Protein compartmentalization in the frame of a liquid-liquid phase separation is a key mechanism to optimize spatiotemporal control of biological systems. Such a compartmentalization process reduces the intrinsic noise in protein concentration due to stochasticity in gene expression. Employing Flory-Huggins solution theory, Avramov/Casalini's model, and the Grüneisen parameter, we unprecedentedly propose a cellular Griffiths-like phase (CGLP), which can impact its functionality and self-organization. The here-proposed CGLP is key ranging from the understanding of primary organisms' evolution to the treatment of diseases. Our findings pave the way for an alternative Biophysics approach to investigate coacervation processes.

Abstract Background Stroke is a leading cause of mortality and disability, and its sequelae are associated with inadequate food intake which can lead to sarcopenia. The aim of this study is to verify the effectiveness of creatine supplementation on functional capacity, strength, and changes in muscle mass during hospitalization for stroke compared to usual care. An exploratory subanalysis will be performed to assess the inflammatory profiles of all participants, in addition to a follow-up 90 days after stroke, to verify functional capacity, muscle strength, mortality, and quality of life. Methods Randomized, double-blind, unicenter, parallel-group trial including individuals with ischemic stroke in the acute phase. The duration of the trial for the individual subject will be approximately 90 days, and each subject will attend a maximum of three visits. Clinical, biochemical, anthropometric, body composition, muscle strength, functional capacity, degree of dependence, and quality of life assessments will be performed. Thirty participants will be divided into two groups: intervention (patients will intake one sachet containing 10g of creatine twice a day) and control (patients will intake one sachet containing 10g of placebo [maltodextrin] twice a day). Both groups will receive supplementation with powdered milk protein serum isolate to achieve the goal of 1.5g of protein/kg of body weight/day and daily physiotherapy according to the current rehabilitation guidelines for patients with stroke. Supplementation will be offered during the 7-day hospitalization. The primary outcomes will be functional capacity, strength, and changes in muscle mass after the intervention as assessed by the Modified Rankin Scale, Timed Up and Go test, handgrip strength, 30-s chair stand test, muscle ultrasonography, electrical bioimpedance, and identification of muscle degradation markers by D3-methylhistidine. Follow-up will be performed 90 days after stroke to verify functional capacity, muscle strength, mortality, and quality of life. Discussion The older population has specific nutrient needs, especially for muscle mass and function maintenance. Considering that stroke is a potentially disabling event that can lead the affected individual to present with numerous sequelae, it is crucial to study the mechanisms of muscle mass loss and understand how adequate supplementation can help these patients to better recover. Summary: Background: Multisystem inflammatory syndrome in children (MIS-C) is a severe complication of SARS-CoV-2 infection. It remains unclear how MIS-C phenotypes vary across SARS-CoV-2 variants. We aimed to investigate clinical characteristics and outcomes of MIS-C across SARS-CoV-2 eras. Methods: We performed a multicentre observational retrospective study including seven paediatric hospitals in four countries (France, Spain, U.K., and U.S.). All consecutive confirmed patients with MIS-C hospitalised between February 1st, 2020, and May 31st, 2022, were included. Electronic Health Records (EHR) data were used to calculate pooled risk differences (RD) and effect sizes (ES) at site level, using Alpha as reference. Meta-analysis was used to pool data across sites. Findings: Of 598 patients with MIS-C (61% male, 39% female; mean age 9.7 years [SD 4.5]), 383 (64%) were admitted in the Alpha era, 111 (19%) in the Delta era, and 104 (17%) in the Omicron era. Compared with patients admitted in the Alpha era, those admitted in the Delta era were younger (ES −1.18 years [95% CI −2.05, −0.32]), had fewer respiratory symptoms (RD −0.15 [95% CI −0.33, −0.04]), less frequent non-cardiogenic shock or systemic inflammatory response syndrome (SIRS) (RD −0.35 [95% CI −0.64, −0.07]), lower lymphocyte count (ES −0.16 × 109/uL [95% CI −0.30, −0.01]), lower C-reactive protein (ES −28.5 mg/L [95% CI −46.3, −10.7]), and lower troponin (ES −0.14 ng/mL [95% CI −0.26, −0.03]). Patients admitted in the Omicron versus Alpha eras were younger (ES −1.6 years [95% CI −2.5, −0.8]), had less frequent SIRS (RD −0.18 [95% CI −0.30, −0.05]), lower lymphocyte count (ES −0.39 × 109/uL [95% CI −0.52, −0.25]), lower troponin (ES −0.16 ng/mL [95% CI −0.30, −0.01]) and less frequently received anticoagulation therapy (RD −0.19 [95% CI −0.37, −0.04]). Length of hospitalization was shorter in the Delta versus Alpha eras (−1.3 days [95% CI −2.3, −0.4]). Interpretation: Our study suggested that MIS-C clinical phenotypes varied across SARS-CoV-2 eras, with patients in Delta and Omicron eras being younger and less sick. EHR data can be effectively leveraged to identify rare complications of pandemic diseases and their variation over time. Funding: None.

Summary: Background: Characterizing Post-Acute Sequelae of COVID (SARS-CoV-2 Infection), or PASC has been challenging due to the multitude of sub-phenotypes, temporal attributes, and definitions. Scalable characterization of PASC sub-phenotypes can enhance screening capacities, disease management, and treatment planning. Methods: We conducted a retrospective multi-centre observational cohort study, leveraging longitudinal electronic health record (EHR) data of 30,422 patients from three healthcare systems in the Consortium for the Clinical Characterization of COVID-19 by EHR (4CE). From the total cohort, we applied a deductive approach on 12,424 individuals with follow-up data and developed a distributed representation learning process for providing augmented definitions for PASC sub-phenotypes. Findings: Our framework characterized seven PASC sub-phenotypes. We estimated that on average 15.7% of the hospitalized COVID-19 patients were likely to suffer from at least one PASC symptom and almost 5.98%, on average, had multiple symptoms. Joint pain and dyspnea had the highest prevalence, with an average prevalence of 5.45% and 4.53%, respectively. Interpretation: We provided a scalable framework to every participating healthcare system for estimating PASC sub-phenotypes prevalence and temporal attributes, thus developing a unified model that characterizes augmented sub-phenotypes across the different systems. Cardiac remodeling is defined as molecular, cellular, and interstitial changes that manifest clinically as alterations in the size, shape, and function of the heart. Despite the pharmacological approaches, cardiac remodeling-related mortality rates remain high. Therefore, other therapeutic options are being increasingly studied. This review highlights the role of omega-3 as an adjunctive therapy to attenuate cardiac remodeling, with an emphasis on its antioxidant and anti-inflammatory actions.

