17. Alemtuzumab-based conditioning regimen before hematopoietic stem cell transplantation in patients with short telomere syndromes: a retrospective study of the SFGM-TC

Abstract

While HSCT is the only curative option for patients with short telomere syndromes (STSs) and severe bone marrow failure (BMF) or myeloid malignancies (MM), their increase sensitivity to conditioning regimen strongly affect outcomes. To minimize HSCT related mortality, alemtuzumab-based conditioning regimens have been proposed, but the number of patients transplanted with those regimens reported in the literature remains very low. We retrospectively analyzed outcome of adults and adolescents with STSs transplanted after an alemtuzumab, fludarabine and cyclophosphamide based regimen registered by the SFGM-TC. Seven patients were transplanted for a BMF and 5 for a MM (median age 34 years, (IQR [22–45])). The 2-year GRFS for patients with MM was 20% (95% CI [3;100]), and 57% (95% CI [30;100]) in others. In univariate (hazard ratio, HR = 6, 95% CI [1;31]) and multivariate analysis (HR = 26, 95% CI [2;414]) stem cell source was a predictive factor for GRFS. Three of the 5 patients with pre-transplant MM relapsed and 2 of them died at last follow up. The 2-year OS was 66% (95% CI [43;99]) in the whole cohort with a median follow up of 32 months (IQR [13–56]). In conclusion, Alemtuzumab-based conditioning regimen with bone marrow is an option for patients with STSs and BMF, but others modalities have to be explored for patients with MM.

Abstract

Background: Autologous and allogeneic hematopoietic stem cell transplantation of cytokine‐mobilized peripheral blood stem cells (PBSCs) is increasingly used to treat patients with hematologic disorders. Different types of vascular access have been exploited for the apheresis procedure, including peripheral veins (PV) and central venous catheter (CVC). In some cases, PV access is unavailable. There are few published data on the efficiency and quality of harvesting with different types of vascular access. This study brings out complications and morbidity of this procedure linked to these different access. Methods: We performed a comparative, retrospective, single‐center study of hematopoietic stem cell collection using these two types of vascular access. We compared the efficiency and complication rate for 617 adults apheresis sessions in 401 patients and healthy donors, for PBSC collection via PV or CVC between 2010 and 2016. The quality of the HSC product was evaluated in terms of the total CD34 + count and neutrophil contamination. Results: The PV and CVC groups did not differ significantly in terms of the quality of the apheresis product, mean ± SD CD34 + cells collected in PV group was 383.1 ± 402.7 × 10e6 and 298.8 ± 372.7 × 10e6 and the level of neutrophil contamination was 21.0 ± 17.8% in the PV group and 20.6 ± 18.4% in the CVC group. The complication rate did not differ between the two groups. Conclusion: The type of vascular access for apheresis hematopoietic stem cell harvesting must be determined by trained staff. Abstract

Les rechutes sont la première cause de décès chez les patients greffés pour une leucémie aiguë myéloïde et lymphoïde ou pour un syndrome myélodysplasique. Le nombre de patients éligibles à l’allogreffe de cellules souches hématopoïétiques augmente au fil des années et pour nombre d’entre eux, seul un conditionnement d’intensité réduite est possible, conditionnement associé à un risque de rechute post-allogreffe plus élevé. De plus, l’amélioration des connaissances et les progrès biologiques permettent de mieux identifier les pathologies à risque très élevé de rechute et d’évaluer la maladie résiduelle pré-allogreffe. Ceci permet de prévoir un traitement de maintenance post-greffe dans le cadre d’une stratégie de prophylaxie. Le suivi des marqueurs de maladie résiduelle et du chimérisme en post-greffe permet une stratégie préemptive. Au cours de cette quatorzième session des ateliers d’harmonisation des pratiques en allogreffe, nous avons mis à jour les recommandations de prévention de la rechute post-allogreffe à partir des données de la littérature. Des médicaments innovants ont été mis au point ces dernières années. Leur profil de toxicité permet leur utilisation sous réserve de précautions. Nous avons revu l’utilisation des inhibiteurs de FLT3 pour les leucémies aiguës myéloïdes, de BCR::ABL pour les leucémies aiguës lymphoïdes à chromosome Philadelphie, des agents hypométhylants et inhibiteurs de Bcl-2 pour les leucémies aiguës myéloïdes et syndromes myélodysplasique. L’immunomodulation et la réinjection de lymphocytes du donneur restent d’actualité et nous avons revu leurs indications. Enfin, les modalités de suivi et d’accompagnement des patients recevant ces traitements de prévention ont été abordées.

