Background: Infective endocarditis continues to be a significant concern and may be undergoing an epidemiological transition. Methods: Were studied 1804 consecutive episodes of infective endocarditis between 1978 and 2022. The mean age was 48 ( ± 19), and 1162 (64%) patients were male. Temporal trends in demographic data, comorbidities, predisposing conditions, microorganisms, complications and in-hospital death have been studied over the decades (1978–1988, 1989–1999, 2000–2010 and 2011–2022). The outcomes and clinical characteristics were modeled using nonlinear cubic spline functions. Findings: Valve surgery was performed in 50% of the patients and overall in-hospital mortality was 30%. Introdução: Endocardite infecciosa é uma doença pouco frequente mas com elevado risco de complicações e morte. Objetivo: Descrever tendências temporais nas características clínicas e epidemiológicas de pacientes hospitalizados com endocardite infecciosa em um único centro de referência ao longo de quatro décadas. Métodos: Coorte de 1.804 episódios consecutivos de endocardite em pacientes (> 12 anos) internados em um hospital cardiológico, 1978-2022. O desfecho foi óbito intra-hospitalar. Foram estudados dados demográficos, comorbidades, condições predisponentes, microrganismos e complicações ao longo do tempo e por décadas (1978-1988, 1989-1999, 2000-2010, 2011-2022). Foi realizada uma análise de séries temporais, modelando cada ano usando uma função spline cúbica não-linear com 4 pontos de inflexão para permitir a análise de não-linearidades ao longo do tempo. As associações brutas de cada resultado com essa função não-linear dos anos foram analisadas e ajustadas para fatores de risco (idade, prótese valvar, S. aureus, evento embólico e abscesso perivalvar). Resultados: A maioria dos pacientes era do sexo masculino (64%). Verificou-se aumento da mediana de idade ao longo das décadas (29 a 57 anos; p < 0,001). Também se observou redução na frequência de cardiopatia reumática (14% para 6%; p < 0,001) assim como das infecções estreptocócicas (46% para 33%; p < 0,001). Houve um aumento progressivo de endocardite em prótese valvar, complicações associadas (abscesso perivalvar, insuficiência cardíaca descompensada, embolização) e infecções por estafilococos coagulase-negativos e MRSA ao longo do tempo. A mortalidade geral intra-hospitalar foi de 30%, com tendência ascendente ao longo das últimas três décadas (p = 0,022). No entanto, ao ajustar para fatores relacionados a pior prognóstico (idade, prótese, infecção por S. aureus, eventos embólicos e abscesso perivalvar), verificou-se uma diminuição nas mortes intra-hospitalares (p = 0,019), variando de 34% na primeira década estudada até 26% na última década. Conclusões: No período de 44 anos, ocorreram mudanças significativas nas características clínicas e epidemiológicas dos pacientes internados com endocardite. Apesar do aumento da idade média dos pacientes, do envolvimento de próteses valvares, das infecções por estafilococos coagulase negativos/MRSA e das complicações, foi observada uma redução progressiva na taxa de mortalidade ajustada aos fatores de risco ao longo das décadas analisadas.

