This study aims to assess breast cancer (BC) association with metals and whether polymorphisms in CYP1A1, CYP1B1, GSTM1 and GSTT1 act as confounders or as modifiers of those relationships. We performed a secondary analysis of 499 histologically confirmed BC cases and the same number of age-matched population controls. We measured urinary concentrations of 18 metals with mass spectrometry. We determined the genetic variants of interest by allelic discrimination and multiplex PCR. After adjusting for covariates, we found BC negatively associated with arsenic, barium, cobalt, copper, magnesium, molybdenum and vanadium concentrations and positively with those of caesium, manganese, tin and thallium. Most associations remained after stratifying by the genetic variants. We identified that polymorphisms in CYP1B1, CYP1A1 and GSTM1 genes interacted with some metals on BC: interaction p-values CYP1B1 G119T × antimony= 0.036, CYP1B1 G119T × cobalt0.001, CYP1B1 G119T × tin= 0.032, CYP1A1 A4889G × aluminium= 0.018, CYP1A1 A4889G × arsenic= 0.031, CYP1A1 A4889G × nickel= 0.036, CYP1A1 A4889G × vanadium= 0.031 and GSTM1 deletion × barium= 0.035. Exposure to various individual metals, along with genetic characteristics may contribute to BC development. Further studies are warranted to confirm our results.

Creatinine values are used to estimate renal function and to correct for urinary dilution in exposure assessment studies. Interindividual variability in urinary creatinine (UCR) is determined positively by protein intake and negatively by age and diabetes. These factors, among others, need to be accounted for, to increase comparability throughout epidemiological studies. Recently, dietary fiber has been shown to improve renal function. This study aims to evaluate dietary fiber intake relationship with UCR and its methodological implications for studies using UCR-corrected measurements. In a cross-sectional study, we analyzed information regarding UCR, dietary fiber, age, and other UCR-related factors in 801 women residing in Northern Mexico during 2007–2009. The median fiber intake in this population was 33.14 g/day, above the adequate intake level for women > 18 years. We estimated an age-adjusted increase of 10.04 mg/dL UCR for a 10 g/day increase in dietary fiber intake. The main dietary sources of fiber in this population were corn tortillas, raw onions, flour tortillas, and beans. Our results suggest that epidemiological studies adjusting analytes by UCR should also consider controlling dietary fiber intake to improve the comparability of creatinine-corrected values and associations across different populations, such as those in Mexico and Latin America, where protein and fiber intake vary significantly.

To describe interindividual metabolism variations and sociodemographic characteristics associated to urinary arsenic, and to estimate the arsenic contamination in water from urinary total arsenic (TAs).Women (n=1 028) from northern Mexico were interviewed about their sociodemographic characteristics and their urinary concentrations of arsenic species were measured by liquid chromatography. Inorganic arsenic (iAs) in water was estimated from urinary TAs.Women were 20-88 years old. TAs in urine ranged from p10=3.41 to p90=56.93 μg/L; 74% of women had levels6.4 μg/L. iAs in water varied from p10=3.04 to p90=202.12 μg/L; 65% of women had concentrations10 μg/L, and 41%, concentrations25 μg/L. Large variations in iAs metabolism were observed. TAs was significantly negatively associated with age and schooling, and positively with the state of residence.Exposure to iAs is an environmental problem in Mexico. Breast cancer (BC) is the most frequently diagnosed cancer in women. However, only 58% of cases have been associated with known risk factors (reproductive, hormonal, lifestyles, and genetic), and the rest to unknown causes. Nevertheless, growing evidence suggests that exposure to environmental contaminants is an important risk factor for BC. Polycyclic aromatic hydrocarbons (PAHs) are formed during organic matter combustion, including smoking, grilled meat, and fuels, and are important carcinogenic constituents of environmental pollution. We examined the information generated by epidemiological studies evaluating the association between BC and PAHs exposure from multiple sources. Our work was conducted according to Conducting Systematic Reviews and Meta-Analyses of Observational Studies of Etiology (COSMOS-E) guidelines. We searched PubMed, Web of Science, and Scopus from January 2000 to December 2019. A total of 124 records were identified, and only 23 articles met all inclusion criteria. Occupational and/or environmental exposure to PAHs was significantly associated with BC, irrespective of exposure being assessed by direct or indirect methods. CYP1A1 and CYP1B1 adverse polymorphisms, familial BC history and smoking status, significantly strengthened the association between PAHs exposure and BC, whereas high fruit and vegetable intake had antagonistic associations. The positive relationships obtained in the studies here reviewed indicated that PAHs exposure is a risk factor for BC. Research needs include the improvement of exposure assessment, particularly identification of specific PAHs, reconstruction of time-varying and distant past exposures and further studies on the interaction between known BC factors and modifiable diet and life-style factors allowing BC prevention and control.

Humans are environmentally exposed to many metals throughout their lives. Simultaneous exposure to several metals could result in synergistic or antagonistic toxicological effects among them; however, the information on exposure to mixtures of metals and breast cancer (BC) is scarce. The objective of this report was to compare metals considered human carcinogens, individually and as mixtures, in women with and without BC. This is a secondary analysis of a population-based case-control study that was carried out from 2007 to 2011 in Northern Mexico. A total of 499 histologically confirmed BC cases and 499 controls were included. Information about sociodemographic, lifestyle and reproductive characteristics was obtained by in-person interviews. Urinary concentrations of aluminum (Al), cadmium (Cd), chromium (Cr), nickel (Ni), lead (Pb), antimony (Sb), cobalt (Co), molybdenum (Mo), tin (Sn), and vanadium (V) were determined by inductively coupled plasma triple quadrupole. Metal mixtures were identified by principal component analysis with creatinine-corrected metals. Over 90% of subjects had metal measurements above the detection limit except tin (86%) and antimony (78.4%). After adjusting by selected covariables, we observed that the individual urinary concentrations of V, Co, and Mo were lower among cases compared to controls; in contrast to Sn that had higher concentrations. We identified two principal component mixtures with opposite relationships with BC: Cr, Ni, Sb, Al, Pb and Sn (OR = 1.15; CI95% 1.06,1.25) and Mo and Co (OR = 0.56; CI95% 0.49,0.64). This is the first study that identified urinary metal mixtures that differed between women with and without BC. Our results warrant confirmation in further prospective epidemiological studies. In addition, the elucidation of underlying mechanisms of metal interactions on BC risk deserves further research.

