Public policies related to comprehensive health care for women have advanced a lot in the last two decades, but there is still a pressing need to overcome difficulties and challenges, especially when addressing the interface between women health and mental health. In this way, this study analyzed the association between mental disorders and risk factors for chronic noncommunicable diseases (CNCD) in women. This was a cross-sectional case-control study conducted in the municipality of Maringá, at Psychosocial Care Centers I and II. In the case group, the sample consisted of 208 randomly selected women with a diagnosis of mental disorder followed-up during the last 12 months, aged between ≥35 and ≥ 59 years, and in the control group, women who were users of Primary Health Care, randomly selected, aged 42-52 years. For data collection, a standardized questionnaire was applied, containing the variables for Socioeconomic data; Obstetric history; Physical Activity; Tracking; Anthropometric evaluation, reproductive and clinical characteristics of the women and search for women's health services. For statistical analysis, raw data were analyzed by odds ratio (OR) and chi-square using the Epi Info 3.5.1 software. The major congenital abnormalities (AC), which present an injury to some organic function, require early identification, assistance and adequate referencing to specialized services. The aim of this study was to inquire the identification and assistance to major AC in the context of the Mãe Paranaense Network in the Paranavaí health region. An observational cross-sectional study was performed, with data from medical records of malformed newborns and pregnant women with diagnosis of fetal malformation, from January 2010 to September 2016, in the Paranavaí health microregion. Data were analyzed through Qualitative Comparative Analysis. The results show that among the 80 cases studied, 30 newborns had unfavorable outcomes when there was no ultrasound (USG) detection performed and / or the birth place of the malformed newborn was in another city than the reference motherhood. Twenty-nine cases were related to insufficient number of USG and consultations performed, and inadequate place of birth, and 7 cases related to lack of recorded malformation detected. In conclusion, there is a need for a broad approach with different collective health measures, ensuring access to quality prenatal care, with all the standardized tests carried out by the Mãe Paranaense Network guide, with quality assurance in the diagnostic services provided, since several malformed newborns had USG exams without abnormalities. As malformações congênitas (MC) maiores, que apresentam prejuízo a alguma função orgânica, necessitam identificação precoce, assistência e referenciamento adequado para serviços especializados. O objetivo deste trabalho foi investigar a identificação e referenciamento às MC maiores no contexto da Rede Mãe Paranaense na regional de saúde (RS) de Paranavaí. Foi realizado estudo transversal observacional, com coleta de dados de prontuários de recém-nascidos malformados e gestantes com diagnóstico de malformação fetal, durante o período de janeiro de 2010 até setembro de 2016, na microrregião de Paranavaí - Paraná. Os dados foram trabalhados através da Análise Comparativa Qualitativa. Os resultados mostram que entre os 80 casos estudados 30 recém-nascidos tiveram seus desfechos desfavoráveis, quando não houve detecção/registro da malformação no exame de ultrassom (USG) realizado e/ou quando o local de nascimento do recém-nascido malformado foi em outra cidade que não a maternidade de referência. Vinte e nove casos foram relacionados com o insuficiente número de USG e consultas realizados, e local de nascimento inadequado, e 7 casos relacionados com a falta de registro de malformação detectada. Em conclusão, destaca-se a necessidade de incremento na qualidade de assistência pré-natal através de medidas simples, tais como o cumprimento da Linha Guia da Rede Mãe Paranaense com a realização de consultas, exames de USG, registros em prontuários e adequado referenciamento de gestantes e neonatos de alto risco. Apesar de simples, tais medidas exigem o treinamento e engajamento das equipes nos diversos pontos de atenção da rede. Outras medidas de maior complexidade (serviços de USG especializados em medicina fetal e ecocardiografia fetal) poderiam potencializar a adequada assistência em rede, minimizando custos e otimizando prognósticos com maiores índices de satisfação aos usuários. 39 f

Recent studies have reported an association between vitamin D status and metabolic syndrome (MS) and its components. In relation to the female population, it is known that the prevalence of this syndrome increases during the menopause. However, studies evaluating the prevalence of MS and its association with serum vitamin D levels in postmenopausal women are still scarce. The present study aimed to analyze the association between vitamin D and MS and their components in pre and post menopausal women. This was an observational, cross-sectional and retrospective study aimed to analyze the association between vitamin D and MS, as well as its components, in pre- and postmenopausal women. It was performed with a sample of 516 women attended in two cardiology services between 2010 and 2014 in northern Paraná. The variables used were characteristics of physical examination, such as blood pressure and abdominal circumference; biochemical tests such as vitamin D, fasting glycemia, triglycerides and HDL cholesterol; in addition to the presence of systemic arterial hypertension (SAH) and type 2 diabetes mellitus (DM2). The patients were characterized in relation to the vitamin D status (adequate, insufficient or deficient), presence or not of the metabolic syndrome and menopause. Patients with the syndrome showed a higher prevalence of vitamin D deficiency (p=0.0007), arterial hypertension (p=0.0001), fasting glycemia (p=0.0037), low HDL (p=0.0017) and elevated triglycerides (p=0.0030). When comparing the menopausal and nonmenopausal women, the prevalence of hypovitaminosis D was higher in the second group. Menopausal women with metabolic syndrome had a higher prevalence of hypovitaminosis D (p = 0.0016). However, the causality between vitamin D status and the prevalence of MS still needs to be investigated and thus longitudinal studies evaluating the consequences of vitamin D supplementation on the components of MS are needed. Estudos recentes têm reportado uma associação entre o status de vitamina D e a síndrome metabólica (SM) e seus componentes. Em relação à população feminina, é sabido que a prevalência dessa síndrome aumenta durante a menopausa. Entretanto, estudos avaliando a prevalência de SM e sua associação com os níveis séricos de vitamina D em mulheres na pósmenopausa ainda são escassos. O presente estudo teve como objetivo analisar a associação entre os níveis de vitamina D e SM e seus componentes em mulheres na pré e pós-menopausa. Tratouse de um estudo observacional, transversal e retrospectivo realizado com uma amostra de 516 mulheres atendidas em dois serviços de cardiologia entre 2010 e 2014 no norte do Paraná. As variáveis utilizadas foram características de exame físico, pressão arterial e circunferência abdominal; exames bioquímicos, como vitamina D, glicemia em jejum, triglicerídeos e HDL colesterol; além de presença de hipertensão arterial sistêmica (HAS) e diabetes mellitus tipo 2 (DM2) em tratamento. As pacientes foram caracterizadas em relação ao status de vitamina D (adequado, insuficiente ou deficiente), presença ou não de síndrome metabólica e menopausa. As pacientes com a síndrome mostraram maior prevalência de deficiência de vitamina D (p=0,0007), de hipertensão arterial (p=0,0001), de glicemia em jejum elevada (p=0,0037), de HDL baixo (p=0,0017) e de triglicerídeos elevados (p=0,0030). Ao comparar as mulheres na pré e na pós menopausa, a prevalência de hipovitaminose D foi maior no segundo grupo. As mulheres na pós-menopausa com síndrome metabólica apresentaram maior prevalência de hipovitaminose D (p=0,0016). No entanto, a causalidade entre o status de vitamina D e a prevalência de SM ainda precisa ser investigada. This is an observational, cross-sectional retrospective study to evaluate the 25- hydroxyvitamin D (25(OH)D) levels in patients with Systemic Lupus Erythematosus (SLE) and determine the relationship between menopause and levels of vitamin D. We evaluated the records of patients treated between lupus 2010-2015 in private health care rheumatology city of Maringa-PR. It was observed the first dose of vitamin D each patient and the following characteristics: gender, age, disease duration, clinical manifestations of SLE, the presence of some autoantibodies (ANA, Anti-Sm, Anti-DNA) and menopause. To implement and to qualify basic care there was created the Family Health Support Center. The work together with the Family Health Teams demonstrated the important role of the same in the development of actions of permanent education in health, practices still undervalued but of great importance in professional s training. Thus, the aim of this study was to analyze the practices of permanent education in health performed by the Family Health Support Center (NASF) in the context of chronic conditions and describe the potentialities and difficulties of the process of permanent education in health from the perspective of NASF professionals, performed in the context of chronic conditions. A qualitative and descriptive study carried out in a municipality in the Northern State of Parana, southern Brazil. The participants of the research were of 24 health professionals who are members of NASF. Data collection took place between June and July 2016 through an individual interview, using a semi-structured script composed of questions related to the characterization of the participants and the subject matter, as appropriate by judges in the study area. Previous contact was made to explain to the professionals the purpose of the study and its methodology and to schedule the date, time and place of the interviews and the choice of the participants. The interviews were recorded and transcribed in their entirety and submitted to content analysis, with the help of Iramuteq® software. Regarding the characterization of the professionals, the predominant were females, the average of four years of performance at NASF, and three out of the nine teams were more representative in the survey. Concerning the textual signifiers and the meaning of content, the arrangement of the speeches was organized and, later, the most frequent keywords organized in the shape of a cloud of words: talk - sense of communication , Community Health Agent, give Refers to explanations, actions, functions to find, to be and to achieve. The thematic analysis allowed us to list the five categories, which are divided into subcategories, and these present the permanent education in health in the context of chronic conditions that appear in the actions of the NASF through prevention and follow-up activities carried out by means of indirect and programmed actions developed in the work process of the Family Health Strategy (FHS). The insertion of the NASF into the FHS is a potential for the development of permanent health education regarding the chronic conditions. However, organizational, physical-structural and role identity aspects can be fragilities in the development of these actions. Thus, the findings of this study demonstrate that the work process of the NASF has been changed but remains focused on the interaction of multiprofessional teams those can be evidenced in group performances, home visits, case discussions and matrix process, these being a basic device to enhance the comprehensiveness of care and the resolution of primary health care. However, this study also allowed us to highlight fragilities such as some challenges to be faced in terms of infrastructure, professional aspects and work processes that end up hindering or sometimes facilitating the daily work of these teams. Para implementar e qualificar a atenção básica criou-se o Núcleo de Apoio à Saúde da Família. O trabalho, junto às equipes Saúde da Família, demonstrou o importante papel do mesmo no desenvolvimento de ações de educação permanente em saúde, práticas ainda pouco valorizadas, mas de grande importância na formação dos profissionais. Dessa forma, objetivou-se analisar as práticas de Educação Permanente em Saúde desempenhadas pelo Núcleo de Apoio à Saúde da Família (NASF) no contexto das condições crônicas e descrever as potencialidades e dificuldades do processo de Educação Permanente em Saúde na perspectiva dos profissionais do NASF, desempenhadas no contexto das condições crônicas. Estudo qualitativo do tipo descritivo realizado em um município ao norte do Estado do Paraná, região sul do Brasil. Os participantes da pesquisa foram 24 profissionais de saúde integrantes dos NASF. A coleta de dados ocorreu no período de junho a julho de 2016 por meio de entrevista individual, utilizando um roteiro semiestruturado composto por questões relacionadas à caracterização dos participantes e da temática em questão, adequado por juízes da área. As entrevistas foram gravadas e transcritas na íntegra e submetidas à análise de conteúdo modalidade temática com o auxílio do o software Iramuteq®. Em relação à caracterização dos profissionais, se destaca a predominância do sexo feminino, a média de quatro anos de atuação no NASF, e três das nove equipes tiveram maior representatividade na pesquisa. Quanto aos significantes textuais e significado do conteúdo, foi organizado o arranjo dos discursos e, posteriormente, elencadas palavras-chaves mais frequentes, organizadas na forma de nuvem de palavras: falar sentido de comunicar , Agente Comunitário de Saúde, dar no que se referem às explicações, ações, funções achar, estar e conseguir. A análise temática permitiu elencar cinco categorias que se desdobram em subcategorias, e estas apresentam a Educação Permanente em Saúde no contexto das condições crônicas, presentes nas ações do NASF por meio de atividades de prevenção e de acompanhamento de casos, efetivadas por meio de ações indiretas e programadas desenvolvidas no processo de trabalho da Estratégia Saúde da Família (ESF). A inserção do NASF junto a ESF é uma potencialidade para se desenvolver a Educação Permanente em Saúde no entorno das condições crônicas. Entretanto aspectos organizacionais, físico-estruturais e de identidade de papel, podem ser fragilidades no desenvolvimento dessas ações. Os achados deste estudo demonstram que o processo de trabalho do NASF vem sendo alterado, mas mantêm-se voltados à interação das equipes multiprofissionais que podem ser evidenciados nas atuações em grupo, visitas domiciliares, discussões de caso e matriciamento, sendo este um dispositivo fundamental para potencializar a integralidade do cuidado e a resolutividade da atenção primária à saúde. Contudo, o estudo também permitiu ressaltar fragilidades, desafios a serem enfrentados quanto à questão organizacional, infraestrutura, gestão, relacionamentos e de processo de trabalho. 93 f

