Introduction and Objectives: Subjects without cardiovascular (CV) disease (CVD) may suffer from subclinical atherosclerosis, and are at increased risk for atherosclerotic CV events (ASCVE). The ESC/EAS risk SCORE was updated by SCORE2, which estimates 10-year risk of fatal and non-fatal CVD in European populations aged 40–69 years without established CVD or diabetes. Our aim was to compare the two ESC/EAS risk scores and to validate SCORE2 in our population. Methods: A total of 1071 individuals (age 57.2±6.1 years; 75.2% male) without CVD or diabetes, from GENEMACOR study controls, were analyzed over 5.4±3.9 years. The population was stratified into risk categories according to the two scores, and the area under the ROC curve (AUC) and Harrell's C-index assessed the scores’ performance. Calibration was performed using the goodness-of-fit test, and occurrence of the first event assessed by Cox regression. Kaplan–Meier analysis estimated SCORE2 survival. Results: SCORE stratified subjects into four risk categories: low (7.4%), moderate (46.5%), high (25.3%) and very high (20.8%), and SCORE2 into three: low-to-moderate (24.7%), high (59.0%) and very high (16.2%). SCORE presented good discrimination for CV mortality (AUC=0.838; C-index=0.834, 95% CI: 0.728–0.940), as did SCORE2 for total CV events (AUC=0.744; C-index=0.728, 95% CI: 0.648–0.808). Calibration did not show a disparity between observed and expected ASCVE. The probability of ASCVE was eight times higher in very-high-risk SCORE2 (p=0.001), and three times in the high-risk group (p=0.049). O esgoto contém grande diversidade de microrganismos, incluindo patógenos como os vírus, sendo, portanto, um meio propício para identificação da circulação de doenças infectocontagiosas – técnica conhecida como wastewater-based epidemiology (WBE). A WBE se mostra uma poderosa ferramenta de saúde pública para rastrear e sinalizar a situação epidemiológica da população, visto que as partículas virais do SARS-CoV-2 podem ser expelidas nas fezes e urina dos contaminados antes mesmo do surgimento de sintomas e do diagnóstico clínico de COVID-19. Este estudo apresenta uma revisão de literatura sobre a presença de SARS-CoV-2 e outros vírus no esgoto doméstico e as possibilidades de remoção em estações de tratamento de esgoto (ETEs) convencionais e avançadas, bem como as técnicas de concentração e detecção em amostras ambientais. Ao final do artigo, foi apresentada uma análise econômica da aplicabilidade da WBE como instrumento de predição epidemiológica da COVID-19 em um município brasileiro.

PurposeThe purpose of this study is to evaluate the interference of the continuous use of drug classes in the expression of biomarkers during the first week of hospitalization and in the prognosis of patients with COVID-19.MethodsThe patients diagnosed with COVID-19 and confirmed with SARS-CoV-2 by RT-qPCR assay underwent the collection of fasting whole blood samples for further analysis. Other data also extracted for this study included age, sex, clinical symptoms, related comorbidities, smoking status, and classes of continuous use. Routine serum biochemical parameters, including alanine aminotransferase, aspartate aminotransferase, lactate dehydrogenase, C-reactive protein, N-terminal fragment of B-type natriuretic peptide, and cardiac troponin, were measured.ResultsIn this cross-sectional study, a total of 176 patients with COVID-19 hospitalizations were included. Among them, 155 patients were discharged (88.5%), and 21 patients died (12%). Among the drug classes evaluated, we verified that the continuous use of diuretic 4.800 (1.853–11.67) (p = 0.0007) and antihypercholesterolemic 3.188 (1.215–7.997) (p = 0.0171) drug classes presented a significant relative risk of death as an outcome when compared to the group of patients who were discharged. We evaluated biomarkers in patients who used continuous antihypercholesterolemic and diuretic drug classes in the first week of hospitalization. We observed significant positive correlations between the levels of CRP with cardiac troponin (r = 0.714), IL-6 (r = 0.600), and IL-10 (r = 0.900) in patients who used continuous anticholesterolemic and diuretic drug classes and were deceased. In these patients, we also evaluated the possible correlations between the biomarkers AST, NT-ProBNP, cardiac troponin, IL-6, IL-8, and IL-10. We observed a significantly negative correlations in AST levels with NT-ProBNP (r = −0.500), cardiac troponin (r = −1.00), IL-6 (r = −1.00), and IL-10 (r = −1.00) and a positive correlation with IL-8 (r = 0.500). We also observed significant negative correlation in the levels of NT-ProBNP with IL-10 (r = −0.800) and a positive correlation with cardiac troponin (r = 0.800). IL-6 levels exhibited positive correlations with cardiac troponin (r = 0.800) and IL-10 (r = 0.700).ConclusionIn this study, we observed that hospitalized COVID-19 patients who continued using anticholesterolemic and diuretic medications showed a higher number of correlations between biomarkers, indicating a poorer clinical prognosis. These correlations suggest an imbalanced immune response to injuries caused by SARS-CoV-2.

