STUDY QUESTION How do age, ethnicity, and other characteristics affect serum anti-mullerian hormone (AMH) levels in Asian women undergoing fertility treatment? SUMMARY ANSWER Age, ethnicity, obesity (BMI ≥ 30 kg/m2), and polycystic ovarian syndrome (PCOS) significantly impacted serum AMH levels, with the rate of decrease accelerating as age increased; a concentration of 4.0 ng/ml was the optimal cut-off for diagnosis of PCOS. WHAT IS KNOWN ALREADY There are significant differences in ovarian reserve among women from different races and ethnicities, and Asian women often have poorer reproductive outcomes during assisted reproductive treatment cycles. STUDY DESIGN, SIZE, DURATION A population-based multi-nation, multi-centre, multi-ethnicity prospective cohort study of 4613 women was conducted from January 2020 to May 2021. Infertile women of 20–43 years of age were enrolled. The exclusion criteria included: age 43, non-Asian ethnicity, and missing critical data. PARTICIPANTS/MATERIALS, SETTING, METHODS Participants were Asian women of Chinese, Japanese, Korean, Thai, Vietnamese, Malay, Indian, and Indonesian ethnicities from 12 IVF centres across Asia. These women were all naïve to ovarian stimulation cycles and attended IVF centres for fertility assessment. The AMH measurement was performed using an AMH automated assay on a clinically validated platform. MAIN RESULTS AND THE ROLE OF CHANCE A total of 4556 infertile Asian women were included in the final analyses. The mean ± SD for serum AMH concentrations (ng/ml) across specific age groups were: overall, 3.44 ± 2.93; age LIMITATIONS, REASONS FOR CAUTION The incidence of PCOS was supposedly high in this cohort as some IVF clinics were tertiary referral centres for managing specific fertility issues encountered by women with PCOS. Treatment with GnRHa or OCP before AMH testing was regionally and ethnically confined, mostly in Hong Kong SAR and Japan. Moreover, this reference for serum AMH value is limited to Asian women of the ethnicities examined and may not apply to other ethnicities not included in the study. WIDER IMPLICATIONS OF THE FINDINGS This is the first study to collate and construct age-specific reference ranges for serum AMH levels using the same bioassay on Asian women of different ethnicities. The findings of this investigation can assist clinicians to counsel and prognosticate about Asian women’s ovarian reserve and reproductive potential, thus providing better strategies for personalized fertility interventions. Giới thiệu: Khoảng 15% nam giới vô sinh có chỉ số tinh dịch đồ bình thường, trong đó 8% những bệnh nhân này có bất thường về DNA tinh trùng. Hiện nay, phương pháp khảo sát cấu trúc nhiễm sắc chất tinh trùng là phương pháp được sử dụng rộng rãi để đánh giá sự phân mảnh DNA tinh trùng. Mục tiêu: Đánh giá tỷ lệ phân mảnh DNA tinh trùng trên bệnh nhân đến khám hiếm muộn tại hệ thống IVFMD bằng phương pháp SCSA. Đánh giá mối tương quan giữa các chỉ số của tinh dịch đồ cũng như một số yếu tố khác ảnh hưởng đến chỉ số phân mảnh DNA tinh trùng (DFI). Đối tượng và phương pháp nghiên cứu: Nghiên cứu cắt ngang mô tả, tiến cứu được thực hiện trên 99 bệnh nhân nam đến khám hiếm muộn và có chỉ định xét nghiệm tinh dịch đồ tại hệ thống IVFMD từ 07/2019 đến 09/2019. Thông tin bệnh nhân được thu nhận, DFI được xác định bằng SCSA. Kết quả: Bệnh nhân được chia thành 3 nhóm DFI thấp (≤ 15%), trung bình (15% < DFI ≤ 30%), và cao (> 30%) với tỷ lệ lần lượt là: 51,5%; 29,3% và 19,2% trên tổng số các bệnh nhân đưa vào nghiên cứu. Chỉ số khối cơ thể, hút thuốc lá, uống rượu, bia và các chỉ số tinh dịch đồ bao gồm mật độ, hình dạng, và tỷ lệ sống không ảnh hưởng đến sự phân mảnh DNA tinh trùng. Tuy nhiên, so với hai nhóm DFI trung bình và thấp, nhóm DFI cao có thời gian kiêng xuất tinh dài hơn (7,58 ± 9,06 ngày so với 3,96 ± 1,92 và 3,67 ± 1,69, p = 0,007) và độ tuổi lớn hơn (38,79 ± 6,36 tuổi so với 32,77 ± 5,41 và 34,42 ± 7,00, p = 0,002). Mặt khác, nhóm DFI thấp có tỷ lệ tinh trùng di động cao hơn hai nhóm còn lại (54,20 ± 13,61% so với 41,14 ± 15,82% và 43,21 ± 15,11%, p < 0,001). Kết luận: Bệnh nhân có chỉ số DFI cao có thời gian kiêng xuất tinh dài và lớn tuổi hơn hai nhóm DFI thấp và trung bình. Tinh trùng ở nhóm DFI thấp di động tốt hơn so với hai nhóm DFI trung bình và cao.