The increase in the elderly population is a relevant fact and that requires attention. Among the diseases that contribute to decreased health conditions / disease of the elderly there are the falls, as the first cause of accidents in people aged 60 or more. Falls are a major public health problem, by its frequency, morbidity and high social and economic costs resulting from injuries. The aim of this study was to associate the use of medicines and the main reasons of falls in elderly patients in the emergency care units of a public reference hospital trauma care. This is a cross-sectional, retrospective, quantitative study with secondary data, in the year 2014. The medical records of elderly people were analyzed, patients with chronic degenerative diseases, fall victims, members of the emergency service, aged over 60 years . Data were collected using a questionnaire, later typed in Excel 2010 software, coded and analyzed. There were a total of 861 assistance per falling. Thun, they were analyzed the medical records of 234 individuals who had the criterion to remain under observation and / or hospitalized for 24 hours or more in the hospital, with hospital admission Authorization. It was observed, the prevalence of falls in the age group between 60 to 70 years 97 (41.5%) and frequently female 146 (62.4%). The participation in activities at home was present in 70 (29.9%) of records. The place of the falling with more frequency records was at home 183 (78.2%), with the reason to fall from height 173 (73.9%). The ISTCE (Integrated Service Trauma Care and Emergency) was the form of help most used by the elderly 88 (37.6%), but there was a high number brought by family 79 (33.7%) and 67 (28.7 %) were conducted by other forms of help. In relation to risk classification 132 (56.4%) elderly were classified in yellow, whose service should be carried out within twenty minutes. Only 4 (1.7%) elderly received immediate care and are classified in red, ie, with risk. Most of the victims belonged to municipality of Maringa 177 (75.6%). A large number of elderly patients remained hospitalized for more than 72 hours 85 (36.3%). They received sedation in hospital 195 (32%) older after the initial medical care. In the analysis of the consequences of the fall, it was observed that the traumatic brain injury mild or moderate was the major 62 (26.5%), followed by fractures of the upper limbs in 57 (24.4%) of the elderly, and fractures of the lower limbs in 52 (22.2%). Fractures of the lower limbs, 34 (65.4%) corresponds to femur fractures. Of the 234 patients, 119 (50.9%) were hospitalized waiting by surgery, these 83 patients (69.7%) were transferred to other hospitals. As for risk factors for fall, it was found that 175 (74.8%) had visual or hearing problems and chronic disease was present in 152 (64.95%) elderly. Of chronic diseases prevailed hypertension in 64 (27.4%) of the elderly, stroke and / or circulatory disease in 11 (4.7%) elderly and psychiatric disorders in 10 (4.3 %). The alcohol use was found in 21 (9.0%) records. Most falls occurred in the afternoon 97 (41.5%). Register of previous falls were present in only 7 (3%) of records. In relation to commitment of the members and difficulty muddle it was found in 14 (6.0%). The use of continuous medication appeared in 226 (96.6%) records, whereas chronic disease was registry in 152 (64.9%). The use of medication without prescription appeared in 87 (38.5%) records. Of those who were using medication 39 (16.7%) used only one, 27 (11.5%) two drugs and 23 (9.8%) three drugs. Antihypertensive drugs were the most commonly used by the elderly 124 (53.0%), followed by anti-diabetic 36 (15.4%) and diuretics (33%). The study showed that the fall is a frequent mechanism of trauma in the elderly and causing injuries that vary in severity. It showed a high number of elderly people who used continuous medication but did not have the information of having chronic disease and that, as polypharmacy may be associated with vulnerability of the elderly to fall. A fall is related to a number of factors risk, so it is important to recognize the causal agents for the targeting of preventive and care actions. O aumento da população idosa é um fato relevante e que requer atenção. Entre os agravos que contribuem para a piora das condições de saúde/doença dos idosos destacam-se as quedas, como a primeira causa de acidentes em pessoas com 60 anos ou mais. As quedas são um importante problema de saúde pública, pela sua frequência, morbidade e elevado custo social e econômico decorrente das lesões provocadas. O objetivo deste estudo foi associar o uso de medicamentos e os principais motivos de queda em idosos atendidos no Pronto Atendimento de um hospital público referência de atendimento ao trauma. Trata-se de um estudo transversal, retrospectivo, quantitativo com dados secundários, do ano de 2014. Foram analisados prontuários de idosos, portadores de doenças crônico degenerativas, vítimas de queda, usuários do serviço de emergência, com idade igual ou superior a 60 anos. Os dados foram coletados utilizando um questionário estruturado e validado, posteriormente digitados no software Excel 2010, codificados e analisados. Ocorreram um total de 861 atendimentos por queda, desta população foram analisados os prontuários de 234 indivíduos que tiveram como critérios a permanência em observação e/ou internados por 24 horas ou mais na instituição hospitalar, com abertura de Autorização de Internação Hospitalar. Foi observada, a predominância das quedas na faixa etária entre de 60 a 70 anos 97 (41,5%) e maior frequência do sexo feminino 146 (62,4%). A participação de atividades no lar esteve presente em 70 (29,9%) dos prontuários. O local da queda com maior frequência de registros foi no domicílio 183 (78,2%), tendo como motivo a queda da própria altura 173 (73,9%). O SIATE (Serviço Integrado de Atendimento ao Trauma e Emergência) foi a forma de socorro mais utilizada pelos idosos 88 (37,6%), porém houve um elevado número trazido por familiares 79 (33,7%) e 67 (28,7%) foram encaminhados por outras formas de socorro. Quanto a classificação de risco 132 (56,4%) idosos foram classificados na cor amarela, cujo atendimento deveria ser realizado em até vinte minutos. Apenas 4 (1,7%) idosos receberam atendimento imediato, sendo classificados na cor vermelha, ou seja, com risco. A maioria das vítimas pertenceu ao município de Maringá 177 (75,6%). Um número elevado de idosos permaneceu internado por mais de 72 horas 85 (36,3%). Receberam analgesia na instituição hospitalar 195 (32%) idosos após o atendimento médico inicial. Na análise das consequências da queda, foi observado que o Traumatismo crânio encefálico leve ou moderado foi o que mais ocorreu 62 (26,5%), seguido das fraturas de membros superiores em 57 (24,4%) dos idosos, e as fraturas de membros inferiores em 52 (22,2%). Das fraturas de membros inferiores, 34 (65,4%) corresponderam a fraturas de fêmur. Dos 234 pacientes, 119 (50,9%) ficaram internados aguardando procedimento cirúrgico, destes pacientes 83 (69,7%) foram transferidos para outros hospitais. Quanto aos fatores de risco para queda, foi verificado que 175 (74,8%) não possuíam problemas visuais ou auditivos e a doença crônica estava presente em 152 (64,95%) idosos. Das doenças crônicas prevaleceu a hipertensão arterial sistêmica em 64 (27,4%) dos idosos, o AVC (Acidente vascular cerebral) e/ou doenças circulatórias em 11 (4,7%) idosos e os transtornos psiquiátricos em 10 (4,3%). O etilismo foi encontrado em 21 (9,0%) prontuários. A maioria das quedas ocorreu no turno vespertino 97 (41,5%). Registros de quedas anteriores estiveram presentes em apenas 7 (3%) dos prontuários. Quanto ao comprometimento dos membros e dificuldades em deambular foi encontrado em 14 (6,0%). O uso de medicamento contínuo apareceu em 226 (96,6%) prontuários, enquanto que a doença crônica foi registrada em 152 (64,9%). O uso de medicação sem prescrição médica apareceu em 87 (38,5%) prontuários. Daqueles que faziam uso de medicação 39 (16,7%) utilizavam apenas uma, 27 (11,5%) duas medicações e 23 (9,8%) três medicações. Os anti-hipertensivos foram os medicamentos mais usados pelos idosos 124 (53,0%), seguido dos anti-diabéticos 36 (15,4%) e diuréticos (33%). O estudo demonstrou que a queda é um mecanismo de trauma frequente em idosos e causadora de lesões que variam quanto a gravidade. Evidenciou um elevado número de idosos que faziam uso de medicação contínua mas que não tinham a informação de possuírem doença crônica e que esta, assim como a polifarmácia pode estar associada a vulnerabilidade dos idosos a queda. A queda está relacionada a um conjunto de fatores de risco, portanto, é importante o reconhecimento dos agentes causais para o direcionamento das ações preventivas ou de cuidado. 53 f

The aim of this study was to monitor the connection between the points of attention and the line running guide advocated more attention to the care of black mothers and the performance of hospitals about the care of newborns. Therefore, we carried forward collections and retrospective analyzes of data records, prenatal card and Child Health Handbook 311 mother-child binomial met in the maternity of the Hospital Universitário Regional de Maringá (HUM) and 126 neonates received at the clinic intermediate risk CISAMUSEP. However, neonates were quickly routed to the reference outpatient hospital at school than those born elsewhere. Overall, black pregnant women did not receive better prenatal care and incompleteness rates annotation of serological tests show the low quality of health care for all independent population of race / color. The neonates born at the hospital school were better assisted, but primary care has shortcomings and needs improvements in maternal and child care. O objetivo deste estudo foi avaliar a vinculação entre os pontos de atenção e execução do protocolo preconizado com atenção ao atendimento das gestantes negras e o desempenho das maternidades quanto à assistência aos neonatos. Para tanto, foram realizadas coletas prospectivas e análises retrospectivas de dados de prontuários, Cartão de Pré-natal e de Caderneta de Saúde da Criança de 311 binômios mãe-filho atendidos na maternidade do Hospital Universitário Regional de Maringá (HUM) e 126 neonatos recebidos no ambulatório de risco intermediário do CISAMUSEP. No HUM, foram atendidas 119 gestantes negras e 191 não negras. 61% das gestantes realizaram 7 ou mais consultas de pré-natal sendo o índice de incompletude das anotações das sorologias no cartão de pré-natal maior que 50% independente da etnia e os índices de realização dos exames de triagem neonatal quase 100%. Dos 126 neonatos encaminhados para o ambulatório de risco intermediário, 80 nasceram em Hospital Escola e apresentaram elevadas taxas de baixo peso além da classificação como pequenos para idade gestacional. Por outro lado, este grupo teve os melhores indicadores de qualidade na realização dos exames de triagem neonatal. Apenas 20,6% dos neonatos acompanhados no ambulatório receberam a visita dos agentes do Programa da Saúde da Família (PSF) independentemente do local de nascimento. Os neonatos nascidos no HUM foram mais rapidamente encaminhados para o ambulatório de referência do que os nascidos em outros locais. No geral, gestantes negras não receberam atendimento de pré-natal adequadamente assim como as outras pacientes. Este fato associado aos elevados índices de incompletude dos dados nos cartões de pré-natal evidenciam a baixa qualidade na atenção à saúde de toda população independente da etnia. Os neonatos nascidos no Hospital Escola foram melhor assistidos que os de maternidades não vinculadas ao ensino, porém, na atenção primária, o cuidado materno-infantil mostrou fragilidade e necessidade de melhorias. 52 f