Experimental studies have shown the diabetogenic potential of inorganic arsenic (iAs); however, the epidemiological evidence is still inconclusive. This could be explained by differences in exposure, metabolism efficiency, nutritional and genetic factors.To evaluate the association between type 2 diabetes mellitus (T2DM) prevalence with arsenic exposure and metabolism, considering one-carbon metabolism nutrient intake and arsenite methyltransferase (AS3MT) polymorphisms.From healthy controls of a case control study for female breast cancer in northern Mexico, 227 self-reported diabetic women were age-matched with 454 non-diabetics. Participants were interviewed about dietary, sociodemographic and clinical characteristics. Urinary iAs metabolites were determined by HPLC-ICP-MS, methylation efficiency parameters were calculated, and AS3MT c.860 T C and c.529-56G C genotypes were determined. Unconditional logistic regression models were used to evaluate associations.Total arsenic in urine (TAs) ranged from 0.73 to 248.12 μg/L with a median of 10.48 μg/L. In unadjusted analysis, TAs (μg/g) was significantly higher in cases than controls, but not when expressed as TAs (μg/L). Cases had significantly lower urinary monomethylarsonic acid percentage (%MMA), first methylation ratio (FMR), creatinine, and choline and selenium intakes. In multi-adjusted models and in women without HTA history T2DM showed significant positive associations with %iAs and FMR, respectively, and a significant negative association with %DMA. In participants with HTA history there was a marginal positive association (p = 0.08) between T2DM and TAs concentrations (μg/g) without other significant associations.Our results support an association between T2DM prevalence and iAs metabolism but not with urinary arsenic levels. However, elucidation of the interplay among iAs metabolism, T2DM and HTA merit further studies.

Background In some epidemiological studies, a positive association of body mass index (BMI) with inorganic arsenic (iAs) metabolism parameters (percentage dimethylarsinic acid [%DMA] and secondary methylation index [SMI]) has been found. In iAs metabolism, S-Adenosyl methionine is converted to S-Adenosyl homocysteine. Sedentarism has been associated with a higher risk of hyperhomocysteinemia. Physical activity has shown an inconsistent negative association with BMI. Therefore, our objective was to evaluate whether physical activity is associated to iAs metabolism independently of BMI. Methods We performed a cross-sectional secondary analysis on 800 non-diabetic women, ≥18 years, who participated as population controls in a previous study on breast cancer in northern Mexico. Participants were interviewed about physical activity during their lifetime, and their weight and size were obtained. Urinary arsenic metabolites concentrations were determined by high performance liquid chromatography coupled with mass spectrometry. Results In the study population, total arsenic ranged from 0.71 to 303.29 μg/L, and the lifetime average physical activity from 0 to 788.40 min/week. BMI was significantly and negatively associated with percentage monomethylarsonic acid (%MMA) and primary methylation index (PMI), and positively associated with %DMA, SMI and TMI, respectively. Likewise, physical activity was negatively associated with %iAs and %MMA, and positively associated with %DMA, SMI and TMI. These results remained after BMI was adjusted for physical activity and viceversa. Conclusion This study confirms the relationship between BMI and iAs metabolism parameters and provides new evidence on the association between physical activity and iAs metabolism.

Background Mexico City has air, water and food pollution problems; however, human exposure to cadmium and its sources have not been described. Objectives To determine the blood cadmium (BCd) level and its main exposure sources among males aged 40 years or older living in different areas of Mexico City. Methods After receiving informed consent, we interviewed 702 males aged ≥40 years to collect data on their sociodemographic characteristics, lifetime occupation, smoking history, and dietary habits, using a validated questionnaire. The BCd level (μg/L) was determined by atomic absorption spectrophotometry. Results The BCd mean level ± SD was 2.61 ± 0.82 μg/L, and 20% of men reported a potential cadmium occupational exposure. After adjusting for age and other potential confounders, the main determinants of the BCd level were the current smoking status at interview, with low (β≤8.5packs/year vs. non-smoker = 0.46; 95% CI: 0.28–0.64 μg/L; p 8.5 packs/year vs. non-smoker = 0.71; 95% CI: 0.56–0.87 μg/L; p 1–3servings/month vs. No = 0.14, 95% CI: 0.01–0.26 μg/L; p Conclusions The BCd levels observed in this population are high and only similar to those observed in workers from a sanitary landfill area in Southern Thailand. Potential environmental Cd exposure sources, such as industrial activity and previous land use, in the West and Center areas of the city should be explored in detail, especially in vulnerable population groups, such as children.

Background Nutrients and genetic polymorphisms participating in one-carbon metabolism may explain interindividual differences in inorganic arsenic (iAs) methylation capacity, which in turn may account for variations in susceptibility to iAs-induced diseases. Objectives 1) To evaluate the association between polymorphisms in five one-carbon metabolism genes (FOLH1 c.223 T > C, MTHFD1 c.1958 G > A, MTHFR c.665 C > T, MTR c.2756 A > G, and MTRR c.66 A > G) and iAs methylation capacity; 2) To assess if previously reported associations between nutrient intake and iAs methylation capacity are modified by those polymorphisms. Methods Women (n = 1027) exposed to iAs in Northern Mexico were interviewed. Blood and urine samples were collected. Nutrient dietary intake was estimated using a validated food frequency questionnaire. iAs methylation capacity was calculated from urinary iAs species (iAs, monomethylarsonic acid [MMA] and dimethylarsinic acid [DMA]) measured by high performance liquid chromatography (HPLC-ICP-MS). One polymorphism in each of the five genes evaluated was genotyped by allelic discrimination. Multivariable linear regression models were used to evaluate if genetic polymorphisms modified the associations between iAs methylation capacity parameters and nutrient intake. Results The median (min-max) concentration of total arsenic (TAs) was 20.2 (1.3–2776.0) µg/g creatinine in the study population. Significant interactions for iAs metabolism were only found with FOLH1 c.223 T > C polymorphism and vitamin B12 intake, so that CT and CC genotype carriers had significantly lower %iAs, and higher DMA/iAs with an increased vitamin B12 intake, as compared to carriers of wild-type TT. Conclusion Differences in dietary nutrient intake and genetic variants in one-carbon metabolism may jointly influence iAs methylation capacity. Confirmation of these interactions in other populations is warranted.

