The article analyses the features of P. Kozytskyi’s choral letter in the cycle “Eight Preludes- Songs”. Written in the traditions of M. Leontovych, the works of P. Kozytskyi show new views on the interpretation of a capella sound as an individually interpreted “choral instrumentalism”. The works demonstrate analysed two of the main semantic spheres of the choral genre in Ukrainian folklore music. The first sphere is the tragedy in the ballad “Oh, in the field, the room was standing...”, where poetic symbolism and polyphonic musical development are attained, as in the best works of M. Leontovych, such as “Pryalya” and others. The second sphere is a baby carol, which goes back to the ancient folklore patterns in music, and resonates with M. Leontovych’s “Dudaryk” and “Shchedryk”. The Eight Prelude Songs cycle by P. Kozytskyi is a collection of pearls of Ukrainian choral music. The purpose of the article is to outline the characteristic features of the works of the choral cycle “Eight Preludes-Songs” by P. Kozytskyi in musical-historical and artistic aspects on the example of selected works. Written as a tribute to the memory of M. Leontovych, this artistic material is set out in the mentioned traditions of musical folklore and “choral instrumentalism” of the Master, and bears the mark of the best modernist quest of the time. P. Kozytskyi’s style in the early 1920’s has quite different incarnations and actively evolves, which M. Yurchenko traces on the example of spiritual works: in the early compositions (1910’s) the influence of the “Petersburg style” is felt, as early as in the 20th. in those years avant-garde features, connected with intense imagery, bold conflicting comparisons, a peculiar mode sphere with natural reversals, harmony with harmless consonants, rare-life doubles, tolerant dissonances, impositions, livings, interferences, are clearly manifested. The musical compositions of the cycle are polyphonic miniatures on folk-song basis, created in continuation of M. Leontovych’s method. The latter, according to P. Kozytskyi, is based on “choral instrumentation” – the interpretation of choral parts as instrumental, which provides the “song soul” with a special colour and richness of the choral palette, in which the choir is a “magic lantern" designed to set the meaning of the song, that is, to be the newest means of creating artistic and emotional expression. Written in the traditions of the contemporary M. Leontovych, a few years after the death of the artist, P. Kozytskyi’s works show new views on the interpretation of acapella sound, above all as individually interpreted “choral instrumentalism”. As a result of the conducted research, we come to the conclusion about the innovative features of P. Kozytskyi’s choral letter in the series “Eight Preludes-Songs”. Written in the traditions of the contemporary M. Leontovych, a few years after the artist’s death, P. Kozytskyi’s works show new facets of the acapella sound, above all as individually interpreted “choral instrumentalism”.

O presente artigo analisa os jogos linguísticos presentes em fontes de diferentes naturezas, no projeto educativo e civilizacional orientado para os indígenas brasileiros, manifestado por um grupo de escritores(as) da virada do século XIX e início do XX. Esse trabalho delimita-se entre 1890 e 1910 e considerou a ambiência intelectual que elegeu a temática indigenista como cerne de um dos projetos educacionais deste período. A análise pauta-se no Contextualismo Linguístico - Pocock e Quentin Skinner - cujo foco encontra-se na dimensão pragmática do discurso político e relação entre a realidade experienciada e a linguagem. Em relação às fontes, as mesmas foram produzidas por diferentes sujeitos, que evidenciaram, na cena pública, concepções acerca do tema deste trabalho. Conquanto os(a) escritores(a) elencados aqui, tenham posicionamentos genéricos sobre algumas questões relativas aos indígenas brasileiros, há um consenso de que a educação seria o condão que viabilizaria o processo de inserção destes povos no processo civilizador, com um viés que tinha como projeto a construção de uma “identidade brasileira”, na qual os indígenas ocupavam um lugar importante. This article analyzes the linguistic games present in sources of different natures, in the educational and civilizing project oriented towards Brazilian indigenous people, manifested by a group of writers from the turn of the 19th century and the beginning of the 20th century. This work is delimited between 1890 and 1910 and considered the intellectual environment that chose the indigenist theme as the core of one of the educational projects of this period. The analysis is guided by Linguistic Contextualism - Pocock and Quentin Skinner - whose focus is on the pragmatic dimension of political discourse and the relationship between experienced reality and language. Regarding the sources, they were produced by different subjects, who showed, in the public scene, conceptions about the theme of this work. Although the writers listed here have generic positions on some issues related to Brazilian indigenous peoples, there is a consensus that education would be the power that would enable the process of insertion of these peoples in the civilizing process, with a bias that had as a project the construction of a “Brazilian identity”, in which the indigenous people occupied an important place. Este artículo analiza los juegos lingüísticos presentes en fuentes de diferente naturaleza, en el proyecto educativo y civilizador orientado hacia los indígenas brasileños, manifestado por un grupo de escritores de finales del siglo XIX y principios del siglo XX. Este trabajo se delimita entre 1890 y 1910 y considera el ambiente intelectual que eligió la temática indigenista como eje de uno de los proyectos educativos de este período. El análisis se guía por el Contextualismo Lingüístico - Pocock y Quentin Skinner - cuyo foco está en la dimensión pragmática del discurso político y la relación entre la realidad experimentada y el lenguaje. L'analyse est guidée par le contextualisme linguistique - Pocock et Quentin Skinner - dont l'accent est mis sur la dimension pragmatique du discours politique et la relation entre réalité vécue et langage. En ce qui concerne les sources, elles ont été produites par différents sujets, qui ont montré, sur la scène publique, des conceptions sur le thème de cette œuvre. Bien que les auteurs listés ici aient des positions génériques sur certaines questions liées aux peuples indigènes brésiliens, il existe un consensus sur le fait que l'éducation serait le pouvoir qui permettrait le processus d'insertion de ces peuples dans le processus de civilisation, avec un parti pris qui avait comme projet la construction d'une « identité brésilienne », dans laquelle les indigènes occupent une place importante.

