Background: Vitamin E has antioxidant properties, which help in scavenging free radicals, thereby reducing oxidation of lipids and proteins. This study aims to evaluate the efficacy of oral vitamin E supplementation in preventing retinopathy of prematurity (ROP) in very low birth weight (VLBW) infants with respiratory distress syndrome (RDS) and decreasing oxidative stress 15 and 28 days post-intervention. Methods: Ninety VLBW infants were randomly assigned to two groups: 1. Treatment (treatment group (T), n = 48) or 2. Placebo (control group (C), n = 42). Each group received 25 IU of vitamin E (T) or placebo (C). Results: The incidence of ROP in groups T and C was 12.5% (n=6) and 31% (n = 13), respectively (RR: 0.40; 95% CI: 0.10–0.96). There were no differences in mortality between groups. As expected, the vitamin E concentration was significantly increased 28 days post-intervention in group T. Conclusion: Oral supplementation with vitamin E may effectively prevent ROP development in VLBW infants with RDS. Oxidative damage markers were significantly lower, whereas total antioxidant capacity was increased in group T. However, levels of other antioxidants as vitamin A and C were not measured in two groups.

Background: During pregnancy, vitamin D requirements are higher due to fetal growth and development. Vitamin D production occurs mainly through sunlight exposure, which is affected by geographic location and lifestyle factors. Methods: This was a case-control study nested within two cohorts of adult pregnant women (n = 298): urban (Mexico City) and rural (Cuetzalan). To reduce confounding, pairs were selected by age, pregestational body mass index, and pregnancy trimester. Generalized linear models were used to assess the two groups according to their vitamin D status. Results: A total of 298 adult women were studied: 149 from a rural area and 149 from an urban area. Vitamin D deficiency and insufficiency were observed in 28% and 38.2% of women, respectively. A trend for higher 25(OH)D concentrations was observed in women from the rural area (27.5 ng/mL vs. 25.8 ng/mL), probably related to the type of job, where women with partial jobs showing less probability of having vitamin D deficiency (OR = 0.26, CI = 0.06–1.16, p = 0.08) and vitamin D insufficiency (OR = 0.24, CI = 0.06–0.99, p = 0.05). Women whose Last Menstrual Period occurred in spring showed lower vitamin D concentration compared to those whose LMP occurred in winter (p &lt, 0.01). Conclusions: A high prevalence of vitamin D deficiency was observed in both rural and urban areas. Women living in rural areas tended to have higher 25(OH)D concentrations, probably related to more sunlight exposure associated with their type of job.

Background Prenatal phthalate exposures may affect processes that underlie offspring cardiometabolic health, but findings from studies examining these associations are conflicting. We examined associations between biomarkers of phthalate exposures during pregnancy with child lipid and adipokine levels. Methods Data were from 463 mother-child pairs in the PROGRESS cohort of Mexico City. We quantified 15 phthalate metabolites in 2nd and 3rd trimester maternal urine samples and created an average pregnancy measure using the geometric mean. We evaluated the 15 metabolites as nine biomarkers, including four metabolite molar sums. We measured fasting serum triglycerides, non-HDL cholesterol, leptin, and adiponectin in children at the six-year follow-up visit (mean = 6.8 years). We estimated associations using linear regression, Bayesian kernel machine regression (BKMR), and weighted quantile sum (WQS) and assessed effect modification by sex. Results In BKMR and WQS models, higher concentrations of the total mixture of phthalate biomarkers were associated with lower triglycerides (β = −3.7% [-6.5, −0.78] per 1 unit increase in WQS biomarker index) and non-HDL cholesterol (β = −2.0 [-3.7, −0.25] ng/ml per increase in WQS biomarker index). Associations between individual biomarkers and child outcomes were largely null. We observed some evidence of effect modification by child sex for mono-3-carboxypropyl phthalate (β = 19.4% [1.26, 40.7] per doubling of phthalate) and monobenzyl phthalate (β = −7.6% [-14.4, -0.23]) in girls for adiponectin. Conclusions Individual prenatal phthalate biomarkers were not associated with child lipid or adipokine levels. Contrary to our hypothesis, the total phthalate mixture was associated with lower child triglycerides and non-HDL cholesterol.

