The case study analyses a specifically described situation from the present and a summary of events from the past, finds alternative solutions, and decides on one of them. The target of such a detailed analysis was a concrete apartment building, which was still in the project phase; the efforts to change its proposed air conditioning system to a more energy-efficient one are the subject of the case study. Apartment buildings with modern technology installed in terms of sanitary equipment, heating, and simple methods of ventilation and cooling do not constitute part of the project.

Current prophylactic treatment of frequent episodic and chronic migraine is often unsuccessful or associated with adverse events. Calcitonin gene-related peptide (CGRP) is a key peripheral and central patophysiological agent in migraine. Data emerging from trials with monoclonal antibodies suggest that specific blockade of the CGRP pathway may provide an effective and treatment in migraine. At present, precise indication criteria for the administration of these antibodies, required duration of their administration and methods of evaluating their effect, especially on the quality of life, are being determined. The Czech Headache Society committee publishes the official Czech translation of the new International Classification of Headache Disorders (ICHD-3), which was published in the first number of the journal Cephalalgia of the International Headache Society (IHS) in January 2018. In this classification headaches are divided into 4 groups: I. Primary headaches; II. Secondary headaches; III. Neuropathies, facial pains and other headaches; and IV. Appendix. Taking into consideration the extent of the original classification, we do not present further specifying texts, comments, and translation of the Appendix where new subunits of the mentioned groups are presented because they are subjects of the next IHS scientific investigation. Hodgkin lymphoma (HL) is a relatively rare malignant disease of lymphatic tissue. It accounts for only 0.5% of all cancer cases with an incidence of 2.9 cases per 100 000 of men and women per year. The typical curve of incidence has 2 peaks in developed industrial countries: young people are most frequently affected (age at diagnosis is 20–35 years in 70% of patients); the second peak of the disease is at the age over 60 years, more frequently in men (3:2). Medical history frequently includes infectious mononucleosis, autoimmune diseases and immunosuppression. Genetic factors (familiar occurrence) have probably additional impact on the disease. Currently, HL is a malignant disease with the highest rate of curability. This success was achieved by is due to immeasurable and financially demanding efforts of haemato-oncological centres worldwide. Long-lasting remissions are achieved (according to the clinical stage and risk factors) in 85–95% of patients and their life expectancy is currently more than 40 years. Late effects of treatment are coming to the fore, especially cardiotoxicity and secondary malignancies. Current research is focused on two goals: reduction of late effects in cured patients (by reducing chemotherapy intensity and by reducing radiotherapy dose and extent) while maintaining excellent efficacy of first-line treatment: positron emission tomography (PET) plays a central role in this effort. The second goal is to rescue the otherwise tragic fate of patients with relapsed/refractory HL (RR HL). New drugs based on immunotherapy such as brentuximab vedotin- an antibody-drug conjugate and anti-PD1 check-point inhibitors (nivolumab and pembrolizumab) are increasing in importance alongside conventional chemotherapy and radiotherapy. Aim: To map the sentence comprehension in Slovak-speak ing patients with Parkinson dis ease. Method: We as ses sed the comprehension of reversible sentences (sentences in which the participants of the action can change their roles) with the Test of sentence comprehension and cognitive functions with a standard neuropsychological battery. Subjects: 37 patients with idiopathic Parkinson dis ease and 37 healthy controls participated in the research. Groups did not diff er regard ing age, education and sex. The dia gnosis of Parkinson disease in patients was established by a neurologist. Results: The statistical comparison of data us ing the Man n- Whitney U test revealed that patients had a signifi cantly lower score only in one out of six sentence types (p = 0.016), namely in comprehension of center-embedded object relative clauses (e. g. Mother, which the daughter kis sed, has the light dres s.). They were signifi cantly worse also in decod ing sentences with non-canonical order of thematic roles (fi rst patient, then agent; p = 0.013). Signifi cant cor relations of individual syntactic constructions appeared in three tests: Rey Complex Figure Test, Semantic fl uency and Letter-Number sequencing. Conclusion: Accord ing to our results, there are sentence comprehension defi cits in Slovak-speak ing patients with Parkinson dis ease, while sentence comprehension ability is cor related with cognitive test performance. We analyzed the current treatment patterns (acute and prophylactic) in migraine patients and the number of migraine patients potentially eligible for treatment with recent calcitonin gene-related peptide (CGRP) pathway-targeted therapies.Methods: This retrospective study utilized the Ministry of the Interior Health Insurance Fund claims database of the Czech Republic wherein every citizen is insured. Migraine patients with or without aura, and potentially on triptan therapy were included in this study (index years 2012– 2016). The prevalence approach included all patients (new and old) present in each index year. Prophylactic therapies were followed f0or three and seven years prior to the index year, including the index year, until 2010. The incidence approach included all patients first diagnosed in each index year. Prophylactic therapies were followed for the next three years, including the index year, until 2017 following incidence approach. The primary endpoint of this study was to determine the rate of migraine prevalence and diagnosis for each index year during the period 2012– 2016. The study also evaluated prophylactic and acute treatment patterns and comorbidities among patients in 2016.Results: The rate of migraine prevalence was 1% and the rate of diagnosis was 0.2– 0.4%. By prevalence approach, approximately 39% of the patients were on prophylactics, and 11.2% and 21.6% of the patient population had two prior treatment failures (three- and seven-year recall period, respectively). Antiepileptics (26%) and beta blockers (15.8%) were the most prescribed prophylactics, and sumatriptan was the predominant triptan used (12%) for acute treatment.Conclusion: Taking into account the number of inhabitants in the Czech Republic (10.7 million), there could be up to 23,000 adult patients eligible for novel CGRP therapies.Keywords: CGRP, prophylactics, triptans, claims database
