OBJETIVO: Determinar as condições visuais e oculares em indivíduos com mais de 99 anos. MÉTODO: Estudo prospectivo e descritivo de série casos. Trinta idosos com mais de 99 anos se inscreveram no Instituto da Visão da Universidade Federal de São Paulo, de forma voluntária, respondendo à busca ativa. O exame oftalmológico constou de anamnese, ectoscopia, acuidade visual, exame refracional, citologia e cultura de cílios e conjuntiva com antibiograma, teste de Schirmer basal, tempo de ruptura do filme lacrimal, coloração com Rosa Bengala da conjuntiva e córnea, exame de biomicroscopia, tonometria de aplanação e fundoscopia direta e indireta. Foram realizados exames complementares, como retinografia e tomografia de coerência óptica, quando indicados. RESULTADOS: Trinta pacientes acima de 99 anos (média de 101,5±1,8 anos), dos quais, 25 mulheres e 5 homens, foram examinados; destes, 10 não tinham condições de exame completo, em razão de limitações de saúde e mobilidade. A doença sistêmica mais encontrada foi hipertensão arterial (8 pacientes = 40%). Cinco pacientes (25%) negaram qualquer doença ou uso de medicação. A melhor acuidade visual corrigida para longe foi de 20/100 ou melhor em 11 pacientes (55%) e para perto variou de J4 ou melhor também em 11 pacientes (55%), sendo que sete destes idosos (63%) não haviam sido submetidos à cirurgia de catarata. A principal queixa foi dificuldade para leitura em 55% e 40% demonstrou estar satisfeito com a sua visão. A melhora da acuidade com prescrição de óculos novos ocorreu em 4 casos (20%). Catarata foi identificada como comprometendo a visão de forma significativa em 5 casos (25%); porém, apenas 2 concordaram com a cirurgia. A principal causa de baixa visão foi a degeneração macular relacionada à idade, presente em todos os casos; na maioria da vezes (95%), na forma seca. CONCLUSÃO: As necessidades da população nessa faixa etária devem ser entendidas para o desenvolvimento de política específica de saúde ocular. Esse estudo demonstrou idosos com maioria do sexo feminino (83,3%), boa saúde geral (35%), hipertensos (40%), com dificuldade para leitura (55%) e com baixa visual em decorrência de degeneração macular relacionada à idade.PURPOSE: To determine vision conditions and ocular findings in patients older than 99 years. METHODS: Patients were recruited by media advertisement and examined at the Vision Institute of UNIFESP. The following examinations were performed: visual acuity, ectoscopy, refraction, biomicroscopy, tear film break-up time, Schirmer basal test, tonometry, direct and indirect ophthalmoscopy. Complementary examinations such as optical coherence tomography were performed when indicated. RESULTS: Thirty patients older than 99 years were identified. Mean age of 101.5±1.8, 25 females and 5 males. Most common systemic disease, according to history, was arterial hypertension. In 11 patients (55%), the best corrected far visual acuity was 20/100 or better. The best corrected near visual acuity was J4 or better also in 55% (11 patients) and 63.0% (7 patients) of these were the ones that had not been submitted to cataract surgery. The most frequent visual complaint was low vision for reading (55%) and other 8 patients (40%) reported no disturbance (satisfied with their vision). Lens opacity was present in 9 patients (60%) and 8 patients (40%) had previous cataract surgery (5 aphakic eyes, 9 pseudophakic eyes). Cataract was identified as important for impairment of vision in 5 patients but only 2 of them wanted to be submitted to surgery. The main cause of visual impairment was age-related macular degeneration, present in all patients, 95% with no exudative form. Vision improved with new refractive prescription in 4 patients. CONCLUSION: Once we understand the needs we can plan specific diagnostic and treatment strategies. This sample showed that most were female (83.3%), with good general health (35%), most with systemic arterial hypertension (40%), complaining of low vision for reading (55%) because of age-related macular degeneration (100%, 95% dry form). Only 40% had been previously submitted to cataract surgery. Most of the patients with cataract said to be satisfied with their vision; of the 5 patients that could be operated only 2 decided to do it. Low vision for reading, AMD and cataract were the main findings.

