Abstract This publication is an update of the “Consensus on the therapeutic management of atopic dermatitis - Brazilian Society of Dermatology” published in 2019, considering the novel, targeted-oriented systemic therapies for atopic dermatitis. The initial recommendations of the current consensus for systemic treatment of patients with atopic dermatitis were based on a recent review of scientific published data and a consensus was reached after voting. The Brazilian Society of Dermatology invited 31 experts from all regions of Brazil and 2 international experts on atopic dermatitis who fully contributed to the process. The methods included an e-Delphi study to avoid bias, a literature search and a final consensus meeting. The authors added novel approved drugs in Brazil and the indication for phototherapy and systemic therapy for AD. The therapeutical response to systemic treatment is hereby reported in a suitable form for clinical practice and is also part of this updated manuscript.

Abstract: BACKGROUND: Atopic dermatitis is a highly prevalent inflammatory and pruritic dermatosis with a multifactorial etiology, which includes skin barrier defects, immune dysfunction, and microbiome alterations. Atopic dermatitis is mediated by genetic, environmental, and psychological factors and requires therapeutic management that covers all the aspects of its complex pathogenesis. OBJECTIVES: The aim of this article is to present the experience, opinions, and recommendations of Brazilian dermatology experts regarding the therapeutic management of atopic dermatitis. METHODS: Eighteen experts from 10 university hospitals with experience in atopic dermatitis were appointed by the Brazilian Society of Dermatology to organize a consensus on the therapeutic management of atopic dermatitis. The 18 experts answered an online questionnaire with 14 questions related to the treatment of atopic dermatitis. Afterwards, they analyzed the recent international guidelines on atopic dermatitis of the American Academy of Dermatology, published in 2014, and of the European Academy of Dermatology and Venereology, published in 2018. Consensus was defined as approval by at least 70% of the panel. RESULTS/CONCLUSION: The experts stated that the therapeutic management of atopic dermatitis is based on skin hydration, topical anti-inflammatory agents, avoidance of triggering factors, and educational programs. Systemic therapy, based on immunosuppressive agents, is only indicated for severe refractory disease and after failure of topical therapy. Early detection and treatment of secondary bacterial and viral infections is mandatory, and hospitalization may be needed to control atopic dermatitis flares. Novel target-oriented drugs such as immunobiologicals are invaluable therapeutic agents for atopic dermatitis.

Abstract: Background: Urticarias are frequent diseases, with 15% to 20% of the population presenting at least one acute episode in their lifetime. Urticaria are classified in acute ( ≤ 6 weeks) or chronic (> 6 weeks). They may be induced or spontaneous. Objectives: To verify the diagnostic and therapeutic recommendations in chronic spontaneous urticaria (CSU), according to the experience of Brazilian experts, regarding the available guidelines (international and US). Methods: A questionnaire was sent to Brazilian experts, with questions concerning diagnostic and therapeutic recommendations for CSU in adults. Results: Sixteen Brazilian experts answered the questionnaire related to diagnosis and therapy of CSU in adults and data were analyzed. Final text was written, considering the available guidelines (International and US), adapted to the medical practices in Brazil. Diagnostic work up in CSU is rarely necessary. Biopsy of skin lesion and histopathology may be indicated to rule out other diseases, such as, urticarial vasculitis. Other laboratory tests, such as complete blood count, CRP, ESR and thyroid screening. Treatment of CSU includes second-generation anti-histamines (sgAH) at licensed doses, sgAH two, three to fourfold doses (non-licensed) and omalizumab. Other drugs, such as, cyclosporine, immunomodulatory drugs and immunosuppressants may be indicated (non-licensed and with limited scientific evidence). Conclusions: Most of the Brazilian experts in this study partially agreed with the diagnostic and therapeutic recommendations of the International and US guidelines. They agreed with the use of sgAH at licensed doses. Increase in the dose to fourfold of sgAH may be suggested with restrictions, due to its non-licensed dose. Sedating anti-histamines, as suggested by the US guideline, are indicated by some of the Brazilian experts, due to its availability. Adaptations are mandatory in the treatment of CSU, due to scarce or lack of other therapeutic resources in the public health system in Brazil, such as omalizumab or cyclosporine.

8. Estudo clínico para avaliar a eficácia e segurança de um hidratante ativo reparador de barreira como auxiliar no tratamento de dermatite atópica em crianças

Introdução: O uso de hidratantes melhora a função de barreira cutânea e pode ser útil na dermatite atópica. Objetivos: Avaliar eficácia e segurança de hidratante ativo reparador de barreira à base de glicerina, erythritol, Imperata cilindrica e homarine como tratamento auxiliar da dermatite atópica. Métodos: Aplicação de hidratante em crianças com dermatite atópica de leve a moderada durante um mês, com avaliações subjetivas e objetivas após 15 e 30 dias. Resultados: Foram analisados os dados de 35 crianças, de um a dez anos, quanto a: alterações do sono, ressecamento, descamação, prurido e maciez da pele, que tiveram redução significativa da visita inicial em relação à final. A avaliação objetiva do índice de gravidade Scorad (score on atopic dermatitis) revelou redução de 25.27 para 9.30 (p < 0.0001). Houve redução no uso de dexclorfeniramina de 25.71% no 15o dia para 5.72% no 30o dia (p = 0.0233) e de 34.29% no 15o dia para 5.72% no 30o dia com hidrocortisona (p = 0.0075). Conclusões: O uso do hidratante durante 30 dias melhorou os sintomas analisados, com significativa diminuição do índice de gravidade e menor necessidade de anti-histamínicos e corticosteroides tópicos. A utilização de hidratante adequado foi eficaz e segura como auxiliar para crianças com dermatite atópica.

