Introdução: Os cistos paralabrais, também chamados de cistos labrais paraglenóideos, são lesões císticas justarticulares que normalmente apresentam estreita relação com o lábio glenoidal — podem representar cistos sinoviais, cistos gangliônicos e pseudocistos. O cisto paralabral do ombro pode ser observado em 2–4% da população geral. Em 79–89% dos casos, apresenta relação com a rotura labral, especialmente o tipo lesão do lábio superior de anterior para posterior. Objetivo: Relatar um caso e revisar a literatura acerca desta lesão incomum. Relato do Caso: Homem de 43 anos que refere dor contínua no ombro esquerdo, principalmente após o trabalho. Não apresenta limitação de movimentos ao exame físico, apesar de relatar sentir dor. A ressonância magnética demonstra dois cistos paralabrais. Conclusão: Relatamos um caso de cisto paralabral, lesão inicialmente insuspeita à anamnese e ao exame físico, diagnosticada por exames de imagem. A ressonância magnética comumente diagnostica o cisto paralabral em pacientes com dor crônica.Palavras Chave: Imagem por ressonância magnética, Articulação do ombro, Lesões do ombro ABSTRACT: Introduction: Paralabral cysts, also named paraglenoid labral cysts, are just articular cystic lesions that usually show a close relationship with the glenoid labrum — they may represent synovial cysts, ganglions cysts, or pseudocysts. Paralabral cysts of the shoulder joint can be observed in 2–4% of the general population, particularly in men during the third and fourth decades. In 79–89% they are related to labral tears, especially superior labrum from anterior to posterior (SLAP) lesions. Objective: To report a case and review the literature about this uncommon lesion. Case report: A 43-year-old man reporting continuous pain on the left shoulder, mainly after work. He did not show movement limitations on physical examination, despite claiming to feel pain. A magnetic resonance demonstrated two paralabral cysts. Conclusion: We report a case of paralabral cyst, an injury initially unsuspected by anamnesis and physical examination, diagnosed by imaging tests. Introdução: o músculo reto femoral é o músculo mais frequentemente lesado do grupo quadríceps durante chutes e corridas repetitivas, apesar de suas avulsões serem raras. A dor localizada na coxa proximal e a incapacidade de flexionar o quadril e/ou estender o joelho associado à história do paciente de contração violenta ou alongamento forçado do grupo muscular do quadríceps femoral podem indicar uma avulsão do reto femoral. Objetivo: relatar um caso e revisar a literatura acerca desta lesão incomum. Materiais e Métodos: revisão do prontuário, registro fotográfico do método diagnóstico e revisão da literatura. Resultados: homem de 46 anos com dor localizada no quadril esquerdo com irradiação para a coxa por três meses após cair no chão com a região dolorida em um jogo de futebol. A dor começou subitamente uma semana após a queda. A ressonância magnética do quadril apresentava avulsão completa do reto femoral esquerdo, distando 2 cm da espinha ilíaca anterior inferior com efusão líquida adjacente. Conclusão: este relato demonstra a dificuldade de diagnosticar essa lesão, pois o seu exame físico é inespecífico, podendo simular patologias mais complexas, necessitando de exames complementares para seu correto diagnóstico.

Approximately 7% to 44% of ankle fractures involve the posterior tibial margin. These fractures tend to have a poorer prognosis than fractures without posterior involvement. Recognition of posterior malleolar fracture patterns is more important than fracture fragment size in decision making. Standard ankle radiographs are not effective for diagnosing isolated posterior malleolar fractures. There is consensus that the true size and geometry of the fragment, specifically its medial propagation, and intercalary fragments may be diagnosed only by computed tomography scanning. Magnetic resonance imaging may yield additional information about syndesmotic ligaments, tendons and osteochondral lesions, although it is used only exceptionally. There are controversies about the treatment of these fractures but authors agree that small fragments can be treated conservatively whereas larger fractures that involve more than 25% of articular surface should be fixed to avoid instability and degenerative changes. The authors report a case with a magnetic resonance imaging diagnosis of this fracture.

Cysts in the anterior cruciate ligament is difficult to diagnose due to the lack of specific symptoms and signs. Incidence of this disorder was the highest for 20–40 years of age group. Occurs predominantly in males. Our aim was to report a case and review the literature about this uncommon lesion. We carried out a review of medical records, photographic record of diagnostic method, and review from the literature. 18 years-old man with pain in the right knee for 04 months. He plays two hours of soccer everyday for a few years. Physical examination with specific knee negative tests, without bruise or edema. Denies torsion, trauma and surgeries. Magnetic resonance imaging demonstrates intrasubstantial lobulated cyst in the anterior cruciate ligament. This report demonstrates the difficulty of diagnosing this lesion, since its physical examination is non-specific, and it can simulate more serious pathologies, requiring complementary tests for its correct diagnosis.

Haglund syndrome is a cause of posterior heel pain and was first described by Patrick Haglund, in 1928, to occur in patients who had a prominence of the posterosuperior surface of the calcaneus.

