1. I want to be safe: understanding the main drivers behind vaccination choice throughout the pandemic

Abstract: Abstract Background Despite being a major advancement in modern medicine, vaccines face widespread hesitancy and refusal, posing challenges to immunization campaigns. The COVID-19 pandemic accentuated vaccine hesitancy, emphasizing the pivotal role of beliefs in efficacy and safety on vaccine acceptance rates. This study explores the influence of efficacy and safety perceptions on vaccine uptake in Italy during the pandemic. Methods We administered a 70-item questionnaire to a representative sample of 600 Italian speakers. Participants were tasked with assessing the perceived effectiveness and safety of each vaccine dose, along with providing reasons influencing their vaccination choices. Additionally, we conducted an experimental manipulation, exploring the effects of four framing messages that emphasized safety and/or efficacy on participants’ willingness to receive a hypothetical fourth vaccine dose. Furthermore, participants were asked about their level of trust in the scientific community and public authorities, as well as their use of different information channels for obtaining COVID-19-related information. Results Our study reveals a dynamic shift in vaccine efficacy and safety perceptions throughout the COVID-19 pandemic, potentially influencing vaccination compliance. Initially perceived as more effective than safe, this assessment reversed by the time of the third dose. Beliefs regarding safety, rather than efficacy, played a significant role in anticipating future vaccinations (e.g., the booster dose). Safety-focused messages positively affected vaccination intent, while efficacy-focused messages showed limited impact. We also observed a changing trend in reasons for vaccination, with a decline in infection-related reasons and an increase in social related ones. Furthermore, trust dynamics evolved differently for public authorities and the scientific community. Conclusions Vaccine perception is a dynamic process shaped by evolving factors like efficacy and safety perceptions, trust levels, and individual motivations. Our study sheds light on the complex dynamics that underlie the perception of vaccine safety and efficacy, and their impact on willingness to vaccinate. We discuss these results in light of bounded rationality, loss aversion and classic utility theory.

Abstract: Introduction The treatment of patients with cardiogenic shock (CS) encompasses several health technologies including Impella pumps and venoarterial extracorporeal membrane oxygenation (VA-ECMO). However, while they are widely used in clinical practice, information on resource use and quality of life (QoL) associated with these devices is scarce. The aim of this study is, therefore, to collect and comparatively assess clinical and socioeconomic data of Impella versus VA-ECMO for the treatment of patients with severe CS, to ultimately conduct both a cost-effectiveness (CEA) and budget impact (BIA) analyses.Methods and analysis This is a prospective plus retrospective, multicentre study conducted under the scientific coordination of the Center for Research on Health and Social Care Management of SDA Bocconi School of Management and clinical coordination of Istituto di Ricovero e Cura a Carattere Scientifico (IRCCS) San Raffaele Scientific Institute in Milan. The Impella Network stemmed for the purposes of this study and comprises 17 Italian clinical centres from Northern to Southern Regions in Italy. The Italian network qualifies as a subgroup of the international Impella Cardiac Surgery Registry. Patients with CS treated with Impella pumps (CP, 5.0 or 5.5) will be prospectively recruited, and information on clinical outcomes, resource use and QoL collected. Economic data will be retrospectively matched with data from comparable patients treated with VA-ECMO. Both CEA and BIA will be conducted adopting the societal perspective in Italy. This study will contribute to generate new socioeconomic evidence to inform future coverage decisions.Ethics and dissemination As of May 2024, most of the clinical centres submitted the documentation to their ethical committee (N=13; 76%), six centres received ethical approval and two centres started to enrol patients. Study results will be published in peer-reviewed publications and disseminated through conference presentations.

Abstract: Abstract Aims The present analysis from the multicentre prospective Altshock‐2 registry aims to better define clinical features, in‐hospital course, and management of cardiogenic shock complicating acutely decompensated heart failure (ADHF‐CS) as compared with that complicating acute myocardial infarction (AMI‐CS). Methods and results All patients with AMI‐CS or ADHF‐CS enrolled in the Altshock‐2 registry between March 2020 and February 2022 were selected. The primary objective was the characterization of ADHF‐CS patients as compared with AMI‐CS. In‐hospital length of stay and mortality were secondary endpoints. One‐hundred‐ninety of the 238 CS patients enrolled in the aforementioned period were considered for the present analysis: 101 AMI‐CS (80% ST‐elevated myocardial infarction and 20% non‐ST‐elevated myocardial infarction) and 89 ADHF‐CS. Abstract: In the frame of the H2020 MORE&LESS project co-funded by European Commission, a test campaign for a hypersonic vehicle demonstrator took place at the INCAS Trisonic Facility. CFD analysis was used to quantify the effects of the wind tunnel model support, the closed engine nacelle, and to perform the Reynolds number extrapolation. Three sets of simulations were used in order to generate the corrections. The wind tunnel configuration with sting, sting cavity, and closed nacelle was used as the baseline, with the aim of matching the experimental results as precisely as possible. A configuration with a flow-through nacelle and the shock cone in the appropriate position for each Mach number and no sting or cavity was used to determine the effect of the sting and the closed nacelle. For the Reynolds extrapolation, a 1:1 model was used, with the boundary conditions deriving from the theoretical trajectory of the vehicle. The CFD results for the wind tunnel configuration closely align with the experimental data. Significant differences between the three configurations can be observed just for the pitching moment, and those are caused by the presence of the sting and the open nacelle. The difference in Reynolds number does not seem to have a significant effect on the aerodynamic coefficients.

