IMT-PG). The IMT started 3 d after surgery and was performed twice daily for 4 wk. Lung function, maximum inspiratory pressure (MIP) as a measure of inspiratory muscle strength, functional capacity, and quality of life were assessed pre-operatively and at the end of training. Results The IMT-G recovered pre-operative MIP and lung function values after 4 wk of training. This group also increased the distance walked during the 6-min walk test (6MWD). In the IMT-PG, the values of MIP were below those found pre-operatively, with impairment of lung function and lower 6MWD in the final evaluation. At the end of IMT, MIP was correlated with the 6MWD and with the spirometry variables. Conclusions IMT performed for 4 wk after HVRS was effective in restoring the values of inspiratory muscle strength and lung function to the pre-operative level and increasing the functional capacity assessed by the 6MWD. Furthermore, an association between lung function and functional capacity was observed, demonstrating the clinical relevance of the use of IMT in the rehabilitation process of these patients.

COVID-19 has impacted how people receive health care for many conditions, including cardiovascular diseases.To examine perceptions of cardiac rehabilitation (CR) participants regarding their health behaviors and information needs during the COVID-19 pandemic in Brazil.In this cross-sectional study, a 27-item questionnaire, developed by the investigators, was administered online to participants from two CR programs. Questions included health literacy (HL; using the Brief Health Literacy Screening Tool), technology use, perceptions before and during the COVID-19 pandemic, and information needs. Pearson correlation coefficients, paired t-tests, and ANOVA were used as appropriate. P0.05 was considered statistically significant for all tests.Overall, 159 (25.5%) CR participants answered the questionnaire. Of these, 89.9% had limited or marginal HL and 96.2% reported having internet access at home. Patients are mainly concerned about their family's health and their own, as well as how the coronavirus is dangerous to their health and how it has changed their lifestyle. Participants perceived that the quality of their health behaviors significantly decreased during the pandemic. The pandemic also changed information needs of CR participants as new needs emerged, such as the control of anxiety levels, staying motivated to live healthily during a pandemic, and how COVID-19 can impact their health condition. Participants with adequate HL significantly perceived the severity of the disease and having access to information significantly more than those with limited HL.Our results highlighted the impact of the pandemic on CR participants' perceptions regarding their health behaviors and information needs, which can be influenced by HL levels.A COVID-19 afetou como as pessoas recebem atendimento de saúde para várias doenças, inclusive doenças cardiovasculares.Examinar as percepções dos participantes de reabilitação cardíaca (RC) sobre seus comportamentos em saúde e necessidades de informação durante a pandemia da COVID-19 no Brasil.Neste estudo transversal, um questionário de 27 itens elaborado pelos investigadores foi administrado online a participantes de dois programas de RC. As perguntas incluíam letramento em saúde (LS; usando a Brief Health Literacy Screening Tool - Breve ferramenta de triagem de letramento em saúde), uso de tecnologia, percepções antes e durante a pandemia da COVID-19, e necessidades de informações. Foram usados coeficiente de correlação de Pearson, testes t pareados e ANOVA, conforme apropriado. Um p0,05 foi considerado estatisticamente significativo para todos os testes.No total, 159 (25,5%) participantes de RC responderam ao questionário. Desses, 89,9% tinham LS limitado ou marginal, e 96,2% relataram ter acesso à internet de casa. Os pacientes se preocupam principalmente com a saúde de sua família e própria, além de como o coronavírus é perigoso para sua saúde e como mudou seu estilo de vida. Os participantes perceberam que a qualidade de seus comportamentos em saúde diminuiu significativamente durante a pandemia. A pandemia também mudou as necessidades de informações dos participantes de RC, já que novas necessidades surgiram, tais como, controle de níveis de ansiedade, manter a motivação para levar uma vida saudável durante a pandemia, e como a COVID-19 pode afetar sua condição de saúde. Participantes com LS adequado perceberam significativamente a gravidade da doença e tinham significativamente mais acesso a informações do que os pacientes com LS limitado.Nossos resultados destacaram o impacto da pandemia nas percepções dos participantes de RC em relação a seus comportamentos em saúde e necessidades de informação, que podem ser influenciados pelos níveis de LS.

