2. Abstract P1-17-08: Abemaciclib and endocrine therapy for hormone receptor-positive, HER2-negative advanced breast cancer: A real-world UK multicentre experience

Background Abemaciclib is approved in 1st and 2nd line for hormone receptor (HR)-positive HER2-negative advanced breast cancer (ABC). However, outcomes on this agent are unclear in the real world.We determined the safety and efficacy profile of abemaciclib across 15 institutions in the United Kingdom. Methods We retrospectively identified HR-positive, HER2-negative ABC patients who received abemaciclib between July 2018 and June 2020. Demographics, disease characteristics, prior treatments, blood tests, toxicities, treatment delays and responses were recorded. Simple statistics, Fisher’s exact test, chi-squared method and Cox regression were used as appropriate. Results 228 patients identified had median age of 64 (31-93). 172 (75.4%) were postmenopausal and 209 (91.7%) had ECOG Performance Status 0-1. 145 (63.6%) had visceral involvement and 44 (19.3%) only bone disease. Patients received a median 1 (range 0-7) prior lines of treatment and 0 (range 0-5) prior chemotherapy lines.148 patients (64.9%) experienced diarrhoea (grade ≥3 in 16 [7.0%]). 146 patients (64.0%) developed neutropenia (grade ≥3 in 40 [17.5%]). 5 experienced febrile neutropenia (2.2%). 32 patients (14.0%) required hospitalisation due to toxicity (diarrhoea in 9 [3.95%]).Dose reductions were required in 107 patients (46.9%), mainly due to diarrhoea (55 [24.1%]) and to 50mg BD in 30 patients (13.2%). Dose delays were in median 14 (range 2-118). Abemaciclib was discontinued in 121 patients (53.1%) due to disease progression in 61 (26.7%) and toxicity in 48 (21.0%) (diarrhoea in 16 [7.0%]).Abemaciclib produced clinical benefit rate of 82.8% and overall response rate of 47.2% in 163 patients assessed. Overall, median progression-free survival (PFS) was 6.4 months (95% confidence interval [CI] 4.4-7.8) and median overall survival (OS) was 8.8 months (95% CI 7.6-10.6). Conclusions This a large real-world analysis of the safety and efficacy of abemaciclib in combination with endocrine therapy for advanced HR-positive breast cancer. In this analysis, the rates of diarrhoea were lower compared with the pivotal trial data, while neutropenia was more frequent. Although the PFS outcomes were similar to those previously reported, median OS was worse in this cohort which may reflect the different population of patients included, who were older and had more frequent visceral involvement. The selection of patients suitable for abemaciclib is crucial to ensure adequate efficacy and safety outcomes in this setting. Background: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92]; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events. This introductory chapter examines previous archaeological thinking regarding the origins, scales, forms of evidence, and nature of social complexity in the human past. As a relative newcomer to this topical area, bioarchaeological approaches are shown to bring unique theoretical and empirical perspectives on these issues. Unique strengths of the bioarchaeological approach include compelling ways of looking at power, hierarchy, and heterarchy, the embodiment of social realities into human behavior and biology, and avenues to bridge long-standing divides between mortuary archaeology and bioarchaeology. The authors also discuss a number of challenges in the bioarchaeology of social complexity, including how best to grapple with typological thinking and interpretation of social structures from human skeletal remains. The chapter closes with a preview summary of the following chapters in the volume.

The electroencephalogram (EEG) remains the primary tool for diagnosis of abnormal brain activity in clinical neurology and for in vivo recordings of human neurophysiology in neuroscience research. In EEG data acquisition, voltage is measured at positions on the scalp with respect to a reference electrode. When this reference electrode responds to electrical activity or artifact all electrodes are affected. Successful analysis of EEG data often involves re-referencing procedures that modify the recorded traces and seek to minimize the impact of reference electrode activity upon functions of the original EEG recordings.We provide a novel, statistically robust procedure that adapts a robust maximum-likelihood type estimator to the problem of reference estimation, reduces the influence of neural activity from the re-referencing operation, and maintains good performance in a wide variety of empirical scenarios.The performance of the proposed and existing re-referencing procedures are validated in simulation and with examples of EEG recordings. To facilitate this comparison, channel-to-channel correlations are investigated theoretically and in simulation.The proposed procedure avoids using data contaminated by neural signal and remains unbiased in recording scenarios where physical references, the common average reference (CAR) and the reference estimation standardization technique (REST) are not optimal.The proposed procedure is simple, fast, and avoids the potential for substantial bias when analyzing low-density EEG data.

The 5-year transformation at Guardian Life Insurance provides significant insights for organizations aspiring to enhance their IT–business alignment. Alignment is more than just organizational structure, there is no silver bullet; it requires effective governance, partnership, communications, value analytics, technical services, and people. Like most companies around the globe, Guardian recognized the importance of moving to a collaborative IT–business alignment relationship; IT–business alignment was a persistent pervasive problem that needed to be improved. In 2005, Guardian viewed IT as an ineffective expensive cost of supporting the business. Was improving the IT–business relationship as difficult as drawing ‘a line’ in the sand? In this qualitative study in a child and adolescent mental health service setting, clinical letters written by one clinician were copied to 67 families. Feedback was sought from these families and was received from just under half of them. A majority found their letters helpful. Nearly all approved of the principleof copying. The requirement to copy meant extra effort for the clinician, but it had positive effects on both letter writing and clinical practice. There are grounds on which to argue that copied letters may have a 'therapeutic effect'.

The health of the Colonial-period Maya from Tipu, Belize, was evaluated using a skeletal series to explore differential effects of European contact by sex. Variables addressed were nutrition and disease patterns, reproductive patterns, and occupational stress. Results suggest that females enjoyed fewer childhood health disruptions, likely as a result of greater genetic buffering. No evidence of male preferential treatment was observed. Frequencies of indicators were similar to those reported for precontact Maya. Markers of adult activity patterns, including timing of parity, were also comparable to those of earlier groups. These findings support the cultural continuity with the Postclassic suggested by the archaeological and ethnohistorical records at Tipu.

This paper proposes a novel technique of camouflaging a block of binary data in an audio file for secured covert communication. The phase of the cover signal is used as a medium for steganography. Cover signal may be a song or a speech signal. Instead of modifying the absolute phase values of selected component frequencies, the difference between the phase values of the selected component frequencies and their adjacent frequencies of the cover signal are modified by a small amount to hide data bits. The change is so minute that it hardly alters the phase spectrum and is imperceptible to human auditory system(HAS). This technique provides robustness, noise insensitivity, high data capacity and almost accurate data recovery from the cover signal.

This paper describes a method for achieving audio steganography through spectrum modification of the cover signal- the signal into which the useful and valuable information is embedded. The embedded data signal is inserted into the regions of the cover signal spectrum where power of frequency content is low. Also the level of the embedded signal is kept low enough inorder to avoid the crossing of thresholds for audio perception. By performing these steps, the data signal shows no trace of being embedded in the cover signal. This method of audio steganography can be used to robust covert communication.
