1. Aldehydes: What We Should Know About Them

Abstract: From Egyptian mummies to the Chanel N° 5 perfume, aldehydes have been used for a long time and continue to impact our senses in a wide range of perfumes, foods, and beverages. Aldehydes represent one of the categories of volatile organic compounds (VOCs), which are categorized as chemicals with boiling points up to 260 °C and can be found in indoor environments in the gaseous phase. Because of their potential or known hazardous properties for humans, the World Health Organization (WHO)-Europe provided some guidelines that may prevent several health risks. Indeed, some aldehydes, reported to be risky for humans, have been retired from the market, such as butylphenyl methylpropional (BMHCA). The purpose of this review is to summarize the most important aldehydes found indoors and outdoors and analyse in depth the toxicological aspects of these compounds, whose presence in perfumes is often underestimated. In addition, the ingredients’ synonyms that are reported in the literature for the same compound were unified in order to simplify their identification.

2. HIV-1 Structural Proteins or Cell-Signaling Factors? That Is the Question!

Abstract: The biological activity of structural HIV-1 proteins is not limited to ensuring a productive viral infection but also interferes with cellular homeostasis through intra- and extracellular signaling activation. This interference induces genomic instability, increases the lifespan of the infected cell by inhibiting apoptosis, and subverts cell senescence, resulting in unrestricted cell proliferation. HIV structural proteins are present in a soluble form in the lymphoid tissues and blood of infected individuals, even without active viral replication. The HIV matrix protein p17, the envelope glycoprotein gp120, the transenvelope protein gp41, and the capsid protein p24 interact with immune cells and deregulate the biological activity of the immune system. The biological activity of HIV structural proteins is also demonstrated in endothelial cells and some tumor cell lines, confirming the ability of viral proteins to promote cell proliferation and cancer progression, even in the absence of active viral replication. This review corroborates the hypothesis that HIV structural proteins, by interacting with different cell types, contribute to creating a microenvironment that is favorable to the evolution of cancerous pathologies not classically related to AIDS.

Abstract: In the developed world, pediatric obesity (PO) has been a major health concern since the last century, and this condition may lead to detrimental life-long physical and mental comorbidities. Currently, its prevalence has increased in low- and middle-income countries and in many high-income countries. Thus, the provision of effective and tailored care for children and their families has become vital. The social consequences of the COVID-19 pandemic are known everywhere, and among these, it has been argued that the COVID-19 pandemic has had a major impact on PO. Overall, the growth of PO over the last decade has been enhanced by the pandemic. During the COVID-19 pandemic, children, adolescents and young adults gained weight as the pediatric population dealt with sedentary lifestyles and changes in food habits. In this review, we want to highlight the impact that the COVID-19 pandemic had on PO.

Abstract: Background/Objectives: Infectious diseases represent a serious threat due to rising antimicrobial resistance, particularly among multidrug-resistant bacteria and influenza viruses. Metal-based complexes, such as N-heterocyclic carbene–gold (NHC–gold) complexes, show promising therapeutic potential due to their ability to inhibit various pathogens. Methods: Eight NHC–gold complexes were synthesized and tested for antibacterial activity against Escherichia coli, Enterococcus faecalis, Staphylococcus aureus, and for anti-influenza activity in lung and bronchial epithelial cells infected with influenza virus A/H1N1. Antibacterial activity was assessed through the determination of the minimum inhibitory concentration (MIC) and the minimum bactericidal concentration (MBC), while the viral load was quantified using qRT-PCR. Results: Complexes 3, 4, and 6 showed significant antibacterial activity at concentrations of 10–20 µg/mL. Additionally, these complexes significantly reduced viral load, with complexes 3 and 4 markedly inhibiting replication. Conclusions: These findings support the potential use of NHC–gold complexes in combined antimicrobial and antiviral therapies, representing an attractive option for fighting resistant infections.

Abstract: N-heterocyclic carbene (NHC)–gold and –silver complexes have attracted the interest of the scientific community because of their multiple applications and their versatility in being chemically modified in order to improve their biological properties. However, most of these complexes contain one or more chiral centers, and have been obtained and studied as racemic mixture. In particular, concerning the interesting biological and medicinal properties, many questions about how the chirality may influence these properties still remain unanswered. Aiming at a better understanding, herein a series of enantiopure NHC–gold and –silver complexes was synthesized, characterized and biologically evaluated in different in vitro systems. The individuated complexes exerted different properties based on the complexed metal and the specific configuration, with the (R)-gold–NHC complexes being the most active, particularly as anti-inflammatory molecules. Docking simulations indicated a different binding mode for each enantiomer. Moreover, anticancer and antibacterial activities were also evaluated for the considered enantiomers. Overall, the reported data may contribute to a better understanding of the different biological properties exerted by the enantiopure gold and silver complexes.

Abstract: More than 80 million people worldwide have been infected with the human immunodeficiency virus (HIV). There are now approximately 39 million individuals living with HIV/acquired immunodeficiency syndrome (AIDS). Although treatments against HIV infection are available, AIDS remains a serious disease. Combination antiretroviral therapy (cART), also known as highly active antiretroviral therapy (HAART), consists of treatment with a combination of several antiretroviral drugs that block multiple stages in the virus replication cycle. However, the increasing usage of cART is inevitably associated with the emergence of HIV drug resistance. In addition, the development of persistent cellular reservoirs of latent HIV is a critical obstacle to viral eradication since viral rebound takes place once anti-retroviral therapy (ART) is interrupted. Thus, several efforts are being applied to new generations of drugs, vaccines and new types of cART. In this review, we summarize the antiviral therapies used for the treatment of HIV/AIDS, both as individual agents and as combination therapies, and highlight the role of both macrophages and HIV cellular reservoirs and the most recent clinical studies related to this disease.

Abstract: Since ancient times, Aloe vera L. (AV) has attracted scientific interest because of its multiple cosmetic and medicinal properties, attributable to compounds present in leaves and other parts of the plant. The collected literature data show that AV and its products have a beneficial influence on human health, both by topical and oral use, as juice or an extract. Several scientific studies demonstrated the numerous biological activities of AV, including, for instance, antiviral, antimicrobial, antitumor, and antifungal. Moreover, its important antidepressant activity in relation to several diseases, including skin disorders (psoriasis, acne, and so on) and prediabetes, is a growing field of research. This comprehensive review intends to present the most significant and recent studies regarding the plethora of AV’s biological activities and an in-depth analysis exploring the component/s responsible for them. Moreover, its morphology and chemical composition are described, along with some studies regarding the single components of AV available in commerce. Finally, valorization studies and a discussion about the metabolism and toxicological aspects of this “Wonder Plant” are reported.