Resumo Fundamento Não está claro se o exercício é seguro em pacientes com formas mais avançadas de doença arterial coronariana, como aqueles com angina refratária (AR). Objetivo Visamos determinar o efeito de uma sessão de exercício aeróbico agudo (SEAA) nos níveis de troponina T cardíaca de alta sensibilidade (TnT-as) em pacientes com AR. Métodos Trata-se de um estudo clínico longitudinal, não randomizado e não controlado. Os participantes foram recrutados de abril de 2015 a janeiro de 2019. Em uma escala visual de dor de 0 a 10, a dor classificada até 3 foi considerada como o nível máximo permitido para continuar o exercício. Avaliamos TnT-as na linha de base e 3 horas após a SEAA. O protocolo consistiu em 5 minutos de aquecimento, 30 minutos de exercício aeróbico contínuo na frequência cardíaca correspondente ao limiar anaeróbio ou limiar de angina obtido no teste de esforço cardiopulmonar e 5 minutos de resfriamento. Foram considerados estatisticamente significativos valores de p menores que 0,05. Resultados Foram incluídos 32 pacientes com AR (61 ± 9 anos, 59,4% do sexo masculino). A concentração basal de TnT-as foi de 10,9 ng/L (intervalo de confiança de 95%: 9,1 a 13,0 ng/L). A TnT-as coletada 3 horas após a SEAA foi de 11,1 ng/L (intervalo de confiança de 95%: 9,1 a 13,5 ng/L). Nenhuma diferença ocorreu na TnT-as antes e após a SEAA (p = 0,657). Conclusões Uma única SEAA realizada no limiar de angina com correspondente escala visual de dor não alterou a TnT-as em pacientes com AR, sugerindo que nenhuma lesão miocárdica significativa foi provocada pelo exercício e que este protocolo de exercício pode ser considerado seguro.

Abstract Chagas disease (CD) is a neglected infectious disease associated with early mortality and substantial disability. Three-dimensional speckle tracking (3D STE) may play a role in the evaluation of CD. We aim to characterize new echocardiographic variables in patients with CD and to assess the hypothesis that 3D STE may predict outcomes. Seventy-two patients with CD were included. Clinical and conventional 2D and 3D STE analysis were performed. Patients were followed up for 60 months. Clinical events were defined as hospitalization for heart failure, complex ventricular arrhythmias, heart transplant and all-cause death. Seventy-two patients were recruited and enrolled in three groups: left ventricular ejection fraction (LVEF) 0.50 (N = 30; preserved LVEF or pLVEF). After a Cox model analysis, the top predictors of composite endpoints were 2D LV global longitudinal strain (GLS) ≤ − 11.3% (AUC = 0.87), 2D LV global circumferential strain (GCS) ≤ − 10.1% (AUC = 0.79), 3D LV GLS ≤ − 13% (AUC = 0.82), 3D LV area strain ≤ − 16% (AUC = 0.81) and right ventricle (RV) GLS ≤ − 17.2% (AUC = 0.78). Patients with CD and mrLVEF were morphologically similar to the rLVEF patients despite the benign evolution as the pLVEF group. RV GLS, 2D LV GLS, 2D LV GCS, 3D LV GLS, and 3D LV area strain are strong predictors of 60 months outcomes in patients with CD.

Introduction: The echocardiographic diagnosis criteria for arrhythmogenic right ventricular cardiomyopathy (ARVC) are highly specific but sensitivity is low, especially in the early stages of the disease. The role of echocardiographic strain in ARVC has not been fully elucidated, although prior studies suggest that it can improve the detection of subtle functional abnormalities. The purposes of the study were to determine whether these advanced measures of right ventricular (RV) dysfunction on echocardiogram, including RV strain, increase diagnostic value for ARVC disease detection and to evaluate the association of echocardiographic parameters with arrhythmic outcomes. Methods: The study included 28 patients from the Heart Institute of São Paulo ARVC cohort with a definite diagnosis of ARVC established according to the 2010 Task Force Criteria. All patients were submitted to ECHO’s advanced techniques including RV strain, and the parameters were compared to prior conventional visual ECHO and CMR. Results: In total, 28 patients were enrolled in order to perform ECHO’s advanced techniques. A total of 2/28 (7%) patients died due to a cardiovascular cause, 2/28 (7%) underwent heart transplantation, and 14/28 (50%) patients developed sustained ventricular arrhythmic events. Among ECHO’s parameters, RV dilatation, measured by RVDd (p = 0.018) and RVOT PSAX (p = 0.044), was significantly associated with arrhythmic outcomes. RV free wall longitudinal strain < 14.35% in absolute value was associated with arrhythmic outcomes (p = 0.033). Conclusion: Our data suggest that ECHO’s advanced techniques improve ARVC detection and that abnormal RV strain can be associated with arrhythmic risk stratification. Further studies are necessary to better demonstrate these findings and contribute to risk stratification in ARVC, in addition to other well-known risk markers.

