Background To demonstrate and compare results obtained with the transtibial double-bundle posterior cruciate ligament (PCL) reconstruction technique using two types of autografts in isolated and combined PCL lesions. Methods Fifty-two patients with isolated or combined PCL injuries underwent double-bundle PCL reconstruction and were retrospectively evaluated. Among them, 34 were reconstructed using ipsilateral quadriceps and semitendinosus tendon grafts, and 18 were reconstructed using bilateral hamstring tendon grafts. The criteria for outcome evaluation were: objective International Knee Documentation Committee (IKDC) score, Lysholm score, posterior stability (posterior drawer test and KT-1000TM), and rate of complications, comparing the two types of grafts and comparing isolated PCL and combined lesions. The minimum follow-up was two years. Results Significant improvements were found in all evaluation methods between the pre- and postoperative periods (all P .05). The whole sample had the following results: objective IKDC score, 96.2% classified as A/B; Lysholm score, 98.1% rated as good or excellent; and KT-1000, 98.1% with a side-to-side difference of up to five millimeters (65.4% with 0 to two millimeters). The complication rate was 9.6%, with no differences between the graft type that was used (P = .585). No significant difference was observed when comparing the results between isolated PCL injuries and combined injuries (all P > .05). Conclusions The proposed PCL reconstruction technique presented satisfactory results in both isolated and combined PCL lesions, and there were no differences between different types of autografts used. ResumoObjetivoApresentar os resultados de uma série de casos de reconstrução do ligamento cruzado posterior (LCP) em dupla banda com o uso dos tendões flexores autólogos, com seguimento mínimo de dois anos.MétodosAvaliação de 16 casos de lesão do LCP submetidos a reconstrução em dupla banda com tendões flexores autólogos entre 2011 e 2013. A amostra final foi composta por 16 pacientes, 15 homens e uma mulher, com média de 31 anos (21‐49). O mecanismo predominante foi acidente motociclístico em metade dos casos. Houve um intervalo médio de 15 meses entre a lesão e a cirurgia (três a 52 meses). Cinco lesões eram isoladas e 11, associadas. Foram feitas avaliação clínica, aplicação de escores validados e mensuração com uso do artrômetro KT‐1000.ResultadosA avaliação pela escala de Lysholm pré‐operatória teve média de 55 pontos (28‐87), evoluiu para uma média pós‐operatória de 94 pontos (85‐100). O IKDC também demonstrou melhoria. Na avaliação pré‐operatória, quatro e 12 pacientes foram respectivamente classificados como C (anormal) e D (muito anormal); na avaliação pós‐operatória, seis foram classificados como A (normal) e dez como B (próximo ao normal). Na avaliação pós‐operatória pelo artrômetro KT1000, 13 pacientes apresentaram diferença entre 0‐2mm e três, entre 3‐5mm, na comparação com o lado contralateral.ConclusãoO uso dos tendões flexores autólogos é uma opção viável na reconstrução do LCP em dupla banda, apresenta bons resultados clínicos em seguimento mínimo de dois anos.AbstractObjectiveTo present the outcomes of posterior cruciate ligament (PCL) double‐bundle reconstruction using autologous hamstring tendons, with a minimum follow‐up of two years.MethodsEvaluation of 16 cases of PCL injury that underwent double‐bundle reconstruction with autogenous hamstring tendons, between 2011 and 2013. The final sample consisted of 16 patients, 15 men and one woman, with a mean age of 31 years (21‐49). The predominant mechanism was motorcycle accident in half of the cases. There was a mean interval of 15 months between the time of lesion and the surgery (three to 52 months). Five lesions were isolated and 11, associated. Clinical evaluation, application of validated scores, and measurements with use of the KT‐1000 were performed.ResultsThe analysis showed a mean preoperative Lysholm score of 50 points (28‐87), progressing to 94 points (85‐100) postoperatively. The IKDC score also demonstrated improvement. In the preoperative evaluation, four and 12 patients were respectively classified as C (abnormal) and D (very unusual), and in the postoperative evaluation six as A (normal) and ten as B (close to normal). In the post‐operative evaluation by KT1000 arthrometer, 13 patients showed difference between 0‐2mm and 3 between 3‐5mm, when compared with the contralateral side.ConclusionAutologous hamstring tendons are a viable option in double‐bundle reconstruction of the PCL, with good clinical results in a minimum follow‐up of two years.

