To report the audiological outcomes of cochlear implantation in two patients with severe to profound sensorineural hearing loss secondary to superficial siderosis of the CNS and discuss some programming peculiarities that were found in these cases. Retrospective review. Data concerning clinical presentation, diagnosis and audiological assessment pre- and post-implantation were collected of two patients with superficial siderosis of the CNS. Both patients showed good hearing thresholds but variable speech perception outcomes. One patient did not achieve open-set speech recognition, but the other achieved 70% speech recognition in quiet. Electrical compound action potentials could not be elicited in either patient. Map parameters showed the need for increased charge. Electrode impedances showed high longitudinal variability. The implants were fairly beneficial in restoring hearing and improving communication abilities although many reprogramming sessions have been required. The hurdle in programming was the need of frequent adjustments due to the physiologic variations in electrical discharges and neural conduction, besides the changes in the impedances. Patients diagnosed with superficial siderosis may achieve limited results in speech perception scores due to both cochlear and retrocochlear reasons. Careful counseling about the results must be given to the patients and their families before the cochlear implantation indication.

Profound hearing loss is a disability that affects personality and when it involves teenagers before language acquisition, these bio-psychosocial conflicts can be exacerbated, requiring careful evaluation and choice of them for cochlear implant. AIM: To evaluate speech perception by adolescents with profound hearing loss, users of cochlear implants. STUDY DESIGN: Prospective. MATERIALS AND METHODS: Twenty-five individuals with severe or profound pre-lingual hearing loss who underwent cochlear implantation during adolescence, between 10 to 17 years and 11 months, who went through speech perception tests before the implant and 2 years after device activation. For comparison and analysis we used the results from tests of four choice, recognition of vowels and recognition of sentences in a closed setting and the open environment. RESULTS: The average percentage of correct answers in the four choice test before the implant was 46.9% and after 24 months of device use, this value went up to 86.1% in the vowels recognition test, the average difference was 45.13% to 83.13% and the sentences recognition test together in closed and open settings was 19.3% to 60.6% and 1.08% to 20.47% respectively. CONCLUSION: All patients, although with mixed results, achieved statistical improvement in all speech tests that were employed. OBJETIVO: Avaliar a percepção de fala em adolescentes portadores de surdez pré-lingual do tipo neurossensorial grave a profunda bilateral com implante coclear. FORMA DE ESTUDO: Prospectivo. CASUÍSTICA E MÉTODO: Vinte e cinco indivíduos com surdez pré-lingual, sensório-neural, de grau severo ou profundo, que foram submetidos ao implante coclear na adolescência, definida entre 10 a 17 anos e 11 meses, passaram por testes de percepção de fala pré-implante e 2 anos após a ativação do dispositivo. Para análise, utilizamos os resultados dos testes de four choice, reconhecimento de vogais e sentenças em conjunto fechado e aberto. RESULTADOS: A média da porcentagem de acertos do teste de four choice antes do implante coclear era de 46,9% e após, 2 anos de uso do dispositivo, a média foi para 86,1%; no teste de vogais, a média foi de 45,13% a 83,13%; e no teste de sentenças em conjunto fechado e aberto foi de 19,3% a 60,6% e de 1,08% a 20,47%, respectivamente. CONCLUSÃO: Embora com resultados heterogêneos, todos obtiveram melhora estatística nos testes aplicados.

