Abstract

ObjectivesThe objectives of this study were to examine left atrial (LA) function and compliance using two-dimensional (2D) strain analysis in adult patients with corrected Tetralogy of Fallot (c-ToF) and to investigate the relationships between LA function and patient characteristics, especially history of life-threatening arrhythmia (h-LTA).MethodsFifty-one c-ToF patients (34 males; age, 39 ± 15 years; h-LTA, n = 13) were included in this retrospective monocenter study. In addition to a 2D standard echocardiography examination, 2D strain analysis was performed to assess left ventricular (LV) and LA functions, including peak-positive LA strain (LAS—reservoir function) and LA compliance [defined as the ratio LAS/(E/Ea)].ResultsPatients with h-LTA were older and exhibited a longer QRS duration. LV ejection fraction, LAS and LA compliance were significantly lower in the group of patients with h-LTA. Indexed LA and RA volumes, RV end-diastolic area was significantly higher and RV fractional area change significantly lower in the h-LTA group. Abstract

Background Right heart catheterization (RHC) is a high‐risk procedure in children with pulmonary arterial hypertension without clear guidelines for the indications and targets of invasive reassessment. Our objectives are to define the aims of repeated RHC and evaluate the correlation between noninvasive criteria and hemodynamic parameters. Methods and Results Clinical and hemodynamic characteristics from 71 incident treatment‐naïve children (median age 6.2 years) with pulmonary arterial hypertension who had a baseline and reevaluation RHC were analyzed. Correlations between noninvasive predictors and hemodynamic parameters were tested. Adverse outcomes were defined as death, lung transplantation, or Potts shunt. At baseline, pulmonary vascular resistance index (hazard ratio [HR] 1.07 per 1 WU·m2 increase [95% CI, 1.02–1.12], P=0.002), stroke volume index (HR 0.95 per 1 L·min−1·m−2 increase [95% CI, 0.91–0.99], P=0.012), pulmonary artery compliance index (HR 0.16 per 1 mL·mm Hg−1·m−2 increase [95% CI, 0.051–0.52], P=0.002), and right atrial pressure (HR, 1.31 per 1 mm Hg increase [95% CI, 1.01–1.71], P=0.043) were associated with adverse outcomes. Pulmonary vascular resistance index, pulmonary artery compliance index, and right atrial pressure were still associated with a worse outcome at second RHC. Noninvasive criteria accurately predicted hemodynamic evolution; however, 70% of the patients who had improved based on noninvasive criteria still presented at least 1 “at risk” hemodynamics at second RHC. Conclusions Pulmonary vascular resistance index, pulmonary artery compliance index, and right atrial pressure are solid predictors of adverse outcomes in pediatric pulmonary arterial hypertension and potential therapeutic targets. Noninvasive criteria accurately predict the evolution of hemodynamic parameters, but insufficiently. Repeated RHC are helpful to identify children with persistent higher risk after treatment introduction.

Abstract

A single missense variant of the TMPO/LAP2α gene, encoding LAP2 proteins, has been associated with cardiomyopathy in two brothers. To further evaluate its role in cardiac muscle, we included TMPO in our cardiomyopathy diagnostic gene panel. A screening of ~5000 patients revealed three novel rare TMPO heterozygous variants in six males diagnosed with hypertrophic or dilated cardiomypathy. We identified in different cellular models that (1) the frameshift variant LAP2α p.(Gly395Glufs*11) induced haploinsufficiency, impeding cell proliferation and/or producing a truncated protein mislocalized in the cytoplasm; (2) the C-ter missense variant LAP2α p.(Ala240Thr) led to a reduced proximity events between LAP2α and the nucleosome binding protein HMGN5; and (3) the LEM-domain missense variant p.(Leu124Phe) decreased both associations of LAP2α/β with the chromatin-associated protein BAF and inhibition of the E2F1 transcription factor activity which is known to be dependent on Rb, partner of LAP2α. Additionally, the LAP2α expression was lower in the left ventricles of male mice compared to females. In conclusion, our study reveals distinct altered properties of LAP2 induced by these TMPO/LAP2 variants, leading to altered cell proliferation, chromatin structure or gene expression-regulation pathways, and suggests a potential sex-dependent role of LAP2 in myocardial function and disease.

