Introducción: La hemorragia subaracnoidea (HSA), representa solo el 3%-5% de todos los accidentes cerebrovasculares (ACV). El objetivo de este trabajo es analizar el error diagnóstico en el Uruguay y sus implicancias en el pronóstico. Materiales y Métodos: Se trata de un estudio prospectivo y multicéntrico, incluyendo a todos los pacientes mayores de 18 años, que presentaron HSA espontánea aneurismática en el Uruguay entre el 1 de noviembre de 2019 y el 31 de julio de 2020. Se realizó una única ficha de recolección de datos, analizando variables relevantes para la patología en estudio. Se definió error diagnóstico a la imposibilidad de reconocer esta patología en la consulta médica inicial y siguientes, otorgándole el alta a dichos pacientes. Esto fue subdividido en dos: 1) pacientes con una sola consulta, y 2) pacientes con dos o más consultas. Se analizó también el retraso en el diagnóstico, y el período de pandemia en que fueron atendidos. El nivel de significación utilizado en los test fue de 5%. La investigación fue aprobada previamente por todos los Comité de Ética institucionales, así como también analizado por la Comisión Nacional de Ética en Investigación (CNEI), por tratarse de un estudio multicéntrico. Resultados: El total de pacientes analizados fue de 211. En cuanto a la clínica de presentación de la HSA, la cefalea y los vómitos fueron los de mayor frecuencia (83% y 46% respectivamente). En 24% de los pacientes se constató un error diagnóstico. Respecto al análisis de las complicaciones que pudieron ser modificadas o incrementadas por el error diagnóstico, se demostró que el vasoespasmo tuvo una vinculación estadísticamente significativa en relación con el error diagnóstico. Conclusiones: De las complicaciones atribuibles a un error diagnóstico, el vasosespasmo fue el único que se mostró correlacionado con éste.

Introducción: Las diferentes vertientes que puede tomar el estudio de la actividad artesanal y su relación con el medio natural han representado un importante reto debido a la diversidad de pensamientos expresados, lo que conlleva un esfuerzo heurístico para identificar elementos explicativos que tuviesen puntos de encuentro suficientes y permitieran establecer una clasificación pertinente. Objetivo: La investigación tuvo como objetivo documentar las orientaciones temáticas que enmarcan los estudios ambientales de la actividad artesanal, esto mediante la revisión de distintas investigaciones que han abordado la relación entre artesanía y naturaleza. Método: Para ello, se llevó a cabo un análisis crítico del acervo bibliográfico identificando las orientaciones temáticas. La indagación se realizó tras el estudio de artículos científicos publicados en revistas indizadas, durante el periodo 2007 a 2022 y disponibles en acceso abierto en idioma español, inglés y portugués; además se consideraron los siguientes criterios: tipo de investigación, objetivo de la contribución, enfoque metodológico, ámbito territorial, orientación temática y disciplinaria. Resultados: Se identificaron cuatro grupos de aproximaciones temáticas: a) proceso de producción, acceso a materias primas, transformación y tecnologías; b) impacto de la producción sobre los recursos naturales; c) formas de organización social en el contexto local; d) comercialización y consumo en el mercado actual. Conclusiones: Si bien es posible establecer una propuesta de clasificación de los estudios ambientales de las artesanías, se requiere una visión ampliada de la realidad que supere las perspectivas parcelarias, y se consideren los paradigmas propios de la complejidad como argumentos para su estudio en México.

Anthropogenic activities are often related to the occurrence of simultaneous contaminations with heavy metals and toxic organic compounds. In addition, the increasing demand for food, clothing, and technology has increased the worldwide contamination level. Although it is not fully demonstrated, the high level of contamination in association with the indiscriminate use of antibiotics, led to the appearance of multi-resistant pathogenic microorganisms. Grey and red biotechnologies try to counteract the negative effects of pollution and antimicrobial resistance respectively. Streptomyces is well known in the field of biotechnology. In this review, we discussed the potential of these bacteria to deal with organic and inorganic pollutants and produce nanostructures with antimicrobial activity. To our knowledge, this is the first work in which a biotechnological bacterial genus such as Streptomyces is revised in two different fields of global concern, contamination, and multi-drugs resistant microorganisms.

