2. Determinants of Corporate Indebtedness in Portugal: An Analysis of Financial Behaviour Clusters

Corporate indebtedness is a powerful tool in determining a company’s financial health with impacts on its image and reputation. The main objective of this research is to study the determining factors in corporate indebtedness in Portugal. It also has the secondary objectives of creating clusters of companies’ behaviour in relation to the use of credit and verifying their differences in relation to the characteristics of the companies. It uses a quantitative methodology based on a questionnaire survey of 1957 Portuguese companies. The results of the factor analysis show the formation of six determining factors in corporate indebtedness, namely the negotiating relationship with banks, financing, cycle and indebtedness, company operating performance, guarantees used to obtain bank financing and financing risk analysis as well as secondary forms of bank financing. The application of cluster analysis to the six factors formed led to the classification of companies into three clusters: the resilient financial cluster, the operational excellence cluster and the strategic financial cluster. There are several statistically significant differences in the corporate financing factors in relation to the clusters to which they belong. The evidence of the factors and clusters explaining company financing provides insights for improving credit access practices and for implementing public policies that facilitate access to credit and promote economic development.

Ischaemic stroke (IS) and venous thromboembolism (VTE) are two forms of thromboembolism that, although distinct, seem to share numerous risk factors. Concerning genetic risk factors, while many VTE genetic markers have been reported, inclusively by genome-wide association studies (GWAS), the identification and validation of genetic determinants underlying IS pathogenesis have been challenging. Considering that IS and VTE shared biological pathways and aetiological factors, the severity of IS might be also influenced by VTE-related genetic variants. Thus, the present study was designed to analyse the impact of six VTE GWAS-identified genetic variants on the clinical outcome of 363 acute IS patients. Results revealed that the single-nucleotide polymorphism (SNP) F11 rs4253417 was an independent predictor of the 5-year risk of death among patients with total anterior circulation infarct (TACI). Namely, the ones carrying the SNP C allele presented a fourfold increase in the 5-year risk of death compared to TT genotype carriers (CC/CT vs. TT; adjusted HR, 4.240; 95% CI, 1.260–14.270; P = 0.020). This SNP is known to be associated with coagulation factor XI (FXI) levels, thus with implications in haemostasis and inflammation. As such, F11 rs4253417 might be a promising prognostic biomarker among TACI patients to aid in clinical decision-making. However, additional investigation is required to confirm the study's results and dissect the underlying mechanisms.

5. Rehabilitation after forearm/hand replantation

Currently, the challenge of replantation of the upper limb after amputation has become an achievable and meticulously perfected reality in recent decades and is in constant evolution. The surgical option for replantation should take into account not only a comprehensive analysis of the viability of the replantation but fundamentally its potential for long-term functional recovery. We present the clinical case of a 40-year-old man, victim of a work accident, with cut trauma, which resulted in distal amputation of his right forearm. The warm ischemia time was 5 h, having been submitted to joint replantation surgery. Following the surgery, physical medicine and rehabilitation (PMR) was referred early, carrying out a sequential functional rehabilitation program. This work aims to emphasize the importance of the role of PMR in an early, careful, and extensive rehabilitation program, a fundamental factor in the functional recovery and long-term prognosis of these injuries and prevention of complications.

6. Focal Periphyseal Oedema: A Physiologic Finding in Paediatric Knee Pain

Knee pain is a common reason for adolescent calls or visits to a primary care clinician. The authors present a case of an 14-year-old male adolescent with progressive bilateral nociceptive somatic knee pain. The pain was worse with weight bearing, and relieved at rest. It was located over the medial joint line. The patient was treated conservatively with nonsteroidal anti-inflammatory drugs and a rehabilitation program. Magnetic Resonance Imaging (MRI) showed bone marrow oedema, and Focal Periphyseal Oedema (FOPE) diagnosis was made. After a total of 6 months of treatment, the patient was asymptomatic. The literature is limited to a few case reports and radiologic studies describing this symptomatic physeal pathology, not reporting physical examination or treatment. The goals of this case report were to elucidate physicians of this condition, a newly reported cause of knee pain in adolescent, and to elicit the importance of a patient-tailored rehabilitation program in the treatment of this finding. FOPE is a clinical entity that should not be ignored and must be treated for the patient’s comfort. It is a self-limited condition and has an excellent prognosis.

