Abstract

OBJECTIVES:. Anemia has been associated with an increased risk of both cardiac arrest and stroke, frequent complications of COVID-19. The effect of hemoglobin level at ICU admission on a composite outcome of cardiac arrest or stroke in an international cohort of COVID-19 patients was investigated. DESIGN:. Retrospective analysis of prospectively collected database. SETTING:. A registry of COVID-19 patients admitted to ICUs at over 370 international sites was reviewed for patients diagnosed with cardiac arrest or stroke up to 30 days after ICU admission. Anemia was defined as: normal (hemoglobin ≥ 12.0 g/dL for women, ≥ 13.5 g/dL for men), mild (hemoglobin 10.0–11.9 g/dL for women, 10.0–13.4 g/dL for men), moderate (hemoglobin ≥ 8.0 and < 10.0 g/dL for women and men), and severe (hemoglobin < 8.0 g/dL for women and men). PATIENTS:. Patients older than 18 years with acute COVID-19 infection in the ICU. INTERVENTIONS:. None. MEASUREMENTS AND MAIN RESULTS:. Of 6926 patients (median age = 59 yr, male = 65%), 760 patients (11.0%) experienced stroke (2.0%) and/or cardiac arrest (9.4%). Cardiac arrest or stroke was more common in patients with low hemoglobin, occurring in 12.8% of patients with normal hemoglobin, 13.3% of patients with mild anemia, and 16.7% of patients with moderate/severe anemia. Time to stroke or cardiac arrest by anemia status was analyzed using Cox proportional hazards regression with death as a competing risk. Covariates selected through clinical knowledge were age, sex, comorbidities (diabetes, hypertension, obesity, and cardiac or neurologic conditions), pandemic era, country income, mechanical ventilation, and extracorporeal membrane oxygenation. Moderate/severe anemia was associated with a higher risk of cardiac arrest or stroke (hazard ratio, 1.32; 95% CI, 1.05–1.67). CONCLUSIONS:. In an international registry of ICU patients with COVID-19, moderate/severe anemia was associated with increased hazard of cardiac arrest or stroke.

Abstract

Objectives The Modern Innovative Solutions to Improve Outcomes in Asthma, Breathlessness and Chronic Obstructive Pulmonary Disease (COPD) (MABC) service aimed to enhance disease management for chronic respiratory conditions through specialist multidisciplinary clinics, predominantly in the community. This study assesses the outcomes of these clinics.Design This study used a prospective, longitudinal, participatory action research approach.Setting The study was conducted in primary care practices across Hampshire, UK.Participants Adults aged 16 years and above with poorly controlled asthma or COPD, as well as those with undifferentiated breathlessness not under specialist care, were included.Interventions Participants received care through the multidisciplinary, specialist-led MABC clinics.Primary and secondary outcome measures Primary outcomes included disease activity, quality of life and healthcare utilisation. Secondary outcomes encompassed clinic attendance, diagnostic changes, patient activation, participant and healthcare professional experiences and cost-effectiveness.Results A total of 441 participants from 11 general practitioner practices were recruited. Ninety-six per cent of participants would recommend MABC clinics. MABC assessments led to diagnosis changes for 64 (17%) participants with asthma and COPD and treatment adjustments for 252 participants (57%). Abstract

El artículo de Mark Sanders, “Introducción: ética e interdisciplinariedad en la filosofía y la teoría literaria” fue publicado originalmente en la revista Diacritics en el año 2002. Sanders parte de la pregunta de si el creciente interés por las discusiones éticas desde la disciplina de los estudios literarios tiene relación con las discusiones sobre ética que se desarrollan en la actualidad en el campo de la filosofía, e indaga si un diálogo interdisciplinario podría resultar útil a ambas disciplinas. El análisis de Sanders muestra de qué manera la discusión ética en los estudios literarios ha seguido un linaje particular. La preocupación por la alteridad, tema central a los estudios literarios dado el carácter de la literatura como “generadora de alteridades”, ha implicado el interés por autores tales como Michel Foucault, Emmanuel Lévinas, Jacques Derrida, Luce Irigaray, Giorgio Agamben y Alain Badiou. Entre tanto, la filosofía contemporánea se ha centrado más en una discusión ligada a la teoría política y la ética aplicada, por lo cual autores tales como Charles Taylor, John Rawls, Will Kymlicka, Iris Young y Seyla Benhabib han ocupado el centro de la discusión. Esta separación está, para Sanders, ligada a la manera como la filosofía continental ha perdido su lugar en las facultades de filosofía, y en cambio ha encontrado un desarrollo propio en la teoría literaria. Abstract