5. Aerodatabase Development and Integration and Mission Analysis of a Mach 2 Supersonic Civil Aircraft

Abstract: The request for faster and greener civil aviation is urging the worldwide scientific community and aerospace industry to develop a new generation of supersonic aircraft, which are expected to be environmentally sustainable and to guarantee a high-level protection of citizens. A key aspect to monitor the potential environmental impact of new configurations is the aerodynamic efficiency and its impact onto the real mission. To pursue this goal, this paper discloses increasing-fidelity aerodynamic modeling approaches to improve the conceptual design of high-speed vehicles. The disclosed methodology foresees the development of aerodynamic aerodatabases by means of incremental steps starting from simplified methods (panels methods and/or low-fidelity CFD simulations) up to very reliable data based on high-fidelity CFD simulations and experimental measurements with associated confidence levels. This multifidelity approach enables the possibility of supporting the aircraft design process at different stages of its design cycle, from the estimation of preliminary aerodynamic coefficients at the beginning of the conceptual design, up to the development of tailored aerodatabases at advanced design phases. For each design stage, a build-up approach is adopted, starting from the investigation of the clean external configuration up to the complete one, including control surfaces’ effects and, if any, the effects of the integration of the propulsive effects. In addition, the applicability of the approach is guaranteed for a wide range of supersonic and hypersonic aircraft, and the developed methodology is here applied to the characterization of Mach 2 aircraft configuration, a relevant case study of the H2020 MORE&LESS project.

6. Chemical Kinetic Analysis of High-Pressure Hydrogen Ignition and Combustion toward Green Aviation

Abstract: In the framework of the “Multidisciplinary Optimization and Regulations for Low-boom and Environmentally Sustainable Supersonic aviation” project, pursued by a consortium of European government and academic institutions, coordinated by Politecnico di Torino under the European Commission Horizon 2020 financial support, the Italian Aerospace Research Centre is computationally investigating the high-pressure hydrogen/air kinetic combustion in the operative conditions typically encountered in supersonic aeronautic ramjet engines. This task is being carried out starting from the zero-dimensional and one-dimensional chemical kinetic assessment of the complex and strongly pressure-sensitive ignition behavior and flame propagation characteristics of hydrogen combustion through the validation against experimental shock tube and laminar flame speed measurements. The 0D results indicate that the kinetic mechanism by Politecnico di Milano and the scheme formulated by Kéromnès et al. provide the best matching with the experimental ignition delay time measurements carried out in high-pressure shock tube strongly argon-diluted reaction conditions. Otherwise, the best behavior in terms of laminar flame propagation is achieved by the Mueller scheme, while the other investigated kinetic mechanisms fail to predict the flame speeds at elevated pressures. This confirms the non-linear and intensive pressure-sensitive behavior of hydrogen combustion especially in the critical high-pressure and low-temperature region which is hard to be described by a single all-encompassing chemical model.

7. Trusting COVID-19 vaccines as individual and social goal

Abstract: Abstract Trust in vaccines and in the institutions responsible for their management is a key asset in the global response to the COVID-19 pandemic. By means of a structured multi-scales survey based on the socio-cognitive model of trust, this study investigates the interplay of institutional trust, confidence in COVID-19 vaccines, information habits, personal motivations, and background beliefs on the pandemic in determining willingness to vaccinate in a sample of Italian respondents (N = 4096). We observe substantial trust in public institutions and a strong vaccination intention. Theory-driven structural equation analysis revealed what factors act as important predictors of willingness to vaccinate: trust in vaccine manufacturers (which in turn is supported by trust in regulators), collectivist goals, self-perceived knowledgeability, reliance on traditional media for information gathering, and trust in institutional and scientific sources. In contrast, vaccine hesitancy, while confined to a minority, is more prominent in less educated and less affluent respondents. These findings can inform institutional decisions on vaccine communication and vaccination campaigns.