Objective We identified clinical, dietary, and socioeconomic factors associated with insufficient gestational weight gain among Mexican pregnant women with human immunodeficiency virus (HIV) infection. Methods This was a cross-sectional study involving 112 pregnant women with HIV infection receiving antiretroviral therapy (ART). Data including viral load, complete blood analysis, and CD4 counts were extracted from medical records. An inquiry form was used to collect data on socioeconomic status and frequency of food intake. Pre-gestational weight was calculated based on pregnancy weight to obtain the body mass index (BMI) and weight gain for gestational age according the US Institute of Medicine. Of the study population, 68.7% were in consensual union, 31.3% were single, and 33.9% belonged to the two lowest socioeconomic strata. The median age and CD4 count were 27 (interquartile range [IQR]: 23–32) years and 418 (IQR: 267–591), respectively. The adequacy of energy was 91.8% (IQR: 74.1–117.7). The median energy intake from protein was 13.5% (IQR: 12.2–14.9) and from lipids, 35.5% (IQR: 31.1–40.3). Pregnant women with gastrointestinal symptoms and CD4 count

Objectives To determine the prevalence of the Metabolically Healthy Obesity (MHO), and Metabolically Obese Normal-Weight (MONW) phenotypes in a sample of children and adolescents. To evaluate which clinical and laboratory variables are related to the MONW and MHO phenotypes. Methods A cross-sectional study was carried out in children and adolescents aged 6-18 years old, presumably healthy. Somatometry, glucose, insulin, triglycerides, HDL-cholesterol, LDL-cholesterol, HOMA-IR, triglycerides/HDL ratio, triglycerides and glucose index, and leptin/adiponectin, were determined. Results Data from 620 children and adolescents were included (50.65% were males); the median age was 11 years. The prevalence of the MONW phenotype was 22.85% (95%CI 16.85%-29.79%), and the MHO phenotype 27.61% (95%CI 22.60%-33.06%). The variables that significantly explained the possibility of presenting the MONW and MHO phenotype were triglycerides/HDL ratio, and product of triglycerides and glucose. Insulin and HOMA-IR were significantly associated with the MHO phenotype but not with the MONW phenotype. Conclusions Prevalence of metabolically healthy obese phenotype is lower in the Mexican population compared to European studies; thus, future studies should determine if this difference relies upon genetic profile or lifestyle. The indices to assess the action of insulin based on lipids can help identify children and adolescents with the MHO and MONW phenotypes.

Objectives Micronutrient deficiencies are common among people living with HIV (PLWHIV). The clinical and immunologic consequences of micronutrient deficiencies have been poorly explored in the context of human immunodeficiency virus (HIV) infection. The aim of this study was to determine the prevalence of zinc and selenium deficiency (dietary intake and serum concentrations) and analyze their associations with absolute CD4+ T-cell counts, inflammation markers, and metabolic disorders in a cohort of antiretroviral-experienced HIV-infected individuals. Methods The zinc and selenium intakes of 124 HIV-infected men were estimated using 3-d food records. In a subcohort of 45 individuals, serum zinc and selenium concentrations and proinflammatory cytokines were determined. Body composition, bone mineral density (BMD), CD4+ T-cell counts, lipid profile, glucose, and blood pressure were determined and were associated with zinc and selenium dietary intake and serum concentrations. Results Of the PLWHIV studied, 58% had suboptimal intake of zinc and 8% demonstrated suboptimal intake of selenium. Serum deficiencies for zinc and selenium were 23.9% and 65.9%, respectively. Zinc and selenium intake were correlated with increased muscle mass. Selenium intake was associated with increased BMD of the lumbar region. An inverse correlation between serum selenium concentration and several proinflammatory cytokines (interleukin-1β, interleukin-6, and tumor necrosis factor-α) was found. Conclusion Suboptimal zinc and selenium intake and serum concentration deficiencies are highly prevalent in treated HIV-positive individuals and are associated with body composition, BMD, and inflammation. Clinical trials should be designed to explore the effect of zinc and selenium supplementation on metabolic, inflammatory, and immunologic parameters on the HIV-positive population.