Background Increased level of physical activity (PA) and health education are known as non-pharmacological treatments of hypertension (HP). There is a lack of studies investigating the influence of HP knowledge on the level of PA among hypertensive patients. Objective To examine the influence of patient’s knowledge about HP on PA level and the relationship between these variables. Methods A cross-sectional study was conducted in in a primary care center located in a city in the southern Brazil. A total of 199 hypertensive patients (median 61.2 [13] years; body mass index (BMI) 21.9 (7.5) kg/m2; 72.4% women) were included. It is believed that existing circulatory disturbance increase oxidative stress and chronic inflammation, predisposing to anabolic-catabolic imbalance. Thus, there is impairment of the efficient use of oxygen by skeletal muscles, limiting the physical-functional performance in these individuals. However, little is known about the influence of frailty on endothelial function in the elderly. Purpose To analyze the influence of frailty on endothelial function in the elderly with and without HF. Methods This was a descriptive cross-sectional study, which included individuals aged ≥60 years, with or without HF, who did not have diabetes, anemia, peripheral obstructive arterial disease and/or congenital heart disease. The Cardiovascular Health Study (CHS) frailty scale criteria were used to assess frailty (phenotype). Endothelial function at rest was evaluated by near-infrared spectroscopy ([NIRS]; slope 1, lowest tissue oxygen saturation [StO2], area under the curve [AUC] of StO2, slope 2, StO2 peak, overshoot, ΔStO2nadir_peak and Δtime nadir_peak) during arterial occlusion maneuver on the forearm. Results were grouped according to the frailty phenotype: robust, pre-frail and frail. Shapiro-Wilk test was used to assess the normality of data. Quantitative data were compared using a two-way analysis of variance plus Bonferroni post hoc test to determine the influence of the frailty or HF on endothelial function variables. Background Cardiopulmonary exercise test (CPET) is the reference method for assessing gas exchange threshold (GET), a surrogate of the anaerobic threshold (AnT). However, muscle oxygenation has been shown as an additional tool for AnT determination, using the lower limb threshold (LLT). Besides, Talk Test (TT), a tool used to assess, prescribe, and monitor exercise intensity through speech comfort level, has shown a correlation with GET as well. Purpose To identify the AnT obtained from the traditional method from CPET (GET) and the LLT; to compare heart rate (HR) and tissue saturation index (TSI) data at AnT (GET and LLT); to compare the parameters of LLT on CPET and TT, and to correlate LLT and TT stages in cardiovascular disease (CVD) patients. Methods CVD patients underwent CPET and TT, both on a treadmill, in two distinctive days. During those tests, peripheral muscle oxygenation was assessed by the near-infrared spectroscopy method. GET (CPET) was determined by the V-slope method; LLT (CPET and TT) was determined by the visual inspection of oxyhemoglobin and deoxyhemoglobin curves. A TT protocol based on the prediction equation for the covered distance of the six-minute walk test (6MWD) was applied. TT protocol was incremental, with two-minutes stages starting at 70% of the average velocity predicted and 2% of inclination, increasing velocity in 10 percentage points (p.p.) at each stage, until the fifth stage, at which the inclination was increased in 2 p.p. until test completion. At the end of each stage, patients read a standardized paragraph and were asked about speech comfort. Shapiro-Wilk test was performed to assess data distribution. Wilcoxon test was applied to compare physiological variables (HR and TSI) at GET and LLT. Repeated measures analysis of variance (ANOVA), followed by Bonferroni post-hoc test was assessed to compare the variables at LLT and TT stages (last TT+ and first TT±). Spearman correlation coefficient was used to assess the relationship between variables. Statistical significance was set at 5%. Results 24 cardiovascular patients were included. Similar values of HR (GET vs. LLT: 102 ± 15 bpm vs. 99 ± 14 bpm, p = 0,08), and TSI (GET vs. LLT: 64,6 ± 8,2% vs. 66,4 ± 7,8%, p = 0,09) were observed during CPET. Regarding LLT at CPET and TT, there was no difference between HR (CPET vs. TT: 99 ± 14 bpm vs. 100 ± 12 bpm, p = 1,00) and TSI (CPET vs. TT: 66,4 ± 7,8% vs. 65,3 ± 4,9%, p = 1,00). Furthermore, good relationship between HR in LLT and TT stages (LLT vs. TT+: r = 0,79; LLT vs. TT±: r = 0,76; p Conclusions Our results showed similarity between LLT and GET. Besides, TT showed a similar response of muscle oxygenation and heart rate comparing to CPET, with a good correlation between LLT and TT stages. These results endorse the validity of TT as an auxiliary and low-cost tool to identify the AnT.