8. Oil Extraction from Hemp Plant as a Potential Source of Cannabidiol for Healthy Protein Foods

Abstract: In recent years, the increasing demand for alternative foods has shifted research toward new sources enriched with nutraceutical molecules. It is well known that many diseases are caused by oxidative stress; thus, the supplementation of antioxidants has been proposed to reduce it. Cannabis sativa L. is an interesting species that could provide an alternative source of antioxidants. This work aimed to investigate the possibility of optimizing the yield of cannabidiol (CBD) and recovering it from residual biomass (stalks), valorizing the residual biomass, and using this for protein bar preparation. Different extraction methods were used, and High-Pressure Liquid Chromatography (HPLC) analysis was used to analyze the extracts. Antioxidant power was investigated using the 2,2-Diphenyl-1-picrylhydrazyl (DPPH) and 2,2′-azinobis-3-ethylbenzothiazoline-6-sulfonic acid (ABTS) assays. The best results in terms of CBD yield were obtained via dynamic maceration after decarboxylation with a quantity of 26.7 ± 2 mgCBD/graw material from inflorescences. The extract also shows good antioxidant power with an IC50 value of 38.1 ± 1.1 µg/mL measured using the DPPH assay. The CBD extract was added to the hemp oil to obtain dough for protein bars. The doughs were studied by taking rheological and technological measurements, and it was found that the protein bars could provide an excellent means for the consumption of products enriched with antioxidants because their CBD anti-inflammatory activity is preserved after cooking.

Abstract: Antibacterial resistance is a renewed public health plague in modern times, and the COVID-19 pandemic has rekindled this problem. Changes in antibiotic prescribing behavior, misinformation, financial hardship, environmental impact, and governance gaps have generally enhanced the misuse and improper access to antibiotics during the COVID-19 pandemic. These determinants, intersected with antibacterial resistance in the current pandemic, may amplify the potential for a future antibacterial resistance pandemic. The occurrence of infections with multidrug-resistant (MDR), extensively drug-resistant (XDR), difficult-to-treat drug-resistant (DTR), carbapenem-resistant (CR), and pan-drug-resistant (PDR) bacteria is still increasing. The aim of this review is to highlight the state of the art of antibacterial resistance worldwide, focusing on the most important pathogens, namely Enterobacterales, Acinetobacter baumannii, and Klebsiella pneumoniae, and their resistance to the most common antibiotics.

Abstract: Urinary tract infections (UTIs) are among the most common bacterial infections worldwide, occurring in both community and healthcare settings. Although the clinical symptoms of UTIs are heterogeneous and range from uncomplicated (uUTIs) to complicated (cUTIs), most UTIs are usually treated empirically. Bacteria are the main causative agents of these infections, although more rarely, other microorganisms, such as fungi and some viruses, have been reported to be responsible for UTIs. Uropathogenic Escherichia coli (UPEC) is the most common causative agent for both uUTIs and cUTIs, followed by other pathogenic microorganisms, such as Klebsiella pneumoniae, Proteus mirabilis, Enterococcus faecalis, and Staphylococcus spp. In addition, the incidence of UTIs caused by multidrug resistance (MDR) is increasing, resulting in a significant increase in the spread of antibiotic resistance and the economic burden of these infections. Here, we discuss the various factors associated with UTIs, including the mechanisms of pathogenicity related to the bacteria that cause UTIs and the emergence of increasing resistance in UTI pathogens.

11. Bowel Angiodysplasia and Myocardial Infarction secondary to an ischaemic imbalance: a case report

Abstract: Angiodysplasia, defined as a vascular ectasia or arteriovenous malformation, is the most frequent cause of occult bleeding in patients older than 60 years and a significant association with several cardiac condition is described. Patients with anemia and negative findings on upper endoscopy and colonoscopy should be referred for further investigation of the small bowel. The investigation of choice, when available, is wireless capsule endoscopy. Several therapeutic options are available in this cases, as we reviewed in this report. We report a case of 78-year old man admitted to our Intensive Coronary Unit for dyspnea and chest pain. A diagnosis of non-ST-segment elevation acute coronary syndrome was made and a concomintant, significant anemia was found (hemoglobin 8.2 g/dl). No cororary disease was found by an angiography though the past medical history revealed systemic hypertension, chronic kidney disease (KDOQY stage III), and diabetes mellitus type II on insuline therapy. A Wireless Video capsule examination was positive for jejunum angiodysplasia and an argon plasma coagulation was chosen as terapeutic option. No subsequent supportive therapy and interventions were required in subsequent one year of follow-up.

Abstract: IntroductionFriedreich Ataxia (FRDA) is an autosomal recessive neurodegenerative disorder that causes gait and limb ataxia, dysarthria, and impaired vibratory sense, with cardiomyopathy being the predominant cause of death. There is no approved therapy, which results in the use of symptomatic treatments and the chronic support of physiotherapy. Dimethyl fumarate (DMF) is a fumaric acid ester used for the treatment of psoriasis and Multiple Sclerosis (MS). It induces Nrf2 in vitro and in vivo, and it increases frataxin in FRDA patient lymphoblasts, in mouse models, and in MS treated patients.MethodsThe aim of our study is to investigate if DMF can increase the expression of the FXN gene and frataxin protein and ameliorate in-vivo detectable measures of mitochondrial dysfunction in FRDA. The study is composed of a screening visit and two sequential 12-week phases: a core phase and an extension phase. During the first phase (core), patients will be randomly assigned to either the DMF or a placebo group in a 1:1 ratio. During the first week, patients will receive a total daily dose of 240 mg of DMF or placebo; from the second week of treatment, the dose will be increased to two 120 mg tablets BID for a total daily dose of 480 mg. During the second phase (extension), all patients will be treated with DMF. Secondary endpoints will be frataxin protein level, cardiopulmonary exercise test outputs, echocardiographic measures, Nrf2 pathway and mitochondrial biogenesis gene expression, safety, clinical scales, and quality of life scales.ConclusionsThis is the first study aimed at exploring the ability of DMF, an already available treatment for MS and psoriasis, to correct the biological deficits of FRDA and potentially improve mitochondrial respiration in-vivo.

