10. Meta -- analysis of placebo -- controlled studies of the efficacy of Epogam in the treatment of atopic eczema. Relationship between plasma essential fatty acid changes and clinical response.

Abstract

Gamma-linolenic acid in the form of a particular variety of evening primrose oil (Epogam) has been reported of value in the treatment of atopic eczema. Nine controlled trials of evening primrose oil were performed in eight centres. Four of the trials were parallel and five cross-over. Doctors and patients assessed the severity of eczema by scoring measures of inflammation, dryness, scaliness, pruritus and overall skin involvement. Individual symptom scores were combined to give a single global score at each assessment point. In the analysis of the parallel studies, both patient and doctor scores showed a highly significant improvement over baseline (P < 0·0001) due to Epogam: for both scores the effect of Epogam was significantly better than placebo. Similar results were obtained on analysis of the cross-over trials, but in this case the difference between Epogam and placebo in the doctors' global score, although in favour of Epogam, failed to reach significance. The effects on itch were particularly striking. There was no placebo response to this symptom, whereas there was a substantial and highly significant response to Epogam (P < 0·000l). When the improvements, or otherwise, in clinical condition were related to changes in plasma levels of dihomogammalinolenic and arachidonic acids, it was found that there was a positive correlation between an improvement in clinical score and a rise in the fatty acid levels. 11. Accumulation of Linoleic and γ-Linolenic Acids in Tissue Lipids of Pyridoxine-Deficient Rats

Abstract

Young male Sprague-Dawley rats were fed diets containing added pyridoxine · HCl at 22 mg/kg (control), 0 mg/kg or 88 mg/kg for 6 weeks. In comparison with control or pyridoxine-supplemented (+PN) rats, growth of the pyridoxine-deficient (-PN) rats was significantly less after 2 weeks. After 6 weeks, liver weight was higher but thymus and epididymal fat weights, in relation to body weight, were significantly lower in -PN compared to control rats. In -PN rats, phospholipid levels of linoleic and γ-linolenic acids were increased, but arachidonic acid was decreased compared to controls in plasma, liver, thymus and skin. In liver triglycerides from -PN rats, all essential fatty acids (n3 and n6) were increased compared to both control and +PN rats. The n3 essential fatty acids were significantly increased in plasma, liver, and thymus phospholipids in the +PN compared to control rats. These results support previous reports of an effect of pyridoxine on essential fatty acid metabolism and suggest that both linoleic desaturation and γ-linolenic acid elongation may be impaired in -PN rats. In addition, the accumulation of essential fatty acids in the liver triglycerides of -PN rats suggests that essential fatty acid turnover between triglyceride and phospholipid may be influenced by pyridoxine.

12. EFFECTS OF PROLACTIN ON THE RESPONSES OF RAT AORTIC AND ARTERIOLAR SMOOTH-MUSCLE PREPARATIONS TO NORADRENALINE AND ANGIOTENSIN

Abstract

The effects of physiological and pathophysiological concentrations of prolactin on the responses of vascular smooth muscle to noradrenaline and to angiotensin were studied. 50 ng. per ml. prolactin caused a sustained potentiation of the responses of rat aortic strips to noradrenaline; after an initial potentiation concentrations of 500 and 5000 ng. per ml. depressed the responses. Similar results were obtained with a rat mesenteric vascular-bed preparation. A prolactin concentration of 100 ng. per ml. seemed to be the point at which the response pattern changed. Prolactin had similar effects on the responses of the mesenteric vascular bed to angiotensin. Since stimuli such as surgery, exercise, emotional stress, pregnancy, and treatment with reserpine and methyldopa may increase human plasma-prolactin levels to 30-500 ng. per ml., these results may have wide clinical importance in both sexes.
