Abstract The aim of the present study was to examine the relationships between gender, sense of coherence (SOC), social support, perception of academic environment and mental health of dental students during the coronavirus disease 2019 (COVID-19) pandemic. A longitudinal study involving 65 undergraduates enrolled from the first to sixth semesters in the bachelor dental course of Universidade Federal Fluminense, Brazil, was conducted. Baseline data (2018) included age, gender, SOC, social support, stress control and perception of education environment. Depression, anxiety and stress were assessed at baseline (response rate: 93%) and two-year follow-up (2020; response rate: 37%). Structural Equation Modelling was used to test the relationships between independent variables and depression, anxiety and stress during COVID-19 pandemic. Lower social support (β = -0.15), lower stress control (β = -0.20) and lower SOC (β = -0.39) were directly linked to higher depression (β = 0.22). Female gender (β = 0.22), worse perception of educational environment (β = -0.24) and lower SOC (β = -0.57) directly predicted higher anxiety. Female gender (β = 0.18), lower stress control (β = -0.21), and lower SOC (β = -0.46) directly predicted higher stress. The link between gender and both depression and stress, was mediated by stress control. Social support was indirectly linked to depression and stress via SOC. Perception of educational environment mediated the link between SOC and anxiety. Mental health of dental students during COVID-19 pandemic was influenced by demographic characteristics, perception of educational environment, social support and SOC through both direct and indirect pathways.

Abstract Background More in-depth evidence about the complex relationships between different risk factors and mental health among adolescents has been warranted. Thus, the aim of the study was to examine the direct and indirect effects of experiencing social pressure, bullying, and low social support on mental health problems in adolescence. Methods A school-based cross-sectional study was conducted in 2022 among 15 823 Norwegian adolescents, aged 13–19 years. Structural Equation Modelling was used to assess the relationships between socioeconomic status, social pressure, bullying, social support, depressive symptoms, self-harm and suicide thoughts. Results Poor family economy and low parental education were associated with high pressure, low parental support and depressive symptoms in males and females. Moreover, poor family economy was associated with bullying perpetration and bullying victimization among males and females, and cyberbullying victimization among females, but not males. Low parental education was associated with bullying victimization among males, but not females. Further, high social pressure was associated with depressive symptoms among males and females, whereas high social pressure was linked to self-harm and suicide thoughts among females, but not males. Bullying victimization and cyberbullying victimization were associated with depressive symptoms, self-harm, and suicide thoughts among males and females. Bullying victimization was associated with depressive symptoms among males, but not females, whereas bullying perpetration was linked to self-harm and suicide thoughts among females, but not males. Low parental support was associated with bullying perpetration, bullying victimization, depressive symptoms, self-harm and suicide thoughts among males and females, whereas low parental support was associated with high social pressure among females, but not males. Low teacher support was associated with high social pressure and depressive symptoms. Low support from friends was associated with bullying victimization, depressive symptoms and suicide thoughts among males and females, whereas low support from friends was linked to self-harm among males, but not females. Finally, results showed that depressive symptoms were associated with self-harm and suicide thoughts among males and females. Conclusion Low socioeconomic status, social pressure, bullying and low social support were directly and indirectly associated with depressive symptoms and self-directed violence among Norwegian adolescents.

Abstract Background The presence of untreated sexually transmitted infections (STIs) significantly increases the chance of acquiring HIV. In Brazil, testing for Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) among Pre-Exposure Prophylaxis (PrEP) users is insufficient, and syndromic treatment is a priority in clinical practice. Multi-site testing for CT/NG improves thescreening of asymptomatic cases and ensures timely treatment. Therefore, it is essential for HIV prevention. This study aims to test the importance of two-site testing for better screening of these pathogens and to determine whether the presence of symptoms is an indicator of CT/NG infection. Methods This is a cross-sectional study carried out in four public infectious diseases clinics in São Paulo State, Brazil between January of 2022 and March of 2023. All participants had an anal swab and a first-pass or mid-stream urine collected for CT/NG analysis by Polymerase chain reaction (PCR). Data about sociodemographic, sexual behavioural and clinical aspects were collected. Pathway analysis was used to examine the direct and indirect relationships between variables according to the theoretical model. Results We screened 171 PrEP users which had two samples collected, resulting in 342 samples. Comparing the anatomic sites, the urine samples showed lower sensitivity for CT and NG than anal samples. Gonorrhoea was directly linked to lower age (β= -0.161, p = 0.001). Time of PrEP use was directly associated with CT infection (β = 0.202; p = 0.042) and inversely associated with dysuria (β= -0.121, p = 0.009). Lower occurrence of yellow-green secretion was linked to detection of CT (β= -0.089, p = 0.005) and NG (β= -0.048, p = 0.002) infections. Foul-smelling discharge was directly associated with CT (β = 0.275, p = 0.004) and NG (β = 0.295, p = 0.037) infection. Conclusion The symptoms are a bad indicator of CT and NG infection, and the screening must be done in more than one site since most of the positive results would be missed if only urines were tested. In the case of testing only one anatomical site, specifically the urethra, the CT/NG incidence and prevalence would be underestimated. The two-sites testing improves detection rates of CT/NG, and PrEP follow-up benefits people offering STI testing.

Background: Effective prevention programs to address the high prevalence of childhood overweight and obesity and the concomitant health consequences have been warranted. Objective: To improve dietary habits and weight status among Norwegian children with overweight/obesity in the primary care setting. Design: A 6-month non-randomized cluster-controlled intervention among 137 children, aged 6–12 years, with overweight/obesity and their parents. Intervention and control groups were recruited by public health nurses and followed-up by 12 Healthy Life Centers across Norway. The intervention group received individual family counseling and participated in nutrition courses and physical activity groups. A frequency questionnaire assessing sociodemographic characteristics and dietary habits was completed by the parents. Trained public health nurses measured height and weight using standardized methods to calculate body mass index (BMI) and BMI z-scores. Results: The intervention resulted in an increased odds of consuming evening meals (OR: 3.42), a decreased availability of salty snacks (β = –0.17), a decreased intake of salty snacks (−0.18), an increased consumption of water (β = 0.20), and a decreased estimated total intake of energy (β = –0.17), carbohydrates (β = –0.17), mono- and disaccharides (β = –0.21), sucrose (β = –0.24), and saturated fatty acids (β = –0.17). The intervention directly predicted lower BMI z-score (β = –0.17), and post-treatment levels of energy (β = –0.65), saturated fat (β = 0.43), and total carbohydrates (β = 0.41) were directly linked to BMI z-score after intervention. Age and sex were indirectly associated with BMI after intervention through energy and saturated fat intake. Conclusions: The intervention had a beneficial impact on nutrient intake and weight status among children with overweight/obesity. These findings provide support for implementing complex intervention programs tailored to local primary care settings. Trial registration: Clinicaltrials.gov, NCT02290171. Registered 13. Abstract Background Oral health needs assessment is important for oral health care planning. This study compared dental treatment needs between normative and sociodental needs. We also longitudinally examined the relationships of baseline sociodental needs measures and socioeconomic status with one-year follow up measures of use of dental services, dental caries, filled teeth, and oral health-related quality of life (OHRQoL). Methods A prospective study was conducted with 12-year-old adolescents from public schools in deprived communities in the city of Manaus, Brazil. Validated questionnaires were used to collect adolescents’ sex and socioeconomic status, OHRQoL (CPQ11 − 14) and behaviours (sugar intake, frequency of toothbrushing, regular use of fluoridated toothpaste and pattern of dental attendance). Normative need was assessed according to decayed teeth, clinical consequences of untreated dental caries, malocclusion, dental trauma, and dental calculus. The relationships between variables were tested thorough Structural equation modelling. Results Overall 95.5% of adolescents had normative dental treatment needs. Of these, 9.4% were classified as high level of propensity. Higher normative/impact need and greater propensity-related need directly predicted use of dental services at one-year follow up. The latter mediated the association of normative/impact need and propensity-related need with incidence of dental caries and filled teeth. Normative/impact need and use of dental services were directly associated with filled teeth at one-year follow up. Poor OHRQoL at one-year follow-up was directly predicted by higher normative/impact need at baseline and less filled teeth at one-year follow up. Greater socioeconomic status was directly associated with better propensity-related need. Socioeconomic status indirectly predicted incidence of dental caries and filled teeth via propensity-related need and use of dental services. Conclusions Sociodental needs measures were related to use of dental services, dental caries, filled teeth and OHRQoL after one year among adolescents living in deprived communities. Adolescents with dental needs treatment priorities according to the sociodental approach had more filled teeth via use of dental services. Dental services utilisation did not attenuate the impact of normative and impact-related need on dental caries incidence and poor OHRQoL after one year. Our findings suggest the importance of developing oral health promotion and enhancing access to dental care to improve oral health of adolescents living in deprived communities.

