The fabrication and testing of Teflon AF-coated channels on silicon and bonding of the same to a similarly coated glass wafer are described. With water or aqueous solutions in such channels, the channels exhibit much better light conduction ability than similar uncoated channels. Although the loss is greater than extruded Teflon AF tubes, light throughput is far superior to channels described in the literature consisting of [110] planes in silicon with 45° sidewalls. Absorbance noise levels under actual flow conditions using an LED source, an inexpensive photodiode and a simple operational amplifier circuitry was 1× 10-4 absorbance units over a 10-mm path length (channel 0.17-mm deep ×0.49-mm wide), comparable to many commercially available macroscale flow-through absorbance detectors. Adherence to Beer's law was tested over a 50-fold concentration range of an injected dye, with the linear r2 relating the concentration to the observed absorbance being 0.9993. Fluorescence detection was tested with fluorescein as the test solute, a high brightness blue LED as the excitation source and an inexpensive miniature PMT. The concentration detection limit was 3 × 10-9 M and the corresponding mass detection limit was estimated to be 5 × 10 -16 mol. [ABSTRACT FROM PUBLISHER]

The objective of this work was to determine markers of endothelial cell activation in valves from patients with antiphospholipid syndrome (APS) and heart valve involvement, in order to establish a role for endothelial cells in the pathogenesis of the valvular disease. Sixteen valves from ten patients with APS, obtained from autopsies or removed during valve replacement, were studied. Two groups of valves were used as controls. One group included seven normal valves from patients who died from non-cardiac diseases. The other group of valves were obtained from patients with bacterial endocarditis during autopsies or valve replacement operations. Immunoperoxidase and immunofluorescence stainings with antibodies to human immunoglobulins, endothelial cells, α[sub 3] β[sub 1] integrin, collagen IV, laminin and fibronectin were employed.Three histopathological patterns were apparent: normal valves, valves with verrucous endocarditis and valves with fibrocalcific changes. In all the valves with verrucous endocarditis the following findings were observed: (1) increased expression of the α[sub 3] β[sub 1] integrin on the endothelial cells, (2) increased amount of collagen IV, laminin and fibronectin, (3) proliferation of blood vessels and (4) linear subendothelial deposition of immunoglobulins and complement. The valves with fibrocalcific changes were deformed and showed a thick layer of collagen IV, laminin and fibronectin, yet in two valves the indothelial cells showed an expression of the α[sub 3] β[sub 1] integrin. The control valves did not express the integrin and had only a thin subendothelial band of collagen IV.In valves from patients with APS,[sup 1] markers of endothelial cell activation are upregulated while the inflammatory exudate is scant. Fifty-seven eyes of 49 patients with senile retinoschisis were treated by argon laser photocoagulation and followed up for 3 to 6 years. In most of the treated eyes the retinoschisis either completely or partially collapsed and in none did the retinoschisis or loss of visual field progress further. No long-term complications due to the treatment were noted. In none of the treated eyes did retinal detachment develop. In view of this the indications at present for argon laser treatment in retinoschisis may be extended to include any case in which the retinoschisis progresses beyond the equator and visual field loss continues. This may prevent further visual field damage and other rare complications of slowly progressive retinoschisis and facilitate the many years' follow-up of elderly patients with retinoschisis who develop cataracts. A large choroidal metastasis was diagnosed in the right eye of a 44-year-old man referred for admission to hospital because of 'retinal detachment.' At the same time in the second, symptomless, eye a very small metastasis was observed. Close follow-up during the next month showed an extremely rapid deterioration of visual acuity and visual field in this eye. This is thought to be characteristic of such metastatic tumours in contrast to the slower progress of choroidal malignant melanoma. The application of local radiotherapy to the same eye led to an impressive improvement in the visual acuity and visual field. Background: Migraine, a condition affecting 12% of the population, is a prevalent cause of disability, significantly impacts individuals during their most productive working years. Several studies have established that a migraine patient's job performance is often limited by absenteeism and presenteeism. The present study aimed to investigate the impact of migraines on occupational burnout, which affects up to 40% of workers., Methods: A subset of participants from the Negev Migraine Cohort, including both migraine patients and non-migraine controls, were asked to complete the study questionnaire. The main exposures of interest were migraine diagnosis and severity. The primary outcome was occupational burnout. Migraine severity and associated disability were evaluated using the Migraine Disability Assessment (MIDAS) score, psychiatric comorbidities using the Depression, Anxiety and Stress Scale - 21 Items (DASS-21) scale and occupational burnout using the Maslach Burnout Inventory (MBI-GS version) scale. Statistical analyses included multivariable quantile regression models to identify associations and adjust for potential confounders., Results: In total, 675 migraine patients and 232 non-migraine participants participated in the study. Migraine patients exhibited higher rates of depression (mean DASS-21: 0.864 vs. 0.664, standardized mean difference (SMD) = 0.262), tended to work longer hours (median weekly hours: 40.0 vs. 36.0, SMD = 0.148) and expressed a preference for remote work (20.3% vs. 10.3%, SMD = 0.097). Migraine patients reported significantly higher levels of occupational burnout (mean burnout score: 3.46 vs. 2.82, SMD = 0.469). Controlling for depression, anxiety and stress, migraine diagnosis (25th percentile estimate = 0.67, p = 0.002, 75th percentile estimate = 0.92, p = 0.032) and migraine severity (estimates: 2.2-5.3, p < 0.001 for all) were associated with higher levels of occupational burnout., Conclusions: Migraine diagnosis and severity is associated with an occupational burnout, after controlling for various psychological and work-related factors. The ethnopharmacological relevance of the Millingtonia hortensis ( M. hortensis ) flower's aqueous extract lies in its traditional use as a herbal remedy in Southeast Asia. With a rich history in folk medicine, this aqueous has been esteemed for its purported sedative and anxiolytic properties. Our research delves into the scientific basis of these traditional claims, exploring the potential mechanisms underlying the observed effects of M. hortensis flower's aqueous extract on sleep promotion and mood regulation. This study aimed to explore the sleep-promoting effects of M. hortensis dried flower in mice, using an aqueous concentration equivalent to a human concentration of 2.7 mg/mL. Anxiolytic and antidepressant properties were evaluated using behavioural tests, while electroencephalography (EEG) analysis probed the neural mechanisms underlying sleep promotion post-consumption. The aqueous extract of M. hortensis dried flower administered to mice showed a decrease in immobility in the forced swimming test, demonstrating antidepressant-like effects. Moreover, mice treated with M. hortensis aqueous exhibited increased non-rapid eye movement (NREM) sleep duration, corroborating sleep-promoting potential. EEG analysis of mice treated with M. hortensis aqueous revealed heightened beta oscillations in the frontal and parietal cortex, while pre-treatment with M. hortensis aqueous or diazepam enhanced rapid eye movement (REM) sleep after thiopental administration. Interestingly, M. hortensis aqueous pre-treatment augmented delta frequency ranges in the frontal cortex.