7. A língua gestual e o ensino de surdos: uma reflexão sobre as práticas bilingues

O desenvolvimento cognitivo, linguístico, comunicacional, social e cultural das crianças surdas depende da aquisição precoce de uma língua, preferencialmente a Língua Gestual Portuguesa (LGP). Se estas crianças forem expostas a esta língua de modalidade visuo-gestual, desde cedo, a sua aquisição e o seu domínio ocorrerão de forma natural, o que facilitará todas as suas futuras aprendizagens. Contudo, a maioria das crianças surdas é filha de pais ouvintes, não tendo, por isso, acesso à LGP, o que as pode conduzir ao fraco rendimento escolar. Através de uma revisão crítica da literatura e das respostas atualmente implementadas em Portugal, pretendemos discutir a aquisição precoce da LGP e os papéis desempenhados pelos serviços de intervenção precoce, pela família e pelas Escolas de Referência para a Educação Bilingue (EREBAS), evidenciando o seu contexto específico e complexo que requer equipas e práticas especializadas e a necessidade de atuação junto das crianças surdas e das suas famílias para que o educando possa alcançar um desenvolvimento integral. As grandes conclusões que resultam da reflexão crítica abordam a necessidade de integração da família do jovem surdo na comunidade surda, a aprendizagem da Língua Gestual, o acompanhamento da família pelas equipas de intervenção precoce e a reflexão sobre a formação de profissionais intervenientes na educação de surdos em Portugal.

O ensino de História no ambiente escolar é obrigatório, sua matéria é obrigatória nos currículos brasileiros, os professores com a evolução da sociedade e o posicionamento dos alunos, adotaram tecnologias que proporcionaram para os alunos uma maior compreensão do caminho da história no Brasil. A metodologia adotada no presente artigo foi pesquisa bibliográfica, que adotou as ideias dos autores como: Teruya e Moraes (2009); Mercado (2002); Lévy (1999); Demo (1998); Libâneo (1998); Moysés (1995); Nóvoa (2004, 2007) e documentos legislativos como os PCN e a LDB. O objetivo geral é propor para os educadores meios tecnológicos capazes de facilitar a melhor explicação da história brasileira e como objetivo específico é preciso compreender como esse ensinamento seria ministrado e observar como os professores irão elaborar e avaliar os alunos a partir das tecnologias. Como resultado foi mostrado a eficiência da tecnologia para explicar a história para os alunos. Sendo assim, a tecnologia vem como uma articulação que os professores adequaram no método de ensino.

Os anticoagulantes orais não antagonistas da vitamina K (NOACs) surgiram como uma alternativa terapêutica aos antagonistas da vitamina K (VKAs). Os NOACs vieram revolucionar a anticoagulação oral, estando indicados para a prevenção de fenómenos tromboembólicos em doentes com fibrilhação auricular não valvular e para a prevenção primária e secundária do tromboembolismo venoso, nomeadamente tromboses venosas profundas e embolia pulmonar. Dada a crescente utilização destes anticoagulantes, é importante um aumento do conhecimento acerca dos mesmos, nomeadamente das suas indicações clínicas, precauções especiais de uso, as suas características farmacocinéticas e farmacodinâmicas, características posológicas e formas de monitorização. Neste sentido, a Associação Portuguesa de Farmacêuticos Hospitalares (APFH) reuniu-se para discutir aspetos críticos desta matéria e, com base na atual evidência científica disponível, adotar uma posição consensual acerca da visão da APFH sobre os aspetos chave e atualizações sobre a coagulação. Os resultados e conclusões do trabalho focaram-se nas razões da escolha dos diferentes anticoagulantes (VKAs e NOACs), e, concretamente em relação aos NOACs, nas necessidades de monitorização, em considerações acerca de populações de risco, tempos de semivida, adesão e falhas na toma dos medicamentos, interações farmacológicas mais relevantes, contraindicações e precauções, antídotos disponíveis, mecanismos de avaliação da anticoagulação e switch entre os vários anticoagulantes.

