1. RoWeeder: Unsupervised Weed Mapping through Crop-Row Detection

Abstract

Precision agriculture relies heavily on effective weed management to ensure robust crop yields. This study presents RoWeeder, an innovative framework for unsupervised weed mapping that combines crop-row detection with a noise-resilient deep learning model. By leveraging crop-row information to create a pseudo-ground truth, our method trains a lightweight deep learning model capable of distinguishing between crops and weeds, even in the presence of noisy data. Evaluated on the WeedMap dataset, RoWeeder achieves an F1 score of 75.3, outperforming several baselines. Comprehensive ablation studies further validated the model's performance. By integrating RoWeeder with drone technology, farmers can conduct real-time aerial surveys, enabling precise weed management across large fields. Abstract

We present Label Anything, an innovative neural network architecture designed for few-shot semantic segmentation (FSS) that demonstrates remarkable generalizability across multiple classes with minimal examples required per class. Diverging from traditional FSS methods that predominantly rely on masks for annotating support images, Label Anything introduces varied visual prompts -- points, bounding boxes, and masks -- thereby enhancing the framework's versatility and adaptability. Unique to our approach, Label Anything is engineered for end-to-end training across multi-class FSS scenarios, efficiently learning from diverse support set configurations without retraining. This approach enables a "universal" application to various FSS challenges, ranging from $1$-way $1$-shot to complex $N$-way $K$-shot configurations while remaining agnostic to the specific number of class examples. This innovative training strategy reduces computational requirements and substantially improves the model's adaptability and generalization across diverse segmentation tasks. Our comprehensive experimental validation, particularly achieving state-of-the-art results on the COCO-$20^i$ benchmark, underscores Label Anything's robust generalization and flexibility. Abstract

A photonic integrated circuit (PIC) for the coherent combination of 32 input optical signals into a single output fiber is reported. The PIC was fabricated using a low-loss thick silicon-on-insulator (SOI) process and packaged with 32 input and 1 output fibers. The basic building block is a 2x2 Mach-Zehnder interferometer (MZI) with an external (to the MZI branches) and an internal thermal phase shifter, and a bandwidth in excess of 80 kHz. The PIC monolithically integrates 31 MZIs and 31 germanium photodetectors, and is suitable in principle for turbulence mitigation in LEO-ground and horizontal free space optics links. Improvements to the device for the coherent combination of 64 inputs and for the reduction of insertion losses are also discussed

8. Contractivity of neural ODEs: an eigenvalue optimization problem

Abstract

We propose a novel methodology to solve a key eigenvalue optimization problem which arises in the contractivity analysis of neural ODEs. When looking at contractivity properties of a one layer weight-tied neural ODE $\dot{u}(t)=\sigma(Au(t)+b)$ (with $u,b \in {\mathbb R}^n$, $A$ is a given $n \times n$ matrix, $\sigma : {\mathbb R} \to {\mathbb R}^+$ denotes an activation function and for a vector $z \in {\mathbb R}^n$, $\sigma(z) \in {\mathbb R}^n$ has to be interpreted entry-wise), we are led to study the logarithmic norm of a set of products of type $D A$, where $D$ is a diagonal matrix such that ${\mathrm{diag}}(D) \in \sigma'({\mathbb R}^n)$. Specifically, given a real number $c$ (usually $c=0$), the problem consists in finding the largest positive interval $\chi\subseteq \mathbb [0,\infty)$ such that the logarithmic norm $\mu(DA) \le c$ for all diagonal matrices $D$ with $D_{ii}\in \chi$. We propose a two-level nested methodology: an inner level where, for a given $\chi$, we compute an optimizer $D^\star(\chi)$ by a gradient system approach, and an outer level where we tune $\chi$ so that the value $c$ is reached by $\mu(D^\star(\chi)A)$. We extend the proposed two-level approach to the general multilayer, and possibly time-dependent, case $\dot{u}(t) = \sigma( A_k(t) \ldots \sigma ( A_{1}(t) u(t) + b_{1}(t) ) \ldots + b_{k}(t) )$ and we propose several numerical examples to illustrate its behaviour, including its stabilizing performance on a one-layer neural ODE applied to the classification of the MNIST handwritten digits dataset., Comment: 23 pages, 5 figures, 3 tables