BackgroundAcute kidney injury (AKI) is frequently observed after transcatheter aortic valve implantation (TAVI). Of note, it is associated with a threefold increase in all-cause and cardiac death. We propose a new non-contrast strategy for evaluating and performing the TAVI procedure that can be especially valuable for patients with aortic stenosis (AS) and chronic kidney disease (CKD) to prevent AKI.MethodsPatients with severe symptomatic AS and CKD stage ≥3a were evaluated for TAVI using four non-contrast imaging modalities for procedural planning: transesophageal echocardiogram (TEE), cardiac magnetic resonance, multidetector computed tomography (MDCT), and aortoiliac CO2 angiography. Patients underwent transfemoral (TF) TAVI using the self-expandable Evolut R/Pro, and the procedures were guided by fluoroscopy and TEE. Contrast MDCT and contrast injection at certain checkpoints during the procedure were used in a blinded fashion to guarantee patient safety.ResultsA total of 25 patients underwent TF-TAVI with the zero-contrast technique. The mean age was 79.9 ± 6.1 years, 72% in NYHA class III/IV, with a mean STS-PROM of 3.0% ± 1.5%, and creatinine clearance of 49 ± 7 ml/min. The self-expandable Evolut R and Pro were implanted in 80% and 20% of patients, respectively. In 36% of the cases, the transcatheter heart valve (THV) chosen was one size larger than the one by contrast MDCT, but none of these cases presented adverse events. Device success and the combined safety endpoint (at 30 days) both achieved 92%. Pacemaker implantation was needed in 17%.ConclusionThis pilot study demonstrated that the zero-contrast technique for procedural planning and THV implantation was feasible and safe and might become the preferable strategy for a significant population of CKD patients undergoing TAVR. Future studies with a larger number of patients are still needed to confirm such interesting findings.

IntroductionCardiopulmonary exercise testing (CPET) may capture potential impacts of COVID-19 during exercise. We described CPET data on athletes and physically active individuals with or without cardiorespiratory persistent symptoms.MethodsParticipants’ assessment included medical history and physical examination, cardiac troponin T, resting electrocardiogram, spirometry and CPET. Persistent symptoms were defined as fatigue, dyspnea, chest pain, dizziness, tachycardia, and exertional intolerance persisting >2 months after COVID-19 diagnosis.ResultsA total of 46 participants were included; sixteen (34.8%) were asymptomatic and thirty participants (65.2%) reported persistent symptoms, with fatigue and dyspnea being the most reported ones (43.5 and 28.1%). BackgroundPost-procedure residual ischemia is associated with worse prognosis in patients with coronary artery diasease (CAD).ObjectiveWe evaluated whether autologous bone marrow-derived cells (BMC) contribute to additional reduction in regional stress-induced myocardial ischemia (SIMI) in patients undergoing incomplete coronary artery bypass graft surgery (CABG).MethodsIn a double-blind, randomized, placebo-controlled trial, we enrolled 143 patients (82% men, 58 ± 11 years) with stable CAD and not candidates for complete CABG. They received 100 million BMC (n = 77) or placebo (n = 66) injected into ischemic non-revascularized segments during CABG. The primary outcome was improvement on SIMI quantified as the area at risk in injected segments assessed by cardiovascular magnetic resonance (CMR) 1, 6, and 12 months after CABG.ResultsThe reduction in global SIMI after CABG was comparable (p = 0.491) in both groups indicating sustained beneficial effects of the surgical procedure over 12 month period. In contrast, we observed additional improvement in regional SIMI in BMC treated group (p = 0.047). Baseline regional SIMI values were comparable [18.5 (16.2–21.0) vs. 18.5 (16.5–20.7)] and reached the lowest values at 1 month [9.74 (8.25; 11.49) vs. 12.69 (10.84; 14.85)] for BMC and placebo groups, respectively. The ischemia’s improvement from baseline represented a 50% difference in regional SIMI in favor of the BMC transplanted group at 30 days. We found no differences in clinical and LVEF% between groups during the 12 month follow-up period. The 1 month rate of major adverse cerebral and cardiovascular events (MACCE) (p = 0.34) and all-cause mortality (p = 0.08) did not differ between groups 1 month post intervention.ConclusionWe provided evidence that BMC leads to additional reduction in regional SIMI in chronic ischemic patients when injected in segments not subjected to direct surgical revascularization. Rheumatic fever (RF) and rheumatic heart disease (RHD) are still highly prevalent, particularly in low- and middle-income countries. RHD is a neglected and underdiagnosed disease for which no specific laboratory diagnostic test is completely reliable. This is a retrospective observational study, which included 118 patients with RHD who underwent cardiac surgery from 1985 to 2018. The aim of this investigation was to evaluate the clinical, epidemiological, echocardiographic and pathological characteristics in two cohorts of RHD patients: one cohort with Aschoff bodies present in their pathological results and the other without such histopathological characteristics. No conventional clinical and laboratory tests for RHD myocarditis were able to identify active carditis during the preoperative phase of valve repair or replacement. Patients who had Aschoff bodies in their pathological results were younger (median age of 13 years (11–24 years) vs. 27 years (17–37 years), p = 0.001) and had higher rate of late mortality (22.9% vs. 5.4%, p = 0.043). In conclusion, the presence of Aschoff bodies in pathological findings may predict increased long-term mortality, emphasizing the importance of comprehensive pathology analysis for suspected myocarditis during heart surgery.