Nowadays, companies are implementing technologies that enable them to guarantee service efficiency and consequent cost savings. In addition, auditors play a key role in assessing processes that are being restructured due to technological advances, so that business continuity is not compromised. The main objective of this dissertation is the development of an academic work related to the area of business continuity and the components of intelligent process automation. The application component supports the end-to-end audit process by automating the activities of audit communication, information requests, and final report submission. The purpose of the implementation is to enable the assessment performed by the auditors to be devoid of repetitive activities and optimized by the automation application. This development is complemented by the application of business intelligence in the treatment of key performance indicators of the audit department, to ensure continuous analysis of the effectiveness of the Annual Audit Plan. To evaluate the work done, surveys were conducted with audit teams from 4 Portuguese banks. The results showed that the implementation would be beneficial in companies and would allow supporting management in the decision-making context, as well as eliminating repetitive tasks that are assigned to auditors during the audit process. Nos dias de hoje, as empresas encontram-se a implementar tecnologias que lhes permitam garantir eficácia dos serviços e uma consequente redução de custos. Complementarmente, os auditores desempenham um papel fundamental no que concerne à avaliação dos processos que se encontram a ser reestruturados devido aos avanços tecnológicos, para garantir que a continuidade de negócio não é comprometida. Esta dissertação tem como principal objetivo o desenvolvimento de um trabalho académico relacionado com a área de continuidade de negócio e as componentes de automação inteligente de processos. A componente aplicacional permite dar auxílio ao processo end-to-end de auditoria no que respeita à automatização das atividades de comunicação de auditoria, pedidos de informação e envio de relatório final. O objetivo da implementação é permitir que a avaliação realizada pelos auditores aos processos é desprovida de atividades repetitivas e otimizada pela aplicação de automação. Este desenvolvimento é complementado com a aplicação de inteligência empresarial no tratamento de indicadores de performance do departamento de auditoria, para garantir uma análise contínua da efetividade do Plano Anual de Auditoria. Para avaliar o trabalho desenvolvido, foram realizados inquéritos a equipas de auditoria de 4 bancos portugueses. Os resultados demonstraram que a implementação seria benéfica em contexto empresarial e permitiria apoiar a gestão no contexto de tomada de decisão, assim como eliminaria tarefas repetitivas que se encontram atribuídas aos auditores durante o processo de auditoria.

Physical exercise is generally recommended. Specific populations have particular needs, but physical exercise is also highly recommended for people with motor, sensory or cognitive disabilities. There are currently adaptive sports, including winter sports. Participation in adaptive sports improves cardiovascular capacity, contributes to integration in the community and decreases pain intensity, leading to improved quality of life. Alpine skiing, Nordic skiing and ice hockey are the most popular paralympic winter sports. The medical teams working with Paralympic winter sports athletes face several challenges such as the need to tailor the adaptative material to the athlete, prevent acute traumatic injuries and treat chronic diseases.

PurposeErrors are inevitable, resulting from the human condition itself, system failures and the interaction of both. It is essential to know how to deal with their occurrence, managing them. However, the negative tone associated with them makes it difficult for most organizations to talk about mistakes clearly and transparently, for fear of being harmed, preventing their detection, treatment and recovery. Consequently, errors are not managed, remaining accumulated in the system, turning into successive failures. Organizations need to recognize the inevitability of errors, making the system robust, through leadership and an organizational culture of error management. This study aims to understand the role of these influencing variables in an error management approach.Design/methodology/approachIn this paper methodology of a quantitative nature based on a questionnaire survey that analyses error management, leadership and the organizational culture of error management of 380 workers in Portuguese companies.FindingsThe results demonstrate that leadership directly influences error management and indirectly through the organizational culture of error management, giving this last variable a mediating role.Originality/valueThe study covers companies from different sectors of activity on a topic that is little explored in Portugal, but part of the daily life of organizations, which should deserve greater attention from directors and managers, as they assume a privileged position to promote and develop error management mechanisms. Error management must be the daily work of leaders. This study contributes to theoretical knowledge and business practice on error management.

Acquired Hemophilia A (AHA) is a rare autoimmune disorder, caused by the development of circulating autoantibodies against coagulation factor VIII (FVIII). AHA is associated with bullous pemphigoid in 2% of patients. We report a case of a 74-year-old man admitted with anemia and a tense subcutaneous and muscular hematoma in the right thigh. Blood analysis confirmed AHA. The patient had a recent diagnosis of bullous pemphigoid. Response to bypass agents and corticosteroids was good with bleeding control and normalization of FVIII and negative autoantibodies, respectively. In a 3-month follow-up period after tapering and stopping prednisolone, a relapse occurred, and immunosuppression was reinitiated. An early diagnosis and effective treatment in AHA are essential to reduce morbimortality. A careful tapering of immunosuppression is important to minimize FVIII inhibitor recurrence, as observed in this case.

The surgical option for replantation should take into account not only a comprehensive analysis of the viability of the replantation but fundamentally its potential for long-term functional recovery. We present the clinical case of a 40-year-old man, victim of a work accident, with cut trauma, which resulted in distal amputation of his right forearm. The warm ischemia time was 5 h, having been submitted to joint replantation surgery. Following the surgery, physical medicine and rehabilitation (PMR) was referred early, carrying out a sequential functional rehabilitation program. This work aims to emphasize the importance of the role of PMR in an early, careful, and extensive rehabilitation program, a fundamental factor in the functional recovery and long-term prognosis of these injuries and prevention of complications.