9. Rhetorical Question Comprehension by Italian-German Bilingual Children

Abstract

This study investigates for the first time the comprehension of rhetorical questions (RhQs) in bilingual children. RhQs are non-canonical questions, as they are not used to request information, but to express the speaker's belief that the answer is already obvious. This special pragmatic meaning often arises by means of specific prosodic and lexical-syntactic cues. Being childhood learners, children have to acquire the concept of rhetoricity, but being bilinguals, they further need to acquire the different cues marking RhQs in their two languages. We tested 85 bilingual children (aged 6-9 years) with Italian as heritage language (HL) and German as majority language (ML) in both of their languages, using a forced-choice comprehension task. Our results show that RhQ comprehension improves with age in both languages. Bilingual children are able to exploit prosodic and syntactic cues to comprehend RhQs in their ML and HL with a slight advantage in the ML. This advantage could be either an effect of the cues used in the experiments in the two languages or of a higher proficiency in the ML. In addition, our results point to a later acquisition of prosodic rhetorical cues, which has implications for bilingual acquisition of non-canonicity in general.

Abstract

The ever-growing knowledge regarding NSCLC molecular biology has brought innovative therapies into clinical practice; however, the treatment situation in the non-metastatic setting is rapidly evolving. Indeed, immunotherapy-based perioperative treatments are currently considered the standard of care for patients with resectable NSCLC in the absence of EGFR mutations or ALK gene rearrangements. Recently, data have been presented on the use of tyrosine kinase inhibitors (TKIs) in the adjuvant and locally advanced setting for patients with NSCLC harboring such driver gene alterations. The aim of the current work is to review the available evidence on the use of targeted treatments in the non-metastatic setting, together with a summary of the ongoing trials designed for actionable gene alterations other than EGFR and ALK. To date, 3-year adjuvant osimertinib treatment has been demonstrated to improve DFS and OS and to reduce CNS recurrence in resected EGFR-mutated NSCLC in stage IB–IIIA (TNM 7th edition). The use of osimertinib after chemo-radiation in stage III unresectable EGFR-mutated NSCLC showed the relevant PFS improvement. In the ALK-positive setting, 2-year alectinib treatment was shown to clearly improve DFS compared to adjuvant standard chemotherapy in resected NSCLC with stage IB (≥4 cm)–IIIA (TNM 7th edition). Several trials are ongoing to establish the optimal adjuvant TKI treatment duration, as well as neoadjuvant TKI strategies in EGFR- and ALK-positive disease, and (neo)adjuvant targeted treatments in patients with actionable gene alterations other than EGFR or ALK. In conclusion, our review depicts how the current treatment scenario is expected to rapidly change in the context of non-metastatic NSCLC with actionable gene alterations, hence appropriate molecular testing from the early stages has become crucial to establish the most adequate approaches both in the perioperative and the locally advanced disease.

Abstract

Introduction Recurrent shoulder dislocations often cause attrition of the labrum and progressive loss of the anterior bony contour of the glenoid. Treatment options for this pathology involve either soft tissue repair or bony augmentation procedure. The optimal management for patients with shoulder instability with subcritical bone loss remains unknown and current clinical practice is highly varied.Methods and analysis The Shoulder instability Trial comparing Arthroscopic stabilisation Benefits compared with Latarjet procedure Evaluation (STABLE) is an ongoing multicentre, randomised controlled trial of 114 patients diagnosed with recurrent anterior shoulder instability and subcritical glenoid bone loss (10%–20%, measured on 3D CT using circle of best fit technique). Patients are randomised either arthroscopic capsuloligamentous repair (Bankart repair+remplissage) or open or arthroscopic coracoid transfer (Latarjet procedure). The primary outcome of this trial will be the between-group difference in the change from baseline to 24 months postintervention in Western Ontario Shoulder Instability Index scores. Secondary outcomes include: (1) rates of recurrent shoulder dislocations and symptoms of instability up to 24 months’ postsurgery; (2) clinical outcomes measured by American Shoulder and Elbow Society score, Shoulder Activity Scale, EQ-5D and Patient Satisfaction Scale; (3) physical examination (range of motion, stability); (4) return to previous level of activity/sport; (5) rate of shoulder-related complications and serious adverse events.Ethics and dissemination This protocol has been reviewed and approved by the Hamilton Integrated Research Ethics Board (HiREB; project number 15998) prior to commencement of the trial. Results from the study will be submitted for publication in a peer-reviewed journal regardless of whether there are statistically significant findings.Trial registration number NCT05705479; this study was prospectively registered on clinicaltrials.gov.