Funding Acknowledgements Type of funding sources: None. Background Talk test (TT) is an alternative and accessible tool for prescribing and monitoring aerobic training intensity. Although the TT is reliable and valid for cardiorespiratory assessment, its responsiveness to exercise training remains unexplored. Purpose To evaluate the responsiveness of TT in cardiovascular disease (CVD) patients who underwent an exercise training program. Methods Twenty-one CVD patients (61.7 ± 8.4 years) performed an exercise-training program on phase II of cardiac rehabilitation (45-min 3-times a week). The six-minute walk test (6MWT) and TT were done to assess functional capacity at baseline and after 8 weeks. In the individualized TT the treadmill’s speed and/or grade were increased every 2-min, with speed changes based on a reference equation for the 6MWT distance (6MWD). The subjects were asked to read a 38 words standard paragraph at the last 30s of each stage and to answer if they could talk comfortably. Answer options were i) YES (TT+), ii) UNCERTAIN (TT±), or iii) NO (TT-). The first ventilatory threshold (VT1) was identified by two reviewers using the heart rate variability analysis. A paired t-test was applied to analyze the TT duration and 6MWD. The VT1 and TT workload were analyzed by the Wilcoxon test. Spearman correlation was adopted to compare the TT± and VT1 stages. Results Improvement in the VT1 (2.9 ± 1.2 vs 4.4 ± 1.4 min; p Conclusion Talk test performed on a treadmill showed responsiveness after eight weeks of exercise training, being sensitive to the physiological changes provided by the rehabilitation program in CVD patients.

This quasi-experimental study investigated the preliminary effects of a structured education intervention in a pooled sample of cardiovascular rehabilitation (CR) patients in Brazil. Recently enrolled (RE) and long-term enrolled (LTE) patients attended 12 weekly education sessions in addition to three weekly exercise sessions. Patients completed surveys assessing disease-related knowledge, physical activity, food intake, self-efficacy, and health literacy. Functional capacity was assessed by the 6-minutes walking test. All outcomes were assessed at pre-,post-CR, and 6-months follow-up. Bonferroni correction was applied. In total, 69 (69.7%) patients completed all three assessments. There were significant improvements in knowledge pre-to post-test in both subgroups (p < 0.001), and in functional capacity (p ≤ 0.001) and food intake (p ≤ 0.001) pre-to post-test in the RE subgroup. Post-test knowledge was correlated to physical activity, functional capacity and health literacy. This preliminary study suggests the importance of structured education for CR patients. A larger study using a randomized controlled design is needed to determine efficacy.