Pregnancy and motherhood are important events in women's lives; however such events can cause physical and emotional changes, making them sometimes vulnerable. The postnatal or puerperal period is the stage when the woman's body undergoes several local and systemic changes to return to pre-pregnancy status, besides demanding a psychical effort generated by the need to adjust to both, the real child and changes in the family structure. This study aimed to analyze the practices of Permanent Education in Health that permeated the processes of implementation and organization of the National Humanization Policy in basic care. It was a research made through a qualitative approach, of the exploratory and descriptive type. For that, a focal group technique was used, with workers from six Basic Health Units from a municipality in the northwest of the State of Paraná-Brazil. The transcripts were submitted to the thematic content analysis technique proposed by Bardin, with the aid of six word clouds generated by the IRAMUTEQ software. These clouds provided the definition of categories, which were: Category 1- The National Humanization Policy devices brought positive changes to the work processes and were mediated by practices of Permanent Education in Health; Category 2 - The Permanent Education in Health that guided the implementation of the guidelines and devices of the National Humanization Policy was marked by collective practices and protagonists; Category 3 - The collective work permeated by learning was a reality in the implementation of the devices of the National Humanization Policy; Category 4 - The collaborative support of the team and the knowledge sharing are scenarios that allow learning and transformation; And Category 5- Work meetings and informal team conversations are scenarios that concretize the permanent evaluation of the implementation of the National Humanization Policy and provide opportunities for learning and transformation. The data were discussed by the light of Paulo Freire's reference, regarding the questioning in the production of knowledge and practices. The research was conducted by following the current ethical precepts and the authorization of the competent body of the Municipal Health Department. This study demonstrated that many guidelines and devices of the Humanization Policy were implemented in the units and this implementation was conducted by permanent education practices. The National Policies of Permanent Education in Health and Humanization have positively influenced the teams' work processes. It was verified that, in the studied scenario, despite all obstacles, the practices of permanent education have permeated the work processes for the implementation of the guidelines and devices of the National Humanization Policy. Este estudo teve como objetivo analisar práticas de Educação Permanente em Saúde que permearam os processos de implantação e organização da Política Nacional de Humanização na atenção básica. Tratou-se de uma pesquisa com abordagem qualitativa, do tipo exploratória e descritiva. Para tanto, foi realizada a técnica de grupo focal, com trabalhadores de seis Unidades Básicas de Saúde de um município do noroeste do Estado do Paraná- Brasil. As transcrições foram submetidas à técnica de análise de conteúdo temática proposta por Bardin, com auxílio de seis nuvens de palavras geradas pelo software IRAMUTEQ. Essas nuvens subsidiaram a definição das categorias, as quais foram: Categoria 1- Os dispositivos da Política Nacional de Humanização trouxeram mudanças positivas para os processos de trabalho e foram mediados por práticas de Educação Permanente em Saúde; Categoria 2- A Educação Permanente em Saúde que orientou a implantação das diretrizes e dispositivos da Política Nacional de Humanização foi marcada por práticas coletivas e protagonistas; Categoria 3- O trabalho coletivo permeado pelo aprendizado foi uma realidade na implantação dos dispositivos da Política Nacional de Humanização; Categoria 4- O apoio colaborativo da equipe e o compartilhamento de saberes é um cenário que oportuniza o aprendizado e as transformações; e Categoria 5- As reuniões de trabalho e conversas informais em equipe são cenários que concretizam a avaliação permanente da implantação dos dispositivos da Política Nacional de Humanização e oportunizam aprendizado e transformações. Cardiovascular diseases have increased around the world. Female sex gender is underrepresented in researches because of the risk of factors, which present different impact between genders. Several factors have contributed for increasing the incidence, such as smoking, hypertension, obesity, a sedentary lifestyle, ageing. Besides all those factors, the signs and symptoms are not specific for women as well, there is not specific test for diagnosis, decreasing the chances of early treatment and raising mortality rates. The objective of this study was to analyze the factors that lead women with coronary artery disease to be submitted to cardiac intervention and progressing to invasive treatment or not, and its outcome characterizing these women according to socio economic, demographic and clinical and identifying the factors risk for cardiovascular diseases. It was evaluated 626 female patients whose average age was about 62.48 years old, all them were referred by performing catheterism procedure at the cardiologic reference hospital in Maringa-Pr. The experience of stillbirth involves aspects of loss and grief which have to be investigated and discussed. Current analysis investigates grief conditions of women who experienced stillbirth in two different cultures; undertake the transcultural adaptation of the Perinatal Grief Scale to Portuguese and French; identify the complex grief state of women who experienced stillbirth; verify the association of complex grief state of women with stillbirth and the socio-demographic, reproductive, mental characteristics, coupled to professional help and conjugal satisfaction; understand the experience of women on stillbirth. The study has been developed within three stages: 1) a methodological research involving the transcultural adaptation of the Perinatal Grief Scale (PGS); 2) exploratory, transversal and descriptive research on the association between the grief state and the women´s characteristics; clinical-qualitative research employing a semi-directed interview with women who had experienced stillbirth. Population was composed of all women who had had a stillbirth in 2013 in the municipality of Maringá PR Brazil and at the Centre d'Études et de Recherche en Intervention Familiale of the University of Quebec in Outaouais, Canada. Data were collected by questionnaire featuring socio-demographic, reproductive and mental characteristics, conjugal satisfaction and professional help, and by a semi-directed interview with open questions on aspects experienced in stillbirth. The methodological referential of transcultural adaptation of instruments and reliability evaluation were employed for PGS consistency between the two populations. Odds Ratio at p≤0.05 verified the association between the grief state and the women´s characteristics. Multivariate analysis of multiple correspondences was undertaken with the main variables associated with the grief state. The semi-structured interview for the application of the clinical and qualitative method was submitted to qualitative content analysis. The PGS in Brazilian Portuguese and in Canadian French was reliable for the two populations. Cronbach´s alpha coefficient in PGS applied in Brazil reached 0.93, with the Canadian one at 0.94. The grief state was greater among Brazilians (35%) when compared to that among Canadians (12%). Post-natal depression and conjugal non-satisfaction were the traits of Brazilian women with low schooling, previous pregnancy with a live born and non-occurrence of a previous loss. The characteristics of Brazilian females in grief featured low schooling, previous pregnancy with live birth, without the occurrence of a previous loss, post-birth depression and conjugal non-satisfaction. Contrastingly, 80% of Canadian women without grief resorted to professional help. The occurrence of grief in the two populations was more frequent in women with pregnancy age over 28 weeks. Content qualitative analysis revealed three categories: evolution of pregnancy in stillbirth, contact with the stillborn, assistance by health professionals. The PGS´s transcultural adaptation for Brazilians and Canadians was established and may be employed to identify the grief state in women with stillborn, in their respective version. Canadian females with low schooling and who do not demand professional help, who do not practice any type of religion, lacking conjugal satisfaction must be analyzed in the grief state. A realidade do óbito fetal envolve aspectos de perda e luto que ainda precisam ser discutidos. O estudo teve como objetivos analisar o estado de luto de mulheres que tiveram óbito fetal em duas diferentes culturas; realizar a adaptação transcultural da Escala de Luto Perinatal para o português e o francês; identificar o estado de luto complicado das mulheres que tiveram óbito fetal; verificar a associação do estado de luto complicado das mulheres que experienciaram óbito fetal e as características sociodemográficas, reprodutivas, mentais, apoio profissional e a satisfação conjugal; compreender a experiência de mulheres na ocorrência da perda fetal. Trata-se de um estudo realizado em três momentos: 1) pesquisa metodológica, de adaptação transcultural da Escala de Luto Perinatal (ELP). 2) estudo descritivo exploratório de análise da associação do estado de luto com as características da mulher. 3) pesquisa clínico-qualitativa, por meio de entrevista semidirigida com as mulheres que perderam seus fetos. A população foi composta por todas as mulheres que tiveram óbito fetal no ano de 2013 residentes no município de Maringá-Brasil e participantes do Centre d'Études et de Recherche en Intervention Familiale na Universidade do Quebec em Outaouais-Canadá. A coleta de dados foi realizada por meio da aplicação de um questionário abrangendo características sociodemográficas, reprodutivas e mentais, de satisfação conjugal e apoio profissional, e entrevista semidirigida com questões abertas abordando aspectos vivenciados na perda fetal. Para a adequação da ELP nas duas populações, foi empregado o referencial metodológico de adaptação transcultural de instrumentos e efetuada a avaliação da confiabilidade. Para verificar a associação entre o estado de luto e as características da mulher, foi utilizado o Odds ratio com p≤0,05. Foi realizada a análise multivariada de correspondência múltipla com as principais variáveis que tiveram associação com o estado de luto. A entrevista semidirigida para aplicação do método clínico-qualitativo foi submetida à análise qualitativa de conteúdo. A ELP, na versão português do Brasil e francês do Canadá, mostrou-se confiável nas duas populações. O coeficiente alfa de Cronbach na ELP aplicada no Brasil foi de 0.93, e na aplicada no Canadá foi de 0.94. O estado de luto foi maior nas brasileiras (35%) em relação às canadenses (12%). As características das brasileiras com luto foram menor escolaridade, gestação anterior com filho nascido vivo, não ocorrência de perda anterior, depressão pós-parto e não satisfação conjugal. Para as canadenses foi observado que 80% das mulheres sem luto haviam utilizado grupo profissional de apoio. Nas duas populações a ocorrência do luto foi mais frequente nas mulheres com idade gestacional maior que 28 semanas, não prática da religião e menor tempo de casal. Da análise qualitativa de conteúdo emergiram três categorias: evolução da gestação em óbito fetal, contato com o bebê nascido morto, assistência no óbito fetal. Foi confirmada a adaptação transcultural da ELP para brasileiras e canadenses, podendo ser utilizada para identificação do estado de luto em mulheres que tiveram óbito fetal, na respectiva versão do idioma de cada país. As mulheres que mais precisam ser investigadas no estado de luto são as habitantes no Brasil, que não utilizam grupo profissional de apoio, não tem satisfação conjugal, não praticam religião e apresentam menor escolaridade. 135 f

Cardiovascular diseases are the leading cause of death for women in Brazil and worldwide. Knowing the risk factors for the development of these diseases is essential to reduce this mortality. This study search to analyze the cardiovascular risk by Global Risk Score (ERG) and the prevalence of metabolic syndrome (MS) and its women component in pre-and postmenopausal women. Search also establish the association between the menopausal status and MS. This is a retrospective study in a reference outpatient clinic in cardiology in a city of Northwest Paraná, southern Brazil, which were reviewed 2052 records of asymptomatic women, attended from 2010-2014, forming two groups: women in pre-menopause, considering the women who still had menstrual cycles, regular or not; and postmenopausal women, those without menstrual cycles more than a year. For the assessment of global cardiovascular risk was used the criterion of I Brazilian Guidelines on Cardiovascular Prevention 2013 and applied the Global Risk Score (ERG). MS was characterized according to the criteria of the National Cholesterol Education Program Adult Treatment Panel III - NCEP ATP-III-2005. It was found that 51% of women at high risk, 21.3% reported a moderate risk and 27.7% had low risk. The mean total cholesterol was 193.14 mg / dl, but in the age groups 50-59 and 60 years of age they were the highest 201.1 and 193 mg / dl, respectively. The presence of diabetes mellitus (DM) was predominant in women over 60 years. The average higher blood pressure also were in the age groups 50-59 years (134.4 mmHg) and those over 60 (134.1 mmHg). Waist circumference 80 cm was found in 61.9% of women with high risk. Relating the global risk score (ERG) and the age groups, the results regarding the high risk were 0.6%, 1%, 6.1%, 12.9% and 79.4%, based on the age groups smallest to the largest. The prevalence of MS was 18.5% in perimenopausal women and 22.2% in women with postmenopausal risk of 2.75. It was also observed that the SM was more frequent with increasing age. Breast cancer is the second most common cancer worldwide, with an estimated 1.67 million of new cases, and represents 25% of all cancers. The incidence varies from region to region, however, the highest rates are found in Western Europe (96/100,000) and the lowest rates in Central Africa and West Asia (27 / 100,000). In Brazil, it is expected 57.120 new cases of breast cancer with an estimated risk of 56.09 cases per 100,000 women in 2014 and 2015. There are several studies that try to associate the risk factors to the increased incidence of breast cancer but few studies have shown the association between hormone therapy and the development of breast cancer. In addition to the risk factors another meaningful character of breast cancer is treatment. Among the factors that permeate the mastectomy there is mammary reconstruction, which helps the body image and quality of life of women. The option for reconstruction is associated with both the type of surgery performed and the acceptance of women. Lack of knowledge about the reconstruction options makes difficult to these women to choose the technique. Some women, besides to breast reconstruction, have the option of external mammary prosthesis which can be used until they adapt to the new circumstances. Some authors report that patient must decide if she wants to make the reconstruction, although it is an important component of treatment. She must be informed of the possibility of delayed reconstruction, i.e., after the supporting treatments. Studies on the impact of the disease on quality of life of these women, body image and predisposing factors for breast cancer, requires knowledge of health professionals in order to discuss, find out the main risk factors such as age, age at menarche, obesity, use of hormone therapy, number of children, use of oral contraceptives, family history and physical activity. To increase knowledge about the influence of hormones, obesity and physical activity on the incidence of breast cancer as well as to address the use of external mammary prosthesis among mastectomized women justify this study. Thus, the objective of this research was to analyze the risk factors of menopausal period for breast cancer and the satisfaction related to the use of external breast prosthesis by a mastectomized woman. It is a case-control study with 600 women in an Oncology Hospital reference center of the State of Paraná, Brazil, and an exploratory cross-sectional study held with the population of mastectomized women who used external breast prosthesis, registered in the services of the Centro de Reabilitação Integral Dom Aquino Corrêa of Cuiabá, State of Mato Grosso during the period from 2009 to 2012. For health professionals, to know the risk factors associated with breast cancer among Brazilian women enables the creation of health policies, with a view to the risk factors found in the population studied, seeking for the reduction of morbidity and mortality for this important disease. O câncer de mama é a segunda neoplasia mais comum em todo o mundo, com uma estimativa de 1,67 milhões de novos casos, e representa 25% de todos os cânceres. A incidência varia de região para região, porém, as maiores taxas são encontradas na Europa Ocidental (96/100mil) e as menores taxas na África Central e na Ásia Ocidental (27/100mil). No Brasil, para o ano de 2014 e 2015 são esperados 57.120 casos novos de câncer de mama com um risco estimado de 56,09 casos a cada 100 mil mulheres. Existem vários estudos que tentam associar os fatores de risco com o aumento na incidência do câncer de mama, porém poucos estudos mostraram a associação entre o uso e hormonioterapia e o desenvolvimento do câncer de mama. Além dos fatores de risco outro ponto importante do câncer de mama é o tratamento. Dentre os fatores que permeiam a mastectomia está a reconstrução mamaria, que ajuda na imagem corporal e na qualidade de vida das mulheres. A opção pela reconstrução está associada tanto ao tipo de cirurgia realizada, como à aceitação da mulher. A falta de conhecimento das mulheres acerca da opção de reconstrução acaba dificultando a escolha da técnica por estas. Algumas mulheres, além da reconstrução mamária, têm a opção da prótese mamária externa que pode ser utilizada até que a mesma se adapte à nova realidade. Alguns autores relatam que a mulher precisa decidir se quer a reconstrução, apesar dela ser um componente importante do tratamento. Ela precisa ser informada da possibilidade de uma reconstrução tardia, ou seja, após os tratamentos coadjuvantes. Estudos sobre o impacto da doença na qualidade de vida das mulheres, imagem corporal e fatores predisponentes para o câncer de mama, requer dos profissionais de saúde conhecimento, a fim de discutir, levantar os principais fatores de risco como idade, idade da menarca, obesidade, uso de hormonioterapia, número de filhos, uso de anticoncepcionais orais, história familiar e atividade física. Este estudo justifica-se no sentido de aprofundar os conhecimentos acerca da influência dos hormônios, da obesidade e da atividade física na incidência do câncer de mama, bem como de abordar a utilização da prótese mamária externa entre mulheres mastectomizadas. Desta forma, o objetivo desta pesquisa foi analisar os fatores de risco do período menopausal para o câncer de mama e a satisfação em relação ao uso da prótese mamária externa pela mulher mastectomizada.Trata-se de um estudo caso controle realizado com 600 mulheres em um centro de referência hospitalar oncológico do Estado do Paraná, Brasil e um estudo exploratório, transversal, realizado com a população de mulheres mastectomizadas que utilizaram prótese mamária externa, cadastradas nos serviços do Centro de Reabilitação Integral Dom Aquino Corrêa de Cuiabá, Mato Grosso no período de 2009 a 2012. Schistosomiasisis a major parasitic diseases in the world due to its prevalence and economic consequences on public health in the places where it occurs. It is endemic in many developing countries, especially in Africa, Asia and Latin America. It is estimated that 779 million people are atrisk of infection and 243 million people needed treatment in 2011. In Brazil, schistosomias is has been reported in then or the astern states, southea stand south, affecting approximately six million people. The objective was to verify the status of transmission of schistosomiasis in an urban area of the city of Bandeirantes, Paraná. Trata is a descriptive epidemiologic study, cross-sectional, composed of two stages. In the first stage a retrospective study was done with the use of information obtained from the files of the municipal Bandeirantes. The second stage was a study of population-based sample sands tool tests and a questionnaire. The sample consisted of 304 people. After collection, the fecal samples were keptun der refrigeratio nuntil the exams. The method used was the Kato- Katz. Toanalyze the datawe used the Fischer Exact Test. The history of schistosomiasis cases in Bandeirantes municipality shows that there was a higher prevalence in the years 2005, 2008 and 2009. The new survey showed 9 individuals Schistosoma mansoni contaminated and two cases were classified as moderate/high grade infection. In the an alysis of the exact Fisher test was verified that there is no association between the variables analyzed. The data show that there was a decrease of schistosomiasis cases over the years in the city of Bandeirantes, but the loss of files over the years prevents comparisons with previous data and indicates the neglect in caring for schistosomiasis in this municipality. A esquistossomose é uma das principais doenças parasitárias do mundo devido a sua prevalência e consequências econômicas na saúde pública nos locais onde ocorre. É endêmica em muitos países subdesenvolvidos, especialmente da África, Ásia e América Latina. As estimativas apontam que 779 milhões de pessoas estão sob risco de infecção e 243 milhões de pessoas necessitaram de tratamento em 2011. No Brasil, a esquistossomose tem sido notificada em estados do nordeste, sudeste e sul, acometendo aproximadamente seis milhões de indivíduos. O objetivo é verificar a situação de transmissão da esquistossomose em área urbana do município de Bandeirantes, Paraná. Trata-se de um estudo de caráter epidemiológico descritivo, de corte transversal, composto por duas etapas. Na primeira etapa foi feito um estudo retrospectivo, com a utilização de dados secundários obtidos nos arquivos da Secretaria Municipal de Bandeirantes. A segunda etapa foi realizado um estudo de base populacional, com coletas e exames de fezes e aplicação de um questionário contendo dados epidemiológicos sobre a população em estudo. A amostra foi composta de 304 pessoas. Após a coleta, as amostras fecais foram mantidas sob refrigeração até a realização do método de Kato-Katz. O histórico dos casos de esquistossomose no município de Bandeirantes mostra que houve maior prevalência nos anos de 2005, 2008 e 2009. O novo levantamento realizado mostrou nove indivíduos infectados por Schistosoma mansoni e dois casos foram classificados com o grau moderado/alto de infecção. Na análise do teste exato de fisher foi possível verificar que não houve diferença significativa quando associados os resultados dos exames e as variáveis obtidas no questionário epidemiológico (sexo e hábitos praticados). Os dados mostram que houve queda nos casos de esquistossomose ao longo dos anos no município de Bandeirantes, porém a perda de arquivos ao longo dos anos impossibilita comparações com os dados anteriores e indica o descaso quanto aos cuidados com a esquistossomose neste município. 37 f