Amoxicillin clavulanate (Augmentin) is one of the most commonly used antibiotics in New Zealand (NZ). There are a number of known adverse effects of this drug, affecting the cutaneous, gastrointestinal, hepatic, and haematologic systems. Hepatic adverse effects, although rare (17 cases per 100,000 patients treated with amoxicillin clavulanate), include drug induced liver injury (DILI). This case of DILI secondary to amoxicillin clavulanate after admission with choledocholithiasis highlights the importance of recognising and treating rare but severe medication adverse effects.

Abstract

IMPORTANCE:. Factors associated with mortality in coronavirus disease 2019 patients on invasive mechanical ventilation are still not fully elucidated. OBJECTIVES:. To identify patient-level parameters, readily available at the bedside, associated with the risk of in-hospital mortality within 28 days from commencement of invasive mechanical ventilation or coronavirus disease 2019. DESIGN, SETTING, AND PARTICIPANTS:. Prospective observational cohort study by the global Coronavirus Disease 2019 Critical Care Consortium. Patients with laboratory-confirmed coronavirus disease 2019 requiring invasive mechanical ventilation from February 2, 2020, to May 15, 2021. MAIN OUTCOMES AND MEASURES:. Patient characteristics and clinical data were assessed upon ICU admission, the commencement of invasive mechanical ventilation and for 28 days thereafter. We primarily aimed to identify time-independent and time-dependent risk factors for 28-day invasive mechanical ventilation mortality. RESULTS:. One-thousand five-hundred eighty-seven patients were included in the survival analysis; 588 patients died in hospital within 28 days of commencing invasive mechanical ventilation (37%). Cox-regression analysis identified associations between the hazard of 28-day invasive mechanical ventilation mortality with age (hazard ratio, 1.26 per 10-yr increase in age; 95% CI, 1.16–1.37; p < 0.001), positive end-expiratory pressure upon commencement of invasive mechanical ventilation (hazard ratio, 0.81 per 5 cm H2O increase; 95% CI, 0.67–0.97; p = 0.02). Time-dependent parameters associated with 28-day invasive mechanical ventilation mortality were serum creatinine (hazard ratio, 1.28 per doubling; 95% CI, 1.15–1.41; p < 0.001), lactate (hazard ratio, 1.22 per doubling; 95% CI, 1.11–1.34; p < 0.001), Paco2 (hazard ratio, 1.63 per doubling; 95% CI, 1.19–2.25; p < 0.001), pH (hazard ratio, 0.89 per 0.1 increase; 95% CI, 0.8–14; p = 0.041), Pao2/Fio2 (hazard ratio, 0.58 per doubling; 95% CI, 0.52–0.66; p < 0.001), and mean arterial pressure (hazard ratio, 0.92 per 10 mm Hg increase; 95% CI, 0.88–0.97; p = 0.003). CONCLUSIONS AND RELEVANCE:. This international study suggests that in patients with coronavirus disease 2019 on invasive mechanical ventilation, older age and clinically relevant variables monitored at baseline or sequentially during the course of invasive mechanical ventilation are associated with 28-day invasive mechanical ventilation mortality hazard. Further investigation is warranted to validate any causative roles these parameters might play in influencing clinical outcomes.