Background Oscillation between successive sinus beats or RR intervals, termed heart rate variability, is an important marker of autonomic function of the heart. However, its analysis may be influenced by the database recorded based on the occurrence of interference. Objective To evaluate if the techniques of identification and editing of artifacts, as well as the selection methods of RR intervals, can interfere with heart rate variability analysis. Methods The RR intervals of 56 subjects (30 aortic stenosis patients, 14 physically active individuals, 12 amateur athletes) were recorded for 10 min using a heart rate monitor. Values with differences greater than 20%, higher than three standard deviations or outside of the normal curve (95% confidence interval) were considered artifacts. These points were corrected through data replacement, adjacent, linear and polynomial interpolation, or excluded. Then, the 256 highest stability points and the last 5 min of recordings were chosen. The software programs, Kubios HRV and GraphPAD, were used to calculate and to analyze the indices of heart rate variability, respectively. Results Strong agreement was observed among the identification algorithms; there was no difference between the correction techniques (p = 0.95); and the selection methods exhibited different sections (p Conclusion With short-term recordings, selection methods may interfere with the non-linear heart rate variability analysis. The confidence interval, the replacement by the average of previous data and the selection of 256 of the highest stability points of the signal seem to be the most adequate procedures to treat the data with prior to analysis.

To verify and compare the responses of the cardiopulmonary variables to the incremental test in physically inactive people living with human immunodeficiency virus (HIV)/acquired immune deficiency syndrome (AIDS) (PLWHA) with well-controlled disease and physically inactive healthy subjects (non-HIV/AIDS). Participants performed a cardiopulmonary exercise test (CPX) on a treadmill. Data were analyzed using the Mann-Whitney test and Spearman correlation. Nine PLWHA (5 women) and 9 non-HIV/AIDS gender and activity level-matched controls were included in the data analysis. Data are expressed in median (range). No difference was shown in the PLWHA group when compared to the control group in functional capacity (peak oxygen consumption [VO2peak]: 29.9 (20.9-36.4) mL/kg/min vs. 32.2 (24.5-39.4) mL/kg/min) and ventilatory efficiency (oxygen uptake efficiency slope [OUES]: 2,058 [1,474-3,204] vs. 2,612 [1,383-4,119]; minute ventilation carbon dioxide production slope: 27.4 [22.5-33.6] vs. 27.5 [20.4-38.1]). The results are also similar to maximal heart rate, oxygen pulse, gas exchange threshold, respiratory compensation point, heart rate recovery, and half-time of VO2peak recovery. OUES had a strong correlation with VO2peak in the PLWHA group (r s =0.70, P=0.04) and control group (r s =0.78, P=0.02). The results of this study indicate that functional capacity and ventilatory efficiency in PLWHA with well-controlled disease are preserved and are not different from sedentary subjects. Exercise training has been shown to be an effective strategy to balance oxidative stress status; however, this is underexplored in people living with HIV/AIDS (PLWHA).To evaluate the effects of exercise training on oxidative stress in PLWHA receiving antiretroviral therapy.Patients performed 24 sessions (3 times per week, 8 weeks) of either aerobic (AT), resistance (RT), or concurrent training (CT). Glutathione disulphide to glutathione ratio (GSSG/GSH) in circulating erythrocytes and thiobarbituric acid-reactive substances (TBARS) in plasma samples were assessed as oxidative stress markers. INTRODUCTION: Brazil is a country with great climatic, socioeconomic, and cultural differences that does not yet have a reference value for the 6-min walk test (6MWT) in healthy children. To avoid misinterpretation, the use of equations to predict the maximum walk distance should be established in each country. OBJECTIVES: We sought to establish reference values and to develop an equation to predict the 6-min walk distance for healthy children in Brazil. METHODS: This is a cross-sectional multi-center study that included 1,496 healthy children, aged 7 to 12 y, assessed across 11 research sites in all regions of Brazil, and recruited from public and private schools in their respective regions. Each child was assessed for weight and height. Walk distance was our main outcome. An open-source software environment for statistical computing was used for statistical analysis. RESULTS: We observed a higher average distance walked by boys (531.1 m) than by girls (506.2 m), with a difference of 24.9 m ( P CONCLUSION: Reference values were established for the 6MWT in healthy children aged 7–12 y in Brazil.