The high-risk pregnancy increases the probability of an adverse evolvement, both of the mother and the fetus, and it is often responsible by high rates of premature birth and neonatal death. These events are considered public health problems worldwide and important indicators of maternal and child health. The objective of the research was to evaluate factors associated with prematurity and neonatal mortality of children born in hospitals, through the monitoring of pregnant women at high risk ambulatory of Parana´s Mother Network. This is an epidemiological, cross-sectional study of documentary basis, conducted with children born of mothers assisted by the high risk ambulatory of a philanthropic hospital in Maringá from September 2012 to September 2013. Data were collected between May and August 2014, through the medical records of pregnant women, newborn´s birth certificate and death certificate. The collected data were entered into spreadsheet Microsoft Office Excel 2010® and processed using the Statistica 7.1 program and Epi Info 3.5.2. In order to search the association between the dependent variables (preterm birth and neonatal death) and the independent, gross analysis, Odds Ratio and chi-square were applied. It was utilized multivariate analysis by logistic regression, with a significance level of 5% and 95% confidence interval. It was also employed spatial analysis with data on prematurity of the 30 municipalities that are part of the 15 Health Regional of Paraná, through the Exploratory Spatial Data Analysis. The prevalence of preterm birth was 19.1%. The multivariate analysis showed that women with moderate to severe hypertension and hypothyroidism have less chance of premature birth. The variables: preterm labor in a previous pregnancy and multiple pregnancy were associated with premature birth. The scoring method for scheduling consultations did not allow the prediction of prematurity. Among the different socioeconomic and demographic indicators analyzed, two variables presented spatial association with prematurity: distance from the reference center for treatment of high risk pregnancies and unemployment rate, with positive spatial autocorrelation (I = 0.2156, p = 0.012) and (I = 0.1877, p = 0.016), respectively, indicating that the greater the distance of the mother´s residence and also unemployment, the higher the rate of preterm infants. The neonatal mortality coefficient was 17.7 deaths/1.000 live births, most of them occurring in the early neonatal period (76.9%). The multivariate analysis demonstrated that the preterm labor, fetal malformation and multiple pregnancy were the clinical complications associated with neonatal death. Premature newborns, with very low birth weight and Apgar score lower than 7 at 5 minutes after delivery presented high risk of death. In conclusion, the identification of risk factors for preterm birth and neonatal mortality may help in the planning of actions for the perinatal network consolidation, with the restructuring and qualification of the assistance processes in the prenatal of high risk pregnancies, childbirth and newborn care. A gestação de alto risco aumenta a probabilidade de evolução desfavorável, tanto para a mãe como para o feto, sendo responsável, muitas vezes, pelas elevadas taxas de parto prematuro e óbito neonatal. Esses eventos são considerados problemas de saúde pública em nível mundial e indicadores importantes da saúde materno-infantil. O objetivo da pesquisa foi analisar os fatores associados à prematuridade e óbito neonatal de crianças nascidas de partos hospitalares, a partir do acompanhamento das gestantes no ambulatório de alto risco da Rede Mãe Paranaense. Trata-se de um estudo epidemiológico, transversal, de base documental, realizado com crianças nascidas de partos hospitalares de mães acompanhadas pelo ambulatório de alto risco de um hospital filantrópico de Maringá no período de setembro de 2012 a setembro de 2013. Os dados foram coletados, entre os meses de maio e agosto de 2014, a partir do prontuário da gestante, do registro de nascimento do recém-nascido e da Declaração de Óbito. Os dados coletados foram transcritos para planilha eletrônica do programa Microsoft Office Excel 2010® e processados utilizando o programa Statística 7.1 e Epi Info 3.5.2. Para pesquisar a associação entre as variáveis dependentes (nascimento prematuro e óbito neonatal) e as independentes, foi utilizada a análise bruta, Odds Ratio e qui-quadrado. Utilizou-se análise multivariada, por meio da regressão logística, com nível de significância de 5% e intervalo de confiança de 95%. Utilizou-se ainda análise espacial com dados sobre prematuridade dos 30 municípios que compreendem a 15º Regional de Saúde do Paraná, por meio da Análise Exploratória de Dados Espaciais. A prevalência de parto prematuro foi de 19,1%. A análise multivariada mostrou que mulheres com hipertensão arterial moderada a grave e com hipotireoidismo apresentaram menos chance de parto prematuro. As variáveis: trabalho de parto prematuro em gestação anterior e gestação múltipla apresentaram associação com o nascimento prematuro. O método de escore para agendamento de consultas não permitiu a predição da prematuridade. Dentre os diferentes indicadores socioeconômicos e demográficos analisados, duas variáveis apresentaram associação espacial com a prematuridade: distância do ambulatório de referência para atendimento da gestante de alto risco e taxa de desemprego, com autocorrelação espacial positiva (I=0,2156, p=0,012) e (I=0,1877, p=0,016), respectivamente, indicando que quanto maior a distância do município de residência da gestante e também a taxa de desemprego maioro número de prematuros. O coeficiente de mortalidade neonatal foi de 17,7 óbitos/1.000 nascidos vivos, sendo sua maioria no período neonatal precoce (76,9%). A análise multivariada mostrou que o trabalho de parto prematuro, malformação fetal e gestação múltipla foram as intercorrências clínicas associadas ao óbito neonatal. Recém-nascidos prematuros, com muito baixo peso ao nascer e Índice de Apgar menor que 7 no 5º minuto de vida apresentaram risco elevado de morte.Conclui-se que a identificação de fatores de risco para a prematuridade e o óbito neonatal podem auxiliar no planejamento de ações para consolidação da rede perinatal, com reestruturação e qualificação dos processos assistenciais no pré-natal da gestante de alto risco, parto e no cuidado ao recém-nascido. 119 f

Group B Streptococcus (GBS) or Streptococcus agalactiae may be part of the intestinal microbiota of the human beings, especially colonizing the gastro-intestinal and genitourinary tract. This species is often related to life-threatening diseases in newborns, such as septicemia, pneumonia and meningitis and is associated with complications during pregnancy and the postpartum period. The aim of this study was to analyze the occurrence of GBS in pregnant women users the public health service, the performance of medium Hitchens-Pike-Todd-Hewitt (HPTH) and antimicrobial susceptibility of strains of GBS. Descriptive analytical cross-sectional study carried out with 556 pregnant women, 496 of whom were between 35 and 37 weeks of pregnancy and 60 of them were ≥ 38 weeks. The study was conducted from September 2011 to March 2014 in northern Paraná State. Vaginal and anorectal clinical samples of each pregnant woman were inoculated on sheep blood agar, in HPTH and Todd-Hewitt medium. The clinical isolates studied were subjected to antimicrobial susceptibility testing by agar diffusion, according to the standards of the Clinical and Laboratory Standards Institute (CLSI). Of the 496 pregnant women, 141 (28.4%) were positive for GBS, based on the combination of the three culture media and on clinical samples. The detection rates were 22.2% for HPTH medium, 21.2% for sheep blood agar, and 13.1% for the Todd-Hewitt enrichment broth. Of the 60 women at ≥ 38 weeks of pregnancy, seven (11.7%) were positive for GBS, and there was growth in HPTH and sheep blood agar media. To the 141 women who were positive for GBS, 136 antimicrobial susceptibility tests were performed, in which 100% proved sensitive to penicillin. Erythromycin resistance showed 8.1% and clindamycin 2.2%. The results demonstrate that HPTH and sheep blood agar media were more sensitive than Todd-Hewitt broth enrichment for screening GBS in pregnant women, suggesting that the two media should be used together for vaginal and anorectal samples. All isolates of GBS showed sensitivity to the most frequently used drug for intrapartum prophylaxis: penicillin. Resistance to erythromycin and clindamycin was also detected, thus reducing the options for prophylaxis in women who are allergic to penicillin. Based on these results, it is recommended that the health unit perform universal screening of pregnant women and adequate prophylaxis when indicated. O Estreptococo do Grupo B (EGB) ou Streptococcus agalactiae pode fazer parte da microbiota dos seres humanos, colonizando principalmente o trato gastro-intestinal e geniturinário. Esta espécie é frequentemente relacionada a doenças potencialmente fatais em recém-nascidos, como septicemia, pneumonia e meningite e está associada a complicações durante a gravidez e período pós-parto. Os objetivos deste estudo foram analisar a ocorrência de EGB em gestantes usuárias do serviço público de saúde, o desempenho do meio de cultura Hitchens-Pike-Todd-Hewitt (HPTH) e a sensibilidade antimicrobiana de isolados clínicos de EGB. Estudo transversal analítico descritivo realizado com 556 gestantes, das quais 496 estavam com 35-37 semanas de gestação e 60 estavam com ≥ 38 semanas. O estudo foi realizado de setembro de 2011 a março de 2014 no norte do Paraná. Amostras clínicas vaginal e anorretal de cada gestante foram semeadas em ágar sangue de carneiro, em meio HPTH e Todd-Hewitt. Os isolados clínicos estudados foram submetidos ao teste de susceptibilidade aos antimicrobianos por difusão em ágar, de acordo com as normas do Clinical and Laboratory Standards Institute (CLSI). Das 496 gestantes, 141 (28,4%) foram positivas para EGB, com base na combinação dos três meios de cultura e espécimes clínicos. As taxas de detecção foram de 22,2% para o meio HPTH, 21,2% para agar sangue de carneiro, e 13,1% para o caldo de enriquecimento Todd-Hewitt. Das 60 gestantes com ≥ 38 semanas de gestação, sete (11,7%) foram positivas para EGB, sendo que houve crescimento nos meios de HPTH e Agar sangue de carneiro. Das 141 gestantes positivas para EGB foram realizados 136 testes de sensibilidade aos antimicrobianos, no qual 100% foram sensíveis a penicilina. A eritromicina apresentou 8,1% de resistência e a clindamicina 2,2%. Os resultados demonstram que o meio HPTH e Agar sangue de carneiro foram mais sensíveis que o caldo de enriquecimento Todd-Hewitt para triagem de EGB em gestantes, indicando que os dois meios devem ser usados em conjunto para amostras vaginal e anorretal. Todos os isolados de EGB mostraram sensibilidade à droga mais frequentemente usada para a profilaxia intraparto: penicilina. A resistência à clindamicina e eritromicina também foi detectada, diminuindo assim as opções para a profilaxia em mulheres que são alérgicas à penicilina. Em função destes resultados, recomenda-se que a unidade de saúde realize rastreio universal das gestantes e profilaxia adequada quando indicado. 87 f