Abstract

BackgroundStroke is one of the major chronic non-communicable diseases (NCDs) with high morbidity, disability and mortality. The key to preventing stroke lies in controlling risk factors. However, screening risk factors and quantifying stroke risk levels remain challenging.MethodsA novel prediction model for stroke risk based on two-level feature selection and deep fusion network (SRPNet) is proposed to solve the problem mentioned above. First, the two-level feature selection method is used to screen comprehensive features related to stroke risk, enabling accurate identification of significant risk factors while eliminating redundant information. Next, the deep fusion network integrating Transformer and fully connected neural network (FCN) is utilized to establish the risk prediction model SRPNet for stroke patients.ResultsWe evaluate the performance of the SRPNet using screening data from the China Stroke Data Center (CSDC), and further validate its effectiveness with census data on stroke collected in affiliated hospital of Jining Medical University. The experimental results demonstrate that the SRPNet model selects features closely related to stroke and achieves superior risk prediction performance over benchmark methods.ConclusionsSRPNet can rapidly identify high-quality stroke risk factors, improve the accuracy of stroke prediction, and provide a powerful tool for clinical diagnosis.

Abstract

Lesion segmentation is a crucial step of the radiomic workflow. Manual segmentation requires long execution time and is prone to variability, impairing the realisation of radiomic studies and their robustness. In this study, a deep-learning automatic segmentation method was applied on computed tomography images of non-small-cell lung cancer patients. The use of manual vs automatic segmentation in the performance of survival radiomic models was assessed, as well. METHODS A total of 899 NSCLC patients were included (2 proprietary: A and B, 1 public datasets: C). Automatic segmentation of lung lesions was performed by training a previously developed architecture, the nnU-Net, including 2D, 3D and cascade approaches. The quality of automatic segmentation was evaluated with DICE coefficient, considering manual contours as reference. The impact of automatic segmentation on the performance of a radiomic model for patient survival was explored by extracting radiomic hand-crafted and deep-learning features from manual and automatic contours of dataset A, and feeding different machine learning algorithms to classify survival above/below median. Models' accuracies were assessed and compared. RESULTS The best agreement between automatic and manual contours with DICE=0.78 +(0.12) was achieved by averaging predictions from 2D and 3D models, and applying a post-processing technique to extract the maximum connected component. No statistical differences were observed in the performances of survival models when using manual or automatic contours, hand-crafted, or deep features. The best classifier showed an accuracy between 0.65 and 0.78. CONCLUSION The promising role of nnU-Net for automatic segmentation of lung lesions was confirmed, dramatically reducing the time-consuming physicians' workload without impairing the accuracy of survival predictive models based on radiomics.

Abstract Background SMYD3 has been found implicated in cancer progression. Its overexpression correlates with cancer growth and invasion, especially in gastrointestinal tumors. SMYD3 transactivates multiple oncogenic mechanisms, favoring cancer development. Moreover, it was recently shown that SMYD3 is required for DNA restoration by promoting homologous recombination (HR) repair. Methods In cellulo and in vivo models were employed to investigate the role of SMYD3 in cancer chemoresistance. Analyses of SMYD3-KO cells, drug-resistant cancer cell lines, patients’ residual gastric or rectal tumors that were resected after neoadjuvant therapy and mice models were performed. In addition, the novel SMYD3 covalent inhibitor EM127 was used to evaluate the impact of manipulating SMYD3 activity on the sensitization of cancer cell lines, tumorspheres and cancer murine models to chemotherapeutics (CHTs). Results Here we report that SMYD3 mediates cancer cell sensitivity to CHTs. Indeed, cancer cells lacking SMYD3 functions showed increased responsiveness to CHTs, while restoring its expression promoted chemoresistance. Specifically, SMYD3 is essential for the repair of CHT-induced double-strand breaks as it methylates the upstream sensor ATM and allows HR cascade propagation through CHK2 and p53 phosphorylation, thereby promoting cancer cell survival. SMYD3 inhibition with the novel compound EM127 showed a synergistic effect with CHTs in colorectal, gastric, and breast cancer cells, tumorspheres, and preclinical colorectal cancer models. Conclusions Overall, our results show that targeting SMYD3 may be an effective therapeutic strategy to overcome chemoresistance.