Trata-se de um estudo descritivo, quantitativo, realizado em uma unidade de saúde. Os dados foram coletados através de formulário para amostra de 49 mulheres, com resultado citopatológico de lesão intraepitelial de baixo grau; a coleta ocorreu entre outubro e dezembro de 2015, e os resultados analisados por meio de estatística descritiva. A prevalência da infecção ocorreu em mulheres adultas jovens e solteiras com ensino médio, renda familiar inferior a cinco salários-mínimos. A realização do exame preventivo e sua periodicidade foram referidas por mais de 70% delas. Conclui-se que a escolaridade da mulher apontou uma forte infl uência sobre a realização do exame preventivo, o conhecimento insufi ciente acerca do Exame Papanicolaou e da sua fi nalidade, contribuindo para as mulheres se tornarem mais vulneráveis às Infecções Sexualmente Transmissíveis (ISTs), incluindo o HPV. Mesmo sendo um exame tão amplamente divulgado, deste modo, impede o estabelecimento de ações efi cazes no âmbito da prevenção. Palavras-chave: Papillomaviridae; Saúde da Mulher; Diagnóstico. ABsTRAcT The aim of this study is to analyze women's knowledge regarding the Pap Smear as a preventive for Human Papillomavirus (HPV). This is a descriptive, quantitative study carried out in a health unit. Data were collected through a sample form of 49 women, with cytopathologic result of low level intraepithelial lesion; the collection took place between October and December 2015, and the results were analyzed by means of descriptive statistics. The infection prevalence occurred in young and single adult women with high school, family income less than fi ve minimum wages. The preventive examination and its periodicity were mentioned by more than 70% of them. It is concluded that women's schooling showed a strong infl uence on the preventive examination, insuffi cient knowledge about the Pap Smear and its purpose, contributing to women becoming more vulnerable to Sexually Transmissible Infections (STIs), including HPV. Even though such an examination is so widely disseminated, it prevents the establishment of effective preventive action. Keywords: Papillomaviridae; Women's Health; Diagnosis.

Objetivo: conhecer o perfil de mulheres com papilomavírus humano genital e investigar o conhecimento sobre a infecção e sua prevenção. Método: estudo descritivo através da aplicação de formulário estruturado para uma amostra de 49 mulheres. Todas apresentaram resultado de papanicolaou com lesão intraepitelial de baixo grau. Os dados foram tratados por meio de análise estatística descritiva utilizando o programa EPI-INFO® 7.1.4. Resultados: as mulheres estudadas, em sua maioria, eram adultas jovens, solteiras e com ensino médio. A incidência da infecção ocorreu em participantes com renda familiar inferior a cinco salários mínimos por mês. A realização do exame de papanicolaou e sua periodicidade foi referida por mais de 70% delas. Conclusão: os dados apontaram ocorrência do papilomavírus humano em mulheres entre 20 e 40 anos de idade, com maior frequência naquelas que iniciaram a atividade sexual após os 15 anos de idade.Descritores: Papilomavírus humano. 11. Lichen planopilaris-like eruption during treatment with tyrosine kinase inhibitor nilotinib

Abstract Tyrosine kinase inhibitors are effective as a target therapy for malignant neoplasms. Imatinib was the first tyrosine kinase inhibitor used. After its introduction, several other drugs have appeared with a similar mechanism of action, but less prone to causing resistance. Even though these drugs are selective, their toxicity does not exclusively target cancer cells, and skin toxicity is the most common non-hematologic adverse effect. We report an eruption similar to lichen planopilaris that developed during therapy with nilotinib, a second generation tyrosine kinase inhibitor, in a patient with chronic myeloid leukemia resistant to imatinib. In a literature review, we found only one report of non-scarring alopecia due to the use of nilotinib.

Abstract: We describe the case of a 9-year-old boy with idiopathic bone marrow aplasia and severe neutropenia, who developed skin ulcers under cardiac monitoring electrodes. The diagnosis of primary cutaneous aspergillosis was made after the second biopsy and culture. Imaging investigation did not reveal internal fungal infection. The child was treated, but did not improve and died 3 months after admission. The report highlights and discusses the preventable risk of aspergillus skin infection in immunocompromised patients.

FUNDAMENTOS: a dermatite atópica é uma doença inflamatória cutânea que apresenta múltiplos fatores desencadeantes. Há vários relatos de autores que confirmaram os aeroalérgenos como fatores agravantes ou desencadeantes. O teste de contato com aeroalérgenos ou teste de contato atópico foi proposto para avaliar a participação destes alérgenos na dermatite atópica. OBJETIVO: objetivo deste estudo foi avaliar a positividade do teste de contato atópico em pacientes com dermatite atópica. MÉTODOS: Avaliamos 50 pacientes com dermatite atópica e 45 do grupo com rinite alérgica, nos quais realizamos teste de contato atópico com extratos de Dermatophagoides pteronissynus, Dermatophagoides farinae e Blomia tropicalis, além de testes cutâneos de leitura imediata para os mesmos alérgenos, acrescidos de epitélio de cão e gato e fungos. RESULTADOS: verificamos que o teste de contato atópico com ácaros apresentou maior positividade nos indivíduos do grupo de dermatite atópica quando comparado ao grupo de rinite alérgica. CONCLUSÕES: o teste de contato atópico apresenta resultados estatisticamente significativos quando realizado com ácaros, em pacientes com dermatite atópica, com p=0,035, OR (odds ratio) = 3,35 e IC(95%) = [ 1,18; 9,47].BACKGROUND: Atopic dermatitis is an inflammatory skin disease that can be triggered by many factors. Several reports confirm the role of airborne allergic agents as aggravating or triggering factors. The patch test with airborne allergic agents or the atopy patch test was suggested to evaluate the role of these allergens in atopic dermatitis. OBJECTIVE: This study aimed at evaluating the positivity of the atopy patch test in patients with atopic dermatitis. METHODS: We evaluated 50 patients with atopic dermatitis and 45 with allergic rhinitis, the atopy patch test was performed in these patiennts with extracts of Dermatophagoides pteronissynus, Dermatophagoides farinae and Blomia tropicalis, as well as immediate skin prick tests for the same allergens with cat and dog epithelia and fungi. RESULTS: It was found that the atopy patch test with dust mites showed higher positivity in individuals with atopic dermatitis when compared to those with allergic rhinitis. CONCLUSIONS: The atopy patch test shows statistically significant results when performed with dust mites in patients with atopic dermatitis, p = 0.035, odds ratio (OR) = 3.35 and CI (95%) = [1.18, 9, 47].