Abstract: Sacubitril/Valsartan (sac/val) has improved clinical prognosis in patients affected by heart failure (HF) with reduced ejection fraction (HFrEF). HF and type 2 diabetes mellitus (T2DM) frequently coexist, with a prevalence of T2DM of 35%–40% in patients with HF. T2DM is the third co-morbidities in patients with HF and a strong independent risk factor for the progression of HF. In a post hoc analysis of PARADIGM-HF, improved glycemic control was shown in patients with T2DM and HFrEF receiving sac/val compared to enalapril at 12 months of follow-up. The aim of the present study was to evaluate, in a series of repeated observations in 90 HFrEF patients, the long term effect of sac/val treatment on renal function, glycometabolic state and insulin sensitivity parameters, according to diabetic status. We studied 90 patients (74 men and 16 women, mean age 68 ± 10 years, 60 diabetics and 30 non-diabetics) suffering from HFrEF and still symptomatic despite optimal pharmacological therapy. Abstract: Abstract Purpose This study was a quality-control study of resting and exercise Doppler echocardiography (EDE) variables measured by 19 echocardiography laboratories with proven experience participating in the RIGHT Heart International NETwork. Methods All participating investigators reported the requested variables from ten randomly selected exercise stress tests. Intraclass correlation coefficients (ICC) were calculated to evaluate the inter-observer agreement with the core laboratory. Inter-observer variability of resting and peak exercise tricuspid regurgitation velocity (TRV), right ventricular outflow tract acceleration time (RVOT Act), tricuspid annular plane systolic excursion (TAPSE), tissue Doppler tricuspid lateral annular systolic velocity (S’), right ventricular fractional area change (RV FAC), left ventricular outflow tract velocity time integral (LVOT VTI), mitral inflow pulsed wave Doppler velocity (E), diastolic mitral annular velocity by TDI (e’) and left ventricular ejection fraction (LVEF) were measured. Results The accuracy of 19 investigators for all variables ranged from 99.7 to 100%. ICC was > 0.90 for all observers. Inter-observer variability for resting and exercise variables was for TRV = 3.8 to 2.4%, E = 5.7 to 8.3%, e’ = 6 to 6.5%, RVOT Act = 9.7 to 12, LVOT VTI = 7.4 to 9.6%, S’ = 2.9 to 2.9% and TAPSE = 5.3 to 8%. Moderate inter-observer variability was found for resting and peak exercise RV FAC (15 to 16%). LVEF revealed lower resting and peak exercise variability of 7.6 and 9%. Conclusions When performed in expert centers EDE is a reproducible tool for the assessment of the right heart and the pulmonary circulation.

Abstract: The article presents the first results of MIRACLE research, aimed at designing, testing, and implementing innovative reinforcement systems with bio-composite matrix, with additives containing natural thermal-plasters, usable in the restoration of residential buildings created before 1945. Starting from a brief description of the properties of fibre-reinforced cement organic matrices, supported by a survey of international research aimed at developing innovative methodologies, systems, and components used to improve the energy and mechanical performances of listed historic buildings, the most suitable thermal-plasters will be analysed (among those existing on the Italian market), identifying the performance characteristics both in terms of mechanic and thermal and environmental sustainability.

Abstract: There are important differences in epidemiology, pathophysiology, HF patterns, prognosis, and treatment. Women have a higher incidence of HFpEF due to sex-specific factors (such as anthropometry, role of estrogens, pregnancy-related cardiomyopathies), increased incidence of comorbidities, and gender-specific conditions. Men instead present a predisposition to the development of HFrEF due to a higher incidence of coronary artery disease and myocardial infarction. However, there are still gaps in the management of women with HF. The poor inclusion of women in clinical trials may have contributed to a lesser understanding of disease behavior than in men. In addition, a full understanding of gender-specific factors that are studied in small populations is lacking in the literature, and only in recent years, studies have increased their focus on this issue. Understanding how society, family, and environment affect the prognosis of HF patients may help clinicians provide more appropriate levels of care. In this overview, we aimed at summarizing all the key available evidence regarding sex/gender differences in heart failure.

Abstract: Heart Failure with preserved Ejection Fraction (HFpEF) is nowadays considered a major healthcare issue. According to forecasts two third of all Heart Failure patients will belong to this phenotype by year 2050, overwhelming those affected by Heart Failure with reduced Ejection Fraction (HFrEF). Both epidemiological and mechanistic studies support the concept that HFpEF represents true HF although aggravated by a collection of comorbidities. There is urgent need of improving its phenotyping due to the high degree of disease heterogeneity within HFpEF that lead to the failure of randomized clinical trials in demonstrating a remarkable impact of drugs in improving its morbidity and mortality.

Abstract: Acute heart failure (AHF) is the most frequent cause of unplanned hospital admission in patients of >65 years of age and it is associated with significantly increased morbidity, mortality, and healthcare costs. Different AHF classification criteria have been proposed, mainly reflecting the clinical heterogeneity of the syndrome. Regardless of the underlying mechanism, peripheral and/or pulmonary congestion is present in the vast majority of cases. Furthermore, a marked reduction in cardiac output with peripheral hypoperfusion may occur in most severe cases. Diagnosis is made on the basis of signs and symptoms, laboratory, and non-invasive tests. After exclusion of reversible causes, AHF therapeutic interventions mainly consist of intravenous (IV) diuretics and/or vasodilators, tailored according to the initial hemodynamic status with the addition of inotropes/vasopressors and mechanical circulatory support if needed. The aim of this review is to discuss current concepts on the diagnosis and management of AHF in order to guide daily clinical practice and to underline the unmet needs. Preventive strategies are also discussed.