O artigo objetiva tratar do caráter utópico do discurso republicano brasileiro que circulou na imprensa periódica, entre o final do século XIX e início do XX. Ocorpo documental desse trabalho compreende duas revistas paranaenses, a Revista do Clube Curitibano (1890-1912) e A Escola (1906-1910). A opção por tais fontes ocorreu em razão de ambas apresentarem um núcleo de comum de colaboradores, fato que garante a detecção de rupturas e continuidades discursivas acerca da república e dos modos como viabilizá-la. O corpo de colaboradores dessas revistas era composto por agentes entendidos aqui como “atores linguísticos”, expressão de John Pocock (2003) que designa os que operam como articuladores da linguagem de uma época, visando à defesa de interesses, ideias e valores.Palavras-chave: República brasileira, imprensa periódica, utopia nacional.

Introdução: A sífilis é uma doença sistêmica, curável e de distribuição epidemiológica mundial. Sua transmissão ocorre na forma sexual, vertical ou sanguínea. No Brasil, a sífilis em gestantes é um agravo de notificação compulsória. O estudo tem como objetivo descrever a epidemiologia de casos notificados de sífilis em gestantes no município de Curitiba/PR. Método: Trata-se de um estudo e observacional descritivo de base documental. Os dados foram fornecidos pela Secretaria Municipal de Saúde de Curitiba através do registro SINAN. Foi utilizado para a análise o período de 2007-20017. Consideraram-se as variáveis: faixa etária, raça/cor, trimestre, escolaridade, classificação clínica, esquemas prescritos e tratamento de parceiro sexual. Para contraponto com média brasileira, utilizou-se o Boletim Epidemiológico da Sífilis, 2018. Purpose of the study: The purpose of the present study is to specify the notion of socio-cultural continuum seen through the choral art as it developed throughout independence in Ukraine and to identify its main tendencies as drivers of choral art development. In this regard, the research tasks are to investigate the meaning of the concepts of “continuum”, “socio-cultural continuum,” and to identify the main socio-cultural tendencies that, in our opinion, have the most significant influence on the development of choral art. Methodology: This study used a meta-analysis approach to solve the issues raised in the research. Reference sources, peer-reviewed databases like Education Resources Information Centre (ERIC), Scopus, Arts & Humanities Database, the Choral Scholar (online journal), Google Scholar, together with open-source-published researches and books (published either in Ukrainian or English) were used to obtain the relevant information. Main findings: It was found, that the socio-cultural continuum in Ukraine of the period of independence emerges as a complex hierarchical system of phenomena characterized by an inextricable link between the Spatio-temporal parameters of human beings, society, and culture. Applications of this study: Interpreting this concept in relation to the development of choral art as an imaginary model of a single socio-cultural space-time in Ukraine (region) occurring throughout the independence period in all the continuous hierarchical multiplicity of its phenomena related to the functioning of choral art identified several major trends that, in our opinion, were influenced by the intensity and nature of such development. This period, as a transition period characterized by a change in outlook, the revival of previously forbidden traditions, and the emergence of new socio-cultural trends not only intensified the development but also caused several challenges that Ukrainian society faced. Novelty/Originality of this study: This scientific exploration was an attempt to understand the theoretical essence of this continuum and its tendencies, which in the future will allow knowing more about their positive and negative orientations and ways.