Introduction: Coronary artery disease (CAD), characterized by an atherogenic process in the coronary arteries, is one of the leading causes of death in Madeira. The GENEMACOR (GENEs in MAdeira and CORonary Disease) study sought to investigate the main risk factors – environmental and genetic – and estimate whether a genetic risk score (GRS) improves CAD prediction, discrimination and reclassification. Methods: Traditional risk factors and 33 CAD genetic variants were considered in a case–control study with 3139 individuals (1723 patients and 1416 controls). The multivariate analysis assessed the likelihood of CAD. A multiplicative GRS (mGRS) was created, and two models (with and without mGRS) were prepared. Two areas under receiver operating characteristic curve (area under curve (AUC)) were analyzed and compared to discriminate CAD likelihood. Net reclassification improvement (NRI) and integrated discrimination index (IDI) were used to reclassify the population. The diversity of juçara palm (Euterpe edulis) must be investigated at the molecular level to enable the adoption of appropriate management strategies that may associate breeding studies with conservation. With the aim of describing the genetic diversity and structure of a population of E. edulis in a commercial plantation area, the genotyping of 199 individuals was performed using nine nuclear microsatellite markers. We detected inbreeding in the population and the diversity within the plots was high showing diversity among the individuals. The individuals from matrices germplasm of the locality Santa Marta, were divergent. The diversity indices observed in the studied population were close to those reported in the literature for natural populations; thus, the ex situ conservation of the population in this managed is feasible, as is their potential use in breeding studies.

O Transtorno do Espectro do Autismo (TEA) é um transtorno do neurodesenvolvimento, cuja etiologia não apresenta fatores bem definidos com evidências de predisposição genética, mas com diagnóstico clínico. Apresenta-se atualmente como questão de saúde pública em virtude da alta prevalência. A intervenção precoce especializada e intensiva é determinante para um bom prognóstico. Daí a importância de que os médicos realizem triagem de indicadores do Transtorno do Espectro Autista para encaminhamento à estimulação e acesso aos direitos. A nota técnica do MEC (2013) orienta os sistemas de ensino sobre o TEA para uma formação em saúde que contemple conhecimentos acerca da deficiência, e as Diretrizes Curriculares Nacionais do Curso de Graduação em Medicina também alinham-se a essa orientação. Tendo em vista este cenário, objetivou-se documentar e compreender a presença de conteúdos sobre TEA nos currículos de graduação médica em Minas Gerais. Foram analisados Projetos Pedagógicos, matrizes curriculares e ementários de 29 cursos mineiros. Os conteúdos teórico-metodológicos foram organizados entre psicopatologia/psiquiatria e saúde da criança/pediatria. A análise documental indica a ausência de especificação de tais conteúdos nos currículos de medicina, ou seja, a ênfase para esta questão de saúde pública ainda requer esforços para mudar a educação médica.

Introduction: Asian Countries contribute more than 55% to the world’s diabetic population. Compared with the European population, Asians develop diabetes with lower thresholds of body mass index and abdominal perimeter and at earlier ages. Material and Methods: An observational, cross-sectional study was conducted based on a population of patients with diabetes of Asian origin enrolled in the USF Rainha D. Amélia. These patients were selected through the MIM@UF® program using the codes of the international primary health classification: T90 and T89. Results: The population obtained consisted of 20 patients with Type 2 Diabetes, originary in Asian countries. At diagnosis the patients had a mean of 42.6 years, and in 30% (n = 6) the diagnosis was established before the age of 40 years. The mean duration of diabetes was 6.75 years. Pancreatic anti-islet antibodies were determined in 10 users, all with negative results, as well as the C-peptide values, which were within the normal range. There were statistically significant differences for total and LDL cholesterol in patients with glycated hemoglobin> 7% when compared to the group with lower values. Discussion: Asian populations have a substantial risk of developing diabetes compared to other populations. In addition they develop the disease earlier and with lower body mass index. One possible reason for this difference is that, compared to Caucasians, Asians have more visceral adiposity, which contributes to lipotoxicity, insulin resistance and diabetes development.

Resumo O processo de envelhecimento é um dos fatores que, através da deterioração do equilíbrio, influenciam o aumento do número de quedas e resultam em sérias complicações, como o medo de cair. O Tai Chi Chuan é uma arte marcial milenar oriental considerada um exercício de intensidade leve a moderada. A prática do Tai Chi Chuan na população idosa apresenta efeitos positivos no controle do equilíbrio, na incidência de quedas e no medo de cair. Sendo assim, o objetivo deste estudo foi realizar uma revisão sistemática que evidenciasse o efeito do Tai Chi Chuan na incidência de quedas, no medo de cair e no equilíbrio em idosos. Foram pesquisados ensaios clínicos aleatórios nas bases de dados Medline, Lilacs, Cochrane e PEDro. Dois revisores avaliaram a qualidade dos estudos e realizaram a extração dos dados de forma independente. Foram selecionados seis artigos, que abrangeram 816 participantes. Uma análise dos resultados demonstrou uma resposta positiva do Tai Chi Chuan na redução da incidência de quedas e no medo de cair em idosos sedentários e que vivem na comunidade. O mesmo efeito não foi observado em relação à melhora da estabilidade postural.

Evidence points epicardial adipose tissue (EAT) as an emerging cardiovascular risk marker. Whether genetic polymorphisms linked with atherosclerosis are associated with higher EAT is still unknown. We aim to assess the role of genetic burden of atherosclerosis and its association to EAT in a cohort of asymptomatic individuals without coronary disease. A total of 996 participants were prospectively enrolled in a single Portuguese center. EAT volume was measured by Cardiac Computed Tomography and participants were distributed into 2 groups, above and below median EAT. SNPs were genotyped and linked to their respective pathophysiological axes. A multiplicative genetic risk score (mGRS) was constructed, representing the genetic burden of the studied SNPs. To evaluate the association between genetics and EAT, we compared both groups by global mGRS, mGRS by functional axes, and SNPs individually. Individuals above-median EAT were older, had a higher body mass index (BMI) and higher prevalence of hypertension, metabolic syndrome, diabetes, and dyslipidemia. They presented higher GRS, that remained an independent predictor of higher EAT volumes. The group with more EAT consistently presented higher polymorphic burden across numerous pathways. After adjustment, age, BMI, and mGRS of each functional axis emerged as independently related to higher EAT volumes. Amongst the 33 SNPs, MTHFR677 polymorphism emerged as the only significant and independent predictor of higher EAT volumes. Patients with higher polymorphism burden for atherosclerosis present higher EAT volumes. We present the first study in a Portuguese population, evaluating the genetic profile of EAT through GWAS and GRS, casting further insight into this complicated matter.