Abstract: Alergia; Anafilaxia; Asma Allergy; Anaphylaxis; Asthma Al·lèrgia; Anafilaxi; Asma Since the discovery of immunoglobulin E (IgE) as a mediator of allergic diseases in 1967, our knowledge about the immunological mechanisms of IgE-mediated allergies has remarkably increased. In addition to understanding the immune response and clinical symptoms, allergy diagnosis and management depend strongly on the precise identification of the elicitors of the IgE-mediated allergic reaction. In the past four decades, innovations in bioscience and technology have facilitated the identification and production of well-defined, highly pure molecules for component-resolved diagnosis (CRD), allowing a personalized diagnosis and management of the allergic disease for individual patients. The first edition of the “EAACI Molecular Allergology User's Guide” (MAUG) in 2016 rapidly became a key reference for clinicians, scientists, and interested readers with a background in allergology, immunology, biology, and medicine. Nevertheless, the field of molecular allergology is moving fast, and after 6 years, a new EAACI Taskforce was established to provide an updated document. The Molecular Allergology User's Guide 2.0 summarizes state-of-the-art information on allergen molecules, their clinical relevance, and their application in diagnostic algorithms for clinical practice. It is designed for both, clinicians and scientists, guiding health care professionals through the overwhelming list of different allergen molecules available for testing. Further, it provides diagnostic algorithms on the clinical relevance of allergenic molecules and gives an overview of their biology, the basic mechanisms of test formats, and the application of tests to measure allergen exposure.

Abstract: Background: Hypertension management in older patients represents a challenge, particularly when hospitalized. Objective: The objective of this study is to investigate the determinants and related outcomes of antihypertensive drug prescription in a cohort of older hospitalized patients. Methods: A total of 5671 patients from REPOSI (a prospective multicentre observational register of older Italian in-patients from internal medicine or geriatric wards) were considered; 4377 (77.2%) were hypertensive. Minimum treatment (MT) for hypertension was defined according to the 2018 ESC guidelines [an angiotensin-converting-enzyme-inhibitor (ACE-I) or an angiotensin-receptor-blocker (ARB) with a calcium-channel-blocker (CCB) and/or a thiazide diuretic; if >80 years old, an ACE-I or ARB or CCB or thiazide diuretic]. Determinants of MT discontinuation at discharge were assessed. Study outcomes were any cause rehospitalization/all cause death, all-cause death, cardiovascular (CV) hospitalization/death, CV death, non-CV death, evaluated according to the presence of MT at discharge. Results: Hypertensive patients were older than normotensives, with a more impaired functional status, higher burden of comorbidity and polypharmacy. A total of 2233 patients were on MT at admission, 1766 were on MT at discharge. Discontinuation of MT was associated with the presence of comorbidities (lower odds for diabetes, higher odds for chronic kidney disease and dementia). An adjusted multivariable logistic regression analysis showed that MT for hypertension at discharge was associated with lower risk of all-cause death, all-cause death/hospitalization, CV death, CV death/hospitalization and non-CV death. Conclusions: Guidelines-suggested MT for hypertension at discharge is associated with a lower risk of adverse clinical outcomes. Nevertheless, changes in antihypertensive treatment still occur in a significant proportion of older hospitalized patients.

Abstract: Aims The association between hyperglycemia at hospital admission and relevant short- and long-term outcomes in elderly population is known. We assessed the effects on mortality of hyperglycemia, disability, and multimorbidity at admission in internal medicine ward in patients aged ≥ 65 years. Methods Data were collected from an active register of 102 internal medicine and geriatric wards in Italy (RePoSi project). Patients were recruited during four index weeks of a year. Socio-demographic data, reason for hospitalization, diagnoses, treatment, severity and comorbidity indexes (Cumulative Illness rating Scale CIRS-SI and CIRS-CI), renal function, functional (Barthel Index), and cognitive status (Short Blessed Test) and mood disorders (Geriatric Depression Scale) were recorded. Mortality rates were assessed in hospital 3 and 12 months after discharge. Results Of the 4714 elderly patients hospitalized, 361 had a glycemia level ≥ 250 mg/dL at admission. Compared to subjects with lower glycemia level, patients with glycemia ≥ 250 mg/dL showed higher rates of male sex, smoke and class III obesity. These patients had a significantly lower Barthel Index (p = 0.0249), higher CIRS-SI and CIRS-CI scores (p = 0.0025 and p = 0.0013, respectively), and took more drugs. In-hospital mortality rate was 9.2% and 5.1% in subjects with glycemia ≥ 250 and p = 0.0010). Regression analysis showed a strong association between in-hospital death and glycemia ≥ 250 mg/dL (OR 2.07; [95% CI 1.34–3.19]), Barthel Index ≤ 40 (3.28[2.44–4.42]), CIRS-SI (1.87[1.27–2.77]), and male sex (1.54[1.16–2.03]). Conclusions The stronger predictors of in-hospital mortality for older patients admitted in general wards were glycemia level ≥ 250 mg/dL, Barthel Index ≤ 40, CIRS-SI, and male sex.

Abstract: Early detection and treatment of cancer have led to a noticeable reduction in both mortality and morbidity. However, chemotherapy and radiotherapy could exert cardiovascular (CV) side effects, impacting survival and quality of life, independent of the oncologic prognosis. In this regard, a high clinical index of suspicion is required by the multidisciplinary care team in order to trigger specific laboratory tests (namely natriuretic peptides and high-sensitivity cardiac troponin) and appropriate imaging techniques (transthoracic echocardiography along with cardiac magnetic resonance, cardiac computed tomography, and nuclear testing (if clinically indicated)), leading to timely diagnosis. In the near future, we do expect a more tailored approach to patient care within the respective community along with the widespread implementation of digital health tools.

Abstract: Data from the "Trattamento Ormonale nello Scompenso CArdiaco" (T.O.S.CA) registry showed that heart failure (HF) represents a complex clinical syndrome with different hormonal alterations. Renal failure represents a frequent complication in HF. We evaluated the relationship between renal function and insuline-like growth factor-1 (IGF-1) deficiency and its impact on cardiovascular mortality (CVM) in patients enrolled in the T.O.S.CA. registry.At the enrolment, all subjects underwent chemistry examinations, including circulating hormones and cardiovascular functional tests. COX regression analysis was used to evaluate factors related to CVM during the follow-up period in all populations, in high-risk patients and in the young-adult population. Also, we evaluate the effects of renal function on the CVM.337 patients (41 deceased) were analyzed. CVM was related to severe renal dysfunction (HR stages IV-V = 4.86), high-risk conditions (HR 2.25), serum IGF-1 (HR 0.42), and HF etiology (HR 5.85 and HR 1.63 for valvular and ischemic etiology, respectively). In high-risk patients, CVM was related to IGF-1 levels, severe renal dysfunction and valvular etiology, whereas in young patients CMV was related to the high-risk pattern and serum IGF-1 levels.Our study showed the clinical and prognostic utility of the IGF-1 assay in patients with HF.