FUNDAMENTO: A queilite actínica crônica é a ceratose actínica localizada no vermelhão labial. O tratamento é de crucial importância, devido ao potencial de transformação maligna. OBJETIVO: Avaliar os resultados estéticos e funcionais das vermelhectomias clássica e em W-plastia na queilite actínica. Na técnica clássica, a cicatriz é linear; na W-plastia, em linha quebrada. MÉTODOS: Foram tratados 32 pacientes com diagnóstico clínico e histopatológico de queilite actínica. Quinze deles foram submetidos à técnica em W-plastia e 17, à técnica clássica. Avaliaram-se parâmetros como retração cicatricial e alterações funcionais. RESULTADOS: Houve associação estatisticamente significativa entre a técnica utilizada e a presença de retração cicatricial, sendo a associação positiva com a clássica (p=0,01 com correção de Yates). O risco relativo (odds ratio - OR) calculado foi de 11,25, ou seja, houve maior chance de retração nos pacientes submetidos à técnica clássica. Nenhuma das técnicas apresentou alterações funcionais. Avaliaram-se complicações pós-operatórias como presença de crostas, lábios secos, parestesia e deiscência de sutura. Não houve associação estatisticamente significante entre as complicações e a técnica utilizada (p=0,69). CONCLUSÃO: Concluiu-se que a vermelhectomia em W-plastia oferece melhores resultados estéticos e índices de complicações semelhantes.BACKGROUND: Chronic actinic cheilitis is actinic keratosis located on the vermilion border. Treatment is essential because of the potential for malignant transformation. OBJECTIVE: To evaluate the aesthetic and functional results of vermilionectomy using the classic and Wplasty techniques in actinic cheilitis. In the classic technique, the scar is linear and in the W-plasty one, it is a broken line. METHODS: 32 patients with clinical and histopathological diagnosis of actinic cheilitis were treated. Out of the 32 patients, 15 underwent the W-plasty technique and 17 underwent the classic one. We evaluated parameters such as scar retraction and functional changes. RESULTS: A statistically significant association between the technique used and scar retraction was found, which was positive when using the classic technique (p = 0.01 with Yates' correction). The odds ratio was calculated at 11.25, i.e., there was a greater chance of retraction in patients undergoing the classic technique. Both techniques revealed no functional changes. We evaluated postoperative complications such as the presence of crusts, dry lips, paresthesia, and suture dehiscence. There was no statistically significant association between complications and the technique used (p = 0.69). CONCLUSION: We concluded that vermilionectomy using the W-plasty technique shows better cosmetic results and similar complication rates.

FUNDAMENTOS: As micoses superficiais e subcutâneas têm alta prevalência e, muitas vezes, caráter crônico, necessitando tratamentos tópicos e/ou sistêmicos com antifúngicos. As drogas de escolha são azóis e alilaminas (terbinafina). É necessário avaliar a eficácia das drogas para tratamento em humanos e em animais. Estudos para avaliar in vitro a ação dos antimicóticos são raros, especialmente, contra fungos filamentosos. OBJETIVO: Avaliar a eficácia in vitro da terbinafina pelo método de disco-difusão contra fungos filamentosos e leveduras agentes de micoses. MÉTODOS: Avaliou-se a ação da terbinafina (0,125µg-100µg) contra dez espécies fúngicas pelos métodos discodifusão e microdiluição/referência, para determinar a concentração inibitória mínima (MIC). RESULTADOS: Observou-se alta sensibilidade à terbinafina em: T. rubrum, M. gypseum, T. mentagrophytes, T. tonsurans, M. canis, C. carrionii e E. floccosum (halo ≥ 40mm com disco de 0,125µg). S. hyalinum e C. parapsilosis foram considerados sensíveis, mas com halos menores. Fusarium spp. apresentou menor sensibilidade (halo=12mm com disco de 2µg; MIC 8µg/mL). CONCLUSÕES: Os resultados reiteram estudos anteriores quanto à alta eficácia da terbinafina em relação a dermatófitos. A técnica de disco-difusão foi de fácil aplicação e adequada na rotina de laboratórios clínicos.BACKGROUND: Superficial and subcutaneous mycoses have a high prevalence and, often, chronic evolution. Therefore, they need extensive treatment with topic and/or systemic antifungal agents. Azoles and alilamines (terbinafine) are first-choice drugs to treat human and animal infections. Thus, evaluation of the efficacy of these drugs is important for a successful treatment. However, there are few studies that evaluate the in vitro activity of antifungal agents. OBJECTIVE: To evaluate the in vitro efficacy of terbinafine activity against filamentous fungi and yeasts that cause mycoses. METHOD: The in vitro activity of terbinafine (0.125-100µg) against 10 fungi species was evaluated by the diskdiffusion and microdilution/reference methods to determine the Minimum Inhibitory Concentration (MIC). RESULTS: We found a high susceptibility to terbinafine in: T. rubrum, M. gypseum, T. mentagrophytes, T. tonsurans, M. canis, C. carrionii and E. floccosum (halo ≥ 40mm with 0.125µg disk). S. hyalinum and C. parapsilosis were considered susceptible, but less than the others. Fusarium spp. showed the lowest susceptibility (halo=12mm with 2µg disk; MIC 8µg/mL). CONCLUSIONS: The results of this research confirm previous findings about the efficacy of terbinafine. The drug was shown to be highly effective to treat dermatophyte infections. The disk-diffusion method was easy to use and is a suitable technique for routine use in clinical laboratories.