Objectives The study evaluated the association of academic environment, lifestyle, sense of coherence (SOC) and social support with self-reported mental health status among dental students.Design Secondary analysis of data from a cross-sectional, questionnaire-based survey conducted from August to October 2018.Setting Dental school of a public-funded university in the south-eastern region of Brazil.Participants 233 undergraduate dental students recruited across all years of the course.Outcome measures Socioeconomic and demographic characteristics, city of origin and student’s academic semester were obtained through self-completed questionnaires. Perception of the academic environment (Dundee Ready Education Environment Measure (DREEM)), individual lifestyle (Individual Lifestyle Profile Questionnaire (ILPQ)), SOC (SOC Scale (SOC-13)), social support (Medical Outcomes Study Scale (MOS)), and depression, anxiety and stress (Depression, Anxiety and Stress Scale-21 (DASS-21)) were assessed using validated instruments. The relationships between variables were investigated through multivariable negative binomial regression to obtain the rate ratios (RRs) and 95% CIs.Results Female sex was associated with greater scores of anxiety (RR 1.74, 95% CI 1.10 to 1.97) and stress (RR 1.52, 95% CI 1.12 to 2.06). Students who perceived a better academic environment and those reporting a greater SOC had a lower probability of depression, anxiety and stress. Furthermore, a favourable lifestyle was associated with lower depression scores (RR 0.99, 95% CI 0.97 to 0.99). Social support did not remain associated with depression, anxiety and stress after adjustment.Conclusions The present findings suggest that self-reported mental health status is associated with students’ sex, academic environment, SOC and lifestyle. Enhancing the educational environment and SOC, and promoting a healthy lifestyle may improve the psychological health of dental students.

Abstract Background There is increasing need for prospective investigations in the preventing role of health-related behaviours on mental health problems. The aim of this study is to identify patterns of health-related behaviours in adolescence, and the association between the behavioural patterns and the subsequent diagnoses and/or drug treatment for anxiety and/or depression in adulthood. Methods This prospective study consisted of 13–19-year-old participants in the Trøndelag Health Study (Young-HUNT3) in 2006–2008 (n = 2061, 1205 females and 856 males) in Norway, who also participated in HUNT4 (2017–2019). Survey data on health-related behaviours in adolescence, including low level of physical activity, low consumption of wholegrain bread, fish, fruit, vegetables and high consumption of sugar-sweetened beverages and insomnia were linked on an individual level to prospective information on drug use and diagnosis in national health registries. The different patterns of health-related behaviours were identified through latent class analysis. Subsequent anxiety or depression was defined as at least one recording in either of three registries covering recorded diagnosis in primary and specialist healthcare, or dispensed prescription drugs during 2008–2019. Additionally, self-reported psychological distress measured in young adulthood was applied as a supplemental outcome measure. Results Four patterns of health-related behaviours were identified: high risk behaviours (class 1), moderate to high risk behaviours (class 2), low to moderate risk behaviours (class 3) and low risk behaviours (class 4). Adolescents in class 3 showed higher odds of subsequent diagnoses for anxiety and/or depression in primary and specialist healthcare compared to class 4 participants. In addition, both class 1 and class 4 participants had higher odds for self-reported psychological distress than those class 4 (OR = 1.56 and OR = 1.86, respectively). Conclusions Our findings suggest that health-related behaviours are clustered among Norwegian adolescents. The patterns of unhealthy behaviours during adolescence only partly increased the risk of anxiety and depression in adulthood. Promoting healthy behaviours during adolescence may potentially reduce the burden of mental illness in adulthood, but further research is needed to clarify the nature of the relationships.

Objective To examine the relationships between sociodemographic characteristics, student academic information, social support, sense of coherence, anxiety, lifestyle, and quality of life among dental students.Methods: A cross-sectional study among 233 dental students in Brazil. We captured data on sociodemographic and socioeconomic characteristics, social support through Social Support Appraisal, psychosocial factors (sense of coherence and anxiety based on SOC-13 and Depression, Anxiety and Stress Scale-21 – anxiety subscale, respectively), lifestyle as per individual Lifestyle Profile and quality of life based on VERAS-Q. Data was analysed through Structural Equation Modelling.Results: Greater social support, higher sense of coherence, lower anxiety, better lifestyle directly predicted better quality of life. Male gender, city of origin did not differ from the city of the campus, higher social support and greater sense of coherence were indirectly linked with better quality of life via better lifestyle. Lower academic semester and higher social support indirectly predicted better quality of life via lower anxiety.Conclusion: Social support, sense of coherence, anxiety, and lifestyle were relevant factors directly associated with dental student’s quality of life. Indirect pathways were observed between gender, moved home to attend dental course, academic semester, social support, sense of coherence, and quality of life.

Abstract Background To examine the role of dental pain, sense of coherence (SOC) and social support on the relationship between dental caries and oral health-related quality of life (OHRQoL) in children aged 12 years. Methods A cross-sectional study involving 400 schoolchildren selected from public schools in a socioeconomically disadvantaged region in the city of Manaus, Brazil was carried out. The predictors of OHRQoL were selected according to the Wilson and Cleary theoretical model, including number of decayed teeth and its clinical consequence (component D of the DMFT index and PUFA/pufa index), dental pain (symptom status), and SOC and social support (individual and environmental characteristics). Statistical analysis was conducted through structural equation modelling and multivariable negative binomial regression. The significance level established for all analyses was 5%. Results Number of dental caries was indirectly linked with OHRQoL (β = 0.19, 95% CI 0.11/0.29) through dental pain, SOC and social support. Clinical consequences of untreated caries directly predicted poor OHRQoL (β = 0.12, 95% CI 0.01/0.23). Dental pain, SOC and social support did not moderate the effect of dental caries measures on OHRQoL. Conclusion Our findings suggest the role of dental pain, SOC and social support as mediator factors on the link between dental caries and OHRQoL. Tackling dental caries along with psychosocial factors may attenuated the impact of oral health on OHRQoL in children.

Abstract Background To estimate the prevalence of malocclusion in individuals with autism spectrum disorders (ASD) and to assess the relationship between ASD and malocclusion. Methods We searched electronic databases including PubMed, Scopus, Web of Science, Cochrane, Embase, SciELO LILACS, Proquest, OpenGrey and Google Scholar. There were no language or publication dates restrictions. Two researchers independently performed selection, data extraction and quality assessment. Quality assessment and risk of bias were evaluated through the Newcastle–Ottawa scale and ROBINS-E tool. Meta-analyses using random effect models were used to estimate pooled measures of prevalence of malocclusion characteristics in individuals with ASD and pooled odds ratio (OR) on the relationship between ASD and malocclusion characteristics. Subgroup meta-analyses were conducted according to children and adolescents, history of orthodontic treatment, and occurrence of other syndromes and medical conditions. Results Searching identified 5549 papers with 238 were selected for full assessment. Eighteen cross-sectional studies were included according to inclusion criteria. Of them, eleven studies were considered of moderate quality. A judgement of critical risk of bias occurred for thirteen studies. The most prevalent malocclusion characteristics in individuals with ASD were crowding (33%; 95% CI 22 to 44%) and increased maxillary overjet (39%; 95% CI 23 to 54%). Individuals with ASD had higher odds of Angle’s Class II (OR 1.92; 95% CI 1.36 to 2.72), Angle’s Class III (OR 2.33; 95% CI 1.29 to 4.23), open bite (OR 1.96; 95% CI 1.21 to 3.16), and increased maxillary overjet (OR 1.53; 95% CI 1.06 to 2.21) than individuals without ASD. Conclusions Angle’s Class II, Angle’s Class III, anterior open bite and increased maxillary overjet were more prevalent in individuals with ASD than those without ASD. Further high-quality studies are needed.