Abstract: Data regarding further risk stratification of intermediate-risk pulmonary embolism (IR-PE) are scanty. Whether transthoracic echocardiography may be helpful in further risk assessment of death in such population has still to be proven. Two-hundred fifty-four consecutive patients (51.6% females, age 63.7 ± 17.3 years) with IR-PE admitted to a tertiary regional referral center were enrolled. Patients underwent a complete transthoracic echocardiography within 36 h from hospital admission, on top of clinical assessment, physical examination, computer tomography pulmonary angiography (CTPA), and serum measurement of Troponin I (TnI) levels. The occurrence of 90 day mortality was chosen as primary outcome measure. When compared to survivors, non-surviving IR-PE patients had smaller left-ventricular end-diastolic volumes (39.8 ± 20.9 vs 49.4 ± 19.9 ml/m2, p = 0.006) with reduced stroke volume index (SVi) (24.7 ± 10.9 vs 30.9 ± 12.6 ml/m2, p: 0.004) and time–velocity integral at left-ventricular outflow tract (VTILVOT) (0.17 ± 0.03 vs 0.20 ± 0.04 m, p = 0.0001), whereas no differences were recorded regarding right heart parameters. Cox regression analysis revealed that right atrial enlargement (RAE) (HR 3.432, 5–95% CI 1.193–9.876, p: 0.022), the ratio between tricuspid annulus plane excursion and pulmonary arterial systolic pressure (TAPSE/PASp) (HR 4.833, 5–95% 1.230–18.986, p = 0.024), as well as SVi (HR 11.199, 5–95% CI 2.697–48.096, p = 0.001) and VTILVOT (HR 4.212, 5–95% CI 1.384–12.820, p = 0.011) were powerful independent predictors of mortality. Neither CTPA RV/LV nor TnI resulted associated with impaired survival. In intermediate-risk pulmonary embolism, RAE, TAPSE/PASp ratio, SVi, and VTILVOT predict independently prognosis to a greater extent than CTPA and TnI.

Abstract: Background. Systemic arterial hypertension (HTN) is the main risk factor for the development of heart failure with preserved ejection fraction (HFpEF). The aim of the study was was to assess the trends in PASP, E/E’ and TAPSE during exercise Doppler echocardiography (EDE) in hypertensive (HTN) patients vs. healthy subjects stratified by age. Methods. EDE was performed in 155 hypertensive patients and in 145 healthy subjects (mean age 62 ± 12.0 vs. 54 ± 14.9 years respectively, p < 0.0001). EDE was undertaken on a semi-recumbent cycle ergometer with load increasing by 25 watts every 2 min. Left ventricular (LV) and right ventricular (RV) dimensions, function and hemodynamics were evaluated. Results. Echo-Doppler parameters of LV and RV function were lower, both at rest and at peak exercise in hypertensives, while pulmonary hemodynamics were higher as compared to healthy subjects. The entire cohort was then divided into tertiles of age: at rest, no significant differences were recorded for each age group between hypertensives and normotensives except for E/E’ that was higher in hypertensives. At peak exercise, hypertensives had higher pulmonary artery systolic pressure (PASP) and E/E’ but lower tricuspid annular plane systolic excursion (TAPSE) as age increased, compared to normotensives. Differences in E/E’ and TAPSE between the 2 groups at peak exercise were explained by the interaction between HTN and age even after adjustment for baseline values (p < 0.001 for E/E’, p = 0.011 for TAPSE). At peak exercise, the oldest group of hypertensive patients had a mean E/E’ of 13.0, suggesting a significant increase in LV diastolic pressure combined with increased PASP. Conclusion. Age and HTN have a synergic negative effect on E/E’ and TAPSE at peak exercise in hypertensive subjects.

Abstract: A metabolic imbalance between anabolic drive and catabolic forces is commonly observed in chronic heart failure (CHF) patients, with the latter prevailing over anabolic hormones. Moreover, anabolic deficiencies are independent markers of poor prognosis. This finding represents a solid background for the implementation of therapeutic trials based on replacement therapy. The somatotropic axis (GH/IGF-1) is the most powerful anabolic axis of the body and its decline is related with a poor outcome and a worse clinical status. Growth hormone (GH) administration may enter the therapeutic arena as adjunctive treatment in patients affected by CHF and GH/IGF-1 deficiency. The T.O.S.CA. project aims at investigating the relationship between CHF and hormonal deficiency.

Abstract: In recent years, the remarkable progress achieved in terms of survival after myocardial infarction have led to an increased incidence of chronic heart failure in survivors. This phenomenon is due to the still incomplete knowledge we possess about the complex pathophysiological mechanisms that regulate the response of cardiac tissue to ischemic injury. These involve various cell types such as fibroblasts, cells of the immune system, endothelial cells, cardiomyocytes and stem cells, as well as a myriad of mediators belonging to the system of cytokines and not only. In parallel with the latest findings on post-infarct remodeling, new potential therapeutic targets are arising to halt the progression of disease. After the evaluation of the results obtained from gene therapy and stem cells, in this part we evaluate micro- RNA, post-translational modification and microspheres based therapy.

Abstract: The manuscript provides a wide database for historic masonry towers collecting their modal, mechanical and geometrical features. This large amount of data was collected according to a literature review, then summing up as many as 56 different case studies. The collected data was then used to identify the main physical parameters influencing the modal behaviour of slender masonry towers, including the openings on the facades. After a critical discussion of the data reported in the database, the existing empirical or semi-empirical formulations available for the estimation of the first natural frequency of such structural typology are first evaluated. Subsequently the effects of the openings, as revealed through the analysis of the experimental results reported in the database, are discussed by comparison with a simple but effective numerical model. Despite the difficulties to quantify the effect of the openings along the height of the tower on the main frequency due to a cross-contribution of mass and stiffness, a simple but effective formulation is proposed which is able to account for this parameter.

