Background and Aim of the Work: Intersection syndrome is an uncommon and underdiagnosed condition, especially found in sport overuse. This case report describes the treatment of a professional motor biker who presented intense wrist pain and swelling after training sessions., Methods: The athlete was treated with rest, the use of a wrist splint combined with NSAIDs, physical therapy, including pulsed Nd-YAG laser therapy and exercise, such as stretching and release, massage and eccentric training., Results: The applied therapeutic protocol was successful and allowed the biker to completely resolve the symptoms and return to training session and professional races at full power. The results are durable at 5 months post injury follow up., Conclusions: This experience suggests that pulsed Nd-YAG laser therapy can be an additional useful tool in the global approach treatment for intersection syndrome, contributing with splint and manual therapy, to full recovery even in mechanically stressful conditions. Proximal row carpectomy (PRC), which was initially described by Stamm in 1944, is a surgical procedure for degenerative disorders of the proximal carpal row of the carpus. Despite the good results reported in the literature, this technique has been gradually replaced over the years by others considered more modern and actual, such as wrist arthroplasty and arthrodesis. In this context the authors performed the following study and analyzed retrospectively 14 patients who underwent PRC during an eighteen years period, between June 1996 and June 2013, in order to determine if this surgical operation can be considered still indicated. The results of this study confirm that PRC, more or less associated with the use of capsular flaps and/or osteochondral grafts, is nowadays a valid and simple procedure in selected patients, ensuring a satisfactory functional recovery and regression of pain.

Background and Aim of the Work: the traumatic amputation or partial amputation of a portion of the lower limb is one of the most serious and not so rare road accident and job injury. There are few cases reported of replantation of the lower extremities rather than amputation surgery. This work describes a case of partial amputation of the right ankle., Methods: the emergency treatment consisted of rigorous lavage and debridement, reduction, stabilization of the ankle and restoreation of the neurovascular and soft tissues lesions. Because of cutaneous necrosis another surgical treatment of reverse rotation flap was needed. The good outcome has been documented with foot pressure analysis, stabilometric evaluation and gait and jump analysis based on measurements of acceleration., Results: after seven months of personalized rehabilitation program the patient walks without devices, has recovered functional of range of movement and had no neurological deficit or subjective problems. Currently the subject has returned to his job and runs without pain., Conclusions: emergency foot salvage treatment was possible thanks to a close collaboration among different physicians with specialized skills, good emergency management and an adequate infrastructure.

Background and Aim of the Work: Displaced acetabular fractures are complex lesions which may cause severe consequences if not appropriately treated. The results are linked to the quality of the articular reduction. Anatomical reduction, through surgical treatment with rigid internal fixation, should be considered to obtain an early mobilization, reduce long term osteoarthritis evolution and make the acetabular cavity suitable to eventually recive a total hip prosthesis. The aim of this study is to evaluate functional and radiographic outcome of patients with displaced acetabular fractures surgically treated., Methods: Between 2005 and 2011, 29 displaced acetabular fracures were treated with open reduction and internal fixation. This study highlight our results in 13 patients with a mean follow-up of 29,5 months (range 5,5-66,3). Clinical evaluation was done according to the Harris hip score, while the radiological criteria were those of Kellgren-Lawrence. The associated injuries and complications were evaluated., Results: At the latest follow-up the radiological results, based on the Kellgren-Lawrence grading scale, showed 3 patients with a I grade of osteoarthritis, 5 with a II grade, 3 with a III grade and 2 with a IV grade. The average Harris hip score was 77 (range 37-100). Postoperative complications included avascular necrosis of the femoral head in 3 patients and heterotopic ossification in 2 patients., Conclusions: This study confirm that open reduction and internal fixation in displaced acetabular fractures represents the best treatment able to lead to a satisfactory functional outcome. Moreover, it posticipates long-term arthrosis and eventually makes easier the implant of total hip prosthesis. (www.actabiomedica.it).
