Abstract

A correct measurement of the water pore pressure is essential to obtain reliable CPT results. Although this has always been a major concern, it still remains a possible relevant source of error in common engineering practice. In addition to usual reasons, such as malfunctioning of the equipment, its poor calibration and/or maintenance, a lack of complete saturation of the piezocone tip can play a major role. In fact, a correct and timely pore pressure measurement relies entirely on the full saturation of the pore pressure system. The variety of saturation fluids and methods that is encountered in practice is a clear evidence of the still existing uncertainties on the most suitable procedure. Furthermore, unless the saturation process is explicitly detailed, its choice is typically left to the contractor and the measurement quality can only be established a posteriori, after the test has been completed. This paper describes a novel experimental equipment that enables quantifying the degree of saturation of the piezocone tip prior to testing. The methodology proposed is inspired to the B-value check, which is routinely used in geotechnical laboratory testing to assess the degree of saturation, e.g. prior to triaxial testing. In addition, it could also be applied after a test, so that it would be possible to assess whether saturation was retained while testing. This promising approach, when extended to different saturation procedures and validated by engineering practice, has the potential to become a relevant benchmark for reliable CPT testing.

Contains fulltext : 220899.pdf (publisher's version ) (Open Access), Conflicting results have been reported so far in pooled analyses and studies evaluating the optimum duration of dual antiplatelet therapy (DAPT) in acute coronary syndrome (ACS) patients. However, randomized clinical trials dedicated to this specific setting of higher thrombotic risk patients have only recently been completed, pointing at the noninferiority of a shorter strategy as compared to the traditional 12-month DAPT, furthermore allowing to reduce the risk of major bleeding complications. Therefore, a reconsideration of current clinical practice and guidelines should be certainly be advocated in light of the most recent updates, especially among ACS patients treated with percutaneous coronary intervention (PCI) and modern drug-eluting stents (DES). Our aim was to provide a comprehensive review of the available evidence on the optimal DAPT duration in ACS patients.

Abstract

Aims Patients with acute coronary syndromes (ACSs) who are managed without coronary revascularization represent a mixed and understudied population that seems to receive suboptimal pharmacological treatment. Methods and results We assessed patterns of antithrombotic therapies employed during the hospitalization and in-hospital clinical events of medically managed patients with ACS enrolled in the prospective, multicentre, nationwide EYESHOT (EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units) registry. Among the 2585 consecutive ACS patients enrolled in EYESHOT, 783 (30.3%) did not receive any revascularization during hospital admission. Of these, 478 (61.0%) underwent coronary angiography (CA), whereas 305 (39.0%) did not. The median GRACE and CRUSADE risk scores were significantly higher among patients who did not undergo CA compared with those who did (180 vs. 145, P < 0.0001 and 50 vs. 33, P < 0.0001, respectively). Antithrombotic therapies employed during hospitalization significantly differ between patients who received CA and those who did not with unfractioned heparin and novel P2Y12 inhibitors more frequently used in the first group, and low-molecular-weight heparins and clopidogrel in the latter group. During the index hospitalization, patients who did not receive CA presented a higher incidence of ischaemic cerebrovascular events and of mortality compared with those who underwent CA (1.6 vs. 0.2%, P = 0.04 and 7.9 vs. 2.7%, P = 0.0009, respectively). Conclusion Almost one-third of ACS patients are managed without revascularization during the index hospitalization. In this population, a lower use of recommended antiplatelet therapy and worse clinical outcome were observed in those who did not undergo CA when compared with those who did. Abstract