Summary: Background: While acute kidney injury (AKI) is a common complication in COVID-19, data on post-AKI kidney function recovery and the clinical factors associated with poor kidney function recovery is lacking. Methods: A retrospective multi-centre observational cohort study comprising 12,891 hospitalized patients aged 18 years or older with a diagnosis of SARS-CoV-2 infection confirmed by polymerase chain reaction from 1 January 2020 to 10 September 2020, and with at least one serum creatinine value 1–365 days prior to admission. Resumo Fundamento: A vitamina D (VD) tem um importante papel na função cardíaca. No entanto, a vitamina exerce uma curva “dose-resposta” bifásica na fisiopatologia cardiovascular e pode causar efeitos deletérios, mesmo em doses não tóxicas. A VD exerce suas funções celulares ligando-se ao seu receptor. Ainda, a expressão da proteína de interação com a tiorredoxina (TXNIP) é positivamente regulada pela VD. A TXNIP modula diferentes visa de sinalização celular que podem ser importantes para a remodelação cardíaca. Objetivos: Avaliar se a suplementação com VD leva à remodelação cardíaca, e se a TXNIP e a tiorredoxina (Trx) estão associadas com esse processo. Métodos: Duzentos e cinquenta ratos Wistar machos foram alocados em três grupos: controle (C, n=21), sem suplementação com VD; VD3 (n = 22) e VD10 (n=21), suplementados com 3,000 e 10,000 UI de VD/ kg de ração, respectivamente, por dois meses. Os grupos foram comparados por análise de variância (ANOVA) com um fator e teste post hoc de Holm-Sidak (variáveis com distribuição normal), ou pelo teste de Kruskal-Wallis e análise post-hoc de Dunn. O nível de significância para todos os testes foi de 5%. Resultados: A expressão de TXNIP foi mais alta e a atividade do Trx foi mais baixa no grupo VD10. Os animais que receberam suplementação com VD apresentaram aumento de hidroperóxido lipídico e diminuição de superóxido dismutase e glutationa peroxidase. A proteína Bcl-2 foi mais baixa no grupo VD10. Observou-se uma diminuição na β-oxidação de ácidos graxos, no ciclo do ácido tricarboxílico, na cadeia transportadora de elétrons, e um aumento na via glicolítica. Conclusão: A suplementação com VD levou à remodelação cardíaca e esse processo pode ser modulado por TXNIP e Trx, e consequentemente por estresse oxidativo.

Introduction: Tobacco smoke is associated with oxidative and inflammatory pathways, increasing the risk of chronic-degenerative diseases. Our goal was to evaluate the effects of acute “Pera” and “Moro” orange juice consumption on inflammatory processes and oxidative stress in microRNA (miRNA) expression in plasma from healthy smokers.Methods: This was a randomized crossover study that included healthy smokers over 18 years old. Blood samples were collected before and 11 h after beverage ingestion. Participants were instructed to drink 400 mL of Pera orange juice (Citrus sinensis), Moro orange juice (Citrus sinensis L. Osbeck), or water. Each subject drank the beverages in a 3-way crossover study design. Inflammatory and oxidative stress biomarkers and circulating miRNA expression profiles were determined. The subjects maintained their usual tobacco exposure during the experiment.Results: We included 18 individuals (12 men and 6 women), with 37.0 ± 12.0 years old. All subjects received the 3 interventions. Increased expression of circulating miRNAs (miR-150-5p, miR-25-3p, and miR-451a) was verified after cigarette smoking, which were attenuated after intake of both types of orange juice. There was no difference regarding serum levels of TNF-α, IL-6, MMP-9, and C-reactive protein. Despite the increased activity of serum superoxide dismutase and glutathione peroxidase after “Pera” or “Moro” orange juice intake, respectively, no changes in lipid hydroperoxide levels were detected.Conclusion: Tobaccos smokers showed increased expression of miR-150-5p, miR-25-3p, and miR-451a was noted, and attenuated by orange juice intake. miRNAs were predicted to regulate 244 target genes with roles in oxidative stress, PI3K-Akt, and MAPK signaling, which are pathways frequently involved in smoking-related cardiovascular diseases and cancer.

Background/Aims: The objective of our study was to evaluate the effects of zinc supplementation on cardiac remodeling following acute myocardial infarction in rats. Methods: Animals were subdivided into 4 groups and observed for 3 months: 1) Sham Control; 2) Sham Zinc: Sham animals receiving zinc supplementation; 3) Infarction Control; 4) Infarction Zinc. After the followup period, we studied hypertrophy and ventricular geometry, functional alterations in vivo and in vitro, changes related to collagen, oxidative stress, and inflammation, assessed by echocardiogram, isolated heart study, western blot, flow cytometer, morphometry, and spectrophotometry. Results: Infarction induced a significant worsening of the functional variables. On the other hand, zinc attenuated both systolic and diastolic cardiac dysfunction induced by infarction. Considering the infarct size, there was no difference between the groups. Catalase and superoxide dismutase decreased in infarcted animals, and zinc increased its activity. We found higher expression of collagens I and III in infarcted animals, but there was no effect of zinc supplementation. Likewise, infarcted animals had higher levels of IL-10, but without zinc interference. Nrf-2 values were not different among the groups. Infarction increased the amount of Treg cells in the spleen as well as the amount of total lymphocytes. Zinc increased the amount of CD4+ in infarcted animals, but we did not observe effects in relation to Treg cells. Conclusion: zinc attenuates cardiac remodeling after infarction in rats; this effect is associated with modulation of antioxidant enzymes, but without the involvement of collagens I and III, Nrf-2, IL-10, and Treg cells.

Background/Aims: The aim of this study was to evaluate the influence of pamidronate on ventricular remodeling after myocardial infarction. Methods: Male Wistar rats were assigned to four groups: a sham group, in which animals were submitted to simulated surgery and received weekly subcutaneous injection of saline (S group; n=14); a group in which animals received weekly subcutaneous injection of pamidronate (3 mg/kg of body weight) and were submitted to simulated surgery (SP group, n=14); a myocardial infarction group, in which animals were submitted to coronary artery ligation and received weekly subcutaneous injection of saline (MI group, n=13); and a myocardial infarction group with pamidronate treatment (MIP group, n=14). The rats were observed for three months. Results: Animals submitted to MI had left chamber enlargement and worse diastolic and systolic function compared with SHAM groups. E/A ratio, LV posterior and relative wall thickness were lower in the MIP compared with the MI group. There was no interaction between pamidronate administration and MI on systolic function, myocyte hypertrophy, collagen content, and calcium handling proteins. Conclusion: Pamidronate attenuates diastolic dysfunction following MI.

Background: Abdominal obesity is an important cardiovascular risk factor. Therefore, identifying the best method for measuring waist circumference (WC) is a priority. Objective: To evaluate the eight methods of measuring WC in patients with acute coronary syndrome (ACS) as a predictor of cardiovascular complications during hospitalization. Methods: Prospective study of patients with ACS. The measurement of WC was performed by eight known methods: midpoint between the last rib and the iliac crest (1), point of minimum circumference (2); immediately above the iliac crest (3), umbilicus (4), one inch above the umbilicus (5), one centimeter above the umbilicus (6), smallest rib and (7) the point of greatest circumference around the waist (8). Complications included: angina, arrhythmia, heart failure, cardiogenic shock, hypotension, pericarditis and death. Logistic regression tests were used for predictive factors. Results: A total of 55 patients were evaluated. During the hospitalization period, which corresponded on average to seven days, 37 (67%) patients had complications, with the exception of death, which was not observed in any of the cases. Of these complications, the only one that was associated with WC was angina, and with every cm of WC increase, the risk for angina increased from 7.5 to 9.9%, depending on the measurement site. It is noteworthy the fact that there was no difference between the different methods of measuring WC as a predictor of angina. Conclusion: The eight methods of measuring WC are also predictors of recurrent angina after acute coronary syndromes.