8. The impact of facemasks on emotion recognition, trust attribution and re-identification

Abstract: Abstract Covid-19 pandemics has fostered a pervasive use of facemasks all around the world. While they help in preventing infection, there are concerns related to the possible impact of facemasks on social communication. The present study investigates how emotion recognition, trust attribution and re-identification of faces differ when faces are seen without mask, with a standard medical facemask, and with a transparent facemask restoring visual access to the mouth region. Our results show that, in contrast to standard medical facemasks, transparent masks significantly spare the capability to recognize emotional expressions. Moreover, transparent masks spare the capability to infer trustworthiness from faces with respect to standard medical facemasks which, in turn, dampen the perceived untrustworthiness of faces. Remarkably, while transparent masks (unlike standard masks) do not impair emotion recognition and trust attribution, they seemingly do impair the subsequent re-identification of the same, unmasked, face (like standard masks). Taken together, this evidence supports a dissociation between mechanisms sustaining emotion and identity processing. This study represents a pivotal step in the much-needed analysis of face reading when the lower portion of the face is occluded by a facemask.

Abstract: The attraction effect emerges when adding a seemingly irrelevant option (decoy) to a binary choice shifts preference towards a target option. This suggests that choice behaviour is dynamic, i.e., choice values are developed during deliberation, rather than manifesting some pre-existing preference set. Whereas several models of multialternative and multiattribute decision making consider dynamic choice processes as crucial to explain the attraction effect, empirically investigating the exact nature of such processes requires complementing choice output with other data. In this study, we focused on asymmetrically dominated decoys (i.e., decoys that are clearly dominated only by the target option) to examine the attentional and comparative processes responsible for the attraction effect. Through an eye-tracker paradigm, we showed that the decoy option can affect subjects’ preferences in two different and not mutually exclusive ways: by focusing the attention on the salient option and the dominance attribute, and by increasing comparisons with the choice dominant pattern. Although conceptually and procedurally distinct, both pathways for decoy effects produce an increase in preferences for the target option, in line with attentional and dynamic models of decision making. Eye-tracking data provide further details to the verification of such models, by highlighting the context-dependent nature of attention and the development of similarity-driven competitive decisional processes.

10. How Soundtracks Shape What We See: Analyzing the Influence of Music on Visual Scenes Through Self-Assessment, Eye Tracking, and Pupillometry

Abstract: This article presents two studies that deepen the theme of how soundtracks shape our interpretation of audiovisuals. Embracing a multivariate perspective, Study 1 (N = 118) demonstrated, through an online between-subjects experiment, that two different music scores (melancholic vs. anxious) deeply affected the interpretations of an unknown movie scene in terms of empathy felt toward the main character, impressions of his personality, plot anticipations, and perception of the environment of the scene. With the melancholic music, participants felt empathy toward the character, viewing him as more agreeable and introverted, more oriented to memories than to decisions, while perceiving the environment as cozier. An almost opposite pattern emerged with the anxious music. In Study 2 (N = 92), we replicated the experiment in our lab but with the addition of eye-tracking and pupillometric measurements. Results of Study 1 were largely replicated; moreover, we proved that the anxious score, by increasing the participants’ vigilance and state of alert (wider pupil dilation), favored greater attention to minor details, as in the case of another character who was very hard to be noticed (more time spent on his figure). Results highlight the pervasive nature of the influence of music within the process of interpretation of visual scenes.

11. Music and Time Perception in Audiovisuals: Arousing Soundtracks Lead to Time Overestimation No Matter Their Emotional Valence

Abstract: One of the most tangible effects of music is its ability to alter our perception of time. Research on waiting times and time estimation of musical excerpts has attested its veritable effects. Nevertheless, there exist contrasting results regarding several musical features’ influence on time perception. When considering emotional valence and arousal, there is some evidence that positive affect music fosters time underestimation, whereas negative affect music leads to overestimation. Instead, contrasting results exist with regard to arousal. Furthermore, to the best of our knowledge, a systematic investigation has not yet been conducted within the audiovisual domain, wherein music might improve the interaction between the user and the audiovisual media by shaping the recipients’ time perception. Through the current between-subjects online experiment (n = 565), we sought to analyze the influence that four soundtracks (happy, relaxing, sad, scary), differing in valence and arousal, exerted on the time estimation of a short movie, as compared to a no-music condition. The results reveal that (1) the mere presence of music led to time overestimation as opposed to the absence of music, (2) the soundtracks that were perceived as more arousing (i.e., happy and scary) led to time overestimation. The findings are discussed in terms of psychological and phenomenological models of time perception.

12. Optimising Speaker-Dependent Feature Extraction Parameters to Improve Automatic Speech Recognition Performance for People with Dysarthria

Abstract: Within the field of Automatic Speech Recognition (ASR) systems, facing impaired speech is a big challenge because standard approaches are ineffective in the presence of dysarthria. The first aim of our work is to confirm the effectiveness of a new speech analysis technique for speakers with dysarthria. This new approach exploits the fine-tuning of the size and shift parameters of the spectral analysis window used to compute the initial short-time Fourier transform, to improve the performance of a speaker-dependent ASR system. The second aim is to define if there exists a correlation among the speaker’s voice features and the optimal window and shift parameters that minimises the error of an ASR system, for that specific speaker. For our experiments, we used both impaired and unimpaired Italian speech. Specifically, we used 30 speakers with dysarthria from the IDEA database and 10 professional speakers from the CLIPS database. Both databases are freely available. The results confirm that, if a standard ASR system performs poorly with a speaker with dysarthria, it can be improved by using the new speech analysis. Otherwise, the new approach is ineffective in cases of unimpaired and low impaired speech. Furthermore, there exists a correlation between some speaker’s voice features and their optimal parameters.