Abstract Background Information is lacking concerning in-hospital echocardiography analysis of COVID-19 infection in Brazil. We evaluated echocardiographic parameters to predict a composite endpoint of mortality, pulmonary thromboembolism or acute renal failure. Methods A prospective full echocardiographic study of consecutive patients hospitalized with COVID-19, single tertiary centre in Brazil. We correlated echocardiographic findings to biomarkers, clinical information, thoracic tomography, and in-hospital composite endpoint of mortality, pulmonary thromboembolism or renal failure. Results One hundred eleven patients from March to October 2020, 67 ± 17 years, 65 (58.5%) men, death was observed in 21/111 (18.9%) patients, 48 (43%) required mechanical ventilation, myocardial infarction occurred in 10 (9%), pulmonary thromboembolism in 7 (6.3%) patients, haemodialysis was required for 9 (9.8%). Echocardiography was normal in 51 (46%) patients, 20 (18%) presented with decreased left ventricle ejection, 18 (16.2%) had abnormal left ventricle global longitudinal strain, 35 (31%) had diastolic dysfunction, 6 (5.4%) had an E/e’ratio > 14, 19 (17.1%) presented with right ventricle dilated/dysfunction, 31 (28%) had pericardial effusion. The echocardiographic parameters did not correlate with mortality, biomarkers, clinical events. Tricuspid velocity was related to the composite endpoint of mortality, pulmonary thromboembolism or acute renal failure (p: 00.3; value: 2.65 m/s; AUC ROC curve: 0.739; sensitivity: 73.3; specificity: 66.7; CI: 0.95, inferior: 0.613; superior: 0,866). Conclusions Among hospitalized patients with COVID-19, echocardiography was normal in 51(46%) patients, and 20 (18%) patients presented with a decreased left ventricle ejection fraction. Tricuspid velocity was related to the composite endpoint of mortality, pulmonary thromboembolism or acute renal failure.