Abstract: Several bluff bodies in an airflow, such as rectangular cylinders with moderate side ratio, in particular conditions of mass and damping can experience the interference of vortex-induced vibration (VIV) and galloping. This promotes a combined instability, which one may call “unsteady galloping”, with peculiar features and possibly large vibration amplitudes in flow speed ranges where no excitation is predicted by classical theories. The mathematical model proposed between the 70's and the 80's by Prof. Y. Tamura to simulate this phenomenon was considered here for the case study of a two-dimensional rectangular cylinder with a side ratio of 1.5, having the shorter section side perpendicular to the smooth airflow. This wake-oscillator model relies on the linear superposition of the unsteady wake force producing VIV excitation and the quasi-steady force that is responsible for galloping. The model formulation was slightly modified, and the way to determine a crucial parameter was changed, revealing a previously unexplored behavior of the equations. In the present form, the model is able to predict the dynamic response of the rectangular cylinder with a satisfactory qualitative and, to a certain extent, quantitative agreement with the experimental data, although the limitations of the present approach are clearly highlighted in the paper. The mathematical modeling of unsteady galloping and the analysis of the results offer a deep insight into this complicated phenomenon and its nonlinear features. The model also represents a useful engineering tool to estimate the vibration of a structure or structural element for which the interference of VIV and galloping is envisaged.

Abstract: Slender structures with a bluff non-axisymmetric cross-section are prone to both vortex-induced vibration (VIV) and transverse galloping. When the mass-damping parameter of the system is low, the two phenomena can interfere giving rise to a peculiar type of instability, for which the quasi-steady theory does not apply, and therefore which one may call “unsteady galloping”. Since for large structures (such as high-rise towers or bridge pylons) this phenomenon seems to be potentially an issue, rather than the quasi-steady galloping, it is particularly important to verify whether the former also occurs in realistic turbulent wind flows, and to understand its specific features. With this aim in mind, static and dynamic wind tunnel tests have been carried out on a two-dimensional rectangular cylinder with a side ratio of 1.5 (having the short side perpendicular to the flow) immersed in various grid-induced homogeneous isotropic turbulent flows. From a quasi-steady perspective, the static tests showed the proneness to galloping instability of the considered cross-section even in highly turbulent flow, though strongly dependent on the integral length scale of turbulence. In addition, they revealed an attenuation of the strength of vortex shedding, but suggested an increased tendency of VIV and galloping to interfere as compared to the smooth-flow case. The dynamic tests confirmed this tendency and highlighted a complicated behavior of the model in turbulent flow, with some features that still remain unexplained. In particular, even larger values of the mass-damping parameter of the system are necessary for the quasi-steady theory to be able to predict correctly the galloping instability threshold. Another important result is that the integral scale of turbulence was found to play a key role also in the unsteady galloping behavior of the considered rectangular cylinder.

Abstract: Background According to current guidelines, the presence of right ventricle dysfunction (RVD) in patients with pulmonary embolism (PE) is a turning point for clinical decision making. Right ventricle-arterial coupling (RVAC) is defined as the capability of the right ventricle to compensate the rise of pulmonary pressures by increasing its contractility. RVAC can easily and quickly be assessed with echocardiography through the ratio between the tricuspid anteroposterior systolic elevation (TAPSE) and the pulmonary arterial systolic pressure (PASp) (TAPSE/PASp) Aim of the current study was to investigate right ventricle-arterial coupling in patients admitted for PE. Methods Consecutive patients admitted in a large tertiary center for PE were retrospectively analyzed, enrolling all subjects with RVD. For each patient we obtained, at the admission: age, sex, body surface area, heart rate, systolic pressure, respiratory frequency and the main echocardiographic findings. We treated the TAPSE/PASp ratio both as continuous and dichotomous variable, adopting a cutoff of 0.32, derived from current literature on pulmonary arterial hypertension. BOVA score was treated as a dichotomous variable, subdividing patients in low-intermediate risk (BOVA≤4, stages I-II) and high risk (BOVA>4, stage III). Continuous variables were related with linear regression analysis and compared with t-test and ANOVA. Results we finally obtained a sample of 109 consecutive subjects. Patients in BOVA stage III showed, when compared with the other subjects: a significant reduction of TAPSE, TAPSE/PASp, significantly lower telediastolic and telesystolic volumes in left ventricle and a significant reduction of stroke volume and VTILVOT. Patients with a TAPSE/PASp ratio ≤0.32 showed: a higher RV/LV ratio, a higher prevalence of paradoxical septum movement, a significant reduction of TAPSE, a significantly reduced telediastolic and telesistolic volumes and a significant reduction of stroke volume and VTILVOT. Conclusion Patients with PE and RVD with a low TAPSE/PASp ratio show significantly worse echocardiographic parameters, suggesting that an alteration of RVAC could have strong implications in the ventricular interdependency and could be suggestive of a worse haemodynamic status. Patients in stage BOVA III show a worse RVAC and, consequently, worse LV performances thus representing the group at highest risk of hemodynamic instabilisation.

Abstract: The aerodynamics of a stationary two-dimensional cylinder with rectangular 5:1 cross section is the object of an international experimental and numerical benchmark study (BARC). The unsteady flow past such a bluff body is governed by the impinging shear-layer instability mechanism, which is also responsible for potential flow-induced vibrations. This paper discusses the characteristics of the high-Reynolds-number unsteady flow over the cylinder, investigated in the wind tunnel through pressure and force measurements on a sectional model in smooth and turbulent flows. In addition, hot-wire anemometry measurements of the velocity in the wake of the cylinder were carried out in smooth flow. The study is particularly focused on understanding the influence of free-stream turbulence and angle of attack on the aerodynamic behavior of the body. Key features are highlighted, such as a slight Reynolds-number dependence of force coefficients and Strouhal number in smooth flow, or the turbulence-induced variation of the length of the separation bubbles along the lateral sides of the cylinder. Incoming turbulence is also found to affect the vortex-shedding mechanism of impinging shear-layer instability, promoting the switch from a dominant mode to another. Finally, the wake measurements described some vortex-shedding features, confirming a slight Reynolds-number dependence of the Strouhal frequency.

Abstract: Depression is often under-recognized in older patients, even if antidepressants (AD) are commonly prescribed, with a prevalence of use that increase with ageing [ 1 ]. Nevertheless, even if a diagnosis of depression is established, inappropriate treatment can occur [ 2 ]. Beers criteria are the most widely screening tools used to detect inappropriate prescription of drugs in people aged 65 years or more [ 3 ]. Since 2010, attempts to adapt the Beers' criteria have been made in Europe [ 4 , 5 ]. Tricyclic drugs are the ADs to be always avoided in the elderly, owing to their anticholinergic side effects, such as cognitive impairment, delirium, urinary retention and falls [ 3 ]. Selective serotonin reuptake inhibitors (SSRIs) should be used with caution because they can cause or exacerbate hyponatraemia [ 3 ]. To our knowledge no studies have been conducted on recent data collected in hospitalized older patients in order to assess the appropriateness of prescription of ADs. With this background, the objectives of this study were to investigate the prevalence of AD use and prescription appropriateness, according to the above-cited criteria, in a large cohort of older patients hospitalized in Italian internal medicine and geriatric wards from 2010 to 2017.