Abstract

International audience; The role of reduced intensity allogeneic stem cell transplantation for the treatment of relapsed/refractory Hodgkin lymphoma remains controversial. We retrospectively analyzed 191 patients who underwent reduced intensity allogeneic stem cell transplantation between 1998 and 2008 for relapsed or refractory Hodgkin lymphoma and whose data were reported to the French registry. Median follow-up was 36 months. Estimated 3 years overall survival, progression free survival, cumulative incidence of relapse and cumulative incidence of non relapse mortality were 63%, 39%, 46%, and 16% respectively. There was no difference in outcome between patients in complete response and in partial response at time of transplantation for overall survival (70% versus 74%, no significant difference) and progression free survival (51% versus 42%, no significant difference). Patients with chemoresistant disease had a shorter overall survival (39% at 3 years p=0.0003) and progression free survival (18% at 3 years p=0.001) than patients in complete remission. The use of umbilical cord blood as stem cell source was associated with a poor outcome with an increased risk of death with a hazard ratio of 3.49 (95% confidence interval 1.26 to 9.63, p=0.016). The use of peripheral blood was associated with a better outcome for patients who where alive after 1 year post transplantation with a hazard ration of 0.38 (95% confidence interval 0.17 to 0.83, p=0.016). Disease status at transplantation remains the most important risk factor for outcome. Our data suggest that the use of peripheral blood should be preferred whereas umbilical cord blood should be used with caution.

Abstract

ObjectivesPrimary GI T-cell lymphoproliferative diseases (T-LPD) are heterogeneous entities, which raise difficult diagnosis and therapeutic challenges. We have recently provided evidences that lymphomas complicating coeliac disease (CD) arise from innate-like lymphocytes, which may carry NK receptors (NKRs).DesignNKRs expression was compared by flow cytometry in intraepithelial lymphocytes (IEL) from CD, type I or type II refractory CD (RCD). NKp46 was next assessed by immunohistochemistry in paraffin-embedded biopsies from 204 patients with CD, RCDI, RCDII or GI T-cell lymphomas and from a validation cohort of 61 patients. The cytotoxic properties of an anti-NKp46 monoclonal antibody conjugated to pyrrolobenzodiazepine (PBD) was tested ex vivoin human primary tumour cells isolated from fresh duodenal biopsies.ResultsNKp46 (but not CD94, NKG2A, NKG2C, NKG2D) was significantly more expressed by malignant RCDII IEL than by normal IEL in CD and RCDI. In paraffin biopsies, detection of >25 NKp46+ IEL per 100 epithelial cells discriminated RCDII from CD and RCDI. NKp46 was also detected in enteropathy-associated T-cell lymphomas (EATL, 24/29) and in monomorphic epitheliotropic intestinal T-cell lymphomas (MEITL, 4/4) but not in indolent T-LPD (0/15). Treatment with anti-NKp46-PBD could efficiently and selectively kill human NKp46+ primary IEL ex vivo.ConclusionNKp46 is a novel biomarker useful for diagnosis and therapeutic stratification of GI T-LPD. Strong preclinical rationale identifies anti-NKp46-PBD as a promising therapy for RCDII, EATL and MEITL.