Abstract: In the building material and stones market, lots of restrictions are coming in different world zones. In Europe, a recent regulatory set up the maximum level of radiological emissions for materials intended for use in public and private building structures. For this reason, companies need to have a very efficient radiological measurements system in their production chain, in order to respect all the rules and to be competitive in the world market. This article describes CORSAIR, a Cloud-Oriented Measurement System for Radiological Investigation and Traceability of Stones. Our cyber-physical system consists of sensing nodes network connected to a data collection gateway through LoRaWAN protocol, and interfaces with a centralized cloud application. CORSAIR introduces a fast, repeatable, real-time and non-destructive method to measure radiological emissions and other parameters of each single building material item, uniquely identified by an applied RFID tag. The validity of this system is confirmed by in-situ measurement campaign compared with high-precision laboratory analysis. The results demonstrate the accuracy of the CORSAIR sensor and the possibility to easily integrate it in the company production chain without any change.

Abstract: Acute heart failure is a heterogeneous clinical syndrome and is the first cause of unplanned hospitalization in people >65 years. Patients with heart failure may have different clinical presentations according to clinical history, pre-existing heart disease, and pattern of intravascular congestion. A comprehensive assessment of clinical, echocardiographic, and laboratory data should aid in clinical decision-making and treatment. In some cases, a more accurate evaluation of patient haemodynamics via a pulmonary artery catheter may be necessary to undertake and guide escalation and de-escalation of therapy, especially when clinical, echo, and laboratory data are inconclusive or in the presence of right ventricular dysfunction. Similarly, a pulmonary artery catheter may be useful in patients with cardiogenic shock undergoing mechanical circulatory support. With the subsequent de-escalation of therapy and haemodynamic stabilization, the implementation of guideline-directed medical therapy should be pursued to reduce the risk of subsequent heart failure hospitalization and death, paying particular attention to the recognition and treatment of residual congestion.

23. Microelectrode voltage mapping for substrate assessment in catheter ablation of ventricular tachycardia: A dual‐center experience

Abstract: IntroductionThe assessment of the ventricular myocardial substrate critically depends on the size of mapping electrodes, their orientation with respect to wavefront propagation, and interelectrode distance. We conducted a dual-center study to evaluate the impact of microelectrode mapping in patients undergoing catheter ablation (CA) of ventricular tachycardia (VT). MethodsWe included 21 consecutive patients (median age, 68 [12], 95% male) with structural heart disease undergoing CA for electrical storm (n = 14) or recurrent VT (n = 7) using the QDOT Micro catheter and a multipolar catheter (PentaRay, n = 9). The associations of peak-to-peak maximum standard bipolar (BVc) and minibipolar (PentaRay, BVp) with microbipolar (BV mu Max) voltages were respectively tested in sinus rhythm with mixed effect models. Furthermore, we compared the features of standard bipolar (BE) and microbipolar (mu BE) electrograms in sinus rhythm at sites of termination with radiofrequency energy. ResultsBV(mu Max) was moderately associated with both BVc (beta = .85, p < .01) and BVp (beta = .56, p < .01). BV mu Max was 0.98 (95% CI: 0.93-1.04, p < .01) mV larger than corresponding BVc, and 0.27 (95% CI: 0.16-0.37, p < .01) mV larger than matching BVp in sinus rhythm, with higher percentage differences in low voltage regions, leading to smaller endocardial dense scar (2.3 [2.7] vs. 12.1 [17] cm(2), p < .01) and border zone (3.2 [7.4] vs. 4.8 [20.1] cm(2), p = .03) regions in microbipolar maps compared to standard bipolar maps. Late potentials areas were nonsignificantly greater in microelectrode maps, compared to standard electrode maps. At sites of VT termination (n = 14), mu BE were of higher amplitude (0.9 [0.8] vs. 0.4 [0.2] mV, p < .01), longer duration (117 [66] vs. 74 [38] ms, p < .01), and with greater number of peaks (4 [2] vs. 2 [1], p < .01) in sinus rhythm compared to BE. Conclusionmicroelectrode mapping is more sensitive than standard bipolar mapping in the identification of viable myocytes in SR, and may facilitate recognition of targets for CA.