Background Maternal 1,1-dichloro-2,2-bis(p-chlorophenyl)ethylene (DDE) serum levels during pregnancy have been negatively linked to child neurodevelopment in contrast to intake of omega-3 and -6 (ω-3 and ω-6) fatty acids. Objectives To assess whether maternal dietary intake of ω-3 and ω-6 during pregnancy modifies the association between exposure to DDE and child neurodevelopment from age 42–60 months. Methods Prospective cohort study with 142 mother–child pairs performed in Mexico. DDE serum levels were determined by electron capture gas chromatography. Dietary ω-3 and ω-6 intake was estimated by questionnaire. Child neurodevelopment was assessed by McCarthy Scales. Results Docosahexaenoic (DHA) fatty acid intake significantly modified the association between DDE and motor component: increased maternal DDE was associated with lower motor development in children whose mothers had lower DHA intake (βlog2DDE = −1.25; 95% CI: −2.62, 0.12), in contrast to the non-significant increase among children whose mothers had higher DHA intake (βlog2DDE-motor = 0.50; 95% CI: 0.55, 1.56). Likewise, arachidonic fatty acid (ARA) intake modified the association between DDE and memory component: increased maternal DDE was associated with a significantly larger reduction in the memory component in children whose mothers had lower ARA intake (βlog2DDE = −1.31; 95% CI: −2.29, −0.32) than children whose mothers had higher ARA intake (βlog2DDE-memory = 0.17; 95% CI: −0.78, 1.11). Conclusions Dietary intake of DHA and ARA during pregnancy may protect against child neurodevelopment damage associated with prenatal maternal DDE levels.

Global concern about arsenic in drinking water and its link to numerous diseases make translation of evidence-based research into national policy a priority. Delays in establishing a maximum contaminant level (MCL) to preserve health have increased the burden of disease and caused substantial and avoidable loss of life. The current Mexican MCL for arsenic in drinking water is 25 μg/l (2.5 times higher than the World Health Organization (WHO) recommendation from 1993). Mexico's struggles to set its arsenic MCL offer a compelling example of shortcomings in environmental health policy. We explore factors that might facilitate policy change in Mexico: scientific evidence, risk communication and public access to information, economic and technological resources, and politics. To raise awareness of the health, societal, and economic implications of arsenic contamination of drinking water in Mexico, we suggest action steps for attaining environmental policy change and better protect population health.

Background Dichlorodiphenyldichloroethene ( p,p´ -DDE), the main metabolite of dichlorodiphenyltrichloroethane (DDT), has been associated with changes in human thyroid hormone levels. Maternal thyroid hormones are essential for adequate fetal neurodevelopment during the first half of pregnancy. Objective To evaluate the association between maternal p,p´ -DDE concentration and the maternal thyroid profile during the first half of pregnancy. Materials and Methods We analyzed the information of 430 pregnant women from a Mexican floriculture area, with a gestational age ≤16 weeks. By questionnaire, we obtained sociodemographic, reproductive, and life-style, information. Serum concentrations of thyroid stimulating hormone (TSH), and total and free T3 and T4 were determined by means of Enzyme-Linked ImmunoSorbent Assay (ELISA). p,p´ -DDE was analyzed by Gas Chromatography. The association between p,p´ -DDE and thyroid profile was assessed through linear and logistic regression models. Results Thirty eight percent of women had p,p´ -DDE levels below the Limit of Detection and 12.3% below the Limit of Quantification. Within the quantifiable range, median was 53.03 ng/g. TSH >2.5 mIU/L was present in 9.3% of women; 47.7% had isolated hypothyroxinemia; 3.5% had subclinical hypothyroidism, and 5.8% had overt hypothyroidism. We observed a significant positive association between quantifiable p,p´ -DDE and total T3 serum levels in comparison with those with concentrations below the Limit of Detection (β=0.19; 95% CI=0.06, 0.34). There were no significant associations with other hormones of the thyroid profile or with clinical diagnosis. Conclusions Our findings suggest that p,p´ -DDE exposure, even at low concentrations, could disrupt thyroid homeostasis during pregnancy.

Objective To describe the studies that have reported association measures between risk of cancer and the percentage distribution of urinary inorganic arsenic (iAs) metabolites by anatomical site, in non-ecological epidemiological studies. Methods Studies were identified in the PubMed database in the period from 1990 to 2015. Inclusion criteria were: non-ecological epidemiological study, with histologically confirmed cancer cases, reporting the percentage distribution of inorganic arsenic (iAs), monomethylated (MMA) and dimethylated (DMA) metabolites, as well as association measures with confidence intervals (CI) between cancer and %iAs and/or %MMA and/or %DMA. A descriptive meta-analysis was performed by the method of the inverse of the variance for the fixed effects model and the DerSimonian and Laird's method for the random effects model. Heterogeneity was tested using the Q statistic and stratifying for epidemiological design and total As in urine. The possibility of publication bias was assessed through Begg's test. Results A total of 13 eligible studies were found, most of them were performed in Taiwan and focused on skin and bladder cancer. The positive association between %MMA and various types of cancer was consistent, in contrast to the negative relationship between %DMA and cancer that was inconsistent. The summary risk of bladder (OR=1.79; 95% CI: 1.42, 2.26, n=4 studies) and lung (OR=2.44; 95% CI: 1.57, 3.80, n=2 studies) cancer increased significantly with increasing %MMA, without statistical heterogeneity. In contrast, lung cancer risk was inversely related to %DMA (OR=0.58; 95% CI: 0.36, 0.93, n=2 studies), also without significant heterogeneity. These results were similar after stratifying by epidemiological design and total As in urine. No evidence of publication bias was found. Conclusion These findings provide additional support that methylation needs to be taken into account when assessing the potential iAs carcinogenicity risk.

