Abstract This nutrition consensus document is the first to coordinate the efforts of three professional organizations - the Brazilian Association of Nutrition (Asbran), the Brazilian Society of Nephrology (SBN), and the Brazilian Society of Parenteral and Enteral Nutrition (Braspen/SBNPE) - to select terminology and international standardized tools used in nutrition care. Its purpose is to improve the training delivered to nutritionists working with adult patients with chronic kidney disease (CKD). Eleven questions were developed concerning patient screening, care, and nutrition outcome management. The recommendations set out in this document were developed based on international guidelines and papers published in electronic databases such as PubMed, EMBASE(tm), CINHAL, Web of Science, and Cochrane. From a list of internationally standardized terms, twenty nutritionists selected the ones they deemed relevant in clinical practice involving outpatients with CKD. The content validity index (CVI) was calculated with 80% agreement in the answers. The Grading of Recommendations, Assessment, Development and Evaluation (GRADE) framework was used to assess the strength of evidence and recommendations. A total of 107 terms related to Nutrition Assessment and Reassessment, 28 to Diagnosis, nine to Intervention, and 94 to Monitoring and Evaluation were selected. The list of selected terms and identified tools will be used in the development of training programs and the implementation of standardized nutrition terminology for nutritionists working with patients with chronic kidney disease in Brazil.

Abstract Introduction: Point-of-care ultrasonography (US) (POCUS) has been used in several specialties, particularly in medical emergency. Despite the confirmation of its numerous benefits, the use of POCUS is still timid in nephrology. In the present study, we aim to investigate the use of POCUS by Brazilian nephrologists. Methods: A survey carried out among the members of the Brazilian Society of Nephrology, through institutional e-mail, using the SurveyMonkey platform. We included 12 self-administered questions, which answers were given anonymously. Results: It was evident that the majority (64%) of the participants did not have the opportunity to practice US during their nephrological training in their residency, specialization, or even in internships; those with experience with US use the method mainly for implanting central vascular accesses (68%), performing a renal biopsy (58%) and evaluating renal morphology (50%); and the main barriers for nephrologists who do not yet use US are the high price of US machines (26%) and the lack of time to learn about US (23%). Also, POCUS use for examinations of other organs, such as the lung (31%) and heart (18%), which are fundamental in the cardiovascular and volume assessment of patients with kidney diseases, is even more limited. However, 95% of the participants expressed an interest in learning POCUS for use in their medical practice. Conclusion: Most of the Brazilian nephrologists interviewed were not trained in US; however, almost all of the research participants expressed an interest in learning to use POCUS in nephrological practice.

ABSTRACT Considering the new coronavirus epidemic (Covid-19), the Brazilian Society of Nephrology, represented by the Peritoneal Steering Committee, in agreement with the and the Dialysis Department, developed a series of recommendations for good clinical practices for peritoneal dialysis (PD) clinics, to be considered during the period of the Covid-19 epidemic. We aim to minimize the disease spread, protecting patients and staff, and ensuring the quality of the treatment provided and adequate follow-up for PD patients. The recommendations suggested at this moment must be adapted to each clinic’s reality and the conditions of the structural and human resources, dependent on the adequate financial provision of the public health system for its full implementation.

ABSTRACT We produced this document to bring pertinent information to the practice of nephrology, as regards to the renal involvement with COVID-19, the management of acute kidney injury cases, and practical guidance on the provision of dialysis support.As information on COVID-19 evolves at a pace never before seen in medical science, these recommendations, although based on recent scientific evidence, refer to the present moment. The guidelines may be updated when published data and other relevant information become available.

ABSTRACT The Covid-19 pandemic brought several challenges to the healthcare system: diagnosis, treatment and measures to prevent the spread of the disease. With the greater availability and variety of diagnostic tests, it is essential to properly interpret them. This paper intends to help dialysis units concerning the use of clinical criteria and diagnostic tests for decision making regarding the discontinuation of isolation of patients with suspected or confirmed Covid-19, as well as the return to work activities for employees with suspected or confirmed Covid-19.

ABSTRACT Patients with Chronic Kidney Disease are among those individuals at increased risk for developing more serious forms of Covid-19. This increased risk starts in the pre-dialysis phase of the disease. Providing useful information for these patients, in language that facilitates the understanding of the disease, can help nephrologists and other healthcare professionals to establish a more effective communication with these patients and help minimize contagion and the risks of serious illness in this population.

ABSTRACT Dialysis units are environments potentially prone to the spread of Covid-19. Patients cannot suspend treatment, and they often have comorbidities, which assigns them a higher risk and worse prognosis. The Brazilian Society of Nephrology prepared this document of good practices, whose technical recommendations deal with general measures that can be implemented to reduce the risk of transmission and prevent the spread of the disease in the unit.

ABSTRACT These recommendations were created after the publication of informative note 3/2020- CGGAP/DESF/SAPS/MS, of April 4, 2020, in which the Brazilian Ministry of Health recommended the use of a cloth mask by the population, in public places. Taking into account the necessary prioritization of the provision of Personal Protective Equipment (PPE) for patients with suspected or confirmed disease, as well as for healthcare professionals, the SBN is favorable concerning the wear of cloth masks by chronic kidney patients in dialysis, in public settings, except in the dialysis setting. The present recommendations have eleven items, related to this rationale, the procedures, indications, contraindications, as well as appropriate fabrics for the mask, and hygiene care to be adopted. These recommendations may change, at any time, in the light of new evidence.