Abstract

BACKGROUND: Limited data are available describing paediatric pulmonary arterial hypertension. AIMS: To characterize the epidemiology, management and impact on quality of life and outcome of paediatric pulmonary arterial hypertension, excluding persistent pulmonary hypertension of the newborn and pulmonary arterial hypertension caused by congenital heart disease. METHODS: In this multicentre study, children with pulmonary arterial hypertension were included and followed prospectively for two years at 21 referral centres in France. WHO functional class, 6-minute walk distance and quality of life (CHQ-PF50 questionnaire) were evaluated. RESULTS: Fifty children were included with a mean age of 8.9 +/- 5.4 years from May 2005 until June 2006. The estimated prevalence for pulmonary arterial hypertension was 3.7 cases/million. Patients had idiopathic pulmonary arterial hypertension (60%), familial pulmonary arterial hypertension (10%), pulmonary arterial hypertension associated with, but not caused by, congenital heart disease (24%), pulmonary arterial hypertension associated with connective tissue disease (4%) or portal hypertension (2%). During follow-up, the combination of pulmonary arterial hypertension-specific therapies was increasingly prescribed (44% patients versus 22% at inclusion). Patients remained stable regarding clinical status, 6-minute walk distance and quality of life. Survival estimates after one and two years were 86% (95% CI 76, 96) and 82% (95% CI 71, 93), respectively. CONCLUSIONS: In children, idiopathic/familial pulmonary arterial hypertension accounts for the majority of cases. A specific pulmonary arterial hypertension group in conjunction with congenital heart disease can be identified that resembles patients with idiopathic pulmonary arterial hypertension. Combined pulmonary arterial hypertension-specific therapies may have contributed to disease stability and favourable survival.

Abstract

Fulminant Influenza type A-associated myocarditis are very rare in children. The clinical presentation is non specific, like flu illness, cardiogenic shock or sudden cardiac arrest. We report the case of a eight years old girl with a fulminant Influenza A-associated myocarditis with a fatal evolution despite the use of an extracorporeal membrane oxygenation (ECMO). The aim of this observation is to remind that influenza in children, usually considered as a benign illness, can exceptionally be complicated by a fulminant myocarditis. Because the possibility to recover a full myocardial function, the persistence of severe heart failure despite the medical treatment should conduct rapidly to a mechanical circulatory assistance.

22. Reversal of Cardiac Hypertrophy With a Ketogenic Diet in a Child With Mitochondrial Disease and Hypertrophic Cardiomyopathy

Abstract

Mitochondrial diseases are rare metabolic disorders that can cause hypertrophic cardiomyopathy. Herein we describe the case of a 3-year-old girl diagnosed with mitochondrial disease (mutation m.5559A>G in the mitochondrial-tRNATrpgene). Echocardiography showed left ventricular hypertrophy with an enlarged septum (9 mm, zscore = 3.26). Antioxidant supplementation associated with a high-fat ketogenic diet was introduced and, as expected, improved neurologic status. In addition, heart parameters improved with normalisation of interventricular septum thickness at 6 years of age (6 mm, zscore = 1.05). In this case report, we suggest that a ketogenic diet may improve hypertrophic cardiomyopathy in the context of mitochondrial disease.