To evaluate breast cancer (BC) molecular subtypes association with reproductive characteristics and an index of cumulative exposure to endogenous estrogens (EEI) in Mexican women. We performed a study of incident cases and population controls in northern Mexico. We included BC cases with tumor molecular classification in their medical records (n = 509), and classified them as HR+/HER2− (ER+ and/or PR+ and HER2−) (n = 289), HER2+ (HR+ or HR−) (n = 117) or triple negative (TN) (n = 103). We matched controls (n = 1030) by age and place of residence with index cases. Women were interviewed about their reproductive history, from which the EEI was developed. We used logistic regression models to estimate BC molecular subtypes associations with reproductive characteristics and EEI. Background Previously we reported that inorganic arsenic (iAs) methylation capacity was associated with breast cancer (BC). BC risk factors may vary according to immunohistochemical subtype. Here we explored the relationships between the capacity to methylate iAs and the risk of BC by subtype. Methods A population-based case-control study was performed in northern Mexico. Patients with available information about BC subtypes (n = 499) were age-matched with healthy controls. Sociodemographic, reproductive, and lifestyle characteristics were obtained. Tumor marker information was obtained from medical records. Cases were classified as HR+ [estrogen receptor (ER+) and/or progesterone (PR+), and human epidermal growth factor receptor 2 (HER2-)], HER2+, or triple negative (TN). Urinary arsenic species were determined by high performance liquid chromatography inductively coupled plasma mass spectrometry (HPLC-ICP-MS), and methylation capacity parameters calculated. Conditional logistic regression models were used to estimate BC risk by subtypes. Results Urinary total arsenic varied from 0.60 to 303.29 μg/L. A significant positive association was found between % monomethylarsonic acid (%MMA) and HR + BC: one percent increase resulted in OR%MMA continuous = 2.73, 95% CI: 1.48, 5.05), and this association remained even when %iAs or % dimethylarsinic acid (%DMA) were added to the models with %MMA. MMA/iAs was positively associated with HR + BC (ORMMA/iAs continuous = 2.03, 95% CI: 1.33–3.10). A significant negative association was observed between DMA/MMA and HR + BC (ORDMA/MMA continuous = 0.43, 95% CI: 0.26, 0.71). MMA/iAs was positively associated with TN BC (OR MMA/iAs continuous = 4.05; 95% CI: 1.63, 10.04). Conclusion Altered iAs methylation capacity resulting in higher %MMA was associated with HR+ and TN BC but not with HER2+. MMA is the iAs metabolite more likely to be related to BC. Further research is needed to confirm these results and elucidate the underlying biological mechanisms.

Bisphenol A (BPA), found in plastics and epoxy resins, is one of the most studied chemicals. BPA is regarded as an endocrine disruptor and has been related to adverse health effects in humans. However, some regulatory agencies around the world have concluded that BPA is safe at current human exposure levels. As the scientific community attempts to settle the debate on BPA's health effects, regulatory agencies have been put into a challenging public health policy situation. The United States has implemented no regulatory actions due to safety concerns, while Europe has used the precautionary principle to guide its regulation in the face of scientific uncertainty. In this paper, we explore the debate surrounding BPA regulation and the possibility for countries to introduce guidelines, using Mexico as an example. Policy change determinants analysis suggest that countries can and should impose regulations on BPA.El bisfenol A (BPA), presente en plásticos y resinas epoxi, es uno de los químicos más estudiados. Se considera un disruptor endocrino y se ha relacionado con efectos adversos para la salud humana. Algunas agencias regulatorias en el mundo han concluido que el BPA es seguro a los niveles de exposición humana actuales. Mientas la comunidad científica intenta resolver el debate sobre dichos efectos, las agencias regulatorias enfrentan una difícil situación de política pública. Los Estados Unidos de América no han implementado acciones reglamentarias por razones precautorias, mientras que Europa ha utilizado el principio precautorio para guiar su regulación ante la incertidumbre científica. En este documento exploramos el debate que rodea la regulación del BPA y la posibilidad de que los países introduzcan directrices, usando a México como ejemplo. El análisis de los determinantes del cambio de políticas sugiere que los países pueden y deben regular el BPA.

Background/Aim: For many years, the presence of arsenic (As) in drinking water of some areas in Northern Mexico has been a major concern. As levels have ranged from 71 to 600 μg/L. We have recently reported that women residing in Northern Mexico, with higher urinary monomethyl arsenic acid percentage (%MMA) had an increased breast cancer (BC) risk (MMA% ORQ5vs.Q1=2.63; 95%CI: 1.89, 3.66; p for trend Breast tumor subtypes with distinctive biology and treatment responses are defined by the immunohistochemical expression of estrogen and progesterone receptors (HR) and human epidermal growth factor receptor 2 (HER2). Epidemiological evidence suggests that BC risk factors (i.e. breastfeeding, parity, menarche, etc.) may vary by tumor pathology.We assessed if %MMA is associated with a particular BC molecular subtype. Methods: A population based case–control study was performed from 2007 to 2011 in five states of Northern Mexico. Incident cases were identified from main public tertiary hospitals in the study area. A total of 499 patients with histopathologically confirmed BC and molecular subtype status available information were identified. Controls were healthy women, with no history of cancer, matched 1:1 by age. Tumor marker information for estrogen receptor (ER), progesterone receptor (PR) and HER2 was obtained from medical records. ER+ and PR+ was based on >=1% cell staining. HER2+ status was determined when at least 30% of protein overexpression was observed in the tumor cell. Cases were assigned to one of three tumor marker categories: HR+ (ER+ and/or PR+ and HER2-), HER+ (regardless of ER or PR status) and TNBC (ER-, PR-, and HER2-). Urinary concentrations (μg/L) of species As3+, As5+, MMA5+, DMA5+, and arsenobetaine (AsB) were determined by high-performance liquid chromatography ICP-MS. Detection limits were As3+: 0.12; As5+: 0.20; MMA5+: 0.12; DMA5+: 0.08; AsB: 0.08. Results were reported by g of creatinine. Total As was calculated by using both the sum of iAs, MMA, DMA plus or minus AsB concentration (TAs or TAs-AsB, respectively). iAs was calculated as the sum of trivalent (As3+) and pentavalent (As5+). %iAs, %MMA and %DMA species were estimated using TAs-AsB as denominator. Using logistic regression models, the associations between BC molecular subtypes and MMA% and DMA% was estimated according to the observed quintile distribution among controls. Based on the median value of each quintile a lineal trend was assessed accordingly. Results.The mean age of cases and controls was around 54 years with 51 years of residence in the study zone. In the entire population, TAs ranged from 0.47 to 303.29 μg/L (0.45 to 1683.83 μg/g creatinine). HR+ BC patients were 57%, HER2 23% and TNBC 20%. These figures are similar to those reported among other groups of Mexican cancer patients. After controlling by key BC factors, %MMA remained as a significant risk factor for BC only for HR+. HER2+ and TNBC tumors were not significantly associated with urinary As metabolites. Conclusion: Our results suggest that increased BC risk related to As exposure is not mediated by HER2 expression. Bisphenol A (BPA) is an endocrine-disrupting chemical widely used in the production of polycarbonate plastics and epoxy resins, which has been previously linked to diabetes among non-Hispanic populations. As part of a case control study for breast cancer, only controls with BPA information were included in this report. The final sample size comprises 70 self-reported diabetics and 334 non-diabetics. Urinary free bisphenol A (BPA-F) (μg/L) was determined by solid-phase extraction and HPLC/FLD analysis. Logistic regression models were used to evaluate the association between BPA-F and self-reported diabetes. After adjusting by age, urinary BPA-F (4.06–224.53 μg/g creatinine) was associated with diabetes exposure (OR = 1.85; 95% CI 1.04, 3.28) compared with women in the reference category (0.67–4.05 μg/g creatinine). BPA may be an environmental cofactor of diabetes. More studies are needed to confirm this result, especially in Hispanic populations.

