Abstract

Background and aim: Over 80% (365/454) of the nation’s centers participated in the Italian Society of Nephrology COVID-19 Survey. Out of 60,441 surveyed patients, 1368 were infected as of April 23rd, 2020. However, center-specific proportions showed substantial heterogeneity. We therefore undertook new analyses to identify explanatory factors, contextual effects, and decision rules for infection containment. Methods: We investigated fixed factors and contextual effects by multilevel modeling. Classification and Regression Tree (CART) analysis was used to develop decision rules. Results: Increased positivity among hemodialysis patients was predicted by center location [incidence rate ratio (IRR) 1.34, 95% confidence interval (CI) 1.20–1.51], positive healthcare workers (IRR 1.09, 95% CI 1.02–1.17), test-all policy (IRR 5.94, 95% CI 3.36–10.45), and infected proportion in the general population (IRR 1.002, 95% CI 1.001–1.003) (all p < 0.01). Conversely, lockdown duration exerted a protective effect (IRR 0.95, 95% CI 0.94–0.98) (p < 0.01). The province-contextual effects accounted for 10% of the total variability. Predictive factors for peritoneal dialysis and transplant cases were center location and infected proportion in the general population. Using recursive partitioning, we identified decision thresholds at general population incidence ≥ 229 per 100,000 and at ≥ 3 positive healthcare workers. Conclusions: Beyond fixed risk factors, shared with the general population, the increased and heterogeneous proportion of positive patients is related to the center’s testing policy, the number of positive patients and healthcare workers, and to contextual effects at the province level. Nephrology centers may adopt simple decision rules to strengthen containment measures timely.

11. Haptoglobin: a new player in the crosstalk between Apolipoprotein E and Beta Amyloid

Abstract

Objectives: Beta amyloid (beta-A) accumulation in brain is a driving force for Alzheimer's disease (AD) pathogenesis. Apolipoprotein E (ApoE) represents a critical player in beta-A homeostasis, but its role in AD progression is still controversial. We previously reported that Haptoglobin (Hpt), whose level increases during inflammation, binds ApoE and impairs apolipoprotein function in cholesterol homeostasis. Hpt was found associated with beta-A plaques, and shown to inhibit beta-A aggregation. The aim of this study was to evaluate whether the interaction between Hpt and ApoE affects the binding of ApoE to beta-A. Methods: ELISA, immunoprecipitation, and Western blotting, by using purified proteins or cerebrospinal fluid (CSF) were performed in this study. Results: We report here, for the first time, that Hpt forms SDS stable complexes with beta-A, and participates to the interaction between ApoE and beta-A, as shown by immunoprecipitating Hpt- betaA and Hpt- betaA -ApoE complexes from CSF. Furthermore Hpt acts as a positive effector by differently increasing the interaction of beta-A with ApoE2, ApoE3, or ApoE4 (ApoE2>ApoE3>ApoE4). Also, our results showed that the known Hpt activity in preventing oxidative modifications of ApoE is limited in the presence of beta Amyloid1-42. Conclusions: The interaction between Hpt and ApoE might represent a mechanism by which inflammation affects AD pathogenesis. We suggest that Hpt and ApoE functions in brain should be evaluated taking into account their interaction with beta-Amyloid.

13. Lecithin cholesterol acyltransferase esterifies 24s-hydroxycholesterol in cerebrospinal fluid

Abstract

Alteration of cholesterol (Ch) homeostasis is involved in the pathogenesis of neurodegeneration. Astrocyte-derived lecithin cholesterol acyltransferase (LCAT) was recently shown capable of esterifying Ch on ApoE-lipoproteins. In order to prevent Ch excess in neurons, it can be converted in 24S-hydroxycholesterol (24-OH Ch). Since this cerebrosterol can cross the brain blood barrier, 24-OH Ch is thought to be a major form of Ch for egress from the brain. 24-OH Ch is known to be produced almost exclusively in the brain, and was so far only suggested to be LCAT substrate in order to explain why most of it can be detected in esterified form. Here we report the first experimental evidence that LCAT is capable of esterifying 24-OH Ch in cerebrospinal fluid (CSF). This result comes from experiments of competition between 24-OH Ch and Ch for LCAT and experiments of in vitro production of 24-OH Ch esters. In detail, increasing amounts of 24-OH Ch, in a standard assay of LCAT activity, decreased Ch esterification. Also, purified 24-OH Ch was incubated in a mixture containing CSF as LCAT source, ApoE, and a fluorescent phospholipid (as acyl chain donor). 24-OH Ch esters level, detected by fluorescence, was found increased in time. So, it can be excluded that 24-OH Ch esters found in CSF are produced by ACAT, and released by necrotic cells. Further experiments are required to shed light on how LCAT function on 24-OH Ch may influence brain Ch homeostasis and recovery from neuronal injury.