El molusco contagioso es una infección viral cutánea, usualmente benigna y autolimitada, causada por un virus del género Molluscipoxvirus. Es más frecuente en niños, adultos jóvenes sexualmente activos e inmunosuprimidos. La lesión clínica característica es una pápula umbilicada eucrómica o de tono perlado, que se disemina rápidamente y puede afectar cualquier superficie muco-cutánea, aunque la localización en los párpados es atípica. Se presentan dos casos de pacientes jóvenes inmunosuprimidos, con moluscos contagiosos palpebrales, en quienes el diagnóstico clínico inicial fue incorrecto. Se enfatiza la importancia de diagnosticar oportunamente las lesiones papulares que afectan la piel del párpado ya que la presencia de molusco contagioso en esta zona se considera una manifestación cutánea de inmunosupresión.

The genus Cohnella belongs to a group of Gram-positive endospore-forming bacteria within the Paenibacillaceae family. Although most species were described as xylanolytic bacteria, the literature still lacks some key information regarding their repertoire of xylan-degrading enzymes. The whole genome sequence of an isolated xylan-degrading bacterium Cohnella sp. strain AR92 was found to contain five genes encoding putative endo-1,4-β-xylanases, of which four were cloned, expressed, and characterized to better understand the contribution of the individual endo-xylanases to the overall xylanolytic properties of strain AR92. Three of the enzymes, CoXyn10A, CoXyn10C, and CoXyn11A, were shown to be effective at hydrolyzing xylans-derived from agro-industrial, producing oligosaccharides with substrate conversion values of 32.5%, 24.7%, and 10.6%, respectively, using sugarcane bagasse glucuronoarabinoxylan and of 29.9%, 19.1%, and 8.0%, respectively, using wheat bran-derived arabinoxylan. The main reaction products from GH10 enzymes were xylobiose and xylotriose, whereas CoXyn11A produced mostly xylooligosaccharides (XOS) with 2 to 5 units of xylose, often substituted, resulting in potentially prebiotic arabinoxylooligosaccharides (AXOS). The endo-xylanases assay displayed operational features (temperature optima from 49.9 to 50.4 °C and pH optima from 6.01 to 6.31) fitting simultaneous xylan utilization. Homology modeling confirmed the typical folds of the GH10 and GH11 enzymes, substrate docking studies allowed the prediction of subsites (− 2 to + 1 in GH10 and − 3 to + 1 in GH11) and identification of residues involved in ligand interactions, supporting the experimental data. Overall, the Cohnella sp. AR92 endo-xylanases presented significant potential for enzymatic conversion of agro-industrial by-products into high-value products. Key points • Cohnella sp. AR92 genome encoded five potential endo-xylanases. • Cohnella sp. AR92 enzymes produced xylooligosaccharides from xylan, with high yields. • GH10 enzymes from Cohnella sp. AR92 are responsible for the production of X2 and X3 oligosaccharides. • GH11 from Cohnella sp. AR92 contributes to the overall xylan degradation by producing substituted oligosaccharides.

Introducción: La HSA constituye del 3 al 5% de todos los ataques cerebro vasculares en la población general. Es una enfermedad de alto impacto en la sociedad, por su elevada morbimortalidad. Se han realizado múltiples estudios retrospectivos, pero no hay ningún estudio prospectivo nacional actualizado de dicha patología en Uruguay. Aquí se publican datos preliminares de un trabajo anual con el objetivo de conocer las características demográficas, epidemiológicas, así como las características topográficas y morfo-estructurales de los aneurismas, y la evolución de los pacientes portadores de esta patología. Materiales y Métodos: Se esta realizando un trabajo multicéntrico de una cohorte longitudinal prospectivo de todos los pacientes que se presentan con HSA en el Uruguay en un período de un año. En esta presentación preliminar se incluyen para un primer análisis todos los pacientes entre el 1 de noviembre de 2019 y el 31 de enero de 2020. Se incluyeron todos los pacientes mayores de 18 años que consultaron y se les diagnosticó una HSA espontánea. Una vez hecho el diagnóstico de HSA, dependiendo de la gravedad del paciente se realizó un estudio vascular (angio-TC o arteriografía) para valorar la presencia o no de aneurisma, y la topografía del mismo para decidir el tratamiento. Se analizaron las siguientes variables: edad, sexo, procedencia geográfica, antecedentes, motivo de consulta, escala de coma de Glasgow (GCS), escala de World Federation of Neurological Surgeons (WFNS), escala Hunt y Hess, escala de Fisher, consultas previas al ingreso, tratamiento indicado, complicación vinculada a la HSA, complicación vinculada al tratamiento indicado, y escala de rankin modificado (mRS) al mes. Se creó una máscara de captura, y el análisis se procesó utilizando el software SPSS versión 22. La investigación fue aprobada por todos los Comité de Ética de los diferentes centros que participaron del estudio. Resultados: De un total de 36 pacientes, la edad media fue de 55,2 años, con mayor frecuencia en el sexo femenino. En el 83,3% de los pacientes el síntoma que predominó fue la cefalea y en la mitad de los casos estuvo asociada a vómitos. El trastorno de conciencia se presentó en 55% de los casos. Hubo 25% de error diagnóstico. El estudio diagnóstico de aneurisma más frecuente usado fue la angio TC en el 80% de los casos, y la arteriografía se realizó en el 41%. En el 63% de los casos se realizó cirugía abierta, y en el 14% se realizó tratamiento endovascular. Los aneurismas más frecuentes fueron el comunicante anterior, el silviano y el comunicante posterior. El 80% presentó complicaciones vinculadas a su HSA. A los 30 días del ictus 11 pacientes (30%) fallecieron (mRS 6), en la gran mayoría de los casos la causa fue neurológica. Debemos destacar que la mayoría de los pacientes que fallecieron ingresaron con un mal estado neurológico. Conclusiones: Este trabajo presenta las primeras cifras sobre las características epidemiológicas de la Hemorragia subaracnoidea en el Uruguay.