To the best of our knowledge, no studies have evaluated oxygen uptake, carbon dioxide production and exercise tolerance in rats that have undergone myocardial infarction classified by myocardial infarct (MI) size. What is the main finding and its importance? Oxygen uptake and exercise intolerance are MI size dependent, and classification based on MI size can distinguish rats with functional capacity impairment. Rats with a large MI (>40% of the left ventricle) might provide a good model for the testing of new therapies that have the potential to modify the variables of functional capacity. Oxygen uptake (VO2) and exercise tolerance in rats classified by myocardial infarct (MI) size are underexplored. Background Aortic stenosis is a valvular heart disease characterised by fixed obstruction of the left ventricular outflow. It can be managed by surgical aortic valve replacement (sAVR) or transcatheter aortic valve implantation (TAVI). This review aimed to describe the evidence supporting a cardiac rehabilitation programme on functional capacity and quality of life in aortic stenosis patients after sAVR or TAVI. Methods The search was conducted on multiple databases from January to March 2016. All studies were eligible that evaluated the effects of a post-interventional cardiac rehabilitation programme in aortic stenosis patients. The methodological quality was assessed using the PEDro scale. Meta-analysis was performed separately by procedure and between procedures. The walked distance during the six-minute walk test (6MWD) and Barthel index were evaluated. The analysis was conducted in Review Manager. The Journal of Bodywork & Movement Therapies recently published an article by Silva et al., entitled “Inspiratory muscle training improves performance of a repeated sprints ability test in professional soccer players” (Silva et al., 2019). After close reading we find that the new findings of Silva et al., (2019) are relevant and provide a promising indication that IMT can improve the performance of young male soccer players. However, some additional important points must be considered when interpreting these positive findings.

Exercise tolerance and maximal oxygen uptake (VO2max) are reduced in heart failure (HF). The influence of combined resistance training (RT) and low-level laser therapy (LLLT) on exercise tolerance and VO2max in HF has not yet been explored. The aim of this study was to evaluate the influence of combined RT and LLLT on VO2max and exercise tolerance in rats with HF induced by myocardial infarction (MI). Rats were allocated to sedentary sham (Sed-Sham, n = 12), sedentary heart failure (Sed-HF, n = 9), RT heart failure (RT-HF, n = 7) and RT associated with LLLT heart failure (RT + LLLT-HF, n = 7) groups. After MI or sham surgery, rats underwent a RT and LLLT protocol (applied immediately after RT) for 8 weeks. VO2max and exercise tolerance were evaluated at the end of protocol. HF rats subjected to LLLT combined with RT showed higher VO2basal (41 %), VO2max (40 %), VO2reserve (39 %), run distance (46 %), time to exhaustion (30 %) and maximal velocity (22 %) compared with HF rats that underwent RT alone. LLLT associated with RT improved oxygen uptake and exercise tolerance compared with RT alone in HF rats.

O presente documento apresenta orientações para o atendimento de Fisioterapia a pacientes com suspeita ou diagnostico de COVID-19, com foco na indicação e uso da ventilação não-invasiva(VNI) e da cânula nasal de alto fluxo (CNAF), bem como sobre manejo da ventilação mecânica invasiva. Algumas intervenções vêm sendo descritas com intuito de melhorar desfechos relacionados à insuficiência respiratória da COVID-19, como a VNI e a CNAF. Com base em estudos científicos atuais, informações advindas da experiência no tratamento da COVID-19, maior risco de disseminação do vírus, aliados à indisponibilidade de alguns recursos na maioria das unidades de terapia intensiva brasileiras, recomenda-se que a VNI e a CNAF não sejam estratégias ventilatórias de primeira linha. Entretanto, em situações específicas, definidas pela equipe multiprofissional, pode ser feito um teste de resposta à VNI ou CNAF, desde que respeitadas condições de biossegurança e resposta do paciente, visando não postergar a intubação orotraqueal.