Background Prenatal exposure to p,p′-DDE is associated with impairments in motor development during the first year of life, with no related repercussions on mental or motor development at 12–30 months and with impairments in cognitive areas, but not in perceptual and motor areas at preschool age. However, its association with particular psychomotor factors, such as establishment of lateralization and spatial orientation, essential elements to the overall learning and specifically reading, writing and spelling in preschoolers, has not been independently evaluated, since cognitive and motor areas have only been explored globally. Objective To determine the association between prenatal exposure to p,p′-DDE and the establishment of lateralization and spatial orientation in children 5 years of age. Materials and methods Establishment of lateralization and spatial orientation was evaluated using the McCarthy Scale of Children's Abilities, with 167 children 5 years of age who participated in a birth cohort in the state of Morelos, Mexico. The information available for each child included: serum concentrations of p,p′-DDE of the mother during at least one trimester of pregnancy, mothers’ intelligence quotients, stimulation at home and anthropometry. A logistic regression model was used to calculate the association between prenatal exposure to p,p′-DDE and lateralization and a multiple linear regression model was used for the association with spatial orientation. Results A two-fold increase in p,p′-DDE in lipid base during the second trimester of pregnancy was associated with a significant reduction, −0.18 points (95% CI −0.41; 0.04, in the spatial orientation index, with no impairment in the establishment of hemispheric dominance. Attending preschool and the maternal intelligence quotient were the main determinants of spatial orientation and the establishment of hemispheric dominance. Conclusions Prenatal exposure to p,p′-DDE may affect the 5 year old's ability to identify spatial orientation of oneself and surrounding objects. Given the observed role of attending preschool in the functions studied, early attendance in formal education might serve as a stimulation strategy for preschoolers. These preliminary results should be verified and expanded in further prospective studies with DDE.

Background Flower growers have high potential for exposures to pesticides. Occupational factors, such as tasks performed, the production method (organic or conventional), the use of personal protective equipment (PPE), and workplace characteristics influence the intensity of pesticide exposure. Objective To evaluate occupational characteristics affecting urinary concentration of dialkylphosphate (DAP) metabolites of organophosphate pesticides among a group of Mexican floricultural workers. Methods A questionnaire was administered to 117 workers who also provided a first morning urine sample. According to tasks performed and the production methods, pesticide contact was defined as low, medium, or high. PPE use was categorized as acceptable, fairly acceptable, and unacceptable. Urinary concentration of DAP metabolites were determined using gas-liquid chromatography. Association between occupational characteristics and DAP urinary concentrations was assessed by means of linear regression models. Results After adjusting for potential confounders, the workers in the medium and high contact categories had significantly higher DAP concentrations than those in the low contact category (β: 0.3, CI 95%: 0.1-0.5). Greenhouse workers had greater DAP concentrations than outdoors workers (β: 0.3, CI 95%: 0.1-0.5). Compared with non-acceptable use of PPE, acceptable use of PPE was associated with lower DAP concentrations (β: -0.4, CI 95% -0.6 to -0.1). Conclusion Improved safety training is needed for correct PPE usage, especially among flower growers who use conventional pest control methods and who work in a greenhouse environment.

Inorganic arsenic (iAs) exposure increases risk of several diseases, including cancer. Some nutrients such as flavonoids enhance glutathione activity, which in turn play a key role in iAs elimination. Our objective was to explore whether dietary non-soy flavonoids are associated with iAs metabolism. We hypothesized that the intake of flavonoids belonging to the following groups, flavan-3-ols, flavone, flavonol, flavanone, and anthocyanidin, is positively associated with urinary dimethylarsinic acid (DMA), which is the most soluble iAs metabolite excreted. We performed a cross-sectional study that included 1027 women living in an arsenic-contaminated area of northern Mexico. Flavonoid intake was estimated using a validated food frequency questionnaire. Concentration of urinary iAs and its metabolites (monomethylarsonic acid and DMA) were determined by high performance liquid chromatography ICP-MS. Results showed positive significant associations between DMA and the flavonoid groups flava-3-ols (β= 0.0112) and flavones (β= 0.0144), as well as the individual intake of apigenin (β= 0.0115), luteolin (β= 0.0138), and eriodictyol (β= 0.0026). Our findings suggest that certain non-soy flavonoids may improve iAs elimination; however, there is still very limited information available regarding the consumption of flavonoids and iAs metabolism.