FUNDAMENTOS - Tacrolimo pomada é eficaz no tratamento da dermatite atópica. OBJETIVOS - Avaliar a eficácia e segurança do tacrolimo pomada 0,03% (Protopic®) no tratamento de pacientes pediátricos com dermatite atópica. MÉTODOS - Estudo multicêntrico, aberto e não comparativo. Incluídos 174 pacientes (dois a 10 anos)com dermatite atópica. Utilizou-se tacrolimo duas vezes ao dia, por seis semanas. O critério primário de eficácia foi a melhora clínica > a 90% avaliada pelo médico (Escala de Avaliação Global da Resposta Clínica). Outros critérios de eficácia foram a redução no índice de área e gravidade do eczema (EASI), a redução do percentual de superfície corporal afetada (%BSA) e a avaliação do prurido pelo paciente (escala analógica visual). Segurança foi avaliada pela ocorrência de eventos adversos relatados pelos pais e pacientes ou pelos investigadores. RESULTADOS - Trinta e três por cento dos pacientes apresentaram melhora clínica >90%. Quando avaliado o escore EASI, houve redução de 45,5% (primeira semana) e 61,8% (sexta semana) quando comparado com o basal (pBACKGROUND: Tacrolimus ointment has been shown to be effective in treatment of atopic dermatitis. OBJECTIVES: To evaluate the efficacy and safety of 0.03% tacrolimus ointment (Protopic®) in pediatric patients with mild, moderate and severe atopic dermatitis. METHODS: Open, non-comparative, multicentric study carried out in Brazil. 174 patients (ages from two to 10) with mild to severe atopic dermatitis were included. Patients were instructed to apply Protopic® twice a day for six weeks. Primary efficacy criterion was clinical improvement > 90% assessed by the physician (Clinical Response Global Evaluation Scale). Other efficacy criteria included reduction of the Eczema Area Severity Index (EASI), decrease of the affected body surface area (%BSA) and evaluation of the itching by the patients or their guardians (visual analogical scale). FUNDAMENTOS: Alergia ao látex é comum em trabalhadores da saúde, resultando em absenteísmo e afastamento das atividades profissionais. Há no Brasil poucos estudos publicados sobre o assunto. Enquanto na população geral a prevalência de alergia ao látex é menor que 2%, nos profissionais de saúde pode chegar a 30%. OBJETIVO: Estudar a prevalência de alergia ao látex em profissionais de saúde do Hospital do Servidor Público Estadual de São Paulo. MÉTODOS: Foi aplicado questionário aberto a profissionais que têm contato com luvas de borracha em todos os serviços do hospital. Foi colhido sangue daqueles que referiram sintomas e dosado anticorpo IgE específico por meio do ImmunoCAP. Para análise estatística utilizaram-se porcentagens, tabelas e teste qui-quadrado. RESULTADOS: Foram distribuídos 2.349 questionários, e respondidos 1.045, dos quais 129 referiram correlação entre uso de látex e sintomas sugestivos de alergia, e 54 permitiram a coleta de sangue para determinação de IgE específica. Neste grupo houve dois casos em que foi demonstrado anticorpo específico, estabelecendo-se prevalência de 3,7% de alergia ao látex. Fatores como gênero, profissão, freqüência de contato com luvas, setor de trabalho e antecedentes de atopia foram relacionados com maior prevalência de alergia ao látex. CONCLUSÃO: A prevalência de alergia ao látex encontrada foi de 3,7%.BACKGROUND: Latex allergy is common among healthcare workers and leads to absence from work and withdraw from professional activities. There are few studies published in Brazil on this subject. In the general population the prevalence of allergy to latex is less than 2%, whereas in healthcare workers it may reach 30%. OBJECTIVE: To study the prevalence of latex allergy in healthcare workers at the Hospital do Servidor Público Estadual de São Paulo. METHODS: A self-administered questionnaire was distributed to healthcare professionals from any sector of the hospital who have contact with rubber gloves. Blood was drawn from those who referred symptoms, for analyses of serum antilatex IgE antibody by Immunocap assay. Percentages, tables and the chi-square test were used for statistical analysis. RESULTS: A total of 2349 questionnaires were distributed, and 1045 were answered. In that, 129 reported association between latex and allergy symptoms and 54 out of them agreed to blood collection for specific IgE determination. Specific antibody was demonstrated in tow cases, determining a prevalence of 3.7% of latex allergy. Gender, occupation, frequency of use of gloves, work sector and past history of atopy were related to greater prevalence of latex allergy. CONCLUSION: The prevalence of latex allergy found was 3.7%.

Epidermodysplasia Verruciformis is a genodermatosis characterized by susceptibility to infection by specific HPV types (HPV 3/10 and beta-HPVs). It is considered to be the first model in human carcinogenesis induced by HPV. In this report we present a rare case of Epidermodysplasia Verruciformis associated with vertical transmission of HIV. Although most patients with HIV present infections by HPV virus, the frequency of Epidermodysplasia Verruciformis is no greater in patients with HIV because in EV there is a deficiency of specific cellular immunity to infection by some types of HPV, called HPV-related Epidermodysplasia Verruciformis.

Comunica-se o resultado do tratamento com acitretina e interferon alfa-2a em doente de 16 anos de idade com epidermodisplasia verruciforme. O diagnóstico foi realizado mediante os achados clínico-histopatológicos e pesquisa in situ positiva para papiloma vírus humano. O quadro clínico era extenso e resistente a outros tratamentos, com risco de desenvolvimento de neoplasias. Houve melhora clínica 30 dias após o início do tratamento, mantida durante os 16 meses de seguimento.We report the result of a treatment with acitretin and alpha-2a interferon in a 16-year-old boy with epidermodysplasia verruciformis. Diagnosis was based on clinical features, histopathological examination and positivity for in situ HPV. The affection was extensive and resistant to other treatment modalities, with a high risk of development of neoplasias. Treatment with oral acitretin associated with subcutaneous recombinant interferon alpha-2a three times per week induced improvement, which was sustained for 16 months of follow-up.

Através de estudo histopatológico retrospectivo de 51 lâminas de eritema nodoso (EN) comum e 39 lâminas de eritema nodoso hansênico (ENH) comprovou-se que o quadro histopatológico de ambos os grupos diferiu em diversos aspectos. Os achados que mais divergiram entre as duas afecções foram: a presença do infiltrado granulomatoso virchoviano em todos os casos de ENH; a localização predominante do infiltrado inflamatório, dermo-hipodérmica no ENH e hipodérmica no EN comum; o acometimento septal preferencial no EN comum e lobular no ENH; o encontro de células gigantes tipo corpo estranho no EN comum e sua ausência no ENH; o achado predominante de células mononucleares no EN comum e neutrófilos no ENH; a presença de infiltrado inflamatório ao redor de nervos no ENH. Comprovou-se que a pesquisa de bacilo álcool-ácido resistente (BAAR) não necessita ser realizada em todos os casos de EN, uma vez que ao exame histopatológico de rotina existem diferenças entre o EN comum e o ENH que podem orientar a realização deste procedimento. A pesquisa deve ser realizada quando ao exame de rotina nas lâminas coradas por hematoxilina-eosina forem observadas as características histopatológicas do ENH descritas no estudo.