Abstract: Octogenarians represent the most rapidly expanding population segment in Europe. The prevalence of heart failure (HF) in this group exceeds 10%. We assessed changes in clinical characteristics, therapy, and 1-year outcomes over 2 decades in chronic HF outpatients aged ≥ 80 years enrolled in a nationwide cardiology registry.We included 2520 octogenarians with baseline echocardiographic ejection fraction measurements and available 1-year follow-up, who were recruited at 138 HF outpatient clinics (21% of national hospitals with cardiology units), across 3 enrolment periods (1999-2005, 2006-2011, 2012-2018).At recruitment, over the 3 study periods, there was an increase in age, body mass index, ejection fraction, the prevalence of obesity, diabetes, dyslipidemia, pre-existing hypertension, and atrial fibrillation history. The proportion of patients with preserved ejection fraction rose from 19.4% to 32.7% (P for trend.0001). Markers of advanced disease became less prevalent. Prescription of beta-blockers and mineralocorticoid receptor antagonists increased over time. During the 1-year follow-up, 308 patients died (12.2%) and 360 (14.3%) were admitted for cardiovascular causes; overall, 591 (23.5%) met the combined primary endpoint of all-cause mortality or cardiovascular hospitalization. On adjusted multivariable analysis, enrolment in 2006 to 2011 (HR, 0.70; 95%CI, 0.55-0.90; P=.004) and 2012 to 2018 (HR, 0.61; 95%CI, 0.47-0.79; P=.0002) carried a lower risk of the primary outcome than recruitment in 1999 to 2005.Among octogenarians, over 2 decades, risk factor prevalence increased, management strategies improved, and survival remained stable, but the proportion hospitalized for cardiovascular causes declined. Despite increasing clinical complexity, in cardiology settings the burden of hospitalizations in the oldest old with chronic HF is declining.

Abstract: This paper explores how social interactions among consumers shape markets. In a two-country model, consumers meet and exchange information about the quality of the goods. As information spreads, the demands evolve, affecting the prices and quantities manufactured by profit-maximizing firms. We show that market prices with informational frictions reach the duopoly price with full information, at the limit. However, this convergence can take two different paths depending on the size asymmetry between countries. In particular, when countries are of very different sizes, the single market does not immediately turn into a duopoly and monopoly prices may persist for several periods. Hence, the price-reducing trade effects may take longer to appear. In view of an intense globalization process, understanding how social meetings affect market outcomes is critical for understanding the performance of international economic integration.

Abstract: Several studies have employed music to affect various tasks through mood induction procedures. In this perspective, music’s emotional content coherently affects the listeners’ mood, which, in turn, affects performance. On the contrary, in film music cognition, schema theories suggest that music adds semantic information that interacts with the viewers’ previous knowledge and influences visual information processing. As in this interpretation the viewers’ mood is not deeply considered, it is not clear the extent to which music effects are also due to its power of affecting the viewers’ mood or rather a mere cognitive priming-like influence. An experiment (N = 169) on how music biases the recognition memory of a scene was built comparing semantic and emotional effects of soundtracks differing in valence (happy vs scary) during a recognition task. The results show that 1) music affected the viewers’ mood coherently with its valence, 2) music led to falsely recognise unseen objects as truly present when coherent with the soundtrack valence; and 3) the effect of music on the biased remembering was not mediated by the viewers’ mood, thus suggesting a strong interpretation of the schema theory in film music processing. Finally, a methodological reflection is provided on the issue of the manipulation check in experiments that employ musical stimuli to assess their influence on cognition.

Abstract: This paper analyzes price collusion in a repeated game with two submarkets; a standard and a premium quality segment. Within this setting, we study four types of price-fixing agreement: (i) a segment-wide cartel in the premium submarket only, (ii) a segment-wide cartel in the standard submarket only, (iii) two segment-wide cartels, and (iv) an industry-wide cartel. We present a complete characterization of the collusive pricing equilibrium and examine the corresponding effeect on market shares and welfare. Partial cartels operating in a sufficiently large segment lose market share and the industry-wide cartel prefers to maintain market shares at pre-collusive levels. The impact on consumer and social welfare critically depends on the cost of producing quality. Moreover, given that there is a cartel, more collusion can be beneficial for society as a whole.