Congenital microcephaly causes smaller than average head circumference relative to age, sex and ethnicity and is most usually associated with a variety of neurodevelopmental disorders. The underlying etiology is highly heterogeneous and can be either environmental or genetic. Disruption of any one of multiple biological processes, such as those underlying neurogenesis, cell cycle and division, DNA repair or transcription regulation, can result in microcephaly. This etiological heterogeneity manifests in a clinical variability and presents a major diagnostic and therapeutic challenge, leaving an unacceptably large proportion of over half of microcephaly patients without molecular diagnosis. To elucidate the clinical and genetic landscapes of congenital microcephaly, we sequenced the exomes of 191 clinically diagnosed patients with microcephaly as one of the features. We established a molecular basis for microcephaly in 71 patients (37%), and detected novel variants in five high confidence candidate genes previously unassociated with this condition. We report a large number of patients with mutations in tubulin-related genes in our cohort as well as higher incidence of pathogenic mutations in MCPH genes. Our study expands the phenotypic and genetic landscape of microcephaly, facilitating differential clinical diagnoses for disorders associated with most commonly disrupted genes in our cohort.

Resumo: Evidências científicas apontam que um sistema fortemente baseado em Atenção Primária à Saúde (APS) promove melhores desfechos em saúde. No Brasil, a APS acontece, no sistema público, preferencialmente, no modelo de Estratégia de Saúde da Família. São considerados atributos essenciais da APS: o acesso, a longitudinalidade, a abrangência dos serviços e coordenação do cuidado. Esta pesquisa, com metodologia qualitativa, visou explorar as barreiras de acesso à APS na visão dos usuários que frequentam uma UPA (Unidade de Pronto Atendimento). Revelaram-se barreiras: distância de casa, demora na resposta da APS, compreensão sobre o funcionamento do sistema, acesso facilitado à UPA para problemas não urgentes e horário de funcionamento abreviado da Unidade Básica de Saúde (UBS). Há percepção de que a UPA tem um acesso mais facilitado do que a UBS, apesar de não ser, em teoria, a porta de entrada oficial do sistema.

Introduction In countries where Haemophilus influenzae type B vaccine is used, Streptococcus pneumoniae is the most common cause of bacterial meningitis in young children and notable cause of morbidity/mortality. The authors present material of magnetic resonance imaging (MRI) of patients with pneumococcal meningitis from archive of Department of Diagnostic Imaging of Institute of Mother and Child in Warsaw. Materials and methods We performed 27 brain MRI scans and 1 follow-up computed tomography (CT) in 10 children (2 girls and 8 boys) aged from neonate to 5 years at disease onset with proven pneumococcal infection. Results Follow-up period range was 0–12 years. Two children underwent only one MRI, one of them died before follow-up and the other was lost from further observation. There was one case of relatively benign disease course with mild changes on MRI. In another seemingly benign case, acute transient hydrocephalus was observed. Six children developed hydrocephalus, and two required ventriculoperitoneal shunting complicated by neuroinfection, shunt malfunction and revisions. Two patients developed epilepsy. In six children, spastic paresis of various severity was diagnosed, up to quadriplegia in one who is under the longest observation (>12 years) and survived in vegetative state. Three other children suffer from delayed psychomotor development to severe intellectual disability. Conclusions MRI shows perfectly the degree of central nervous system (CNS) damage during and after pneumococcal invasion. Despite appropriate treatment, disease course may be unpredictably serious. Attempts to eliminate the obligation to vaccinate are extremely irresponsible taking into account potential danger of death, vegetative state or another form of severe damage to CNS. Social and financial costs of care of survivors are very high with shunts placements and changes, (neuro)infections, rehabilitation, families breakdown, etc.