Abstract The inclusion of a genetic risk score (GRS) can modify the risk prediction of coronary artery disease (CAD), providing an advantage over the use of traditional models. The predictive value of the genetic information on the recurrence of major adverse cardiovascular events (MACE) remains controversial. A total of 33 genetic variants previously associated with CAD were genotyped in 1587 CAD patients from the GENEMACOR study. Of these, 18 variants presented an hazard ratio >1, so they were selected to construct a weighted GRS (wGRS). MACE discrimination and reclassification were evaluated by C-Statistic, Net Reclassification Index and Integrated Discrimination Improvement methodologies. After the addition of wGRS to traditional predictors, the C-index increased from 0.566 to 0.572 (p=0.0003). Subsequently, adding wGRS to traditional plus clinical risk factors, this model slightly improved from 0.620 to 0.622 but with statistical significance (p=0.004). NRI showed that 17.9% of the cohort was better reclassified when the primary model was associated with wGRS. The Kaplan-Meier estimator showed that, at 15-year follow-up, the group with a higher number of risk alleles had a significantly higher MACE occurrence (p=0.011). In CAD patients, wGRS improved MACE risk prediction, discrimination and reclassification over the conventional factors, providing better cost-effective therapeutic strategies.

Most cyanobacteria produce extracellular polymeric substances (EPS) that fulfill different biological roles depending on the strain/environmental conditions. The interest in the cyanobacterial EPS synthesis/export pathways has been increasing, not only to optimize EPS production but also to efficiently redirect carbon flux toward the production of other compounds, allowing the implementation of industrial systems based on cyanobacterial cell factories.

O presente artigo objetiva discutir alguns pressupostos epistemológicos e pedagógicos que sustentam a parceria entre o Instituto Ayrton Senna e a Secretaria Municipal de Educação do Rio de Janeiro para a implementação de uma política pública destinada a estudantes em situação de fracasso escolar. A discussão emerge da imersão de bolsistas de um projeto de pesquisa e extensão em uma turma de aceleração de educação básica do Projeto Acelera Brasil. O entrelaçamento entre o percurso proporcionado por essa experiência, os estudos realizados pelo grupo de pesquisa em torno dos cotidianos escolares e dos currículos neles criados e as narrativas dos bolsistas coloca em suspeição a eficácia e possibilidade de pacotes prontos replicáveis em escala para solucionar a questão do fracasso escolar.

Abstract Introduction The bone-anchored hearing aid (BAHA) is a bone conduction system that transmits the sound directly to the inner ear by surpassing the skin impedance and the subcutaneous tissue. It is indicated for patients with mixed, conductive and unilateral sensorineural hearing loss who did not benefit from conventional hearing aids (HAs). Although the benefits from BAHA are well demonstrated internationally, this field still lacks studies in Brazil. Objective To assess the auditory rehabilitation process in BAHA users through audiological, speech perception and tinnitus aspects. Methods Individuals with hearing loss were assessed before and after the implantation. The participants were subjected to pure tone audiometry in free field, functional gain audiometry, speech perception tests, tinnitus handicap inventory (THI) in open format, and to the visual analog scale (VAS). Results It was found that the participants benefited from the use of BAHA. The difference in the performance of the participants before and after the BAHA surgery was significant in terms of hearing acuity. There was no statistically significant difference in the speech perception tests. The tinnitus assessment showed that 80% of the participants scored slight tinnitus severity in THI after using a BAHA. Eighty percent of the participants classified their tinnitus as absent to mild in the VAS after the surgery. Conclusion Based on the results of the current study, we can conclude that the participants improved both the auditory perception and the tinnitus handicap.

Abdominal surgery and the confection of a stoma causes trauma to the abdominal muscles. The quality of life and the adaptation of the person with ostomy are conditioned by complications that may arise in the postoperative period. Among the complications of ostomies, parastomal hernias are those with a higher incidence, making it essential to develop specifically targeted interventions for their prevention. The UK Association of Stoma Care Nurses in 2016(1) recommended that patients with a stoma undergo an appropriate abdominal exercise program after surgery to strengthen the abdominal muscles and reduce the risk of hernia. The teaching / learning process of the patients with a stoma should begin in the preoperative period, in order to achieve a more rapid adaptation to lifestyle modifications, thus ensuring a more effective management of the therapeutic regimen and, consequently, a better quality of life.

The use of the versatile cyanobacterial extracellular polymeric substances (EPS) for biotechnological/biomedical applications implies an extensive knowledge of their biosynthetic pathways to improve/control polymer production yields and characteristics. The multiple copies of EPS-related genes, scattered throughout cyanobacterial genomes, adds another layer of complexity, making these studies challenging and time-consuming. Usually, this issue would be tackled by generating deletion mutants, a process that in cyanobacteria is also hindered by the polyploidy. Thus, the use of the CRISPRi multiplex system constitutes an efficient approach to addressing this redundancy. Here, three putative Synechocystis sp. PCC 6803 kpsM homologues (slr0977, slr2107, and sll0574) were repressed using this methodology. The characterization of the 3-sgRNA mutant in terms of fitness/growth and total carbohydrates, released and capsular polysaccharides, and its comparison with previously generated single knockout mutants pointed towards Slr0977 being the key KpsM player in Synechocystis EPS production. This work validates CRISPRi as a powerful tool to unravel cyanobacterial complex EPS biosynthetic pathways expediting this type of studies.