Abstract

Carbon Capture and Sequestration (CCS) has been recognized as an important means of mitigating global climate change, but apart from several pilots, it has not yet been successfully implemented on the large scale needed to live up to the expectations as a mitigation method. In Netherlands, the option of CCS has been the subject of debate for a long time, as three unsuccessful projects – two onshore in Barendrecht and the Northern regions, and one offshore near the Port of Rotterdam – demonstrate. Nevertheless, CCS has been accorded an important place in the current Dutch climate policies, being expected to contribute up to 7 Megaton of CO2 reduction. This is reflected in a fresh crop of CCS project plans. For the most, these plans have a long way to go from the drawing board to actual operations due to the technical, economic, legal and societal challenges ahead. In this article we review the status and possibilities of CCS in Netherlands based on an analysis of existing literature in the relevant disciplines. First, a brief overview of the technology options for carbon capture and storage or utilization is given. This is followed by a detailed analysis of the governmental support for CCS, given the vital role that fit-for-purpose legal frameworks and policy instruments will play in CCS deployment. Technical, legal and policy uncertainties translates into factors inhibiting CCS investment and so the paper then presents a CCS investment project to illustrate how such risks affect the business case for CCS. Finally, bearing in mind that societal acceptance has proved to be a major barrier for CCS, both in Netherlands and elsewhere, the conditions that enhance public acceptance of CCS are examined. Our work shows that while CCS is technically a straightforward proposition, its deployment has historically been hindered by the lack of a sound business case and a compelling and stable socio-technical narrative. The main argument in favor of CCS today is that it offers a transition pathway for rapidly and massively reducing CO2 emissions beyond what could be accomplished by alternative methods like electrification and renewable fuels in near future. The introduction of new financial instruments, increased government support and an improvement in social engagement appear to have enhanced the prospects of CCS in Netherlands, but we feel it is premature to assume that this time everything is different.

Abstract

As the energy transition accelerates and renewable energy technologies become cost-competitive with fossil fuels in many countries, the availability of finance could become a bottleneck. Integrated assessment models (IAMs) and other macro-economic transition (MET) models typically do not feature detailed financial markets and do not sufficiently consider financing barriers and opportunities for the transition to carbon neutrality. While progress has been made in the representation of financial markets in macro-models since the financial crisis of 2008 the focus has been on financial (in)stability of the financial sector, not its ability to finance investment projects in the energy transition. Hence, a crucial gap remains, preventing macro model-based analysis of financing barriers and policy interventions that may accelerate the energy transition. In this article we review how state-of-the-art macro-economic models consider the financial sector. From this review we identify what elements are still missing to adequately model the financial dynamics and challenges for the energy transition specifically. Based on a discussion of relevant parts of the finance literature, we then propose four steps to improve the representation of finance in global IAMs and MET models more generally.

Abstract

Abstract Background Self-administration of medicines by patients whilst in hospital is being increasingly promoted despite little evidence to show the risks and benefits. Pain control after total knee replacement (TKR) is known to be poor. The aim of the study was to determine if patients operated on with a TKR who self-medicate their oral analgesics in the immediate post-operative period have better pain control than those who receive their pain control by nurse-led drug rounds (Treatment as Usual (TAU)). Methods A prospective, parallel design, open-label, randomised controlled trial comparing pain control in patient-directed self-management of pain (PaDSMaP) with nurse control of oral analgesia (TAU) after a TKR. Between July 2011 and March 2013, 144 self-medicating adults were recruited at a secondary care teaching hospital in the UK. TAU patients (n = 71) were given medications by a nurse after their TKR. PaDSMaP patients (n = 73) took oral medications for analgesia and co-morbidities after two 20 min training sessions reinforced with four booklets. Primary outcome was pain (100 mm visual analogue scale (VAS)) at 3 days following TKR surgery or at discharge (whichever came soonest). Seven patients did not undergo surgery for reasons unrelated to the study and were excluded from the intention-to-treat (ITT) analysis. Results ITT analysis did not detect any significant differences between the two groups’ pain scores. A per protocol (but underpowered) analysis of the 60% of patients able to self-medicate found reduced pain compared to the TAU group at day 3/discharge, (VAS -9.9 mm, 95% CI -18.7, − 1.1). One patient in the self-medicating group over-medicated but suffered no harm. Conclusion Self-medicating patients did not have better (lower) pain scores compared to the nurse-managed patients following TKR. This cohort of patients were elderly with multiple co-morbidities and may not be the ideal target group for self-medication. Trial Registration ISRCTN10868989. Abstract