Microcirculatory disorders have been consistently linked to the pathophysiology of sepsis. One of the major organs affected is the kidneys, resulting in sepsis-associated acute kidney injury (SA-AKI) that correlates considerably with mortality. However, the potential role of clinical assessment of peripheral perfusion as a possible tool for SA-AKI management has not been established. To address this gap, the purpose of this study was to investigate the prevalence of peripheral hypoperfusion in SA-AKI, its association with mortality, and fluid balance. This observational cohort study enrolled consecutive septic patients in the Intensive Care Unit. After fluid resuscitation, peripheral perfusion was evaluated using the capillary filling time (CRT) and peripheral perfusion index (PI) techniques. The AKI was defined based on both serum creatinine and urine output criteria. One hundred and forty-one patients were included, 28 (19%) in the non-SA-AKI group, and 113 (81%) in the SA-AKI group. The study revealed higher peripheral hypoperfusion rates in the SA-AKI group using the CRT (OR 3.6; 95% CI 1.35-9.55; p < 0.05). However, this result lost significance after multivariate adjustment. Perfusion abnormalities in the SA-AKI group diagnosed by both CRT (RR 1.96; 95% CI 1.25-3.08) and PI (RR 1.98; 95% CI 1.37-2.86) methods were associated to higher rates of 28-day mortality (p < 0.01). The PI's temporal analysis showed a high predictive value for death over the first 72 h (p < 0.01). A weak correlation between PI values and the fluid balance was found over the first 24 h (r = - 0.20; p < 0.05). In conclusion, peripheral perfusion was not different intrinsically between patients with or without SA-AKI. The presence of peripheral hypoperfusion in the SA-AKI group has appeared to be a prognostic marker for mortality. This evaluation maintained its predictive value over the first 72 hours. The fluid balance possibly negatively influences peripheral perfusion in the SA-AKI.

ABSTRACT Chloroquine and hydroxychloroquine have shown promising preliminary results and have been discussed as therapeutic options for patients with Covid-19. Despite the lack of robust evidence demonstrating the benefits and justifying the use of one of these drugs, the final decision is the responsibility of the attending physician and should be individualized and shared, whenever possible. This position statement recommends dosage adjustment for these drugs in the context of renal impairment.

Abstract According to data from the last census of the Brazilian Society of Nephrology (SBN), the prevalence of hepatitis C virus (HCV) in Brazilian hemodialysis units (HU) is 3.3%, about three times higher than what is reported for the Brazilian general population. Often, professionals working in HU are faced with clinical situations that require rapid HCV diagnosis in order to avoid horizontal transmission within the units. On the other hand, thanks to the development of new antiviral drugs, the cure of patients with HCV, both in the general population and in patients with chronic kidney disease and the disease eradication, appear to be very feasible objectives to be achieved in the near future . In this scenario, SBN and the Brazilian Society of Hepatology present in this review article a proposal to approach HCV within HUs.

Resumo Introdução: As glomerulopatias são as doenças renais mais frequentemente diagnosticáveis por biópsia. O levantamento epidemiológico das glomerulopatias permite identificar sua distribuição e principais etiologias e serve de subsídio para definição de estratégias de prevenção e tratamento. Objetivo: O presente estudo pretende identificar a frequência e a correlação clínico-patológica das glomerulopatias diagnosticadas por biópsia no HC-UFPR durante 5 anos. Métodos: Foram realizadas 131 biópsias no período de 1 de janeiro de 2008 a 31 de dezembro de 2012, submetidas à microscopia óptica e de imunofluorescência. Todas as lâminas de microscopia óptica foram revistas por um patologista. Dados clínicos e laboratoriais e resultados da microscopia de imunofluorescência foram obtidos por revisão dos prontuários. Resultados: Foram reanalisados 128 de 131 casos; 46,5% foram obtidos em homens. A idade média de realização da biópsia foi 43 anos para os homens e 38 anos para as mulheres. Em 99 casos identificou-se a indicação da biópsia; 49,5% apresentaram síndrome nefrótica; 17,17%, insuficiência renal aguda e 15,15% insuficiência renal crônica; 8,08%, síndrome nefrítica; 6,06%, proteinúria isolada e 4,04%, hematúria isolada. 61,21% tratavam-se de glomerulopatia secundária, 33,62% glomerulopatia primária e 5,17% não puderam ser classificados. Dentre as glomerulopatias secundárias, a mais frequente foi a nefrite lúpica (49,29%), e, dentre as primárias, glomeruloesclerose segmentar e focal (30,77%) e nefropatia membranosa (25,64%). Conclusão: O paciente com glomerulopatia neste serviço é adulto e portador de síndrome nefrótica. Ao contrário de outros relatos, observamos predomínio das glomerulopatias secundárias, refletindo possivelmente o perfil terciário de atendimento do HC-UFPR.

ABSTRACT Introduction: Anemia is a frequent multifactorial complication of CKD seen in patients on dialysis derived mainly from impaired erythropoietin (EPO) production. A less common cause of anemia in individuals with CKD is pure red cell aplasia (PRCA) secondary to the production of anti-EPO antibodies. Objective: This paper aimed two describe two cases of PRCA secondary to the production of anti-EPO antibodies including choice of treatment, patient progression, and a literature review. Materials: This study included the cases of two patients with CKD on hemodialysis with severe anemia in need of specific investigation and management. Results: Patient 1 with CKD secondary to hypertension treated with EPO for 7 months showed persistent decreases in hemoglobin (Hb) levels despite the subcutaneous administration of increasing doses of EPO; the patient required recurring blood transfusions. Workup and imaging tests were negative for the main causes of anemia in individuals with CKD on dialysis. Patient 2 with CKD secondary to adult polycystic kidney disease had been taking EPO for 2 years. The patient developed severe abrupt anemia the month he was started on HD, and required recurring transfusions to treat the symptoms of anemia. Workup and imaging findings were inconclusive. Specific laboratory tests confirmed the patients had anti-EPO antibodies. After six months of immunosuppressant therapy (corticosteroids + cyclosporine) the patients were stable with Hb > 9.0 g/dl. Conclusion: PRCA is a rare condition among patients on dialysis treated with rhEPO and should be considered as a possible cause of refractory anemia. Treating patients with PRCA may be challenging, since the specific management and diagnostic procedures needed in this condition are not always readily available.

