Abstract

International audience; BackgroundNeuromuscular blocking agents (NMBAs) are among the leading cause of perioperative anaphylaxis, and most of these reactions are IgE mediated. Allergic sensitisation induced by environmental exposure to other quaternary ammonium-containing compounds, such as pholcodine, has been suggested. The aim of this study was to assess the relationship between pholcodine exposure and NMBA-related anaphylaxis.MethodsALPHO was a multicentre case-control study, comparing pholcodine exposure within a year before anaesthesia between patients with NMBA-related perioperative anaphylaxis (cases) and control patients with uneventful anaesthesia in France. Each case was matched to two controls by age, sex, type of NMBA, geographic area, and season. Pholcodine exposure was assessed by a self-administered questionnaire and pharmaceutical history retrieved from pharmacy records. The diagnostic values of anti-pholcodine and anti-quaternary ammonium specific IgE (sIgE) were also evaluated.ResultsOverall, 167 cases were matched with 334 controls. NMBA-related anaphylaxis was significantly associated with pholcodine consumption (odds ratio 4.2; 95% confidence interval 2.3–7.0) and occupational exposure to quaternary ammonium compounds (odds ratio 6.1; 95% confidence interval 2.7–13.6), suggesting that apart from pholcodine, other environmental factors can also lead to sensitisation to NMBAs. Abstract

To the Editors: Allergic bronchopulmonary aspergillosis (ABPA) is a pulmonary disorder that results from a hypersensitivity reaction to Aspergillus spp. It has been estimated to occur in 1–3% of people with chronic asthma and 2–15% of those with cystic fibrosis [1]. The natural history of ABPA is characterised by exacerbations that can threaten the patient’s survival and prognosis [1]. Repetition of such exacerbations is responsible for the development of bronchiectasis, permanent obstructive ventilation defect or fibrotic lung lesions. Prognosis mainly depends on the very early treatment of exacerbations before bronchiectasis sets in. In 2008, the guidelines of the Infectious Diseases Society of America advised combination therapy in ABPA [2]: systemic glucocorticoids to limit the inflammatory component and antifungals to limit mycelium proliferation. However, even though glucocorticoid therapy and antifungals are the treatment of choice for acute-stage ABPA and exacerbations, there are no data to guide the duration of this treatment. Therefore, the current objectives for the management of ABPA are a decrease in the frequency and duration of exacerbations, and a limited solicitation of glucocorticoids. Consequently, the maintenance treatment in the management of ABPA remains a current and progressive problem for pneumologists. We describe a case of ABPA that was difficult to control using the standard treatment regimen, but which improved dramatically and durably following administration of nebulised liposomal amphotericin B (LAmB). In May 2010, a 67-yr-old female presented with fever and productive cough, with sputum plugs and a history of epilepsy treated with phenobarbital. Abstract

International audience; BACKGROUND: Serotonin (5-HT) has a role in respiratory function and dysfunction. Although 5-HT affects respiratory drive to both phrenic and cranial motoneurons, relatively little is known about the role of 5-HT receptor subtypes in the control of upper airway muscle (UAM) respiratory activity. MATERIALS AND METHODS: Here, we performed central injections of 5-HT1A agonist (8-OHDPAT) or antagonist (WAY100635) in anesthetized rats and analyzed changes in the electromyographic activity of several UAM and other cardiorespiratory parameters. We also compared the pattern of Fos expression induced after central injection of a control solution or 8-OHDPAT. RESULTS: Results showed that 8-OHDPAT induced a robust increase in UAM activity, associated with either tachypnea under volatile anesthesia or bradypnea under liquid anesthesia. Injection of WAY100635 switched off UAM respiratory activity and led to bradypnea, suggesting a tonic excitatory role of endogenous 5-HT1A receptor activation. Co-injection of the agonist and the antagonist blocked the effects produced by each drug alone. Besides drug-induced changes in respiratory frequency, only slight increases in surface of diaphragm bursts were observed. Significant increases in Fos expression after 5-HT1A receptor activation were seen in the nucleus tractus solitarius, nucleus raphe pallidus, parapyramidal region, retrotrapezoid nucleus, lateral parabrachial, and Kölliker-Fuse nuclei. This restricted pattern of Fos expression likely identified the neural substrate responsible for the enhancement of UAM respiratory activity observed after 8-OHDPAT injection. CONCLUSIONS: These findings suggest an important role for the 5-HT1A receptors in the neural control of upper airway patency and may be relevant to counteract pharyngeal atonia during obstructive sleep apneas.