Background: The effects of modern therapy on functional recovery after acute myocardial infarction (AMI) are unknown.Objectives:To evaluate the predictors of systolic functional recovery after anterior wall AMI in patients undergoing modern therapy (reperfusion, aggressive platelet antiaggregant therapy, angiotensin-converting enzyme inhibitors and beta-blockers).Methods:A total of 94 consecutive patients with AMI with ST-segment elevation were enrolled. Echocardiograms were performed during the in-hospital phase and after 6 months. Systolic dysfunction was defined as ejection fraction value < 50%.Results:In the initial echocardiogram, 64% of patients had systolic dysfunction. Patients with ventricular dysfunction had greater infarct size, assessed by the measurement of total and isoenzyme MB creatine kinase enzymes, than patients without dysfunction. Additionally, 24.5% of patients that initially had systolic dysfunction showed recovery within 6 months after AMI. Patients who recovered ventricular function had smaller infarct sizes, but larger values of ejection fraction and E-wave deceleration time than patients without recovery. At the multivariate analysis, it can be observed that infarct size was the only independent predictor of functional recovery after 6 months of AMI when adjusted for age, gender, ejection fraction and E-wave deceleration time.Conclusion: In spite of aggressive treatment, systolic ventricular dysfunction remains a frequent event after the anterior wall myocardial infarction. Additionally, 25% of patients show functional recovery. Finally, infarct size was the only significant predictor of functional recovery after six months of acute myocardial infarction.

OBJECTIVE: After acute myocardial infarction, during the cardiac repair phase, periostin is released into the infarct and activates signaling pathways that are essential for the reparative process. However, the role of periostin in chronic cardiac remodeling after myocardial infarction remains to be elucidated. Therefore, the objective of this study was to investigate the relationship between tissue periostin and cardiac variables in the chronic cardiac remodeling induced by myocardial infarction. METHODS: Male Wistar rats were assigned to 2 groups: a simulated surgery group (SHAM; n = 8) and a myocardial infarction group (myocardial infarction; n = 13). After 3 months, morphological, functional and biochemical analyses were performed. The data are expressed as means±SD or medians (including the lower and upper quartiles). RESULTS: Myocardial infarctions induced increased left ventricular diastolic and systolic areas associated with a decreased fractional area change and a posterior wall shortening velocity. With regard to the extracellular matrix variables, the myocardial infarction group presented with higher values of periostin and types I and III collagen and higher interstitial collagen volume fractions and myocardial hydroxyproline concentrations. In addition, periostin was positively correlated with type III collagen levels (r = 0.673, p = 0.029) and diastolic (r = 0.678, p = 0.036) and systolic (r = 0.795, p = 0.006) left ventricular areas. Considering the relationship between periostin and the cardiac function variables, periostin was inversely correlated with both the fractional area change (r = -0.783, p = 0.008) and the posterior wall shortening velocity (r = -0.767, p = 0.012). CONCLUSIONS: Periostin might be a modulator of deleterious cardiac remodeling in the chronic phase after myocardial infarction in rats.

FUNDAMENTO: Estudos de intervenção mostraram aumento da mortalidade em pacientes que receberam betacaroteno. Contudo, não são conhecidos os mecanismos envolvidos nesse fenômeno. OBJETIVO: Avaliar a influência do betacaroteno sobre o estresse oxidativo e a expressão de conexina 43 em coração de ratos. MÉTODOS: Ratos Wistar, pesando aproximadamente 100 g, foram alocados em dois grupos: Grupo Controle (n = 30), que recebeu a dieta usada de rotina em nosso laboratório, e Grupo Betacaroteno (n = 28), que recebeu betacaroteno (na forma de cristal, adicionado e misturado à dieta) na dose de 500 mg de betacaroteno/kg de dieta. Os animais receberam tratamento até que atingissem entre 200 e 250 g, quando eram sacrificados. Foram coletados sangue, fígado e coração para realização de Western blotting e imunoistoquímica para conexina 43; foram realizados estudos morfométricos, dosagens de betacaroteno por cromatografia líquida de alta eficiência bem como de glutationa reduzida, glutationa oxidada e hidroperóxidos de lipídeos por análises bioquímicas. RESULTADOS: O betacaroteno foi detectado apenas no fígado dos animais do Grupo Betacaroteno (288 ± 94,7 µg/kg). Os níveis de glutationa reduzida/glutationa oxidada foram maiores no fígado e no coração dos animais do Grupo Betacaroteno (fígado - Grupo Controle: 42,60 ± 1,62; fígado - Grupo Betacaroteno: 57,40 ± 5,90; p = 0,04; coração: - Grupo Controle: 117,40 ± 1,01; coração - Grupo Betacaroteno: 121,81 ± 1,32 nmol/mg proteína; p = 0,03). O conteúdo de conexina 43 total foi maior no Grupo Betacaroteno. CONCLUSÃO: O betacaroteno apresentou efeito benéfico, caracterizado pelo aumento da comunicação intercelular e melhora do sistema de defesa antioxidante. Nesse modelo, os mecanismos não explicam a maior mortalidade observada com a suplementação de betacaroteno em estudos clínicos. (Arq Bras Cardiol. 2013; [online].ahead print, PP.0-0)

FUNDAMENTO: A relevância do padrão de geometria após o infarto do miocárdio não é conhecida. OBJETIVOS: Analisar a presença de diferentes padrões de geometria ventricular esquerda (VE) e seu impacto como preditor de remodelação em pacientes com infarto do miocárdio. MÉTODOS: Pacientes com infarto agudo anterior (n = 80) foram divididos de acordo com o padrão de geometria: normal (índice de massa [IMVE] normal e espessura relativa da parede [ERP] normal), remodelação concêntrica (IMVE normal e ERP aumentada), hipertrofia concêntrica (IMVE e ERP aumentadas) e hipertrofia excêntrica (IMVE aumentado e ERP normal). Após seis meses, foi repetido o ecocardiograma. RESULTADOS: Quatro pacientes foram a óbito. Dos sobreviventes, 41 apresentaram remodelação (R+), enquanto 39 não remodelaram (R-). Considerando-se o padrão geométrico, houve a seguinte distribuição: 24 pacientes com padrão normal, 13 com remodelação concêntrica, 29 com hipertrofia concêntrica e 14 com hipertrofia excêntrica. Os pacientes que remodelaram apresentaram maiores tamanhos de infarto analisados pelo pico da CPK (R+ = 4.610 (1.688 - 7.970), R- = 1.442 (775 - 4.247), p < 0,001) e da CK-MB (R+ = 441 (246 - 666), R- = 183 (101 - 465), p < 0,001), tendência a maior prevalência de remodelação concêntrica (R+ = 10, R- = 3, p = 0,08) e menor prevalência de hipertrofia excêntrica (R+ = 2, R- = 12, p = 0,006). Na análise de regressão multivariada, o tamanho do infarto foi preditor (OR = 1,01; p = 0,020) e a hipertrofia excêntrica foi fator protetor (OR = 0,189; p = 0,046) de remodelação ventricular após a oclusão coronariana. CONCLUSÃO: O padrão de geometria ventricular pode ter impacto no processo de remodelação em pacientes com infarto do miocárdio.