BackgroundINPLASY® is an international platform for registering systematic reviews and meta-analysis protocols that was launched in March 2020. INPLASY® provides an online database in which the protocols are maintained as permanent public records and can be accessed on its website (www.inplasy.com).MethodsWe described the database features and registered information of all records published since the launch of the registry on March 31, 2023. Additionally, we analyzed the website statistics dataset to explore user experience and promote data transparency.ResultsFour thousand six hundred fifty-eight records were registered in INPLASY®, and more than 94% of the protocols were published within 24 h. Most of the submissions were from China, followed by Portugal, Taiwan, Malaysia, and Brazil. The INPLASY® website received 386,395 page views from 64,568 visitors during the first three years. The accesses were obtained from 170 countries. Most of the accesses were from China, followed by the US, the UK, and Portugal. The review status “completed and published” was observed in 898 protocols, and these studies were published in 372 different scientific peer-reviewed journals. The features of INPLASY® include the following: (i) INPLASY® identifier, a unique protocol number; (ii) the digital object identifier (DOI) number, the URL of the protocol linked to a specific DOI; (iii) ORCID update, INPLASY® automatically updates authors' ORCID page, including their protocol; and (iv) search tools, the protocols are freely accessible on www.inplasy.com.ConclusionsINPLASY® has several practical and useful features that should be considered when planning the registration of a systematic review protocol. Furthermore, the sharp increase in the number of protocols registered in INPLASY® in the first three years and the database statistics demonstrate that INPLASY® has become an important source of systematic review protocols. Therefore, authors should access INPLASY® before planning a future review study to avoid unintended duplication of efforts and to obtain timely registration.

Abstract Background Social factors are important determinants of health. However, evidence from longitudinal studies on the possible role of changes in socioeconomic circumstances on adult’s oral health is scarce. This study aimed to test whether changes in income and changes in social networks of family members and friends were associated with trajectories of self-rated oral health (SROH) among adults over a 13-year period. Methods A prospective cohort study (Pro-Saude Study) was conducted involving non-faculty civil servants at university campi in Rio de Janeiro, Brazil. Individual data was collected through self-completed questionnaires in four waves (1999, 2001, 2007 and 2012). SROH trajectories between 2001 and 2012 were “Good-stable SROH”, “Changed SROH”, “Poor-stable SROH”. Per capita family income and social networks of family members and friends data obtained in 1999 and 2012 were grouped into “High stable”, “Increase”, “Decrease”, “Low stable”. Ordinal logistic regression using complete data of 2118 participants was used to estimate odds ratio (OR) and 95% CIs of changes in income and changes in social networks with SROH trajectories, adjusted for age, sex, skin colour and marital status. Results Participants in the low income-stable and small social networks-stable groups showed 2.44 (95% CI 1.68–3.55) and 1.98 (95% CI 1.38–2.85) higher odds for worst trajectory of SRHO than those in the respective high-stable groups. Those in the decrease income group and decrease social networks group were 78% (95% CI 1.25–2.54) and 58% (95% CI 1.07–2.34) more likely to worst trajectory of SRHO than those in the high income-stable and high social networks-stable groups. Conclusions Adults reporting low income and low social networks of family members and friends over 13 years and those with income and social networks decrease during the study period were at higher risk of having worsened their self-rated oral health.

Abstract Background This study aimed to evaluate the moderating effect of sense of coherence (SOC) on the relationship between social capital and oral health-related quality of life (OHRQoL) among schoolchildren. Methods A cohort study was conducted in the city of Santa Maria, Brazil, involving children aged 1–5 years at baseline who were reassessed after 10 years in adolescence (11–15 years-old). Social capital was assessed at baseline and follow-up through social networks and social trust. Sense of coherence scale (SOC-13) and the short form of the Child Perceptions Questionnaire 11–14 (CPQ11–14) were measured at 10-years follow-up. Demographic and socioeconomic characteristics, and dental caries were also evaluated. Moderating effect of SOC on the relationship between social capital and OHRQoL was tested using multilevel adjusted Poisson regression analysis and simple slope test. Results From the 639 subjects assessed at baseline, 429 were reassessed at follow-up (cohort retention rate 67.1%). Moderate and high levels of SOC demonstrated a moderating effect on the relationship between social capital and OHRQoL. Among individuals who presented low social capital at baseline and follow-up, those who had high SOC reported, respectively, an impact 63% and 70% lower on OHRQoL when compared to those with low SOC. Abstract Background To investigate the influence of change on sense of coherence (SOC) on dental services use in adolescents over a two-year period. Methods A prospective follow-up study was conducted involving 334 12-year-old adolescents from public schools in the city of Manaus, Amazonas, Brazil. The predictors of use of dental services in the last 12 months were selected according to the Andersen’s behavioural theoretical model. The predisposing factors included sex, self-reported skin colour and SOC. The enabling factors were dental insurance, monthly family income and parents/guardians schooling. Dental pain, perceived oral health status, dental caries and gingival status were used to assess need factors. Multivariable Poisson regression with robust variance was used to estimate incidence-rate ratios (IRR) and 95% confidence intervals between the independent variables and use of dental services. Results Adolescent’s SOC scores decreased significantly between baseline and one-year follow-up. SOC decline decreased the likelihood of using dental services in the last 12 months (IRR = 0.96 95%CI 0.92–0.99). Dental caries (IRR = 1.03 95%CI 1.01–1.04) and gingival bleeding (IRR = 1.01 95%CI 1.01–1.02) remained associated with use of dental services in the last 12 months. Adolescents with dental pain were more likely to have visited a dentist in the last year (IRR = 1.03, 95%CI 1.01–1.06). Conclusion SOC decrease over one-year period was a meaningful factor of dental services use among 12-year-old adolescents. Dental pain and clinical conditions were also relevant factors that can influence use of dental services in this group.

Social capital has been widely inserted in health discussions in recent decades. In this sense, social capital has become a popular term and has been highlighted as one of the main determinants of health in the conceptual framework of the social determinants of the World Health Organization. The concept of social capital focuses on the positive consequences of sociability and places these consequences in the broader discussion of capital. In this sense, social capital reflects the benefits that individuals and communities derive from having broad social networks or high levels of social trust. Despite controversies regarding its definition and numerous criticisms, a growing body of evidence suggests that high levels of social capital benefit oral health. This factor has also been recognized as a potential softener of the impact of oral conditions on oral health, through behavioural and psychosocial processes. Thus, efforts to reduce inequities in oral health preferably should be based on their origins and on their complex causal process, such as the social determinants. The future challenges in the area are specially related to the development of interventions and health promotion actions that aim to stimulate social capital, aiming to reduce the impact of social inequalities on oral health throughout the life course.

ABSTRACT OBJECTIVE To investigate the influence of protective psychosocial factors on the incidence of dental pain in the last six months among 12-year-old children living in Manaus (AM). METHODS A prospective school-based cohort study was conducted with 210 12-year-old students enrolled in public schools in the eastern zone of Manaus (AM). Students were followed up for two years. Validated questionnaires were used to assess sociodemographic characteristics, protective psychosocial factors, including sense of coherence, social support, and self-esteem at baseline and after two years. Calibrated examiners clinically assessed dental caries and gingival bleeding. Multivariate multilevel Poisson regression was used to estimate the relative risk (RR) and 95% confidence interval (95%CI) between the changes on psychosocial factors scores and incidence of dental pain, adjusted for psychosocial factors scores at baseline, dental health insurance, frequency of tooth brushing, and dental caries. RESULTS Mean scores for sense of coherence and social support reduced significantly from baseline to 2-year follow-up. The incidence of dental pain along the two-year follow-up was 28.6%. The risk of dental pain was 14% higher for every 10 points in the mean reduction of sense of coherence score (RR = 1.14; 95%CI: 1.02–1.20), and 6% higher for every 10 points of the mean reduction in social support score (RR = 1.06; 95%CI: 1.01–1.11). Change on self-esteem was not associated with risk of dental pain. CONCLUSION Change on sense of coherence and social support over the two-year period influenced the incidence of dental pain among children, suggesting that protective psychosocial factors, health behaviours, dental health insurance, and clinical oral condition have an important role in the incidence of dental pain.