Objective To evaluate whether the association between breast cancer (BC) and inorganic arsenic (iAs) exposure is modulated by selected polymorphisms in iAs metabolism. Methods A population based case-control (1016/1028) study was conducted in Northern Mexico. Urinary arsenic metabolites were measured by High Performance Liquid Chromatography. Metabolites percentages and methylation ratios, were estimated. Genotypes of selected polymorphisms were determined by allelic discrimination. The interaction between polymorphisms and iAs metabolites percentages and methylation ratios on BC was assessed with unconditional logistic regression models. Results A significant interaction (p = 0.002) between MTR c.2756A>G polymorphism and percentage dimethylarsinic acid (DMA) on BC was found; BC risk related with %DMA was lower in AG + GG carriers than in AA carriers. No other significant interactions were found. Conclusion MTR c.2756A>G polymorphism may confer protection for BC associated with iAs exposure. Further research is warranted to elucidate the potential involvement of other polymorphisms in iAs-related BC.

Context: Breast cancer (BC) risk has been differentially associated with urinary levels of some phthalate metabolites.Objective: To investigate whether PPARγ and PPARGC1B polymorphisms modulate these associations.Materials and methods: 208 BC cases were age-matched with 220 population controls. Phthalate metabolites were determined by HPLC-MS. PPARγ Pro12Ala (rs1801281) and PPARGC1B Ala203Pro (rs7732671) and Val279Ile (rs17572019) were genotyped.Results: The association between mono-(2-ethyl-5-hydroxyhexyl) phthalate (MEHHP) and BC risk was positively modified in PPARγ Pro12Ala C carriers. The association with mono-iso-butyl phthalate (MiBP) in PPARGC1B Ala203Pro G carriers was negatively modified.Conclusion. PPARγ and PPARGC1B polymorphisms modulate the association between phthalate exposure and BC risk.

Objective: To evaluate if variants in the genes CYP1A1 (T3801C and A4889G), CYP1B1 (G119T), GSTM1 (indel) and GSTT1 (indel) are associated with breast cancer (BC) among Mexican women. Materials and methods: 952 incident cases with histologically confirmed BC were matched by age (± 5 years) and zone of residence with 998 healthy population controls. Genetic variants in genes CYP1A1, CYP1B1, GSTM1 and GSTT1were genotyped by allelic discrimination and multiplex PCR. In a subsample of women, 105 markers for ancestry were determined. Results: An increased BC risk, independent of other BC risk factors, was observed among carriers of CYP1B1 G119T genotype (T/T vs. G/G: OR=1.9; 95%CI 1.4-2.5). Conclusion: Our results support the existence of genetic susceptibility for BC conferred by CYP1B1 G119T variant among Mexican women.

Background: The results of previous studies suggest that prenatal exposure to bis[p-chlorophenyl]-1,1,1-trichloroethane (DDT) and to its main metabolite, 2,2-bis(p-chlorophenyl)-1,1-dichloroethylene (DDE), impairs psychomotor development during the first year of life. However, information about the persistence of this association at later ages is limited. Objectives: We assessed the association of prenatal DDE exposure with child neurodevelopment at 42–60 months of age. Methods: Since 2001 we have been monitoring the neurodevelopment in children who were recruited at birth into a perinatal cohort exposed to DDT, in the state of Morelos, Mexico. We report McCarthy Scales of Children’s Abilities for 203 children at 42, 48, 54, and 60 months of age. Maternal DDE serum levels were available for at least one trimester of pregnancy. The Home Observation for Measurement of the Environment scale and other covariables of interest were also available. Results: After adjustment, a doubling of DDE during the third trimester of pregnancy was associated with statistically significant reductions of –1.37, –0.88, –0.84, and –0.80 points in the general cognitive index, quantitative, verbal, and memory components respectively. The association between prenatal DDE and the quantitative component was weaker at 42 months than at older ages. No significant statistical interactions with sex or breastfeeding were observed. Conclusions: These findings support the hypothesis that prenatal DDE impairs early child neurodevelopment; the potential for adverse effects on development should be considered when using DDT for malaria control.

To examine the role of iron and zinc in arsenic excretion and metabolism in children.An analysis of urinary arsenic (UAs) concentrations from a double-blind randomized trial originally testing the efficacy of iron and zinc for lowering blood lead levels in children. A 2 × 2 factorial design was used, with children randomized individually, stratified by sex and classroom, to receive 30?mg ferrous fumarate (n?=?148), 30?mg zinc oxide (n?=?144), iron and zinc together (n?=?148), or placebo (n?=?151). Of the 602 children enrolled, 527 completed the 6-month treatment, and 485 had both baseline and final UAs values. The baseline total UAs concentration ranged from 3.2 to 215.9?µg/L.At baseline, children in the highest tertile of serum ferritin concentration had higher excretion of dimethylarsinic acid (DMA; 1.93?±?0.86%; P??.05), but lower excretion of monomethylarsonic acid (-0.91?±?0.39%; P??.05), compared with children in the lowest tertile. In an intention-to-treat analysis, iron had no effect on arsenic methylation or UAs excretion, but children receiving zinc had lower %DMA in urine (-1.7?±?0.8; P??.05).Iron and zinc status are not related to arsenic metabolism in children, and supplementation with these minerals has limited application in lowering arsenic concentrations.ClinicalTrials.gov: NCT02346188.