ABSTRACT Introduction: Pseudoporphyria is a rare photodermatosis with characteristics similar to those of porphyria cutanea tarda, without, however, presenting abnormalities in porphyrin metabolism. Its etiology is related to chronic kidney disease, ultraviolet radiation and certain medications. The aim of the present study is to describe a case of furosemide-related pseudoporphyria in a patient with chronic kidney disease. Case description: A 76-year-old male patient with stage 4 chronic kidney disease and in continuous use of furosemide presented ulcerated lesions with peripheral erythema and central hematic crust in the legs. On a skin infection suspicion, treatment with quinolone and neomycin sulfate was initiated, without improvement. A biopsy of the lesion was performed, with histopathological examination demonstrating findings compatible with porphyria, although the patient did not present high porphyrin levels. The diagnosis of furosemide-induced pseudoporphyria was then established, with medication suspension, and there was a significant improvement of the lesions. Discussion: There are few cases of pseudoporphyria described, but it is believed that this condition is underdiagnosed, especially in patients with chronic kidney disease. Both clinical and histopathological findings closely resemble porphyria, differentiating it from normal levels of porphyrin in plasma, urine, or feces. Conclusions: Although the lesions are mostly benign, they may increase the morbidity and mortality of these patients, so a proper diagnosis and early treatment are extremely important.

INTRODUÇÃO: O vírus Influenza A (H1N1) foi primeiramente descrito em abril de 2009 e, desde então, diversos estudos relataram as características pertinentes à apresentação clínica e ao acometimento pulmonar da doença. Contudo, informações precisas referentes à insuficiência renal aguda (IRA) e às alterações histopatológicas renais nesses pacientes ainda são escassas. OBJETIVO: O objetivo deste estudo é descrever os achados histopatológicos renais de seis pacientes comprovadamente infectados pelo H1N1, que desenvolveram IRA e realizaram biópsia renal, correlacionando-os com os aspectos clínicos. MÉTODOS: Avaliamos seis pacientes do Hospital de Clínicas da UFPR com diagnóstico de H1N1 por PCR viral em 2009 que evoluíram com IRA e que foram submetidos à biópsia renal. Foram revisados os seus prontuários e das lâminas da biópsia renal. RESULTADOS: Todos os casos estudados apresentaram dados clínicos e/ou laboratoriais de IRA, sendo que somente um não apresentou oligúria. À biópsia renal, dois pacientes apresentaram alterações glomerulares: um deles, portador de lupus eritematoso sistêmico, apresentou lesões compatíveis com nefrite lúpica classe III A-C da ISN/RPS 2003 e microangiopatia trombótica focal; outro paciente apresentou glomerulosclerose nodular intercapilar, porém, sem comemorativos clínicos ou laboratoriais de diabetes. Todos os pacientes mostraram graus variáveis de alterações degenerativas vacuolares dos túbulos, com focos de oxalose em dois casos. Dois pacientes possuíam arteriosclerose em grau discreto a moderado. CONCLUSÃO: Em nosso estudo, todos os pacientes apresentarem graus variáveis de alteração degenerativa vacuolar, contudo, não foram encontrados sinais evidentes de necrose tubular aguda, parecendo existir um componente pré-renal como a causa principal de IRA nestes pacientes.

PURPOSES: To evaluate whether polyurethane foam leads more intense foreign-body reaction than silicone foam. To compare the vascularization of the capsules surrounding the foam implants. To investigate if the capsule of polyurethane foam implanted has greater amount of collagen than that of silicone foam. METHODS: Sixty-four young male Wistar rats were allocated into two groups: polyurethane foam and silicone foam. Subcutaneous discs were implanted into the dorsum of the animals in both groups. The capsules were assessed 28 days, two months, three months and six months postoperatively. Microscopic analysis with H&E stain was performed to evaluate the acute and chronic inflammatory process, foreign-body reaction and neovascularization. The analysis with picrosirius red was performed using the ImageProPlus software, to measure the number of vessels and collagen types I and III. RESULTS: There were no statistical differences between the two groups regarding the acute and chronic inflammatory processes. All rats from the polyurethane group, in all times, exhibited moderate or intense foreign-body reaction, with statistic significant difference (p=0.046) when compared with the silicone group, in which the reaction was either mild or nonexistent at two months. Vascular proliferation was significantly different between the groups at 28 days (p=0.0002), with the polyurethane group displaying greater neovascularization with H&E stain. Similar results were obtained with picrosirius red, which revealed in the polyurethane group a much greater number of vessels than in the silicone group (p=0.001). The collagen area was larger in the polyurethane group, significantly at 28 days (p=0.001) and at two months (p=0.030). CONCLUSIONS: Polyurethane foam elicited more intense foreign-body reaction when compared with silicone foam. The number of vessels was higher in the capsules of the polyurethane foam implants 28 days after the operation. The capsule of the polyurethane foam implants showed a greater amount of collagen than that of the silicone foam implants.OBJETIVOS: Avaliar, em relação ao uso de próteses, se a espuma de poliuretano apresenta maior reação de corpo estranho no organismo ao ser comparada com a espuma de silicone. Se há diferenças na vascularização das cápsulas formada ao redor das duas espumas implantadas. Se as cápsulas dos implantes de espuma de poliuretano apresentam quantidade maior de fibras colágenas ao serem comparadas com as da espuma de silicone. MÉTODOS: Utilizou-se 64 ratos albinos da linhagem Wistar, distribuídos em dois grupos de 34, grupo espuma de poliuretano e grupo espuma de silicone e receberam implantes discóides subcutâneos em seu dorso. Foram analisadas as cápsulas peri-implante com 28 dias, dois, três e seis meses após a introdução. A análise microscópica com H&E considerou as variáveis: inflamação aguda, inflamação crônica, reação de corpo estranho e neoformação vascular. A análise da coloração com picrosirius-red usando ImageProPlus considerou o número de vasos e colágeno tipo I e tipo III. RESULTADOS: Em relação à inflamação aguda e crônica, não foram encontradas diferenças estatísticas nos dois grupos. Todos os animais do grupo poliuretano, em todos os momentos, apresentaram reação de corpo estranho moderada ou intensa e foi encontrada diferença estatística significativa (p=0,046) ao serem comparados com o grupo silicone, cuja reação era ausente ou discreta aos dois meses. A neoformação vascular apresentou diferenças significativas nos dois grupos, aos 28 dias (p=0,0002); o grupo poliuretano com H&E apresentava quantidade maior de vasos neoformados e o mesmo ocorrendo com o picrosirius, cujo número de vasos era maior que no grupo silicone (p=0,001). A área de colágeno em todos os momentos foi maior no grupo poliuretano, sendo significativa com 28 dias (p=0,001) e com dois meses (p=0,030). CONCLUSÕES: A espuma de poliuretano apresentou maior reação de corpo estranho no organismo do que a espuma de silicone. A quantidade de vasos foi maior na cápsula da espuma de poliuretano com 28 dias após o implante. Aos 28 dias as cápsulas dos implantes de espuma de poliuretano apresentaram quantidade significativamente maior de colágeno do que as de espuma de silicone.