Femoral neck fractures are a major source of disability in the elderly. Rehabilitation is fundamental to recover pre-fracture functionality. We conducted an observational cohort study with the aim of comparing the efficacy of rehabilitation programs in different therapeutic settings. We included elderly patients who had undergone surgical stabilization of a hip fracture. The participants were divided into 3 groups: group 1, outpatient rehabilitation, group 2, inpatient rehabilitation, group 3, home-based rehabilitation. Patients were evaluated at baseline, at three months, and at six months after fracture. Our outcome measures were the Barthel Index (BI), Functional Ambulation Categories, passive and active range of motion of hip flexion and abduction, and muscle strength in hip flexion, abduction, and knee extension. At six months, all three groups showed an average statistically significant improvement (p <, 0.05) in all outcome measures compared to the baseline. Considering the between-group analysis, final BI was significantly higher in outpatient than inpatient-treated patients (p = 0.018), but no statistical difference was found between outpatient and home-based patients. Our findings suggest that rehabilitation leads to significant functional recovery after hip fracture in elderly patients. Both outpatient and home-based rehabilitation seem to be reasonable options for hip fracture rehabilitation.

Atrial fibrillation (AF) and stroke are included in a group of complex traits that have been approached regarding of their study by susceptibility genetic determinants. Since 2007, several genome-wide association studies (GWAS) aiming to identify genetic variants modulating AF risk have been conducted. Thus, 11 GWAS have identified 26 SNPs (p -2), of which 19 reached genome-wide significance (p -8). From those variants, seven were also associated with cardioembolic stroke and three reached genome-wide significance in stroke GWAS. These associations may shed a light on putative shared etiologic mechanisms between AF and cardioembolic stroke. Additionally, some of these identified variants have been incorporated in genetic risk scores in order to elucidate new approaches of stroke prediction, prevention and treatment.

A Pancoast tumor is a rare condition, representing 3% to 5% of all lung cancers. The particular location of these lesions leads to the invasion of structures in the thoracic inlet, causing a constellation of symptoms known as Pancoast-Tobias syndrome. Diagnosis can be challenging due to their low prevalence and the possibility of being asymptomatic. Most of these tumors are non-small cell lung cancers. However, rare conditions might arise at the same location, and histologic confirmation is relevant. We report the case of a 45-year-old man admitted to the internal medicine department with a one-month history of night sweats. A full-body computed tomography (CT) scan revealed a mass on the upper lobe of the left lung, with soft tissue invasion. Histopathologic examination revealed an adenocarcinoma pattern originating from the colon. Colonoscopy showed two synchronous lesions. Hitherto, this is the second case ever described of a Pancoast tumor as metastasis of colon adenocarcinoma.

Despite the clinical benefits of aspirin, the interindividual variation in response to this antiplatelet drug is considerable. The manifestation of aspirin resistance (AR) is frequently observed, although this complex process remains poorly understood. While AR etiology is likely to be multifactorial, genetic factors appear to be preponderant. According to several genetic association studies, both genome-wide and candidate gene studies, numerous SNPs in cyclooxygenase, thromboxane and platelet receptors-related genes have been identified as capable of negatively affecting aspirin action. Thus, it is essential to understand the clinical relevance of AR-related SNPs as potential predictive and prognostic biomarkers as they may be essential to defining the AR phenotype.