FUNDAMENTO: O papel de metaloproteinases (MMP) séricas após o infarto do miocárdio (IM) é desconhecido. OBJETIVO: O objetivo deste estudo foi o de avaliar o papel das MMP-2 e -9 séricas como marcadores prognósticos da remodelação ventricular seis meses após o IM anterior. MÉTODOS: Fizemos um registro prospectivo dos pacientes após o seu primeiro IM anterior. A atividade de MMP foi analisada entre 12 a 72 horas após o IM. Foi feito um ecocardiograma durante a internação e seis meses depois. RESULTADOS: Incluímos 29 pacientes; 62% mostraram remodelação ventricular. Os pacientes que mostraram remodelação tinham maior tamanho de infarto baseado nos valores pico da creatinofosfoquinase (CPK) (p = 0,037), alta prevalência de insuficiência cardíaca congestiva em hospitais (p = 0,004), e redução da fração de ejeção (FE) (p = 0,007). Os pacientes com remodelação ventricular tiveram menores níveis séricos de MMP-9 inativa (p = 0,007) e maiores níveis da forma ativa da MMP-2 (p = 0,011). Em um modelo de regressão logística multivariada, ajustado pela idade, pico de CPK, FE e prevalência de insuficiência cardíaca, os níveis séricos da MMP-2 e -9 estavam associados à remodelação (p = 0,033 e 0,044, respectivamente). CONCLUSÃO: Níveis séricos mais elevados da MMP-9 inativa foram associados com a preservação dos volumes ventriculares esquerdos, e níveis séricos mais elevados da forma ativa da MMP-2 foram um preditor da remodelação seis meses após o IM.

FUNDAMENTO: Em estudo anterior, utilizando o modelo de ratos, a exposição à fumaça do cigarro durante 5 semanas aumentou a sobrevida após IAM, apesar da idade similar e tamanho do infarto entre fumantes e não fumantes, e da ausência de reperfusão. OBJETIVO: Dessa forma, o presente estudo teve como objetivo analisar os efeitos da exposição à fumaça do cigarro sobre a intensidade, distribuição ou fosforilação da conexina 43 no coração de ratos. MÉTODOS: Ratos Wistar, pesando 100 g, foram distribuídos aleatoriamente em 2 grupos: 1) Controle (n = 25); 2) Expostos à fumaça do cigarro (ETS), n = 23. Depois de 5 semanas, foram conduzidas análise morfométrica do ventrículo esquerdo, imuno-histoquímica e Western blot para conexina 43 (Cx43). RESULTADOS: A fração do volume de colágeno, as áreas transversais e o peso ventricular não foram estatisticamente diferentes entre os grupos controle e ETS. O grupo ETS apresentou uma coloração de menor intensidade da Cx43 em discos intercalados (Controle: 2,32 ± 0,19; ETS: 1,73 ± 0,18; p = 0,04). A distribuição da Cx43 em discos intercalados não diferiu entre os grupos (Controle: 3,73 ± 0,12; ETS: 3,20 ± 0,17; p = 0,18). Os ratos do grupo ETS mostraram um nível maior de forma desfosforilada da Cx43 (Controle: 0,45 ± 0,11; ETS: 0,90 ± 0,11; p = 0,03). Por outro lado, o Cx43 total não diferiu entre os grupos de controle e ETS (Controle: 0,75 ± 0,19; ETS: 0,93 ± 0,27; p = 0,58). CONCLUSÃO: A exposição à fumaça do cigarro resultou na remodelação das junções comunicantes cardíacas, caracterizada por alterações na quantidade e fosforilação da Cx43 em corações de ratos. Essa constatação pode explicar o paradoxo dos fumantes observado em alguns estudos.

FUNDAMENTO: A predição do processo de remodelação ventricular após o infarto agudo do miocárdio (IAM) pode ter importantes implicações clínicas. OBJETIVO: Analisar as variáveis ecocardiográficas preditoras da remodelação no modelo do infarto em ratos. MÉTODOS: Os animais foram submetidos ao ecocardiograma em dois momentos, cinco dias e três meses após o infarto (grupo IAM) ou a cirurgia simulada (grupo controle). A regressão linear foi utilizada para identificar as variáveis ecocardiográficas no quinto dia posteriormente ao infarto, as quais foram preditoras de remodelação depois de três meses da oclusão coronariana. Consideramos, como critério de remodelação, neste estudo, os valores do diâmetro diastólico do ventrículo esquerdo (DDVE) após três meses do infarto. RESULTADOS: O infarto induziu o aumento das câmaras esquerdas, associado com alterações das funções sistólica e diastólica. As variáveis peso corporal, índice do estresse parietal ventricular esquerdo (IEPVE), área sistólica (AS), área diastólica (AD), DDVE, diâmetro sistólico do ventrículo esquerdo (DSVE), fração de variação de área (FVA), fração de ejeção (FE), porcentagem de encurtamento endocárdico (% Enc), velocidade de encurtamento da parede posterior (VEPP) e tamanho do infarto, avaliadas cinco dias depois do infarto, foram preditoras do DDVE após três meses. Na análise de regressão multivariada, incluímos o tamanho do infarto, o IEPVE e a VEPP. O IEPVE (coeficiente: 4,402, erro padrão: 2,221, p = 0,05), mas não o tamanho do infarto e a VEPP, foi fator preditor de remodelação após três meses do infarto. CONCLUSÃO: O IEPVE foi preditor independente de remodelação após três meses do infarto e poderia ser incorporado na estratificação clínica depois da oclusão coronariana.