Resumen Objetivo Reportar la utilidad de un metodo simplificado con extraccion directa de la leche materna y deteccion por cromatografia liquida de alta resolucion en fase inversa, para la cuantificacion de retinol y α-tocoferol. Material y metodos Se empleo un diseno experimental 23 con las variables de temperatura, tipo de solvente y efecto antioxidante, en 2 niveles. Posterior a la extraccion de la leche materna, las muestras se saponificaron con hidroxido de potasio-metanol, se extrajeron con eter de petroleo y se secaron y reconstituyeron con etanol. Para establecer las condiciones del sistema cromatografico se utilizaron columnas microbondapack con detector UV/visible: fase movil metanol-agua (96:4 v/v), longitud de onda 325 para retinol y 290 nm para α-tocoferol. Resultados Los parametros analiticos fueron linealidad: r2 = 0.9955 (retinol), r2 = 0.9808 (α-tocoferol); limites de deteccion y de cuantificacion: 1.1 y 2.7 μg/dL (retinol), 0.9 y 2.3 μg/dL (α-tocoferol); precision del metodo: desviacion estandar relativa 4.5% intradia y 4.8% entre dias (retinol) y 4.9% intradia y 4.1% entre dias (α-tocoferol); exactitud: recuperacion = 85.8 ± 7.8% para retinol y 98 ± 1.9% para el α-tocoferol. El metodo permitio cuantificar la concentracion de retinol y α-tocoferol eficientemente. Una vez validado el metodo, se aplico para la cuantificacion de retinol y α-tocoferol en 100 muestras de leche materna. Los valores fueron para retinol 63.9 ± 5.2 y para α-tocoferol 359 ± 115 μg/dL. Conclusion El metodo fue selectivo, lineal, preciso, sensible y exacto, caracteristicas que junto con su sencillez hacen de el un metodo conveniente para la determinacion de retinol y α-tocoferol simultaneamente en leche materna.

Controversy remains surrounding vitamin D routine supplementation in healthy pregnancy, and the doses are unclear. The aim of this study was to describe maternal vitamin D status throughout pregnancy in a group of Mexican women and evaluate the effect of frequently prescribed doses of vitamin D3 on longitudinal 25-OH-D concentrations, adjusting for obesity, season, and other factors. We conducted a cohort study (Instituto Nacional de Perinatolog&iacute, a-INPer) (2017&ndash, 2020)) of healthy pregnant women without complications. Pregestational overweight/obesity (body mass index &ge, 25), vitamin D3 supplementation (prescribed by physician, 0&ndash, 250, 250&ndash, 400, and &gt, 400 IU/day), and serum 25-OH-D concentrations (ELISA) were evaluated in each trimester of pregnancy. Vitamin D deficiency or insufficiency was computed (&lt, 20 and &lt, 30 ng/mL, respectively). We studied 141 adult women, 58.5% had pregestational obesity or overweight. In the first trimester, 45.8% of the women were supplemented with vitamin D3, 51.4% had vitamin D insufficiency and 37.3%, deficiency. In the third trimester, 75.4% of the women were supplemented, and 20% of them still had deficiency. The final general mixed linear model showed that 25-OH-D significantly increased throughout pregnancy (p &lt, 0.001), the highest increase was observed in the third trimester in women with doses &gt, 400 IU/day of vitamin D3 (+4 ng/mL, 95% CI: 1.72&ndash, 8.11 ng/mL). In winter/autumn, 25-OH-D concentrations were also lower (p &le, 0.05). In this group of pregnant Mexican women, the prevalence of vitamin D deficiency and insufficiency was high. A higher increase in 25-OH-D concentrations during pregnancy was observed when the women were supplemented with &gt, 400 IU/day. Common supplementation doses of 250&ndash, 400 IU/day were insufficient for achieving an adequate maternal vitamin D status.