IntroductionHeart transplantation represents main therapy for end-stage heart failure. However, survival after transplantation is limited by development of graft rejection. Endomyocardial biopsy, an invasive and expensive procedure, is gold standard technique for diagnosis of rejection. Most of biopsy complications are observed using echocardiography. Novel echocardiographic techniques, such as myocardial strain and three-dimensional reconstruction, can be useful in heart transplant patients.PurposeTo evaluate ventricular strain in heart transplant patients and association with rejection, cellular or humoral, as well as two- and three-dimensional echocardiographic parameters.MethodsCohort of patients from heart transplant program taken to echocardiography after endomyocardial biopsy, from December 2017 to January 2020. Ventricular strain and three-dimensional left ventricle parameters were studied. Rejection results were retrieved from medical record. Qualitative variables were expressed by absolute frequency and percentages, while continuous variables by means and standard deviations. Association between rejection and variables of interest was measured by odds ratio and confidence interval of 95%, with p-value < 0.05.Results123 post-endomyocardial biopsy echocardiographic exams were performed in 54 patients. Eighteen exams were excluded, lasting 105 exams to be evaluated for conventional and advanced echocardiographic parameters. Male patients were 60.4%. Prevalence of cellular rejection was 8.6%, humoral rejection 12.4%, and rejection of any type 20%. There was no association between right ventricular strain and rejection, whether cellular (p = 0.118 and p = 0.227 for septum and free wall, respectively), humoral (p = 0.845 and p = 0.283, respectively), or of any type (0.504 and 0.446). There was no correlation between rejection and left ventricle global longitudinal strain, three-dimensional ejection fraction or desynchrony index. Conventional parameters associated to rejection were left ventricle posterior wall thickness [OR 1.660 (1.163; 2.370), p = 0.005] and left ventricle mass index [OR 1.027 (1.011; 1.139), p = 0.001]. Left ventricle posterior wall thickness remained significant after analysis of cellular and humoral rejection separately [OR 1.825 (1.097; 3.036), p = 0.021 and OR 1.650 (1.028; 2.648), p = 0.038, respectively].ConclusionsThere was no association between ventricular strain, three-dimensional left ventricular ejection fraction and the desynchrony index and rejection, cellular or humoral. Evidence of association of graft rejection with left ventricle posterior wall thickness and left ventricle mass index was observed.

Resumo Com o aumento da expectativa de vida da população e a maior frequência de fatores de risco como obesidade, hipertensão arterial e diabetes, espera-se um aumento na prevalência de insuficiência cardíaca com fração de ejeção preservada (ICFEp). Entretanto, no momento, o diagnóstico e o tratamento de pacientes com ICFEp permanecem desafiadores. O diagnóstico sindrômico de ICFEp inclui diversas etiologias e doenças com tratamentos específicos, mas que apresentam pontos em comum em relação à apresentação clínica e à avaliação laboratorial no que diz respeito aos biomarcadores como BNP e NT-ProBNP, à avaliação ecocardiográfica do remodelamento cardíaco e às pressões de enchimento diastólico ventricular esquerdo. Extensos ensaios clínicos randomizados envolvendo a terapia nesta síndrome falharam na demonstração de benefícios para o paciente, fazendo-se necessária uma reflexão acerca do diagnóstico, dos mecanismos de morbidade, da taxa de mortalidade e da reversibilidade. Na revisão, serão abordados os conceitos atuais, as controvérsias e, especialmente, os desafios no diagnóstico da ICFEp através de uma análise crítica do escore da European Heart Failure Association.

