Through its effects on gastric secretion, we hypothesized that Helicobacter pylori infection may influence oral immunization. Accordingly, we examined the association between H. pylori infection, serum pepsinogen (PG) (measures for H. pylori gastritis) and vibriocidal antibody (a correlate of protection) seroconversion following oral immunization with CVD 103-HgR live cholera vaccine among children of different ages.Sera from 422 Chilean children who were vaccinated with a single dose of CVD 103-HgR were tested by ELISA for serum IgG antibodies to H. pylori, PG I and PG II levels and antibodies to Shigella flexneri 2a lipopolysaccharide and hepatitis A virus (as markers of low socioeconomic status and exposure to enteric pathogens).The likelihood of vibriocidal antibody seroconversion following vaccination with CVD 103-HgR was significantly decreased in H. pylori-seropositive children age 6 months to 4 years with PG II>8 µg/L (adjusted OR 0.14 (95% CI 0.03-0.61; P = 0.009), and also in H. pylori seropositives with lower PG II level (adjusted OR 0.34, 95% CI 0.14-0.83; P = 0.017), compared to H. pylori-seronegatives. H. pylori-seropositive children aged 5-9 years with serum PG I>30 µg/L (indicating more severe gastritis) had higher odds of vibriocidal seroconversion than those with lower PG I levels (adjusted OR 4.41, 95%CI 1.26-15.38; P = 0.02). There was no significant association between exposures to S. flexneri 2a or hepatitis A virus and vibriocidal seroconversion.As H. pylori gastritis progresses with increasing pediatric age in developing country venues, changes in gastric secretion ensue that we believe explain the observed differences in age-related immune responses to immunization with live oral cholera vaccine. The effect of H. pylori and changes of gastric acid secretion on the immunogenicity of various oral vaccines should be studied in different developing, transitional and industrialized country settings.

Primary carcinoma of the fallopian tube is a rare entity that accounts for 0.2-0.5% of all gynecologic malignancies, and most are discovered during or after surgery. Primary transitional cell carcinoma of the fallopian tube is an extremely rare tumor that is reported only occasionally in the worldwide literature. As primary transitional cell carcinoma (TCC) of the fallopian tube is so rare, the clinicopathologic characteristics are as yet unknown. The authors recently experienced a case of primary transitional cell carcinoma arising in the left fallopian tube and thus report the clinical features, management, and also a review of the past pertinent literature. A 52 years old woman presenting with lower abdominal pain was found to have a left adnexal mass. Exploratory laparotomy revealed a mass arising from the left fallopian tube with the histologic features of transitional cell carcinoma.

Tubular Krukenberg tumor with an occult primary can cause problems in histopathologic diagnosis, by mimicking primary ovarian tumors. We present one such occurrence in a 32year old female who underwent surgery for bilateral malignant ovarian tumor. Gross examination of both ovarian tumors revealed bosselated, smooth outer surface with a few cysts on the surface. Cut surface was grey-white to yellowish in colour with cysts filled with serous fluid at the periphery. Microscopic examination revealed mucin laden signet ring cells, predominantly showing tubular architecture within a cellular ovarian stroma. The cytoplasm of these cells varied from granular eosinophilic to pale vacuolated appearance and showed PAS and mucicarmine positive mucin. Differential diagnosis with other primary ovarian tumors is discussed.

In this article, we demonstrate how participants make use of others’ gazing actions to monitor and engage events in their surroundings. Specifically, we focus on a tension between one sort of gazing action that participants freely join in, “noticing”, and another, “watching”, that is subject to constraints related to participant identities toward a collective and their corresponding rights of membership that include toward what and with whom they may gaze. Employing the method of conversation analysis, we provide a fine-grained examination of differences between the two gazing actions that include movements of the head, body, eyes, and feet, and highlight how these differences provide a resource for differentially orienting to the environment and joining in visually based activities with others.

The use of the term "Hürthle cell neoplasm" as the gold standard should be discouraged as it makes evaluating these lesions more confusing. Recently, a number of studies have been conducted to define criteria that are more specific for Hürthle cell carcinoma (HCC). We herein report two cases of HCC of thyroid which were accurately diagnosed preoperatively using various cytological features described in the recent studies. A review of the literature is also presented.

Herpes simplex virus (HSV) is a rare cause of breast infection. Few cases of maternal-infant transmission of the virus during breastfeeding resulting in nipple lesions have been documented. Only three cases have been reported in nonlactating women. We report an additional case of HSV mastitis in a 36-year-old nonlactating female who was diagnosed on scrape cytology smears.

Matrix-producing carcinoma of the breast is a unique subclass of metaplastic carcinoma which is characterized by the existence of a ductal carcinomatous component with direct transition to areas showing cartilagenous or osseous differentiation, lacking an interspersed spindle cell component. This article reports one such rare case in a 50-year-old woman who had a right breast mass. f0 ine needle aspiration (FNA) smears showed abundant chondromyxoid extracellular matrix to which were variably admixed carcinomatous cells. Histological examination revealed a neoplasm composed of invave ductal carcinoma with a direct transition to chondrosarcomatous areas. The case is reported not only for its peculiar microscopic characteristics but also to highlight its better prognostic features and hence, the need for its recognition.