Hereditary spastic paraplegias (HSPs) are clinically and genetically heterogeneous neurodegenerative disorders. Numerous genes linked to HSPs, overlapping phenotypes between HSP subtypes and other neurodegenerative disorders and the HSPs’ dual mode of inheritance (both dominant and recessive) make the genetic diagnosis of HSPs complex and difficult. Out of the original HSP cohort comprising 306 index cases (familial and isolated) who had been tested according to “traditional workflow/guidelines” by Multiplex Ligation-dependent Probe Amplification (MLPA) and Sanger sequencing, 30 unrelated patients (all familial cases) with unsolved genetic diagnoses were tested using next-generation sequencing (NGS). One hundred thirty-two genes associated with spastic paraplegias, hereditary ataxias and related movement disorders were analysed using the Illumina TruSight™ One Sequencing Panel. The targeted NGS data showed pathogenic variants, likely pathogenic variants and those of uncertain significance (VUS) in the following genes: SPAST (spastin, SPG4), ATL1 (atlastin 1, SPG3), WASHC5 (SPG8), KIF5A (SPG10), KIF1A (SPG30), SPG11 (spatacsin), CYP27A1, SETX and ITPR1. Out of the nine genes mentioned above, three have not been directly associated with the HSP phenotype to date. Considering the phenotypic overlap and joint cellular pathways of the HSP, spinocerebellar ataxia (SCA) and amyotrophic lateral sclerosis (ALS) genes, our findings provide further evidence that common genetic testing may improve the diagnostics of movement disorders with a spectrum of ataxia-spasticity signs. São considerados atributos essenciais da APS, o acesso, a longitudinalidade, a abrangência dos serviços e a coordenação do cuidado. Metodologia: Qualitativa, visou explorar as barreiras de acesso à Unidade Básica de Saúde na visão dos usuários que frequentam uma UPA (Unidade de Pronto Atendimento). Resultados: Os discursos dos usuários revelaram diversas barreiras: distância de casa, organização do processo de trabalho da unidade, compreensão sobre o funcionamento do sistema de saúde, acesso facilitado à UPA para problemas não urgentes, horário de funcionamento abreviado da APS em relação à UPA. Conclusão: A análise de discurso dos entrevistados mostra que a UPA apresenta um acesso mais facilitado do que a APS. As políticas de gestão do Sistema Único de Saúde (SUS) precisam levar em conta a sobreposição de serviços entre UPA's e APS e avaliar experiências internacionais que simplificam o acesso do usuário ao sistema. Abstract: Introduction: Consistent scientific evidence indicates that a health system strongly based on primary health care (PHC) promotes better health outcomes. In Brazil, APS happens, in the public system, preferentially, in the Family Health Strategy (FHS) model. Accessibility, longitudinality, comprehensiveness and coordination of care are considered essential attributes of PHC. Methodology: Qualitative, aimed at exploring the barriers to access to the Basic Health Unit in the view of users who attend a ECU (Emergency Care Unit). Results: Users' discourses revealed several barriers: distance from home, organization of the work process of the unit, understanding of the functioning of the health system, facilitated access to ECU for non-urgent problems, PHC abbreviated working hours in relation to ECU . Conclusion: The interviewees' discourse analysis shows that ECU is easier to access than PHC. Health Unic System (HUS) management policies need to take into account the overlapping of services between UPAs and APS and to evaluate international experiences that simplify user access to the system.

В настоящее время возникает необходимость в новых подходах к преподаванию эстетических искусств, способных решать современные задачи творческого восприятия и развития личности в целом. В системе эстетического, творческого воспитания подрастающего поколения особая роль принадлежит изобразительному и музыкальному искусству. Авторами предложены правила и даны советы, как помочь детям видеть и понимать красоту окружающего мира, которая способствует воспитанию культуры чувств, развитию художественно-эстетического вкуса, трудовой и творческой активности.

Malformations of cortical development (MCDs) manifest with structural brain anomalies that lead to neurologic sequelae, including epilepsy, cerebral palsy, developmental delay, and intellectual disability. To investigate the underlying genetic architecture of patients with disorders of cerebral cortical development, a cohort of 54 patients demonstrating neuroradiologic signs of MCDs was investigated. Individual genomes were interrogated for single-nucleotide variants (SNV) and copy number variants (CNV) with whole-exome sequencing and chromosomal microarray studies. Variation affecting known MCDs-associated genes was found in 16/54 cases, including 11 patients with SNV, 2 patients with CNV, and 3 patients with both CNV and SNV, at distinct loci. Diagnostic pathogenic SNV and potentially damaging variants of unknown significance (VUS) were identified in two groups of seven individuals each. We demonstrated that de novo variants are important among patients with MCDs as they were identified in 10/16 individuals with a molecular diagnosis. Three patients showed changes in known MCDs genes and a clinical phenotype beyond the usual characteristics observed, i.e., phenotypic expansion, for a particular known disease gene clinical entity. We also discovered 2 likely candidate genes, CDH4, and ASTN1, with human and animal studies supporting their roles in brain development, and 5 potential candidate genes. Our findings emphasize genetic heterogeneity of MCDs disorders and postulate potential novel candidate genes involved in cerebral cortical development.

The creation of the National Freight Network as part of the Moving Ahead for Progress in the 21st Century Act (MAP-21) requires states to designate critical rural freight corridors (CRFCs). The MAP-21 act lists the criteria for designating CRFCs but does not specify the method. This paper proposes mileage-based, segment-based, and weighted average approaches to determine whether a corridor meets one of the CRFC criteria: a rural principal arterial that has a minimum 25% of truck traffic. The three approaches are explained, assessed, and mapped for the reader to compare the resulting networks. The paper then uses policy analysis techniques to assess the advantages and disadvantages of each approach. The analysis compares the approaches on the basis of network connectivity of the resulting CRFCs, the mileage of non-Interstate rural principal arterials, and robustness. Results indicate that state departments of transportation should use the mileage-based approach for computing truck traffic percentages when determining whether a corridor meets the minimum threshold of 25% truck traffic. The mileage approach combines the highest network connectivity, designates the highest number of non-Interstate rural principal arterial miles, and is a robust technique that controls for variable segment lengths.