Abstract Background Health-related quality of life (HQoL) indicators are considered valid measures of patient assessment in physical, mental and oral healthcare. This study aimed to examine the evidence on the relationship of oral health status, demographic and socioeconomic characteristics with oral health-related quality of life (OHRQoL) in children. Methods Studies in English published up to December 2019 were searched on PsycINFO, PubMed, SciELO, Scopus, and Web of Science databases. Epidemiological studies simultaneously assessing sociodemographic factors related to oral health (age, income, gender, maternal education), oral health measures (orthodontic treatment needs, dental caries and periodontal disease) and OHRQoL in children aged 3–12 years were included. Methodological quality was assessed using a Critical Appraisal Checklist. Meta-analysis was used to estimate pooled measures between sociodemographic factors and oral health measures with OHRQoL. Results Eleven articles were included. Lower children’s age (3–5 years vs > 5), gender (girls vs boys), lower income ( 6 years) were associated with poor OHRQoL among children. Orthodontic treatment needs, dental caries and periodontal diseases were also associated with poor children’s OHRQoL. Meta-regression showed that Human Development Index, sample size, year of publication and participant’s age were relevant aspects that influenced the above mentioned relationships. Conclusions Our findings suggest that oral health promotion strategies to improve children’s OHRQoL should consider the social and environmental where they live as well their oral health status. Further longitudinal studies are needed to explore the determinants of OHQoL in children.

Abstract Background The utilisation of health services is determined by complex interactions. In this context, rural populations face greater barriers in accessing dental services than do urban populations, and they generally have poorer oral health status. The evaluation of the determinants of health services utilisation is important to support planning and management of dental services. The aim of this study was to evaluate the predictors of dental services utilisation of Brazilian adults living in rural and urban areas. Methods Data from 60,202 adults aged 18 years or older who took part in the Brazilian National Health Survey carried out in 2013 were analysed. Predisposing (age, sex, education, social networks), enabling financing (income, durable goods and household’s crowding), enabling organisation (health insurance, registration in primary health care [PHC]) and need variables (eating difficulties, self-perceived tooth loss and self-perceived oral health) were selected based upon the Andersen behavioural model. Multi-group structural equation modeling assessed the direct and indirect associations of independent variables with non-utilisation of dental services and the interval since the last dental visit for individuals living in rural and urban areas. Results Adults living in urban areas were more likely to use dental services than those living in rural areas. Lower enabling financing, lower perceived dental needs and lack of PHC registration were directly associated with lower utilisation of dental services (non-utilisation, β = − 0.36, β = − 0.16, β = − 0.03, respectively; and interval since last dental visit, β = 1.25, β = 0.82, β = − 0.12, respectively). The enabling financing (non-utilisation, βrural = − 0.02 [95%CI: − 0.03 to − 0.02], βurban = 0.00 [95%CI: − 0.01 to 0.00]) and PHC registration (non-utilisation, βrural = − 0.03 [95%CI: − 0.04 to − 0.02], βurban = − 0.01 [95%CI, − 0.01 to − 0.01]) non-standardised total effects were stronger in rural areas. Enabling organisation (β = 0.16) and social network (β = − 2.59) latent variables showed a direct effect on the interval since last dental visit in urban areas. Education and social networks influenced utilisation of dental services through different pathways. Males showed less use of dental services in both urban and rural areas (non-utilisation, βrural = − 0.07, βurban = − 0.04; interval since last dental visit, βrural = − 0.07, βurban = − 0.07) and older adults have used dental services longer than younger ones, mainly in rural areas (βrural = 0.26, βurban = 0.17). Conclusion Dental services utilisation was lower in rural areas in Brazil. The theoretical model was supported by empirical data and showed different relationships between the predictors in the two geographical contexts. In rural areas, financial aspects, education, primary care availability, sex and age were relevant factors for the utilisation of services.

Abstract Poor oral health has been associated with frailty among older adults. However, limited evidence has been available on whether frailty can affect oral health-related quality of life (OHRQoL) in older adults. This study aimed to investigate the relationship between frailty and OHRQoL among community-dwelling older adults. A household-based cross-sectional study involving community-dwelling older adults aged 65 years and older was conducted in the city of Bauru, Brazil. Data on frailty status, sociodemographic characteristics, self-perceived dental care needs, and OHRQoL (OHIP-14) were collected through individual interviews. The use of and the need for total dental prostheses were assessed through clinical examinations. Logistic regression was used to determine whether frailty status and covariates were associated with OHRQoL prevalence measures (OHIP-14 total score ≥ 1 and OHIP-14 fairly/very often ≥ 1). The sample comprised 334 participants, among whom 58.7% and 41.3% were between 65–74 and 75–102 years old, respectively. The prevalence of moderate to severe frailty was 12.3%. Moderate to severe frailty (OR = 4.49; 95%CI 1.29–15.66), the need for lower dental prosthesis (OR = 2.20; 95%CI 1.27–3.81), and self-perceived dental care need (OR = 3.90; 95%CI 2.14–7.14) were associated with OHIP-14 total score ≥1. Moderate to severe frailty (OR = 2.95; 95%CI 1.33–6.55), being female (OR = 2.24; 95%CI 1.34–3.75), and self-perceived dental care need (OR = 4.80; 95%CI 2.86–8.03) were associated with OHIP-14 fairly/very often ≥1. Overall, our results showed that moderate to severe frailty was significantly associated with poor OHRQoL in community-dwelling older adults.

Abstract Periodontal diseases are infectious inflammatory chronic diseases of a multifactorial nature. They are primarily caused by dysbiotic ecological changes in dental biofilm, which may be influenced by risk factors. The prevention of periodontal diseases may involve different strategies focused on reducing distal, intermediate, and proximal risk factors at both the population and individual levels. Effective prevention depends on interdisciplinary and common risk factor approaches. Also, patient-centered preventive models are more effective than professional-centered models in the management of periodontal diseases. Regular and periodic control of dental biofilm is an essential measure for the different levels of prevention of periodontal diseases. The effectiveness of periodontal disease prevention largely depends on positive modifications of behavior, knowledge, health literacy, patient empowerment, motivation, and compliance.

Few studies have evaluated oral health promotion (OHP) programs in nursery schools. This research aimed to assess whether there were differences in oral health knowledge between preschoolers and carers who received or did not receive an OHP program in nursery schools. A post-intervention observational study involving 5-6 years-old preschoolers and 169 carers was carried out in 10 nursery schools. The preschoolers in the OHP program group (N=93) received information on oral health and recreational activities relating to oral health, whereas in the control group (N=90), no OHP activity was provided. Children's age and sex, carer's age, child kinship, education and family income were collected via questionnaire. Children and carer's oral health knowledge scores were assessed through structured interviews. Independent variables were compared between groups by t-test, Chi-square and Mann-Whitney test. The association between children and carer's oral health knowledge and the OHP program group was tested using multivariate Poisson regression. Children's oral health knowledge was significantly associated with the OHP program group. Carer's oral health knowledge did not differ between intervention and control groups. Children attending nursery schools provided with OHP programs had greater oral health knowledge than those attending nursery schools without OHP activities.

This community study evaluates complex interactions between macro and micro determinants of oral health in a local Indigenous population based on a theoretical framework of demographic, economic, and healthcare transformation over the last half century. The study population included all residents of eight Xavante villages in Central Brazil. Our hypothetical model posited multiple direct and indirect associations between dental caries and village groups with differentiated territorial and oral care histories, as well as household socioeconomic indicators and food acquisition patterns, individual sociodemographic characteristics, use of dental health services, and oral hygiene practices. Structural equation modelling methods were used to evaluate direct and indirect associations linking exogenous factors and dental caries. Results include 18 direct and 14 indirect statistically significant pathways between determinant variables and dental caries. Significant links with dental caries were shown for socioeconomic indicators, oral healthcare variables, household food acquisition patterns, sex, and age. These findings suggest that the oral health of Xavante residents in the villages studied is associated with determinant factors of different epidemiological and historical scales. The specific historical frame of territorial circumscription and demographic crisis followed by rapid population increase since the 1970s should be considered a cause-of-cause determinant of the economic, healthcare, and sociodemographic profile contributing to oral health among the Xavante. Considering the limitations of cross-sectional studies, our findings underline the importance for oral health determination of historical currents affecting minority ethnic groups within national societies.