Dissertação de mestrado em Educação Especial (área de especialização em Intervenção Precoce), Este estudo tem por finalidade conhecer e compreender as perspetivas de pais acompanhados num Grupo de Apoio a Pais de uma Equipa Local de Intervenção Precoce da Zona Norte de Portugal. Para a compreensão do fenómeno em estudo foi utilizada uma metodologia de natureza qualitativa, com recurso à entrevista semiestruturada, como instrumento de recolha de dados. Participaram neste estudo 7 pais (5 mães e 2 pais) que recebem apoio numa ELI da Zona Norte de Portugal e que se encontram a participar num Grupo de Apoio a Pais. Globalmente, as perspetivas dos participantes permitem concluir que, no momento em que decidiram participar no Grupo de Apoio a Pais, tinham expetativas de encontrar pais numa situação semelhante e partilhar as experiências entre si. Aquando do seu envolvimento, neste Grupo, todos apresentavam algum tipo de preocupação com os filhos, no que às áreas do desenvolvimento diz respeito, nomeadamente, a fala e linguagem, comportamento e preocupações futuras, como a transição para o primeiro ciclo. Relativamente aos benefícios da participação no Grupo de Apoio a Pais, a maioria dos participantes realçou o facto de a mesma permitir um maior empoderamento parental, através do aumento de informação, segurança, autoestima e redução dos níveis de ansiedade. Os participantes identificaram algumas barreiras à sua participação neste Grupo, muito conexas com a dinâmica de funcionamento do grupo, nomeadamente, o horário, a falta de à vontade para falar em grupo, entre outras. Como recomendações futuras, vários participantes reportaram-se à necessidade de uma maior flexibilidade, no dia e horário das sessões. Foi igualmente sugerida a possibilidade de criação de grupos de apoio para casais, para pais e filhos, e um outro para pais de jovens que se encontram no período da adolescência. Uma maior divulgação do projeto Grupo de Apoio a Pais, sobre “O que é?” e “Para que serve?”, tal como a necessidade de uma maior consciencialização sobre o papel de ELI foram consideradas igualmente, como recomendações, para o sucesso de uma maior adesão dos pais ao Grupo de Apoio. Considera-se que este estudo pode ser relevante para que as Equipas Locais de Intervenção Precoce possam refletir sobre a importância de Grupos de Apoio a Pais, dando assim resposta às preocupações das famílias e em simultâneo diminuir a escassez destes grupos em Portugal., This study aims at getting to know and understand the perspective of parents that took part in a Parents Supporting Group of a Local Team of Early Intervention in the Northern Area of Portugal. For the understanding of the phenomenon being studied a qualitative methodology was used, resorting to the semi-structured interview, as a data collection tool. 7 parents (5 mothers and 2 fathers) participated in this study and they get support at an LTI in the Northern Area of Portugal and are taking part in in a Parents Support Group. Worldwide, the participants’ perspectives allow us to conclude that, the moment they decided to participate in the Parents Support Group, they expected to find parents in a similar situation and to share their experiences with one another. In the moment of their participation, in this Group, everyone showed some kind of concern towards their children, regarding the development areas, namely, speech and language, behaviour and future concerns, such as the transition into primary school. In what comes to the benefits of taking part in the Parents Support Group, most participants enhanced the fact that it allows a greater parental empowerment, by increasing the amount of information, safety, self-esteem and by decreasing the anxiety levels. The participants identified some barriers to their participation in this Group, significantly connected to the group’s operating dynamics, namely, the schedule, not feeling at ease talking in a group, and so on. 1As future recommendations, several participants reported to the need for more flexibility, of day and schedule for the sessions. A possibility for creating supporting groups for couples, parents and children, and another one for parents of teenagers that are going through adolescence was also suggested. The spreading of the Parents Support Group project, about “What is it?” and “What is it for?”, as well as the need for more awareness on the role of LTI were also considered, as recommendations, for the success of a greater participation by parents in the Support Group. It is considered that this study may be relevant in order for the Local Teams of Early Intervention to be able to reflect upon the significance of Parents Support Groups, thus responding to the concerns of families and simultaneously lessening the scarcity of these groups in Portugal.

In Western European countries, acute ischemic stroke (AIS) remains the third leading cause of death. Among the risk factors for cerebrovascular disease, some have more influence than others in certain stroke subtypes. The aim of this study was to evaluate the impact of risk factors among Stroke Subtypes on the clinical outcome of Portuguese patients under previous aspirin therapy.We studied a cohort of 371 patients diagnosed with AIS and a clinical follow-up protocol was set up.The patients were admitted in a Department of Internal Medicine of a major hospital. Standardized data assessment and stroke subtype classification (Oxfordshire Community Stroke Project) were used.Arterial hypertension (80.4 %), overweight (72.6 %) and dyslipidemia (62.0 %) were the most prevalent risk factors with no statistical differences among the group's subtypes. Current smoking was more prevalent in POCI(62.9 %) with differences among subtypes (p = 0.002). Atrial fibrillation was more commonly reported in TACI (39.3 %) and less common in POCI (8.1 %) (p 0.001).Comparing TACI vs Non TACI Stroke Subtypes demonstrated major differences in cumulative survival,among the cases with no previous aspirin treatment, after 3 years (51.9 % vs 88.8 %).The increased risk of mortality at 12 months is consistently observed for the presence of a previous atrial fibrillation (OR 3.01 95 %CI 1.69-5.39), TACI subtype (OR 10.4 95 %CI 4.83-22.6) and NIHSS over 10 (OR 9.33 95 % CI 4,49-19.4). When we analyze the impact of previous aspirin treatment in the risk for a new stroke event, it seems to have a protective effect in a time frame of 12 months, but this protection is lost extending at 24 months (p = 0.094 vs p = 0.005).Our results indicate that smoking, atrial fibrillation and age have different relevance in their distribution among ischemic stroke subtypes at the time of diagnosis. Concerning the influence of the main stroke risk factors on the clinical outcome, our results present a strong influence of atrial fibrillation and of age. Severity of disease at diagnosis, represented by TACI subtype is clearly associated to decreased survival among patients with no record of previous aspirin therapy. Our results reinforce the relevance cohort studies of different populations, to achieve a more comprehensive knowledge of the impact of risk factors on stroke subtypes and on its clinical outcome.