RESUMO Objetivos trata-se de revisão sistemática da literatura científica sobre a associação entre o acidente vascular cerebral, desidratação e disfagia orofaríngea. Estratégia de pesquisa o levantamento bibliográfico foi realizado nas bases de dados científicos: MEDLINE, LILACS, SciELO, Web of Science e Cochrane. Critérios de seleção foram incluídos os estudos que preencheram os seguintes critérios de inclusão: ser artigo original, com resumo disponível; ter sido publicado entre os anos de 2001 e 2018 e nos idiomas português, inglês ou espanhol; abordar o tema desidratação em indivíduos após acidente vascular cerebral. A triagem e análise dos estudos foram realizadas por dois avaliadores independentes. Resultados dentre os 484 artigos localizados para a triagem, 43 foram selecionados para leitura completa e 18 foram incluídos na análise final. Foram descritos diferentes métodos de avaliação do estado de hidratação nos indivíduos após acidente vascular cerebral, tais como: análise da relação BUN /creatinina, osmolaridade plasmática, relação ureia/creatinina, gravidade específica da urina, coloração da urina, ingestão hídrica, balanço hídrico, bioimpedância elétrica, avaliação clínica, análise de eletrólitos isolados. A prevalência de desidratação em pacientes após acidente vascular cerebral, durante a internação, variou de 11% a 66% e está associada à gravidade e piora na evolução clínica. Conclusão foi possível compreender a complexidade do processo de mensuração do estado de hidratação em indivíduos após acidente vascular cerebral e sua associação com a disfagia. Estudos enfocando essa temática são de extrema relevância, visto a sua influência sobre a taxa de mortalidade e morbidade nesta população.

Apresentar as manifestações de crianças com Diabetes mellitus tipo 1 participantes em uma sessão de Brinquedo Terapêutico Instrucional-BTI sobre a insulinoterapia. Estudo de Caso qualitativo, com análise temática indutiva, realizado em um acampamento de diabetes em 2014, com participação de duas crianças em idade escolar em sessão de BTI. As manifestações permitiram compreender que o domínio da insulinoterapia é um assunto complexo para a faixa etária, e demanda reforço contínuo visando a adaptação cognitiva da criança. A sessão de BTI oportunizou identificar o conhecimento prévio e as necessidades de aprendizagem sobre a conservação e técnica de injeção de insulina das crianças, com uso de linguagem adequada como o brincar. O BTI foi instrumento facilitador para identificar necessidades educacionais, além de sua ação assistencial. Seu uso reforça o potencial do lúdico na promoção do autocuidado relacionado à insulinoterapia da criança com diabetes.

Abstract Many cyanobacteria produce extracellular polymeric substances (EPS) mainly composed of heteropolysaccharides with unique characteristics that make them suitable for biotechnological applications. However, manipulation/optimization of EPS biosynthesis/characteristics is hindered by a poor understanding of the production pathways and the differences between bacterial species. In this work, genes putatively related to different pathways of cyanobacterial EPS polymerization, assembly, and export were targeted for deletion or truncation in the unicellular Synechocystis sp. PCC 6803. No evident phenotypic changes were observed for some mutants in genes occurring in multiple copies in Synechocystis genome, namely ∆wzy (∆sll0737), ∆wzx (∆sll5049), ∆kpsM (∆slr2107), and ∆kpsM∆wzy (∆slr2107∆sll0737), strongly suggesting functional redundancy. In contrast, Δwzc (Δsll0923) and Δwzb (Δslr0328) influenced both the amount and composition of the EPS, establishing that Wzc participates in the production of capsular (CPS) and released (RPS) polysaccharides, and Wzb affects RPS production. The structure of Wzb was solved (2.28 Å), revealing structural differences relative to other phosphatases involved in EPS production and suggesting a different substrate recognition mechanism. In addition, Wzc showed the ATPase and autokinase activities typical of bacterial tyrosine kinases. Most importantly, Wzb was able to dephosphorylate Wzc in vitro, suggesting that tyrosine phosphorylation/dephosphorylation plays a role in cyanobacterial EPS production.

Objetivo: Conhecer as percepções da equipe de enfermagem no manejo junto ao paciente psiquiátrico em medida de segurança. Metodologia: Este estudo trata-se de uma pesquisa exploratória descritiva, de caráter qualitativo, realizado com 10 integrantes da equipe de enfermagem, atuantes em um hospital de custódia. A coleta de dados realizou-se de Março a Maio de 2017, por meio de entrevista e roteiro semiestruturado. Resultados: Os resultados foram submetidos à análise de conteúdo a partir do critério de saturação de dados, e permitiram elencar três categorias: A história do cotidiano dos profissionais de enfermagem, referente aos pacientes em medida de segurança; As condições de trabalho em um hospital de custodia e tratamento psiquiátrico e A vivência da equipe de enfermagem frente ao manejo junto a pacientes psiquiátricos em medida de segurança. A partir da análise dos resultados das entrevistas, o estudo pretende ajudar a garantir a melhoria na qualidade da assistência de enfermagem levando em consideração principalmente a saúde física e mental dos profissionais, devido à exposição diária a diversos fatores de risco. Conclusão: A equipe de enfermagem forense tem papel fundamental na assistência junto ao paciente psiquiátrico em medida de segurança, e para isso, deve estar em equilíbrio com sua saúde física e mental. Este estudo visa contribuir para a reflexão das questões relacionadas a qualidade de vida e na saúde dos trabalhadores de saúde mental forense, de modo a reduzir os fatores que contribuem negativamente no exercício profissional.