Inequalities in the utilisation of dental services in Brazil are remarkable. The aim of this study was to evaluate the association of contextual and individual factors with the utilisation of dental services by Brazilian adults using the Andersen's behavioural model.Individual-level data from 27,017 adults residents in the State capitals who were interviewed in the 2013 Brazilian National Health Survey were pooled with contextual city-level data. The outcomes were non-utilisation of dental services and last dental visit over 12 months ago. Individual predisposing variables were age, sex, race/skin colour, schooling and social network. Individual enabling variables included income, health insurance and registration in primary health care. Individual need variables were self-perceived dental health and self-reported missing teeth. Multilevel logistic regression models were performed to estimate odds ratio (OR) and 95% confidence intervals (95% CIs) of the association of contextual and individual predisposing, enabling and need-related variables with dental services outcomes.Predisposing (OR = 0.89; 95% CI 0.81-0.97) and enabling (OR = 0.90; 95% CI 0.85-0.96) contextual factors were associated with non-utilisation of dental services. Individual predisposing (sex, race/skin colour, schooling), enabling (income, health insurance) and need (self-perceived oral health, missing teeth) were associated with non-utilisation of dental services and last dental visit over 12 months ago. The latter was also associated with other individual predisposing (age, social network) and need (eating difficulties due to oral problems) characteristics.Individual and contextual determinants influenced dental services utilisation in Brazilian adults. These factors should be on the policy agenda and considered in the organisation of health services aiming to reduce oral health inequalities related to access and utilisation of dental services.

OBJECTIVE: To investigate the relationship of contextual social capital (neighborhood empowerment) and individual social capital (social support and social network) with dental caries experience in adolescents and adults. METHODS: A population-based multilevel study was conducted involving 573 subjects, 15-19 and 35-44 years of age, from 30 census tracts in three cities of Paraíba, Brazil. A two-stage cluster sampling was used considering census tracts and households as sampling units. Caries experience was assessed using the DMFT index (decayed, missing and filled teeth) and participants were divided into two groups according to the median of the DMFT index in low and high caries experience. Demographic, socioeconomic, behaviors, use of dental services and social capital measures were collected through interviews. Neighborhood empowerment was obtained from the mean scores of the residents in each census tract. Multilevel multivariate logistic regression was used to test the relationship between neighborhood empowerment and caries experience. RESULTS: High caries experience was inversely associated with neighborhood empowerment (OR = 0.58; 95%CI 0.33 - 0.99). Individual social capital was not associated with caries experience. Other associated factors with caries experience were age (OR = 1.15; 95%CI 1.12 - 1.18) and being a female (OR = 1.72; 95%CI 1.08 - 2.73). CONCLUSION: The association between neighborhood empowerment and caries experience suggests that the perception of features of the place of residence should be taken into account in actions of oral health promotion.

OBJETIVO: Estimar a prevalência e a distribuição geográfica da doença periodontal na população adulta brasileira e sua associação com desigualdades sociais contextuais e individuais. MÉTODOS: Foram utilizados dados dos adultos de 35 a 44 anos de idade da Pesquisa Nacional de Saúde Bucal – SBBrasil 2010 (n = 9.564). O Índice Periodontal Comunitário (CPI) e o Índice de Perda Periodontal (PIP) foram usados para definir a doença periodontal em “moderada a grave” (CPI > 2 e PIP > 0) e “grave” (CPI > 2 e PIP > 1). As desigualdades sociais contextuais incluíram o índice de desenvolvimento humano e a desigualdade de renda (Índice de Gini). Outras variáveis contextuais foram a cobertura de equipes de saúde bucal da Estratégia de Saúde da Família e o percentual de adultos fumantes. Modelos de regressão logística multinível para os participantes com dados completos (n = 4.594) foram usados para estimar odds ratios (OR) e intervalos de 95% de confiança (IC95%) entre desigualdades sociais e doença periodontal. RESULTADOS: A prevalência da doença periodontal “moderada a grave” em brasileiros adultos foi de 15,3% e 5,8% para a condição “grave”, com variações consideráveis entre os municípios. Dentre as variáveis contextuais, a desigualdade de renda foi independentemente associada com a doença periodontal “grave” (OR = 3,0; IC95% 1,5;5,9). A menor cobertura de equipes de saúde bucal foi associada com as duas formas de doença periodontal, enquanto o percentual de fumantes manteve-se associado com a doença periodontal “moderada a grave”. Adultos com idade mais avançada, de cor de pele parda, sexo masculino, menor renda familiar e menor escolaridade apresentaram maiores chances para ambas as condições periodontais investigadas. CONCLUSÕES: No Brasil, a prevalência da doença periodontal variou conforme o município e a definição de doença empregada. A desigualdade de renda teve um papel importante na ocorrência da doença periodontal “grave”. Características individuais de posição social foram associadas com as duas formas de doença periodontal.

OBJETIVO: Estimar a prevalência e identificar fatores sociodemográficos e parâmetros bucais associados ao impacto negativo da condição bucal na qualidade de vida de adolescentes. MÉTODOS: Foram analisados dados de 5.445 adolescentes entre 15 e 19 anos que participaram do inquérito nacional de saúde bucal (SBBrasil 2010), considerando a complexidade do desenho amostral. O desfecho foi a qualidade de vida relacionada à saúde bucal, avaliada por meio do questionário Oral Impacts on Daily Performance e analisada de forma discreta. As variáveis de exposição foram sexo, cor da pele, escolaridade, renda familiar, idade, cárie não tratada, perda dentária, dor de dente, oclusopatias, sangramento gengival, cálculo dentário e bolsa periodontal. Foram conduzidas análises de regressão de Poisson e apresentadas as razões de médias (RM), com respectivos intervalos de 95% de confiança (IC95%). RESULTADOS: Dos pesquisados, 39,4% relataram pelo menos um impacto negativo na qualidade de vida. Após o ajuste, a média do impacto negativo foi de 1,52 (IC95%1,16;2,00) vez maior no sexo feminino e 1,42 (IC95% 1,01;1,99), 2,66 (IC95% 1,40;5,07) e 3,32 (IC95% 1,68;6,56) vezes maior nos pardos, amarelos e indígenas, respectivamente, em relação aos brancos. Quanto menor a escolaridade, maior a média de impacto negativo (RM 2,11, IC95% 1,30;3,41), assim como em indivíduos com renda familiar até R$ 500,00 (RM 1,84, IC95% 1,06;3,17) comparados aos de maior renda. Encontrou-se maior impacto na qualidade de vida entre adolescentes com quatro ou mais lesões de cáries não tratadas (RM 1,53, IC95% 1,12;2,10), uma ou mais perdas dentárias (RM 1,44, IC95%1,16;1,80), com dor de dente (RM 3,62, IC95% 2,93;4,46) e com oclusopatia grave (RM 1,52, IC95% 1,04;2,23) e muito grave (RM 1,32, IC95% 1,01;1,72). CONCLUSÕES: Os adolescentes brasileiros relataram alto impacto negativo da saúde bucal na sua qualidade de vida. A iniquidade em sua distribuição deve ser considerada ao planejar medidas de prevenção, monitoramento e tratamento dos agravos bucais nos grupos com maior impacto na qualidade de vida.

This ecological study investigated the association between social and environmental inequities and dental caries among indigenous people in Brazil. Dental caries data were gathered from articles identified from electronic databases for the period between 2000 and 2007. Independent variables were obtained from the census of Health Information System for Sanitation Indigenous Populations. Multiple linear regression analysis was conducted to test the association between social and environmental characteristics and dental caries (DMFT index) according to the age group. Results were analyzed for 48 indigenous peoples from 19 selected studies. The occurrence of dental caries in particular age groups was inversely associated with the location of villages outside the Amazon region (12, 15 - 19, and 20 - 34 years), availability of electricity (15 - 19 and 20 - 34 years) and proportion of households covered with straw/thatch (20 - 34 years). The presence of schools was statistically associated with higher DMFT averages (15 - 19 and 20 - 34 years). It can be concluded that aspects of location and existing infrastructure in indigenous communities, which are linked to the availability of oral health services, are associated with the occurrence of dental caries in indigenous populations in Brazil.