Este artigo tem por objetivo apresentar uma ação que tem como fundamento o carisma da unidade, e que, ao desenvolver suas atividades numa região precisa, como será apresentado – no coração da Amazônia brasileira, no município de Óbidos – dá a perceber a atualidade do carisma onde quer que o carisma chegue – quer seja numa metrópole, quer seja no coração de uma floresta. O texto deseja enfatizar a atualidade e a eficácia do carisma em comunidades na floresta, comunidades à beira de lagos, e à beira do grande rio, e os frutos que esse mesmo carisma gera ao tocante à evangelização, à irradiação espiritual e às necessidades na sociedade civil, porque é um serviço à população e, sobretudo, à população que precisa. Também quer mostrar as relações de afinidades do carisma da unidade com a tradição da Doutrina Social da Igreja que ganha seu ápice no Concílio Vaticano II. Esta tradição está na base da teologia e na espiritualidade do papa Francisco que ganha concretude em seu pontificado e em seus documentos e dialoga com a teologia da criação de John Wesley, revelando, assim, uma perspectiva ecumênica já presente nas respectivas tradições aqui representadas. Por fim, o conjunto deste artigo move-se no âmbito de uma teologia narrativa.

Dissertação de Mestrado apresentada no ISPA - Instituto Universitário, na especialidade de Psicologia Clínica. O presente estudo tem como intuito aprofundar o conhecimento sobre a vivência migratória e expressão identitária de refugiados da Guerra Civil na Síria. Este movimento migratório recente merece um olhar mais atento e profundo. Deste modo, a pertinência desta investigação prende-se à escassez de trabalhos realizados numa perspetiva vivencial da experiência migratória dos refugiados. Para proporcionar uma compreensão mais completa e profunda este trabalho baseia-se numa lógica etnopsicanalítica. Esta investigação inscreve-se numa metodologia qualitativa narrativa. Deste modo, foi aplicado o instrumento FANI (Hollway & Jefferson, 2000; 2008), pois possibilita o acesso à vivência subjetiva dos entrevistados. Os participantes deste estudo são um casal de refugiados palestinianos que viviam na Síria e que estão em Portugal há cerca de um ano. Na análise do material empírico foram destacados os seguintes temas para cada um dos participantes: questões identitárias; representação do país de partida e do país de chegada; e como se reportam ao mundo de onde partiram. Como principais conclusões podemos salientar a dupla precariedade experienciada por estes refugiados que dificulta a sua projeção no país de acolhimento. Deste modo, reportam-se a dois mundos precários – o lá e o cá. É possível observar como, apesar da dupla precariedade experienciada por ambos, cada um dos participantes demonstra um modo distinto de se relacionar com a nova realidade. Por sua vez, a precariedade que reportam ao mundo que deixaram para trás impõe a necessidade de preservar esse lugar. ABSTRACT: The aim of the present study is to increase the knowledge on the migration process and the identity expression of refugees from the war in Syria. This recent migration deserves a thorough and profound analysis. Thus, the relevance of this investigation derives from the lack of publications on the experiential perspective of the migration process. This work is based in an etnopsychoanalytic perspective to provide a more complete and profound understanding. This investigation follows a qualitative narrative methodology. Therefore, the FANI instrument (Hollway & Jefferson, 2000; 2008) was applied, which allows access to the subjective experience of those interviewed. Participants in this study is a couple of Palestinian refugees whom fled from Syria to Portugal, where they live in the last year. The following topics were highlighted in the empiric analysis for each participant: identity issues; representation of the country of departure and of arrival; and the way participants report to the world from where they fled. We can conclude the double precariousness felt by the couple makes it difficult for them to project in the host country. In this way, they relate to two precarious worlds – there and here. Although, they both experience the double precariousness, it is possible to observe how each one shows a distinctive way of connecting to the new reality. On the other hand, the precariousness they report to the world left behind stresses the need to preserve such place.

Addison’s disease is an endocrine disorder characterized by primary adrenal insufficiency due to various causes. Mycobacterium tuberculosis infection was a major cause in the past but is rare nowadays. We describe a patient admitted to our hospital who was diagnosed with tuberculous Addison’s disease. LEARNING POINTS Tuberculosis is a rare cause of Addison’s disease in developed countries and so diagnosis is a clinical challenge and requires a high index of suspicion. Tuberculosis should always be considered as a cause of Addison’s disease since treating active tuberculosis with steroids alone may have serious consequences. Computed tomography findings can be very helpful in the diagnosis of tuberculous Addison’s disease.