FUNDAMENTO: A relevância do padrão de remodelamento no modelo de ratos infartados não é conhecida. OBJETIVO: Analisar a presença de diferentes padrões de remodelamento nesse modelo e suas implicações funcionais. MÉTODOS: Ratos infartados (n=46) foram divididos de acordo com o padrão de geometria, analisado pelo ecocardiograma: normal (índice de massa normal e espessura relativa normal), remodelamento concêntrico (índice de massa normal e espessura relativa aumentada), hipertrofia concêntrica (índice de massa aumentado e espessura relativa aumentada) e hipertrofia excêntrica (índice de massa aumentado e espessura relativa normal). Os dados estão em mediana e intervalo interquartil. RESULTADOS: Ratos infartados apresentaram apenas dois dos quatro padrões de geometria: padrão normal (15%) e hipertrofia excêntrica - HE (85%). Os grupos de padrão normal e HE não apresentaram diferenças nos valores de fração de variação de área (Normal = 32,1 - 28,8 a 50,7; HE = 31,3 - 26,5 a 36,7; p=0,343). Dos animais infartados, 34 (74%) apresentaram disfunção sistólica, detectada pela fração de variação de área. Considerando os dois padrões de geometria, 77% dos animais com hipertrofia excêntrica e 57% com geometria normal apresentaram disfunção sistólica (p=0,355). A espessura relativa da parede, os padrões de geometria e o índice de massa não foram fator de predição de disfunção ventricular (p>0,05). Uma das estratégias mais utilizadas para o estudo das alterações fisiopatológicas decorrentes da oclusão coronariana é o uso do modelo do infarto experimental no rato. Entre outros fatores, esse fato é decorrência da similaridade com as alterações fisiopatológicas que ocorrem após o infarto, em humanos. Devemos considerar, entretanto, que esse modelo possui características que podem dificultar tanto o uso como a interpretação de eventuais resultados. Assim, essa revisão tem como objetivo discutir as principais características do modelo do infarto experimental no rato, abordando a técnica da oclusão coronariana, as consequências e os métodos de avaliação morfológica e funcional do infarto e suas implicações clínicas.Una de las estrategias que se utiliza más en el estudio de las alteraciones fisiopatológicas resultantes de la oclusión coronaria es el uso del modelo del infarto experimental en el ratón. Entre otros factores, este hecho resulta de la similitud con las alteraciones fisiopatológicas que ocurren tras el infarto, en humanos. Sin embargo, debemos tener en cuenta que este modelo tiene características que pueden dificultar tanto el empleo como la interpretación de eventuales resultados. Así, esta revisión tiene por objeto discutir las principales características del modelo del infarto experimental en el ratón, y que se aborde la técnica de la oclusión coronaria, las consecuencias y los métodos de evaluación morfológicos y funcionales del infarto y sus implicaciones clínicas.One of the most often used strategies to study the physiopathological alterations caused by coronary occlusion is the use of the experimental infarction model in rats. Among other factors, this is due to the similarities in the physiopathological alterations that occur after the infarction in humans. One must consider, however, that this model has characteristics that can hinder the use as well as the interpretation of eventual outcomes. Thus, this review aims at analyzing the main characteristics of the experimental infarction model in rats, discussing the coronary occlusion technique, the consequences and the methods of morphological and functional assessment of the infarction and its clinical implications.

OBJETIVO: Analisar o papel da lipoperoxidação na remodelação ventricular após o infarto do miocárdio, em ratos suplementados com betacaroteno. MÉTODOS: Ratos foram infartados e distribuídos em dois grupos: C (controle) e BC (500mg/kg/dieta). Após seis meses, foram realizados ecocardiograma e avaliação bioquímica. Utilizamos o teste t, com significância de 5%. RESULTADOS: Os animais do grupo BC apresentaram maiores médias das áreas diastólicas (C = 1,57 ± 0,4 mm²/g, BC = 2,09 ± 0,3 mm²/g; p < 0,001) e sistólicas (C = 1,05 ± 0,3 mm²/g, BC = 1,61 ± 0,3 mm²/g; p < 0,001) do VE, ajustadas ao peso corporal do rato. A função sistólica do VE, avaliada pela fração de variação de área, foi menor nos animais suplementados com betacaroteno (C = 31,9 ± 9,3 %, BC = 23,6 ± 5,1 %; p = 0,006). Os animais suplementados com betacaroteno apresentaram valores maiores da relação E/A (C = 2,7 ± 2,5, BC = 5,1 ± 2,8; p = 0,036). OBJECTIVE: To analyze the role of lipoperoxidation in the ventricular remodeling after infarct of the myocardium in rats supplemented with beta-carotene. METHODS: Rats were infarcted and divided into two groups: C (control) and BC (500mg/kg/regimen). After six months, echocardiogram and biochemical evaluation were performed. The t test was used, with 5% significance. RESULTS: The animals from BC group presented highest means of the diastolic (C = 1.57 ± 0.4 mm²/g, BC = 2.09 ± 0.3 mm²/g; p < 0.001) and systolic (C = 1.05 ± 0.3 mm²/g, BC = 1.61 ± 0.3 mm²/g; p < 0.001) areas of LV, which were adapted according to the rat's body weight. The systolic function of LV, evaluated by the area variation fraction, was lower in the animals supplemented with beta-carotene (C = 31.9 ± 9.3%, BC = 23.6 ± 5.1%; p = 0.006). The animals supplemented with beta-carotene presented higher values of the E/A relation (C = 2.7 ± 2.5, BC = 5.1 ± 2.8; p = 0.036). No differences were found between the groups concerning the cardiac levels of the GSH (C = 21 ± 8 nmol/mg of protein, BC = 37 ± 15 nmol/mg of protein; p = 0.086), GSSG (C = 0.4 (0.3-0.5) nmol/g of protein, BC = 0.8 (0.4-1.0; p = 0.19) of protein; p = 0.246) and lipoperoxides (C = 0.4 ± 0.2 nmol/mg of tissue, BC = 0.2 ± 0.1 nmol/mg of tissue; p = 0.086). CONCLUSION: The highest remodeling in infarcted rats supplemented with beta-carotene does not depend on the lipoperoxidation.

OBJETIVO: Investigar a influência do propranolol na remodelação induzida pela exposição à fumaça de cigarro. MÉTODOS: Ratos foram alocados em três grupos: 1) C, n=10 - animais controle; 2) F, n=10 - animais expostos à fumaça de cigarro; 3) BB, n=10 - animais expostos à fumaça de cigarro e que receberam propranolol (40 mg/kg/dia). Após dois meses, os animais foram submetidos a estudo ecocardiográfico e morfométrico. Utilizou-se análise de variância (ANOVA) de uma via (média ± desvio padrão) ou Kruskal-Wallis (mediana e intervalo interquartil). RESULTADOS: O Grupo BB apresentou menor frequência cardíaca que o Grupo F (C = 358 ± 74 btm, F = 374 ± 53 bpm, BB = 297 ± 30; P = 0,02). O Grupo F apresentou maiores diâmetros diastólicos (C = 18,6 ± 3,4 mm/kg, F = 22,8 ± 1,8 mm/kg, BB = 21,7 ± 1,8 mm/kg; P = 0,003) e sistólicos (C = 8,6 ± 2,1 mmkg, F = 11,3 ± 1,3 mm/kg, BB = 9,9 ± 1,2 mm/kg; P = 0,004) do ventrículo esquerdo (VE), ajustado ao peso corporal (PC) e tendência de menor fração de ejeção (C = 0,90 ± 0,03, F = 0,87 ± 0,03, BB =0,90 ± 0,02; P = 0,07) que o Grupo C. O Grupo BB apresentou tendência de menor relação VE/PC que o Grupo F (C = 1,94 (1,87 - 1,97), F = 2,03 (1,9-2,1) mg/g, BB = 1,89 (1,86-1,94); P = 0,09). After 2 months, the animals underwent echocardiographic and morphometric analyses. One-way ANOVA (mean ± standard deviation) or the Kruskal-Wallis test (median and interquartile interval) was used. RESULTS: Group BB showed a lower heart rate than group F (C = 358 ± 74 bpm, F = 374 ± 53 bpm, BB = 297 ± 30; P = 0.02). Group F showed greater end-diastolic diameters (C = 18.6 ± 3.4 mm/kg, F = 22.8 ± 1.8 mm/kg, BB = 21.7 ± 1.8 mm/kg; P = 0.003) and left ventricular (LV) end-systolic diameters (C = 8.6 ± 2.1 mm/kg, F = 11.3 ± 1.3 mm/kg, BB = 9.9 ± 1.2 mm/kg; P = 0.004), adjusted for body weight (BW) and a tendency towards a lower ejection fraction (C = 0.90 ± 0.03, F = 0.87 ± 0.03, BB =0.90 ± 0.02; P = 0.07) than group C. Group BB showed a tendency towards a lower LV/BW ratio than group F (C = 1.94 (1.87 - 1.97), F = 2.03 (1.9-2.1) mg/g, BB = 1.89 (1.86-1.94); P = 0.09). CONCLUSION: Administration of propranolol attenuated some of the variables of ventricular remodeling induced by the exposure to tobacco smoke in rats.