OBJETIVOS: avaliar adequação, acompanhamento pré-natal, satisfação e riscos gestacionais das gestantes com história de prematuridade. MÉTODOS: estudo seccional com 1239 gestantes, 259 com história de prematuridade no Sistema Único de Saúde (SUS) do RJ em 2007/2008. Informações demográficas, socio-econômicas, história obstétrica, adequação pré-natal e satisfação foram coletadas através de entrevistas e do cartão pré-natal. Utilizou-se teste χ² para comparar o grupo com história de prematuridade com os grupos de baixo risco e sem história de prematuridade. RESULTADOS: não houve diferenças na adequação e no cuidado pré-natal entre as gestantes com história de prematuridade comparadas às de baixo risco gestacional. Houve insatisfação com tempo de espera pelas consultas, horário de funcionamento das unidades de saúde e explicações dos profissionais de saúde. CONCLUSÕES: a presença de história de prematuridade não influenciou para que essas gestantes tivessem um pré-natal de melhor qualidade. As unidades de saúde apresentaram problemas, segundo a percepção das gestantes, em relação ao modo de funcionamento. As explicações dos profissionais de saúde sobre risco da prematuridade não atenderam às expectativas das gestantes.

O objetivo deste estudo foi avaliar o perfil sociodemográfico de risco para infecção do trato urinário e para inadequação do pré-natal, segundo índice de Kotelchuck, e avaliar o manejo da infecção do trato urinário durante o pré-natal segundo o profissional de saúde, o serviço de saúde e a mulher, em gestantes no município do Rio de Janeiro. Um estudo seccional foi realizado com 1.091 gestantes, 501 com infecção do trato urinário, na rede do SUS do Rio de Janeiro em 2007/2008. Informações demográficas, socioeconômicas, história obstétrica e adequação do pré-natal foram coletadas através de entrevistas e do cartão do pré-natal. O manejo inadequado da infecção do trato urinário foi avaliado pelas dimensões: profissional de saúde, serviços de saúde e mulher. Utilizou-se o teste χ2 e regressão logística multivariada para comparação entre os grupos e identificação dos fatores associados ao manejo inadequado da infecção do trato urinário. As gestantes adolescentes, anêmicas, diabéticas e com qualidade do pré-natal parcialmente adequado ou inadequado apresentaram maior chance de infecção do trato urinário. Na avaliação global, 72% tiveram manejo inadequado da infecção do trato urinário. O manejo inadequado da infecção do trato urinário foi associado à cor parda em comparação com a cor branca. Na avaliação do profissional de saúde, o manejo inadequado para infecção do trato urinário foi menos comum nas gestantes com baixo peso e com sobrepeso e obesidade e, na avaliação da gestante, as primíparas tiveram menor chance de manejo inadequado para infecção do trato urinário em relação àquelas com um ou mais filhos.

Neste estudo, avaliaram-se a adequação e o acompanhamento pré-natal de gestantes com hipertensão arterial e as de baixo risco, além dos fatores associados ao manejo adequado do pré-natal nas hipertensas. Um estudo seccional foi realizado com 1.947 gestantes, 187 com hipertensão arterial, na rede SUS do Rio de Janeiro, Brasil, em 2007/2008. Informações demográficas, socioeconômicas, história obstétrica e adequação do pré-natal foram coletadas através de entrevistas e do cartão pré-natal. O manejo adequado da hipertensão arterial foi avaliado pelas dimensões: profissional de saúde, serviços de saúde e mulher. Utilizou-se o teste Χ ² e regressão logística multivariada para comparação entre os grupos e identificação dos fatores associados ao manejo da hipertensão arterial. A adequação do pré-natal foi de 79% e não diferiu entre os grupos. Apenas 27% das hipertensas tiveram manejo da hipertensão arterial adequado, com pior desempenho do profissional. Gestantes hipertensas com melhor assistência ao pré-natal foram aquelas com antecedentes de nati e/ou neomortalidade e aquelas com 35 ou mais anos de idade. Apesar da adequada utilização do pré-natal, o manejo da hipertensão arterial nas gestantes foi inadequado.The aim of this study was to assess the adequacy of prenatal care for pregnant women with hypertension as compared to those at low risk. Adequate management of hypertension and associated factors were also investigated. A cross-sectional study was conducted with 1,947 women, 187 of whom with hypertension, in public prenatal care units in Rio de Janeiro, Brazil, in 2007-2008. Demographic and socioeconomic data, obstetric history, and information on adequacy of prenatal care were collected using interviews and prenatal care cards. Adequacy of management of hypertension was evaluated according to performance of health professionals and health services and women's individual characteristics. Chi-square and multivariate logistic regression were used to compare groups and identify factors associated with management of hypertension. Adequacy of prenatal care was 79% and did not differ between groups. Only 27% of pregnant women with hypertension received appropriate management, with poor professional performance. Hypertensive pregnant women with better prenatal care were those with previous neonatal deaths and/or stillbirths and those with 35 years of age and older. Despite adequate use of prenatal care, management of hypertension in pregnant women was inadequate.

OBJETIVO: Caracterizar, segundo o sexo, as condições de saúde bucal em idosos residentes no município de Manaus, AM. MÉTODOS: Estudo seccional de base populacional com 667 indivíduos com idade entre 65-74 anos, aleatoriamente selecionados. As informações demográficas e socioeconômicas foram obtidas através de entrevista. O exame bucal para cárie, edentulismo, uso e necessidade de próteses foi conduzido de acordo com as normas da OMS. RESULTADOS: Participaram do estudo 206 homens e 461 mulheres, com idade média de 69,2 anos, e 71,8% se declararam de cor parda. Em média, os sujeitos apresentaram 4,6 anos de estudo e renda familiar de R$ 1586,27. O índice de dentes cariados, perdidos e obturados foi de 29,0 ± 4,3, com predomínio do componente perdido (95%). A média de dentes por indivíduo foi de 4,1 ± 5,7 e a prevalência de edentulismo foi de 52,2%, sendo que apenas 3% dos idosos apresentaram 20 dentes ou mais. O uso de prótese total superior e inferior foi observado em 79,2% e 37,1%, respectivamente, e a necessidade de prótese total foi de 42,6% para o arco superior e 34,7% para o inferior. Idosos do sexo masculino apresentaram menor índice de dentes cariados, perdidos e obturados, mais dentes e menos edentulismo em comparação com o sexo feminino. O uso de prótese total superior e inferior foi maior nas idosas, enquanto a necessidade de prótese unitária ou parcial superior e inferior foi maior nos idosos. CONCLUSÃO: As condições de saúde bucal dos idosos em Manaus caracterizaram-se pela elevada ocorrência de perda dentária, especialmente entre as mulheres. As taxas de uso e a necessidade de prótese total foram elevadas e diferentes segundo sexo.AIM: To characterize the oral health conditions of elderly people living in the city of Manaus, AM, according to sex. METHODS: A cross-sectional population-based study was conducted involving 667 subjects, 65-74 years of age, randomly selected. Demographic and socioeconomic data were obtained through interviews. Oral measures were performed for caries, edentulism, use and need of prosthesis, following WHO criteria. RESULTS: The study comprised 206 men and 461 women, mean age of 69.2 years, and 71.8% self-reported as brown skin color. On average they had 4.6 years of schooling and family income of $ 916.92. The DMF-T mean was 29.0+4.3 and the "missing" component was the most common (95%). The mean number of teeth per individual was 4.1±5.7 and the prevalence of edentulism was 52.2%. Only 3% of the elderly had 20 or more teeth. The use of upper and lower total prostheses was 79.2% and 37.1%, respectively. The need for total prosthesis was 42.6% for the upper arch and 34.7% for the lower arch. Elderly men presented lower DMF-T, more teeth and less edentulism compared to women. The use of upper and lower total prosthesis was higher in the elderly women and the need of upper and lower unitary and partial denture was higher in elderly men. CONCLUSION: Oral health conditions of the elderly in Manaus revealed a high occurrence of tooth loss, mainly among women. The use and need for total prosthesis were high and different according to sex.