OBJETIVOS: Correlacionar os achados da biópsia transcutânea hepática guiada por ultrassonografia com os dados ultrassonográficos modo B e Doppler da veia hepática direita; comparar os padrões de onda entre os grupos de estudo (hepatopatas) e controle (sadios); e avaliar se o Doppler da veia hepática direita serve como marcador de hepatopatia crônica. MÉTODOS: Foram estudados 38 pacientes portadores de hepatopatia crônica comprovada por sorologia e biópsia (grupo de estudo) e dez pacientes sem hepatopatia sorológica (grupo controle), avaliados pela ultrassonografia modo B e Doppler. Os critérios histológicos foram a classificação da Sociedade Brasileira de Patologia de Hepatite Crônica. RESULTADOS: A ultrassonografia modo B e o Doppler diferenciaram os indivíduos portadores de hepatopatia crônica dos normais (p=0,047). Houve diferença significativa entre o grupo de estudo e o controle na comparação entre os achados histopatológicos, ultrassonográficos modo B e o Doppler nos padrões de onda da veia hepática direita (p=0,001). CONCLUSÃO: Foi possível correlacionar a biópsia hepática com a ultrassonografia modo B e o Doppler da veia hepática direita; os hepatopatas apresentaram alteração no fluxo da veia hepática direita e os normais não, sendo que o padrão de onda nos controles saudáveis foi trifásico e nos hepatopatas bifásico ou monofásico; e o Doppler da veia hepática direita serviu como marcador de hepatopatia crônica.OBJECTIVE: To correlate chronic liver disease diagnosed by transcutaneous liver biopsy guided by ultrasound, with ultrasound findings with B-mode and Doppler of the right hepatic vein; 2) to compare the wave patterns between the study group and the control group; 3) to compare the right hepatic vein Doppler findings with histopathology findings as a possible marker of chronic liver disease. METHODS: Were studied 38 patients with chronic liver disease diagnosed by biopsy and serology (study group) and 10 persons without serologic liver disease (control group), assessed only by ultrasound B-mode and Doppler. The criteria were based on histology classification of the Brazilian Society of Pathology for chronic hepatitis. Chi-square, Fisher's exact and Student t tests were used. RESULTS: The B-mode and Doppler ultrasound were useful in inferring the differentiation between individuals with chronic liver disease from normal. There were significant differences between the study group and the controls when comparing the histopathology findings, ultrasound B-mode and Doppler in relationship to the wave patterns of the right hepatic vein. CONCLUSION: The correlation of liver biopsies with ultrasound B-mode and Doppler of hepatic vein was positive; 2) individuals with liver disease showed alterations in the flow of the right hepatic vein and normal subjects not, being the wave pattern in normal triphasic and in patients with chronic liver disease monophasic or biphasic; 3) Doppler of the right hepatic vein is useful marker for chronic liver disease.

INTRODUÇÃO: A síndrome de quebra-nozes (SQN) é uma condição na qual há compressão da veia renal esquerda pela aorta e a artéria mesentérica superior, associada a uma sintomatologia característica, porém pouco específica. RELATO DO CASO: Trata-se de uma paciente de 37 anos, apresentando hematúria macroscópica intermitente sem etiologia estabelecida até então. Durante a investigação, uma angiotomografia evidenciou compressão da veia renal esquerda no seu trajeto entre a artéria mesentérica superior e aorta abdominal, associada a importante dilatação de veia ovariana esquerda. DISCUSSÃO: A síndrome desencadeada pela compressão da veia renal esquerda pela aorta e artéria mesentérica superior apresenta prevalência provavelmente aumentada no sexo feminino e postula-se que seja associada a situações particulares, como a nefroptose e a escassez de gordura retroperitoneal. Hematúria, sinal típico, teria como causa a ruptura do fino septo que separa as veias do sistema coletor urinário. Vários exames de imagem podem auxiliar no diagnóstico. O tipo de tratamento que deve ser adotado ainda é controverso, existindo diversas opções cirúrgicas, como o uso de stent intravascular e a conduta conservadora. De diagnóstico costumeiramente tardio, essa síndrome deve ser incluída na investigação de pacientes com quadro de hematúria a esclarecer.