While cesarean section is an essential life-saving strategy for women and newborns, its current overuse constitutes a global problem. The aim of this formative research is to collect information from hospitals, health professionals and women regarding the use of cesarean section in Argentina. This article describes the methodology of the study, the characteristics of the hospitals and the profile of the participants.This formative research is a mixed-method study that will be conducted in seven provinces of Argentina. The eligibility criteria for the hospitals are (a) use of the Perinatal Information System, (b) cesarean section rate higher than 27% in 2016, (c) ≥ 1000 deliveries per year. Quantitative and qualitative research techniques will be used for data collection and analysis. The main inquiry points are the determining factors for the use of cesarean section, the potential interventions to optimize the use of cesarean section and, in the case of women, their preferred type of delivery.It is expected that the findings will provide a situation diagnosis to help a context-sensitive implementation of the interventions recommended by the World Health Organization to optimize cesarean section use. Trial registration IS002316 Cesarean section is an essential medical tool for mothers and their children, but nowadays its overuse is a problem worldwide. Our purpose is to get information from hospitals, health professionals and women about how cesarean section is used in Argentina. In this protocol we describe how we will carry out the study and the characteristics of the hospitals and participants. We will implement this study in seven provinces of Argentina, in hospitals that have more than 1,000 births each year, had a cesarean section rate higher than 27% in 2016 and use the Perinatal Information System. We will gather information using forms, surveys and interviews. We want to identify the factors that decide the use of a cesarean section, the potential interventions that can improve the use of cesarean section and, in the case of women, the type of delivery they prefer. We expect that this study will give us a diagnosis of how cesarean section is used in Argentina, and that this will help to apply the interventions that the World Health Organization recommends to optimize the use of cesarean section in our specific context.INTRODUCCIóN: Aun cuando la cesárea es una intervención que puede ser esencial para salvar la vida de una mujer y su hijo, el crecimiento excesivo de su uso es un problema global. El propósito de esta investigación formativa es recolectar información sobre las instituciones, profesionales de la salud y mujeres acerca del uso de la cesárea en la Argentina. Este artículo describe la metodología del estudio, las características de los hospitales y el perfil de los participantes. METODOLOGíA: Esta investigación formativa usa un diseño mixto aplicado en siete provincias del país. Los criterios de elegibilidad para los hospitales son: (a) uso del Sistema Informático Perinatal, (b) tasa de cesáreas mayor al 27% en 2016, y (c) ≥ 1000 partos por año. Se usarán técnicas cualitativas y cuantitativas para la recolección de datos y el análisis. Los principales temas a indagar son los determinantes del uso de la cesárea, las intervenciones para optimizar su uso y, en el caso de las mujeres, sus preferencias sobre el modo de parto. DISCUSIóN: Se espera que los resultados den lugar a un diagnóstico de situación que permita una implementación de las intervenciones propuesas por la OMS para optimizar el uso de la cesárea más ajustada al contexto. La ley 27.610 estipula un sistema mixto de acceso al aborto: plazo y causales. Si bien no modifica sustancialmente el acceso al aborto luego de las 14 semanas de gestación (SDG) respecto de lo que estaba vigente, presenta un contexto oportuno para mejorar el acceso y la calidad de la atención a estos abortos, que enfrentan una serie de barreras adicionales. Nos propusimos explorar y analizar las condiciones que influyen en el acceso y la atención al aborto después de las 13 SDG, a fin de identificar barreras y sugerir estrategias para afrontarlas. Realizamos 14 entrevistas en profundidad a profesionales del sistema público de salud de ciudades seleccionadas de Argentina, quienes garantizaban el acceso al aborto en el