Background: It has been reported that the uncoupling proteins (UCPs) can contribute to energy metabolism, and are thus involved in the pathogenesis of obesity. The objective of the study was to analyze the association between UCP polymorphisms, clinical parameters and leptin and adiponectin plasma levels in an adolescent population with overweight and obesity. Methods: We analyzed the UCP1 -3826 C/T, UCP2-866 G/A, Ala55Val and UCP3 -55 C/T polymorphisms and the levels of adipokines in adolescents with normal weight and with overweight or obesity. The study included 270 students aged between 12 and 18 years categorized according to the percentiles from Mexico City. Adipokines levels were measured by immunoassay methods and the UCP polymorphisms were determined using Taqman real-time polymerase chain reaction (RT-PCR). Results: No significant differences were found in the UCP polymorphisms in seven inheritance models studied. Most of the significant differences in the clinical parameters were found under a recessive model, the UCP2 -866 polymorphism was associated with diastolic blood pressure (p=0.008), triglycerides (p=0.045), low-density lipoprotein-cholesterol (LDL-C) (p=0.003), high-density lipoprotein-cholesterol (HDL-C) (p=0.050) and plasma levels of leptin (p Conclusions: This study demonstrated a direct relationship between the clinical characteristics and UCP2-866 in a recessive model, associated with high levels of leptin and decreased levels of adiponectin in an obese or overweight Mexican adolescent population.

Nurses are at risk of having burnout due to workload and job stress&mdash, studies have reported that chronic stress is associated with metabolic syndrome. This study aimed to assess the association between burnout and metabolic syndrome in a sample of female nurses. Data were collected from a cross-sectional study from 2016 to 2018 in a tertiary hospital in Mexico City. All nurses that work in the hospital were invited to participate. Information pertaining to sociodemographic (age, education level), work (labor seniority, service area, shift work), anthropometric (weight, waist circumference, blood pressure) and biochemical (glucose, serum lipids) variables were collected. Burnout was assessed using the Maslach Burnout Inventory test, and metabolic syndrome was defined according to the International Diabetes Federation criteria. A total of 168 nurses participated with a median age of 44 years. The prevalence of burnout and metabolic syndrome was 19.6% and 38.7%, respectively. There was no association between burnout and metabolic syndrome (p = 0.373). However, associations of emotional exhaustion (aOR: 14.95, 95% CI: 1.5&ndash, 148.7), personal accomplishment (aOR: 0.13, 95% CI: 0.01&ndash, 0.99), and night shift (aOR: 12.39, 95% CI: 1.02&ndash, 150.5) with increased waist circumference were found. Strategies are needed to prevent burnout and metabolic syndrome in nurses, especially in those who work at night shift.

Background/Aims Obesity during pregnancy increases the risk of adverse clinical outcomes and is associated with low-grade chronic inflammation. We describe maternal metabolic risk and inflammation by maternal weight status, and evaluate the association of metabolic and inflammatory markers with birthweight in a group of pregnant Mexican women. Interventions in children with adiposity decrease less than 0.2 the body mass index (BMI) z-score less than 0.2 and only in 21&ndash, 23% of cases. Experts recommend focusing on the habits of a healthy lifestyle (HLS) but considering the sociocultural context of children and their parents. Our objective was to achieve a higher percentage of success in lowering the BMI z-score in children with adiposity and their parents through a pilot program &ldquo, Sacbe&rdquo, based on HLS, sensitive to the sociocultural context previously explored and with the active participation of parents. This is a pilot study in children aged 8 to 18 years with adiposity according to the BMI z-score. The program consisted of two workshops on HLS and nutrition given by the pediatric endocrinologist in group sessions with 3&ndash, 5 families and reinforcements in each visit by registered dietitians. We recorded lifestyle habits and anthropometric characteristics of children and their parents at the baseline visit and every month for 3&ndash, 4 months. Forty-nine families, 55 children and 64 parents participated, 60% of the children were female, the average age was 13.95 &plusmn, 3.3 years, 72.7% and 86.7% lowered the z score of the BMI due to intention to treat and protocol analysis (p &lt, 0.001), respectively, BMI z-score decreased by 0.22 &plusmn, 0.21, from 2.13 &plusmn, 0.57 to 1.91 &plusmn, 0.58 (p &lt, 0.001). In total, 83% of the parents involved were mothers, the average age was 45.8 &plusmn, 9.4 years, 77% lost weight and body fat (p &lt, 0.001), the frequency of unhealthy habits decreased. The results of &ldquo, exceeded expectations by combining the active participation of parents, sessions in groups, and the education on various components of an HLS inside sociocultural context. The main challenge will be to standardize and reproduce this type of complex interventions, as well as to assure long-term success.