Abstract: Background Acute myocarditis (AM) is thought to be a rare cardiovascular complication of COVID-19, although minimal data are available beyond case reports. We aim to report the prevalence, baseline characteristics, in-hospital management, and outcomes for patients with COVID-19–associated AM on the basis of a retrospective cohort from 23 hospitals in the United States and Europe. Methods A total of 112 patients with suspected AM from 56963 hospitalized patients with COVID-19 were evaluated between February 1, 2020, and April 30, 2021. Inclusion criteria were hospitalization for COVID-19 and a diagnosis of AM on the basis of endomyocardial biopsy or increased troponin level plus typical signs of AM on cardiac magnetic resonance imaging. We identified 97 patients with possible AM, and among them, 54 patients with definite/probable AM supported by endomyocardial biopsy in 17 (31.5%) patients or magnetic resonance imaging in 50 (92.6%). We analyzed patient characteristics, treatments, and outcomes among all COVID-19–associated AM. Results AM prevalence among hospitalized patients with COVID-19 was 2.4 per 1000 hospitalizations considering definite/probable and 4.1 per 1000 considering also possible AM. The median age of definite/probable cases was 38 years, and 38.9% were female. On admission, chest pain and dyspnea were the most frequent symptoms (55.5% and 53.7%, respectively). Thirty-one cases (57.4%) occurred in the absence of COVID-19–associated pneumonia. Twenty- one (38.9%) had a fulminant presentation requiring inotropic support or temporary mechanical circulatory support. The composite of in-hospital mortality or temporary mechanical circulatory support occurred in 20.4%. At 120 days, estimated mortality was 6.6%, 15.1% in patients with associated pneumonia versus 0% in patients without pneumonia (P=0.044). During hospitalization, left ventricular ejection fraction, assessed by echocardiography, improved from a median of 40% on admission to 55% at discharge (n=47; P Conclusions AM occurrence is estimated between 2.4 and 4.1 out of 1000 patients hospitalized for COVID-19. The majority of AM occurs in the absence of pneumonia and is often complicated by hemodynamic instability. AM is a rare complication in patients hospitalized for COVID-19, with an outcome that differs on the basis of the presence of concomitant pneumonia.

Abstract: Cardiac magnetic resonance (CMR) imaging has progressively become part of the imaging methods recommended in patients with heart failure. CMR represents the gold standard for assessing volumes, function, biventricular kinetics and providing tissue characterization through scans with and without contrast medium. In patients with heart failure with reduced ejection fraction (HFrEF) and ischemic dilated cardiomyopathy, CMR allows to search for viability, accurately estimate volumes and ejection fraction. It can assess scar extent for predicting response to cardiac resynchronization therapy and for establishing an indication for implanting a defibrillator in borderline cases. In patients with HFrEF and non-ischemic dilated cardiomyopathy, CMR helps to identify specific etiological subgroups and to estimate the arrhythmic risk beyond ejection fraction. In patients with heart failure with preserved ejection fraction, CMR offers the possibility of diagnosing specific phenotypes, including sarcomeric hypertrophic cardiomyopathy, amyloidosis or Fabry disease, and adds prognostic information. Both clinical and scientific interest in this imaging method is constantly expanding; the clinicians dealing with heart failure cannot fail to know the technique, the indications and all the potential that CMR can offer.

Abstract: Heart failure with improved ejection fraction (HFimpEF) represents a nosological entity that has recently been recognized and has little evidence from the literature. Available data indicate an increasing incidence of this patient group, consistent with the progressive improvement and implementation of medical therapy of heart failure with reduced ejection fraction (HFrEF). Furthermore, it is important to underline that the therapy itself should not be suspended after ejection fraction recovery, to avoid the recurrence of worse systolic dysfunction and patient outcomes. Only recently a randomized clinical study has been published, which enrolled also this patient subgroup, the DELIVER trial. Other data will soon become available, given the interest of the scientific community for this subgroup of patients, whose best management remains controversial. Since many studies suggest that the probability of myocardial recovery in HFrEF patients might be as high as 40%, depending on the case series taken into account, whereas the time to recovery might even be 12 months, the appropriate timing of device implantation, such as the defibrillator, in this setting deserves careful consideration.

Abstract: Pharmacotherapy of chronic heart failure with mildly reduced (HFmrEF) and preserved ejection fraction (HFpEF) remains challenging. We aimed to assess whether combined neuro-humoral modulation (NHM) (renin–angiotensin system inhibitors, betablockers, mineralocorticoid receptor antagonists) was differentially associated with outcome according to phenotype and age groups. Between 1999 and 2018 we recruited in a nationwide cardiology registry 4707 patients (HFmrEF n = 2298, HFpEF n = 2409) from three age groups: 40%, triple NHM treatment increased over time and was associated with better patient outcomes.