Este documento fornece orientação sobre o papel do fisioterapeuta em diferentes cenários para lidar com a COVID-19. Desde dezembro de 2019, com o surgimento do primeiro caso de COVID-19 e a expansão de casos em todo o mundo, muitos esforços foram direcionados para conter a pandemia. O fisioterapeuta, como um membro importante da equipe de saúde, desempenha um papel importante em diferentes cenários de desempenho, do primário ao terciário. Este documento apresenta uma tabela com os cenários e ações a serem realizados no nível primário, antes de uma possível contaminação, bem como as estratégias de tratamento no nível hospitalar, antes e após a intubação orotraqueal.

Neste documento apresentamos uma atualização do documento pioneiro da ASSOBRAFIR produzido no final do mês de janeiro de 2020 quando da disseminação do então chamado novo Betacoronavírus (NCoV-2019). Ao longo de menos de seis meses a realidade alterou radicalmente, o vírus passou a ser chamado de SARS-CoV-2, a doença causada pelo vírus recebeu o nome de Coronavirus Disease 2019 (COVID-19) e o vírus passou a afetar a vida de todos os cidadãos do Mundo. Atualmente são mais de 60 mil mortes confirmadas no país e mais de 500 mil mortes no mundo. O presente documento traz uma visão geral sobre o vírus, sobre os sintomas da doença, faixa etária acometida, período de incubação, formas de tratamento, meios de disseminação, análise de letalidade, formas de prevenção da disseminação do vírus, enfrentamento aos casos suspeitos, e um breve ensaio da atuação do fisioterapeuta no tratamento do paciente com a COVID-19.

Com a pandemia de COVID-19, é necessário o rápido entendimento das prováveis e graves sequelas que os pacientes sobreviventes podem desenvolver, assim como torna-se urgente traçar planos de ação para enfrentar tal situação desde o processo de alta hospitalar até a inserção em serviços de reabilitação cardiopulmonar. De acordo com recomendações internacionais, uma avaliação individualizada deve ser realizada e documentada no momento da alta, a qual deve contemplar as necessidades imediatas (controle dos sintomas como dispneia, fadiga e dor), assim como as necessidades a curto e médio prazo (melhora da função física e emocional; retorno ao trabalho; dentre outros). É aconselhável que nas primeiras semanas após a alta, o paciente seja atendido através de comunicação digital, a qual deve incluir imagem e áudio por questões de segurança. Componentes mandatórios dos programas de reabilitação cardiopulmonar incluem exercícios de força e resistência, além do trabalho da musculatura inspiratória. A abordagem educacional, é item importante no processo de tratamento. A reabilitação de doentes críticos acometidos por COVID-19 após alta hospitalar é de fundamental importância, especialmente naqueles que evoluíram com o quadro grave da doença, e que necessitaram de internação em UTI.

O presente documento apresenta recomendações para atuação do fisioterapeuta na reanimação cardiopulmonar em pacientes adultos com suspeita ou diagnóstico de COVID-19, apontando considerações principalmente aos cuidados específicos que devem ser adotados nesta situação clínica, os quais diferem das recomendações habituais. Tem como foco a preocupação com a proteção dos profissionais de saúde atuantes na reanimação cardiopulmonar e a melhor assistência aos pacientes, minimizando riscos, padronizando ações e recomendando condutas. Dentre as principais recomendações, há indicação de não desconexão da assistência ventilatória mecânica, e de uso de parâmetros ventilatórios específicos, além da abordagem da posição prona. Sabemos ainda que, dentro da atual situação mundial frente à pandemia COVID-19, é possível que estudos futuros possam contribuir para consolidar e acrescentar as recomendações apresentadas neste documento.