Abstract Background: Considering the potential deleterious effects of right ventricular (RV) pacing, the hypothesis of this study is that isolated left ventricular (LV) pacing through the coronary sinus is safe and may provide better clinical and echocardiographic benefits to patients with bradyarrhythmias and normal ventricular function requiring heart rate correction alone. Objective: To assess the safety, efficacy, and effects of LV pacing using an active-fixation coronary sinus lead in comparison with RV pacing, in patients eligible for conventional pacemaker (PM) implantation. Methods: Randomized, controlled, and single-blinded clinical trial in adult patients submitted to PM implantation due to bradyarrhythmias and systolic ventricular function ≥ 0.40. Randomization (RV vs. LV) occurred before PM implantation. The main results of the study were procedural success, safety, and efficacy. Secondary results were clinical and echocardiographic changes. Chi-squared test, Fisher's exact test and Student's t-test were used, considering a significance level of 5%. Results: From June 2012 to January 2014, 91 patients were included, 36 in the RV Group and 55 in the LV Group. Baseline characteristics of patients in both groups were similar. PM implantation was performed successfully and without any complications in all patients in the RV group. Of the 55 patients initially allocated into the LV group, active-fixation coronary sinus lead implantation was not possible in 20 (36.4%) patients. The most frequent complication was phrenic nerve stimulation, detected in 9 (25.7%) patients in the LV group. During the follow-up period, there were no hospitalizations due to heart failure. Reductions of more than 10% in left ventricular ejection fraction were observed in 23.5% of patients in the RV group and 20.6% of those in the LV group (p = 0.767). Tissue Doppler analysis showed that 91.2% of subjects in the RV group and 68.8% of those in the LV group had interventricular dyssynchrony (p = 0.022). Conclusion: The procedural success rate of LV implant was low, and the safety of the procedure was influenced mainly by the high rate of phrenic nerve stimulation in the postoperative period.

Background: Bioprosthetic heart valve has limited durability and lower long-term performance especially in rheumatic heart disease (RHD) patients that are often subject to multiple redo operations. Minimally invasive procedures, such as transcatheter valve-in-valve (ViV) implantation, may offer an attractive alternative, although data is lacking. The aim of this study was to evaluate the baseline characteristics and clinical outcomes in rheumatic vs. non-rheumatic patients undergoing ViV procedures for severe bioprosthetic valve dysfunction.Methods: Single center, prospective study, including consecutive patients undergoing transcatheter ViV implantation in aortic, mitral and tricuspid position, from May 2015 to September 2020. RHD was defined according to clinical history, previous echocardiographic and surgical findings.Results: Among 106 patients included, 69 had rheumatic etiology and 37 were non-rheumatic. Rheumatic patients had higher incidence of female sex (73.9 vs. 43.2%, respectively; p = 0.004), atrial fibrillation (82.6 vs. 45.9%, respectively; p < 0.001), and 2 or more prior surgeries (68.1 vs. 32.4%, respectively; p = 0.001). Although, device success was similar between groups (75.4 vs. 89.2% in rheumatic vs. non-rheumatic, respectively; p = 0.148), there was a trend toward higher 30-day mortality rates in the rheumatic patients (21.7 vs. 5.4%, respectively; p = 0.057). Still, at median follow-up of 20.7 [5.1–30.4] months, cumulative mortality was similar between both groups (p = 0.779).Conclusion: Transcatheter ViV implantation is an acceptable alternative to redo operations in the treatment of patients with RHD and severe bioprosthetic valve dysfunction. Despite similar device success rates, rheumatic patients present higher 30-day mortality rates with good mid-term clinical outcomes. Future studies with a larger number of patients and follow-up are still warranted, to firmly conclude on the role transcatheter ViV procedures in the RHD population.