Abstract: BACKGROUND: Among patients with stable coronary disease and moderate or severe ischemia, whether clinical outcomes are better in those who receive an invasive intervention plus medical therapy than in those who receive medical therapy alone is uncertain. METHODS: We randomly assigned 5179 patients with moderate or severe ischemia to an initial invasive strategy (angiography and revascularization when feasible) and medical therapy or to an initial conservative strategy of medical therapy alone and angiography if medical therapy failed. The primary outcome was a composite of death from cardiovascular causes, myocardial infarction, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest. A key secondary outcome was death from cardiovascular causes or myocardial infarction. RESULTS: Over a median of 3.2 years, 318 primary outcome events occurred in the invasive-strategy group and 352 occurred in the conservative-strategy group. At 6 months, the cumulative event rate was 5.3% in the invasive-strategy group and 3.4% in the conservative-strategy group (difference, 1.9 percentage points; 95% confidence interval [CI], 0.8 to 3.0); at 5 years, the cumulative event rate was 16.4% and 18.2%, respectively (difference, -1.8 percentage points; 95% CI, -4.7 to 1.0). Results were similar with respect to the key secondary outcome. The incidence of the primary outcome was sensitive to the definition of myocardial infarction; a secondary analysis yielded more procedural myocardial infarctions of uncertain clinical importance. There were 145 deaths in the invasive-strategy group and 144 deaths in the conservative-strategy group (hazard ratio, 1.05; 95% CI, 0.83 to 1.32). CONCLUSIONS: Among patients with stable coronary disease and moderate or severe ischemia, we did not find evidence that an initial invasive strategy, as compared with an initial conservative strategy, reduced the risk of ischemic cardiovascular events or death from any cause over a median of 3.2 years. The trial findings were sensitive to the definition of myocardial infarction that was used. Abstract: The treatment of patients with advanced acute heart failure is still challenging. Intra-aortic balloon pump (IABP) has widely been used in the management of patients with cardiogenic shock. However, according to international guidelines, its routinary use in patients with cardiogenic shock is not recommended. This recommendation is derived from the results of the IABP-SHOCK II trial, which demonstrated that IABP does not reduce all-cause mortality in patients with acute myocardial infarction and cardiogenic shock. The present position paper, released by the Italian Association of Hospital Cardiologists, reviews the available data derived from clinical studies. It also provides practical recommendations for the optimal use of IABP in the treatment of cardiogenic shock and advanced acute heart failure.

Abstract: A significant decline in the admission to intensive cardiac care unit (ICCU) has been noted in Italy during the COVID-19 outbreak. Previous studies have provided data on clinical features and outcome of these patients, but information is still incomplete. In this multicenter study conducted in six ICCUs, we enrolled consecutive adult patients admitted to ICCU in three specific time intervals: from February 8 to March 9, 2020 [before national lockdown (pre-LD)], from March 10 to April 9, 2020 [during the first period of national lockdown (in-LD)] and from May 18 to June 17, 2020 [soon after the end of all containment measures (after-LD)]. Compared to pre-LD, in-LD was associated with a significant drop in the admission to ICCU for all causes (− 35%) and acute coronary syndrome (ACS; − 49%), with a rebound soon after-LD. The in-LD reduction was greater for women (− 49%) and NSTEMI (− 61%) compared to men (− 28%) and STEMI (− 33%). Length-of-stay, and in-hospital mortality did not show any significant change from to pre-LD to in-LD in the whole population as well as in the ACS group. This study confirms a notable reduction in the admissions to ICCUs from pre-LD to in-LD followed by an increment in the admission rates after-LD. These data strongly suggest that people, particularly women and patients with NSTEMI, are reluctant to seek medical care during lockdown, possibly due to the fear of viral infection. Such a phenomenon, however, was not associated with a rise in mortality among patients who get hospitalization. Abstract: This paper will provide a comprehensive and detailed description of the multidisciplinary activities performed by the Italian Aerospace Research Center (CIRA) and Tecnosistem-Engineering and Technology (TET) to design the hypersonic non-propelled glider Experimental Flight Test Vehicle (EFTV) and its experimental flight trajectory. A synthesis of the relevant aspects, means and methodologies used for assessing EFTV’s aerodynamic behaviour and aerothermodynamic features, materials and thermal protection system, structural loads and stress analysis, and trajectory is reported. All these aspects depend on each other and together they lead to the definition of the vehicle and the mission matching the initial requirements. Different approaches for the different phases of the flight are also defined. The final aim of the activity is to define methods and standards for different critical aspects of high-speed flying systems and then to collect valuable flight data to validate methods and technologies in a dedicated experimental flight campaign.

Abstract: Background and aims: Familial hypercholesterolaemia (FH) is a powerful risk factor for cardiovascular (CV) events. High levels of low-density lipoprotein cholesterol (LDL-C) since birth are linked to the early onset of atherosclerotic disease. A genetic mutation determining FH is present in about one subject out of 250; FH should be more represented among subjects with a documented diagnosis of coronary artery disease (CAD). The POSTER Study evaluated the prevalence of FH in Italian patients with a recent CAD event. Methods: Eighty-two cardiology centres enrolled patients with a documented CAD event; CV risk profile, drug therapy and biochemical parameters were collected. Dutch Lipid Clinic Network (DLCN) criteria were used to define patients with a potential FH diagnosis (score ≥6); these patients underwent molecular testing for genetic diagnosis of FH. Results: Overall, 5415 patients were enrolled and the main index events were myocardial infarction with ST-elevation, non ST-elevation acute coronary syndrome (ACS), or a recent coronary revascularization (34.8%, 37.2%, and 28% respectively). Mean age was 66 ± 11 years, men were 78%; about 40% were already treated with statins, proportion that increased after the acute event (96.5%). Based on the DLCN score, the prevalence of potential FH was 5.1%, 0.9% of them had a diagnosis of definite FH (score >8). These patients were younger than patients with a score 190 mg/dL. FH was genetically confirmed in 42 subjects (15.9%); genetic diagnosis was defined as not conclusive for FH in 63 patients (23.9%). Finally, in 159 subjects (60.2%) no pathogenic mutations in the tested genes were identified, defining them as negative for monogenic familial hypercholesterolemia. Conclusions: Results underscore a relatively high prevalence of potential FH in patients with a recent CAD event. Therefore, an early identification of these subjects may help improve the management of their high CV risk and, by cascade screening, identify possible FH relatives.