Many of N.P. van Wyk Louw’s essays address the role of the intellectual. In the 1930s, Louw struggled to define a place for the intellectual in relation to the Afrikaner volk and its cultural movements and political parties. At the end of “Kultuurleiers sonder kultuur” (Cultural Leaders without Culture) (1939), Louw turns to the simile of the cave from book seven of Plato’s Republic. There the cave represents the city and its people, who are trapped in illusion. Plato’s “philosopher” escapes the dark chamber where the prisoners observe only shadows, gains enlightenment, and returns to open the eyes of his fellow inhabitants. Returning is the duty of the philosopher. How did N.P. van Wyk Louw imagine descending back into the “cave,” into the midst of the city, to be among the people of his country? One answer lies in a pair of unpublished fragments dating from the 1940s entitled “In die bus afgeluister” (Overheard on the Bus). In these fragments, Louw eavesdrops, as he takes the bus to and from work, on the conversations of people of Cape Town of various races. We get a slice of city life, and a sense of how Louw tried to embrace that life rather than isolate himself from it. These two urban sketches nevertheless show that the task of enlightening one’s fellow citizens proves more complicated than Louw expects because the intellectual is more deeply implicated in the illusory play of shadows than he imagines.

Unveiled at the conclusion of a meeting of the Royal Medical and Chirurgical Society in 1861,[1] ‘Dr Nelson’s Improved Inhaler’ was one of the most important milestones in the genesis of reliable treatment of respiratory ailments in the modern era. Affordable and suitable for self-medication, the Dr Nelson’s Inhaler offered simple and reliable relief for patients with respiratory and pulmonary ailments. Conspicuous for its modesty and simplicity, it was one of the most widely produced, reproduced, and used inhalation devices in the final third of the nineteenth century. By reconstructing the ‘biography’ of the Nelson Inhaler, this article will attempt to sketch a network of medical and commercial interests and expertise in London which aligned in the 1860s to help establish inhalation as a popular, inexpensive, and trusted form of medical therapy for pulmonary ailments. This article will look at what connects physicians, apothecaries, and patients in the era: the medicines and technologies that were prescribed, made, bought, and which caused wellness, side-effects, and even death. This approach allows us to develop a narrative of respiratory illness as it was experienced by practitioners and patients alike.

24. A Rationale for Going Back to the Future: Use of Disposable Spacers for Pressurised Metered Dose Inhalers

The introduction of pressurised metered dose inhalers (MDIs) in the mid-1950s completely transformed respiratory treatment. Despite decades of availability and healthcare support and development of teaching aids and devices to promote better use, poor pMDI user technique remains a persistent issue. The main pMDI user aid is the spacer/valved holding chamber (VHC) device. Spacer/chamber features (size, shape, configuration, construction material, and hygiene considerations) can vie with clinical effectiveness (to deliver the same dose as a correctly used pMDI), user convenience, cost, and accessibility. Unsurprisingly, improvised, low-cost alternatives (plastic drink bottles, paper cups, and paper towel rolls) have been pressed into seemingly effective service. A UK law change permitting schools to hold emergency inhalers and spacers has prompted a development project to design a low-cost, user-friendly, disposable, and recyclable spacer. This paper spacer requires neither preuse priming nor washing, and has demonstrated reproducible lung delivery of salbutamol sulphate pMDI, comparable to an industry-standard VHC, an alternative paperboard VHC, and pMDI alone. This new device appears to perform better than these other VHC devices at the low flow rates thought achievable by paediatric patients. The data suggest that this disposable spacer may have a place in the single-use emergency setting.