Abstract

Pasireotide-LAR is recommended as a second-line treatment for patients with acromegaly. Although the effects of pasireotide-LAR have been well characterized in clinical studies, real-practice evidence is scant, especially in the long term and within the individualization of therapy in patients with comorbidities. To provide additional insight on the individualized approach to acromegaly management, six clinical cases of complex acromegaly treated with pasireotide-LAR for more than 5 years were reported. Pasireotide-LAR allowed the normalization of insulin-like growth factor 1 (IGF1) values in all patients and reduced tumour residue volume where present. A good safety profile and long-term tolerability were also reported.

Abstract

Abstract Background Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann’s procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Abstract

BackgroundSkeletal fragility is characterized by increased frequency of vertebral fractures (VFs) in acromegaly. Several trials were conducted to identify modifiable risk factors and predictors of VFs, with limited data on the prognostic role of GH receptor (GHR) isoforms. In this study, we investigated the potential role of GHR polymorphism on the occurrence of incidental VFs (i-VFs), in patients treated with second-line medical therapies.MethodsA longitudinal, retrospective, observational study was conducted on a cohort of 45 acromegalic patients not-responsive to first-generation somatostatin receptor ligands (fg-SRLs) and treated with GHR antagonist (Pegvisomant) or with the second-generation SRLs (Pasireotide long-acting release).ResultsSecond line treatments were Pegvisomant plus fg-SRLs in 26 patients and Pasireotide LAR in 19 patients. From the group treated with fg-SRLs+Peg-V, the fl-GHR isoform was identified in 18 patients (69.2%) and the d3-GHR isoform in 8 patients (30.8%). I-VFs arose exclusively in fl-GHR isoform carriers (p=0.039). From the group treated with Pasireotide LAR, the fl-GHR isoform was identified in 11 patients (57.9%), and the d3-GHR isoform in 8 patients (42.1%). I-VFs arose exclusively in d3-GHR isoform carriers (p=0.018). Patients with fl-GHR isoform had a higher risk for i-VFs if treated with fg-SRL+Peg-V (OR: 1.6 95%IC: 1.1-2.3, p=0.04), and a lower risk if treated with Pasi-LAR (OR: 0.26 IC95%: 0.11-0.66, p=0.038).ConclusionsOur data support a predictive role of the GHR isoforms for the occurrence of i-VFs in acromegalic patients treated with second-line drugs, tailored to the individual patient. The knowledge of the GHR polymorphism may facilitate the choice of second-line therapies, improving the therapeutic approach, in the context of personalized medicine.

Abstract

HIV persistence in the brain is a barrier to cure, and potentially contributes to HIV-associated neurocognitive disorders. Whether HIV transcription persists in the brain despite viral suppression with antiretroviral therapy (ART) and is subject to the same blocks to transcription seen in other tissues and blood, is unclear. Here, we quantified the level of HIV transcripts in frontal cortex tissue from virally suppressed or non-virally suppressed people with HIV (PWH). HIV transcriptional profiling of frontal cortex brain tissue (and PBMCs where available) from virally suppressed (n = 11) and non-virally suppressed PWH (n = 13) was performed using digital polymerase chain reaction assays (dPCR). CD68+ myeloid cells or CD3+ T cells expressing HIV p24 protein present in frontal cortex tissue was detected using multiplex immunofluorescence imaging. Frontal cortex brain tissue from PWH had HIV TAR (n = 23/24) and Long-LTR (n = 20/24) transcripts. Completion of HIV transcription was evident in brain tissue from 12/13 non-virally suppressed PWH and from 5/11 virally suppressed PWH, with HIV p24+CD68+ cells detected in these individuals. While a block to proximal elongation was present in frontal cortex tissue from both PWH groups, this block was more extensive in virally suppressed PWH. These findings suggest that the brain is a transcriptionally active HIV reservoir in a subset of virally suppressed PWH.