INTRODUÇÃO: A presença de desnutrição tem sido associada a inflamação e estresse oxidativo (EO) em pacientes em hemodiálise (HD) crônica. OBJETIVO: Verificar a associação entre marcadores do estado nutricional, incluindo a gordura corporal (GC), marcadores inflamatórios e de EO em pacientes em HD. MÉTODOS: Estudo transversal, realizado em 40 pacientes em HD. O estado nutricional foi avaliado por avaliação subjetiva global modificada (SGAm), equivalente protéico do aparecimento de nitrogênio total normalizado (PNAn), albumina sérica (Alb-s), índice de massa corporal (IMC), GC e massa corporal magra (MCM). Inflamação e EO foram avaliadas através da proteína C-reativa de alta sensibilidade (PCRas), interleucina-6 (IL-6), produtos protéicos de oxidação avançada (PPOA), 8-hidroxideoxiguanosina (8OHdG) e pentosidina, respectivamente. RESULTADOS: Trinta e sete por cento dos pacientes apresentavam algum grau de desnutrição avaliada pela SGAm. A mediana e variação da GC (kg) foram de 16,2 (5,3 - 36,7). Com relação aos marcadores inflamatórios e de EO, houve uma correlação positiva e significativa entre IMC e PCRas (R = 0,37; p = 0,02), GC e PCRas (R = 0,32; p = 0,04) e entre PCRas e IL-6 (R = 0,51; p = 0,0007). Correlação negativa foi encontrada entre Alb-s e PCRas (R = -0,31; p = 0,05). Apenas no sexo masculino, a PCRas apresentou relação com IMC (R = 0,54; p = 0,01) e com a GC (R = 0,52; p = 0,01). Nenhuma associação foi encontrada entre marcadores inflamatórios e de EO. CONCLUSÃO: Marcadores de desnutrição e de excesso de peso não foram correlacionados com EO. A associação da PCRas com IMC e GC somente no sexo masculino pode sugerir diferenças na resposta inflamatória entre os sexos.INTRODUCTION: The presence of malnutrition has been associated with inflammation and oxidative stress (OS) in patients on chronic hemodialysis (HD). OBJECTIVE: To assess the association between nutritional status markers, such as body fat (BF), markers of inflammation and of OS in patients on HD. METHODS: Cross-sectional study performed with 40 patients on HD. The nutritional status was evaluated by use of the modified subjective global assessment (SGAm), normalized total protein equivalent of nitrogen appearance (PNAn), serum albumin (Alb-s), body mass index (BMI), BF, and lean body mass (LBM). Inflammation and OS were assessed by use of highsensitivity protein C-reactive (HS-PCR), interleukin-6 (IL-6), advanced oxidation protein products (AOPP), 8-hydroxydeoxyguanosine (8OHdG), and pentosidine. RESULTS: Some degree of malnutrition was observed in 37% of the patients assessed through SGAm. Median and variation of BF (kg) were 16.2 and 5.3-36.7, respectively. Regarding the markers of inflammation and of OS, a positive and significant correlation was observed between BMI and HS-PCR (R = 0.37; p = 0.02), BF and HS-PCR (R = 0.32; p = 0.04), and between HS-PCR and IL-6 (R = 0.51; p = 0.0007). A negative correlation was found between Alb-s and HS-PCR (R = -0.31; p = 0.05). Only in males HS-PCR related to BMI (R = 0.54; p = 0.01) and to BF (R = 0.52; p = 0.01). No association was found between markers of inflammation and of OS. CONCLUSION: Markers of malnutrition and of overweight did not correlate with OS. The association of HS-PCR with BMI and BF only in the male sex may suggest differences in the inflammatory response between the sexes.

PURPOSE: To evaluate swine vocal fold healing in a period of 30 days after topical mitomycin-C application. METHODS: Twelve swine underwent exeresis of mucosal flaps from the free edge of the anterior third of both vocal folds with a cold instrument (laryngeal scissors). The animals were divided into two groups: EG (Experimental Group), consisting of 6 animals undergoing topical MMC application (4 mg/dL) on the operated area for 4 minutes; CG (Control Group), 6 animals undergoing topical saline solution application on the operated area for 4 minutes. After 30 days, the animals were sacrificed and the larynges were collected and examined for the presence of synechiae as well as a histological immunohistochemical assessment of immature and mature collagen deposition, number of blood vessels and myofibroblasts. RESULTS: Mature collagen deposition in the EG was 452.12 μm² and 1332.31μm² in the CG; immature collagen deposition was 1511.73μm² in the EG and 1020.61μm² in the CG. The number of myofibroblasts was 1.556 in the EG and 3.583 in the CG. The number of blood vessels was 2.565 in EG and 6.917 in the CG. There were no synechiae in the two studied groups. CONCLUSIONS: There was an increase in immature collagen deposition in the experimental group when compared with the control group. There was a decrease in mature collagen deposition in the experimental group when compared with the control group. There was a decrease in the number of myofibroblasts in the experimental group when compared with the control group. A decrease in blood vessels was observed in the experimental group when compared with the control group. There was no synechia formation in either studied group.OBJETIVO: Avaliar a cicatrização de pregas vocais de suínos utilizando mitomicina-C tópica, em 30 dias. MÉTODOS: Doze suínos foram submetidos à exérese de mucosa do bordo livre do terço anterior de ambas as pregas vocais com instrumental frio (tesoura curva) e divididos em dois grupos: Grupo E (GE), composto por seis animais com utilização tópica de mitomicina-C(4 mg/dL) na área cruenta por quatro minutos e Grupo Controle (GC), composto por seis animais com utilização tópica de soro fisiológico na área cruenta por quatro minutos. Após 30 dias os animais foram sacrificados e coletadas amostras das pregas vocais para avaliação de sinéquias e análise histológica com quantificação da deposição de colágeno maduro e imaturo, quantificação do número de vasos sanguíneos e miofibroblastos, por método imunoistoquímico. RESULTADOS: A deposição do colágeno maduro no GE foi de 452,12 μm² e 1332,31μm² no GC. A do colágeno imaturo foi de 1511,73μm² no GE e de 1020,61μm² no GC. O número de miofibroblastos foi de 1,556 no GE e de 3,583 no GC. O número de vasos sanguíneos foi de 2,565 no GE e de 6,917 no GC. Não houve formação de sinéquias nos grupo s estudados. CONCLUSÕES: No grupo experimento, quando comparado ao controle, houve significativamente aumento da deposição do colágeno imaturo e diminuição da deposição do colágeno maduro, do número de miofibroblastos e de vasos sanguíneos. Não houve formação de sinéquias em ambos os grupos.