ABSTRACT INTRODUCTION: The cochlear implant device has the capacity to measure the electrically evoked compound action potential of the auditory nerve. The neural response telemetry is used in order to measure the electrically evoked compound action potential of the auditory nerve. OBJECTIVE: To analyze the electrically evoked compound action potential, through the neural response telemetry, in children with bilateral cochlear implants. METHODS: This is an analytical, prospective, longitudinal, historical cohort study. Six children, aged 1-4 years, with bilateral cochlear implant were assessed at five different intervals during their first year of cochlear implant use. RESULTS: There were significant differences in follow-up time (p = 0.0082) and electrode position (p = 0.0019) in the T-NRT measure. There was a significant difference in the interaction between time of follow-up and electrode position (p = 0.0143) when measuring the N1-P1 wave amplitude between the three electrodes at each time of follow-up. CONCLUSION: The electrically evoked compound action potential measurement using neural response telemetry in children with bilateral cochlear implants during the first year of follow-up was effective in demonstrating the synchronized bilateral development of the peripheral auditory pathways in the studied population.

Glyphosate is the most widely used herbicide worldwide. It may reach water bodies and affect microbial communities. We compared the effects of two commercial formulations and technical-grade glyphosate on a freshwater periphytic community. A laboratory bioassay was carried out with four treatments: technicalgrade glyphosate acid, Glifosato II Atanor®, Roundup Max®, at a concentration of 3 mg active ingredient/L, and Control. At 2, 5 and 9 days after herbicide application, pigments concentration, dry weight, ash-free dry weight, and periphytic algal density were determined. An increase of Cyanobacteria and a decrease of Chlorophyta and Bacillariophyta abundances were registered in all treatments with the herbicide. This effect was greater when Roundup Max® and Glifosato II Atanor® were added, and was lower with technical-grade glyphosate, suggesting that additives in the commercial formulations may enhance glyphosate effects. https://doi.org/10.25260/EA.19.29.1.0.816

Objetivo: Esse estudo objetivou avaliar as características demográficas, clínicas e os desfechos da infecção por tuberculose (TB) após o transplante renal (TxR). Métodos: Estudo de coorte retrospectiva, incluindo receptores de TxR de um único centro de transplantes localizado no Nordeste do País. Foram considerados para análise os pacientes com diagnósticos realizados no período de 2004 a 2018 (n=33). Resultados: Os pacientes da coorte eram predominantemente homens (54,5%), adultos jovens (39,8+12,3 anos), com mediana de 29,8 meses de transplante. Nove fizeram tratamento para TB latente (LTBI). O regime imunossupressor principal foi indução com Thymoglobuline (48,4%) em associação com tacrolimo e ácido micofenólico (MPA) (72,7%). O sintoma inicial mais frequente foi febre (63,6%), seguido da perda ponderal (21,2%) e tosse (21,2%). Vinte e quatro pacientes tiveram TB localizada, cinco tiveram formas disseminadas e quatro pacientes receberam tratamento empírico sem definição de sítio. Todos foram submetidos ao tratamento com rifampicina, isoniazida e pirazinamida, com ou sem etambutol. O tempo médio de tratamento foi de 8,4+3,8 meses. Sete pacientes apresentaram hepatotoxicidade e dois apresentaram neuropatia periferica. A concentração de tacrolimo reduziu de 5,4 +1,9 ng/mL pré-tratamento para 1,6+0,6 ng/mL pós-tratamento (menor concentração), sendo a dose aumentada em 75,5% durante o tratamento. Doze pacientes apresentaram suspensão parcial ou completa da imunossupressão durante o tratamento. 79% dos pacientes evoluíram com lesão renal aguda (LRA), sendo, de acordo com o estadiamento KDIGO, 15 pacientes eram KDIGO 1, 8 eram KDIGO 2 e 3 eram KDIGO 3, e a principal causa foi LRA pré-renal. Três pacientes morreram no curso da doença ou do tratamento. Conclusões: A TB aconteceu tardiamente no curso do transplante e febre foi o principal sintoma inicial. Houve um elevado percentual de formas extrapulmonares. Como reportado em estudos prévios, o tratamento empírico é uma realidade no contexto do transplante. O tratamento associou-se à redução da concentração do inibidor de calcineurina por provável interação com a rifampicina. Foi elevado o percentual de disfunção aguda do enxerto renal.

Introdução: Pacientes com DPOC geralmente experimentam uma diminuição na qualidade de vida relacionada à saúde (QVRS) a partir da percepção própria de que se sentem limitados em algum aspecto de suas vidas. Objetivo: Analisar as produções científicas que abordam o uso de questionários de qualidade de vida para acompanhar evolução de doença em portadores de Doenças Pulmonares Obstrutivas Crônicas. Métodos e Materiais: Revisão Não Sistemática da literatura incluindo artigos nos idiomas inglês, português e espanhol, a partir das bases de dados Medline, SciELO e Lilacs, no período de 1998 a 2017, além de livros técnicos que versam sobre o assunto. Foram utilizados os termos: “chronic obstructive pulmonary disease”, “quality of life”, “specific health-related quality of life questionnaires”. Resultados/Síntese dos dados: Os 39 artigos encontrados foram organizados em duas seções, a saber: estudos de validação de questionários de qualidade de vida (n=22) e estudos de comparação entre instrumentos genéricos e específicos para avaliação de qualidade de vida (n=17). Conclusões: A avaliação da qualidade de vida deve ser incorporada ao seguimento clínico, uma vez que a doença crônica repercute nas diversas dimensões da vida dos pacientes. Os questionários específicos de qualidade de vida dão uma melhor dimensão das condições de saúde no portador de DPOC no que se refere a sua relação com a doença.