The topical immunotherapy is used to treat alopecia areata and recalcitrant warts since the 1970s. Diphencyprone is a contact sensitizer used to treat dermatological conditions resulting from as altered immunological state, such as extensive alopecia areata, being partially effective and safe. Side effects include local eczema with blistering, regional lymphadenopathy and contact urticaria. Rare adverse effects include an erythema multiforme-like reaction, hyperpigmenttion, hypopigmentation, and vitiligo. We report a 30-year-old, Brazilian male who developed vitiligo lesions following DPCP therapy for alopecia areata.

Atopic dermatitis (AD) is a systemic, multifactorial disease that causes significant morbidity and health care burden in Latin America (LA). Data on AD are scarce in LA. Lack of disease registries and non-standardized study methodologies, coupled with region-specific genetic, immunological, and environmental factors, hamper data collection. A panel of LA experts in AD was given a series of relevant questions to address before a conference. Each narrative was discussed and edited through numerous rounds of deliberation until achieving consensus. Identified knowledge gaps in AD research were updated prevalence, adult-disease epidemiology, local phenotypes and endotypes, severe-disease prevalence, specialist distribution, and AD public health policy. Underlying reasons for these gaps include limited funding for AD research, from epidemiology and public policy to clinical and translational studies. Regional heterogeneity requires that complex interactions between race, ethnicity, and environmental factors be further studied. Informed awareness, education, and decision making should be encouraged.

OBJETIVO: Relatar caso em que as manifestações dermatológicas conduziram ao diagnóstico de mastocitose sistêmica. MÉTODOS: As informações apresentadas foram obtidas durante o acompanhamento do paciente, análise de prontuário e revisão de literatura. RESULTADOS: Paciente de 72 anos com lesões cutâneas disseminadas e acometimento de múltiplos órgãos. A avaliação histopatológica de lesão cutânea confirmou a hipótese de mastocitose e levou a avaliação dos demais acometimentos, preenchendo critérios diagnósticos para mastocitose sistêmica associada à neoplasia hematológica e possibilitando tratamento apropriado. CONCLUSÃO: Ressaltamos a importância do conhecimento dessa entidade pelas suas manifestações cutâneas características e possibilidade de rastreio e diagnóstico de neoplasia interna associada, além de abordagem apropriada para controle dos sintomas.

Background and objectives Atopic dermatitis is a chronic inflammatory skin disease with a prevalence of 0.02% to 8.1% in adults. Adult patients with moderate-to-severe atopic dermatitis have frequent relapses and significant disease burden. Objectives: Characterize clinical, immunological and therapeutic features of Brazilian adults with atopic dermatitis. Methods A multicenter, observational, retrospective, descriptive registry-based study, conducted at reference hospitals, between December 2016 and October 2017. Demographics, personal and family history of atopic diseases, clinical manifestations, laboratory tests, disease severity and management were collected. Results From the 187 subjects included in the analysis, 56.1% were female and 71.7% were Caucasian, with a mean age of 24.7 years. Mean follow-up duration was 9 years. Asthma or other allergic diseases were reported by 80.2% of the patients. The main comorbidity was hypertension (10.2%) and common disease manifestations included pruritus and erythema. Lesions were generally distributed in non-flexural and flexural areas, with typical morphology. Around 83% of patients had moderate to severe disease and 8.6% reported at least one hospitalization. Most patients received topical and/or systemic pharmacological therapies, including omalizumab (5.9%); 4.3% received phototherapy. Moreover, 66.8% of patients received adjuvant therapy and 79.1% changed or discontinued treatment for atopic dermatitis due to remission (46.5%), poor effectiveness (33.7%) or lack of therapeutic adherence (12.9%). Most patients presented characteristics of type 2 inflammation, with immunoglobulin E levels above 100 IU/mL (94.4%) and peripheral blood eosinophils above 5% (55.9%). Conclusion Brazilian adult patients with severe atopic dermatitis need treatments to efficiently control the disease and improve quality of life.

BACKGROUND: Atopic dermatitis is a highly prevalent inflammatory and pruritic dermatosis with a multifactorial etiology, which includes skin barrier defects, immune dysfunction, and microbiome alterations. Atopic dermatitis is mediated by genetic, environmental, and psychological factors and requires therapeutic management that covers all the aspects of its complex pathogenesis. OBJECTIVES: The aim of this article is to present the experience, opinions, and recommendations of Brazilian dermatology experts regarding the therapeutic management of atopic dermatitis. METHODS: Eighteen experts from 10 university hospitals with experience in atopic dermatitis were appointed by the Brazilian Society of Dermatology to organize a consensus on the therapeutic management of atopic dermatitis. The 18 experts answered an online questionnaire with 14 questions related to the treatment of atopic dermatitis. Afterwards, they analyzed the recent international guidelines on atopic dermatitis of the American Academy of Dermatology, published in 2014, and of the European Academy of Dermatology and Venereology, published in 2018. Consensus was defined as approval by at least 70% of the panel. RESULTS/CONCLUSION: The experts stated that the therapeutic management of atopic dermatitis is based on skin hydration, topical anti-inflammatory agents, avoidance of triggering factors, and educational programs. Systemic therapy, based on immunosuppressive agents, is only indicated for severe refractory disease and after failure of topical therapy. Early detection and treatment of secondary bacterial and viral infections is mandatory, and hospitalization may be needed to control atopic dermatitis flares. Novel target-oriented drugs such as immunobiologicals are invaluable therapeutic agents for atopic dermatitis.

