Objective: This study investigates the perceptions of Long COVID patients in Spain regarding the healthcare they receive to identify demands and areas for improvement. Methods: Using a qualitative descriptive phenomenological approach, the study included 27 participants selected through non-probabilistic convenience sampling. Data were collected via online semi-structured interviews and analyzed using thematic analysis. Results: The findings reveal three key themes: (i) health status and challenges in healthcare during the initial COVID-19 infection; (ii) perceptions about healthcare as Long COVID patients; and (iii) demand for and aspects of improving quality of healthcare. The participants, predominantly women (66.67%) with a median age of 51 years, experienced symptoms that they generally perceived as severe, although only 14.81% required hospitalization. The participants reported initial self-management of symptoms at home, which was influenced by familial responsibilities and hospital overcrowding, and the persistence of a wide range of Long COVID symptoms that significantly impacted their daily lives. Satisfaction with healthcare services varied, with frustrations over systemic inefficiencies and long waiting times. Conclusions: The study highlights the need for timely access to medical care, comprehensive and empathetic healthcare services, and specialized Long COVID units. The results emphasize the importance of patient-centered approaches and multidisciplinary care to address the complex nature of Long COVID effectively. These findings provide crucial insights for improving healthcare protocols and systems to better support Long COVID patients. This study was prospectively registered with the Ethics Committee for Research on Medicines of the Albacete Integrated Health Care Management System (registry) on 22 February 2022 with registration number 2022/001.

Summary: Background: Paediatric acute bronchiolitis normally occurs from October to April in the temperate northern hemisphere, peaking in January. Nonpharmacological measures to control the spread of COVID-19 resulted in a decrease in the number of cases of bronchiolitis during the 2020–21 season. The discontinuation of these measures created an uncertain scenario. Aim: To describe the epidemiological evolution of acute bronchiolitis seasons and the changes in the demographics of the affected population before, during, and after the implementation of nonpharmacological interventions for COVID-19 in Spain. Methods: This was a multicentre and descriptive study. A total of 6,334 infants aged up to 12 months who were diagnosed with acute bronchiolitis were recruited from sixteen Spanish hospitals. We collected data from participants from September 1st, 2021, through August 31st, 2023, as part of the ECEALHBA research project. The study periods were before (P1), during (P2), and after (P3) the COVID-19 pandemic. Findings: In P2 and after the discontinuation of nonpharmacological interventions, an unexpected increase in the number of acute bronchiolitis cases was observed from June–August 2021, resulting in an out-of-season peak. A subsequent peak was observed in November 2021, earlier than expected for the 2021-22 season. Rural industrial heritage reflects the ongoing changes in natural environmental, socio-economic, and cultural historical processes in the countryside. In the context of urbanization, the study, conservation, and reuse of rural industrial heritage have become necessary to consolidate the local community identity and promote rural development. This study aims at establishing an intervention tool with general applicability to regenerate rural industrial heritage while integrating local sustainable development. Through a diagrammatic analysis of the natural and artificial environment, this paper combines three aspects via a layered approach: landscape analysis, urban morphology, and spatial cognition. A layered model of rural industrial heritage regeneration is established, with space and consciousness as its core. The empirical study, using the example of the Shecun lime kiln, demonstrates that the spatial-consciousness layered model (SCLM) clearly explains the core roles of transforming material elements as spatial power and the evolution of cognitive elements as consciousness power in the regeneration of rural industrial heritage. It is a systemic model with adaptable and sustainable analysis. SCLM not only provides indicative strategies but serves as a management tool for the regeneration practices of rural industrial heritage.

Abstract The morphogenesis of higher plants requires communication among distant organs throughout vascular tissues (xylem and phloem). Numerous investigations have demonstrated that phloem also act as a distribution route for signalling molecules being observed that different macromolecules translocated by the sap, including nucleic acids and proteins, change under stress situations. The participation of extracellular vesicles (EVs) in this communication has been suggested, although little is known about their role. In fact, in the last decade, the presence of EVs in plants has originated a great controversy, where major concerns arose from their origin, isolation methods, and even the appropriate nomenclature for plant nanovesicles. Phloem sap exudates from melon plants, either aphid‐free or infested with Aphis gossypii, were collected by stem incision. After sap concentration (Amicon), phloem EVs (PhlEVs) were isolated by size exclusion chromatography. PhlEVs were characterised using Nanoparticle Tracking Analysis, Transmission electron microscopy and proteomic analysis. Here we confirm the presence of EVs in phloem sap in vivo and the detection of changes in the particles/protein ratio and composition of PhlEVs in response to insect feeding, revealing the presence of typical defence proteins in their cargo as well as components of the proteasome complex. PhlEVs from infested plants showed lower particles/protein ratio and almost two times more proteolytic activity than PhlEVs from aphid‐free plants. In both cases, such activity was inhibited in a dose‐dependent manner by the proteasome inhibitor MG132. Our results suggest that plants may use this mechanism to prepare themselves to receive infectious agents and open up the possibility of an evolutionary conserved mechanism of defence against pathogens/stresses in eukaryotic organisms.

