BACKGROUND/OBJECTIVES: Long-term observational cohorts provide the opportunity to investigate the potential impact of dietary patterns on death. We aimed to investigate all-cause death according to the consumption of selected food groups, and then to identify those independently associated with reduced mortality. SUBJECTS/METHODS: Population survey of middle-aged men randomly selected in the period 1995-1997 from the general population of three French areas and followed over a median of 14.8 years. Dietary data were collected through a 3-day food record. Cox modeling was used to assess the risk of death according to selected foods groups after extensive adjustment for confounders, including a diet quality index. RESULTS: The study population comprised 960 men (mean age 55.5 [+ or -]6.2 years). After a median follow-up of 14.8 (interquartile range 14.3-15.2) years, 150 (15.6%) subjects had died. Food groups that remained independently predictive of a lower risk of death after extensive adjustment were an above-median consumption of milk (adjusted relative risk: 0.61, 95% confidence interval (CI): 0.43-0.86, P-value = 0.005), fruits and vegetables (0.68, 0.46-0.98, P-value = 0.041) and a moderate consumption of yogurts and cottage cheese (0.50, 95% CI: 0.31-0.81, P-value = 0.005), other cheeses (0.62, 0.39-0.97, P-value = 0.036) and bread (0.57, 0.37-0.89, P-value = 0.014). Besides, there was a nonsignificant trend for a higher risk of death associated with highest sodium intakes. CONCLUSIONS: Consumption of food groups that largely match recommendations is associated with a reduced risk of all- cause death in men. A diet providing moderate amounts of diverse food groups appears associated with the highest life expectancy. A study was conducted on the effects of cigarette smoking on serum lipid and lipoprotein levels taking into account the nutritional intakes. A case-control design was used to compare smokers and non-smokers. The results indicate an association between smoking and serum lipid/lipoprotein alterations independently of nutritional factors. Smokers also have less 'healthy' nutritional habits than non-smokers.

Background and aims: The association between adiposity measures and dyslipidemia has seldom been assessed in a multipopulational setting. Methods and results: 27 populations from Europe, Australia, New Zealand and Canada (WHO MONICA project) using health surveys conducted between 1990 and 1997 in adults aged 35-64 years (n = 40,480). Dyslipidemia was defined as the total/HDL cholesterol ratio >6 (men) and >5 (women).Overall prevalence of dyslipidemia was 25% in men and 23% in women. Logistic regression showed that dyslipidemia was strongly associated with body mass index (BMI) in men and with waist circumference (WC) in women, after adjusting for region, age and smoking. Among normal-weight men and women (BMI84.8 cm) in normal-weight men, menopause in women and regular smoking further defined subgroups at increased risk. Conclusion: standard categories of BMI and WC, or their combinations, do not lead to optimal risk stratification for dyslipidemia in middle-age adults. Sex-specific adaptations are necessary, in particular by taking into account abdominal obesity in normal-weight men, post-menopausal age in women and regular smoking in both sexes

- Abstract
Background and aims: The association between adiposity measures and dyslipidemia has seldom been assessed in a multipopulational setting. Methods and results: 27 populations from Europe, Australia, New Zealand and Canada (WHO MONICA project) using health surveys conducted between 1990 and 1997 in adults aged 35-64 years (n = 40,480). Dyslipidemia was defined as the total/HDL cholesterol ratio >6 (men) and >5 (women).Overall prevalence of dyslipidemia was 25% in men and 23% in women. Logistic regression showed that dyslipidemia was strongly associated with body mass index (BMI) in men and with waist circumference (WC) in women, after adjusting for region, age and smoking. Among normal-weight men and women (BMI<25 kg/m2), an increase in the odds for being dyslipidemic was observed between lowest and highest WC quartiles (OR = 3.6, p < 0.001). Among obese men (BMI ≥ 30), the corresponding increase was smaller (OR = 1.2, p = 0.036). A similar weakening was observed among women. Classification tree analysis was performed to assign subjects into classes of risk for dyslipidemia. BMI thresholds (25.4 and 29.2 kg/m2) in men and WC thresholds (81.7 and 92.6 cm) in women came out at first stages. High WC (>84.8 cm) in normal-weight men, menopause in women and regular smoking further defined subgroups at increased risk. Conclusion: standard categories of BMI and WC, or their combinations, do not lead to optimal risk stratification for dyslipidemia in middle-age adults. Sex-specific adaptations are necessary, in particular by taking into account abdominal obesity in normal-weight men, post-menopausal age in women and regular smoking in both sexes

Metab. 40): E855-E861, 1999.--To assess the impact of the macronutrient content of a meal on the postprandial leptin response and its relationship with postprandial satiety, 22 young healthy subjects (11 men and 11 women) were given, in a randomized order, an isoenergetic meal [carbohydrate (81%) or fat (79%)] or remained fasting. Blood sampling and hunger and satiety scores were collected hourly during 9 h after the meal. Spontaneous intake was measured at a buffet meal at 9 h postprandially. In both genders, leptin response was higher after the carbohydrate meal than after the fat meal and while fasting. In women, leptin levels were higher after the fat meal than while fasting. Leptin response was significantly correlated to insulin response (r = 0.51, P [is less than] 0.0001). Hunger and satiety ratings and subsequent energy intake were not different after carbohydrate or fat intake. In conclusion, a carbohydrate meal induces higher postprandial leptin levels than an isoenergetic fat meal. Short-term regulation of postprandial satiety and food intake is not influenced by circulating leptin.