Cervical cancer (CC) still represents a major public health problem. The estimated incidence and of mortality rates in Brazil are gradually decreasing and presenting intermediate values in relation to developing countries. In 2014, the estimates in Brazil were of 15,590 new cases, with an average estimated risk of 15.33 cases per 100,000 women, with large variations in regional incidence rates. Several studies have described the incidence of HPV and its importance in the CC, but there is a scientific gap in relation to the follow-up and treatment of diagnosed cases in our region. The aim of this study was to investigate the risk behavior for CC prevalence and HPV resolution in women of a private network of a municipality in the Southern of Brazil. It is a descriptive and retrospective study with 25 women aged from 20 to 59 years of a private network healthcare attended by a specialized clinic in the period from January to December 2012 in a city in the Northwest of Parana State, Southern Brazil. Data medical records with cytological results and positive HPV were used. After treatment, these women were subjected to follow-up and after 6 months. Data were statistically analyzed using the t Test and Chi-square test with 5% significance. The samples were collected from the cervicovaginal region and were considered normal those exams who did not present: atypical squamous cells of undetermined significance (ASC-US) or does not exclude intraepithelial lesion high grade typical squamous cell high grade (ASC-H), atypical glandular cells (AGC), low grade (LSIL) or high grade (HSIL). Cervical squamous intraepithelial lesion of squamous cell cervical carcinoma in situ (ISCC) and invasive squamous cervical carcinoma (CC). The mean age of the women studied was of 27.76 ± 7.75 years, being the majority married, employed and non-smokers. The average age of menarche was of 13.00 ± 0.50 years and of 17.48 ± 1.78 years, was the first sexual intercourse, with only 8.0% (2) initiating sexual activity with about 15 years. Most of them had 1-2 children (60.0%) About 88.0% reported having a sexual partner throughout their lives, and all women were currently sexually active. About 68.0% used hormonal contraception methods. All women presented leukorrhea and pain and infection by a single HPV type. As for the degree of the lesion, 80.0% had high-risk and 20.0%, low-risk. The top-most prevalent risk HPV was of 16. The findings showed relevant information on the risk factors and HPV infection, in addition to the treatment and follow-up of 6 months in women economically and socially differentiated, with the absence of' traditional risk factors, corroborating with the information that different risk factors can be described in different populations. Thus, this study reinforces the fact that even women who do not have the usual risk factors should be subjected to follow-up and study of HPV in order to verify the prevalence of the infection, and indeed being subjected to a continuous monitoring to check persistent infection and promoting early diagnosis of the lesions, as well as appropriate treatment, preventing the occurrence of the CC. O câncer de colo de útero (CCU) ainda se configura como um importante problema de saúde publica. As taxas de incidência estimada e de mortalidade no Brasil apresentam tendência de queda, com valores intermediários em relação aos países em desenvolvimento. Em 2014, no Brasil, as estimativas foram de 15.590 casos novos, com risco estimado de 15,33 casos a cada 100 mil mulheres, com grandes variações regionais. Diversos trabalhos descrevem a incidência de HPV e sua importância no CCU, porém existe uma lacuna científica em relação ao acompanhamento e tratamento dos casos diagnosticados, em nossa região. O objetivo deste estudo foi analisar a prevalência, os fatores de risco para CCU e o desfecho da infecção por HPV apos intervenção terapêutica em mulheres da rede particular de saúde em um município do Sul do Brasil Estudo descritivo, retrospectivo, realizado com 25 mulheres na faixa etária de 20 a 59 anos, atendidas em uma clinica especializada no período de janeiro a dezembro de 2012, em um município do Noroeste do Paraná, Sul do Brasil. Foram utilizados dados de fichas médicas, com resultados citológicos e diagnóstico positivo para HPV. Após implementação do tratamento, estas mulheres foram acompanhadas por 6 meses. As amostras foram coletadas da região cérvico-vaginal, sendo considerados exames normais aqueles que não apresentaram: células escamosas atípicas de significado indeterminado (ASC-US), lesão intra-epitelial de alto grau (ASC-H), células glandulares atípicas (AGC), baixo grau (LSIL) ou de alto grau (HSIL), lesão intra-epitelial cervical escamosa de células escamosa in situ carcinoma cervical (ISCC) e de carcinoma cervical escamoso invasive (CC). Os dados foram analisados estatisticamente através do Teste t e Teste de Qui-Quadrado, 5% de significância. A media de idade das mulheres estudadas foi de 27,76=h7,75 anos, sendo a maior parte casada, com ocupação remunerada e não fumante. A idade média da menarca foi de 13,00±0,50 anos e da coitarca l7,48±1,78 anos, com 8,0% (2) iniciando a atividade sexual com idade 15 anos. A maioria tinha 1 a 2 filhos (60,0%), 88,0% relataram ter um parceiro sexual durante toda a vida, sendo que todas as mulheres eram ativas sexualmente. Um total de 68,0% usava método contraceptivo hormonal. Todas as mulheres apresentaram leucorréia, dor e tiveram infecção por um único tipo de HPV. Quanto ao grau da lesão, 80,0% apresentararn alto risco e 20,0% baixo risco. O HPV de alto risco mais prevalente foi o 16. O resultado anátomo patolégico mostrou que 48% apresentaram displasia com neoplasia intra-epitclial cervical (NIC I), 48% apresentaram NIC II e 4%, NIC III. Seis meses após serem submetidas a traquelectomia com cirurgia de alta frequência, todas as mulheres apresentaram resultados normais nos exames. Os achados sinalizam informações relevantes sobre os fatores de risco e infecção por HPV, além do tratamento e seguimento de 6 meses em mulheres econômica e socialmente diferenciadas, com ausência de fatores de risco habituais, corroborando com a informação de que diferentes fatores de risco podem ser descritos em diferentes populações. Desta forma, este estudo reforça o fato de que, mesmo as mulheres que não possuem os fatores de risco habituais devem ser submetidas a acompanhamento e estudo do HPV, a fim de verificar a presença da infecção, além de serem submetidas a acompanhamento continuo para verificar a persistência da infecção, promovendo o diagnóstico precoce das lesões apresentadas, bem como tratamento adequado. 44 f

We report the results of a serological and molecular research in individuals with no clinical manifestations of American cutaneous leishmaniasis (ACL) who lived in endemic area and we report the results of association between HLA class I genes (HLA-A, -B, and -C) and ACL in an endemic region of southern Brazil. A retrospective study was carried out on the epidemiological records of patients from the 13th and 15th Health Sections of the state of Paraná diagnosed with ACL. Patients with clinical manifestations of the disease and with positive parasite detection and/or Montenegro skin test were contacted for the collection of biological material and composition cases of group. The control group consisted of individuals with no clinical manifestations of ACL who lived in endemic region. Antibodies were detected in the healthy individuals by enzyme-linked immunosorbent assay (ELISA), and the positive samples were analyzed by indirect immunofluorescence (IIF) for Leishmania braziliensis and Trypanosoma cruzi. Polymerase chain reaction (PCR), employing the MP3H/MP1L primers, amplified a fragment of Leishmania (Viannia) k-DNA. All cases and controls group samples were subjected to HLA class I (HLA-A, -B, -C) typing were carried out through PCR-SSO using Luminex technology. The results reveal the presence of subclinical Leishmania infections and demonstrate the involvement of HLA-C*04 in the ACL susceptibility in individuals from an ACL-endemic region of southern Brazil. Neste estudo são relatados os resultados de uma investigação sorológica e molecular em indivíduos sem manifestações clínicas da leishmaniose tegumentar americana (LTA) e demonstrados os resultados da associação com marcadores genéticos HLA de Classe I (-A, -B, -C) e LTA em moradores de uma região endêmica do sul do Brasil. Foi realizado um estudo retrospectivo das fichas epidemiológicas de pacientes atendidos no Laboratório de Leishmanioses da Universidade Estadual de Maringá, para o diagnóstico de LTA, pertencentes a 13ª e 15ª Regional de Saúde do Estado do Paraná. Os pacientes que apresentaram positividade na pesquisa do parasito e/ou intradermorreação de Montenegro foram contatados para a coleta de material biológico e composição do grupo de casos. O grupo controle foi composto por indivíduos saudáveis com ausência de manifestações clínicas da doença e moradores de locais endêmicos para LTA. A detecção de anticorpos do grupo sem manifestação clínica da LTA foi realizada por Enzyme-linked immunosorbent assay (ELISA), e as amostras positivas, foram analisadas pela imunofluorescência indireta (IFI) para Leishmania braziliensis e Trypanosoma cruzi. A reação em cadeia da polimerase (PCR) foi realizada com os iniciadores MP3H/MP1L que amplificam fragmento do k-DNA de Leishmania (Viannia). Todas as amostras do grupo de casos e controles foram submetidas à tipificação HLA-A, -B, -C pela metodologia PCR-SSO (tecnologia Luminex). Os resultados demonstraram a presença de infecção subclínica por Leishmania e o envolvimento do HLA-C*04 na susceptibilidade a LTA em moradores de uma região endêmica para LTA no sul do Brasil. 46 f

Obesity has been reaching alarming indexes all over the world. The numbers are particularly high among young women, in reproductive age, who have been in great number submitted to bariatric surgery. With the consequent weight loss and improvement on the fertility, the number of pregnancies in the postoperative period has been increasing. This cross-sectional, descriptive, retrospective research, evaluated the obstetric and perinatal results of pregnancies in 19 women submitted to the bariatric surgery from 1999 to 2011, as well as the growth and development of children born before and after the surgery. The study was carried in the municipal district of Maringa, South of Brazil. From them 11 children were born before and 32 after the surgery. Previously to the surgery, 84.2% of the women showed obesity grade 3. The body mass index in the beginning of the pregnancies previous to the surgery showed 63.6% of obesity, with a weight gain above the ideal in 72.7% of them, in detriment of 26.7% of obesity in the beginning of the pregnancies occurred after the surgical procedure. It was evidenced weight gain below normal in 53.3% of the cases. High indexes of Cesarean section were found for the childbirths previous to the bariatric surgery (100%) as well for the ones subsequent to the surgical procedure (93.7%). The mean time between the surgery and the first pregnancy was of 42.96±29.05 months. Preterm newborn babies were found in greater percentage among the children born after the bariatric surgery (6-18.8%) than among the children born previous to the surgery (1-9.1%). In relation to the growth, normal weight was described in 27.3% and obesity in 54.5% of the children born before the surgical procedure, in detriment of normal weight in 59.4% and reduction of the obesity in 31.2% of the children born after the surgery. The excess of weight lost up to the pregnancy was of 64.88±22.89%. Three male children evaluated by the Development Screening Test Denver - II showed doubtful results, for they presented delay in the speech. They were born from C-section, full term, normal weight at the birth, in an interval of 3, 14 and 114 months after the surgery. In those pregnancies the weight gain was reported was adequate, below and above normal. Statistical association was found between the interval among surgery to pregnancy and the outcome of the suspect Denver II test (p= 0.011). Except for the largest index of low birth weight, it can be concluded that pregnancy after the surgery was shown to be safe, without important intercurrences for mother and child. The children's growth was found to be adequate, with obesity reduction. Although changes in the development of speech were detected, no factors were studied to support its association with pregnancy after surgery. Further researches are necessary on the subject. Seus números são particularmente elevados nas mulheres jovens, em idade reprodutiva, as quais têm se submetido cada vez mais às cirurgias bariátricas. Com a conseqüente perda de peso e melhora da fertilidade, tem aumentado o número de gravidezes no pós-operatório destas cirurgias. Esta pesquisa retrospectiva, descritiva e transversal avaliou os resultados obstétricos e Peri natais de gravidezes de 19 mulheres submetidas à cirurgia bariátrica entre os anos de 1999 a 2011, bem como o crescimento e desenvolvimento dos filhos nascidos antes e depois da realização da cirurgia, no município de Maringá, Paraná, Sul do Brasil. Estas mulheres tiveram, ao todo, onze filhos antes e trinta e dois filhos depois da cirurgia. Anteriormente à cirurgia, 84,2% das mulheres apresentam classificação de obesidade de grau 3. O índice de massa corporal no início das gravidezes anteriores à cirurgia foi de 63,6% de obesidade, com um ganho de peso acima do ideal em 72,7% delas, contra 26,7% de obesidade no início das gravidezes ocorridas após o procedimento cirúrgico, tendo sido evidenciado ganho de peso abaixo do normal em 53,3%. Índices elevados de cesariana foram encontrados nos partos anteriores (100%) e posteriores (93,7%) à cirurgia bariátrica. O tempo médio entre a cirurgia e a primeira gravidez foi de 42,96±29,05 meses. Recém-nascidos pré-termo foram encontrados com maior porcentagem entre filhos nascidos após a cirurgia bariátrica (6-18,8%) do que em filhos nascidos antes da cirurgia (1-9,1%). Em relação ao crescimento, peso normal foi descrito em 27,3% e obesidade em 54,5% das crianças nascidas antes do procedimento cirúrgico, contra peso normal em 59,4% e redução da obesidade para 31,2% nas crianças nascidas após a cirurgia. A média do excesso de peso perdido até o momento da gravidez foi de 64,88±22,89%. Em três crianças do sexo masculino avaliadas pelo Teste de Desenvolvimento de Denver II o resultado obtido foi suspeito, por elas apresentarem atraso na aquisição da linguagem. Estas eram nascidas de parto cesariano, a termo, com peso normal ao nascimento, num intervalo de 3, 14 e 114 meses após a cirurgia. Nestas gestações, o ganho de peso foi classificado como adequado, abaixo e acima do normal. Foi encontrada associação estatística entre o intervalo entre a cirurgia e a gravidez e o resultado do teste de Denver II suspeito (p= 0,011). Excetuando-se o maior índice de baixo peso ao nascer, pode-se concluir que a gravidez após a cirurgia se mostrou segura, sem intercorrências importantes para a mãe e a criança. As crianças apresentaram crescimento adequado, inclusive com redução da incidência de obesidade. Embora tenham sido evidenciadas alterações no desenvolvimento, relacionadas à linguagem, não foram estudados fatores que suportam sua associação com a gravidez no pós-operatório, sendo necessárias pesquisas mais aprofundadas sobre o tema. 162 f

Torque Teno Vírus (TTV) is a recently discovered virus in the periphery blood of Japanese-stock patients diagnosed with post-transfusion hepatitis, with high alanine aminotranspherase levels (ALT). TTV is associated with several pathologies and occurs in apparently healthy people. High predominance levels of infection by TTV may be found in different regions of the world and its transmission may occur through different modes. Current research detected TTV predominance in healthy donors living in the northern and northwestern regions of the state of Paraná, Brazil, in 2010, and verified by DNA samples from bone marrow volunteer donors through nested Polymerase Chain Reaction with a set of oligoinitiators for the N22 region. Virus has been detected in 69% (380/551) of healthy donors, with significant prevalence (74.7%; 124-551) within the 18 - 24-year-old bracket and in 70% (144/206) males and 68.4% (236/345) females. No significant association existed between the TTV and the variables ethnic group and civil state (p>0.05). The proportion of healthy donors but virus carriers did not differ with regard to the regions under analysis. TTV in apparently healthy individuals may be a risk factor for the development of diseases. Further studies in different healthy populations are required to discover transmission modes and its participation in different pathologies. O Torque Teno Virus (TTV) é um vírus recentemente descoberto no sangue periférico de pacientes japoneses diagnosticados com hepatite pós-transfusional, com elevados níveis de Alanina Aminotransferase (ALT). Encontra-se associado a diferentes patologias e presente em indivíduos aparentemente saudáveis. Altas taxas de prevalências da infecção pelo vírus são encontradas em diferentes regiões do mundo e a transmissão pode ocorrer por diversas vias. O objetivo do estudo foi detectar a prevalência do TTV nos doadores saudáveis das regiões Norte e Noroeste do Estado do Paraná/Brasil no ano de 2010. A presença do TTV foi verificada nas amostras de DNAs dos candidatos voluntários a doação de medula óssea, pela Reação em Cadeia da Polimerase Aninhada (nested-PCR), utilizando um conjunto de oligoiniciadores para a região N22. O vírus foi observado em 69% (380/551) dos doadores saudáveis, sendo significativamente prevalente na faixa etária de 18 a 24 anos, 74,7% (124/551) e esteve presente em 70% (144/206) e 68,4% (236/345) dos gêneros, masculino e feminino, respectivamente. Não houve associação significativa entre a presença do vírus com as seguintes variáveis: grupo étnico e estado civil (p>0,05). A proporção de doadores saudáveis portadores do vírus não diferiu quanto às mesorregiões do estudo. A presença do TTV em indivíduos aparentemente saudáveis poderá ser um fator de risco para o desenvolvimento de patologias. Novos estudos, em diferentes populações saudáveis, serão necessários para elucidar a via de transmissão bem como sua participação em diferentes patologias. 63 f