Objectives: To describe the microscopic anatomy of the posterior cruciate ligament femoral insertion in order to identify and establish differences between the direct and indirect insertions of this ligament. Methods: Ten cadaveric knees were used for this study. The posterior cruciate ligament femoral insertion was observed microscopically. Hematoxylin and eosin staining was performed to observe the morphology of the posterior cruciate ligament insertion. Alcian blue staining was performed to determine the location of the cartilage matrix and better assist in the observation and differentiation between direct and indirect insertions. Results: The direct insertion was observed to be a more complex structure than the indirect insertion because it showed four different histological layers (ligament, uncalcified fibrocartilage, calcified fibrocartilage, and bone). Chondrocytes were observed in the uncalcified and calcified fibrocartilage layers. It was observed that the indirect insertion was composed of two layers in which the ligament was anchored directly to the bone by collagen fibers. Indirect insertion was located in the marginal region of the posterior cruciate ligament between the direct insertion and the anterior articular cartilage. Conclusion: Through histological analysis, it was demonstrated that the indirect insertion was adjacent to the anterior articular cartilage and presents a histological pattern where the collagen fibers insert directly into the bone (two-layer insertion). The direct insertion is posterior to the indirect insertion and has four histologically distinct layers. RESUMO Objetivos: Descrever a anatomia microscópica da inserção femoral do ligamento cruzado posterior a fim de identificar e estabelecer diferenças entre as inserções direta e indireta desse ligamento. Métodos: Foram usados dez joelhos procedentes de amputações transfemorais. A inserção femoral do ligamento cruzado posterior foi observada microscopicamente. A coloração hematoxilina e eosina foi feita para observar a morfologia da inserção do ligamento cruzado posterior. A coloração azul de Alcian foi feita para determinar a localização da matriz de cartilagem e melhor ajudar na observação e diferenciação entre a inserção direta e indireta. Resultados: Observou-se que a inserção direta do ligamento cruzado posterior é uma estrutura mais complexa do que a inserção indireta, por apresentar quatro camadas histológicas distintas (ligamento, fibrocartilagem não calcificada, fibrocartilagem calcificada e osso). Os condrócitos foram observados nas camadas não calcificadas e calcificadas de fibrocartilagem. Foi observado que a inserção indireta, composta de duas camadas nas quais o ligamento está inserido diretamente ao osso por fibras de colágeno, está localizada na região marginal do ligamento cruzado posterior entre a inserção direta e a borda da cartilagem anterior do côndilo. Conclusão: Através de análise histológica, o presente estudo demonstrou que a inserção indireta do ligamento cruzado posterior situa-se adjacente à borda da cartilagem anterior do côndilo femoral e apresenta um padrão histológico no qual as fibras de colágeno se inserem diretamente no osso. A inserção direta encontra-se posterior à inserção indireta e apresenta quatro camadas histológicas distintas.

Introducao/Objetivos : A osteocondrite dissecante e uma lesao que acomete o osso subcondral do joelho com efeitos secundarios sobre a cartilagem articular. Nao ha consenso na literatura sobre as melhores opcoes de tratamento. Reportamos o caso de um paciente submetido a fixacao com cavilhas osseas corticais autologas, por via aberta. Relato de Caso : Paciente de 16 anos de idade, sexo masculino, com queixa de dor e derrame articular no joelho esquerdo, exacerbados por atividades fisicas, havia dez meses, com diagnostico de osteocondrite dissecante em sua localizacao mais comum (face lateral do condilo femoral medial). Paciente foi submetido a tratamento cirurgico com reducao do fragmento e fixacao por via aberta, atraves de cavilhas osseas corticais retiradas da regiao anteromedial da tibia. Resultados : Paciente evoluiu bem, com cicatrizacao da lesao osteocondral, e boa funcao no pos-operatorio. A fixacao com parafusos metalicos permite compressao dos fragmentos, o que favorece a consolidacao; entretanto, e necessario a retirada do material apos oito semanas. Parafusos biodegradaveis nao precisam ser retirados, mas podem evoluir com sinovite asseptica por reacao de corpo estranho. A fixacao com cavilhas osseas corticais autologas, que dispensa a utilizacao de material de sintese, tem resultados satisfatorios na literatura, e e uma tecnica simples, nao agressiva, que diminui os custos do procedimento, e que vem sendo menos utilizada nos ultimos anos, em detrimento de tecnicas mais moderna e mais caras. Conclusao : Este caso enfatiza a eficacia da fixacao com cavilhas osseas no tratamento da osteocondrite dissecante do joelho. Descritores : Joelho, Osteocondrite, Osteocondrite dissecante Abstract Introduction/Purpose : Osteochondritis dissecans is a lesion that affects the subchondral bone of the knee with secondary effects on joint cartilage. A consensus is yet to be reached on literature about the best treatment option. This case report consists of patient that underwent an open surgery with autologous cortical bone sticks fixation. Case report: 16-year-old