Abstract

Apolipoprotein A-I (ApoA-I) is the main protein component of the high density lipoproteins and it plays an important role in the reverse cholesterol transport. In particular, it stimulates cholesterol efflux from peripheral cells toward liver and activates the enzyme lecithin-cholesterol acyltransferase (LCAT). Haptoglobin (Hpt), a plasma ?(2) -glycoprotein belonging to the family of acute-phase proteins, binds to ApoA-I inhibiting the stimulation of the enzyme LCAT. Previously, we reported that a synthetic peptide, P2a, binds to and displaces Hpt from ApoA-I restoring the LCAT cholesterol esterification activity in the presence of Hpt. Here, we investigate the molecular determinants underlining the interaction between Hpt and P2a peptide. Analysis of truncated P2a analogs showed that P2a sequence can only be slight reduced in length at the N-terminal to preserve the ability of binding to Hpt. Binding assays showed that charged residues are not involved in Hpt recognition; actually, E146A and D157A substitutions increase the binding affinity to Hpt. Biological characterization of the corresponding P2a peptide analogs, Apo146 and Apo157, showed that the two peptides interfere with Hpt binding to HDL and are more effective than P2a peptide in rescue LCAT activity from Hpt inhibition. This result suggests novel hints to design peptides with anti-atherogenic activity.

Abstract

Intravascular ultrasound is a catheter-based imaging modality that was developed to investigate the condition of coronary arteries and assess the vulnerability of coronary atherosclerotic plaques in particular. Since its introduction in the clinic 20 years ago, use of intravascular ultrasound innovation has been relatively limited. Intravascular ultrasound remains a niche technology; its clinical practice did not vastly expand, except in Japan, where intravascular ultrasound is an appraised tool for guiding percutaneous coronary interventions. In this qualitative research study, we follow scholarship on the sociology of innovation in exploring both the current adoption practices and perspectives on the future of intravascular ultrasound. We conducted a survey of biomedical experts with experience in the technology, the practice, and the commercialization of intravascular ultrasound. The collected information enabled us to map intravascular ultrasound controversies as well as to outline the dynamics of the international network of experts that generates intravascular ultrasound innovations and uses intravascular ultrasound technologies. While the technology is praised for its capacity to measure coronary atherosclerotic plaque morphology and is steadily used in clinical research, the lack of demonstrated benefits of intravascular ultrasound guided coronary interventions emerges as the strongest factor that prevents its expansion. Furthermore, most of the controversies identified were external to intravascular ultrasound technology itself, meaning that decision making at the industrial, financial and regulatory levels are likely to determine the future of intravascular ultrasound. In light of opinions from the responding experts', a wider adoption of intravascular ultrasound as a stand-alone imaging modality seems rather uncertain, but the appeal for this technology may be renewed by improving image quality and through combination with complementary imaging modalities, ImPhys/Imaging Physics, Applied Sciences

Abstract

On interroge dans cet article la notion de transition énergétique en explorant la non-prise en compte par les pouvoirs publics de l’écart entre les pratiques de chauffage des ménages et les dispositifs d’injonctions mis en œuvre. Après avoir discuté les résultats d’enquêtes récentes sur les températures de chauffe des ménages, on revisite l’évolution et le mode de production des normes de confort dans l’habitat et on analyse les pratiques de chauffage à l’aune de la notion de transaction sociale. Les pratiques des ménages, qui conduisent à des températures réelles de chauffe des habitations plus élevées que les 19 degrés de la norme, constituent une critique de fait de la transition énergétique.