Objective: To evaluate if selected phthalate exposure and flavonoid intake interact on breast cancer (BC) risk. Material and methods: Interviews and urine samples were obtained from 233 women with histologically confirmed BC and 221 healthy controls matched by age and place of residence, from various states of northern Mexico. Urinary metabolites concentrations of diethyl phthalate (DEP), butyl benzyl phthalate (BBzP) and dioctyl phthalate (DOP) were determined by solid-phase extraction coupled with high-performance liquid chromatography/isotope dilution/tandem mass spectrometry. Using a semiquantitative food frequency questionnaire, consumption of five types of flavonoids (anthocyanidins, flavan-3-ols, flavanones, flavones and flavonols) was estimated according to three food groups: vegetables, fruits and legumes-oil seeds. Results: A higher intake of anthocyanidins and flavan-3-ols (from vegetables), synergistically increased the negative association between BBzP and BC. No other significant flavonoid-phthalate multiplicative interactions on the risk for BC were found. Conclusion: The consumption of some flavonoids may interact with exposure to phthalates on the risk of BC. Chronic arsenic (As) exposure decreases adult and children's ability to methylate inorganic As (iAs); however, few studies have examined children's sex differences. We measured urinary concentrations of iAs, monomethylarsonic (MMA), and dimethylarsinic (DMA) acids, and calculated the primary (PMI: MMA/iAs) and secondary (SMI: DMA/MMA) methylation capacity indexes in 591 children 6-8 years in Torreón, Mexico. We determined iAs, MMA, and DMA by hydride generation cryotrapping AAS. Lineal regression models estimated associations between methylation capacity and total As (TAs) or iAs. Interactions with sex were tested at p0.10. Boys had significantly higher TAs levels, (58.4µg/L) than girls (46.2µg/L). We observed negative associations between TAs and PMI (β=-0.039; p0.18) and SMI (β=-0.08; p=0.002) with significant sex differences; PMI reduction was significant in boys (β=-0.09; p=0.02) but not in girls (β=0.021; p=0.63), p for interaction=0.06. In contrast, SMI reduction was significantly more pronounced in girls. Furthermore, negative associations PMI (β=-0.19; p0.001) and SMI (β=-0.35; p0.001) were a function of urinary iAs levels, independently of TAs; however, the reduction in PMI was more pronounced in boys (β=-0.24; p0.001; girls β=-0.15; p0.001), p for interaction=0.04. A significant negative association was observed between SMI and iAs levels without significant sex differences. TAs and iAs associations with metabolite percentages were in good agreement with those observed with methylation indexes. Our results suggest that iAs plays an important role in reducing As methylation ability and that significant sex differences are present in As metabolism. These differences merit further investigation to confirm our findings and their potential implications for arsenic toxicity in children.

Background Due to its long-term persistence in the environment and its ability to cross the placental barrier, prenatal p,p′-dichlorodiphenyldichloroethene (DDE) exposure continues to be a public health concern. This study aimed to evaluate the association between prenatal DDE exposure and child growth, at birth and during the first year of life. Methods 253 pregnant women were recruited between January 2001 and June 2005 in a prospective cohort in Morelos, Mexico. Serum levels of DDE were measured during each trimester of pregnancy by gas chromatography with an electron capture detector. Using the generalized mixed-effects models, the association between DDE and child growth parameters (weight-for-age, length-for-age, weight-for-length, BMI-for-age and head circumference-for-age Z -scores) from birth to 1 year of age was assessed. Maternal dietary intake was considered as covariable among others. Results DDE levels were 6.3±2.8 ng/mL (first trimester), 6.6±2.9 ng/mL (second trimester), and 7.6±2.9 ng/mL (third trimester). After adjusting for potential confounder variables, no significant associations were observed with prenatal DDE exposure and each of the selected parameters. Conclusions Our results show no evidence of an association between prenatal DDE exposure and child growth during the first year of life.

Background: Arsenic (iAs) exposure has been associated with several types of cancer. However, most studies to date have not yet implicated iAs as a cofactor for breast cancer (BC). BC incidence and mortality are 3-fold higher in Northern Mexico (NM) than in the rest of the country. iAs levels in drinking water are a source of concern since they have been above international standards. We hypothesized that iAs exposure and individual Arsenic-methylation ability are potential cofactors for BC risk. Material and Methods: We investigated 840 BC incident cases and 973 controls from a population-based case-control study performed in NM. Women were directly interviewed about dietary and reproductive BC covariables. iAs metabolites in urine were determined by HPLC/ICP-MS. Methylation capacity was assessed by calculating the percentage of iAs species and primary [MMA(V)/(As(III)+As(V)] (PMI) and secondary [DMA(V)/MMA(V)] methylation indexes (SMI). Results: The range of total urinary Arsenic was 0.4−303.9 ug/L. Most women (91%) had values above 50 ugAs/L.%MMA(V) was significantly higher in BC cases while%DMA(V) was significantly lower. Phthalates are ubiquitous industrial chemicals used as plasticizers in plastics made of polyvinyl chloride (PVC) to confer flexibility and durability. They are also present in products used for personal-care, industry and in medical devices. Phthalates have been associated with several adverse health effects, and recently it has been proposed that exposure to phthalates, could have an effect on metabolic homeostasis. This exploratory cross-sectional study evaluated the possible association between phthalate exposure and self-reported diabetes among adult Mexican women.As part of an on-going case-control study for breast cancer, only controls were selected, which constituted 221 healthy women matched by age (±5 years) and place of residence with the cases. Women with diabetes were identified by self-report. Urinary concentrations of nine phthalate metabolites were measured by online solid phase extraction coupled to high performance liquid chromatography-isotope-dilution tandem mass spectrometry.Participants with diabetes had significantly higher concentrations of di(2-ethylhexyl) pththalate (DEHP) metabolites: mono(2-ethyl-5-hydroxyhexyl) phthalate (MEHHP), mono(2-ethyl-5-oxohexyl) phthalate (MEOHP) and mono(2-ethyl-5-carboxypentyl) phthalate (MECPP) but lower levels of monobenzyl phthalate (MBzP) a metabolite of benzylbutyl phthalate, compared to participants without diabetes. A marginally significant positive associations with diabetes status were observed over tertiles with MEHHP (OR(T3 vs. T1)=2.66; 95% CI: 0.97-7.33; p for trend=0.063) and MEOHP (OR(T3 vs. T1)=2.27; 95% CI; 0.90-5.75; P for trend=0.079) even after adjusting for important confounders.The results suggest that levels of some phthalates may play a role in the genesis of diabetes.

Prenatal exposure to dichlorodiphenyldichloroethylene (DDE) is associated with decreased motor development during the first year of life, though the effects of DDE in the neonatal stage are not conclusive. The main aim of this study was to evaluate the association between prenatal DDE exposure and neonatal neurodevelopment in a cohort from four municipalities in the state of Morelos, Mexico. The children (265), born from pregnancies with no perinatal complications, were evaluated at 1 month of age (± 7 days) for the presence of abnormal reflexes with the Brazelton Scale (NBAS), neurological soft signs with the Graham-Rosenblith Scale, as well as mental and psychomotor development by the Bayley Scales of Infant Development. Maternal DDE concentrations during each trimester of the pregnancy were determined by gas chromatography. Multiple linear and ordinal logistic models assessed the association between DDE and the outcomes of interest. Prenatal exposure to DDE was associated with a non-significant increase in neurological soft signs (6-8%) and a decrease in psychomotor (β(1T) = -0.02) and mental (β(2T) = -0.03 and β(3T) = -0.19) development. Our results are consistent with previous studies and suggest that prenatal DDE exposure is not associated with neurological functions present in the neonatal stage.