Introduction: Clinical characteristics of frontal fibrosing alopecia (FFA) have been studied since its description in 1994 in Europe, North America and Australia, and the present study is the largest on FFA features in Latin America. Methods: This study, through questionnaire and medical records, describes characteristics of a Brazilian population of 59 patients with FFA, concerning clinical forms, individual features and use of cosmetic products. Results: Mean patients’ age was 58.4 years (range 35-84y). Duration of disease varied from six months to 20y (median 5y). Most women were postmenopausal (83.1%) and non-smokers (83.1%). Five percent had at least one associated autoimmune disease, most commonly hypothyroidism (13.6%). Six of 40 patients (15%) had lichen planus pigmentosus and ten had facial papules (25%). Eyebrow alopecia occurred in 50 (84.7%), and body hair loss in 47 (79.7%). Facial papules were more prevalent in premenopausal women. Conclusion: Studying FFA epide- miology may help understanding the pathophysiology of this epidemic disease and this study highlights similarities and differences to previously published studies in FFA, such as greater frequency in postmenopausal women, association with hypothyroidism and facial irritation with cosmetic products.

Urticaria are classified in acute ( 6 weeks). They may be induced or spontaneous. OBJECTIVES: To verify the diagnostic and therapeutic recommendations in chronic spontaneous urticaria (CSU), according to the experience of Brazilian experts, regarding the available guidelines (international and US). Methods: A questionnaire was sent to Brazilian experts, with questions concerning diagnostic and therapeutic recommendations for CSU in adults. RESULTS: Sixteen Brazilian experts answered the questionnaire related to diagnosis and therapy of CSU in adults and data were analyzed. Final text was written, considering the available guidelines (International and US), adapted to the medical practices in Brazil. Diagnostic work up in CSU is rarely necessary. Biopsy of skin lesion and histopathology may be indicated to rule out other diseases, such as, urticarial vasculitis. Other laboratory tests, such as complete blood count, CRP, ESR and thyroid screening. Treatment of CSU includes second-generation anti-histamines (sgAH) at licensed doses, sgAH two, three to fourfold doses (non-licensed) and omalizumab. Other drugs, such as, cyclosporine, immunomodulatory drugs and immunosuppressants may be indicated (non-licensed and with limited scientific evidence). CONCLUSIONS: Most of the Brazilian experts in this study partially agreed with the diagnostic and therapeutic recommendations of the International and US guidelines. They agreed with the use of sgAH at licensed doses. Increase in the dose to fourfold of sgAH may be suggested with restrictions, due to its non-licensed dose. Sedating anti-histamines, as suggested by the US guideline, are indicated by some of the Brazilian experts, due to its availability. Adaptations are mandatory in the treatment of CSU, due to scarce or lack of other therapeutic resources in the public health system in Brazil, such as omalizumab or cyclosporine. We describe the case of a 9-year-old boy with idiopathic bone marrow aplasia and severe neutropenia, who developed skin ulcers under cardiac monitoring electrodes. The diagnosis of primary cutaneous aspergillosis was made after the second biopsy and culture. Imaging investigation did not reveal internal fungal infection. The child was treated, but did not improve and died 3 months after admission. The report highlights and discusses the preventable risk of aspergillus skin infection in immunocompromised patients.

The use of moisturizers improves the skin barrier’s function and might be useful in atopic dermatitis. Objectives: To assess the efficacy and safety of an active moisturizer-barrier repairer based on glycerin, erythritol, Imperata cylindrica and homarine as an adjuvant treatment in atopic dermatitis. Methods: Application of the moisturizer in children with mild to moderate atopic dermatitis for 1 month, with subjective and objective evaluations after 15 and 30 days. Results: Data from 35 children – 20 girls (57.15%), 15 boys (42.86%) / 26 Caucasians (74.29%), 6 mulattos (17.14%), 3 dark skinned (8.57%) – aged 1 to 10 years (mean age = 5.6) were analyzed. The values of the variables sleeping disorders, dryness, desquamation, pruritus and softness of the skin had significantly decreased on the 30th day of treatment as compared to the baseline. Background Epicutaneous test made with dust mite antigens. Objective Evaluation of the response of the epicutaneous test with Aleuroglyphus ovatus antigen in atopic patients. Methods We patch tested 119 individuals, 48 with atopic dermatitis, 50 with respiratory allergy and 21 healthy controls. We compare the positive response frequency to a closed patch test using Aleuroglyphus ovatus antigen in different concentrations and 48 and 96h reading times among those individuals. Results Six patients with atopic dermatitis (12.5%) and 4 with respiratory atopy (8.0%) had positive reactions. None of the non-atopic controls had a positive response. As the antigen concentration raised, the number of positive reactions to epicutaneous test raised as well. Conclusion Our data suggest a positive relation between Atopy Patch Test positive responses and Aleuroglyphus ovatus antigen concentration, no matter the kind of the atopic clinical expression.

BACKGROUND: Patients with oral sensitivity are common in our practice. Allergic contact dermatitis is one of the most frequent etiologies. OBJECTIVES: Evaluate oral contact dermatitis using the Brazilian standard series and complementary dental series in patients using dental prostheses, with or without oral complaints. Determine specific dental Brazilian series. METHODS: Patients using dental prostheses with or without oral complaints realized patch tests. Brazilian standard series and complementary dental series were used according to ICDRG recommendations. The results were analysed according to age, sex, race, atopic conditions and symptoms associated. RESULTS: From 54 patients tested, 34 (63%) were positive at least to one substance. Nineteen had oral complaints, such as burning mouth, itch or oral erythema. There was no association between atopic condition and tests results. Without the oral series, just 23(42,6%) patients had a positive result. Using the Brazilian standard series with the complementary dental series we improved the positivity of the patch test to 47%. CONCLUSION: In patients using prostheses and with oral complaints, patch tests with Brazilian standard series with complementary dental series improve the tests positivity.