INTRODUÇÃO: A regeneração hepática é um mecanismo para superar a perda de tecido funcional do fígado. Este processo é estudado através de diferentes métodos. OBJETIVO: Avaliar o efeito da oclusão parcial da drenagem venosa hepática sobre a regeneração do fígado remanescente de ratos submetidos à hepatectomia parcial. MÉTODO: Foram colhidas biópsias de fígado em 30 ratos Wistar machos, e a seguir realizada hepatectomia a dois terços. Os animais foram divididos em três grupos: um grupo controle e dois grupos de estudo, submetidos a diferentes graus de estenose da veia hepática direita. Após 96 horas do estímulo para regeneração hepática, todos submeteram-se à outra biópsia hepática. Analisaram-se os fragmentos por imunoistoquímica para os marcadores Ki-67 e fator de von Willebrand. Para a leitura das amostras utilizou-se o sistema SAMBA 4000. A deposição de colágeno foi avaliada pela coloração tricrômico de Masson. RESULTADOS: A proliferação celular dos animais submetidos à hepatectomia parcial e estenose moderada ou severa da veia hepática direita persistiu mais elevada quando comparada ao grupo controle. O Índice de Marcação para o Ki-67 foi significativamente mais elevado após a hepatectomia nos grupos submetidos à oclusão parcial da veia hepática, tanto moderada quanto severa. A expressão de fator de von Willebrand estava diminuída após a hepatectomia parcial nos três grupos. Houve pouco depósito de colágeno no tecido hepático nos animais dos dois grupos com estenose da veia hepática direita. CONCLUSÃO: A oclusão parcial da drenagem venosa hepática em ratos submetidos à hepatectomia parcial prolonga o tempo de proliferação de células hepáticas quando comparado aos animais com veias de calibre normal. Como consequência, também houve atraso na restauração da matriz extracelular e na formação de novos vasos sinusoidais.BACKGROUND: Hepatic regeneration is a mechanism to overcome the loss of liver functional tissue. This process has been studied through different methods. AIM: To evaluate the effect of the partial occlusion of the hepatic venous drainage on the regeneration of the remainder livers in rats submitted to partial hepatectomy. METHOD: Liver biopsies from 30 male Wistar rats were collected, and after they were submitted two-third hepatectomy. The animals were divided in three groups: a control group and two study groups, which were submitted to different degrees of the right hepatic vein stenosis. After 96 hours of the stimulation for regeneration all rats were submitted to another hepatic biopsy. The analysis of the fragments was performed by immunohistochemistry for the Ki-67 and von Willebrand factor markers. The reading of the samples was done using the SAMBA 4000 system. Collagen deposition was evaluated by the trichromic Masson's staining. RESULT: The cellular proliferation of the animals submitted to partial hepatectomy and stenosis of the right hepatic vein persists higher when compared to control group. The Label Index for Ki-67 was significantly higher post-hepatectomy in groups submitted to hepatic vein partial occlusion. The expression of von Willebrand factor was strongly decreased after the hepatectomy in all groups. There was little deposit of collagen in the hepatic tissue of animals with hepatic vein stenosis. CONCLUSION: Partial occlusion of the hepatic venous drainage in rats submitted to partial hepatectomy prolongs the time of hepatic cells proliferation when compared to the animals with vein of normal caliber. Consequently, there will be also a delay in the restoration of the extracellular matrix and in the formation of new sinusoidal vessels.

PURPOSE: To evaluate the deposition of collagen fibers at pig's vocal folds after topical use of mitomycin or 5-fluorouracil, when partial exeresis of mucosa layer had been promoted by CO2 laser. METHODS: There were used 18 Larger white pigs which were anesthetized and submitted to mucosa fragment's exeresis, bilaterally, at its free border. The animals were divided into 3 groups, each one with 6 animals: control group, without topical drug application; mitomycin group; and 5-fluorouracil group. After 30 days, the animals were subjected to euthanasia, and samples of the vocal folds were collected and stained by picrosirius red technique with polarization for quantification of total collagen deposition. RESULTS: In control group, the mean rate of right vocal fold's collagen deposition at submucosa consisted in a 3428.66 micrometers area. There was found an area whose size had, in average, 2196.36 micrometers, in mitomycin group, and 2269.19 micrometers, in 5-fluorouracil group. CONCLUSION: Mitomycin and 5-fluorouracil had promoted beneficial change in vocal fold's cicatrization with less collagen deposition, but there was no significant statistically difference when they were compared between themselves.OBJETIVO: Avaliar a deposição das fibras de colágeno total em pregas vocais suínas após o uso tópico de mitomicina ou 5-fluorouracil nas exéreses parciais de mucosa com laser de CO2. MÉTODOS: Foram utilizados 18 porcos da raça Larger white anestesiados e submetidos à exérese de fragmento de mucosa de borda livre da prega vocal direita e prega vocal esquerda. Os animais foram divididos em 3 grupos com 6 animais cada: grupo controle, sem aplicação de medicação tópica; grupo mitomicina, com uso tópico dessa substância; grupo 5-fluorouracil, uso tópico. Após 30 dias do experimento os animais foram submetidos à eutanásia, coletadas amostras das pregas vocais e coradas pela técnica do picrosirius red com polarização para a quantificação computadorizada da deposição do colágeno total. RESULTADOS: No grupo controle, a média da área do colágeno depositado na submucosa da prega vocal direita foi de 3428,66 micrômetros. No grupo mitomicina a média foi de 2196,36 micrômetros. No grupo 5-fluorouracil, a média foi de 2269,19 micrômetros. CONCLUSÃO: A mitomicina e o 5-fluorouracil promoveram mudança benéfica na cicatrização da prega vocal, com menor deposição de colágeno, porém, quando comparados entre si, eles não apresentaram diferença estatisticamente significante.

RACIONAL: O câncer gástrico continua sendo objeto de grande número de estudos que tentam entender melhor sua gênese, e conseqüentemente propor novos tratamentos. Na atualidade destaque especial está sendo dado na marcação imunoistoquímica onde estão sendo utilizados marcadores diversos sem se saber ainda qual ou quais são os mais efetivos no diagnóstico. OBJETIVO: Identificar e quantificar por citometria de imagem a expressão dos marcadores de angiogênese Fator VIII e CD-34 em tumores gástricos. MÉTODOS:Foram utilizados 29 casos de adenocarcinomas gástricos, todos oriundos de material arquivado e conservado em blocos de parafina. Após a desparafinização, realizou-se coloração com marcadores imunoistoquímicos de angiogênese Fator VIII e CD-34, e as lâminas coradas foram submetidas a estudo citofotométrico de imagem em sistema informatizado SAMBA 4000. RESULTADOS: Foram comparados os dois parâmetros oferecidos pelo método, índice de marcagem e densidade óptica, apenas nos 17 casos em que ocorreu identificação de ambos marcadores. Observou-se expressão numericamente significativa de ambos, porém o Fator VIII apresentou melhor média de densidade óptica, enquanto o CD-34 melhor resultado quanto ao índice de marcagem. CONCLUSÃO:A citometria de imagem foi capaz de identificar e quantificar a expressão do Fator VIII e CD-34 de maneira confiável e satisfatória demonstrando presença de angiogênese, sendo que o Fator VIII marcou menores áreas com melhor qualidade e o CD-34 marcou maiores áreas com menor qualidade.BACKGROUND: Gastric cancer remains a subject of great interest in several studies searching for understanding its genesis and, therefore, proposing new treatments. Currently special credits have been given to immunohistochemistry as various markers are being tested with uncertain efficacy on diagnosis. AIM: To identify and quantify the expression of Factor VIII and CD-34, angiogenesis markers, in gastric tumors by imaging cytometry. METHODS: Twenty-nine 29 gastric adenocarcinomas sampled-tissue in paraffin blocks underwent immunohistochemical angiogenesis markers evaluation for factor VIII and CD-34 and the marked slides were submitted to SAMBA 4000 reading. RESULTS:The investigation method offered two comparison parameters, marking index and optical density, showing up both in 17 cases. Significant numerical expression had been observed for both markers, but Factor VIII showed a better optical density average while CD-34 presented a better result for marking index. CONCLUSIONS: Image cytometry identified and quantified Factor VIII and CD-34 expressions in a reliable and satisfactory manner, revealing the presence of angiogenesis. Factor VIII enhanced in smaller areas with better quality while CD-34 enhanced greater areas with lower quality.