37. Infective endocarditis in children with congenital heart disease compare to structurally normal heart.

Abstract

Epidemiologic of infective endocarditis (IE) is changing and paediatric population is heterogeneous. We compare clinical characteristics according cardiac status among children. A retrospective multi centric study was conducted over a decade. Clinical presentation, treatment, outcome, microorganism, underlying congenital heart disease data were recorded. We identified 64 cases of definite IE between January 2008 to December 2018 median age was 8.0 [0.8–13.2] and 26 were girls, S aureus was the most common pathogen, n = 22 (34%), presented in Table 1. CHD represented 45 cases of IE and 38 patients had prior cardiac surgery, they were significantly younger and community acquired IE was more common. New cardiac murmur 11 (24%) vs. 11 (58%), P < 0.02, acute ischemic stroke 1 (2%) vs. 4 (21%), P < 0.02, and heart failure 11 (22%) vs. 10 (54%), P < 0.03 were significantly higher in non-CHD group. Major MDC was met more often in patients without CHD than in patients with CHD (84% versus 46%, P < 0.006). There was no difference between IE related to children with or without CHD considering the side of the localisation. Valvular replacement was more frequent in this population [13 (28%) vs. 1 (5%) P < 0.05]. Overall morbidity rate was not significantly different but neurologic complications were significantly more prevalent in patients without CHD 7 (15) vs. 11 (58). Children without CHD presented significantly more neurologic impairment 1 (2%) vs. 4 (21%), P < 0.05. CHD patients seem older, require more valve replacement and present community acquired infection when patients without CHD presented more neurological outcome and more hospital acquired infection. Abstract

L’hémangiome est une tumeur vasculaire bénigne qui atteint 4 % à 10 % des nouveau-nés. Depuis 13 ans le propranolol est devenu le traitement de 1èreintention des hémangiomes [1]bien que cet effet ait été découvert par hasard chez des enfants traités pour des indications cardiaques. À ce jour un seul cas de MIN a été publié, par les autorités de santé du Canada. Nous rapportons 5 autres cas mondiaux de nourrissons décédés de MIN alors qu’ils recevaient du propranolol depuis plusieurs semaines. Les causes de MIN/SIDS (Sudden Infant Death Syndrome) sont complexes et font appel à un modèle dit de ‘triple risque’ [2]associant causes congénitales, environnement et facteurs déclenchants. Le propranolol pourrait-il être un facteur favorisant la MIN ?

L’hémangiome est une tumeur bénigne des tissus mous vasculaires observée chez 4 % à 10 % des nourrissons. La plupart d’entre eux disparaissent spontanément, mais certains peuvent nécessiter un traitement médical ou chirurgical en raison du risque de défiguration ou de risque vital par extension aux voies aériennes supérieures. Les corticostéroïdes systémiques ont été utilisés comme traitement de première intention mais au prix de graves effets indésirables. Le propranolol, un bêta-bloquant non sélectif, a été par hasard découvert comme efficace dans l’hémangiome infantile chez plusieurs nourrissons traités pour des indications cardiaques. Nous rapportons le cas d’un nourrisson de 5 mois décédé de façon inattendue (mort inattendue du nourrisson [MIN]) alors qu’il recevait un traitement au propranolol depuis 9 semaines.

49. 284: Longitudinal left ventricular strain impairment in type 1 diabetic children: a prospective 2D speckle strain imaging study.

Abstract

Background The relation between type 1 diabetes and cardiac structure and function in children is poorly documented. We used 2D speckle strain imaging to investigate whether children and adolescents with type 1 diabetes have early echocardiographic signs of subclinical cardiac dysfunction and whether state of metabolic control and diabetes duration are of influence. Methods Standard 2D echocardiography, mitral TDI and 2D speckle strain imaging were prospectively performed in type 1 diabetic children and compared them to age and sex-matched healthy control subjects. Standard echocardiographic indices of global systolic and diastolic function, early peak diastolic mitral velocity (Ea), longitudinal strain (LS), radial strain (RS) and circumferential strain (CS) were investigated. Results Overall 49 consecutive type 1 diabetic children (age: 12.3 [6-18] years; BMI: 19±2.8; males: 20) were compared to 49 control subjects. There was no difference between two groups for mean arterial blood pressure, heart rate, left ventricular ejection fraction, left ventricular mass, conventional diastolic mitral Doppler parameters (E, E/A, MTD) and Ea. A significant decrease of LS was identified in type 1 diabetic children while RS and CS did not differ. LS was positively correlated with HbA1c (r: 0,28; p<0,001) and with diabetes duration (r: 0,20; p<0,001). Conclusion We demonstrated that left ventricular longitudinal myocardial strain is decreased in young patients with uncomplicated type 1 diabetes. Metabolic control may be the main risk factor for these myocardial changes. This finding might be considered a very early preclinical alteration potentially related to subsequent development of diabetic cardiomyopathy. [ABSTRACT FROM AUTHOR]