patient, male, with pain and joint effusion on the left knee, that gets worse with physical activity, since ten months, with osteochondritis dissecans diagnosis located in the most usual place, in the lateral portion of the medial femoral condyle. Patient went through an open surgery that fixed the fragment with cortical bone sticks withdrawn from anteromedial portion of the tibia. Results : Patient resulted in full recovery, with healing of the osteochondral lesion and good post-operative function. The fixation with metal screws causes compression of the fragments, which is beneficial for the consolidation; however, it is necessary to remove the material after eight weeks. Biodegradable screws don’t need to be removed, but can progress with aseptic synovitis by foreign body reaction. Autologous cortical bone sticks fixation, which does not require the use of synthetic material, has satisfactory results in the literature, and it is a simple, non-aggressive technique that reduces the surgery costs. However, in the recent years, it has been less used in detriment of more modern and expensive techniques. Conclusion : This case report emphasizes the effectiveness of fixation with bone sticks on the treatment of osteochondritis dissecans of the knee. Objective The objective of this study was to evaluate the results obtained from posterior cruciate ligament (PCL) reconstruction with the double femoral tunnel technique, using quadriceps tendon and semitendinous autograft, in patients with isolated PCL tears or PCL tears associated with other ligament lesions, 2years after surgery. Methods The study included 14 patients with isolated PCL lesions and 20 with combined ligament lesions, who underwent PCL reconstruction using the double femoral tunnel technique and were evaluated 24months after surgery using the International Knee Documentation Committee (IKDC) and Lysholm scores, a KT-1000 arthrometer, and the graduation of the posterior drawer test. Results Knees were considered normal ("A") or nearly normal ("B") in 92.9% of patients with isolated lesions and in 95.0% of patients with combined tears, according to the IKDC score. Good or excellent results were obtained in 100% of patients in both groups according to the Lysholm score. Absence or outstanding reduction of posterior tibial translation was seen in 92.9% of patients with isolated lesions and in 100% of patients with combined ligament tears, in the posterior drawer evaluation. Conclusion The PCL double femoral tunnel reconstruction technique using autografts was effective in restoring posterior knee stability, in isolated and/or combined PCL tears, showing remarkable clinical improvement in all patients. OBJETIVO: Identificar radiograficamente alterações anatômicas e da maturidade esquelética em paciente com epifisiólise do quadril, que auxiliem na indicação do tratamento cirúrgico profilático dos quadris não escorregados, no mesmo paciente. MÉTODOS: Foi realizada uma avaliação radiográfica retrospectiva com 61 pacientes, acompanhados até o final do crescimento, divididos em dois grupos: 37 com escorregamento unilateral e 24 com escorregamento bilateral, diagnosticados durante o acompanhamento. A maturidade esquelética foi avaliada nas radiografias da bacia (método de Oxford) e comparada entre os grupos, nos pacientes do mesmo sexo. Além disso, diversos ângulos descritos para a região proximal do fêmur foram medidos e comparados: ângulo de Southwick (nas incidências ântero-posterior e Lauenstein), ângulo de inclinação da fise e ângulo de sloping posterior da fise. RESULTADOS: A maturidade esquelética mostrou diferença estatisticamente significante entre os grupos, em ambos os sexos. Com relação aos ângulos avaliados, o de Southwick no perfil mostrou-se matematicamente igual ao de "sloping " posterior, os únicos a mostrar diferenças relevantes. CONCLUSÃO: O método de Oxford e o ângulo de Southwick no perfil podem ser utilizados como parâmetros para uma melhor indicação do tratamento profilático do quadril contralateral em pacientes com epifisiolise. Objective To identify anatomical changes and skeletal maturity through radiographic analysis, allowing more accuracy for indication of surgical management of non-slipped hips in patients with epiphysiolisys. Method A retrospective study of the radiographs of 61 patients followed until the end of skeletal growth, assigned to two groups: 37 patients with unilateral epiphysiolysis, and 24 patients with contralateral epiphysiolysis diagnosed during follow-up. The skeletal maturity was evaluated using pelvis radiographs (Oxford method) and compared between the groups for patients of the same gender. In addition, the Southwick angle (in anteroposterior and in Lauenstein view), physeal sloping angle and physeal posterior sloping angle were compared as well. Results Skeletal maturity showed a statistically significant difference between the two groups for both genders. It was observed that the lateral view of the Southwick angle is mathematically equal to the physeal posterior sloping angle, and were the only ones to show relevant differences between the groups. Conclusion The Oxford method and the Southwick angle in Lauenstein view can be utilized as parameters to help the physician to better indicate the prophylactic surgical treatment of the contralateral hip, in patients with slipped capital femoral epiphysis (SCFE).