The thermal decomposition of a heat-not-burn (HNB) tobacco at four temperatures (250–400 °C) was studied via thermogravimetric analysis (TGA) and Multi-shot pyrolizer experiments (Py-GC/MS), and the effect of four potential additives, USY Beta and beta zeolites and Silica Lovel 6000 and SBA-15 silicates at three concentrations (5, 15 and 25% w/w) under an inert and oxidative atmosphere was analyzed. Different techniques were applied showing that the presence of the additives modifies the decomposition processes (TGA). Py-GC/MS showed that these tobaccos generate large amounts of Nicotine and Glycerine. Acid compounds are the most abundant compounds under an inert atmosphere, while Oxygenated compounds predominate under an oxidative atmosphere. In both atmospheres, Furans and Aromatics present in a significant abundance at high temperatures. The additives used reduce both the number and the concentration of most of the compounds generated, especially at high temperatures and concentrations. Moreover, SBA-15 shows good aptitudes to reduce the formation of some individual compounds included in the FDA’s HPHC list, such as Acetone and Acetaldehyde. Finally, smoking experiments corroborated that all additives produce marked reductions in TPM, i.e., the majority fraction obtained, and in practically all the compounds generated. Phenol, a toxicant compound that was detected in a significant amount, is also markedly reduced. SBA-15 is the material that presents a major reduction in the TPM and the principal compounds generated. These results may be of great interest for further reducing the toxicity of smoking this type of heat-not-burn tobacco product.

El objetivo de este estudio es determinar la capacidad de almacenamiento energético del material obtenido de cascarilla de cacao en configuración de capacitor. El material lignocelulósico proviene de la cosecha de cacao ecuatoriano utilizado en procesos agroindustriales. Se realizó el análisis de los volátiles generados durante la pirólisis instantáneo mediante la técnica GC/MS; además, de la comparación del capacitor con dos electrólitos diferentes: ácido sulfúrico (H2SO4) y cloruro de sodio (NaCl). El procesamiento del material se realizó mediante un proceso de carbonización y otro de activación por vía química, los cuales se llevaron a cabo en un reactor de pirólisis en atmósfera inerte de nitrógeno (N2) a una velocidad de calefacción de 20 oC min-1 y se activaron a 850 oC en la misma atmósfera por 4 horas. A partir de este material se prepararon los electrodos y se probó mediante voltametría cíclica el almacenamiento de energía a diferentes velocidades de barrido para cinco ciclos. Los resultados del análisis GC/MS y de voltametría cíclica demostraron que la materia prima secundaria preparada puede utilizarse para obtener productos de alto valor agregado. Los principales compuestos volátiles identificados corresponden a fenoles, ácido acético y alcoholes. El máximo potencial fue de 1,1 kW kg-1 con electrolito de NaCl y, la máxima durabilidad se logró con electrolito de H2SO4 con una potencia de 0,29 kW kg-1 estos valores fueron obtenidos a una velocidad de 100 mV s-1.