Abstract Aim To develop and psychometrically test an instrument to assess nurses' evidence‐based knowledge and self‐efficacy regarding insertion and management of venous access devices (short peripheral catheter (SPC), long peripheral catheter/midline (LPC) and PICC) and the management of totally implantable central venous catheter (Port) in adult patients. Design Multicenter cross‐sectional observational study with questionnaire development and psychometric testing (validity and reliability). Methods An evidence‐based instrument was developed including a 34‐item knowledge section and an 81‐item self‐efficacy section including four device‐specific parts. Nineteen experts evaluated content validity. A pilot study was conducted with 86 nurses. Difficulty and discrimination indices were calculated for knowledge items. Confirmatory factor analyses tested the dimensionality of the self‐efficacy section according to the development model. Construct validity was tested through known group validity. Reliability was evaluated through Cronbach's alpha coefficient for unidimensional scales and omega coefficients for multidimensional scales. Results Content validity indices and results from the pilot study were excellent with all the item‐content validity indices >0.78 and scale‐content validity index ranging from 0.96 to 0.99. The survey was completed by 425 nurses. Difficulty and discrimination indices for knowledge items were acceptable with most items (58.8%) showing desirable difficulty and most items (58.8%) with excellent (35.3%) or good (23.5%) discrimination power, and appropriate to the content. The dimensionality of the model posited for self‐efficacy was confirmed with adequate fit indices (e.g., comparative fit index range 0.984–0.996, root mean square error of approximation range 0.054–0.073). Construct validity was determined and reliability was excellent with alpha values ranging from 0.843 to 0.946 and omega coefficients ranging from 0.833 to 0.933. Therefore, a valid and reliable tool based on updated guidelines is made available to evaluate nurses’ competencies for venous access insertion and management.

Abstract

A systematic review (SR) is a research method for synthesizing evidence on a specific topic. Among the various types of systematic reviews, there are SRs of guidelines (CPGs) and SRs of SRs. Traditionally, they are limited to just one type of secondary evidence. This paper introduces an innovative SR methodology that combines CPGs and SRs to improve evidence synthesis and overcome the limitations of isolated use.Essential steps that should always precede the actual research process include registering the research protocol, formulating research questions and setting inclusion/exclusion criteria. Using the PRISMA protocol for comprehensive database searches, it's crucial to combine keywords with boolean operators and remove duplicates. The eligibility of studies should be assessed by selecting potentially relevant articles through an initial screening of titles and abstracts, followed by a meticulous analysis of the full-texts. Rigorous evidence evaluation tools, such as AGREE II for CPGs and AMSTAR 2 for SRs, and the double reviewer approach ensure high-quality selections. Additionally, converting summarized results into percentages and applying statistical analyses facilitate interpretation and improve the reliability of rater assessments. A further characteristic of this methodology is its adaptability to the evolution of healthcare research.

Abstract

In-network function offloading using programmable data plane languages like P4 offers computational resource savings and efficient operations at the network edge. However, the deployment of ML functions in P4~switches exploiting no additional hardware remains a challenge. In this paper, we tackle the challenges in deploying Deep Neural Networks (DNNs) within programmable network devices, introducing a novel distillation based on Look-Up Tables (LUTs). The proposed method maps quantized DNNs into a cascaded arrangement of LUTs without loss in accuracy and independently of the quantized network depth. The developed approach is demonstrated in two network function use cases: a cyber security use case focused on mitigating Distributed Denial of Service attacks and a malicious activity classification task in IoT Networks. Experimental results show a trade-off between model’s accuracy, expressed in terms of F1-Score and the computational demands, influenced by bit size and number of LUTs. In addition, the latency for deploying these models ranged from 54ns to 112ns, showing the method's practical applicability in network functions.

Abstract

Background: Quadriceps muscle layer thickness (QMLT), which is measured using ultrasound, is an emerging strategy to identify sarcopenia. Purpose: The purpose of the study was to assess whether pre-operative QMLT values are associated with a prolonged length of stay (LOS; defined as >14 days) following a renal transplant. Methods: Between March 2019 and January 2020, we performed a prospective study among patients undergoing renal transplantation. Physical Frailty scores and QMLT measurements were performed pre-operatively. The primary outcome was a greater LOS following transplant. Secondary outcomes included complications and renal function. Statistical analysis: Percentiles divided patients into two categories of QMLT (low and high). Continuous outcomes were compared using a two-sided t-test or Mann–Whitney U test, and Chi-square analysis and Fisher exact testing were used for nominal variables. Results: Of 79 patients, the frailty prevalence was 16%. Among patients with low and higher QMLTs, LOS of >14 days were 21% vs. 3% [p = 0.04], respectively. Demographically, there was a higher percentage of patients with living donors in the high- vs. low-QMLT groups (40 vs. 7%). However, in a subgroup analysis excluding living-donor recipients, the difference between groups was preserved (23% vs. 0%, p = 0.01). No differences in secondary outcomes were seen between groups. Conclusions: Low quadriceps muscle layer thickness may be associated with a prolonged length of stay for renal recipients. Further research is needed to confirm our findings.