25. Community College Presidents and Their Role in the American Democracy: A Narrative Inquiry

Abstract

The American democracy is experiencing strain from the erosion of democratic norms and its political, judicial, social, and economic institutions. In short, the American democracy shows signs of democratic deconsolidation. Community colleges are higher education institutions that help consolidate the U.S. democracy by representing democratic values such as equality and opportunity. The purpose of my study was to explore how selected community college presidents understand and articulate the responsibility of their institutions to prepare students for a meaningful role in the American democracy. Qualitative narrative inquiry methods, including in-depth semistructured interviews and document analysis, were used to collect data for the study. Three primary themes emerged from the data that addressed the purpose of this study: (1) community college philosophy: blueprint for a vision; (2) consolidating local democracies; and (3) citizens as students, students as citizens. These three themes supported further interpretation of the data that was organized under these headings: (1) the community college democratic mission; (2) community colleges help deepen democracy; and (3) the role of community colleges in the American democracy: public goods, private goods. In summary, my research found that first, my participants believed that community colleges have a responsibility to the American democracy and this responsibility is reflected in their community college mission. Second, my participants framed the American democracy as a continual work in progress and that community colleges help deepen democracy. Third, for my participants, community colleges are not merely distinct in their institutional mission and philosophy, but in their role in supporting the American democracy. Abstract

Many river restoration projects fail. Inadequate project planning underpins many of the reasons given for failure (such as setting overly ambitious goals; selecting inappropriate sites and techniques; losing stakeholder motivation; and neglecting to monitor, assess, and document projects). Another major problem is the lack of an agreed guiding image to direct the activities aimed at restoring the necessary biophysical and ecological processes within the logistic constraints of on-ground works. Despite a rich literature defining the components of restoration project planning, restoration ecology currently lacks an explicit and logical means of moving from the initial project vision through to on-ground strategies. Yet this process is fundamental because it directly links the ecological goals of the project to the on-ground strategies used to achieve them. We present a planning process that explicitly uses an interdisciplinary mechanistic model of disturbance drivers and system responses to build from the initial project vision to the implementation of on-ground works. A worked example on the Upper Hunter River in southeastern Australia shows how understanding catchment history can reveal disturbance and response mechanisms, thus facilitating process-based restoration.

Plain English SummaryEntrepreneurship research questions the core assumptions of other academic fields and legitimizes them both practically and academically. Since the 1980s, entrepreneurship research has seen tremendous growth and development, establishing itself as an academic field. Entrepreneurship is also taught extensively in leading business schools around the world. Indeed, few business schools do not address entrepreneurship in their curriculum. This represents a sea change: although entrepreneurs and new ventures had a remarkable impact on society, academia barely noticed it in the 1980s. Simply put: economics and business students rarely, if ever, encountered any mention of entrepreneurship during their studies. While entrepreneurship research has now developed its own methodological toolbox, it has extensively borrowed perspectives, theories, and methods from other fields. In the 2020s, we now find that entrepreneurship scholars are sharing its toolbox with other academic fields, questioning the core assumptions of other academic fields and providing new insights into the antecedents, mechanisms, and consequences of their respective core phenomena. Moreover, entrepreneurship research helps to legitimize other academic fields both practically and academically. Hence, entrepreneurship research now plays not just an important role in entrepreneurship education, practice, and policy but also throughout many other research fields.The remarkable ascent of entrepreneurship witnessed as a scientific field over the last 4 decades has been made possible by entrepreneurship's ability to absorb theories, paradigms, and methods from other fields such as economics, psychology, sociology, geography, and even biology. The respectability of entrepreneurship as an academic discipline is now evidenced by many other fields starting to borrow from the entrepreneurship view. In the present paper, seven examples are given from this "pay back" development. These examples were first presented during a seminar at the Erasmus Entrepreneurship Event called what has the entrepreneurship view to offer to other academic fields? This article elaborates on the core ideas of these presentations and focuses on the overarching question of how entrepreneurship research impacts the development of other academic fields. We found that entrepreneurship research questions the core assumptions of other academic fields and provides new insights into the antecedents, mechanisms, and consequences of their respective core phenomena. Moreover, entrepreneurship research helps to legitimize other academic fields both practically and academically.

This paper describes idle vibration reduction methods using a Stellantis vehicle as a case study. The causes of idle vibration are investigated using the NVH source, path, and receiver method. The torque transfer path into a vehicle has shown to be very important in determining vehicle idle vibration response. New electronic control enablers that affect idle vibration are tested and discussed, including Neutral Idle Control (NIC), Transfer-case Idle Control (TIC,®), and Switchable Engine Mounts (SEM). The Design For Six Sigma (DFSS) analysis method is used to arrive at an optimized result for vehicle idle vibration. This paper also discusses the results confirming TIC’s capability of reducing idle vibration on all-wheel drive vehicles.Transfer-case Idle Control is a new idle vibration control enabler developed by Stellantis and a patent was awarded by the United State Patent and Trademark Office.