OBJETIVO: Estudar a prevalência de anticorpos anticardiolipinas em pacientes com úlceras venosas, diabéticas e arteriais e verificar se a contagem de plaquetas, antecedentes obstétricos e de trombose venosa profunda e achados de livedo reticularis ao exame físico servem como marcadores para os casos positivos. MÉTODOS: Estudaram-se 151 pacientes com úlcera de perna (81 com úlceras venosas, 50 com úlceras diabéticas e 20 com úlceras arteriais) e 150 controles. Pesquisou-se, nos dois grupos, a presença de anticorpos anticardiolipina IgG e IgM e contagem de plaquetas. No grupo úlcera foram coletados dados de antecedentes de trombose venosa profunda e de abortamentos e os pacientes foram examinados para presença de livedo reticularis. Os dados obtidos foram agrupados em tabelas de frequência e contingência utilizando-se dos testes de Fisher e qui-quadrado para variáveis nominais e de Mann-Whitney e Kruskall-Wallis para as numéricas. Adotou-se significância de 5%. RESULTADOS: Encontrou-se prevalência de anticorpos anticardiolipina de 7.2% (n=12) no grupo com úlceras e de 1.3% (n=2) no controle (p=0.01). As úlceras de perna anticardiolipinas positivas não diferiram daquelas sem anticardiolipinas quanto ao gênero do paciente (p=0.98) e história de trombose prévia (p=0.69), abortamentos anteriores (p=0.67) e contagens de plaquetas (p=0.67). Só dois pacientes tinham livedo reticularis não permitindo inferências estatísticas a respeito deste dado. CONCLUSÃO: Existe aumento de prevalência de anticorpos anticardiolipinas nos portadores de úlceras de perna em relação à população geral. As características clínicas das úlceras anticardiolipinas positivas e a contagem de plaquetas não auxiliam na identificação desses pacientes.

OBJETIVO: Estudar a relação entre deficiência de vitamina D e cicatrização de pele em pacientes com úlceras de perna, relacionar esta deficiência com características da úlcera e avaliar se a reposição de vitamina D nos indivíduos deficientes acelera a cicatrização da úlcera. MÉTODOS: Foram escolhidos aleatoriamente 26 pacientes com úlceras venosas de perna e 26 sem úlcera pareados para sexo, idade, HAS e tabagismo. Os grupos foram comparados com relação à dosagem sérica de vitamina D. O grupo úlcera foi dividido em dois subgrupos: um que tomou placebo e outro que recebeu vitamina D 50.000UI por semana durante dois meses. Foi realizada a dosagem da 25-OH-vitamina D e avaliados o tamanho da úlcera e a gravidade da dor, antes e após o tratamento. RESULTADOS: A maioria dos pacientes apresentava níveis insuficientes de vitamina D. Não foi encontrada correlação entre o tamanho da úlcera sem tratamento e os níveis de vitamina D. Nos pacientes que receberam vitamina D, após o tratamento, o tamanho mediano da área da úlcera, diminui de 25cm², para 18cm² e no grupo placebo, de 27cm² para 24,5cm² (p=0,7051 e p=0,7877, respectivamente). Considerando-se a variabilidade da área da úlcera do grupo vitamina D versus placebo, a mediana foi igual a -0,75cm² no primeiro grupo e 4cm² no segundo grupo (p=0,0676). CONCLUSÃO: Pacientes com úlcera de perna têm mais deficiência de vitamina D que os sem. A deficiência de vitamina D não influiu nas características das lesões. A cicatrização nos pacientes com hipovitaminose D mostrou tendência para ser maior naqueles que receberam reposição vitamínica.

Abstract Introduction: The persistence of mineral metabolism disorders after renal transplant (RT) appears to possess a negative impact over graft and patient's survival. Objectives: To evaluate the parameters of mineral metabolism and the persistence of hyperparathyroidism (HPT) in transplanted patients for a 12-month period after the procedure. Methods: Retrospective analysis of 41 transplants (18 women- 44%, mean age of 39 ± 15 years) performed in a University Hospital, evaluating changes of calcium (Ca), phosphorus (P) and parathyroid hormone (PTH) and the prevalence of persistent HPT. The patients were divided into two groups accordingly to PTH levels prior to Tx: Group 1 with PTH ≤ 300 pg/mL (n = 21) and Group 2 with PTH > 300 pg/mL (n = 20). The persistency of HPT after transplant was defined as PTH ≥ 100 pg/mL. The evolution of biochemical parameters and the persistency of HPT were analyzed in each group after 1 year of transplant. Results: After a one-year of follow up, 5% of the patients presented hypophosphatemia (p < 2.7 mg/dL), 24% hypercalcemia (Ca > 10.2 mg/dL) and 48% persistency of HPT (PTH ≥ 100 pg/mL). There was a positive correlation between the PTH pre and post Tx (r = 0.42/p = 0.006) and a negative correlation between PTH and Ca pre-Tx (r = -0.45/p = 0.002). However, there was no significant difference among groups 1 and 2 regarding PTH levels pre and post Tx. Conclusion: The findings in this article suggest that mineral metabolism alterations and the persistency of HPT may occur after one year of renal Tx, mainly in patients which present high PTH levels prior toTx.