OBJETIVO: Estudar a ação da injeção subconjuntival de soro autógeno na evolução das complicações, após queimadura ocular grave por álcali induzida experimentalmente em coelhos. MÉTODOS: Foram utilizados trinta olhos de coelhos da raça Nova Zelândia, divididos em dois grupos, de 15 coelhos cada, ambos submetidos à queimadura grave por álcali. Em 15 olhos (grupo tratado), efetuamos, imediatamente após a queimadura, injeção de soro autógeno subconjuntival. Os resultados foram avaliados e comparados logo após a queimadura e nos dias 1, 3, 7, 15 e 30 por meio de ectoscopia biomicroscopia ocular. RESULTADOS: O grupo tratado apresentou melhor resposta de reepitelização da córnea, na fase inicial do processo do que o grupo controle (grupo sem soro autógeno), diferença esta estaticamente significante, assim como resultado final com um menor número de complicações. CONCLUSÃO: Os resultados obtidos neste estudo experimental sugerem que soro autógeno pode ter efeito no processo de cicatrização dos olhos após queimadura alcalina, diminuindo complicações tardias e melhorando o prognóstico quanto à estabilidade do processo inflamatório cicatricial.PURPOSE: To determine the activity of subconjunctival injection of autogenous serum in the treatment of ocular alkali burn induced experimentally in rabbits. METHODS: Thirty eyes of 30 New Zeland albino rabbits were divided into two groups of 15 and submitted to ocular alkali burn. One group (treated group) received immediately after the alkali burn a subconjunctival injection of autogenous serum. The results were compared and recorded immediately after the alkali burn and on days 1, 3, 7, 15, and 30 by external ocular examination and manual portable slit lamp. RESULTS: The treated group presented a better reepithelialization of the cornea than the control group, in the beginning of the process, with a statistically significant difference; and a final result with less complications, in the study group. CONCLUSION: The results obtained in this experimental study suggest that the autogenous serum may have a primary effect on the process of scarring of the eyes after ocular alkali burn, decreasing late complications and improving the prognosis related to the stability of the inflammatory scarring process.

OBJETIVO: Traçar o perfil do paciente portador de alergia ocular baseado nos seus dados epidemiológicos, na sua resposta ao tratamento e nas complicações de sua doença. MÉTODOS: Foram analisados 172 prontuários dos pacientes com diagnóstico de ceratoconjuntivite alérgica primaveril (CCP), atópica (CCA), sazonal (CAS) e perene (CAP) e que tiveram seguimento mínimo de 6 meses no Ambulatório de Alergia Ocular do Departamento de Oftalmologia da Santa Casa de São Paulo. A análise estatística foi feita pelo método da variância e qui-quadrado. RESULTADOS: A alergia ocular mais freqüente foi a CCP (n=95; 55,2%); com predominância do sexo masculino (n=117; 68,1%). A idade média foi 11,7 anos (± 8,7 anos). Também foi a doença que mais acometeu a visão, sendo que 52,7% tiveram AV = 1,0; dos pacientes com ceratoconjuntivite atópica, 54,4% tinham AV=1,0, daqueles com conjuntivite alérgica sazonal, 75% e daqueles com conjuntivite alér-gica perene, 100% tinham AV=1,0. 96,8% dos portadores de ceratoconjuntivite alérgica primaveril apresentaram maior freqüência das crises no calor, e 91,4% dos portadores de ceratoconjuntivite alérgica atópica no frio. As alterações corneais foram mais freqüentes nos pacientes com ceratoconjuntivite alérgica primaveril, com ceratite presente em 57 pacientes (60,0%). Entre as medicações usadas, 21,6% (n=45) precisaram de corticosteróides, sendo que 36,8% destes pacientes portavam conjuntivite alérgica perene (n=35). O cromoglicato dissódico foi, dentre as demais medicações, a que em mais pacientes pareceu controlar os sintomas, com algum sucesso, em todas as formas de alergia ocular. A ressecção de papilas gigantes com transplante autólogo de conjuntiva foi feita em oito pacientes, sendo sete deles portadores de ceratoconjuntivite alérgica primaveril e um de ceratoconjuntivite alérgica atópica. CONCLUSÕES: Ceratoconjuntivite alérgica primaveril é o tipo de conjuntivite alérgica mais freqüente em nosso serviço. A droga mais eficaz na nossa experiência parece ser o cromoglicato dissódico, sendo que os corticosteróides são potentes agentes antiinflamatórios que, nestes pacientes, muitas vezes são as únicas drogas capazes de fazer cessar as crises.PURPOSE: To establish a profile of the allergic patient based on clinical manifestations, epidemiological data, treatment response and complications of ocular allergy. METHODS: A retrospective and descriptive study, in which we analyzed data of 172 patients suffering from perennial conjunctivitis, seasonal conjunctivitis, atopic keratoconjunctivitis and vernal keratoconjunctivitis, with at least 6 months of follow-up. The study was performed at the Ocular Allergy Sector of the "Santa Casa de São Paulo". Statistical analysis was performed using variance method and qui-square test. RESULTS: The most frequent form of ocular allergy was vernal keratoconjunctivitis (n=95; 55.2%) and the male sex was predominant (n=117; 68.1%). Mean age was 111.7 years (± 8.7). Vernal keratoconjunctivitis was the disease that most impaired vision, also responsible for corneal complications in 60% of the patients (n=57). Among medications, corticosteroids were used by 21.6% (n=45) of patients and 36.8% of them had vernal keratoconjunctivitis. Disodium cromoglycate was not only the most used medication, but able to successfully control symptoms in most cases. Papilla resection with autologous transplantation had to be performed in 8 patients. CONCLUSIONS: Vernal keratoconjunctivitis showed to be the most frequent form of ocular allergy in our service. The most efficient drug in our experience seemed to be disodium cromoglycate. Corticosteroids were shown to be as potent anti-inflammatory drugs and sometimes were the only way to discontinue crises.