By using a domain-specific description language, this article presents how the interface of a measurement system, prototypically based on HTTP and MQTT, can be generated based on a meta-data-model and how measurement data can be annotated with context- and metadata.

Abstract

Background: Despite their effectiveness in reducing low-density lipoprotein-cholesterol (LDL-C) and cardiovascular disease (CVD) risk, high-intensity statins are underutilized among patients with LDL-C ≥ 190 mg/dL. We aimed to determine if a patient outreach program (KP Sure Net) automating high-intensity statin orders and lipid panels improved statin initiation and lab completions. Methods: Kaiser Permanente Southern California adults 20-60 years of age with LDL-C ≥ 190 mg/dL and no recent statin use were compared pre- (January 2016-September 2018, n=3,534) and post- (April 2019-July 2021, n=3,555) program implementation to examine statin prescriptions and fills, lab completions, and improved LDL-C ( Results: Among adults identified during pre- and post-implementation, 759 (21.5%) and 976 (27.5%), respectively, received a statin prescription. Adults with a prescription post- versus pre-implementation were more likely to be younger, identify as Black or Asian ethnicity, be English-speaking and have higher income, but were similar by sex, education, insurance, and comorbidity burden. (Table) After multivariable adjustment, adults post- versus pre-implementation had a higher likelihood of receiving a statin prescription (RR 1.36, 95% CI 1.25, 1.48), filling their statin (RR 1.32, 95% CI 1.26, 1.38), completing their lipid panel (RR 1.60, 95% CI 1.44, 1.78), and improving LDL-C (RR 1.36, 95% CI 1.20, 1.53). Conclusion: Automating statin orders and lipid panels for high-risk adults improved prescription rates, fills and LDL-C. Understanding facilitators and barriers to adherence is an important next step in improving patients’ health.

biBackground:/i/bSuboptimal use of pressurized metered dose inhaler (pMDI) remains a major barrier to inhaled therapy success. Verbal inhaler technique training (VT) fails to maintain patients' good pMDI use, thus training tools might help. Trainhalersup®/sup(THR device) and Flo-Tonesup®/supCR (FTCR device), two novel pMDI technique training tools, were evaluated and compared in terms of relative lung and systemic bioavailability and oropharyngeal deposition of salbutamol inhaled from Ventolinsup®/supEvohalersup®/sup(GlaxoSmithKline) either alone following THR or connected to FTCR.biMethods:/i/bSixteen healthy adults inhaled 2 × 100 μg salbutamol puffs (1 minute apart) from Ventolin using the THR device or FTCR device in a two-period, randomized crossover study. A 7-day washout separated THR and FTCR approaches. Immediately after each puff inhalation, each subject gargled with 20 mL water for oropharyngeal deposition determination. Urine samples were collected 0.5 hour (pre-inhalation) and 0.5, 1.0, and 2.0 hours post-inhalation. Urine was then pooled till 24-hour post-inhalation. The relative lung bioavailability (0- to 0.5-hour urinary salbutamol excretion-USAL0.5) and relative systemic bioavailability (0- to 24-hour urinary excretion of salbutamol and its metabolite-USALMET24) were determined.biResults:/i/bThe mean (standard deviation [SD]) USAL0.5 of the THR and FTCR groups was 5.70 (6.43) and 11.39 (9.67) μg, respectively. The mean (SD) oropharyngeal deposition was 11.11 (4.37) and 6.09 (1.89) μg, respectively. The THR and FTCR devices were statistically significantly different in USAL0.5 and oropharyngeal deposition (ip/i lt; 0.001), whereas there was no statistically significant difference in USALMET24.biConclusion:/i/bThe THR device and the FTCR device showed positive impact on inhaled pMDI delivery. Indeed, the FTCR device doubled the relative lung bioavailability and minimized the unwanted oropharyngeal deposition of inhaled salbutamol. Abstract