Abstract: The aims of this study were to assess the prevalence of use and prescription appropriateness of drugs for peptic ulcer and gastrooesophageal reflux disease (GERD) at hospital admission and discharge. Patients aged 65 years or more hospitalized from 2010 to 2016 in 101 Italian internal medicine and geriatric wards in the context of the REPOSI register were scrutinized to assess if they were prescribed with drugs for peptic ulcer and GERD at hospital admission and discharge. Appropriateness of prescription was assessed considering the presence of specific conditions (i.e., history of peptic ulcer or gastrointestinal hemorrhages, advanced age, Helicobacter Pylori) or gastro-toxic drug combinations, according to the criteria provided by the reimbursement rules of the Agenzia Italiana del Farmaco (NOTA 1 and 48). Among 4715 enrolled patients, 3899 were discharged alive. At hospital discharge, 2412 (61.9%, 95%CI: 60.3–63.4%) patients were prescribed with drugs for peptic ulcer and GERD, a 12% of increase from hospital admission. Almost half of the patients (N = 1776, 45.6%, 95%CI: 44.0–47.1%) were inappropriately prescribed or not prescribed: among the drugs for peptic ulcer and GERD users, about 60% (1444/2412) were overprescribed, and among nonusers, 22% (332/1487) were underprescribed. Among patients newly prescribed at hospital discharge, 60% (392/668) were inappropriately prescribed. The appropriateness of drugs for peptic ulcer and GERD therapy decreased by 3% from hospital admission to discharge. Hospitalization missed the opportunity to improve the quality of prescription of this class of drug.

Abstract: The formulas provided by the standards or available in the literature for the calculation of the elastic modulus don’t consider many variables that influence the results of flat jack tests such as, first of all, the influence of the wall thickness. In this paper, flat-jack tests were performed - in the laboratory - on masonry wall specimens having the same geometrical and mechanical properties but different thickness. Then, a numerical assessment by FEM was carried out in order to extend the experimental evidences to the case of masonry walls with large thickness. This study allows to more accurately predict the masonry elastic modulus by double flat-jack test and it provides important data useful for the research activity as well as for the engineering practice.

Abstract: The numerical model of existing masonry buildings, especially in case of monumental constructions, must consider the unavoidable lack of knowledge and the consequent effects of the uncertain parameters (material properties, geometry, boundary conditions, etc.). In this work, a Bayesian approach is proposed to update the finite element model of masonry towers by using experimental data. The towers of San Gimignano (Italy) were considered as an effective case study to test this approach thanks to the availability both of geometric data and dynamic measurements. The possibility to obtain a reliable numerical model is relevant also from the point of view of the seismic risk assessment, which is a crucial issue to ensure the conservation of heritage over the centuries. In fact, both seismic capacity and demand are strictly dependent on the dynamic characteristics of the structure, and a reliable updating of the modal properties of numerical models plays a primary role in the assessment of the seismic risk. In this respect, Bayes’ theorem is herein employed to convert the prior distribution of the elastic modulus E , into the posterior distribution by using the experimental data (first and second natural period). The measurement errors were accounted for by means of a Gaussian distribution centred on the measured values of the natural periods. In addition, modelling uncertainties were defined to incorporate the lack of knowledge on the restraint effect caused by the neighbouring buildings. The oscillating height of the tower was modelled according to a lognormal distribution whose interval starts from the height of the confined buildings to the tower top. Particular attention was devoted to the parameters involved in the Bayesian procedure to define their effect on the obtained posterior distributions. The achieved results encourage the spread this approach, already employed in many engineering fields, to the safeguard of cultural heritage. In view of further challenges, the methodology will be extended to seismic analyses, aiming to obtain the assessment of the seismic risk of the particular structural typology herein considered: the historic masonry towers.

45. Interference of vortex-induced vibration and transverse galloping for a rectangular cylinder

Abstract: The phenomenon of interference between vortex-induced vibration (VIV) and galloping in the transverse degree of freedom was studied in the wind tunnel in the case of a spring-mounted slender rectangular cylinder with a side ratio of 1.5 having the short side perpendicular to the flow. The tests were carried out in a wide Scruton number range, starting from low values and increasing it in small steps by using eddy-current viscous dampers. This study helped understanding the dynamics of the interaction between the two excitation mechanisms and clearly highlighted the transition through four regimes of VIV-galloping interference. It was found that a high value of the mass-damping parameter is required to decouple the ranges of excitation of vortex-induced vibration and galloping completely, and for the quasi-steady theory to predict the galloping critical wind speed correctly. This conclusion is also relevant from the engineering point of view, as it means that structures and structural elements with ordinary mass-damping properties can exhibit sustained vibrations in flow speed ranges where no excitation is predicted by classical theories of vortex-induced vibration and galloping. Although most of the experimental tests were conducted in smooth flow at zero angle of attack, the paper also discusses the sensitivity of the results to a small variation of the mean flow incidence and to the presence of a low-intensity free-stream turbulence.

