Este trabalho tem como objetivo analisar a relação ou o lugar da alteridade na dinâmica psíquica das psicoses a partir das ideias de Freud, sobretudo aquelas vinculadas ao conceito de narcisismo, e que influenciaram as teorias de Tausk e Federn, autores que se aventuraram no campo do estudo e tratamento das psicoses no momento em que o próprio pai da psicanálise dizia ser impossível. Deste estudo podemos concluir que a alteridade concreta se fez e se faz presente no psiquismo psicótico para os três autores, de modo que o conflito e o processo terapêutico implicam a existência e a relação com o outro. This paper analyzes the relation or place of otherness in the psychic dynamics of psychosis based on Freud’s ideas, especially those that are related to the concept of narcissism and that influenced the theories developed by Tausk and Federn, authors who ventured in the field of study and treatment of psychosis at a time when the very father of psychoanalysis declared that this was not feasible. We conclude that the concrete otherness occurred and is present in the psychotic psyche according to the three authors, so that the conflict and the therapeutic process imply the existence and the relationship with the other. Cet article vise à analyser le rapport ou le lieu de l’altération dans la dynamique psychique de la psychose à partir des idées de Freud, en particulier celles liées à la notion de narcissisme et qui ont influencé les théories de Tausk et Federn, auteurs qui se sont aventurés dans le domaine de l’étude et le traitement de la psychose alors que le père même de la psychanalyse jugeait cela irréalisable. Nous concluons que l’altérité concrète s’est opérée et est présente dans la psyché psychotique selon les trois auteurs, de sorte que le conflit et le processus thérapeutique impliquent l’existence et le rapport avec l’autre. Este artículo pretende analizar la relación o el lugar de la alteridad en la dinámica psíquica de la psicosis a partir de las ideas de Freud, especialmente de aquellas vinculadas al concepto de narcicismo, y que influenciaron a las de Tausk y Federn, autores que se aventuraron en el campo del estudio y del tratamiento de la psicosis en un momento en el que el propio padre del psicoanálisis decía que era imposible. A partir de este estudio, podemos concluir que, para los tres autores, la alteridad concreta se hizo y se hace presente en la psique psicótica, de forma que el conflicto y el proceso terapéutico implican la existencia y la relación con el otro. Diese Arbeit analysiert die Beziehung oder den Ort der Alterität in der psychischen Dynamik der Psychose, basierend auf Freuds Ideen, die mit dem Konzept des Narzissmus in Verbindung stehen und die Tausk und Federn beeinflusst haben. Diese Autoren haben sich in den Bereich der Studie und Behandlung von Psychosen gewagt und zwar zu einer Zeit, als der Vater der Psychoanalyse behauptete, dass dies ein unmögliches Unterfangen sei. Aus unserer Studie schließen wir, dass die konkrete Alterität in der psychotischen Psyche gemäß diesen Autoren vorhanden ist, so dass der therapeutische Konflikt und Prozess die Existenz des Andern und die Beziehung mit ihm einschließen.

La gestione integrata del sistema di controllo interno e gestione dei rischi negli intermediari ‘minori’: assetto organizzativo e meccanismi operativi