Due to low bioavailability of dietary phenolic compounds in small intestine, their metabolism by gut microbiota is gaining increasing attention. The microbial metabolism of theasinensin A (TSA), a bioactive catechin dimer found in black tea, has not been studied yet. Here, TSA was extracted and purified for in vitro fermentation by human fecal microbiota, and epigallocatechin gallate (EGCG) and procyanidin B2 (PCB2) were used for comparison. Despite the similarity in their flavan-3-ol skeletons, metabolic fate of TSA was distinctively different. After degalloylation, its core biphenyl-2,2',3,3',4,4'-hexaol structure remained intact during fermentation. Conversely, EGCG and PCB2 were promptly degraded into a series of hydroxylated phenylcarboxylic acids. Computational analyses comparing TSA and PCB2 revealed that TSA's stronger interflavanic bond and more compact stereo-configuration might underlie its lower fermentability. These insights in the recalcitrance of theasinensins to degradation by human gut microbiota are of key importance for a comprehensive understanding of its health benefits.

Zes gemeenten – Groningen, Utrecht, Tilburg, Wageningen, Deventer en Nijmegen – hebben de afgelopen jaren (van 1 oktober 2017 tot 31 december 2019) unieke randomised controlled trials uitgevoerd in de bijstand. De volgende (combinatie van) interventies zijn onderzocht: ontheffing van re-integratieverplichtingen, intensivering van begeleiding en vrijlating van bijverdiensten. Er is zowel gekeken naar uitstroom naar werk als naar baanzoekintensiteit, welbevinden, zelfredzaamheid en sociaal vertrouwen. De uitstroom naar werk is bij alle interventies niet lager dan de huidige aanpak, terwijl de uitstroom naar deeltijdwerk in sommige gemeenten hoger is. Vooral intensivering op maat en vrijlating kan uitstroom naar werk (minimaal in deeltijd) vergroten. Voor wat betreft de effecten op baanzoekintensiteit, zelfredzaamheid, welbevinden en vertrouwen, is het beeld diffuus. We vinden kleine en soms ook positieve effecten, vooral voor zelfredzaamheid en vertrouwen, maar het beeld is niet eenduidig. De kleine aantallen deelnemers in de experimenten maken het lastig statistisch significante effecten te vinden., Which municipalities' action and intervention strategies contribute to job prospects and an improvement in people well-being and confidence in social assistance benefits? Six municipalities – Groningen, Utrecht, Tilburg, Wageningen, Deventer and Nijmegen – have conducted unique randomized controlled trials for social assistance benefits in recent years (October 1, 2017 - December 31, 2019). The following (combination of) interventions have been investigated: exemption from reintegration obligations, intensification of counselling and earnings release of extra earnings. The outflow to work as well as job search intensity, well-being, self-efficacy and trust were examined. The outflow to work for all interventions is not lower than in the current approach, while the outflow to part-time work is higher in some municipalities. In particular, tailor-made intensification and earnings release can increase outflow to (at least part-time) work. The picture is diffuse concerning the effects on job search intensity, well-being, self-efficacy and trust. We find small, and sometimes positive effects, especially for self-reliance and confidence, but the picture is not clear-cut. The small numbers of participants in the experiments make it difficult to find statistically significant effects.

Abstract

How do weather anomalies affect the economy at the local level? This paper presents a new data set that links weather data to annual average night-light emission data for 24.000 0.5°× 0.5° grid-cells around the globe for the period 1992–2013. Interpreting night-light emission as a proxy for economic activity, these data allow one to investigate how weather anomalies affect economic activity. Global coverage avoids selection bias, while high spatial resolution avoids averaging out heterogeneity in local impacts at higher aggregation levels. Our data show significant effects on the local growth of night-light for storms, excessive precipitation, droughts, and cold spells. Moreover, we find evidence for significant spatial spillovers to neighboring areas. Our results suggest that these offsetting spillovers are typically local. As positive and negative effects average out in larger areas, our results call for the analysis of economic effects of weather anomalies at a high geographical resolution. Finally, our results are driven by events in lower income regions. As climate change is expected to make weather patterns more erratic, our new data can inform emerging debates on how this will affect the economy in both science and politics.