ABSTRACT DNA fingerprinting based on SSR amplification profiles was applied to native species of Piper from the Atlantic Forest to compare the utility of this type of molecular marker with the morphological characters traditionally applied in Piper taxonomy and identification. Fifty-one SSR markers developed for four species of Piper native to Asia and Mesoamerica were applied to 16 species, together with 63 morphological characters, for species characterization. Molecular and morphological data were analysed by cluster analysis, followed by a cluster sharpness test and the construction of a heat map to visualize the association of characters with species groups. A multivariate regression tree determined the number of loci needed for species identification. Forty-five primers were transferable to at least four species. Molecular data were more efficient in detecting sharp groups than morphological data. Species groups delimited by a set of shared morphological characters were differentiated based on molecular data. The sixteen studied species could be separated by nine primers, demonstrating the cross-species transferability of SSR markers and the usefulness of DNA fingerprinting for both the delimitation and the identification of species of Piper.

Several alternatives have been considered to minimize the environmental impact caused by conventional polymers. This study aims to evaluate the chemical, mechanical and morphological properties of flexible thermoplastic films of cassava and corn starch, using glycerol and lignin as plasticizers and reinforcement. The films were produced through casting. It was observed that the presence of lignin effectively increased the maximum stress and the elastic modulus by about of 840% and 4200%, respectively, when comparing to the film containing only glycerol. In addition, lignin improved thermal properties, modified some structural properties and made the surface of the material rougher.

The interest in examining the subset of proteins present in the extracellular milieu, the exoproteome, has been growing due to novel insights highlighting their role on extracellular matrix organization and biofilm formation, but also on homeostasis and development. The cyanobacterial exoproteome is poorly studied, and the role of cyanobacterial exoproteins on cell wall biogenesis, morphology and even physiology is largely unknown. Here, we present a comprehensive examination of the Anabaena sp. PCC 7120 exoproteome under various growth conditions. Altogether, 139 proteins belonging to 16 different functional categories have been identified. A large fraction (48%) of the identified proteins is classified as “hypothetical”, falls into the “other categories” set or presents no similarity to other proteins. The evidence presented here shows that Anabaena sp. PCC 7120 is capable of outer membrane vesicle formation and that these vesicles are likely to contribute to the exoproteome profile. Furthermore, the activity of selected exoproteins associated with oxidative stress has been assessed, suggesting their involvement in redox homeostasis mechanisms in the extracellular space. Finally, we discuss our results in light of other cyanobacterial exoproteome studies and focus on the potential of exploring cyanobacteria as cell factories to produce and secrete selected proteins.

A goiabeira (Psidium guajava L.) está entre as espécies cultivadas da família Myrtaceae possuindo grande importância econômica e social. A espécie possui alta diversidade genética, no qual é favorecida pela polinização cruzada e uso de sementes oriundas de progenitores heterozigotos na produção de mudas. Neste trabalho a diversidade genética da progênie da cultivar Paluma foi avaliada. Para tanto marcadores microssatélites foram avaliados em plantas oriundas de sementes de frutos grandes e pequenos da cultivar Paluma que coexiste em um pomar com outras 12 cultivares e 10 genótipos melhorados. Foi verificada diversidade genética na progênie. Os marcadores amplificaram 18 alelos, a heterozigosidade esperada foi de 0,57 e a observada de 0,86 e o índice de fixação foi negativo (-0,52), indicando excesso de heterozigotos. Os marcadores não acessaram diferenças genéticas entre progênies oriundas de frutos grandes e pequenos. A diversidade na progênie é importante para o melhoramento da espécie.

Dentre as mais importantes frutas no cenário brasileiro e mundial, destaca-se a goiaba. Sabe-se que o genótipo doador de pólen pode influenciar nas condições física e química dos frutos, por isso, é relevante entender mais amplamente as características relacionadas à polinização. Nesse trabalho, propuseram-se como objetivos determinar a taxa de autopolinização e polinização cruzada e inferir sobre um possível doador preferencial de pólen para o genótipo Paluma. Para isso, foram coletadas folhas de 23 genótipos (possíveis pais) presentes em um pomar comercial no município de Linhares, Espírito Santo. Foram também coletados 15 frutos grandes e 15 pequenos do genótipo Paluma, extraídas as sementes (filhos), colocadas para germinar. Em seguida, foi realizado teste de paternidade, utilizando marcadores microssatélites, comparando os genótipos das plântulas obtidas dos frutos com os possíveis polinizadores presentes no pomar. Verificou-se 100% de fecundação cruzada. O genótipo Século XXI foi identificado como doador preferencial de pólen, com 72,73% e 94,44% de paternidade, para filhos oriundos de frutos grandes e pequenos, respectivamente. Os dados obtidos nesse trabalho revelam uma futura possibilidade de seleção de genótipos para composição de um pomar e planejamento de cruzamentos, a fim de aumentar a produtividade e qualidade dos frutos.

O descarte de medicamento feito pela população é um tema que vem sendo cada vez mais discutido, Há uma crescente preocupação, pois a grande maioria da população não tem o conhecimento das consequências ambientais e nem das consequências à saúde pública que o descarte incorreto de medicamentos pode causar, Surgem ao redor do país, algumas iniciativas privativas e isoladas apresentando soluções à problemática, oferecendo à população estrutura para o descarte correto e informação sobre o tema, Por ainda não haver uma legislação nacional efetiva sobre o assunto, os estados lançam leis, regulamentando a problemática em seus territórios, No Ceará, foi sancionada em 2012 a lei no 15.192, obrigando às farmácias, drogarias e distribuidoras de medicamentos a recolherem os medicamentos da população, porém, devido à realidade local, não está sendo totalmente cumprida, Com a intensificação de discussões sobre as iniciativas particulares e as legislações locais, com o envolvimento de vários setores da sociedade e com a troca de experiências de programas em funcionamento, torna-se possível impulsionar projetos em locais onde a população ainda não tem oportunidade de dar a destinação correta aos medicamentos em desuso, Esse artigo traz à discussão a ocorrência de resíduos de medicamentos no meio ambiente, aspectos relacionados aos avanços na legislação brasileira quanto ao manejo de resíduos de medicamentos, às propostas de implantação de programas de recolhimento de medicamentos no país e alguns de seus desafios.