Objetivo: comparar o reconhecimento de fala na situação de silêncio e ruído com a prótese auditiva de orelha média totalmente implantável Esteem® em relação à prótese auditiva. Descrever a satisfação auditiva e as complicações cirúrgicas encontradas. Desenho do Estudo: estudo exploratório, prospectivo, intragrupo, sujeito como seu próprio controle. Pacientes: usuários de prótese auditiva com perda neurossensoraial moderada a severa bilateral. Intervenção: dez pacientes submetidos a cirurgia para colocação da prótese totalmente implantável Esteem®. Aplicação de questionário SADL no pré- e pós-operatório. Desfechos: reconhecimento de fala no silêncio e ruído, resultados do SADL e complicações do procedimento foram avaliadas. Resultados: Não houve diferença estatisticamente significante entre reconhecimento de fala nas condições de silencio e ruído em relação as próteses auditivas (p=0,23 e p= 0,0438). Houve diferença estatisticamente significante no score do SADL pré- e pós-operatório (p= 0,0076). Houve três necessidades de reintervenção cirúrgica, em uma delas a prótese precisou ser removida. Conclusões: Melhora da satisfação auditiva, tendência a melhora no reconhecimento de fala nas situações de silêncio e ruído. Procedimento deve ser realizado por cirurgiões otológicos experientes. Objective: to compare speech discrimination in silence and in noise with the Esteem® totally implantable hearing device versus hearing aids, and to describe satisfaction with amplification and surgical complications in patients undergoing implantation of the Esteem® device. Study Design: this was an exploratory, prospective, within-group, subject-as-own-control study. Setting: tertiary referral center. Patients: hearing aid users with moderate to severe bilateral sensorineural hearing loss. Interventions: ten patients underwent placement of the Esteem® totally implantable hearing device. The Satisfaction with Amplification in Daily Life (SADL) questionnaire was administered pre- and postoperatively. Main Outcome Measures: speech discrimination in silence and noise, SADL scores, and complications were evaluated. Results: there were no statistically significant differences between hearing aids and the Esteem® device in speech discrimination in silence (p=0.23) or noise (p=0.0438). There was a significant difference between pre and postoperative SADL scores (p=0.0076). Three patients required reintervention, and one ultimately required explantation of the device. Conclusions: Use of the Esteem® device was associated with improved satisfaction with amplification and a trend toward improvement in speech discrimination in silence and noise. Implantation of the device is challenging and should be performed by experienced otologic surgeons.

OBJETIVO: Avaliar a percepção de fala em adolescentes portadores de surdez pré-lingual do tipo neurossensorial severa a profunda bilateral com implante coclear. FORMA DE ESTUDO: Prospectivo. CASUÍSTICA E MÉTODO: O estudo foi desenvolvido no Ambulatório de Implante Coclear do grupo de Otologia do Hospital das Clinicas da Faculdade de Medicina da USP. 25 indivíduos com surdez pré-lingual, sensório-neural, de grau severo ou profundo, que foram submetidos ao implante coclear na adolescência, definida entre 10 a 17 anos e 11 meses, passaram por testes de percepção de fala pré-implante coclear e 2 anos após a ativação do dispositivo. Para a comparação e análise utilizamos os resultados dos testes de four choice, reconhecimento de vogais e reconhecimento de sentenças em conjunto fechado e aberto. RESULTADOS: Devido a assimetria na distribuição dos dados, os mesmos estão sumarizados como mediana e intervalo interquartis (percentil 25 percentil 75). Houve variação estatisticamente significante entre as avaliações pré e pós- implante para todos os testes analisados. The results have very significant differences between pre-and post-implant for all tests analyzed. Conclusion: The extended retrolabyrinthine approach (RLA) is a safe and reliable approach for auditory brainstem placement in children. The surgical landmarks to reach cochlear nucleus are adequately exposed by this approach. Objective: To describe a new approach option for auditory brainstem implants (ABIs) in children, highlighting the anatomical landmarks to appropriately expose the foramen of Luschka. Methods: Three prelingually deafened children consecutively operated for ABIs via the RLA. Results: ABI placement via the RLA was successfully performed in all children without any further complications except multidirectional nystagmus in one child. The RLA we employed differed from that used for vestibular schwannoma only in the removal of the posterior semicircular canal. The lateral and superior semicircular canals and the vestibule remained intact, and there was no need to expose the dura of the internal auditory meatus. Cochlear implantation is a safe and reliable method for auditory restoration in patients with severe to profound hearing loss.To describe the surgical complications of cochlear implantation.Information from 591 consecutive multichannel cochlear implant surgeries were retrospectively analyzed. All patients were followed-up for at least one year. Forty-one patients were excluded because of missing data, follow-up loss or middle fossa approach.Of 550 cochlear implantation analyzed, 341 were performed in children or adolescents, and 209 in adults. The mean hearing loss time was 6.3 ± 6.7 years for prelingual loss and 12.1 ± 11.6 years for postlingual. Mean follow-up was 3.9 ± 2.8 years. Major complications occurred in 8.9% and minor in 7.8%. Problems during electrode insertion (3.8%) were the most frequent major complication followed by flap dehiscence (1.4%). Temporary facial palsy (2.2%), canal-wall lesion (2.2%) and tympanic membrane lesion (1.8%) were the more frequent minor complications. No death occurred.There was a low rate of surgical complications, most of them been successfully managed. These results confirm that cochlear implant is a safe surgery and most surgical complications can be managed with conservative measures or minimal intervention.

