The tyrosine kinase Inhibitor (TKI) imatinib is approved for the treatment of the chronic phase of chronic myeloid leukemia (CP-CML). Pharmacokinetic studies have highlighted the importance of inter-patient variability on imatinib plasma trough concentrations (ima[C]min). In the OPTIM-imatinib trial, we demonstrated that therapeutic drug monitoring (TDM) is able to improve the molecular response of CP-CML patients treated with imatinib. Here, we analyzed the constitutional exomes and RNAseq data of these patients. We performed an association analysis between the constitutional genetic variants of the patients and their ima[C]min, measured after 12 weeks of treatment with 400 mg once daily. Using linear regression, we identified 50 SNPs that showed excess heterozygosity depending on the ima[C]min. Ten SNPs were from non-coding sequences, and among the 40 remaining, 30 (from 25 genes) could be split into two categories. The first group of 16 SNPs concerns genes encoding extracellular matrix, cell junction, and membrane proteins. Coincidentally, cell adhesion proteins were also identified by RNA-seq as being overexpressed in patients with high ima[C]min. The other group of 14 SNPs were from genes encoding proteins involved in transcription/translation. Although most of the SNPs are intronic variants (28), we also identified missense (3), synonymous (4), 5′/3′ (2), splicing (1), and upstream (4) variants. A haplotype analysis of four genes showed a significant association with high ima[C]min. None of the SNPs were significantly associated with the response. In conclusion, we identified a number of ima[C]min-associated SNPs, most of which correspond to genes encoding proteins that could play a role in the diffusion and transit of imatinib through membranes or epithelial barriers. Background. Knowing how health problems are perceived is an element of primary importance for health systems, since it is the first step for the population to recognize a disease and demand medical attention. Objectives. To develop and evaluate the implementation of a new healthcare model adapted to the necessities and illness self-perception of a community in the urban area of Greater La Plata, Buenos Aires, Argentina. Material and methods. The community’s self-rated general health status and illness self-perception were explored by a survey, followed by an intervention study performed on 2,430 participants randomly divided in 2 groups (Intervention: received personalized home health care; Control: health care was provided from hospitals and primary care health services). Results. Almost 70% of the participants were satisfied with their health status. In cases where a disease was detected; older people and men shown to perceive their illness later compared to women and young adults. Only 21% of the population had suitable access to a health service. Compared with the Control Group, regular and personalized home visits significantly improved treatment adherence (43% vs 94%), reduced annual undesirable events (stroke, myocardial infarcts, etc.) associated to chronic diseases (2.7% vs 1.2%) and lowered hospitalization rates (2.9% vs 2.0%). Conclusions. The data obtained showed that the population had low illness awareness, which reduced the spontaneous demand of care in health services. A model of health care based on regular home visits demonstrated efficacy in terms of disease prevention, illness control, avoiding consequences of chronic disease and reducing hospitalizations. Various spectroscopic and electrochemical methods characterize two products obtained from the potential-controlled electrochemical oxidation of 2,6-dihydroxynaphthalene. 1H NMR chemical shifts of peracetyl derivatives are vital for the structural analysis of the products. The naphthalene rings are linked at the 1- and 5-positions. Solubility and spectroscopic data reveal the absence of cross-linking between chains.

Abstract Endometrial Regenerative Cells (ERC) are a population of mesenchymal-like stem cells having pluripotent differentiation activity and ability to induce neoangiogenesis. In vitro and animal studies suggest ERC are immune privileged and in certain situations actively suppress ongoing immune responses. In this paper we describe the production of clinical grade ERC and initial safety experiences in 4 patients with multiple sclerosis treated intravenously and intrathecally. The case with the longest follow up, of more than one year, revealed no immunological reactions or treatment associated adverse effects. These preliminary data suggest feasibility of clinical ERC administration and support further studies with this novel stem cell type.

Benzodiazepines and “Z-drugs” (BZD/Z) are overprescribed in many countries. This study evaluates their consumption in a social security sector health insurance provider with national coverage in Argentina. With a descriptive and observational approach, outpatient dispensations of BZD/Zs were analyzed for people over 18 years old from April 2020 to March 2021, disaggregated by sex, age, active ingredient, and half-life. An annual prevalence of use of 11.6% was found among the 431,445 adult affiliates, with higher rates in women and in those over age 60. Overall consumption of BZD/Zs was 77.6 defined daily doses (DDD) per 1000 enrollee-days. The average user received 5.1 annual dispensations and the equivalent of 1.4 DDD for each day of the year. BZD/Zs with long half-life were the most used. We found high levels of BZD/Z consumption and for longer periods than recommended. It is necessary to improve the quality of consumption and reduce the negative impact of inappropriate use of these drugs among treated individuals. [ABSTRACT FROM AUTHOR]