Background: Food frequency questionnaires (FFQs) are often used to evaluate individuals' food intakes in epidemiologic studies because of their simplicity and low cost., Objective: To assess the validity of a short (24 items), qualitative FFQ used in the MONA LISA-NUT study., Design: Cross-sectional study of a representative sample in three French counties., Participants/setting: The sample included 2,630 participants aged 35 to 65 years from the MONA LISA-NUT study., Main Outcome Measures: Food consumption was measured with the FFQ and via food records for 3 consecutive days. Plasma fatty acids were measured from a subset of participants., Statistical Analyses Performed: The FFQ items' validity was assessed by calculating crude and deattenuated Pearson correlation coefficients between frequencies reported by the FFQ and average weights reported by the food records. Furthermore, the validity of some items of the FFQ measuring the consumption of fatty foods was assessed by calculating Pearson correlation coefficients between frequencies of consumption of these foods and dosages of the corresponding plasma fatty acids: fish and eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), olive oil and oleic acid, margarine and elaidic acid, and dairy products and pentadecanoic and heptadecanoic acids., Results: The mean of the deattenuated Pearson correlation coefficients for all items was 0.46, with values ranging from 0.22 (fried food) to 0.77 (breakfast cereal). The correlation coefficient was ≤ 0.4 for one third of the 24 items. Moderate correlations were found between fish and EPA/DHA (EPA: r=0.43, 95% CI 0.33 to 0.51; DHA: r=0.39, 95% CI 0.30 to 0.47), but not for other food items., Conclusions: One third of the 24 items in the short, qualitative FFQ evaluated here were not sufficiently valid. However, for the food groups most commonly studied in the literature, this FFQ had the same degree of validity as other questionnaires designed to classify subjects according to their level of intake., (Copyright © 2014 Academy of Nutrition and Dietetics. - Abstract
The relationship between menopause and cardiovascular risk factors of progestin alone or in combination with estrogen were analysed in a sample of French women. Three thousand four hundred and forty consecutive women, between 45 and 65 years of age, who received a systematic check-up between January 1991 and April 1993 were enrolled in this study. All biological measurements were performed at a central laboratory. Women were classified as premenopausal (n = 1233), postmenopausal (n = 1774) if they had not menstruated in the 12 months before examination, and perimenopausal (n = 433) if they met at least two of the following criteria: elevated levels of serum follicle-stimulating hormone (FSH), irregular menses, amenorrhea for less than 12 months, hot flushes. The effect of menopause on cardiovascular risk factors was determined in 2167 women between 45 and 65 years of age (premenopausal n = 790, postmenopausal n = 1377), none of whom were presently treated with hormonal replacement therapy (HRT). In addition, the effect of progestin was assessed in a group or 397 perimenopausal women, and the effect of combined estrogen and progestin replacement therapy in another group of 1746 postmenopausal women. Menopause was associated with higher levels of serum cholesterol (6.4 vs. 5.9 mmol/l), triglycerides (1.2 vs. 1.0 mmol/l), apolipoprotein (apo) B (1.3 vs. 1.1 g/l), apo A-I (1.9 vs. 1.8 g/l), as well as with elevated diastolic blood pressure (79.7 vs. 77.0 mmHg). Multivariate analysis indicated that these effects were independent of age, body mass index (BMI), glycemia, smoking, alcohol intake, exercise and parity. Perimenopausal women treated with progestin alone (n = 95) were compared to perimenopausal women not using HRT (n = 302). There were no statistically significant differences in the levels of cholesterol, triglycerides, apo B, apo A-I, glycemia and blood pressure between the two groups. Postmenopausal women using a combination of estrogen and progestin (n = 369) had significantly lower levels of serum cholesterol (6.1 vs 6.4 mmol/l), triglycerides (1.0 vs. 1.2 mmol/l), apo B (1.2 vs. 1.3 g/l), systolic (131.9 vs. 137.9 mmHg) and diastolic (76.9 vs. 79.7 mmHg) blood pressure than postmenopausal women without hormonal therapy (n = 1377), taking into account confounding variables. In contrast, serum apo A-I levels were not altered by the combined hormonal therapy. We conclude that menopause is associated with the aggravation of multiple cardiovascular risk factors. These deleterious factors are affected by a treatment of combining estrogen and progestin.