Objetivo: Estabelecer os agentes mais comumente isolados nas ceratites, conjuntivites e endoftalmites no Laboratório de Microbiologia da Santa Casa de Misericórdia de São Paulo, assim como determinar o perfil de sensibilidade destes agentes aos antibióticos. Métodos: Realizamos estudo retrospectivo analisando todos os prontuários oftalmológicos do banco de dados do Laboratório de Microbiologia da Santa Casa de São Paulo. Foram coletados os dados de todos os pacientes submetidos à colheita de material ocular nos últimos 5 anos (1994-1999). Selecionamos para o estudo apenas os pacientes com diagnóstico clínico de conjuntivite, ceratite e endoftalmite. Os respectivos antibiogramas foram realizados e somente dados de antibiótico com aplicação em oftalmologia foram analisados. Resultados: Dos 568 casos pesquisados, foram encontrados 282 casos de ceratite bacteriana (49,6%), 214 de conjuntivites (37,7%), 72 de endoftalmites (12,7%). Obtivemos cultura negativa em 333 casos (58,9%). O agente mais freqüentemente isolado foi S. aureus (73 casos; 31,0%). Nas conjuntivites, o segundo agente mais freqüente foi Candida sp (6,6%) e 38,3% das culturas foram negativas. Nas ceratites, Pseudomonas aeruginosa apresentou 3,9% de positividade. Nas endoftalmites, nas quais houve apenas 32% de positividade, o segundo agente mais freqüente foi o S. pneumoniae (4,2%). Conclusão: Culturas negativas foram achado mais freqüente. Quando nos deparamos com uma infecção ocular, as chances de o causador ser S. aureus são muito grandes, portanto antes mesmo de se obter o resultado da cultura, tratamento que inclua S. aureus pode ser indicado.Purpose: To establish the most frequently isolated agents in keratitis, conjunctivitis and endophthalmitis by the Laboratory of Microbiology/ Santa Casa of São Paulo, and to determine their antimicrobial sensitivity. Methods: A retrospective study including all ophthalmologic charts of the Laboratory of Microbiology/ Santa Casa of São Paulo was conducted. All ocular material collected during the last five years was analyzed. We included in this study only patients whose diagnoses were conjunctivitis, keratitis and endophthalmitis. Susceptibility tests of drugs with ophthalmological application were analyzed. Results: In a total of 568 cases, we found 282 of bacterial keratitis (49.6%), 214 of conjunctivitis (37.7%) and 72 of endophthalmitis (12.7%); three hundred thirty-three negative culture cases were found (58.9%). S. aureus was the most frequently isolated agent (n=73; 31.0%). Conjunctivitis showed Candida sp in 6.6% of all cases, and 38.3% of the cultures were negative. When the diagnosis was keratitis, P. aeruginosa was positive in 3.9% and 71.6% of the cultures were negative. Endophthalmitis showed only 32% of positivity. Conclusion: Inconclusive cultures were the majority. Facing an ocular infection, there is a considerable chance to find S. aureus.

OBJETIVO: Verificar a existência de diferença estatisticamente significativa entre exames de campimetria computadorizada, realizados com a utilização da correção total e do equivalente esférico, em pacientes com ametropia cilíndrica de valores iguais ou maiores que 1,50 dioptrias. MÉTODOS: Vinte pacientes (35 olhos) foram submetidos a exame de campo visual, perimetria computadorizada Humphrey - estratégia SITA 24-2, usando em um exame a correção total e em outro o equivalente esférico. Foram utilizados como parâmetros de comparação os valores de Mean Deviation, Pattern Standard Deviation, perdas de fixação, falso-positivos, falso-negativos e duração dos exames. RESULTADOS: Os parâmetros Mean Deviation, Standard Pattern Deviation, falso-positivos, falso-negativos e duração do exame não apresentaram diferença estatisticamente significativa. A perda de fixação foi maior no grupo usando correção cilíndrica total, dado estatisticamente significante. CONCLUSÃO: Exames de campo visual realizados com equivalente esférico não mostram diferença na sensibilidade retínica quando comparados com o uso de correção cilíndrica total, dentro dos padrões adotados neste estudo. METHODS: Twenty patients (35 eyes) underwent visual field testing with Humphrey (SITA 24-2) perimetry using the full cylindrical correction and the spherical equivalent at a random sequence. Mean Deviation, Standard Pattern Deviation, fixation loss, false positive and false negative results and test duration were compared. RESULTS: No difference was found regarding Mean Deviation, Standard Pattern Deviation, false positive and false negative results and test duration. Fixation loss was significantly higher with the full cylindrical correction. CONCLUSION: Visual fields performed with the spherical equivalent show no change in the retinal sensitivity when compared to examinations done with the full cylindrical correction for the same eye.