In the first day, patients performed the heart rate variability assessment followed by a cardiopulmonary exercise test. In the second day, patients performed a resting echocardiography. Heart transplant recipients were eligible if they were in a stable condition and without any evidence of tissue rejection diagnosed by endomyocardial biopsy. Patients with pacemaker, noncardiovascular functional limitations such as osteoarthritis and chronic obstructive pulmonary disease were excluded from this study. Fundamento: O ecocardiograma fornece dados importantes na avaliação cardiológica de pacientes em insuficiência cardíaca. A identificação de parâmetros ecocardiográficos na cardiopatia chagásica grave auxiliaria na implementação terapêutica e na avaliação prognóstica. Objetivo: Correlacionar parâmetros ecocardiográficos com desfecho mortalidade cardiovascular em pacientes com fração de ejeção < 35%. Métodos: Estudo de análise retrospectiva de parâmetros ecocardiográficos coletados prospectivamente e pré-especificados em 60 pacientes incluídos no Estudo Multicêntrico Randomizado de Terapia Celular em Cardiopatias - braço cardiopatia chagásica. Os parâmetros foram: diâmetros e volumes diastólico e sistólico do ventrículo esquerdo, fração de ejeção, diâmetro do átrio esquerdo, volume do átrio esquerdo, volume indexado do átrio esquerdo, pressão sistólica da artéria pulmonar, integral da velocidade do fluxo aórtico, índice de performance miocárdica, taxa de aumento da pressão do ventrículo esquerdo, tempo de relaxamento isovolumétrico, velocidade das ondas E, A, Em, Am e Sm, tempo de desaceleração da onda E, relação E/A , E/Em e insuficiência mitral. Resultados: No seguimento médio de 24,18 meses, 27 pacientes faleceram. a fração de ejeção média era de 26,6 ± 5,34%. Na análise multivariada, foram incluídos os parâmetros de fração de ejeção (HR = 1,114; p = 0,3704), volume indexado do átrio esquerdo (HR = 1,033; p < 0,0001) e relação E/Em (HR = 0,95; p = 0,1261). O volume indexado do átrio esquerdo foi um fator de predição independente em relação ao desfecho e observou-se que um valor > 70,71 mL/m2 foi associado ao aumento significativo na mortalidade (log rank p < 0,0001). Conclusão: O volume indexado do átrio esquerdo mostrou-se como único fator de predição independente de mortalidade nesta população de pacientes chagásicos e com disfunção sistólica importante.

Os contrastes ecocardiográficos à base de microbolhas estão aprovados para várias situações da prática clínica. Novos softwares, como a imagem em segunda harmônica e o power pulse inversion, podem melhorar a visualização do contraste no exame ecocardiográfico. Neste artigo, discutem-se o início da prática da ecocardiografia com contraste, as novas tecnologias que ajudam na aquisição e melhora da imagem, e seu uso no infarto do miocárdio.

FUNDAMENTO: Remodelamento ventricular esquerdo (RVE) após IAM caracteriza fator de mau prognóstico. Há pouca informação na literatura sobre o RVE analisado com ecocardiografia tridimensional (ECO 3D) OBJETIVO: Analisar com ECO 3D as modificações geométricas e volumétricas do ventrículo esquerdo (VE) seis meses após IAM em pacientes submetidos a tratamento primário percutâneo. MÉTODOS: Estudo prospectivo com ECO 3D de 21 indivíduos (16 homens, 56 ± 12 anos), acometidos por IAM com elevação do segmento ST. Foi feita a análise morfofuncional (VE) com ECO 3D (volumes, FEVE, índice de esfericidade 3D) até sete dias e seis meses após o IAM. RVE foi considerado para aumento > 15% do volume diastólico final do VE (VDFVE) após seis meses do IAM, comparado ao VDFVE até sete dias do evento. RESULTADOS: Oito (38%) pacientes apresentaram RVE. Medidas ecocardiográficas (n = 21 pacientes): I- até sete dias do IAM: 1- VDFVE: 92,3 ± 22,3 mL; 2- FEVE: 0,51 ± 0,01; 3- índice de esfericidade: 0,38 ± 0,05; II- após seis meses: 1- VDFVE: 107,3 ± 26,8 mL; 2- FEVE: 0,59 ± 0,01; 3- índice de esfericidade: 0,31 ± 0,05. Coeficiente de correlação (r) entre índice de esfericidade até sete dias do IAM e VDFVE aos seis meses (n = 8) após o IAM: r: 0,74, p = 0,0007; (r) entre índice de esfericidade após seis meses do IAM e VDFVE aos seis meses do IAM: r: 0,85, p < 0,0001. CONCLUSÃO: Nesta série, foi observado RVE em 38% dos pacientes seis meses após IAM. O índice de esfericidade tridimensional foi associado à ocorrência de RVE.