Aims In the last decades, several new therapies have emerged for the treatment of acute coronary syndromes (ACS). We sought to describe real-world patterns of use of antithrombotic treatments in the catheterization laboratory for ACS patients undergoing percutaneous coronary interventions (PCI). Methods EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units was a nationwide, prospective registry aimed to evaluate antithrombotic strategies employed in ACS patients in Italy. Results Over a 3-week period, a total of 2585 consecutive ACS patients have been enrolled in 203 cardiac care units across Italy. Among these patients, 1755 underwent PCI (923 with ST-elevation myocardial infarction and 832 with non-ST-elevation ACS). In the catheterization laboratory, unfractioned heparin was the most used antithrombotic drug in both ST-elevation myocardial infarction (64.7%) and non-ST-elevation ACS (77.5%) undergoing PCI and, as aspirin, bivalirudin and glycoprotein IIb/IIIa inhibitors (GPIs) more frequently employed before or during PCI compared with the postprocedural period. Any crossover of heparin therapy occurred in 36.0% of cases, whereas switching from one P2Y12 inhibitor to another occurred in 3.7% of patients. Multivariable analysis yielded several independent predictors of GPIs and of bivalirudin use in the catheterization laboratory, mainly related to clinical presentation, PCI complexity and presence of complications during the procedure. Conclusion In our contemporary, nationwide, all-comers cohort of ACS patients undergoing PCI, antithrombotic therapies were commonly initiated before the catheterization laboratory. In the periprocedural period, the most frequently employed drugs were unfractioned heparin, leading to a high rate of crossover, followed by GPIs and bivalirudin, mainly used during complex PCI. Abstract

Currently, two main lead configurations are used for implantable cardioverter-defibrillators (ICD). One generates a monodirectional electrical vector by using the can surface as an active part (hot can) together with a right ventricular defibrillation coil. The other one (TRIAD) produces a bidirectional electrical vector by adding a proximal defibrillation electrode on the same lead. The purpose of this prospective study was to determine whether there is a difference between these configurations in terms of the acute defibrillation threshold (DFT). The secondary objective was to evaluate the possible sequential effect of successive arrhythmia induction and defibrillation shocks on the final DFT value.In 44 patients (37 males, 7 females, mean age 59.18 +/- 12.05 years; mean ejection fraction 35.21 +/- 11.69%), a Hot Can Ventak family ICD (Guidant, St. Paul, MN, USA) was implanted in a left pectoral pocket. During the implant procedure, step-down to failure DFT testing was performed twice in each patient using the two different above-mentioned configurations: the bidirectional and the monodirectional. The first configuration to be tested was determined by a 1:1 randomization by center.The step-down DFT protocol was followed in 35 patients. The average DFT was 8.6 +/- 4.0 J for TRIAD and 10.4 +/- 4.3 J for the monodirectional (p = 0.009) lead configuration; this represents a 16.3% decrease in the DFT using a bidirectional configuration. Furthermore, no relationship between the final DFT and the number of ventricular fibrillation inductions and shocks received was observed, confirming the secondary objective.Compared to the monodirectional electrical vector, the bidirectional electrical vector is clearly more beneficial for the patient.

40. Effect of aging and degeneration on the human intervertebral disc during the diurnal cycle: A finite element study

Abstract

Alterations in the major biochemical constituents of intervertebral discs coincide with aging and degeneration, and can alter the disc's ability to support load. The most significant biochemical change that occurs in degeneration is the loss of proteoglycans in the nucleus pulposus. During a diurnal cycle, the disc experiences approximately 16 hours of functional loading, followed by 8 hours of recovery. An axisymmetric, poroelastic model was created using ABAQUS finite element software. Standard poroelastic theory is utilized, but a user-defined material was written to include the effects of osmotic swelling, which is directly related to proteoglycan content. Due to the high stresses in the nucleus, the annulus fibrosus must remodel itself to account for the change in properties of the nucleus. The stress experienced by the nucleus increases greatly in Grade 2 from Grade 1, but then decreases in Grade 3, and even Grade 4 experiences lower stresses than in Grade 2. The osmotic pressure in the central nucleus decreases approximately 75% with degeneration. This explains the increasing inability of Grades 3 through 5 to recover the fluid lost during loading, since the osmotic pressure gradient is the primary mechanism with which fluid flows back into the disc.