Riassunto: Nel sito archeologico in località Cuccurada, in territorio comunale di Mogoro, le ricerche hanno evidenziato un insediamento pluristratificato, con fasi di occupazione del Neolitico recente (cultura di Ozieri), dell’Eneolitico (cultura di Monte Claro) e delle Età del Bronzo e del Ferro, con un riutilizzo dell’area in epoca romana e medievale. Il complesso è già abbastanza noto in letteratura, ma ancora parzialmente inedito per quanto riguarda i materiali e i dati di scavo. Nel presente contributo si vuole presentare nel dettaglio le diverse fasi di occupazione del sito, soprattutto in relazione ai momenti finali della frequentazione protostorica (Bronzo Finale/I Ferro) ed alla rioccupazione del complesso in età romana e medievale. Abstract: In the archaeological site of Cuccurada, in the territory of Mogoro, the researches have evidenced a pluristratified settlement, with phases of occupation of the recent Neolithic (culture of Ozieri), of the Eneolithic age (culture of Monte Claro) and of the Bronze and Iron Ages, with a re-use of the area in roman and medieval period. The complex is already enough famous in literature, but still partially unknown regarding the materials and the excavation data. In the present work we want to present in detail the different occupation phases of the site, especially in relationship to the final moments of the proto-historic frequentation (Final Bronze/I Iron Age) and to the last re-use of the complex in Roman and medieval age., Layers. The construction of Brazilian's public policy on mental health is a result of the organization of the Psychiatric Reform Movement, criticizing the assistance offered, which is based on the principles of the traditional organicist psychiatry. This kind of assistance made the violation of the basic and essential rights of people with psychic suffering possible for a long time. Therefore, the new public policy compromises socially with ensuring users respect and citizenship. Considering the importance of implementing actions that ensure this compromise, this research sought to understand the normative elements that support the concept of modern citizenship and its relationship to the processes of struggle and conflict in favor of the statement and guarantee rights of people with psychological distress. And thus identify in what extent actions practiced by the policy on mental health in the town of Maringá-PR, represented by two assistance services (CAPS AD e CAPS II), has been contributing to the building and ensuring of users citizenship. In this regard, bibliographic and documental research on the subject was conducted as well as semi-structured interviews with samples of users of the already mentioned services, with the managers of the mains specialized assistance services and with the coordinator of the town's policy. It was concluded that quality psychiatric assistance based on welcome and bond is the main contribution of the services to ensure the users citizenship. It was also concluded that some challenges need to be overcome to ensure users full citizenship, the main ones being: increased professional qualification in order to avoid the reproduction of discrimination as well as authority and guardianship relationships in the services; the establishment of intersectoral actions with other public polices so that all rights can be accessed. A construção da política pública de saúde mental brasileira é consequência da organização do Movimento de Reforma Psiquiátrica, em crítica à assistência oferecida com base nos princípios da psiquiatria organicista tradicional. Essa forma de assistência possibilitou que as pessoas com sofrimento psíquico fossem, por longo período, violadas em seus direitos básicos e essenciais. Por isso, a nova política de assistência assume o compromisso social de garantir o respeito e a cidadania dos usuários. Considerando a importância da implementação de ações que assegurem esse compromisso, essa pesquisa procurou compreender os elementos normativos que dão sustentação ao conceito de cidadania moderna e sua relação com os processos de lutas e conflitos em prol da afirmação e garantia de direitos da pessoa com sofrimento psíquico. E, assim, identificar em que medida às ações desenvolvidas pela política de saúde mental do município de Maringá-PR, representada por dois serviços de assistência (CAPS AD e CAPS II), tem contribuído para a construção/garantia da cidadania de seus usuários. Para isso, foi realizada uma pesquisa bibliográfica e documental sobre a temática, e entrevistas semi-estruturadas com amostra de usuários dos serviços já citados, com as gestoras dos principais serviços de assistência especializada e com a coordenadora da política de saúde mental municipal. Como resultado, conclui-se que a maior contribuição dos serviços para a garantia da cidadania dos usuários se refere à assistência psiquiátrica de qualidade, pautada no acolhimento e no vinculo. E que alguns desafios precisam ser superados para a garantia da cidadania plena dos usuários, sendo os principais: maior capacitação dos profissionais, a fim de que discriminações e relações de autoridade e tutela não sejam reproduzidas nos serviços, e o estabelecimento de ações intersetoriais com as demais políticas públicas, para que todos os direitos sejam acessados. 174 f

We performed a dynamic susceptibility contrast magnetic resonance imaging (DSC-MRI) analysis to study the role of the demographic/clinical information on perfusion parameters between patients with schizophrenia and normal control subjects. 39 schizophrenia patients and 27 normal controls were studied with a Siemens 1.5T magnet. PWI images were obtained following intravenous injection of paramagnetic contrast agent (gadolinium-DTPA). For each perfusion parameter, i.e. relative cerebral blood flow (rCBF), relative cerebral blood volume (rCBV), mean transit time (MTT) and time-to-peak (TTP), the best predictor model was computed in left and right frontal cortex following a stepwise strategy. First of all, a linear model, including all the sociodemographic information and clinical variables as predictors was computed. At each step, the least significant predictor was excluded and a new linear model was evaluated until all predictors were excluded. Then, the best predictor model was selected based on the F statistic value and on the p value. The models for the rCBF and the rCBV both in the left and right frontal cortex were estimated independently from each other, and the best models contained the same predictors, i.e. clinical state, age, and length of illness. No significant models were obtained for the MTT and the TTP. This study showed a decrease in rCBF and rCBV frontal cortex values in subject affected by schizophrenia. Future DSC-MRI studies should further investigate the role of cerebral perfusion for the pathophysiology of the disease by recruiting first-episode patients and by considering cerebellar, parietal and temporal regions.