FUNDAMENTO: A avaliação da função Ventricular Direita (VD) pelo ecocardiograma em pacientes com Tromboembolismo Pulmonar (TEP) é complexa, frequentemente qualitativa; o Doppler tecidual tem sido utilizado para avaliação semiquantitativa dessa câmara, com algumas limitações. OBJETIVO: Avaliar a função do VD no TEP pelo ecocardiograma com Doppler tecidual, complementando com o peptídeo atrial natriurético (BNP). MÉTODOS: Foram estudados pacientes com TEP pelo ecocardiograma com Doppler tecidual e BNP até 24 horas do diagnóstico, obtendo-se as velocidades miocárdicas (s'), strain, strain rate e índice de performance miocárdica do VD; disfunção do VD foi iagnosticada por hipocinesia da câmara, movimento anormal septal e relação VD/VE >1. De acordo com o BNP os pacientes foram divididos em Grupo I, BNP < 50 pg/mL e Grupo II, BNP > 50 pg/mL. RESULTADOS: De 118 pacientes, 100 (60 homens, idade = 55 ± 17 anos) foram analisados; observou-se disfunção do VD em 28%, mais frequentemente no grupo II (19 vs. 9 pacientes, p < 0,001). O grupo II era mais idoso (64 ± 19 vs. 50 ± 15 anos), apresentava menor velocidade de s' (10,5 ± 3,5 vs. 13,2 ± 3,1 cm/s) e maior pressão pulmonar (48 ± 11 vs. 35 ± 11 mmHg), p < 0,001 para todos. O ponto de corte de s' para disfunção do VD foi de 10,8 cm/s (especificidade = 85%, sensibilidade = 54%), com moderada correlação entre o BNP e a onda s'(r = -0,39). CONCLUSÃO: No TEP, a disfunção do VD pelo ecocardiograma se acompanha de elevação do BNP; apesar confirmar adequadamente a presença de disfunção do VD, o Doppler tecidual apresenta sensibilidade limitada para este diagnóstico.

FUNDAMENTO: A função diastólica em portadores de estenose aórtica (EAO) é pouco estudada. Não é possível estabelecer uma possível correlação entre biomarcadores e disfunção diastólica. OBJETIVO: Avaliar a função diastólica em portadores de EAO submetidos à troca valvar através de ecocardiografia transtorácica e correlacionar com valores de NT-proBNP. MÉTODOS: Estudamos 31 pacientes com EAO (11 homens), com idade entre 21 e 81 anos (média de 61±15 anos), submetidos a tratamento cirúrgico de EAO. Foi realizada dosagem sérica de NT-proBNP e o resultado, comparado com variáveis ecocardiográficas obtidas pelo Doppler pulsado em fluxo mitral, Doppler pulsado em veias pulmonares e Doppler tecidual, tanto no pré quanto no pós-operatório. RESULTADOS: Observou-se melhora na Fração de Ejeção (FE) de AE e Tempo de Relaxamento Isovolumétrico (TRIV), além de redução nos seguintes parâmetros: a) Relação da velocidade diastólica precoce mitral (onda E) com a velocidade do anel mitral (onda E´) (E/E'), b) Volume sistólico de átrio esquerdo (AE), c) Volume sistólico de AE indexado, d) Diâmetro diastólico de ventrículo esquerdo (DDVE), e) Diâmetro sistólico de VE (DSVE), f) Volume diastólico final (VDF), g) Massa indexada de VE e h) Relação Volume/Massa de ventrículo esquerdo (VE). Os valores do NT-proBNP correlacionaram-se de modo positivo com os diversos graus de disfunção diastólica, tanto no pré como no pós-operatório. CONCLUSÃO: 1) A cirurgia de troca valvar para correção de EAO resultou em melhora significativa da função diastólica ventricular. 2) Os valores de NT-proBNP correlacionaram-se positivamente com as variáveis ecocardiográficas que determinam disfunção diastólica, sendo o NT-proBNP um bom marcador para caracterizar essa disfunção em portadores de EAO.