OBJECTIVE: To assess the relationship between housing conditions and low birthweight and preterm low birthweight among low-income women. METHODS: A case-control study was conducted with post-partum women living in the city of Rio de Janeiro, Southeast Brazil, in 2003-2005. Two groups of cases, low birthweight (n=96) and preterm low birthweight infants (n=68), were compared against normal weight term controls (n=393). Housing conditions were categorized into three levels: adequate, inadequate, and highly inadequate. Covariates included sociodemographic and anthropometric characteristics, risk behaviors, violence, anxiety, satisfaction during pregnancy, obstetric history and prenatal care. RESULTS: Poor housing conditions was independently associated with low birthweight (inadequate - OR 2.3 [1.1;4.6]; highly inadequate - OR 7.6 [2.1;27.6]) and preterm low birthweight (inadequate - OR 2.2 [1.1;4.3]; highly inadequate - OR 7.6 [2.4;23.9]) and factors associated with outcomes were inadequate prenatal care and previous preterm birth. Low income and low maternal body mass index remained associated with low birthweight. CONCLUSIONS: Poor housing conditions were associated with low birthweight and preterm low birthweight.

Este estudo analisou o grau de adequação da gestão da Atenção Básica (AB) em saúde bucal no município de Fortaleza, Ceará, entre 1999 e 2006. A metodologia empregada teve como base uma matriz de análise de cinco dimensões: Planejamento e Programação, Suporte da Gestão aos Profissionais e às Práticas na AB, Oferta de Assistência Odontológica, Utilização de Serviços e Integralidade da Atenção. As informações foram obtidas de planos municipais de saúde, de relatórios de gestão e análise de dados secundários do Sistema de Informação Ambulatorial (SIA-SUS) e do IBGE. De maneira complementar, a partir da análise dos indicadores do Pacto da AB (2006), obtiveram-se informações com gestores de saúde bucal. Observou-se uma melhora nas quatro primeiras dimensões da matriz de análise, e apenas a dimensão relativa à utilização de serviços declinou no período. A tendência dos indicadores de saúde bucal foi heterogênea. Enquanto a "Cobertura de Primeira Consulta odontológica" reduziu, a "Proporção de procedimentos odontológicos especializados em relação às ações individuais da AB" apresentou discreto aumento. Concluiu-se que o município de Fortaleza apresentou adequação da gestão da AB em saúde bucal ao modelo de vigilância em saúde, entre 1999 e 2006. Entretanto, o declínio na utilização de serviços odontológicos associado à estabilidade dos indicadores de cobertura e assistência odontológica sugere a necessidade de sistematizar atividades de planejamento e avaliação das ações de saúde bucal na AB em Fortaleza.This study assessed the degree of adequacy of Primary Care management in oral health in the city of Fortaleza, state of Ceará, Brazil, between 1999 and 2006. The employed methodology was based on a matrix of analysis including five dimensions: Planning and Programming, Management Support to Primary Care Professionals and Practices, Dental Assistance Offered, Health Services Utilization and Integrality of Care. The information was obtained from municipal health plans, management reports and analysis of secondary data from Sistema de Informação Ambulatorial (SIA-SUS - Outpatient Clinic Information System of Brazil's National Health System) and from IBGE (Brazilian Institute of Geography and Statistics). In addition, health indicators from the Primary Health Pact (2006) and information from oral health managers were analyzed. An improvement was observed in the first four dimensions of the matrix of analysis and only the dimension related to health services utilization declined during the period of the study. The trend of the oral health indicators was heterogeneous. While the oral health indicator "Coverage of the First Dental Consultation" decreased, the "Proportion of Specialized Dental Procedures in Relation to Individual Actions in Primary Care" showed a slight increase. It was concluded that the Primary Care management in oral health in the city of Fortaleza became adequate to the health surveillance model between 1999 and 2006. However, the decline in dental health services utilization, associated with the stability of the indicators of dental coverage and assistance, suggest the need to systematize the activities of planning and assessment of oral health actions in Primary Care in Fortaleza.

The objective of the present study was to compare the reliability of four partial-mouth protocols for assessing shallow, moderate, and deep sites for periodontal pocket depth and clinical attachment levels. Periodontal pocket depth and clinical attachment level measurements were recorded for 156 subjects (age > 30). The four models of partial-mouth protocols compared were: Model I: all sites per tooth in the random half-mouth protocol randomly selecting one maxillary and mandibular quadrant, Model II: buccal sites in a full-mouth protocol, Model III: buccal sites in the random half-mouth protocol randomly selecting one maxillary and mandibular quadrant, Model IV: all sites per tooth using Community Periodontal Index teeth. In comparison with full mouth examination, Model I did not show significant differences for periodontal pocket depth and clinical attachment level parameters. Models II and III were different for some periodontal pocket depth means, and Model IV significantly overestimated all clinical parameters related to periodontal disease. Model I appears to be adequate to substitute for the full-mouth examination to assess the prevalence and severity of chronic periodontal disease in adults.O objetivo deste estudo foi comparar a confiabilidade para medidas periodontais de profundidade de bolsa à sondagem e nível clínico de inserção de quatro protocolos de registros parciais. Exames periodontais completos foram realizados em 156 indivíduos com idade > 30 anos. Quatro protocolos foram comparados ao exame periodontal completo: Modelo I: protocolo de dois quadrantes aleatórios, um superior e um inferior (seis sítios por dente), Modelo II: protocolo de quatro quadrantes (três sítios vestibulares por dente), Modelo III: protocolo de dois quadrantes aleatórios, um superior e um inferior (três sítios vestibulares por dente), Modelo IV: protocolo de seis sítios por dente nos dentes índices do Índice Periodontal Comunitário. Ao comparar com o exame periodontal completo, o Modelo I não apresentou diferenças para nenhum dos parâmetros periodontais, os Modelos II e III foram diferentes em algumas médias de profundidade de bolsa à sondagem. O Modelo IV superestimou todos os parâmetros clínicos associados à doença periodontal. O Modelo I pareceu ser apropriado para substituir o exame periodontal completo para caracterização da prevalência e severidade da periodontite crônica em populações adultas.

Investigou-se a associação entre comportamentos relacionados à saúde bucal e geral e condição socioeconômica, e a relação entre comportamentos relacionados à saúde e escovação dentária em adolescentes. A base de dados utilizada foi da Pesquisa Nacional de Saúde do Escolar (PeNSE), um estudo transversal de base populacional realizado em 2009 com alunos das 27 capitais brasileiras. Informações sociodemográficas e comportamentos relacionados à saúde foram coletados. O estudo envolveu 49.189 adolescentes (47,5% do sexo masculino), sendo a maioria com 14 anos de idade e de escola pública. A frequência de escovação dentária assim como outros comportamentos relacionados à saúde se associou com a condição socioeconômica de maneira distinta entre sexos. Foram observadas associações entre hábitos relacionados à saúde e a frequência de escovação dentária em ambos os sexos, porém com variações conforme a condição socioeconômica. O planejamento de intervenções para promoção da saúde de adolescentes deve levar em conta suas características individuais, o contexto familiar e social.This study investigated the association between oral and general health-related behaviors and socioeconomic status, and the relationship between health-related behaviors and toothbrushing among adolescents. The database used here was the National School-Based Health Survey (PeNSE), a cross-sectional population-based study in 2009 with students from 27 Brazilian State capitals. Socio-demographic and health-related behavior data were collected. The survey included 49,189 adolescents (47.5% males), the majority of whom were 14 years of age and enrolled in public schools. The associations between toothbrushing frequency and other health-related behaviors and socioeconomic status varied between boys and girls. Associations were observed between health-related habits and toothbrushing frequency in both sexes, but with variations according to socioeconomic status. Planning health promotion interventions for adolescents should take their individual characteristics and family and social context into account.