Introduction/Background Sialorrhea consists of an involuntary loss of saliva, and is considered pathological after 4YO. The aim of this study is to describe our methodology and evaluate the efficacy and safety of botulinum toxin type A (BTX-A) in sialorrhea in children and adults with cerebral palsy (CP) or cognitive deficit, referenced to PMR consultation between 2002–2017. Material and method Longitudinal descriptive study with clinical records analysis and consultation of Drooling Rate Scale (DRS) and Teacher Drool Scale (TDS) prior to BTX-A injections and 1-month after. Injection of BTX-A performed with ultrasound and sedation. Results Total of 104 patients, 86 patients selected, 32 female (37.21%), 54 male (62.79%), current ages 7–29YO (mean 18), age of the 1st application 3–22YO (mean 9.76), age of last application 4–23YO (mean 11.83). Fifty patients (58.14%) diagnosed with CP. Two hundred and three total applications (average 2.36), maximum 10 applications. Parotids (55.56%), submandibular (9.09%) or both (35.35%), mean periodicity of 11.81 months. Sedation: 49.38% midazolam, 41.36% nitrogen protoxide, 0.62% chloral-hydrate and 8.64% without. Surveys: mean pre-application-DRS of 33.98 (minimum 19, maximum 58), mean post-application-DRS of 28.46 (min 15, max 54), with P = 6.48–5, average difference 4.28 points in DRS (min −27, max 22). Mean pre-application-TDS of 4.04 (min 2, max 5), mean post-applicantion-TDS of 3.38 (min 1, max 4), with P = 2.80–5, an average difference of 0.69 points in TDS (min −1, max 3). Difference in DRS and TDS of 0.76 and 0.24 respectively in the parotids, 6.33 and 0.83 in the submandibular and 8.52 and 1.43 in both. Improvements were statistically significant. Surgery: 6 (6.98%). Side effects: 5 children (5.81%) insomnia/vomiting. No complications related to the administration technique. Conclusion The application of BTX-A in the treatment of sialorrhea is a safe, easy-to-apply technique, with statistically significant improvement in QoL and severity. Benefit in the application of BTX-A in both glands treatment is shown.

The prevalence of mental health problems among the adolescent population raises concerns around the developmental path and the outcomes for the youths with mental health needs. Especially, the high prevalence of mental health problems among juvenile offenders highlights the urgent need for reliable and efficient assessment methods, in order to better understand how these needs affect the development and behavior of those youths, and to make better use of the limited resources of the mental health and juvenile justice systems. The main goal of the present investigation is to give continuity to the validation program of Massachusetts Youth Screening Instrument (MAYSI-2) in the Portuguese population, analyzing to what extent the relations between psychopathology symptoms and psychopathic features are high in adolescent males, given the possibility that mental health syndromes may reflect or be a reflex of common or overlapping psychopathological expressions and symptoms, and result in similar behavioral outcomes. Simultaneously, we also examined to what extent brief assessment and screening tools, namely Massachusetts Youth Screening Instrument – Second Version (MAYSI-2) and Youth Psychopathic Trait Inventory (YPI), could assist in the assessment and comprehension of these interactions. We assessed a total of 76 adolescent males, distributed by a community (N=43) and a forensic sample (N=33), between ages 12 and 19, and 14 and 19, respectively. The protocol was composed by MAYSI-2, YPI and Coimbra’s Social Desirability Scale (EDSC). The analysis of the prevalence rates of self-reported mental health needs and psychopathic traits, revealed a higher prevalence of both in the detained youths, compared to youths with no history of offending. Findings also revealed a positive association between the behavioral dimensions of both measures used, and a significant interaction between history of traumatization and psychopathy and psychopathic traits in the juvenile delinquents sample. These relations are not influenced by social desirability. The combined administration of MAYSI-2 and YPI may be of value as screening tools for cases requiring further assessment, although a more extensive evaluation protocol is advised. A prevalência de problemas de saúde mental entre a população adolescente levanta preocupações em torno do percurso desenvolvimental e dos desfechos nas vidas dos jovens com necessidades de saúde mental. Especificamente, a elevada prevalência de psicopatologia entre os jovens delinquentes evidencia a necessidade de métodos e instrumentos de avaliação fiáveis, a fim de melhor compreender como essas necessidades afectam o desenvolvimento e comportamento dos jovens, e a rentabilizar os recursos limitados dos sistemas de saúde mental e de justiça juvenil. A presente investigação tem como principal objectivo continuar o programa de validação do Massachusetts Youth Screening Instrument (MAYSI-2) para a população portuguesa procurando analisar as relações entre necessidades de saúde mental, identificadas por este instrumento, e a presença de traços psicopáticos identificados através do Youth Psychopathic Traits Inventory (YPI). Foram avaliados 76 jovens do sexo masculino, distribuídos por uma amostra de jovens da comunidade (N=43) e de jovens institucionalizados contexto forense (N=33), de idades compreendidas respectivamente entre os 12 e 19, e 14 e 19 anos. O protocolo administrado integra os instrumentos MAYSI-2, YPI e a Escala de Desejabilidade Social de Coimbra (EDSC), uma medida de controlo do estilo e validade das respostas. A análise da prevalência de necessidades de saúde mental e traços psicopáticos revelou predomínio dos mesmos nos jovens que integram a amostra forense. Os resultados revelaram igualmente uma associação positiva entre as dimensões do domínio comportamental de ambas as medidas centrais, e uma relação positiva entre história prévia de vitimização e traços de psicopatia, na amostra de jovens delinquentes. Estas relações não sofrem a influência da desejabilidade social. A administração combinada dos instrumentos MAYSI-2 e YPI revelou poder ser útil, enquanto instrumentos de rastreio e triagem de casos que necessitem de uma avaliação mais compreensiva, sendo no entanto recomendável o recurso a um protocolo de avaliação mais exaustivo.