segundo trimestre durante el 2017. Las principales barreras identificadas son:1) A nivel de las políticas públicas: poca claridad y apoyo desde las políticas públicas; marco jurídico en edad gestacional avanzada; no regulación de la objeción de conciencia; 2)A nivel institucional: falta de apoyo institucional; falta de formación en carreras de medicina; sobrecarga de tareas a quienes garantizan el acceso; sistema de salud expulsivo; dificultades de acceso geográfico a los pocos efectores que garantizan; y 3)A nivel subjetivo e interpersonal: carga emocional para profesionales en casos avanzados; estigmatización y soledad de profesionales que garantizan; vergüenza y temor al maltrato y denuncia por parte de las personas que abortan. El foco de las recomendaciones se orienta a políticas públicas rectoras en: capacitaciones, provisión y aprobación de insumos clave y difusión pública de información sobre IVE e ILE., Law 27.610 establishes access to abortion up to 14 weeks gestation (WG) at the woman´s request, and without a limit regarding specific situations. The law does not substantially modify access to abortion after 14 WG compared to what was legally possible before this new law. However, it presents an opportune context to improve access and quality of care for these abortions which face a series of additional barriers. We aim to explore and analyze the conditions that influence access and care for abortion after 13 WG, in order to identify barriers and suggest strategies. We conducted 14 in-depth interviews with public health system providers in selected cities of Argentina, who provided access to abortion after 13 WG. We identified as main barriers: 1) public policies barriers: lack of clarity and support from public policies, advanced gestational age legal framework, conscientious objection absence of regulation; 2) institutional barriers: lack of institutional support, lack of training in medical careers, overload of tasks for those who provide access, denial of abortion services, geographical access barriers to the few institutions that provide care; and 3) subjective and interpersonal barriers: emotional burden for providers in advanced WG; providers stigmatization and isolation; people seeking abortion’s shame and fear of being mistreated and reported., Dossier: Implementación de IVE. Herbicides have contributed to increased agricultural production. However, their residual amount can cause negative effects on environmental and public health. Therefore, this work aimed to determine the occurrence of both atrazine and diuron in surface and well water and investigate their link with drinking use. The samples were collected during dry and rainy seasons in three wells and surface water from a river and a pond located in the low plains of the Ixcatepec catchment, at the Amacuáhuitl community of the municipality of Arcelia, Guerrero State, in the center south of México, which is a rural community where farming is the main activity. The compounds were obtained by solid phase extraction and determined by HPLC-MS quadrupole with positive electrospray ionization mode. A geomorphic analysis was conducted inside the Ixcatepec catchment using the digital elevation model of the Shuttle Radar Topography Mission, SRTM-v4. The human risk for drinking water was calculated according to the Hazard Quotient. The concentrations of atrazine and diuron were between 5.77 and 402 ng L−1. Atrazine was the most abundant and frequent pesticide found with an average concentration of 105.18 ng L−1, while that of diuron was 86.56 ng L−1. The highest levels were found in pond Ushe, likely being the result of the lowest flow and stagnation of water, and during the cold-dry season a consequence of mobilization by irrigation runoff. The morphological analysis indicated that the compounds mainly reached body water located in the lower surfaces from cultivated areas. Therefore, the occurrence is mainly linked to agriculture activity within the rural community. However, chemical properties of compounds, crop irrigation, and environmental conditions could be contributing to the dispersion of residual amounts of herbicides within the hydrological system. The estimation of risk showed that atrazine can mainly generate health problems for children using the Azul well as a source of drinking water.