For more than three decades non-transmissible chronic diseases have been responsible for the largest load of disease in Brazil. They are susceptible to prevention and they represent the most cost effective investment in the health system. Among those diseases the cardiovascular ones are now the major cause of death in the world, being hypertension an important arterial risk factor. Globally, the prevalence of arterial hypertension in adults from 25 years of age was estimated to be about 40%, in 2008, and in Brazil its prevalence was found to be between 22.3% and 43.9%. In agreement with the plan of strategic actions to face non-transmissible chronic diseases, the appropriate management of arterial hypertension requests articulating actions: the surveillance of hypertension, co-morbidities and its determinants; the integrality ofthe care and the promotion of health. For that, the accomplishment of this study is justified due to the high rate of morbi-mortality caused by non-transmissible chronic diseases in the country and in the world, to the increase of risk factors associated to the adult individual, and the absence of survey on the population in small municipal districts and the ignorance of the prevalence of risk factors in the population of Paiçandu, PR. The main objective of this study was to estimate the prevalence of factors and risk behaviors for cardiovascular diseases, to implement actions for the prevention and control of the cardiovascular diseases, and to evaluate the effectiveness of an intervention program in the reduction of the factors of cardiovascular risk in adults with arterial hypertension, residents in Paiçandu - PR. The study was divided into two stages: A cross-sectional, descriptive study of population base and a randomized clinical essay. In the cross-sectional research 415 adults were assessed, being the sedentary(77.77%) the greatest prevalence, followed by overweight (57.11%) and central obesity (49.63%). The increased prevalence of risk factors with increasing age was evident in relation to hypertension, diabetes mellitus, dyslipidemia, central obesity, overweight and inappropriate food habits. For the association of the arterial hypertension with other risk factors 408 individuals were appraised and the greatest prevalence was among women (24.64%). It was noticed that from 50 to 59 years of age, 40.38% of the hypertensive individuals possess five or more factors of cardiovascular risk. In the randomized clinical study 94 individuals pronounced themselves as hypertensive during the population research randomly divided into two groups, intervention group - IG (21) and control group - CG (21). The intervention was accomplished in 16 weeks and consisted of orientations on health and nutrition, and of physical exercises. In the first month the IG showed a drop of six participants. Of the 15 members who remained six accompanied the group for eight weeks, and nine for 16 weeks. Thus, we considered two distinct groups: those who participated until the end of test - IGa, and those who attended eight weeks - IGb. The CG finished with 15 members and presented a leakage of six participants. Comparison of blood pressure before and after the intervention showed that the IGb variation was statistically significantly for systolic blood pressure (143.3 ± 11.0 to 121.7 ± 14.6, p = 0.024). The IGa has significantly ecreased values to systolic blood pressure and diastolic blood pressure (132.2 ± 13.9 to 118.9 ± 8.7, p = 0.036 and 86.7 ± 9.4 to 77.8 ± 6.3, p = 0.041 respectively), whereas the control group showed no significant decrease in the systolic blood pressure and diastolic blood pressure. The biochemical variables showed changes in IGa for all variables, and the HDL showed a significant difference (50.4 ± 6.6 to 54.2 ± 7.8, p = 0.034) for this group. The IGb and CG showed changes in biochemical parameters, however, both without statistically significant differences. IGa presented a decrease in the values of all anthropometric variables, and revealed significant differences for hip circumference (108.9 ± 11.6 to 106.4 ± 12.1, p = 0.042) and waist/hip relation (1.00 ± 0.07 for 0.99 ± 0.07, p = 0.030). The IGb only showed decreased values on waist/hip relation (0.98 ± 0.06 to 0.97 ± 0.05). Body fat percentage (38.0 ± 6.4 to 37.9 ± 6.9) and hip circumference (105.3 ± 6.4 to 103.6 ± 5.7) presented decreased values in CG, but these last 3 cases showed no significant values decreasing. The comparison of the blood pressure before and after the intervention showed that the intervention group presented significant difference for systolic and diastolic blood pressure measurements. The survey on cardiovascular risk factors through the accomplishment of the population inquiry made possible to know the profile of health of this population and it allowed to identify the need of interventions in health, giving the opportunity to discuss subjects referring to chronic condition and the risk factors involved, besides participating in actions that promote the adoption of a healthier lifestyle. These proposed intervention can constitute an important tool for health professionals work, especially nurses, since health education permeates his daily life, and also because this professional plays a significant role in prevention and health promotion. Há mais de três décadas as doenças crônicas não transmissíveis são responsáveis pela maior carga de doença no Brasil. Elas são passiveis de prevenção e representam o investimento mais custo efetivo entre todos aqueles do setor saúde. Dentre essas doenças destacam-se as cardiovasculares, atualmente a maior causa de morte no mundo, sendo a hipertensão arterial importante fator de risco. Globalmente a prevalência da hipertensão arterial em adultos a partir dos 25 anos foi estimada em cerca de 40%, em 2008, e no Brasil sua prevalência foi determinada entre 22,3% e 43,9%. De acordo com o plano de ações estratégicas para o enfrentamento das doenças crônicas não transmissíveis, o manejo adequado da hipertensão arterial requer ações articuladas: a vigilância da hipertensão, das comorbidades e de seus determinantes; a integralidade do cuidado e a promoção da saúde. Diante deste contexto compreende-se a importância da educação em saúde associada a atividade física sistematizada e mudança no estilo de vida. A educação em saúde tem por objetivo oportunizar momentos de reflexões e ações capazes de possibilitar às pessoas um aprendizado consciente, sem a intenção de controlar suas vidas. Deve ter como princípio estimular experiências que facilitem a realização de ações que melhorem as condições de vida e promovam saúde. Baseado no exposto acima a realização deste estudo justifica-se devido a elevada taxa de morbimortalidade por doenças crônicas não transmissíveis no país e no mundo, ao aumento dos fatores de risco associados ao indivíduo adulto e a não realização de inquéritos de base populacionais em municípios de pequeno porte e também pelo desconhecimento da prevalência dos fatores de risco na população de Paiçandu. O objetivo deste estudo foi estimar a prevalência de fatores e comportamentos de risco para doenças cardiovasculares, implementar ações para a prevenção e controle das doenças cardiovasculares e avaliar a eficácia de um programa de intervenção na redução dos fatores de risco cardiovascular em adultos com hipertensão arterial, residentes em Paiçandu - PR. O estudo foi dividido em duas etapas: estudo transversal, descritivo, de base populacional e um ensaio clínico randomizado. Na pesquisa transversal foram analisados 415 adultos, sendo o fator mais prevalente o sedentarismo (77,77%), seguida pelo excesso de peso (57,11%) e obesidade central (49,63%). O aumento da prevalência dos fatores de risco com o avançar da idade foi evidente em relação à hipertensão arterial, diabetes mellitus, dislipidemia, obesidade central, excesso de peso e hábito alimentar inadequado. Para a associação da hipertensão arterial com outros fatores de risco foram analisados 408 indivíduos e encontrou-se prevalência maior entre as mulheres (24,64%), observando-se, que entre a faixa etária dos 50 aos 59 anos, 40,38% dos hipertensos possuem cinco ou mais fatores de risco cardiovascular. No estudo ensaio clínico randomizado foram identificados 94 indivíduos que se declararam hipertensos durante a pesquisa populacional, aleatoriamente divididos em dois grupos, grupo intervenção - GI (21) e grupo realizada durante 16 semanas e consistiu em orientações de saúde e nutricionais, e realização de exercícios físicos. No primeiro mês o GI apresentou uma evasão de seis participantes. Dos 15 integrantes que permaneceram seis acompanharam o grupo por oito semanas, e nove durante 16 semanas. Assim, consideraram-se dois grupos de intervenção: os que participaram até o final do seguimento - GIa, e os que freqüentaram oito semanas - GIb. O GC terminou o seguimento com 15 integrantes e apresentou uma evasão de seis participantes. A comparação da pressão arterial antes e após a intervenção mostrou que o GIb apresentou diferença estatisticamente significativa para pressão arterial sistólica (143,3 ± 11,0 para 121,7 ± 14,6, p=0,024). O GIa apresentou diminuição significativa da pressão arterial sistólica e da pressão arterial diastólica (132,2 ± 13,9 para 118,9 ± 8,7, p=0,036 e 86,7 ± 9,4 para 77,8 ± 6,3, p=0,041, respectivamente); já, o GC apresentou diminuição não significativa dos valores de pressão arterial sistólica e pressão arterial diastólica. As variáveis bioquímicas no GIa apresentaram alterações para todas as variáveis analisadas, sendo que o HDL revelou diferença significativa (50,4 ± 6,6 para 54,2 ± 7,8, p=0,034) para este grupo. O GIb e GC mostraram alterações nos parâmetros bioquímicos, no entanto, ambos sem diferenças estatisticamente significativas. O GIa apresentou diminuição nos valores de todas as variáveis antropométricas, e revelou diferença significativa para circunferência do quadril (108,9 ± 11,6 para 106,4 ± 12,1, p= 0,042) e (1,00 ± 0,07 para 0,99 ± 0,07, p=0,030). O GIb apresentou diminuição apenas na relação cintura quadril (0,98 ± 0,06 para 0,97 ± 0,05) e o GC em percentual de gordura corporal (38,0 ± 6,4 para 37,9 ± 6,9) e circunferência do quadril (105,3 ± 6,4 para 103,6 ± 5,7), ambos não apresentaram diminuição significativa. O levantamento dos fatores de risco cardiovasculares por meio da realização do inquérito populacional possibilitou conhecer o perfil de saúde desta população e permitiu identificar a necessidade da implementação de intervenções em saúde, para que tenha a oportunidade de discutir assuntos referentes a sua condição crônica e os fatores de risco envolvidos, além de participar de ações que promovam a adoção de estilos de vida mais saudáveis. Esta proposta de intervenção pode constituir-se em uma ferramenta importante para o trabalho dos profissionais de saúde, principalmente do enfermeiro, pois a educação em saúde perpassa o seu cotidiano, e também porque esse profissional exerce um papel significativo na prevenção e promoção da saúde. 171 f

Leishmaniasis is widespread in the world and has a significant impact on public health. The life cycle of Leishmania involves sand fly vectors and mammalian reservoirs that sustain it in rural and urban areas. The Multiplex Polymerase Chain Reaction (multiplex PCR) is a technique with high specificity and sensitivity that has been useful for detecting the presence of Leishmania in sand flies. The objective of this study was to investigate the natural infection of sand flies by Leishmania in Bandeirantes Municipality, Parana State. Two pairs of primers were used for multiplex PCR: i) the MP3H and MP1L to amplify a fragment of 70 pb of the conserved region from the minicircle of the kinetoplast of the subgenus Leishmania (Viannia), and ii) the 5Llcac and 3Llca to amplify a fragment of 220 pb from the IVS6 gene region of the cacophony in insects of the genus Lutzomyia, for internal control of the reaction. Were identified 737 sand flies female, of which 640 (66 pools) were tested by multiplex PCR. Were identified specimens of Pintomyia pessoai, Nyssomyia whitmani, Pintomyia fischeri, Nyssomyia neivai, Migonemyia migonei, Expapillata firmatoi e Brumptomyia brumpti. All pools showed the 220 bp fragment for sand flies DNA detection but were not found anyone with Leishmania infection. The multiplex PCR containing an internal amplification control has proved effective for detecting Leishmania infection in sand flies. Despite not having been detected the presence of Leishmania in analyzed sandflies, can not be denied the importance of Pi. pessoai, Ny. whitmani, Pi. fischeri and Ny. neivai in LT epidemiology, since these species have been found with natural infection in other studies. The population density, geographic distribution, domicile and peridomicile frequency, anthropophily, adaptation of these insects in anthropic environments and climatic conditions are favorable factors to the persistence of enzootic cycle of Leishmania. These factors highlight the need for research on sand flies behavior, including the infection rate, such results can be an indicator of the need for intervention by public health services, triggering measures of these insects' population control. O ciclo evolutivo de Leishmania envolve mamíferos reservatórios e flebotomineos vetores que o mantém em zonas rurais e urbanas. A Múltipla Reação em Cadeia da Polimerase (multipla-PCR) e uma técnica com alta especificidade e sensibilidade e tem sido útil para detectar a presença de Leishmania em flebotomineos. O objetivo deste estudo foi investigar a infecção natural por Leishmania (Viannia) em flebotomineos no município de Bandeirantes, estado do Paraná. Apesar de não ter sido detectada a presença de Leishmania nos flebotomineos analisados, não se pode negar a importância de Pi. pessoai, Ny. whitmani, Pi. fischeri e Ny. neivai na epidemiologia da LT, uma vez que estas espécies tem sido assinaladas com infecção natural. A densidade populacional, a distribuição geográfica, a freqüência no peridomicilio e domicilio, a antropofilia, a adaptação nos ambientes antro picos desses insetos e as condições climáticas são fatores favoráveis a persistência do ciclo enzootico de Leishmania. Estes fatores põem em evidencia a necessidade de trabalhos de investigação sobre o comportamento de flebotomineos, incluindo a taxa de infecção, cujos resultados podem ser um indicador da necessidade da intervenção dos serviços de saúde publica para o desencadeamento de medidas de controle da população desses insetos. 39 f

