Abstract

The use of local regional flaps is often the most practical and easy means of reconstruction in the management of head and neck tumors. The temporalis muscle coronoid swing has been used to reconstruct the orbital floor and is described in literature as early as 1983. Difficulty is encountered when the medial wall and orbital floor are completely removed and a temporalis muscle-coronoid swing is rendered insufficient for orbital reconstruction. Objective: To describe a combination of forehead island flap and temporalis muscle-coronoid swing in orbital reconstruction. Design: Surgical Innovation/ Case Report. Subject: 12-year-old female with recurrent maxillary chondroblastic osteosarcoma one year after chemotherapy. Methods: The course of tumor excision and defect reconstruction is described. Results: There was no diplopia or other morbidity. There was minimal added operative time for reconstruction with acceptable results. Conclusion: Local flaps can be combined in order to repair the orbital floor. Acceptable function with minimal cosmetic deformity can be achieved with less extensive surgery. 23. Attenuation of virulence in an apicomplexan hemoparasite results in reduced genome diversity at the population level

Abstract

Abstract Background Virulence acquisition and loss is a dynamic adaptation of pathogens to thrive in changing milieus. We investigated the mechanisms of virulence loss at the whole genome level using Babesia bovis as a model apicomplexan in which genetically related attenuated parasites can be reliably derived from virulent parental strains in the natural host. We expected virulence loss to be accompanied by consistent changes at the gene level, and that such changes would be shared among attenuated parasites of diverse geographic and genetic background. Results Surprisingly, while single nucleotide polymorphisms in 14 genes distinguished all attenuated parasites from their virulent parental strains, all non-synonymous changes resulted in no deleterious amino acid modification that could consistently be associated with attenuation (or virulence) in this hemoparasite. Interestingly, however, attenuation significantly reduced the overall population's genome diversity with 81% of base pairs shared among attenuated strains, compared to only 60% of base pairs common among virulent parental parasites. There were significantly fewer genes that were unique to their geographical origins among the attenuated parasites, resulting in a simplified population structure among the attenuated strains. Conclusions This simplified structure includes reduced diversity of the variant erythrocyte surface 1 (ves) multigene family repertoire among attenuated parasites when compared to virulent parental strains, possibly suggesting that overall variance in large protein families such as Variant Erythrocyte Surface Antigens has a critical role in expression of the virulence phenotype. In addition, the results suggest that virulence (or attenuation) mechanisms may not be shared among all populations of parasites at the gene level, but instead may reflect expansion or contraction of the population structure in response to shifting milieus.

Abstract

Le singe a ventre rouge ( Cercopithecus erythrogaster erythrogaster Gray) qui est une sous espece en danger et endemique du Dahomey Gap est tres peu documente au Togo. Pour connaitre sa distribution spatiale, les formes et l’ampleur des menaces auxquelles il est soumis dans le complexe d’aires protegees Togodo, des investigations et prospections pedestres ont ete menees dans six zones correspondant aux six villages riverains et dans la foret sacree de Godjinme situee a 9 km environ au Sud du complexe. Les effectifs des singes identifies ainsi que, leurs coordonnees geographiques, leurs activites au moment de l’observation, la distance d’observation et autres informations utiles sont portes sur une fiche technique. Les donnees collectees ont ete traitees grâce a Ms Excel 2013, au logiciel R version 3.1.3. Le logiciel libre Qgis 2.10 a ete exploite pour la representation de la distribution de la sous espece dans le complexe. Il a ete denombre, 99 individus dans le complexe et 5 dans la foret sacree de Godjinme. Cependant, de nombreuses pressions menacent la survie des populations de ces primates non humains. On note le braconnage, la destruction et les perturbations de leurs habitats par l’agriculture, la transhumance et l’exploitation forestiere. Abstract

It behooves us to understand the power of gravity or its diminished or negative effect as we, Homo sapiens, evolve from going around from four limbs to bipedalism or working mostly erect or sitting on our buttocks. Evolution doesn’t favor a particular desire or anticipate the future; its particular end is to get that individual to live long and propagate more efficiently. Evolution cannot give you all the favorable advantages; only that selfish genes will make us the top predators. Unfortunately the locations of the paranasal sinus ostia are not top in the list of favorable advantages. Hemorrhoids, chronic back pain, varicous veins, etc are other morbid consequences of bipedalism.