This Viewpoint discusses passage and implementation of Law 27.610, which legalized abortion in Argentina under certain circumstances, and examines the ongoing clinical issues and legal challenges to the law.

ANTECEDENTES: El 11 de marzo del 2020 se declaró como pandemia a la infección por SARS-CoV-2, estableciendo principalmente clínica respiratoria y gastrointestinal. La infección en pacientes pediátricos ha tenido una variedad importante de sintomatologías, destacando las alteraciones neurológicas, así como un aumento de informes por síndrome de Guillain-Barré (SGB); sin embargo, estos últimos son escasos.CASO CLÍNICO: Niño de 10 años de edad, el cual inició con debilidad de extremidades inferiores, tos productiva y fiebre; con antecedente de hospitalización a los 2 años por Síndrome de Guillain-Barré durante 15 días, con recuperación completa. Al llegar a nuestra unidad se realizó Tomografía Axial Computarizada de tórax con datos sugestivos de COVID-19, se solicitó RT-PCR para SARS-Cov-2 con resultado positivo, además de una electromiografía en la que se obtuvieron resultados concordantes con una polineuropatía desmielinizante compatibles con variante de neuropatía axonal aguda motora en las cuatro extremidades. A pesar del manejo con gammaglobulina y plasmaféresis el paciente falleció.CONCLUSIÓN: El diagnóstico y tratamiento del Síndrome de Guillain-Barré relacionado con COVID-19 en pacientes pediátricos se ha mostrado como un verdadero reto al disponer de pocos reportes en la bibliografía, por lo que la importancia de la revisión del presente caso es tener en mente la diversidad de manifestaciones que se presentan en la infección por COVID-19 y contemplarlo en el diagnóstico diferencial de las encefalitis y cuadros infecciosos con manifestaciones neurológicas.

Stroke is a worldwide leading cause of mortality and disability, and there are substantial economic costs for poststroke care. Disadvantaged populations show increased incidence, severity, and unfavorable outcomes. This study aimed to report the survival, functional outcome, and caregiver satisfaction of low-income patients diagnosed with a large hemispheric infarction (LHI) who underwent decompressive craniectomy (DC).A retrospective analysis was conducted in consecutive adult patients with an LHI who underwent DC at a single center between October 2015 and September 2019. Demographic, clinical, and radiologic data were reviewed. The primary outcomes were 1-year survival and favorable functional outcome.Forty-nine patients were included; those60 years of age showed a higher proportion of favorable functional outcomes (76% vs. 33%; P = 0.031) but similar survival (52% vs. 56%; P = 0.645) than older patients, respectively. Performing the craniectomy in48 hours from stroke onset compared with ≥48 hours showed no statistically significant differences in survival (59% vs. 46%; P = 0.352) and favorable functional outcomes (56% vs. 70%; P = 0.683), respectively. In retrospective thinking, 79% of caregivers would decide to perform the surgery again.Age group and time from stroke onset to craniectomy were not associated with survival; notwithstanding, a higher proportion of patients60 years of age were associated with a favorable functional outcome compared with older patients. Additionally, if given the option, most caregivers would decide to perform the surgery again, independently of the grade of disability of the patient.