A cirurgia de implante coclear (IC) é método seguro e factível de restaurar audição em pacientes com surdez grave a profunda. OBJETIVO: Descrever complicações do IC. MATERIAL E MÉTODOS: Dados de 591 IC com tempo de seguimento de pelo menos 1 ano foram retrospectivamente analisados. Foram excluídos 41 pacientes por dados incompletos, acesso via fossa media ou perda de seguimento. RESULTADOS: Dos 550 IC analisados, 341 foram realizados em crianças e adolescentes e 209 em adultos. O tempo médio de surdez foi de 6,3 ± 6,7 anos para pré-linguais e 12,1 ± 11,6 anos para os pós-linguais. O tempo médio de seguimento foi 3,9 ± 2,8 anos. Complicações maiores ocorreram em 8,9% casos e menores em 7,8%. Problemas durante a inserção do feixe de eletrodos (3,8%) foi a complicação maior mais frequente, seguido pela deiscência de ferida operatória (1,4%). Paralisia facial temporária (2,2%), lesão muro facial (2,2%) e lesão de membrana timpânica (1,8%) foram as complicações menores mais frequentes. Nenhum óbito foi observado. CONCLUSÃO: Complicações cirúrgicas tiveram baixa ocorrência, sendo a maioria solucionada com sucesso. Esses resultados confirmam que o IC é uma cirurgia segura e a maioria das complicações cirúrgicas pode ser manejada com medidas conservadoras ou intervenções menores. INTRODUÇÃO: A exposição a níveis elevados de pressão sonora é a segunda causa mais comum de perda auditiva sensorioneural adquirida. Está relacionada à morte celular por apoptose, necrose e/ou necrose programada (necroptose) devido ao dano mecânico e/ou metabólico, ocasionando a degeneração de estruturas cocleares como células ciliadas, sobretudo externas, células de suporte e de fibras aferentes do nervo coclear. Estudos têm demostrado um aumento na citocina inflamatória TNFa após a exposição ao ruído, bem como a melhoria auditiva relacionada ao uso de etanercepte, que é um bloqueador destacitocina. Neste contexto, este estudo teve por objetivo avaliar o efeito do silenciamento gênico do Tnfa na audição de ratos Wistar, expostos ao ruído branco, e identificar o perfil de expressão gênica na via metabólica desse gene. METODOLOGIA: Foram incluídos ratos Wistar do sexo masculino, jovens com limiar auditivo no Peate clique de 50 dBSPL. Os animais foram submetidos à introdução do siRNA Tnfa em uma orelha, e siRNA scramble na orelha contralateral por via trans-timpânica com posterior exposição à 120 dBSPL de ruído branco por 3h. Foi realizado Peate clique e remoção das cócleas para obtenção do cDNA e avaliação da expressão gênica da via metabólica do Tnfa por meio de qRT-PCR. Um grupo de animais, não submetidos ao silenciamento, foi exposto ao ruído para análise dessa via metabólica. Para o cálculo da expressão gênica relativa (Rq) utilizamos o método do deltaCT comparativo e o teste t-Student pareado para avaliar os parâmetros no Peate clique. RESULTADOS: A taxa de silenciamento observada foi de 74,1%. A média do limiar eletrofisiológico nas orelhas silenciadas foi estaticamente melhor que a orelha scramble (p < 0,001) com valores médios de 49,5 +- 10.5. A amplitude média da onda I em 80 dBSPL e das ondas II e IV em 90 dBSPL foi estatisticamente maior na orelha siRNA Tnfa, sem alteração na latência nas intensidades audíveis,para ambos os lados. A exposição ao ruído promoveu aumento da expressão do gene Tnfa e de seu receptor Tnfrsf1A, 24h após à exposição, associado ao aumento da expressão de genes relacionados à apoptose e diminuição de genes relacionados à sobrevida celular. CONCLUSÃO: O ruído promoveu aumento de expressão