Introdução: A Síndrome de Alstrom é uma doença muito rara, causada pela mutação no gene Obesidade infantil; ALMS1, que apresenta uma degeneração progressiva das funções sensoriais, resultando em de-Diabetes mellitus tipo 2; ficiências visuais e auditivas, além de distúrbios metabólicos como obesidade na infância, hipe-Retinite pigmentosa rinsulinemia e diabetes tipo II. Objetivo: Apresentar o perfil audiométrico de dois irmãos da mesma família afetados pela Síndrome de Alström. Método: Estudo prospectivo, analítico descritivo, os pacientes afetados foram submetidos a um questionário previamente testado, audiometria tonal e vocal seriadas, análise de emissões otoacústicas, e de respostas de potencial evocado auditivo de tronco encefálico, além de análise genético-molecular para comprovação diagnóstica. Resultados: Ambos os pacientes apresentaram perda auditiva bilateral com o início na infância e progressão lenta para perda auditiva neurosensorial severa no primeiro caso e, profunda, no segundo. As emissões otoacústicas estavam ausentes, e o potencial evocado auditivo de tronco encefálico estava normal em ambos os pacientes, bilateralmente. Conclusão: A Síndrome de Alström apresenta início precoce de perda auditiva neurossensorial, antes da adolescência, 10 a 20 anos para desenvolver perda auditiva severa a profunda. A lesão auditiva é essencialmente coclear, de acordo com os resultados dos testes de emissões otoacústicas e de potenciais evocados auditivos de tronco encefálico.

Myrsine coreacea tem se destacado na medicina popular por apresentar diferentes fitoquímicos em suas folhas e caules. A aplicação de compostos naturais extraídos de plantas com propriedades medicinais ou alelopáticas pode reduzir efeitos colaterais em relação à saúde humana, quando comparados com moléculas sintéticas. Mas para ser utilizado com segurança a ação dos fitoquímicos precisa ser testada. Neste trabalho objetivou-se avaliar a instabilidade genética, por meio de marcadores ISSR, induzida pelo extrato aquoso de Myrsine coriacea nos modelos Lactuca sativa e Sorghum bicolor. Utilizou-se como tratamentos quatro diferentes concentrações do extrato (100%, 50%, 25%, 12,5%) e o controle negativo foi água. Verificou-se mudança nos padrões de amplificação entre os tratamentos e o controle. A concentração 100% se manteve isolado e geneticamente distante das demais nos dois bioensaios. Os resultados sugerem ação mutagênica do extrato aquoso de M. coreacea sob as duas espécies modelos, Lactuca sativa e Sorghum bicolor.

Resumo: Psidium guineense Swartz pertence à família Myrtaceae, oferece potencial para exploração comercial e em virtude das propriedades nutricionais e farmacêuticas desperta a atenção da indústria. Entretanto, por ser uma espécie poliplóide, estudos de diversidade genética são escassos, dado a dificuldade das análises. O objetivo deste estudo foi avaliar a diversidade genética de duas populações P. guineense em dois municípios do estado do Espírito Santo, utilizando sete marcadores microssatélites. Todos os locos analisados foram polimórficos com média de 11,14 por locus. O método UPGMA agrupou os genótipos em dois grupos principais, diferenciando as populações das duas localidades. Dois índividuos de Águia Branca alocaram-se separadamente dos demais, demonstrando divergência genética. Os marcadores SSR utilizados neste estudo demonstraram eficiência na detecção de polimorfismos moleculares, revelando que houve grande diversidade genética entre os indivíduos das duas populações amostradas, sendo importantes para o melhoramento e conservação da espécie.

O presente texto promove um diálogo entre o que intenta a Proposta de Lei Escola Sem Partido (que visa combater o que chama de doutrinação ideológica nas escolas e universidades) com as noções que vêm sendo tecidas pelo campo de estudos do currículo. Ao trazermos a compreensão dos currículos como criação cotidiana, dos professorxs e alunxs como autores dos seus processos de ensinoaprendizagem e da produção do conhecimento como tessitura em rede evidencia-se que um currículo neutro, não ideológico e apolítico é impraticável, assim como é impossível que os professorxs aprendam a separar seus valores morais, políticos e ideológicos dos conteúdos, pois nem mesmo os conteúdos escolares previstos nos currículos formais o são. Além da discussão epistemológica, são trazidas narrativas docentes como um recurso que nos ajuda a compreender a indissociabilidade entre as múltiplas dimensões (fazer/sentir/pensar) das quais está embebida a prática docente. Diante de tal impossibilidade percebemos que aqueles que promovem a ideia de uma escola neutra, ou possuem uma ingenuidade político-epistemológica ou, na verdade, maquiam um projeto de Escola de Um Único Partido: o hegemônico, que toma como verdade absoluta os conhecimentos validados pela alta cultura e, portanto, não relativiza o status quo. Concluímos que, em qualquer uma dessas possibilidades, devemos combater o projeto Escola Sem Partido assegurando o respeito mútuo e a expressão efetiva do direito à diferença nas nossas políticas curriculares educacionais, pois lecionar por um projeto de cidadania não pode ser crime!