espanolResumen El objetivo fue identificar el nivel de consumo de alcohol y los parametros biometricos en pacientes con enfermedad cardiovascular en la poblacion rural colombiana de Tausa (Cundinamarca). Se realizo una investigacion cuantitativa, con un diseno descriptivo y transversal. La poblacion estuvo constituida por 29 pacientes que cursaban con enfermedad cardiovascular y que acudieron entre octubre de 2017 y mayo de 2018 a la consulta del programa de cronicos de una Empresa Social del Estado de primer nivel en el municipio de Tausa. A estos pacientes se les realizo una valoracion inicial y posterior, de parametros biometricos y aplicacion del instrumento AUDIT-C. Los resultados muestran que la edad media (DE) de la muestra fue de 70 anos. Segun los puntajes obtenidos con la aplicacion del AUDIT-C, el 75,9% de los participantes se encontraba sin riesgo de consumo de alcohol o alguna de sus complicaciones derivadas. Este estudio constituye un aporte a la caracterizacion del consumo de alcohol y la valoracion de parametros biometricos en poblacion rural con enfermedad cardiovascular. portuguesResumo O objetivo foi identificar o nivel de consumo de alcool e os parâmetros biometricos em pacientes com doenca cardiovascular na populacao rural colombiana de Tausa (Cundinamarca). Esses pacientes foram submetidos a uma avaliacao inicial e subsequente dos parâmetros biometricos e a aplicacao do instrumento AUDIT-C. Os resultados mostram que a media de idade (DP) da amostra foi de 70 anos. De acordo com os escores obtidos com a aplicacao do AUDIT-C, 75,9% dos participantes estavam sem risco de consumo de alcool ou de quaisquer complicacoes. Este estudo constitui uma contribuicao para a caracterizacao do consumo de alcool e a avaliacao de parâmetros biometricos na populacao rural com doenca cardiovascular. EnglishAbstract The objective was to identify the level of alcohol consumption and biometric parameters in patients with cardiovascular disease in the rural Colombian population of Tausa (Cundinamarca). Quantitative research was carried out, with a descriptive and cross-cutting design. The population consisted of 29 patients who were in cardiovascular disease and who attended between October 2017 and May 2018 the consultation of the chronic program of a State Social of first level in the municipality of Tausa. These patients were given an initial and subsequent assessment of biometric parameters and application of the AUDIT-C instrument. The results show, the average age (ED) of the sample was 70 years. According to the scores obtained with the application of the AUDIT-C, the 75,9% of the participants were without risk of alcohol consumption or any of its relapse complications. This study constitutes a contribution to the characterization of alcohol consumption and the assessment of biometric parameters in rural population with cardiovascular disease.

Given that the cadmium (Cd) toxicity could be due to its interference with the calcium (Ca) homeostasis, the aim of this work was to study the effect of Cd over the presence, distribution and volume density (Vv) of Ca and Ca-ATPase in the secretory cells of the pars preconvoluta (PPC) and the pars convoluta (pc) in Rhinella arenarum. The severe effect of the xenobiotic (CdCl2 2.5 mg/kg) in sexually matured females was evaluated. Co-localization, as well as a marked reduction of Ca and Ca-ATPase, was observed in treated animals, in the areas analyzed, compared to control. Low calcium deposits were found in the secreting granules (SG) of the epithelial (ESC) and glandular secretory cells (GSC), while an increase in their cytoplasm and intracellular space was observed. The Ca-ATPase in treated and control animals was detected at the SG and the plasmatic membrane of the ESC and GSC. In relation to the Vv estimates, a substantial reduction of Ca deposits and Ca-ATPase activity was observed in the treated group, with respect to the control. Both amounts of Vv of Ca and Ca-ATPase activity were higher in PPC than in pc, and, higher in ESC than in GSC. These results were associated with the Cd concentration in the oviductal PC, determining that it is a bioaccumulator organ. Thus, this work demonstrated that the Cd interacted with Ca-ATPase, leading to an increase of cytosolic Ca, which is responsible for the possible disruptions in cellular metabolism. In this work, an enzyme extract immobilization over carbon nanomaterials is presented; that is, carbon nanospheres (CNSs), and multi-walled carbon nanotubes (CNTs), using covalent bonds with glutaraldehyde as a crosslinking compound. Carbon nanostructures were obtained by chemical vapor deposition using an iron–cobalt powder alloy prepared by the sol–gel process and using silica powder as support. Acetylene gas was used as carbon carrier to prepare both the CNTs and CNSs. Both nanostructures were oxidized with an acid treatment in order to add functional groups to the surface and subsequently immobilize the enzymes. Carbon nanostructures were characterized using scanning electron microscopy, infrared spectroscopy and Raman spectroscopy to confirm oxidation, immobilization and structural properties. Electrochemical impedance spectroscopy revealed that carbon nanostructures improve the electron transfer of the electrode. Finally, the cyclic voltammetry provided information about the redox process before and after immobilization. Even more, the catalytic activity of the enzymes extract was evaluated showing that CNSs are the best support.