Background Diphallus, also known as penile duplication, is a rare malformation, seen once in every 5 to 6 million births. Newborns showing this condition present higher mortality rates due to malformations and infections. The underlying etiology of this malformation is uncertain, but it is thought to be associated with trauma, drug use, or infections that may affect fetal the mesoderm between the 23rd and 25th day of pregnancy. Our objective is to describe this rare malformation - diphallus - through magnetic resonance imaging, as well as additional findings. Case A Three-month-old male patient with a 33-week ultrasound demonstrating genital malformation presented to our clinic. At birth, the physical examination revealed diphallia and imperforated anus. Surgical procedures were carried out right after birth to correct the anus malformation. The child did not present any alteration in skin color, and no signs of pain were shown in the abdomen, pelvis, and penises palpations. Urination was observed only through the right penis. Magnetic resonance imaging (MRI) showed two penile structures, each one presenting developed with corpus cavernosum. The penis located on the right showed a complete urethral path in the corpus spongiosum to the vesical floor while the penis located on the left was bigger and did not present a urethral path. Conclusions Penile duplication is a rare condition that is often, associated with other malformations, especially anorectal. To fully understand the extension of congenital anomalies and to determine the optimal surgical approach, MRI yields detailed imaging of the entire pelvic region, providing a thorough anatomical frame of reference, and should be routinely incorporated into presurgical evaluation.

Introdução: O osso navicular é um dos ossos do tarso que mais raramente sofre uma fratura. Com o formato de um barco e posicionado entre a cabeça do tálus e dos três cuneiformes, desempenha um papel importante na sustentação da coluna medial do pé. Uma vez fraturado, pode ter seu diagnóstico feito através da radiografia, tomografia computadorizada, ressonância magnética ou ainda por cintilografia óssea. Objetivo: Relatar um caso incomum de fratura do osso navicular, diagnosticado apenas pela ressonância magnética. Relato do Caso: Mulher de 41 anos que após uma queda referiu dor em seu pé direito com piora ao caminhar. Apresentava dor à palpação na porção medial e dorsal do mediopé e edema. Radiografia do pé normal. A ressonância magnética apresentou traço de fratura na porção medial do osso navicular. Conclusão: Apesar de incomum, as fraturas do mediopé devem ser pesquisadas com métodos de imagem e, mesmo quando a radiografia seja normal, na continuação da sintomatologia do paciente, exames de maior acurácia devem ser realizados, tais como a TC e a RM.Palavras chave: Ossos do tarso, Fraturas ósseas, Imagem por ressonância magnéticaABSTRACT:Introduction: The navicular bone is one of the tarsal bones that most rarely suffers a fracture. Shaped like a boat and positioned between the head of the talus and the three cuneiforms, it plays an important role in supporting the medial column of the foot. Once fractured, the diagnosis can be made through radiography, computed tomography, magnetic resonance imaging, or even bone scintigraphy. Objective: To report an unusual case of fracture of the navicular bone, diagnosed only by magnetic resonance imaging. Case Report: A 41-year-old woman who, after a fall, reported pain in her right foot, which worsened when walking. He had pain on palpation in the medial and dorsal portion of the midfoot and edema. The radiograph of the foot is normal. Magnetic resonance imaging showed a fracture in the medial portion of the navicular bone. Conclusion: In spite of being uncommon, midfoot fractures should be investigated with imaging methods and, even when the radiograph is normal, in the continuation of the patient’s symptoms, more accurate tests should be performed, such as CT and MRI. Introdução: A coalizão tarsal representa uma comunicação anormal entre ossos do tarso e pode ser óssea, cartilaginosa ou fibrosa, se desenvolvendo de forma secundária a insuficiência da diferenciação e segmentação do mesênquima primitivo nas primeiras fases do desenvolvimento, diminuindo a mobilidade e deformando os pés, quando congênita. Quando adquirida ocorre devido à artrite inflamatória, infecção, trauma, neoplasia, entre outras causas. Relato de Caso: Relatamos o caso de uma mulher de 37 anos com queixa há um mês, desmontando a importância da investigação da causa e a necessidade de atenção por parte do radiologista para a avaliação das coalizões tarsais visto que, uma coalizão, independentemente de quais ossos envolvidos, pode levar a uma sintomatologia limitante. When acquired it occurs due to inflammatory arthritis, infection, trauma, neoplasia and other causes. Case report: We report the case of a 37-year- -old woman with a complaint a month ago, dismantling the importance of investigating the cause and the need for attention by the radiologist for the evaluation of tarsal coalitions since, a coalition, regardless of which bones involved, can lead to limiting symptoms.Keywords: Tarsal bones, Tarsal coalition, Magnetic resonance imaging, Foot def

Calcaneal apophysitis is a common cause of heel pain in children and is also known as Sever´s disease. Rarely causes important disability and is transient in most of the times, but it can interfere with walking and physical performance in sports, causing concern to the patient and parents. It is the most common cause of heel pain in the physically active growing children–8-25 years-old–and is considered a benign, self-limiting condition of childhood and adolescence. Boys constitute two-thirds of the patients and the sport that tends to dominate is soccer. The diagnosis of calcaneal apophysitis is usually made with a typical history and physical examination. Pain with activity, especially running and jumping, and a positive “squeeze test”: pain is produced by medial and lateral compression of the heel where the calcaneal apophysis attaches to the main body of the calcaneus. Children with Sever’s disease don´t have growth disturbances in the calcaneal apophysis or any vary in bone density compatible with a necrotic or a repair process. Children with Sever’s disorder respond variably, poorly, or not at all to anti-inflammatory drugs. Heel cup fixating the heel pad provided better pain relief in young boys with Sever’s injury compared with the traditional heel wedge.