46. Aeroelastic stability of two long-span arch structures: A collaborative experience in two wind tunnel facilities

Abstract: In this paper, a rare example of comparison between sectional and full-aeroelastic model tests is presented. Interestingly, the experiments were conducted in two very different wind tunnel facilities by different research teams. The study concerns two long-span steel arch structures recently built in Milan, Italy, for Expo 2015 World Fair. The structures have only aesthetic purposes and are therefore very flexible and light, which makes them sensitive to wind-induced excitation and prone to aeroelastic instabilities. In particular, in smooth flow an interesting phenomenon of interference between vortex-induced vibration and galloping was observed up to high values of the Scruton number. This aeroelastic instability is very dangerous as large-amplitude vibrations can occur in wind speed ranges where they are not expected, at least for what classical theories for vortex-induced vibration and quasi-steady galloping are concerned. Moreover, the provisions of Eurocode 1 resulted clearly unsuitable and non-conservative to address such a phenomenon. Despite the differences in the facilities and in the models, a good agreement was found between the results obtained in the two laboratories. The major discrepancies were observed in the transitional behavior for intermediate values of the Scruton number, the sectional model showing a more unstable behavior. The tests on the full-aeroelastic model also allowed considering the effect of the angle of wind exposure of the structures, both the in-plane and the out-of-plane vibrations of the arches and the dynamic response to turbulent wind. In particular, a set of tests in smooth flow was performed accounting for the presence of the other arch and of the surrounding buildings. A particular dynamic excitation of the in-plane flexural modes of the structures was observed in well defined ranges of flow speeds when one arch is in the wake of the other. Finally, both experimental campaigns highlighted the need for the installation of tuned mass dampers on the real structures to guarantee their safety. The effectiveness of these devices against the observed galloping-type instability was also verified through wind tunnel tests on the full-aeroelastic model.

Abstract: Benzodiazepines (BDZs) are widely prescribed in older people. The aims of the study are to assess the prevalence of inappropriate prescription of BZDs and the associated factors in acutely hospitalized older patients. Patients aged 65 years or more hospitalized from 2010 to 2017 in more than 100 Italian internal medicine and geriatric wards in the frame of the REPOSI register were included if prescribed with BDZs at hospital admission or discharge. Appropriateness of prescription was assessed according to the 2015 Beers criteria and their modified French and German versions. Among 4681 patients discharged from hospital, 15% (N = 710) were discharged with BDZs, and 62% of them (N = 441, 95% CI: 58.5%–65.6%) were inappropriately prescribed, being prescribed with BDZ to be always avoided in the elderly (45%), at higher doses than recommended (31%) or with no appropriate clinical conditions (19%). From admission to discharge the prevalence of inappropriate BDZ prescription decreased by 4%, but 62% of patients inappropriately prescribed at admission were still inappropriately prescribed at discharge. Among the 179 patients first prescribed at the time of discharge, half were inappropriately prescribed. Being female (OR 1.32, 95%CI 0.95–1.85), enrolled in REPOSI during the years 2016 and 2017 (OR 1.94, 95%CI 1.10–3.39; OR 1.57, 95%CI 0.95–2.58) and living in nursing homes (OR 2.04, 95%CI 0.95–4.37) were associated with an increased risk to be inappropriately prescribed. This study shows a high prevalence of inappropriate use of BDZ in acutely hospitalized older patients both at hospital admission and discharge.

Abstract: Objectives We aimed to analyze the prevalence and impact of COPD in older patients hospitalized in internal medicine or geriatric wards, and to investigate adherence to the Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines, associated clinical factors, and outcomes. Design Data were obtained from REgistro POliterapie SIMI (REPOSI), a prospective multicenter observational registry that enrolls inpatients aged ≥65 years. Setting and Participants Older hospitalized patients enrolled from 2008 to 2016 with a diagnosis of COPD. Measures We evaluated adherence to the 2018 GOLD guidelines at admission and discharge, by examining the prescription of inhaled bronchodilators and corticosteroids in COPD patients. We also evaluated the occurrence of outcomes and its association with COPD and guideline adherence. Results At hospital admission, COPD was diagnosed in 1302 (21.5%) of 6046 registered patients. COPD patients were older, with more impaired clinical and functional status and multiple comorbidities. Overall, 34.3% of COPD patients at admission and 35.6% at discharge were adherent to the GOLD guidelines. Polypharmacy (≥5 drugs) at admission [odds ratio (OR): 3.28, 95% confidence interval (CI): 2.24-4.81], a history of acute COPD exacerbation (OR: 2.65, 95% CI: 1.44-4.88) at admission, smoking habit (OR: 1.45, 95% CI: 1.08-1.94), and polypharmacy at discharge (OR: 6.76, 95% CI: 4.15-11.0) were associated with adherence to guidelines. COPD was independently associated with the risk of cardiovascular and respiratory death and rehospitalization occurrence compared to patients without COPD during follow-up. Adherence to guidelines was inversely associated with the occurrence of death from all causes (OR: 0.12, 95% CI: 0.02-0.90). Conclusions/Implications COPD was common in older patients acutely hospitalized, showing an impaired functional and clinical status. Prescriptions for older COPD patients were often not adherent to GOLD guidelines. Poor adherence to guidelines was associated with a worse clinical status. There is a need to improve adherence to guidelines in treating COPD patients, with the ultimate goal of reducing clinical events.

Abstract: A first step of a broader framework, concerning the investigation of tall buildings’ dynamic response to wind load, is presented. In order to accomplish this challenging task, a well-known benchmark has been selected: the CAARC (Commonwealth Advisory Aeronautical Research Council) standard tall building. Decades of literatures on the subject make it the perfect candidate to tackle the complexity of wind-structure interaction phenomena. The present work concerns the design of a full-aeroelastic multi degrees of freedom model. The configuration proposed by J. Holmes at International Conference on Wind Engineering (2007), accounting for both flexural and torsional decoupled modes of vibrations, is adopted. A design theory present in literature is re-proposed, from a different prospective, in a semi-automated fashion through a Python script. At last, preliminary results are shown, demonstrating the method’s efficiency and applicability, not only to tall buildings, but more generally to structures with continuous distribution of mass.

Abstract: Correct determination of masonry deformability is fundamental for evaluating the seismic response of masonry structures. For existing buildings, double flat-jack tests are largely used to estimate elastic modulus, but many problems arise for a reliable interpretation of the test outcomes. Formulas provided by standards or available in the literature follow different approaches, the results of which are strongly variable. Furthermore, these formulas do not take into consideration important practical aspects of the test apparatus, like the position of the measurement reference points in the masonry sample. The arrangement of the gauge points can influence the displacement measurements and, consequently, the value of elastic modulus. This work aims at numerically investigating—through finite element (FE) models—the effects of the position of the gauge points on the outcomes of a double flat-jack test. In particular, a continuous FE mode is adopted, in which the concrete damage plasticity model implemented in Abaqus code is used to model the nonlinear behavior of the masonry. This study provides recommendations on the correct way to determine reliable values of elastic modulus from double flat-jack testing. Furthermore, numerical results and their comparison with experimental data allow for important considerations with regard to both the research field and professional and practical activities.