FUNDAMENTO: A queilite actínica crônica é a ceratose actínica localizada no vermelhão labial. O tratamento é de crucial importância, devido ao potencial de transformação maligna. OBJETIVO: Avaliar os resultados estéticos e funcionais das vermelhectomias clássica e em W-plastia na queilite actínica. Na técnica clássica, a cicatriz é linear; na W-plastia, em linha quebrada. MÉTODOS: Foram tratados 32 pacientes com diagnóstico clínico e histopatológico de queilite actínica. Quinze deles foram submetidos à técnica em W-plastia e 17, à técnica clássica. Avaliaram-se parâmetros como retração cicatricial e alterações funcionais. RESULTADOS: Houve associação estatisticamente significativa entre a técnica utilizada e a presença de retração cicatricial, sendo a associação positiva com a clássica (p=0,01 com correção de Yates). O risco relativo (odds ratio - OR) calculado foi de 11,25, ou seja, houve maior chance de retração nos pacientes submetidos à técnica clássica. Nenhuma das técnicas apresentou alterações funcionais. Avaliaram-se complicações pós-operatórias como presença de crostas, lábios secos, parestesia e deiscência de sutura. Não houve associação estatisticamente significante entre as complicações e a técnica utilizada (p=0,69). CONCLUSÃO: Concluiu-se que a vermelhectomia em W-plastia oferece melhores resultados estéticos e índices de complicações semelhantes. BACKGROUND: Chronic actinic cheilitis is actinic keratosis located on the vermilion border. Treatment is essential because of the potential for malignant transformation. OBJECTIVE: To evaluate the aesthetic and functional results of vermilionectomy using the classic and Wplasty techniques in actinic cheilitis. In the classic technique, the scar is linear and in the W-plasty one, it is a broken line. METHODS: 32 patients with clinical and histopathological diagnosis of actinic cheilitis were treated. Out of the 32 patients, 15 underwent the W-plasty technique and 17 underwent the classic one. We evaluated parameters such as scar retraction and functional changes. RESULTS: A statistically significant association between the technique used and scar retraction was found, which was positive when using the classic technique (p = 0.01 with Yates' correction). The odds ratio was calculated at 11.25, i.e., there was a greater chance of retraction in patients undergoing the classic technique. Both techniques revealed no functional changes. We evaluated postoperative complications such as the presence of crusts, dry lips, paresthesia, and suture dehiscence. There was no statistically significant association between complications and the technique used (p = 0.69). CONCLUSION: We concluded that vermilionectomy using the W-plasty technique shows better cosmetic results and similar complication rates.

Summary: The authors report a case of tinea pedis with interdigital lesions of the feet, caused by Scytalidium lignicola, anamorphous state of Hendersonula toruloidea. Microscopic examination of pota-to-dextrose agar culture showed catenulate and bicellular ochreous arthroconidia. Initially diagnosed as dermatophytosis, the case showed no success with the use of itraconazole. The main taxonomic aspects referring to Scytalidium lignicola, S. hyalinum and Hendersonula toruloidea are discussed due to the mycological importance of the present case in dermatology. Scytalidium hyalinum, actually anamorph of Hendersonula toruloidea, should be incorporated in the future in a new genus (Dickinson et al. 1980, McGinnis 1985). Zusammenfassung: Die Autoren beschreiben einen Fall von Tinea pedis verursacht durch Scytalidium lignicola, die anamorphe Form von Hendersonula toruloidea. Das mikromorphologische Aussehen auf Kartoffel-Dextrose-Agar zeigte die Anwesenheit von ockerfarbigen, uni- und doppelzellularen langen Ketten von Arthrokonidien. Anfangs wurde die Diagnose einer Dermatophytose gestellt und deshalb mit Itraconazol behandelt, aber ohne klinische Besserung. Da dieser Fall fur die Dermatologie recht wichtig erscheint, wird die Taxonomie des Erregers naher erortert, insbesondere in Bezug auf die Pilze Scytalidium lignicola, S. hyalinum und Hendersonula toruloidea. Scytalidium hyalinum sollte kunftig nicht mehr nur als anamorphe Form von Hendersonula toruloidea (Dickinson et al. 1983; Mc Ginnis, 1985) aufgefast, sondern einer neuen Gattung zugeordnet werden.

FUNDAMENTOS - Tacrolimo pomada é eficaz no tratamento da dermatite atópica. OBJETIVOS - Avaliar a eficácia e segurança do tacrolimo pomada 0,03% (Protopic®) no tratamento de pacientes pediátricos com dermatite atópica. MÉTODOS - Estudo multicêntrico, aberto e não comparativo. Incluídos 174 pacientes (dois a 10 anos)com dermatite atópica. Utilizou-se tacrolimo duas vezes ao dia, por seis semanas. O critério primário de eficácia foi a melhora clínica > a 90% avaliada pelo médico (Escala de Avaliação Global da Resposta Clínica). Outros critérios de eficácia foram a redução no índice de área e gravidade do eczema (EASI), a redução do percentual de superfície corporal afetada (%BSA) e a avaliação do prurido pelo paciente (escala analógica visual). Segurança foi avaliada pela ocorrência de eventos adversos relatados pelos pais e pacientes ou pelos investigadores. Summary Background The patch test is an essential procedure for the investigation of aetiologic diagnosis of allergic contact dermatitis, although it is not yet able to fully reproduce the events of the initial site of contact with the allergen. Objectives The aims of the present study are (i) to assess whether removal of the superficial corneous layer results in test positivity differences vs. the traditional technique, (ii) to assess the probable and/or possible and past and/or present sensitivity and relevance for each method, and (iii) to compare specific relevance of nickel sulphate for each method. FUNDAMENTOS: As micotecas são coleções de fungos conservadas para estudo futuro ou para a obtenção de extratos, drogas e outros fins. A conservação em meios de cultura por repiques sucessivos exige cuidados e nem sempre é fácil. Há necessidade de novas técnicas para a manutenção de fungos viáveis por longos períodos. OBJETIVO: avaliar a eficácia da preservação de fungos em água destilada num período de 12 meses. MÉTODOS: 43 espécies de fungos foram mantidas em frascos de vidro com água destilada estéril. Mensalmente eram colhidos de 200 a 250mil do líquido e inoculado em ágar batata dextrose para avaliar o crescimento e viabilidade dos fungos. RESULTADOS: houve crescimento dos fungos em todos os meses pelo período de um ano. CONCLUSÕES: A preservação de cepas pelo método da água destilada possibilitou, nesse curto espaço de tempo, provar a viabilidade e a capacidade de esporulação das cepas submetidas ao estudo. A conservação de fungos em água destilada é método barato e prático para manutenção de micoteca. BACKGROUND: Mycology collections are used to preserve fungi for future studies or to obtain extracts, drugs and others. Fungal preservation in culture media by successive sampling requires care and is not always easy. Novel techniques are necessary to maintain viable fungi for longer periods. OBJECTIVE: To evaluate the efficacy of preservation of fungi in distilled water for 12 months. METHODS: 43 species of fungi were maintained in glass flasks with sterile distilled water. Two hundred to 250 _l of the liquid were monthly collected and inoculated in potato dextrose agar to evaluate growth and viability of the fungi. RESULTS: All fungi grew every month during one year. CONCLUSIONS: reservation of fungal strains in distilled water allowed, in a short time interval, to prove viability and sporulation capacity of the fungi. Preservation of fungi in distilled water is a cheap and practical method to maintain a mycology collection.