Background Acute ischemic stroke (AIS) is a complex disease, and the therapeutic control of its risk factors may influence the efficacy of acetylsalicylic acid (ASA) and the occurrence of new vascular events. The aim of this study was to investigate the potential in vivo properties of a previous treatment controlling the main risk factors of cerebrovascular disease, in the ASA-nonresponsive status in a Portuguese population. Methods We conducted a prospective cohort study with the recruitment of 90 patients diagnosed with AIS and a follow-up protocol was set up with recurrent stroke as the main clinical end point under evaluation. At admission, PFA-100 (platelet function analyzer) test was evaluated in blood samples from AIS patients treated with 100 mg/day of ASA and previously treated with antihypertensive, antidiabetic, or statin drugs. Results We observed that 30% of patients were ASA nonresponders. Multivariate regression analysis indicated that the previous treatment with antihypertensive drugs emerged with a significant risk reduction of an ASA-nonresponsive status (odds ratio, .119; 95% confidence interval, .026-.538; P = .006). Furthermore, our results indicated an influence of ASA-nonresponsive status in a decreased period to a new recurrent stroke event in the time frame of 24 months ( P = .005, log-rank test). Conclusions ASA is an important part of treatment of AIS, and its efficacy may be improved in previous high-risk cerebrovascular patients, particularly with antihypertensive therapeutic control.

Background. The glutathione S-transferases (GSTs) are a group of multifunctional enzymes that catalyze the conjugation of glutathione with a variety of electrophilic compounds, including cytotoxic agents. A significant percentage of normal individuals exhibit genetic polymorphism with a homozygous deletion (null genotype) of the genes, leading to absence of the enzyme. Methods. In the present study we analyzed GSTM1 and GSTT1 polymorphisms in the genomic DNA isolated from peripheral blood of patients with ovarian cancer treated with chemotherapy (paclitaxel and cisplatinum) after cytoreductive surgery and assessed its correlation with the clinical outcome of these patients. The median follow-up for the patients was 30 months. Results. The estimated 3-year survival rate was 59.8% for all patients and 20.8% for carriers of GSTM1-wt/GSTT1-wt (wt indicates wild type) genotype combination (37.7% for GSTM1-wt alone) compared with 83.1% for non-GSTM1-wt/GSTT1-wt genotype carriers (100% for GSTM1-null). The mean survival time was significantly better in patients who are carriers of the GSTM1-null genotype (40.5 vs. 33.5; P = 0.006) or carriers of non-GSTM1-wt/GSTT1-wt genotypes (55.4 vs. 30.7; P = 0.009). The progression-free interval was more favorable for GSTM1-null carriers (41.9 vs. 27.4; P = 0.024). Conclusion. The study suggests that characterization of the drug-metabolizing genetic individual profile can be of great interest in clinical oncology. It can define the optimal chemotherapy for each patient, improve the efficiency, and reduce the incidence of drug toxicity and poor drug responses.

Ora, para fazer esta análise importa, obrigatoriamente, refletir sobre a defesa dos mais fracos.Segundo alguns autores, a proteção da parte mais fraca remonta ao direito romano. E, na Grécia Antiga, Aristóteles apontava a necessidade de existência de fiscais para obviar a vícios nos produtos comercializados.Procura-se, assim, demostrar neste estudo que o próprio texto constitucional arvorou a defesa dos direitos dos consumidores ao reconhecer-lhes o estatuto de direitos constitucionais.