IntroductionEffective implementation of strategies to promote health and prevent noncommunicable illnesses requires a profound understanding of the interaction between the individual and society. This study brings to health research the consideration of psychosocial factors that influence the maintenance and change of health behaviors and conduct. From a primary care perspective, it is crucial to propose a biopsychosocial approach for the development of health promotion and self-care programs that embrace personal aptitudes as a relevant individual aspect.ObjectivesTo explore experiences related to personal aptitudes and personality traits that influence health behaviors and conduct, taking into account the social determinants of health, through a thematic analysis based on the capability-opportunity-motivation and behavior (COM-B) system.Methods and analysisThis qualitative research is carried out from a descriptive phenomenological perspective, based on 17 focus groups in which 156 people participated. Inductive and deductive analysis techniques were used following Lincoln and Guba’s criteria of methodological rigor. In addition to 7 different triangulations of analysts, 6 main categories were identified based on the COM-B system: psychological capacity, physical capacity, physical opportunity, social opportunity, reflective motivation, and automatic motivation. The importance of considering these factors to promote healthy behaviors was stressed.DiscussionThis study examined how personal experiences related personal aptitudes and personality traits influence health behaviors and conduct in Spain. It was found that personality traits such as health literacy, self-efficacy, activation, and self-determination can influence the adoption of healthy behaviors. Likewise, the need for control, overthinking, and ambivalence made it impossible. Abstract Cyclin-dependent kinases 4 and 6 (CDK4/6) inhibitors demonstrated activity in terms of progression-free survival (PFS) in advanced dedifferentiated liposarcoma (DD-LPS), a sarcoma with CDK4 amplification. CDK4 overexpression is by far more common than amplification in sarcomas and it might be a rational target for CDK inhibitors. Preclinical investigators of this study found that CDK4 overexpression, while not of CDKN2A, was the most consistent predictive factor for palbociclib efficacy in sarcomas. Advanced adult-type soft-tissue sarcoma, excluding DD-LPS, or bone sarcoma patients, progressing after at least one systemic line, whose tumors overexpressed CDK4, but not CDKN2A at baseline biopsy, were accrued in this single-arm phase II trial (EudraCT number: 2016-004039-19). With the main endpoint of a 6-month PFS rate, 40% was considered promising in this population. Palbociclib was administered orally at 125 mg/day for 21 days in 28-day cycles. A total of 214 patients with 236 CDK4/CDKN2A determinations were assessed for prescreening, archival material (141), and screening, baseline biopsy (95). There were 28 (29%) with favorable mRNA profiles from 95 screened patients at baseline. From 23 enrolled patients, 21 evaluable, the 6-month PFS rate was 29% (95% CI 9–48), and there were 6 patients out of 21 with a PFS longer than 6 months. The median PFS and overall survival were 4.2 (95% CI 3.6–4.8) and 12 (95% CI 8.7–15.4) months, respectively. Translational research showed a significant correlation between CDK4 mRNA and protein expression. Palbociclib was active in a variety of sarcoma subtypes, selected by CDK4/CDKN2A, and deserves further investigation in the sarcoma context.

IntroductionLong COVID patients experience a decrease in their quality of life due to the symptomatology produced by the disease. It is also important to understand how long COVID affects both men and women. The objective of this study is to examine the impact of long COVID symptomatology on the quality of life of Spanish adults from a gender perspective.MethodsAn observational and cross-sectional study was carried out. Participants were able to complete an online questionnaire using an online platform. A sample of 206 people participated in the study.ResultsThe 80.6% of the sample were women with a mean age of 46.51 (±8.28) and the 19.4% were men with a mean age of 48.03 (±9.50). The medium score in the PAC19-QoL test was 141.47 (±24.96) and segmented by gender, 141.65 (±23.95) for women and 140.82 (±28.66) for men. The most common symptoms in women were muscle and joint pain (94.6%), fatigue (94.0%), discomfort (92.2%), difficulty concentrating (91.0%), and memory loss (88.6%). For men the symptoms included muscle and joint pain (97.5%) and fatigue (97.5%) both occupying first position, discomfort (92.0%), difficulty concentrating (90.0%), mood disturbances (90.0%), and memory loss (87.5%). Candida auris has globally emerged as a multidrug-resistant fungus linked to healthcare-associated outbreaks. There is still limited evidence on its virulence, pathogenicity determinants, and complex host–pathogen interactions. This study analyzes the in vivo fungal behaviour, immune response, and host–pathogen interactions upon C. auris infection compared to C. albicans and C. parapsilosis in G. mellonella. This was performed by immunolabelling fungal structures and larval plasmatocytes and using a quantitative approach incorporating bioinformatic morphometric techniques into the study of microbial pathogenesis. C. auris presents a remarkably higher immunogenic activity than expected at its moderate degree of tissue invasion. It induces a greater inflammatory response than C. albicans and C. parapsilosis at the expense of plasmatocyte nodule formation, especially in non-aggregative strains. It specifically invades the larval respiratory system, in a pattern not previously observed in other Candida species, and presents inter-phenotypic tissue tropism differences. C. auris filaments in vivo less frequently than C. albicans or C. parapsilosis mostly through pseudohyphal growth. Filamentation might not be a major pathogenic determinant in C. auris, as less virulent aggregative phenotypes form pseudohyphae to a greater extent. C. auris has important both interspecific and intraspecific virulence and phenotype heterogeneity, with aggregative phenotypes of C. auris sharing characteristics with low pathogenic species such as C. parapsilosis. Our work suggests that C. auris owns an important morphogenetic plasticity that distinguishes it from other yeasts of the genus. Routine phenotypic identification of aggregative or non-aggregative phenotypes should be performed in the clinical setting as it may impact patient management.