Epidermodysplasia Verruciformis is a genodermatosis characterized by susceptibility to infection by specific HPV types (HPV 3/10 and beta-HPVs). It is considered to be the first model in human carcinogenesis induced by HPV. In this report we present a rare case of Epidermodysplasia Verruciformis associated with vertical transmission of HIV. Although most patients with HIV present infections by HPV virus, the frequency of Epidermodysplasia Verruciformis is no greater in patients with HIV because in EV there is a deficiency of specific cellular immunity to infection by some types of HPV, called HPV-related Epidermodysplasia Verruciformis.

Darier’s disease is characterized by dense keratotic lesions in the seborrheic areas of the body such as scalp, forehead, nasolabial folds, trunk and inguinal region. It is a rare genodermatosis, an autosomal dominant inherited disease that may be associated with neuropsichiatric disorders. It is caused by ATPA2 gene mutation, presenting cutaneous and dermatologic expressions. Psychiatric symptoms are depression, suicidal attempts, and bipolar affective disorder. We report a case of Darier’s disease in a 48-year-old female patient presenting severe cutaneous and psychiatric manifestations.

Systemic antibiotics have bacteriostatic and anti-inflammatory effects and are mainly indicated in cases of moderate or severe inflammatory acne, and papulopustulous and conglobate acne. Tetracyclines are the preferred choice, followed by macrolides. Other options include sulfonamides and quinolones. It is important that systemic antibiotics are selected and used according to strict criteria in order to avoid the development of super-infection and bacterial resistance.

FUNDAMENTOS: a dermatite atópica é uma doença inflamatória cutânea que apresenta múltiplos fatores desencadeantes. Há vários relatos de autores que confirmaram os aeroalérgenos como fatores agravantes ou desencadeantes. O teste de contato com aeroalérgenos ou teste de contato atópico foi proposto para avaliar a participação destes alérgenos na dermatite atópica. OBJETIVO: objetivo deste estudo foi avaliar a positividade do teste de contato atópico em pacientes com dermatite atópica. MÉTODOS: Avaliamos 50 pacientes com dermatite atópica e 45 do grupo com rinite alérgica, nos quais realizamos teste de contato atópico com extratos de Dermatophagoides pteronissynus, Dermatophagoides farinae e Blomia tropicalis, além de testes cutâneos de leitura imediata para os mesmos alérgenos, acrescidos de epitélio de cão e gato e fungos. RESULTADOS: verificamos que o teste de contato atópico com ácaros apresentou maior positividade nos indivíduos do grupo de dermatite atópica quando comparado ao grupo de rinite alérgica. CONCLUSÕES: o teste de contato atópico apresenta resultados estatisticamente significativos quando realizado com ácaros, em pacientes com dermatite atópica, com p=0,035, OR (odds ratio) = 3,35 e IC(95%) = [ 1,18; 9,47].

FUNDAMENTOS: As micoses superficiais e subcutâneas têm alta prevalência e, muitas vezes, caráter crônico, necessitando tratamentos tópicos e/ou sistêmicos com antifúngicos. As drogas de escolha são azóis e alilaminas (terbinafina). É necessário avaliar a eficácia das drogas para tratamento em humanos e em animais. Estudos para avaliar in vitro a ação dos antimicóticos são raros, especialmente, contra fungos filamentosos. OBJETIVO: Avaliar a eficácia in vitro da terbinafina pelo método de disco-difusão contra fungos filamentosos e leveduras agentes de micoses. MÉTODOS: Avaliou-se a ação da terbinafina (0,125µg-100µg) contra dez espécies fúngicas pelos métodos discodifusão e microdiluição/referência, para determinar a concentração inibitória mínima (MIC). RESULTADOS: Observou-se alta sensibilidade à terbinafina em: T. rubrum, M. gypseum, T. mentagrophytes, T. tonsurans, M. canis, C. carrionii e E. floccosum (halo ≥ 40mm com disco de 0,125µg). S. hyalinum e C. parapsilosis foram considerados sensíveis, mas com halos menores. Fusarium spp. apresentou menor sensibilidade (halo=12mm com disco de 2µg; MIC 8µg/mL). CONCLUSÕES: Os resultados reiteram estudos anteriores quanto à alta eficácia da terbinafina em relação a dermatófitos. A técnica de disco-difusão foi de fácil aplicação e adequada na rotina de laboratórios clínicos. BACKGROUND: Superficial and subcutaneous mycoses have a high prevalence and, often, chronic evolution. Therefore, they need extensive treatment with topic and/or systemic antifungal agents. Azoles and alilamines (terbinafine) are first-choice drugs to treat human and animal infections. Thus, evaluation of the efficacy of these drugs is important for a successful treatment. However, there are few studies that evaluate the in vitro activity of antifungal agents. OBJECTIVE: To evaluate the in vitro efficacy of terbinafine activity against filamentous fungi and yeasts that cause mycoses. METHOD: The in vitro activity of terbinafine (0.125-100µg) against 10 fungi species was evaluated by the diskdiffusion and microdilution/reference methods to determine the Minimum Inhibitory Concentration (MIC). RESULTS: We found a high susceptibility to terbinafine in: T. rubrum, M. gypseum, T. mentagrophytes, T. tonsurans, M. canis, C. carrionii and E. floccosum (halo ≥ 40mm with 0.125µg disk). S. hyalinum and C. parapsilosis were considered susceptible, but less than the others. Fusarium spp. showed the lowest susceptibility (halo=12mm with 2µg disk; MIC 8µg/mL). CONCLUSIONS: The results of this research confirm previous findings about the efficacy of terbinafine. The drug was shown to be highly effective to treat dermatophyte infections. The disk-diffusion method was easy to use and is a suitable technique for routine use in clinical laboratories.