OBJECTIVE: To evaluate the roles of oxidative stress and lipid peroxidation in the ventricular remodeling that is induced by tobacco smoke exposure after myocardial infarction. METHODS: After induced myocardial infarction, rats were allocated into two groups: C (control, n=25) and ETS (exposed to tobacco smoke, n=24). After 6 months, survivors were submitted to echocardiogram and biochemical analyses. RESULTS: Rats in the ETS group showed higher diastolic (C = 1.52 ± 0.4 mm², ETS = 1.95 �� 0.4 mm²; p=0.032) and systolic (C = 1.03 ± 0.3, ETS = 1.36 ± 0.4 mm²/g; p=0.049) ventricular areas, adjusted for body weight. The fractional area change was smaller in the ETS group (C = 30.3 ± 10.1 %, ETS = 19.2 ± 11.1 %; p=0.024) and E/A ratios were higher in ETS animals (C = 2.3 ± 2.2, ETS = 5.1 ± 2.5; p=0.037). ETS was also associated with a higher water percentage in the lung (C = 4.8 (4.3-4.8), ETS = 5.5 (5.3-5.6); p=0.013) as well as higher cardiac levels of reduced glutathione (C = 20.7 ± 7.6 nmol/mg of protein, ETS = 40.7 ± 12.7 nmol/mg of protein; p=0.037) and oxidized glutathione (C = 0.3 ± 0.1 nmol/g of protein, ETS = 0.9 ± 0.3 nmol/g of protein; p=0.008). No differences were observed in lipid hydroperoxide levels (C = 0.4 ± 0.2 nmol/mg of tissue, ETS = 0.1 ± 0.1 nmol/mg of tissue; p=0.08). CONCLUSION: In animals exposed to tobacco smoke, oxidative stress is associated with the intensification of ventricular remodeling after myocardial infarction.

Background: Heart failure prediction after acute myocardial infarction may have important clinical implications. Objective: To analyze the functional echocardiographic variables associated with heart failure in an infarction model in rats. Methods: The animals were divided into two groups: control and infarction. Subsequently, the infarcted animals were divided into groups: with and without heart failure. The predictive values were assessed by logistic regression. The cutoff values predictive of heart failure were determined using ROC curves. Results: Six months after surgery, 88 infarcted animals and 43 control animals were included in the study. Myocardial infarction increased left cavity diameters and the mass and wall thickness of the left ventricle. Additionally, myocardial infarction resulted in systolic and diastolic dysfunction, characterized by lower area variation fraction values, posterior wall shortening velocity, E-wave deceleration time, associated with higher values of E / A ratio and isovolumic relaxation time adjusted by heart rate. Among the infarcted animals, 54 (61%) developed heart failure. Rats with heart failure have higher left cavity mass index and diameter, associated with worsening of functional variables. The area variation fraction, the E/A ratio, E-wave deceleration time and isovolumic relaxation time adjusted by heart rate were functional variables predictors of heart failure. The cutoff values of functional variables associated with heart failure were: area variation fraction < 31.18%; E / A > 3.077; E-wave deceleration time < 42.11 and isovolumic relaxation time adjusted by heart rate < 69.08. Conclusion: In rats followed for 6 months after myocardial infarction, the area variation fraction, E/A ratio, E-wave deceleration time and isovolumic relaxation time adjusted by heart rate are predictors of heart failure onset.

OBJETIVO: Analisar os efeitos do betacaroteno no processo de remodelação ventricular após o infarto agudo do miocárdio (IAM), em ratos expostos à fumaça do cigarro. MÉTODOS: Após o IAM, os animais foram divididos em quatro grupos: 1) grupo C, 24 animais que receberam dieta-padrão; 2) grupo BC, 26 animais que receberam betacaroteno; 3) grupo EFC, 26 animais que receberam dieta-padrão e foram expostos à fumaça de cigarro; e 4) grupo BC+EFC, 20 animais que receberam betacaroteno e foram expostos à fumaça de cigarro. Após seis meses, foi realizado estudo morfofuncional. Utilizou-se significância de 5%. RESULTADOS: Em relação às áreas diastólicas (AD) e sistólicas (AS), os valores do grupo BC foram maiores que os do grupo C. Considerando a AD/peso corporal (PC) e AS/PC, os valores do grupo BC+EFC foram maiores que os valores de C. Em relação à fração de variação de área, foram observadas diferenças significativas entre EFC (valores menores) e C (valores maiores) e entre BC (valores menores) e C (valores maiores). Não foram observadas diferenças entre os grupos em relação ao tamanho do infarto. O grupo EFC apresentou valores maiores da área seccional dos miócitos (ASM) que os animais-controle. Em adição, o grupo BC+EFC apresentou maiores valores de ASM que BC, EFC e C. CONCLUSÃO: Após o infarto do miocárdio, o tabagismo e o betacaroteno promoveram intensificação do processo de remodelação cardíaca; houve potencialização dos efeitos deletérios no processo de remodelação com os dois tratamentos em conjunto.OBJECTIVE: To analyze the effects of beta-carotene on the ventricular remodeling process following myocardial infarction (MI) in rats exposed to cigarette smoke. METHODS: After acute myocardial infarction (AMI), the animals were divided into four groups: 1) Group C, 24 animals that were given standard diet; 2) Group BC, 26 animals that were given beta-carotene; 3) Group ECS, 26 animals that were given standard diet and were exposed to cigarette smoke; and 4) Group BC+ECS, 20 animals that were given beta-carotene and were exposed to cigarette smoke. After six months, a morphofunctional study was performed. We used a 5% significance level. RESULTS: As regards diastolic areas (DA) and systolic areas (SA), the values for the BC group were higher than those for the C group. If DA/body weight (BW) and SA/BW are considered, the values for group BC+ECS were higher than the values for group C. As regards the fractional area change, we observed significant differences between ECS (lower values) and C (higher values) and between BC (lower values) and C (higher values). Differences between groups regarding infarction size were not observed. The ECS group presented higher values for myocyte cross-section area (MCA) than control animals. Additionally, the BC+ECS group presented higher MCA values than the BC, ECS and C groups. CONCLUSION: After myocardial infarction, smoking and beta-carotene intensified the heart remodeling process; harmful effects of the remodeling process were heightened when the two treatments were used in conjunction.