Previous studies suggest adverse effects of arsenic exposure on children’s cognitive function. In this study, we examined the potential association between arsenic exposure and children’s behavior. Five hundred and twenty-six children, 6–7 years old, living near a metal foundry in Torreon, Mexico, participated in the study. Arsenic exposure was measured as total urinary arsenic (UAs) and arsenic metabolites—monomethylarsonic acid (MMA) and dimethylarsinic acid (DMA) concentrations. Children’s behavior was assessed by Conners Behavior Rating Scales for parents and teachers. Sources of phthalates other than Polyvinyl chloride (PVC) related products are scarcely documented in Mexico. The objective of our study was to explore the association between urinary levels of nine phthalate metabolites and the use of personal care products.Subjects included 108 women who participated as controls in an ongoing population-based case-control study of environmental factors and genetic susceptibility to breast cancer in northern Mexico. Direct interviews were performed to inquire about sociodemographic characteristics, reproductive history, use of personal care products, and diet. Phthalate metabolites measured in urine by high performance liquid chromatography–isotope dilution tandem mass spectrometry were monoethyl phthalate (MEP), monobenzyl phthalate (MBzP), mono-n-butyl phthalate (MBP), mono-isobutyl phthalate (MiBP), mono-3-carboxypropyl phthalate (MCPP) as well as mono-2-ethylhexyl phthalate (MEHP), mono-2-ethyl-5-oxohexyl phthalate (MEOHP), mono-2-ethyl-5-hydroxyhexyl phthalate (MEHHP), mono-2-ethyl-5-carboxypentyl phthalate (MECPP) that are metabolites of di-ethylhexyl phthalate (DEHP). Detectable urinary concentrations of phthalate metabolites varied from 75% (MEHP) to 100% (MEP, MBP, MEOHP, MEHHP and MECPP).Medians of urinary concentrations of some phthalate metabolites were significantly higher among users of the following personal care products compared to nonusers: body lotion (MEHHP, MECPP and sum of DEHP metabolites (ΣDEHP)), deodorant (MEHP and ΣDEHP), perfume (MiBP), anti-aging facial cream (MEP, MBP and MCPP) and bottled water (MCPP, MEHHP and MEOHP). Urinary concentrations of MEP showed a positive relationship with the number of personal care products used.Our results suggest that the use of some personal care products contributes to phthalate body burden that deserves attention due to its potential health impact. Objectives p,p9-Dichlorodiphenyldichloroethene (p,p9-DDE) acts as an androgen receptor antagonist, however data regarding its hormonal effects in men are limited. The objective of this study was to evaluate the association between serum levels of p,p9-DDE and reproductive hormone profile in Mexican male flower growers. Methods A longitudinal study was carried out in a population of men working in the production of flowers and ornamental plants in two Mexican states during July–October 2004 (rainy season) and December 2004–May 2005 (dry season). A questionnaire including information on socioeconomic characteristics, tobacco and alcohol use, presence of chronic and acute diseases, occupational history and anthropometry was used and blood and urine samples were obtained. Serum levels of p,p9-DDE were analysed by gas chromatography; FSH, LH, testosterone, oestradiol, inhibin B and prolactin levels were measured by enzymatic immunoassay. Urinary levels of dialkylphosphates (DAPs) were analysed by gas chromatography. Associations between serum levels of p,p9-DDE and male reproductive hormones (both transformed to their natural logarithm) were evaluated using multivariate generalised estimating equation (GEE) models. Results Median p,p9-DDE levels were 677.2 ng/g lipid (range 9.4–12 696.5) during the rainy season and 626.7 ng/g lipid (range 9.4–13 668.1) during the dry season. After adjusting for potential confounders (age, body mass index, state of residence and DAPs), p,p9-DDE levels were negatively associated with prolactin (β=−0.04; 95% CI −0.07 to −0.008) and testosterone (β=0.04; 95% CI −0.08 to 0.005) and positively with inhibin B (β=0.11; 95% CI 0.02 to 0.21). Conclusion These results indicate that p,p9-DDE can affect hypothalamic–pituitary–gonadal axis function in humans.

Objective. Our aim was to evaluate possible associations between consumption of micronutrients involved in one-carbon metabolism, MTHFR genotypes, and global DNA methylation in pregnant women.Methods. A semi-quantitative dietary questionnaire was administered to 195 women during their first trimester in Morelos, Mexico. Two functional polymorphisms of the key folate-metabolizing gene, i.e. MTHFR 677 C>T and 1298 A>C, as well as global DNA methylation were assessed in peripheral blood drawn during the interview.Results. Independent of maternal age and caloric intake, vitamin B6 deficiency was associated with 1.8 fold increased risk of hypomethylation in women carrying the MTHFR 677 T allele.Conclusions. There exists a subpopulation that is more susceptible to B vitamin deficiencies.

In this work, a Gram-positive bacterium with bacillus-type morphology was isolated from low-quality coffee beans in a nutritive medium supplemented with 178 μM of Cd [Cd(NO3)2 4H2O]. PCR showed 99% similarity of the isolated bacteria with Bacillus cereus QD232. This bacterium produced a biosurfactant after 120 h of growth with an average production of 480 mg l−1 and was able to emulsify various hydrocarbons such as diesel (60%), cyclohexane (48%), benzene (48%), isooctane (47%) and toluene (40%). The molecular weight of the biosurfactant, as determined by high-performance liquid chromatography, was 34,194 Da. The cell-free media had a surface tension of 45 mN m−1 and a critical micellar concentration in the range of 0.2–2.5% (v/v), as evaluated by surface tension and conductance, respectively. The emulsifying agent maintained its properties over a pH range from 6 to 10. The composition of the biosurfactant was 53% proteins, 44.4% lipids and 2.6% carbohydrates. Only a few reports have described the production of biosurfactant from Bacillus cereus strains, and the results from this study show that the biosurfactant properties of Bacillus cereus may have potential environmental applications.