O Projeto SBBrasil 2010 (SBB10) foi concebido como um levantamento epidemiológico em saúde bucal, de base nacional, dentro da estratégia de vigilância em saúde. O objetivo deste artigo é apresentar aspectos da metodologia do SBB10 que possam contribuir para ampliar e desenvolver conhecimentos na área de saúde. Com relação ao plano amostral, trata-se de uma pesquisa por conglomerados e com múltiplos estágios. Capitais e municípios do interior das cinco regiões brasileiras compõem os domínios da amostra, cujas unidades amostrais foram, respectivamente, setor censitário e domicílio para as capitais, e município, setor censitário e domicílio para o interior. Nas capitais foram sorteados 30 setores e, no interior de cada região, 30 municípios. A precisão considerou os domínios agrupados segundo o grau de densidade no total da população e a variabilidade interna dos índices. Foram avaliadas as condições de cárie dentária, doença periodontal, oclusopatias, fluorose, traumatismo dentário e edentulismo em cinco grupos etários (5, 12, 15 a 19, 35 a 44 e 65 a 74 anos).The SBBrasil 2010 Project (SBB10) was designed as a nationwide oral health epidemiological survey within a health surveillance strategy. This article discusses methodological aspects of the SBB10 Project that can potentially help expand and develop knowledge in the health field. This was a nationwide survey with stratified multi-stage cluster sampling. The sample domains were 27 State capitals and 150 rural municipalities (counties) from the country's five major geographic regions. The sampling units were census tracts and households for the State capitals and municipalities, census tracts, and households for the rural areas. Thirty census tracts were selected in the State capitals and 30 municipalities in the countryside. The precision considered the demographic domains grouped by density of the overall population and the internal variability of oral health indices. The study evaluated dental caries, periodontal disease, malocclusion, fluorosis, tooth loss, and dental trauma in five age groups (5, 12, 15-19, 35-44, and 65-74 years).

This study investigated the relationship between social capital and social support and the adequate use of prenatal care. A follow-up study involving 1,485 pregnant women was conducted in two cities in the Rio de Janeiro State, Brazil. Demographic and socioeconomic characteristics, social support and social capital data were collected during the first trimester of pregnancy. The post-partum period included information on levels of prenatal care utilization, social networks, parity, obstetric and gestational risk and prenatal care attendance. Hierarchized multinomial logistic regression was used in the statistical analysis. Prenatal care use above adequate levels was associated with high social capital at the city level (aggregated social capital), socioeconomic status and working during pregnancy. Lower non-aggregated contextual and compositional social capital, gestational risk and pattern of prenatal care were associated with inadequate prenatal care utilization. Contextual social capital and social support were found to be social determinants for the appropriate use of prenatal care.O objetivo deste estudo foi investigar a associação entre capital social e apoio social com a adequação da atenção pré-natal. Um estudo de seguimento que envolveu 1.485 gestantes foi realizado em duas cidades do Estado do Rio de Janeiro, Brasil. Características demográficas e socioeconômicas e dados de capital social e apoio social foram coletados no 1º trimestre da gestação. No pós-parto, registraram-se informações sobre adequação do pré-natal, redes sociais, risco obstétrico e gestacional e padrão de atendimento pré-natal. Regressão logística hierarquizada multinomial foi utilizada na análise. A utilização mais que adequada do pré-natal foi associada com maior capital social em nível de cidade (capital social agregado), status socioeconômico e trabalho durante a gravidez. Baixo capital social contextual não agregado e composicional, gestação de risco e padrão do uso do pré-natal foram associados com a utilização inadequada do pré-natal. O capital social contextual e o apoio social foram determinantes sociais para a adequação da utilização do pré-natal.

Theoretical frameworks on health promotion focusing on social determinants of oral health have highlighted promising approaches for improving the oral health of populations and reducing inequities in oral health. In the last two decades the salutogenic theory has gained ground in the field of health promotion, but not in oral health promotion. Instead of focusing on risk factors and behavior change, the theory highlights the importance of resources and the ability to use them. The model's central construct, sense of coherence (SOC), suggests explanations of the relationship between life stressors and health status. The stronger the SOC, the more successfully people will cope with stressors and thus maintain their health. This paper discusses the potential of the salutogenic theory to guide the development of actions in the five fields of oral health promotion: creating supportive environments; promoting health through public policy; strengthening community action; developing personal skills; and reorienting health services. The theory can serve as a framework for oral health promotion measures that strengthen the available resources, create better ones, and enable people to identify and benefit from them.Os referenciais teóricos para a promoção da saúde que priorizam os determinantes sociais da saúde bucal têm destacado abordagens promissoras para melhorar a saúde bucal das populações e reduzir as iniqüidades em saúde bucal. Nas duas últimas décadas, a teoria salutogênica tem merecido destaque no campo da promoção da saúde geral, mas não especificamente na promoção da saúde bucal. Ao invés de priorizar fatores de risco e mudanças comportamentais, a teoria destaca a importância dos recursos e da capacidade de utilizá-los. O constructo central do modelo, chamado senso de coerência (SOC), sugere explicações para a relação entre estressores e status de saúde. Quanto mais elevado o SOC, maior a capacidade dos indivíduos para lidar com os estressores e, portanto, manter a própria saúde. O artigo discute o potencial da teoria salutogênica no sentido de orientar o desenvolvimento de ações nos cinco campos da promoção da saúde bucal: criação de ambientes favoráveis à saúde; elaboração e implementação de políticas públicas saudáveis; fortalecimento da ação comunitária; desenvolvimento de habilidades pessoais e reorientação dos serviços de saúde. A teoria pode servir como referencial teórico para o desenvolvimento de ações de promoção da saúde bucal que objetivem fortalecer os recursos disponíveis,criar outros melhores e permitir que as pessoas os identifiquem e deles possam se beneficiar.

OBJECTIVE: To assess the relationship between housing conditions and low birthweight and preterm low birthweight among low-income women. METHODS: A case-control study was conducted with post-partum women living in the city of Rio de Janeiro, Southeast Brazil, in 2003-2005. Two groups of cases, low birthweight (n=96) and preterm low birthweight infants (n=68), were compared against normal weight term controls (n=393). Housing conditions were categorized into three levels: adequate, inadequate, and highly inadequate. Covariates included sociodemographic and anthropometric characteristics, risk behaviors, violence, anxiety, satisfaction during pregnancy, obstetric history and prenatal care. RESULTS: Poor housing conditions was independently associated with low birthweight (inadequate - OR 2.3 [1.1;4.6]; highly inadequate - OR 7.6 [2.1;27.6]) and preterm low birthweight (inadequate - OR 2.2 [1.1;4.3]; highly inadequate - OR 7.6 [2.4;23.9]) and factors associated with outcomes were inadequate prenatal care and previous preterm birth. Low income and low maternal body mass index remained associated with low birthweight. CONCLUSIONS: Poor housing conditions were associated with low birthweight and preterm low birthweight.

INTRODUCTION: Periodontal disease is an inflammatory response to bacteria that reside in the gum tissue and can have systemic repercussion. OBJECTIVE: The aim of this study was to assess the relationship between periodontitis and markers of cardiovascular risk. MATERIAL AND METHOD: Ninety selected patients were assigned into two groups in accordance with their levels of probing pocket depth (PPD) and Clinical Attachment Level (CAL): control group, n= 45 (< 4 sites with PPD ≥ 4.0 mm and CAL ≥ 3.0 mm) and case group, n= 45 (≥ 30% of sites with PPD ≥ 4.0 mm and CAL ≥3.0 mm). Plasma concentrations of C-reactive protein, high sensitive CRP, high-density lipoproteins (HDL-c) and electronegative low density lipoproteins (LDL) were assessed in all participants. Data from medical history and socioeconomic level were also collected from patients. RESULT: Plasma levels of HDL-c were lower in subjects with periodontal disease (p = 0.016) and were inversely associated with the number of sites with PPD ≥ 3 mm (rho= -0.325) and number of sites with PPD ≥ 3 mm and CAL ≥ 3 mm (rho= -0.216). These associations remained significant after adjustments for dental plaque and smoking using Univariate Analysis of Covariance (p < 0.05). Adjusted odds ratio between periodontal disease and levels of HDL-c was 0.94 (CI95% 0.88-0.99) after adjusting for age, smoking and dental plaque. Other investigated markers of cardiovascular risk were not related to periodontal disease. CONCLUSION: Clinical parameters of periodontitis were inversely associated with plasma concentrations of HDL-c.