OBJETIVO: Avaliar as diferenças entre três métodos para medida do infarto experimental em ratos, em relação ao método tradicional. MÉTODOS: A área infartada por histologia (AREA), o perímetro interno da cavidade infartada por histologia (PER) e o perímetro interno por ecocardiograma (ECO) foram comparados ao método tradicional (análise histológica das circunferências epicárdicas e endocárdicas da região infartada - CIR). Utilizaram-se ANOVA de medidas repetidas, complementada com o teste de comparações múltiplas de Dunn, o método de concordância de Bland & Altman e o teste de correlação de Spearman. A significância foi p < 0,05. RESULTADOS: Foram analisados dados de 122 animais, após 3 a 6 meses do infarto. Houve diferença na avaliação do tamanho do infarto entre CIR e os outros três métodos (p < 0,001): CIR = 42,4% (35,9-48,8), PER = 50,3% (39,1-57,0), AREA = 27,3% (20,2-34,3), ECO = 46,1% (39,9-52,6). Assim, a medida por área resultou em subestimação de 15% do tamanho do infarto, enquanto as medidas por ecocardiograma e pelo perímetro interno por meio de histologia resultaram em superestimação do tamanho do infarto de 4% e 5%, respectivamente. Em relação ao ECO e PER, apesar de a diferença entre os métodos ser de apenas 1,27%, o intervalo de concordância variou de 24,1% a -26,7%, sugerindo baixa concordância entre os métodos. Em relação às associações, houve correlações estatisticamente significativas entre: CIR e PER (r = 0,88 e p < 0,0001); CIR e AREA (r = 0,87 e p < 0,0001) e CIR e ECO (r = 0,42 e p < 0,0001). CONCLUSÃO: Na determinação do tamanho do infarto, apesar da alta correlação, houve baixa concordância entre os métodos.OBJECTIVE: To evaluate the differences between three methods for the measurement of experimental infarction in rats in comparison to the traditional method. METHODS: Histological analysis of the infarction area (AREA), histological analysis of the internal cavity perimeter (PER) and echocardiogram analysis of the internal perimeter (ECHO) were compared to the traditional method (histological analysis of the epicardial and endocardial circumferences of the infarction region - CIR). Repeated ANOVA measurements were used in conjunction with the Dunn multiple comparison test, the Bland and Altman concordance method and the Spearman correlation test. Significance was established as p < 0.05. RESULTS: The data of 122 animals were analyzed, 3 to 6 months after the infarction. Infarction size assessments revealed differences between CIR and the other three methods (p < 0.001): CIR = 42.4% (35.9-48.8), PER = 50.3% (39.1-57.0), AREA = 27.3% (20.2-34.3), ECHO = 46.1% (39.9-52.6). Therefore, measurement by area underestimated the infarct size by 15%, whereas the echocardiogram and histological internal perimeter measurements overestimated the infarct size by 4% and 5%, respectively. In relation to ECHO and PER, even though the difference between the methods was only 1.27%, the concordance interval ranged from 24.1% to -26.7%, suggesting a low level of concordance between the methods. In relation to associations, statistically significant correlations were found between: CIR and PER (r = 0.88 and p < 0.0001); CIR and AREA (r = 0.87 and p < 0.0001) and CIR and ECHO (r = 0.42 and p < 0.0001). CONCLUSION: Despite the high level of correlation, there was a low level of concordance between the methods to define infarct size.

OBJETIVO: Analisar os efeitos da exposição à fumaça de cigarro (EFC) na remodelação ventricular após o infarto agudo do miocárdio (IAM). MÉTODOS: Ratos foram infartados e distribuídos em dois grupos: C (controle, n = 31) e F (EFC: 40 cigarros/dia, n = 22). Após seis meses, foi realizado ecocardiograma, estudo funcional com coração isolado e morfometria. Para comparação, foi utilizado o teste t (com média ± desvio padrão) ou teste de Mann-Whitney (com mediana e percentis 25 e 75). RESULTADOS: Os animais EFC apresentaram tendência a maiores áreas ventriculares diastólicas (C = 1,5 ± 0,4 mm², F = 1,9 ± 0,4 mm²; p = 0,08) e sistólicas (C = 1,05 ± 0,3 mm², F = 1,32 ± 0,4 mm²; p = 0,08) do VE. A função sistólica do VE, avaliada pela fração de variação de área, tendeu a ser menor nos animais EFC (C = 31,9 ± 9,3 %, F = 25,5 ± 7,6 %; p = 0,08). Os valores da - dp/dt dos animais EFC foram estatisticamente inferiores (C = 1474 ± 397 mmHg, F = 916 ± 261 mmHg; p = 0,02) aos animais-controle. Os animais EFC apresentaram maior peso do VD, ajustado ao peso corporal (C = 0,8 ± 0,3 mg/g, F = 1,3 ± 0,4 mg/g; p = 0,01), maior teor de água nos pulmões (C = 4,8 (4,3-4,8)%, F = 5,4 (5,1-5,5); p = 0,03) e maior área seccional do miócito do VE (C = 239,8 ± 5,8 µm², F = 253,9 ± 7,9 µm²; p = 0,01). CONCLUSÃO: A exposição à fumaça de cigarro intensifica a remodelação ventricular após IAM.OBJECTIVE: To evaluate the role of cigarette smoke exposure (CSE) on ventricular remodeling following acute myocardial infarction (AMI). METHODS: Rats were submitted to myocardial infarction and divided into two groups: C (control, n = 31) and F (CSE: 40 cigarettes/day, n = 22). After 6 months, the survivors were submitted to echocardiogram, functional study with isolated heart, and morphometric analysis. For comparison purposes, we used the t test (mean ± standard deviation) or the Mann-Whitney test (with median and 25th and 75th percentiles). RESULTS: The CSE animals tended to have larger diastolic (C = 1.5 ± 0.4 mm², F = 1.9 ± 0.4 mm²; p = 0.08) and systolic (C = 1.05 ± 0.3 mm², F = 1.32 ± 0.4 mm²; p = 0.08) left ventricular(LV) areas. The systolic function of the LV, assessed according to the fractional area change, tended to be impaired in CSE animals (C = 31.9 ± 9.3%, F = 25.5 ± 7.6%; p = 0.08). The - dp/dt values for CSE animals were statistically lower (C = 1474 ± 397 mmHg, F = 916 ± 261 mmHg; p = 0.02) than for control animals. The CSE animals presented higher right ventricle (RV) weight adjusted for body weight (C = 0.8 ± 0.3 mg/g, F = 1.3 ± 0.4 mg/g; p = 0.01), higher content of water in lungs (C = 4.8 (4.3-4.8)%, F = 5.4 (5.1-5.5); p = 0.03), and larger LV myocyte cross-sectional areas (C = 239.8 ± 5.8 µm², F = 253.9 ± 7.9 µm²; p = 0.01). CONCLUSION: Cigarette smoke exposure intensifies ventricular remodeling following acute myocardial infarction.