The infection of the lower genital tract may contribute from 25.0% to 40.0% of preterm births. The most common infections are bacterial vaginosis (BV), vulvovaginal candidiasis (VC) and trichomoniasis, responsible for 90% of the cases. The objective of this work is to analyze the prevalence and factors associated with the vaginal infections of the lower genital tract in pregnant women. This is a descriptive, quantitative and cross-sectional study, developed in a municipality of northern Parana, during the period from July to November of 2013. The sample was composed of 196 pregnant women. The data collection occurred through a questionnaire and the collection of vaginal secretion. Crude analysis, odds ratio (OR), Chi-Square test and Fisher's exact test were performed. The prevalence of vulvovaginitis was of 47.9 %, and the most prevalent were VC (60.7 % ), BV (33 %) and a mixed infection of VC and BV (6.3 %).The significant result obtained was the variable marital status (p= 0.04) (OR= 2.3 CI95 % = 1.02 -5,31) and regarding the gynecological and obstetric aspects, it was observed that the age of the first sexual relation ≤ 15 years is associated with the presence of vulvovaginitis in pregnancy (OR = 2.7, 95% CI = 1.51 -5,13). In the logistic regression model the factors marital status and age of the first sexual relation ≤ 15 years associated with the vulvovaginitis were analyzed. Pregnant women without husbands and those whose ages of the first sexual relation were ≤ 15 years presented a risk of 2.3 and 2.8, respectively, for the development of vulvovaginitis. Considering the interaction between the variables marital status and age of the first sexual relation, it was observed that for pregnant women with companion and first sexual relation ≤ 15 years, that represented a protective factor for the presence of vulvovaginitis in pregnancy, on the other hand, for pregnant women without companions and first sexual relation ≤ 15 years, that represented a risk factor for vaginal infections in pregnancy. The occurrence of signs and symptoms among pregnant women with vulvovaginitis was reported by 43.3% of the pregnant women, prevailing VC (26.0 %), BV (14.2 %) and mixed infection (VC and BV) (3.0 %). It was observed that the pregnant women with normal vaginal flora showed signs or symptoms of vulvovaginitis (p= 0.04). The relationship between the laboratory diagnosis and the signs and symptoms of VC among the pregnant women was significant, itching (p= < 0.01) and burning sensation (p= < 0.01), with an OR= 4.54 (95% CI = 2,17 -9,56) and OR=3.32 (95% CI = 1,62 -6,84), respectively. Another important finding was the fissure OR=2.67 (95% CI 0,85 -8,38). Regarding the signs and symptoms associated to the BV among pregnant women, only fetid odor had a significant result (p= 0.04) and OR=2.26 (95% CI = 0,94 -5,37). In our study there was a high prevalence of vaginal infections among pregnant women, important information for the assistance during pre-natal care. The factors associated to vulvovaginitis are also instructive in helping to identify the risk criterion, because we are aware that there is evidence that the premature birth may be associated with the vulvovaginitis. Therefore, it is suggested the implementation of a tracking program for vulvovaginitis in pregnant women. As infecções mais comuns são a vaginoses bacteriana (VB), candidíase vulvovaginal (CV) e tricomoníase, responsáveis por 90% dos casos. O objetivo é analisar a prevalência e fatores associados às vulvovaginites do trato genital inferior das gestantes. Trata de um estudo descritivo de corte transversal e de caráter quantitativo, desenvolvido em um município do norte do Paraná durante o período de julho a novembro de 2013. A amostra foi composta por 196 gestantes. A coleta de dados ocorreu por meio da aplicação de um questionário e realização de coleta de secreção vaginal. Foi utilizado a analise bruta, odds ratio (OR), teste Chi-Square e teste exato de Fisher. A prevalência de vulvovaginites foi de 47,9%, e as mais prevalentes foram CV (60,7%), VB (33%) e infecção mista CV e VB (6,3%). Obteve-se como resultado significativo a variável estado civil (p=0,04) (OR= 2,3 IC95%=1,02-5,31) e com relação aos aspectos ginecológicos e obstétricos, foi observado que a idade da primeira relação sexual ≤ 15 anos está associada à presença de vulvovaginites na gestação (OR = 2,7, IC95%=1,51-5,13). No modelo de regressão logística utilizou-se os fatores estado civil e a idade da primeira relação sexual ≤ 15 anos associado às vulvovaginites. Gestantes sem companheiro e aquelas com a idade da primeira relação sexual ≤ 15 anos apresentaram risco de 2,3 e 2,8, respectivamente, para o desenvolvimento de vulvovaginites. A interação entre as variáveis estado civil e idade da primeira relação sexual, observou-se que gestantes com companheiro e a primeira relação sexual ≤ 15 anos é um fator de proteção para presença de vulvovaginites na gestação, por outro lado gestantes sem companheiros e a primeira relação sexual ≤ 15 anos foi um fator de risco para vulvovaginites na gestação. A ocorrência de sinais e sintomas entre as gestantes com vulvovaginites foi relatado por 43,3% das gestantes, prevalecendo a CV (26,0%), VB (14,2%) e infecção mista (CV e VB) (3,0%). Foi observado que as gestantes com flora vaginal normal apresentaram sinais ou sintomas de vulvovaginites (p=0,04). A woman with low-risk pregnancy can have as environment to give birth her own home. The World Health Organization recognizes the home as a suitable and safe place for the birth, due to the good obstetric outcomes and for facilitate the rescue of the birth physiology, since it is the woman's choice and that she and her family receive a safe care in the moment of the birth, with guarantee of quality of care with reference systems. The home birth is still rising in the country and it is an irreversible trend. Thus, it is essential to know the particularities that permeate it in order to contribute in building of foundations for this practice. So, this study aimed to understand the experience of planned home birth by the actors involved in the birth process. It is a descriptive research, with qualitative approach. It was approved by the Standing Committee on Ethics in Research Involving Humans (COPEP) of State University of Maringá / PR, under the approval number 544/2010 and developed in the municipality of Cascavel/PR. The data collection occurred between April and May 2011, through an episodic recorded interview using a semi-structured form. 34 people participated of the study, 08 of these were women, 18 family members and 08 professionals who had the experience of giving birth or following the birth process at home in a planned way. For data analysis it was used content analysis proposed by Minayo (2008). The results were showed in three articles. In Article 1, which sought to understand the women's perception, there was indication that the home allowed their protagonism and of their family by the peace and calm that the environment provides and for being of them domain. The women noticed the home care by the nurses as something that favors the autonomy, which contributed to a successful and physiological evolution of birth, lower painful perception, absence of unnecessary interventions, respect to the contact and the link between mother-child at birth, better physical recovery at postpartum and higher adaptation to maternity. Still, the successful outcome of births generated in women the desire to repeat the experience in future pregnancies, as well as they recommended this type of birth for other women. The Article 2 revealed that the partner and family members were invited and encouraged to be part of the whole process. They felt them as fundamental in the physical and emotional support to woman and as contributors for a good evolution of birth. In Article 3, the environment was considered safe to follow requirements for the treatment as low-risk pregnancy, suitable assessment during the birth, presence of suitable materials, transdisciplinary network and predefined local for referrals. It was concluded that home is a space of care that provides to the woman and to the family autonomy and protagonism at birth process. It is a safe strategy and an option to transform and improve the quality of obstetric care, contributing to the reduction of iatrogenesis and unnecessary interventions and consequently contributes to the reduction of maternal and perinatal mortality. Uma mulher com gestação de baixo risco pode ter como ambiente para dar à luz a sua própria casa. A Organização Mundial de Saúde reconhece o domicílio como um local adequado e seguro para o nascimento, em função dos bons resultados obstétricos e por favorecer o resgate da fisiologia do parto, desde que seja da escolha da mulher e que ela e sua família recebam um cuidado seguro no momento do parto, com garantia da qualidade da assistência com sistemas de referência. O parto domiciliar ainda está em ascensão no país e é uma tendência irreversível. Desta forma, é imprescindível conhecer as particularidades que o permeiam no intuito de contribuir na construção de alicerces para esta prática. Assim, este estudo teve como objetivo compreender a vivência do parto domiciliar planejado pelos atores envolvidos no processo do nascimento. Trata-se de uma pesquisa descritiva, com abordagem qualitativa. Foi aprovada pelo Comitê Permanente de Ética em Pesquisa Envolvendo Seres Humanos (Copep) da Universidade Estadual de Maringá-PR, sob o Parecer no 544/2010 e desenvolvida no município de Cascavel-PR. A coleta de dados ocorreu entre abril e maio de 2011, por meio de entrevista gravada episódica com utilização de formulário semiestruturado. Participaram do estudo 34 sujeitos, destes oito eram mulheres, 18 familiares e oito profissionais que tiveram a experiência de parir ou acompanhar o processo de nascimento no domicílio de forma planejada. Para a análise dos dados, utilizou-se a análise de conteúdo proposta por Minayo (2008). Os resultados foram apresentados em três artigos. No artigo 1 - buscou-se compreender a percepção das mulheres, houve indicação que o domicílio possibilitou o seu protagonismo e da sua família pela tranquilidade e calma que o ambiente proporciona e por ser de domínio destes. As mulheres perceberam o cuidado no domicílio pelas enfermeiras como favorecedor da autonomia, o qual contribuiu para uma evolução fisiológica e exitosa do parto, menor percepção dolorosa, ausência de intervenções desnecessárias, respeito ao contato e vínculo da díade mãe-filho no nascimento, melhor recuperação física no pós-parto e maior adaptação à maternidade. Ainda, o desfecho bem sucedido dos partos gerou nas mulheres o desejo de repetir a experiência em gestações futuras, bem como, estas recomendam este tipo de parto para outras mulheres. No artigo 2 - revelou-se que o parceiro e familiares foram convidados e incentivados a fazerem parte de todo o processo. Sentiram-se fundamentais no suporte físico e emocional à mulher e como colaboradores para uma boa evolução do parto. No artigo 3 - o ambiente foi considerado seguro por seguir requisitos para o atendimento como baixo risco gestacional, avaliação adequada no decorrer do trabalho de parto, presença de materiais adequados, rede transdiciplinar e local pré-definido para encaminhamentos. Conclui-se que o domicílio é um espaço de cuidado que oportuniza à mulher e à família a autonomia e o protagonismo no processo de nascimento. É uma estratégia segura e uma opção para transformar e melhorar a qualidade da atenção obstétrica, contribuindo para a redução das iatrogenias e intervenções desnecessárias e, consequentemente, colabora com a redução da mortalidade materna e perinatal. 109 f

Several situations in human day-to-day life cause turmoil in the subject's mental health. When the latter is unprepared for overloads of stressing factors they will bring about behavior changes which, if not perceived, understood or accepted within the family context, affect relationships among family members. Current research aims at understanding the motives that cause subjects to integrate artisan groups as a prop for their mental health, the meaning of group participation and its influence on family relationship. This fact may help them to seek a mechanism or a support basis that would make them find a new path to face and adapt themselves to unexpected events. Thus, group space may be used as a tool with a therapeutic potential for the mental state against a return to pain conditions. Psychic ailment is consequently avoided. A descriptive research with eleven females was undertaken and featured participatory observation and qualitative approach with regard to an artisan group of a Health Basic Unit in the municipality of Maringá PR Brazil, between March and August 2009. Data collection techniques comprised participatory observation, systematically reported in a field diary, and an open interview carried out at the subjects' home. Data were analyzed according to Bardin, coupled to organization of analysis, codification and categorization by the identification of themes: a space for the promoção and rehabilitation of mental health; Finding a space that promotes speaking, listening and sharing; seeking a space to attend to real needs; the precious stones that we meet in our path; changes perceived with regard to one self and reflected in familial conviviality. Results showed that the artisan group represented a solution from tedious routine life, moments of leisure and a rupture from the tensions caused by daily problems. Further, the artisan group was counterpoised to the extenuating and stress context which the female participating subjects experienced, or rather, a way to experience a period of creativity, leisure, occupation of space and time, distraction and laughter. In fact, it was experienced as a period dedicated to self-care and a better life quality which directly affected the maintenance of mental health. The recovery of self-esteem and self-confidence was important so that the female subjects could live a different type of life. In fact, their identities had been lost owing to unexpected situations they were experiencing when they started to participate in the group. Group conviviality made these women perceive changes in behavior which affected their way of thinking, acting and reacting in the wake of common day-to-day situations. It also made them think on the respect one must have with regard to the individuality of the other so that they could live harmoniously and healthily within the family. The participants' perceptions on group meetings also showed an improvement in family relationships which caused a significant approach, agreement and communication among the members. This fact was also perceived by members of the family who continued their support so that they could further their participation in the group. Exchange of life experience among the female subjects was a lively factor that made possible the acquisition of important knowledge and information according to the needs of each one so that a certain type of behavior could be acquired or changed. The artisan group showed up to be not a mere leisure as many believed but a factor of paramount important. In fact, transformation through the conviviality with other people became possible. A new meaning to the subjects' life was thus provided when prior to the event their lives was full of anxiety, sadness, loneliness and lacking expectation of any solutions to face their woes. The artisan group within the prospect of mental health is a strategy which contributes towards favorable and healthy conditions for an improvement in life quality and, consequently, the maintenance and re-equilibrium of the female subjects' mental health. It is highly important to discover other ways of mental health care so that other people who suffer and need care prior to their sickness may be accepted. This fact may be found in the interview answers as one of the motives that caused some of the female subjects to integrate themselves in the artisan group to lessen their anxieties and unhappiness found in the daily course of their lives. Carrying out activities within a group is an important strategy that should be employed by health professionals, especially by nurses, for the development of activities to promote mental health as basic care. Current analysis showed the importance of group space and its positive influence in the life of the subjects. In fact, it directly affected one's relationship and the relationship with other people in their lives. Direct influence in the maintenance of the subjects' mental health has been successfully perceived. Muitas das situações que acontecem no cotidiano da vida do ser humano podem provocar uma desestrutura da saúde mental dos indivíduos, e essa quando não preparada para suportar a sobrecarga de alguns fatores estressantes podem levar a mudanças de comportamentos que quando não percebidas, compreendidas e aceitas dentro do contexto familiar, podem afetar as relações entre os membros familiares. Isso pode levá-los a buscar um mecanismo ou um recurso de suporte que os possibilitem reencontrar um novo caminho para enfrentamento e adaptação de eventos inesperados, nos permitindo pensar que o espaço grupal vem sendo utilizado como uma ferramenta que tem um potencial terapêutico que auxilia no cuidado da saúde mental para reverter à condição de sofrimento evitando, assim, um adoecimento psíquico. Desta forma, o presente estudo tem por objetivos compreender o motivo que leva as pessoas a buscar o grupo de artesanato como cuidado à saúde mental e apreender o significado da participação no grupo e a interferência nas relações familiares. Realizou-se uma pesquisa descritiva de abordagem qualitativa, em um grupo de artesanato de uma Unidade Básica de Saúde do município de Maringá - PR, no período de março a agosto de 2009, que foi desenvolvida junto a 11 mulheres. As técnicas de coleta de dados utilizadas foram: a observação participante, sendo registrada sistematicamente em um diário de campo e a entrevista aberta realizada em domicílio. Os dados obtidos foram tratados segundo a análise de conteúdo proposta por Bardin, onde se procedeu à organização da análise, à codificação e à categorização a partir da identificação dos seguintes temas: Grupo de Artesanato: um espaço de promoção e reabilitação da saúde mental; Encontrando um espaço que promove a fala, a escuta e a partilha; Buscando um espaço para atender as reais necessidades; As pedras preciosas que encontramos em nosso caminho e; Mudanças percebidas em relação a si mesmo refletindo no convívio familiar. Os resultados revelaram que o grupo de artesanato representa uma saída da rotina, uma busca por momentos de prazer e uma ruptura com a tensão dos problemas cotidianos e se coloca em contraposição ao contexto em que as participantes vivenciam, muitas vezes, desgastante, extenuante e estressante, sendo uma forma de usufruir momentos de expressão de criatividade, de gozo, de ocupação de espaço e tempos, de distrair, de rir, sendo momentos de cuidado de si, na busca por uma melhor qualidade de vida, que refletem diretamente na manutenção da saúde mental. O resgate da autoestima e da autoconfiança foi essencial para que algumas mulheres pudessem viver a vida novamente, já que estavam com suas identidades perdidas, devido às situações inesperadas que estavam vivenciando no momento que começaram a participar do grupo. Por meio da convivência em grupo, estas mulheres perceberam algumas mudanças de comportamentos que refletiram em seu modo de pensar, agir e reagir diante das situações corriqueiras do dia-a-dia e também fez com que elas refletissem sobre o respeito que cada um deve ter em relação à individualidade do outro para que pudessem conviver em família de forma harmoniosa e saudável. As percepções das participantes sobre os encontros grupais também expressaram uma melhoria da qualidade das relações familiares que resultou em uma maior aproximação, união e melhora da comunicação entre os membros da família, e isso também foi percebido pelos familiares que continuaram dando apoio para que elas continuassem a participar do mesmo. A troca de experiências de vida de cada uma destas mulheres foi um fator muito presente que possibilitou a aquisição de conhecimentos e informações essenciais de acordo com a necessidade de cada participante para que alguns comportamentos fossem adquiridos ou mudados. Diante disso, consideramos que o grupo de artesanato não vem sendo apenas um passatempo como alguns acreditam ser, mas tem muita importância para seus participantes, possibilitando-lhes uma transformação por meio da convivência com outras pessoas, dando, assim, um novo sentido para vida daquela mulher que antes se sentia inundada em suas angústias, tristezas, solidão e sem muitas expectativas de soluções para o enfrentamento de seus padeceres. Nesse sentido, dentro da visão de promoção da saúde mental, o grupo de artesanato pode ser considerado um dispositivo que vem contribuindo no encontro de condições favoráveis e saudáveis para a melhoria da qualidade de vida e, conseqüentemente, possibilitando a manutenção e o reequílibrio da saúde mental destas mulheres. Julgamos ser relevante agregar novas formas de cuidar em saúde mental, de forma a acolher as pessoas que estão passando por algum sofrimento e que precisam ser cuidadas antes de adoecerem, o que pôde ser constatado nos depoimentos como um dos motivos que levaram algumas mulheres a buscar o grupo de artesanato como um modo de aliviar as suas angústias e suas tristezas cotidianas. Por fim, consideramos que realizar atividades por meio de um espaço grupal é uma importante estratégia a ser utilizada pelos profissionais de saúde, em particular o enfermeiro, para o desenvolvimento de ações de promoção da saúde mental na atenção básica, pois com este estudo foi possível perceber o quanto o espaço grupal influenciou de forma positiva na vida dessas pessoas, interferindo diretamente na relação consigo mesma e na relação com outras pessoas significativas em suas vidas, e isso refletiu diretamente na manutenção da saúde mental. 115 f