do gene Tnfa e de genes envolvidos na apoptose, associado à perda auditiva em modelo experimental. A administração trans-timpânica do siRNA Tnfa promoveu preservação do limiar eletrofisiológico e da amplitude da onda I, II e IV no Peate clique, após exposição ao ruído intenso, sugerindo que a inibição deste pode ser uma estratégia de preservação auditiva promissora. O silenciamento do Tnfa inibiu a disfunção coclear após estímulo acústico, sugerindo que esta proteína é um dos principais agentes envolvidos na perda auditiva induzida pelo ruído, devendo ser considerada como alvo terapêutico na estratégia de preservação auditiva INTRODUCTION: Exposure to high levels of sound pressure is the second most common cause of acquired sensorineural hearing loss. It is related to cell death through apoptosis, necrosis and/or programmed necrosis (necroptosis) due to mechanical and/or metabolic damage, causing the degeneration of cochlear structures, such as cilliary cells, mainly external, as well as support cells and afferent fibers of the cochlear nerve. Studies have demonstrated an increase in the inflammatory cytokine TNFalfa after exposure to noise, as well as auditory improvement related to the use of etanercept, a cytokine blocker. In this context, this study aimed to evaluate the effect of Tnfa gene silencing on the hearing of Wistar rats exposed to white noise and to identify the expression profile in the metabolic pathway of this gene. METHODOLOGY: Young male Wistar rats with 50 dBSPL threshold in auditory brainstem responses click (ABR) were included in this study and submitted to the introduction of the TnfasiRNA in one ear and the scramble siRNA in the contralateral ear by trans-tympanic route, with subsequent exposure to 120 dB SPL of white noise for 3h. ABR was measured and the cochleae were dissected and used for the extraction of total RNA to obtain cDNA and conduct evaluations of the Tnfa metabolic pathway gene expression by qRT-PCR. A group of animals, not submitted to silencing, was exposed to noise to analyze this metabolic pathway. The relative gene expression (Rq) was calculated by the comparative deltaCT method and the paired Student t-test were applied to evaluate the parameters of the ABR click. RESULTS: The silencing rate was 74.1%. The mean electrophysiological threshold in the silenced ears was statistically higher than the scrambled ear (p < 0.001) with mean values of 49.5 +- 10.5. The mean amplitudes of wave I at 80 dBSPL and waves II and IV at 90dBSPL were statistically higher in the TnfasiRNA ear, with no change in latency at audible intensities for both sides. Noise exposure promoted increased expression of the Tnfa gene and its receptor, Tnfrsf1A, 24h after exposure, associated with increased expression of genes related to apoptosis and decreased expression of genes related to cell survival. CONCLUSIONS: Noise exposure promoted increased in Tnfa gene expression as well as in genes involved in apoptosis, associated with hearing loss in an experimental model. The trans-tympanic administration of TnfasiRNA promotes the preservation of the electrophysiological threshold and the amplitude of wave I, II and IV in the ABR click, after exposure to intense noise, suggesting that this inhibition may be a promising auditory preservation strategy. The Tnfa knockdown inhibited cochlear dysfunction after acoustic injury, suggesting that this protein plays an important role in noise induced hearing loss and should be considered a therapeutic target in auditory preservation strategies