OBJETIVO: Determinar as alterações cardíacas estruturais e funcionais causadas pela exposição à fumaça do cigarro em ratos. MÉTODOS: Os animais foram aleatoriamente distribuídos em dois grupos: fumante (F), composto por 10 animais, expostos à fumaça do cigarro, na taxa de 40 cigarros/dia e controle (C), constituído por 10 animais não submetidos à exposição. Após 4 meses, os animais foram submetidos a estudo morfológico e funcional por meio do ecocardiograma. As variáveis estudadas foram analisadas pelo teste t ou pelo teste de Mann-Whitney. Background/Aims: This study aimed to discern whether the cardiac alterations caused by retinoic acid (RA) in normal adult rats are physiologic or pathologic. Methods and Results: Wistar rats were assigned into four groups: control animals (C, n = 20) received a standard rat chow; animals fed a diet supplemented with 0.3 mg/kg/day all-trans-RA (AR1, n = 20); animals fed a diet supplemented with 5 mg/kg/day all-trans-RA (AR2, n = 20); and animals fed a diet supplemented with 10 mg/kg/day all-trans-RA (AR3, n = 20). After 2 months, the animals were submitted to echocardiogram, isolated heart study, histology, energy metabolism status, oxidative stress condition, and the signaling pathway involved in the cardiac remodeling induced by RA. RA increased myocyte cross-sectional area in a dose-dependent manner. The treatment did not change the morphological and functional variables, assessed by echocardiogram and isolated heart study. In contrast, RA changed catalases, superoxide dismutase, and glutathione peroxidases and was associated with increased values of lipid hydroperoxide, suggesting oxidative stress. RA also reduced citrate synthase, enzymatic mitochondrial complex II, ATP synthase, and enzymes of fatty acid metabolism and was associated with increased enzymes involved in glucose use. In addition, RA increased JNK 1/2 expression, without changes in TGF-β, PI3K, AKT, NFκB, S6K, and ERK. Conclusion: In normal rats, RA induces cardiac hypertrophy in a dose-dependent manner. The non-participation of the PI3K/Akt pathway, associated with the participation of the JNK pathway, oxidative stress, and changes in energy metabolism, suggests that cardiac remodeling induced by RA supplementation is deleterious.

A deficiência de tiamina pode apresentar quatro formas clássicas de apresentação clínica: polineuropatia periférica, anorexia e fraqueza muscular (beribéri seco); insuficiência cardíaca de alto débito com sinais congestivos (beribéri úmido); beribéri associado ao choque (Shoshin beribéri) e encefalopatia de Wernicke. Neste relato, descrevemos quadro sugestivo de hipertensão pulmonar grave e cor pulmonale, com estase jugular, hepatite congestiva e edema generalizado, que apresentou reversão completa após administração de tiamina.

Abstract Cardiac remodeling is defined as a group of molecular, cellular and interstitial changes that manifest clinically as changes in size, mass, geometry and function of the heart after injury. The process results in poor prognosis because of its association with ventricular dysfunction and malignant arrhythmias. Here, we discuss the concepts and clinical implications of cardiac remodeling, and the pathophysiological role of different factors, including cell death, energy metabolism, oxidative stress, inflammation, collagen, contractile proteins, calcium transport, geometry and neurohormonal activation. Finally, the article describes the pharmacological treatment of cardiac remodeling, which can be divided into three different stages of strategies: consolidated, promising and potential strategies.

Na Faculdade de Medicina de Botucatu (FMB), os cursos de clínica médica (CM) vêm sendo tradicionalmente ministrados nas enfermarias, ambulatórios, UTI e pronto-socorro do Hospital das Clínicas da FMB, sob a supervisão do Departamento de CM. Em outubro de 2002, a FMB passou a gerenciar o Hospital Estadual Bauru (HEB), tornando possível a transferência de parte do ensino de CM no internato para hospital secundário. Desde janeiro de 2004, grupos de internato, compostos por oito alunos, passaram a estagiar em enfermaria de clínica geral do HEB, sob a supervisão de preceptores da disciplina de CM geral. Entre as vantagens decorrentes dessa mudança, citamos a possibilidade de atender doentes com patologias mais simples, maior responsabilidade com o paciente, maior contato com o preceptor e o aprendizado sobre a necessidade de cumprir metas estabelecidas pelo hospital. A principal dificuldade na execução do estágio é a sobrecarga de trabalho para os preceptores, que é causada, em parte, pela ausência de residentes na enfermaria. Consideramos que a experiência em transferir parte do internato de CM para hospital secundário foi bem-sucedida e esta pode vir a estimular outras áreas da FMB para a busca de novos cenários de ensino.

OBJECTIVE: The aim of this study was to evaluate the roles of the Taql and Bsml vitamin D receptor gene polymorphisms in hospital mortality of burn patients. METHODS: In total, 105 consecutive burn injury patients over 18 years in age who were admitted to the Burn Unit of Bauru State Hospital from January to December 2013 were prospectively evaluated. Upon admission, patient demographic information was recorded and a blood sample was taken for biochemical analysis to identify the presence of the Taql(rs731236) and Bsml(rs1544410) polymorphisms. All of the patients were followed over their hospital stay and mortality was recorded. RESULTS: Eighteen of the patients did not sign the informed consent form, and there were technical problems with genotype analysis for 7 of the patients. Thus, 80 patients (mean age, 42.5±16.1 years) were included in the final analysis. In total, 60% of the patients were male, and 16.3% died during the hospital stay. The genotype frequencies for the Taql polymorphism were 51.25% TT, 41.25% TC and 7.50% CC; for the Bsml polymorphism, they were 51.25% GG, 42.50% GA and 6.25% AA. In logistic regression analysis, after adjustments for age, gender and total body surface burn area, there were no associations between the Taql (OR: 1.575; CI95%: 0.148-16.745; p=0.706) or Bsml (OR: 1.309; CI95%: 0.128-13.430; p=0.821) polymorphisms and mortality for the burn patients. CONCLUSIONS: Our results suggest that the Taql and Bsml vitamin D receptor gene polymorphisms are not associated with hospital mortality of burn patients.

Resumo Fundamento O suco de laranja (SL) é rico em polifenóis com propriedades anti-inflamatórias e antioxidantes. Após o infarto do miocárdio (IM), mudanças complexas ocorrem na estrutura e na função cardíacas, processo conhecido como remodelação cardíaca (RC). O estresse oxidativo e a inflamação podem modular esse processo. Nossa hipótese foi a de que o consumo de SL atenua a RC após o IM. Objetivos Avaliar a influência do SL sobre a RC após IM pela análise de variáveis funcionais, morfológicas, de estresse oxidativo, de inflação, e de metabolismo energético. Métodos Um total de 242 ratos machos pesando entre 200 e 250g foram submetidos a um procedimento cirúrgico (ligação da artéria coronária ou cirurgia simulada). Sete dia após a cirurgia, os animais sobreviventes foram divididos para um dos quatro grupos: 1) SM, animais sham que receberam água e maltodextrina (n= 20); 2) SSL, animais sham que receberam SL (n= 20); 3) IM, animais infartados que receberam água e maltodextrina (n= 40); e 4) ISL, animais infartados que receberam SL (n = 40). A análise estatística foi realizada pelo teste de ANOVA com dois fatores com o teste de Holm-Sidak. Os resultados foram apresentados em média ± desvio padrão, e o nível de significância adotado foi de 5%. Resultados Três meses depois, o IM levou à hipertrofia do ventrículo esquerdo (VE), com disfunção sistólica e diastólica, e aumento nos mediadores inflamatórios e de estresse oxidativo. Os animais que consumiram SL apresentaram menor atividade da glutationa peroxidase e maior expressão da heme-oxigenase-1 (HO-1). Conclusão O SL atenuou a RC, e a HO-1 pode exercer um importante papel nesse processo.