OBJETIVOS: Verificar as alterações do peso e índice de massa corporal em pacientes obesos grau II e III com diabete melito tipo 2 nos períodos pré e pós-operatório e as alterações dos parâmetros laboratoriais de glicemia de jejum, glicemia pós-prandial, hemoglobina glicada, insulina nos períodos pré e pós-operatório. MÉTODOS: Realizou-se estudo prospectivo selecionando 40 pacientes com obesidade grau II e III, submetidos à derivação gastrojejunal em Y-de-Roux sem anel. Analisou-se no pré e pós-operatório de 60 dias o peso, índice de massa corporal, glicemia de jejum, glicemia pós-prandial, hemoglobina glicada e insulina. RESULTADOS: O peso médio pré-operatório foi de 107,3Kg diminuindo para 89,5Kg no pós-operatório. O índice de massa corporal médio inicial foi de 39,5Kg/m2 e 32,9Kg/m2 com 60 dias de pós-operatório. A glicemia de jejum no pré-operatório foi de 132 mg/dl e no pós-operatório diminuiu 40,4 mg/dl em média. A glicemia pós-prandial foi de 172 mg/dl no pré-operatório e 111,6 mg/dl no controle pós-operatório. A hemoglobina glicada inicial foi de 7% declinando para 5,7% no pós-operatório. Abstract Introduction: Chronic kidney disease (CKD) affects 10-12% of the adult population in many countries. In Brazil, there is no reliable information about the actual prevalence of CKD. Objective: To determine the prevalence of CKD by estimated glomerular filtration rate (eGFR) and proteinuria/albuminuria in an urban population randomly selected in Southern Brazil. Patients and Methods: 5,216 individuals were randomly selected out of a pool of 10,000 individuals identified from the database of a local energy company. The screening consisted of collection of demographic data, history of diabetes mellitus, hypertension, kidney/cardiovascular disease in the family and obesity through the body mass index - BMI (CKD risk factors). Blood samples were collected for determination of serum creatinine and subsequent eGFR estimation by the MDRD formula and urine samples for determination of albuminuria by dipstick. Albuminuria was further evaluated by HemoCue© in a selected CKD risk group. Results: The population was predominantly Caucasians (93%), 64% were females and the mean age of participants was 45 years old (18-87). BMI (kg/m2) was 27±5. Albuminuria was found in 5.25% of individuals. 88.6% of this population had no CKD (eGFR > 60 ml/min/1.73m2 & normoalbuminuria) and 11.4% were identified as having CKD, with majority on stages 3A (7.2%) and 3B (1.1%). Hypertension, diabetes, older age and obesity was associated with a higher prevalence of CKD (p < 0.001). Conclusions: The prevalence of CKD in an urban population in southern Brazil mirrors other developed countries and indicates that kidney disease is an important public health problem in Brazil.

OBJETIVO: Verificar a prevalência de anticorpos anticardiolipinas IgG e IgM em pacientes com úlcera de perna e se os seus portadores podem ser identificados clinicamente. MÉTODOS: Estudaram-se 151 pacientes com úlcera de perna (81 venosas, 50 diabéticas e 20 arteriais) e 150 controles. Pesquisou-se, nos dois grupos, a presença de anticorpos anticardiolipina IgG e IgM pelo método de ELISA. No grupo úlcera foram coletados dados demográficos dos pacientes, de tamanho e número de úlceras e gravidade da dor medido por escala visual analógica. Os dados obtidos foram agrupados em tabelas de frequência e contingência. Adotou-se significância de 5%. RESULTADOS: Encontrou-se prevalência de anticorpos anticardiolipina de 7.2% (n=12) no grupo com úlceras e de 1.3% (n=2) no controle (p=0.01). Comparando-se a prevalência dos anticorpos anticardiolipina nos diferentes tipos de úlcera verificou-se aumento nas de origem venosa (p=0.02) e diabéticas (p=0.01), mas não nas arteriais (p=0.31) em relação à população controle. As úlceras de perna anticardiolipinas positivas não diferiram daquelas sem anticardiolipinas quanto a tamanho da ferida (p=0.6); gravidade da dor (p=0.67), número médio de úlceras (p=0.38), tempo de duração de doença (p= 0.59), gênero do paciente (p=0.98) e história de trombose prévia (p=0.69). CONCLUSÃO: Existe aumento de prevalência de anticorpos anticardiolipinas nos portadores de úlceras de perna venosas e diabéticas, mas não nas arteriais. As características clínicas das úlceras anticardiolipinas positivas não auxiliam na identificação desses pacientes.

OBJETIVOS: Correlacionar os achados da biópsia transcutânea hepática guiada por ultrassonografia com os dados ultrassonográficos modo B e Doppler da veia hepática direita; comparar os padrões de onda entre os grupos de estudo (hepatopatas) e controle (sadios); e avaliar se o Doppler da veia hepática direita serve como marcador de hepatopatia crônica. MÉTODOS: Foram estudados 38 pacientes portadores de hepatopatia crônica comprovada por sorologia e biópsia (grupo de estudo) e dez pacientes sem hepatopatia sorológica (grupo controle), avaliados pela ultrassonografia modo B e Doppler. Os critérios histológicos foram a classificação da Sociedade Brasileira de Patologia de Hepatite Crônica. RESULTADOS: A ultrassonografia modo B e o Doppler diferenciaram os indivíduos portadores de hepatopatia crônica dos normais (p=0,047). Houve diferença significativa entre o grupo de estudo e o controle na comparação entre os achados histopatológicos, ultrassonográficos modo B e o Doppler nos padrões de onda da veia hepática direita (p=0,001). CONCLUSÃO: Foi possível correlacionar a biópsia hepática com a ultrassonografia modo B e o Doppler da veia hepática direita; os hepatopatas apresentaram alteração no fluxo da veia hepática direita e os normais não, sendo que o padrão de onda nos controles saudáveis foi trifásico e nos hepatopatas bifásico ou monofásico; e o Doppler da veia hepática direita serviu como marcador de hepatopatia crônica.