Abstract: BACKGROUND Atrial fibrillation is common in the setting of acute coronary syndromes (ACS) although its impact on ACS remains controversial. AIM To describe in-hospital management of patients with atrial fibrillation and ACS evaluating the impact of atrial fibrillation on in-hospital and mid-term outcome. METHODS We analysed the data of two prospective multicentre nationwide registries (IN-ACS Outcome and MANTRA) to assess clinical characteristics, management, and outcomes of patients with ACS and atrial fibrillation. Study outcomes included death from any cause and a composite end-point of death/re-infarction/stroke/major bleeding within index admission and 6 months' follow-up. RESULTS Out of 12 288 ACS patients, 1236 (10.1%) had atrial fibrillation at admission or developed it during hospitalization. 48. Context-effect bias in capuchin monkeys (Sapajus spp.): exploring decoy influences in a value-based food choice task

Abstract: Decision making is known to be liable to several context effects. In particular, adding a seemingly irrelevant alternative (decoy) to a set of options can modify preferences: typically, by increasing choices towards whatever option clearly dominates the decoy (attraction effect), but occasionally also decreasing its appeal and generating a shift in the opposite direction (repulsion effect). Both types of decoy effects violate rational choice theory axioms and suggest dynamic processes of preference-formation, in which the value of each alternative is not determined a priori, but it is instead constructed by comparing options during the decision process. These effects are well documented, both in humans and in other species: e.g., amoebas, ants, honeybees, frogs, birds, cats, dogs. However, evidence of decoy effects in non-human primates remains surprisingly mixed. This study investigates decoy effects in capuchin monkeys (Sapajus spp.), manipulating time pressure across different conditions, to test whether such effects require time-consuming comparative processes among available alternatives. Whereas the time-dependent nature of decoy effects is a robust finding in the human literature, this is its first investigation in non-human animals. Our results show that capuchins exhibit an attraction effect with decoys targeting their preferred food, and that this effect disappears under time pressure; moreover, we observe preliminary evidence of a repulsion effect when decoys target instead the less-preferred food, possibly due to the larger distance between decoy and target in the attribute space. Taken together, these results provide valuable insight on the evolutionary roots of comparative decision making.

Abstract: Background: Acute myocarditis (AM) is thought to be a rare cardiovascular complication of COVID-19, although minimal data are available beyond case reports. We aim to report the prevalence, baseline characteristics, in-hospital management, and outcomes for patients with COVID-19–associated AM on the basis of a retrospective cohort from 23 hospitals in the United States and Europe. Methods: A total of 112 patients with suspected AM from 56 963 hospitalized patients with COVID-19 were evaluated between February 1, 2020, and April 30, 2021. Inclusion criteria were hospitalization for COVID-19 and a diagnosis of AM on the basis of endomyocardial biopsy or increased troponin level plus typical signs of AM on cardiac magnetic resonance imaging. We identified 97 patients with possible AM, and among them, 54 patients with definite/probable AM supported by endomyocardial biopsy in 17 (31.5%) patients or magnetic resonance imaging in 50 (92.6%). We analyzed patient characteristics, treatments, and outcomes among all COVID-19–associated AM. Results: AM prevalence among hospitalized patients with COVID-19 was 2.4 per 1000 hospitalizations considering definite/probable and 4.1 per 1000 considering also possible AM. The median age of definite/probable cases was 38 years, and 38.9% were female. On admission, chest pain and dyspnea were the most frequent symptoms (55.5% and 53.7%, respectively). Thirty-one cases (57.4%) occurred in the absence of COVID-19–associated pneumonia. Twenty-one (38.9%) had a fulminant presentation requiring inotropic support or temporary mechanical circulatory support. The composite of in-hospital mortality or temporary mechanical circulatory support occurred in 20.4%. At 120 days, estimated mortality was 6.6%, 15.1% in patients with associated pneumonia versus 0% in patients without pneumonia ( P =0.044). During hospitalization, left ventricular ejection fraction, assessed by echocardiography, improved from a median of 40% on admission to 55% at discharge (n=47; P Conclusions: AM occurrence is estimated between 2.4 and 4.1 out of 1000 patients hospitalized for COVID-19. The majority of AM occurs in the absence of pneumonia and is often complicated by hemodynamic instability. AM is a rare complication in patients hospitalized for COVID-19, with an outcome that differs on the basis of the presence of concomitant pneumonia.