The objective of this study was to describe the trend of bone biomarkers in adults and adolescents women at 15, 90, 180 and 365 postpartum days (ppd) and its relation with bone mineral density (BMD). It was a prospective cohort of 32 teenager's ≤17 and 41 women from 18 to 29 years old. We evaluated diet, anthropometry, BMD, bone biomarkers and hormonal profile. In all, the concentration of N-telopeptide was higher at 15 days postpartum decreasing during first year postpartum, but adolescents had the highest concentration. The lowest N-telopeptide concentration was associated with highest increasing of the BMD. Osteocalcin concentration was lower in adolescents than in adults women (5 ± 3 vs 13 ± 4 ng/mL, p0.001) during first year postpartum. Exclusive breastfeeding did not affect the BMD (p0.050) or bone biomarkers. Osteocalcin concentration was positively associated with bone BMD (p0.040), breastfeeding did not affect osteocalcin concentrations. Prolactin was higher among women who breastfed exclusively (p0.001). Age and breastfeeding inversely correlated with bone biomarkers (p0.001) N-telopeptide and PTHi respectively. We concluded that a lower N-telopeptide concentration and a higher osteocalcin concentration were associated with a higher increasing of BMD, so then, adolescents showed the lowest recovery of the BMD. Breastfeeding does not affect the BMD.

The urinary ratio 2-hydroxyoestrone/16-hydroxyoestrone (URME), has been proposed in various populations on the world as a risk indicator for breast cancer (BC), however in the Mexican population has never been determined.To determine URME Mexican women and establish its relationship with risk factors for BC.Cross-sectional study of 142 premenopausal and 42 posmenopausal women. The URME was determined with the kit ESTRAMETTM and was related to risk factors for BC. Correlations and linear regressions were performed.The median URME was 0.90 (RIQ 0.64-1.18). To evaluate the differences in inflammation markers and cardiovascular risk factors in a group of school-aged children with and without excessive adiposity. To examine the relationship between adiposity, inflammation, and cardiovascular risk factors.Cross-sectional study of 285 school children (8-12 years old). Adiposity (body mass index, BMI, total body fat, TBF, and waist circumference), inflammatory markers (C-reactive protein [CRP], interleukin 1β [IL-1β], interleukin 6 [IL-6], and tumor necrosis factor-a [TNF-α]) and cardiovascular risk factors were analyzed. Mean differences were calculated and multiple regression models were made.Obese children had higher concentrations of CRP and IL-1β; children with abdominal obesity also had higher CRP levels. A higher BMI was associated with higher blood pressure, and higher total cholesterol, triglycerides-TAG, and insulin concentrations, and with lower HDL-cholesterol. The CRP and IL-1β concentrations correlated significantly with the three adiposity indices. The IL-6 concentrations were associated with TAG, and IL-1β with HDL-cholesterol concentration, after adjustment by BMI.In a group of school-aged Mexican children, obesity increases cardiovascular risk and inflammation. Both IL-6 and IL-1β appear to be factors involved in lipid alterations in these children. More research is needed in order to explore the role of subclinical inflammation in the development of cardiovascular alterations that have already been described in Mexican children with obesity.

The aim of this study was to evaluate the association between the risk of abnormal eating behaviors (AEB) and vitamin and mineral deficiencies among women. Women of childbearing age (n = 282) were systematically sampled with a random start (21.9% adolescents) in 6 suburbs in the west side of Mexico City, they were non pregnant or breastfeeding. Vitamin A, C, E, B12, folic acid, hemoglobin, ferritin, cupper, iron and zinc concentrations were measured. A questionnaire validated in the Mexican population was used for screening AEB. Data were analyzed by descriptive statistics and by using Fisher's test. Approximately 68% of the sample belonged to a mid-low or lower socioeconomic status. 14% had risk of AEB, without statistical differences between adults and teenagers. 10% used diuretics or laxatives to reduce weight within the trimester preceding the survey. Vitamin E, zinc and iron were the most widespread deficiencies affecting 47%, 44% and 27% of the population, respectively. There was no association between the AEB and micronutrient deficiencies neither when AEB were analyzed globally nor individually. Considering these results and the high prevalence of the AEB and overweight in this population, it is important to promote the adoption or healthy behaviors to achieve an adequate weight.