Neoplasms are one of the main causes of morbidity and mortality in world population and among women breast cancer is one of that generates more concerns. The statistics estimates by year more than 1.050.000 new cases of breast cancer all over the world and the projection to 2009 in Brazil estimates 49.400 thousand new cases (50,71/100.000). The aim of this study was to make an analysis of the secondary prevention actions of breast cancer and related factors among women aged between 40 and 69 years old in the city of Maringá, Paraná State, Brazil. It was made an analytical, exploratory and transversal study as population investigation in domiciles in the city of Maringá, Paraná State, Brazil. The sample was composed by 429 women aged between 40 and 69 years old. The sampling stratified and randomized was distributed in four strata, representing geographically the position of the incidence. From each stratus was selected a simple and randomized sample proportional to the number of women who were living in each stratus. The data were collected by domiciliary interviews using a structured and previously tested questionnaire. The data were analyzed by using the SPSS software. It was made a bivariate analysis and a multiple analysis. The project was approved by the Ethics Committee at UEM. The studied population was constituted by 439 women aged between 40 and 69 years, 45,8% among 40 to 50 years, 29,6% between 51 and 60 and 24,6% between 61 and 69 years old. The predominant ethnical group was Caucasian and the most prevalent economical status was classified as C. About 33,9% made only elementary school and only 7,7% of women made full higher education studies. The majority (76,1%) of them related no use of alcoholic drinks and only 13% of women referred to smoke habits. About 62% already used contraception pills. The majority of women interviewed were classified as overweighed; the women who experienced menopause were averaged in 47 years. Only 6,1% presented breast cancer history, 69,2% never made use of hormone reposition therapy. The prevalence of routine habits to make the BSE (Breast Self-exam) of 64,5% and BCE (Breast Clinical Exam) in the last gynecological visit was of 71,5%. The prevalence of mammography revealed 79% of women who made it. The related factors to the BSE making was the social status, school background, age, menarche and the treatment of menopause. The associated variables to make BSE were: social status, scholarship, age of menarche and treatment of menopause. The following factors as age, ethnic condition, economical status, religion, age of menarche, age of menopause, number of pregnancies and treatment of menopause were associated to the mammography routine habit. The association between social and economical level and the conduct preventive habits is directly proportional that means, higher the social and economical level will be higher the number of visits made and consequently more exams requested and greater the chances to the early diagnostic. It was attested an imbalance in health attention and a lack of integrality in women assistance. This deficiency in Health System may result an inequity related to the prevention actions of breast cancer, reducing the chances of an early diagnostic and an effective treatment, consequently increasing the number of deaths by breast cancer. As neoplasias destacam-se como uma das principais causas de morbidade e mortalidade na população mundial e entre as mulheres destaca-se como preocupação premente o câncer de mama. Estima-se que por ano ocorram mais de 1.050.000 casos novos de câncer de mama em todo o mundo e a estimativa para 2009 no Brasil refere que ocorrerão 49.400 mil casos novos de câncer de mama (50,71/100.000). O objetivo deste estudo foi analisar a realização de ações de prevenção secundária do câncer de mama e fatores associados entre mulheres de 40 a 69 anos do município de Maringá-PR. Foi realizado um estudo analítico, exploratório de corte transversal tipo inquérito populacional domiciliar no município de Maringá, PR. A amostra constitui-se de 429 mulheres na faixa etária de 40 a 69 anos. A amostragem foi aleatória estratificada onde todos os bairros, jardins e zonas foram geograficamente colocados em cinco estratos. De cada estrato foi selecionada uma amostra aleatória simples proporcional ao número de mulheres residentes em cada um desses estratos. Os dados foram coletados por entrevistas domiciliares, com questionário estruturado e pré-testado. Os dados foram analisados por meio do programa SPSS. Foi realizada a análise bivariada e a análise múltipla. O projeto foi aprovado pelo Comitê de Ética da UEM. A população estudada constitui-se de 439 mulheres entre 40 e 69 anos, 45,8% na faixa etária de 40 a 50 anos, 29,6% entre 51 e 60 e 24,6% entre 61 e 69. A raça predominante foi a branca e a classe econômica mais prevalente foi classificada como C. 33,9% possuíam somente o primário completo e somente 7,7% das mulheres possuíam superior completo. A maioria (76,1%) relataram nunca terem feito uso de bebida alcoólica e apenas 13% das mulheres referiram fazer uso de cigarro. 62% já fizeram uso de contraceptivo oral. A maior parte das mulheres entrevistadas foram classificadas como sobrepeso. A média de idade em que as mulheres entraram na menopausa foi de 47 anos. Somente 6,1% possuíam histórico de câncer de mama. 69,2% nunca fizeram uso de terapia de reposição hormonal. A prevalência do hábito de realizar o AEM encontrada foi de 64,5% e do ECM na última consulta ginecológica foi de 71,5%. Quanto à prevalência de mamografia 79% relataram já terem realizado esse exame. Os fatores associados à realização do AEM foi a classe social, escolaridade, idade da menarca e realização de tratamento para menopausa. As variáveis associadas à realização do ECM são: classe econômica, escolaridade, idade da menarca e realização de tratamento para menopausa. Os fatores faixa etária, raça/cor, classe econômica, religião, idade da menarca, idade da menopausa, número de gestações e realização de tratamento para menopausa são fatores que estão associados à realização da mamografia. A associação entre o nível sócio-econômico e a realização de condutas preventivas está diretamente proporcional, ou seja, quanto maior o nível sócio-econômico maior o número de consultas realizadas e conseqüentemente mais exames realizados e oportunidade de diagnóstico precoce. Percebe-se uma desigualdade na atenção á saúde e falta de integralidade na assistência à mulher. Essa deficiência no sistema de saúde pode resultar numa iniqüidade em relação às ações de prevenção do câncer de mama, diminuir as chances de um diagnóstico precoce e de um tratamento eficaz e conseqüentemente aumentar o número de mortes por câncer de mama. 118 f

The dimensioning of the nursing staff in the Intensive Care Unit (ICU) aims at providing favorable working conditions for taking care of seriously ill patients, with lower risks for the staff's and their patients' health. In order to have an adequate dimensioning in the ICU it is also necessary to quantify the workload of nursing so as the several particular activities in charge of this sector may be considered. In doing so, the Nursing Activities Score (NAS) has been developed and it is available for use in the Brazilian context. Furthermore, concerning the nursing workload measure it is necessary to consider the minimum criteria established by the COFEN Resolution # 293/2004, such as the Technical Security Rate (TSR) and the proportion of nurses over the totality of the nursing staff. As a result of noticing some workload of the nursing team in a ICU for adults (ICU-A) of a School Hospital of the Northwest of Paraná, this research study was carried out in an effort to analyze the dimensioning of the nursing staff of the ICU-A, according to the nursing workload obtained by the NAS instrument and the criteria established in the COFEN Resolution # 293/2004. It is a descriptive exploratory study which consisted of 107 patients/clients who were placed in the ICU-A for at least 24 hours between November 13, 2007 and May 15, 2008. The data was daily collected by filling in an instrument consisting of three parts: Data of the Patient and of the Submit, NAS and, the Proportion of the Nursing Professionals in Relation to the Number of Patients. Microsoft Office Excel 2003 and Statistics 8.0 were used in order to manage and analyze the data. This research is registered upon the Written Opinion # 412/2007 from the Ethical Permanent Committee of Research with Human Beings (EPCHB) of the State University of Maringá. Out of the results the score of NAS was obtained for all the patients and on the day it varied from 479.7 to 1007.2 scores, with an average of 697.3 scores (DP ± 83.5) and a median value of 687.0 scores. Basing on the average of staff estimated by NAS (n=7) and following the COFEN Resolution # 293/2004, it was evidenced that the staff team of the ICU-A should have 40 nursing professionals, from which 21 (52.5%) should be nurses divided into five work teams. It was verified that there is a deficit of 12 (30%) workers and the proportion of nurses (35.7%) is below the expected number. Finally, it showed as evidence that there is a lack of nursing professionals in the ICU-A, particularly, due to the fact that in this sector, complex activities are done and/or they are not considered in the traditional dimensioning methods. As to solve or reduce this problem, the daily use of NAS in a predictive way is recommended in order to adequate the work demand and the number of nursing professionals. O dimensionamento de pessoal de enfermagem em uma unidade de terapia intensiva (UTI) visa garantir condições de trabalho favoráveis para o cuidado aos pacientes gravemente enfermos, com menor risco à saúde dos trabalhadores e da clientela por eles assistida. Para um dimensionamento adequado em UTI é preciso também quantificar a carga de trabalho de enfermagem, de modo que sejam consideradas as diversas atividades específicas desse setor. Para tanto, foi desenvolvido o Nursing Activities Score (NAS), que já se encontra validado para ser utilizado no contexto brasileiro. Acresce que, quanto à medida da carga de trabalho de enfermagem, é necessário considerar os critérios mínimos estabelecidos pela Resolução COFEN n.° 293/2004, tais como o índice de segurança técnica (IST) e a proporção de enfermeiros sobre o total de trabalhadores de enfermagem. O presente trabalho foi realizado em face da sobrecarga de trabalho incidente sobre a equipe de enfermagem percebida numa UTI para adultos (UTI-A) de um hospital-ensino do Noroeste do Paraná, e teve como objetivo principal analisar o dimensionamento de pessoal de enfermagem da UTI-A de acordo com a carga de trabalho de enfermagem obtida pelo uso do instrumento NAS e os critérios estabelecidos na Resolução COFEN n.° 293/2004. Constitui-se de um estudo descritivo-exploratório, realizado com 107 pacientes/clientes que permaneceram internados na UTI-A por no mínimo 24 horas, entre 13 de novembro de 2007 e 15 de maio de 2008. Os dados foram coletados diariamente, preenchendo-se um instrumento composto de três partes: dados do paciente e de internação, NAS e proporção de profissionais de enfermagem em relação ao número de pacientes. Foram utilizados os programas Microsoft Office Excel 2003 e Statistica 8.0 para tratamento e análise dos dados. A realização da pesquisa foi autorizada pelo Comitê Permanente de Ética em Pesquisas com Seres Humanos (COPEP) da Universidade Estadual de Maringá, mediante o Parecer n.° 412/2007. Entre os resultados obteve-se que a pontuação do NAS de todos os pacientes no dia variou de 479,7 a 1007,2 pontos, com uma média de 697,3 pontos (DP ± 83,5) e mediana de 687,0 pontos. Com base na média de trabalhadores estimada pelo NAS (n=7) e estabelecida pela Resolução COFEN n.° 293/2004, obteve-se que o quadro funcional da UTI-A deveria ser composto de 40 profissionais de enfermagem, dos quais 21 (52,5%) deveriam ser enfermeiros, distribuídos em cinco equipes de trabalho. Verificou-se que há déficit de 12 (30%) trabalhadores e a proporção de enfermeiros (35,7%) está aquém do recomendado. Conclui-se que há falta de profissionais de enfermagem na UTI-A, devida, principalmente, ao fato de que neste setor são desenvolvidas atividades complexas e/ou que não são computadas nos métodos de dimensionamento tradicionais. Para solucionar ou minimizar esse problema sugere-se o uso do NAS diariamente de forma preditiva, com vistas à adequação entre demanda de trabalho e número de profissionais. 78 f