BACKGROUND:Schistosomal myeloradiculopathy (SMR), the most severe and disabling ectopic form of Schistosoma mansoni infection, is caused by embolized ova eliciting local inflammation in the spinal cord and nerve roots. The treatment involves the use of praziquantel and long-term corticotherapy. The assessment of therapeutic response relies on neurological examination. Supplementary electrophysiological exams may improve prediction and monitoring of functional outcome. Vestibular evoked myogenic potential (VEMP) triggered by galvanic vestibular stimulation (GVS) is a simple, safe, low-cost and noninvasive electrophysiological technique that has been used to test the vestibulospinal tract in motor myelopathies. This paper reports the results of VEMP with GVS in patients with SMR. METHODS:A cross-sectional comparative study enrolled 22 patients with definite SMR and 22 healthy controls that were submitted to clinical, neurological examination and GVS. Galvanic stimulus was applied in the mastoid bones in a transcranial configuration for testing VEMP, which was recorded by electromyography (EMG) in the gastrocnemii muscles. The aim of the study was to investigate the association of the physical activity domains and types of physical activity practiced during leisure time with the occurrence of stress in workers. This is a cross-sectional survey, conducted from 2006 to 2008, with a representative sample of industrial workers (n = 47, 477; 69% men). Data were collected via questionnaire. For statistical analysis, a Poisson regression with adjustment for robust variance was used in Stata 13.0. Women who were physically active during leisure time (19.9%; IC95%: 19.0−20.7) had lower occurrences of stress than those who were physically inactive in this domain (14.2%; IC95%: 13.3−15.2). Among men, those active at home (11.6; IC95%:11.1−12.0 vs. 12.9%; IC95%:12.4−13.5), during leisure time (10.2%; IC95%: 9.8−10.6 vs. 15.1%; IC95%: 14.4−15.7), and at work (11.7%; IC95%: 11.3−12.1 vs. 13.3%; IC95%: 12.6−14.0) had lower occurrences of stress than their peers. As the number of domains that contained physical activity increased, stress occurrence tended to decrease for both sexes. The types of leisure time physical activities associated with a lower occurrence of stress in women were sports (11.1%), gymnastics/weight lifting (13.2%), and dance (14.5%); similar results were observed for men who engaged in jogging (9.0%), sports (9.7%), and walking (10.1%). Physical activity in the leisure time domain, for women and men workers, including sports practice among women and jogging among men, were associated with lower stress occurrences.

Thyroid cancer has a multifactorial aetiology resulting from the interaction of genetic and environmental factors. Several low penetrance susceptibility genes have been identified but their effects often vary between different populations. Somatic point mutations and translocations of the REarranged during Transfection (RET) proto-oncogene are frequently found in thyroid cancer. The aim of this case-control study was to determine the effect of four well known RET single nucleotide polymorphisms (SNPs) on the risk for differentiated thyroid carcinoma. A total of 545 Portuguese patients and 543 controls were genotyped by PCR and restriction enzyme analysis, for the following SNPs: G691S (exon 11, rs1799939 G/A), L769L (exon 13, rs1800861 T/G), S836S (exon 14, rs1800862 C/T), and S904S (exon 15, rs1800863 C/G). The minor allele of S836S was overrepresented in patients with papillary thyroid carcinoma (PTC) when compared to controls (OR 1.57; 95% CI 1.05-2.35; p = 0.026). The GGTC haplotype was also overrepresented in PTC (OR 2.51; 95% CI 1.07-5.91; p = 0.029). No associations were found in follicular thyroid carcinoma (FTC). Multivariate logistic regression analysis showed no differences regarding gender, age at diagnosis, lymph node or distant metastasis. However, a near significant overrepresentation of the minor alleles of G691S and S904S was found in patients with tumours greater than 10 mm of diameter at diagnosis. These data suggest that the RET S836S polymorphism in exon 14 and the GGTC haplotype are risk factors for PTC, but not FTC, and that the G691S/S904S polymorphisms might be associated with tumour behaviour.

Purpose: To evaluate changes in color and translucency of stained and glazed monolithic lithium disilicates and zirconia after toothbrushing with conventional or whitening dentifrices followed by thermocycling. Materials and Methods: Samples (n = 20/material) from CAD/CAM (IPS e.max CAD [EC]) and pressable (IPS e.max Press [EP]) lithium disilicates and 5Y-PSZ zirconia (Prettau 4 Anterior [PA]) were prepared and received staining and glazing procedures. Aging was performed by toothbrushing with conventional (n = 10) or whitening (n = 10) dentifrices, followed by thermocycling simulating 5, 10, and 15 years. Color coordinates L*, a*, b*,, and C* and hº values were assessed with a spectrophotometer at baseline and after each aging period to calculate color changes (ΔE00), lightness (ΔL*), and translucency parameter (TP). Data were analyzed using three-way repeated measures ANOVA's followed by Sidak post-hoc test (α = .05). Results: When comparing the dentifrices, EC and EP produced the greatest ΔE00 after 15 years with the whitening dentifrice (P < .05). Regardless of the dentifrice, PA showed greater ΔE00 than EC after 10 years (P < .05). When comparing the aging periods, EC and PA did not show differences in ΔE00 for both dentifrices (P ≥ .05). EC and EP produced an increased ΔL* after 15 years in the whitening group (P < .05). In the whitening groups, the ΔL* values of PA differ (P < .005) from those of lithium disilicates. Regardless of the experimental group, the TP values followed the inequality: PA < EC < EP (P < .05). Conclusions: Overall, the color of stained and glazed monolithic lithium disilicates and zirconia changed under aging with greater effects for whitening dentifrice.