Introducción: Las lesiones arteriales aneurismáticas cobran gran relevancia hoy en día ya que si se realiza un diagnóstico precoz se puede prevenir la rotura de las mismas. Aneurisma no roto o grado 0 se define a aquel que no sangro. El riesgo de tener una HSA de un aneurisma no roto es de aproximadamente 1,5% anual, acumulable. En el caso de lesiones grandes o gigantes como nuestro caso ese riesgo aumenta mucho porcentualmente, y aneurismas disecantes hasta 40%. Materiales y Métodos: En primer lugar se presenta caso clínico de una paciente que fue asistida en el Hospital Maciel, donde se estudia y opera de un aneurisma fusiforme mediante clipado, progresando la enfermedad requiriendo tratamiento endovascular y posteriormente exéresis asociada a bypass extra-intracraneano con injerto radial para lograr la curación del mismo. A partir de este caso clínico ilustrativo se realizó una puesta al día sobre esta patología. Discusión: Se discute la clasificación de aneurismas, etiología, opciones terapéuticas y el seguimiento de estos pacientes. Conclusiones: En caso de los aneurismas fusiformes debemos sospechar una base disecante, lo cual es importante para definir el tratamiento de estas lesiones.

Background Benefit of carboplatin and dose-dense weekly paclitaxel (ddCT) in first line treatment of ovarian cancer patients has been different in Western and Asian studies. In the present study we compare progression-free survival (PFS) of ddCT to three-weekly carboplatin and paclitaxel (CT) in first-line treatment of ovarian carcinoma in a single institution in a Western population. Materials and methods We conducted a retrospective review of medical records from patients with ovarian carcinoma treated in a tertiary cancer center from 2007 to 2018. All patients treated with ddCT or CT in the first-line setting were included. Patients who received first-line bevacizumab were not included. PFS and overall survival (OS) were compared in a propensity score-matched cohort to address selection bias. Patients were matched according to age, ECOG performance status, CA 125, FIGO stage, residual disease, and histological subtype, in a 1:2 ratio. Results Five hundred eighty-eight patients were eligible for propensity score matching, the final cohort consisted of 69 patients treated with ddCT and 138 CT group. Baseline characteristics were well-balanced. After a median follow-up of 65.1 months, median PFS was 29.3 vs 20.0 months, favouring ddCT treatment (p = 0.035). In the multivariate cox regression ddCT showed a 18% lower risk of progression (HR 0.82, 95% CI 0.68–0.99, p = 0.04). Overall survival data is immature, but suggested better outcomes for ddCT (not reached versus 78.8 months; p = 0.07). Conclusion Our retrospective study has shown superior PFS of ddCT over CT regimen in first-line treatment of ovarian carcinoma in a Western population not treated with bevacizumab.

Lipase immobilization using adsorption on magnetic nanoparticles, cross-linked enzyme aggregates (CLEA), and a combination of both techniques was investigated. Experimental designs were used for the optimization of the immobilization observing that the pH and ionic strength play a principal role during the lipase immobilization and its activity. For adsorption on magnetic nanoparticles and CLEA synthesis the optimal condition was pH and 100 mM. Besides, during the CLEA synthesis, glutaraldehyde concentration showed to be a significant effect on the enzyme activity. A comparison between a magnetic CLEA prepared with (Lip@mCLEA) and without (mCLEA) biological functionalized magnetic nanoparticles was made observing that the use of functionalized support showed the best performance activity. All biocatalytic systems developed gives to the enzyme thermal stability between 45 and 70 °C, being Lip@mCLEA the more stable biocatalyst. Similar behavior was observed at different pH, where both Lip@mCLEA and mCLEA showed stability at a range of pH 5 to 8. The immobilized biocatalysts showed the same affinity of the subtract that the free enzyme suggested that the enzyme structure not modified the active site. The combination of both types of immobilization show evidenced the importance of the biological functionalization of the support when magnetic CLEA is produced.

BackgroundThis paper reports on women's perspectives on the birth mode in five public maternity hospitals in Argentina. The study is a formative research study component aimed at tailoring interventions to reduce unnecessary caesarean section (CS) use. MethodsParticipants were postpartum women aged ≥15 years old in five hospitals in the provinces of Salta, Corrientes, Tucuman, Santa Fe and Buenos Aires City. Hospitals completed an institutional survey indicating the availability of obstetric services. The fieldwork was carried out from November 2018 to June 2019. Trained interviewers gathered data on obstetric history, companionship, mode of delivery preferences, and general opinion on vaginal and caesarean section births through semi-structured interviews. The interviews were coded and analysed with standard quantitative methods. ResultsThe five hospitals had a CS rate between 29.2 and 45.5. Four institutions indicated limited access to epidural and other pain management strategies and a restricted antenatal education schedule. The sample included 621 postpartum women with a mean age of 26 years (± SD 6). 60% of them had a vaginal birth (VB). More than 90% of women in three hospitals favoured VB, and in two, 67% (p