Background: Vitamin C is involved in the synthesis and degradation of collagen and is important for maintenance of the chorioamniotic membranes. Inadequate availability of ascorbic acid during pregnancy has been proposed as a risk factor for premature rupture of the chorioamniotic membranes (PROM). Objective: The objective of the study was to evaluate the effectiveness of 100 mg vitamin C/d in preventing PROM. Design: A controlled double-blind trial was performed. Pregnant women (n = 126) in their 20th wk of gestation were invited; 120 accepted and were randomly assigned to 2 groups (100 mg vitamin C/d or placebo). Every 4 wk, plasma and leukocyte vitamin C concentrations were measured, and each subject was evaluated for cervicovaginal infection. The incidence of PROM was recorded for each group as an indicator of the protective effect of vitamin C supplementation. Results: One hundred nine patients finished the study. Mean plasma vitamin C concentrations decreased significantly throughout the pregnancy in both groups (P = 0.001), and there were no significant differences between groups. Between weeks 20 and 36, mean leukocyte vitamin C concentrations decreased from 17.5 to 15.23 microgram/10(8) cells in the placebo group and increased from 17.26 to 22.17 microgram/10(8) cells in the supplemented group (within- and between-group differences: P = 0.001). The incidence of PROM was 14 per 57 pregnancies (24.5%) in the placebo group and 4 per 52 pregnancies (7.69%) in the supplemented group (relative risk: 0.26; 95% CI: 0.078, 0.837). Conclusion: Daily supplementation with 100 mg vitamin C after 20 wk of gestation effectively lessens the incidence of PROM.

Abstract 2284 Background: Apheresis is an efficient method to collect specific blood components such as platelets, leukocytes, plasma and stem cells. Its advantages include: the collection of standardized and high-quality blood products and a higher collection frequency. However, apheresis is usually accompanied by acute metabolic changes (specially in serum calcium and magnesium concentrations) due to citrate infusion during the whole procedure. Aim: To compare pre and post-plateletpheresis serum level concentrations of calcium, magnesium, zinc, copper and parathyroid hormone among donors and correlate these levels with symptoms that donors may experience during the procedure performance. Materials and Methods: This is a prospective study including 105 healthy plateletpheresis donors who attended the National Cancer Institute. Basal and post-procedure serum levels of parathyroid hormone (PTH), calcium (Ca), magnesium (Mg), zinc (Zn) and copper (Cu) were measured by ion selective potentiometry (Ca), final dot (Mg), atomic absorption (Cu, Zn) and solid phase two site chemoluminiscence (PTH) methods. In case of adverse reactions, a blood sample was drawn immediately in order to analyze concentrations of such hormone and minerals. We intended to correlate both, diet and adverse reactions with serum levels of aforementioned elements. Statistical analysis was performed by chi-square and ANOVA. Protocol was approved by the institutional ethical and research committee. Results: One hundred and five consecutive donors were included in the study conducted from June 28th through July 27th, 2012. There were 68 males and 37 females. Median age was 31 years (range 18–56). Pre and post-apheresis PTH, Ca, Mg, Zn and Cu serum levels are shown in table 1. Furthermore we found significant differences in women older than 30 years but did not find any relationship between donors' symptoms and diet. Conclusions: Plateletpheresis induced a secondary hyperparathyroidism state resulting from chelation of calcium and magnesium. We demonstrated that Cu was also chelated by citrate. These results urges us to assess the kinetics of these metabolic changes as well as to follow a cohort of donors with bone densitometry in order to assure that frequent and chronic infusion of citrate during plateletpheresis donations does not become a long-term risk for osteopenia and osteoporosis as already suggested by some authors1,2. If such a risk is demonstrated other preventive measures must be implemented like: 1.- To restrict the number of apheresis donations per person and 2.- To develop new and safer anticoagulants to be used during the apheresis procedures.