Mục tiêu: Đánh giá tương quan giữa số lượng noãn thu nhận và kết quả thụ tinh trong ống nghiệm. Đối tượng và phương pháp nghiên cứu: Nghiên cứu hồi cứu trên 2.209 chu kỳ IVF/ICSI từ 3 trung tâm IVF tại Việt Nam trong thời gian từ tháng 1/2015 đến tháng 5/2017. Hồi qui đa biến được sử dụng để đánh giá mối liên hệ giữa các biến khảo sát và kết cục trẻ sinh sống cộng dồn. Đường cong ROC mô tả độ nhạy và độ đặc hiệu của việc tiên lượng số lượng noãn thu nhận đối với kết cục trẻ sinh sống cộng dồn. Chi - square test được sử dụng để phân tích các tỉ lệ trẻ sinh sống cộng dồn, quá kích buồng trứng và phôi còn dư sau khi đạt được một trẻ sinh sống ở các nhóm theo số lượng noãn thu nhận. Kết quả: Số noãn trung bình thu nhận đạt 12,23 ± 5,74. Tuổi vợ trung bình là 32,38 ± 4,33. Tỉ lệ trẻ sinh sống cộng dồn đạt 33,3%. Số lượng noãn thu nhận và tuổi vợ có tương quan với kết cục trẻ sinh sống cộng dồn (lần lượt: OR = 1,05; 95% CI: 1,04 - 1,07 và OR = 0,94; 95% CI: 0,92 - 0,96). Số lượng noãn thu nhận có giá trị tiên lượng thấp đối với kết cục trẻ sinh sống cộng dồn (AUC = 0,62; 95% CI: 0,57 - 0,62). Tỉ lệ quá kích buồng trứng tăng có ý nghĩa thống kê khi số noãn thu nhận vượt quá 12 noãn (1 - 8: 0%; 9 - 12: 0,45%; 13 - 16: 0,67% và 16 - 38: 1,64%; p < 0,001). Đồng thời, tỉ lệ trẻ sinh sống cộng dồn và tình trạng phôi trữ đông còn dư tăng có ý nghĩa thống kê cho tới khi đạt 16 noãn rồi bão hòa (1 - 8: 22,9% và 11,0%; 9 - 12: 33,1% và 23,6%; 13 - 16: 40,1% và 33,2%; 16 - 38: 40,3% và 32,7%, p < 0,001). Kết luận: Số lượng noãn thu nhận có mối tương quan thuận với kết cục trẻ sinh sống cộng dồn. Việc thu nhận nhiều hơn 16 noãn làm tăng đáng kể tỉ lệ quá kích buồng trứng nhưng không cải thiện kết cục trẻ sinh sống cộng dồn. Do đó, 13 đến 16 noãn là số noãn tối ưu nhằm đạt hiệu quả và an toàn trong điều trị.

STUDY QUESTION Does the presence of single nucleotide polymorphisms (SNPs) in the FSH receptor gene (FSHR) and/or FSH beta subunit-encoding gene (FSHB) influence ovarian response in predicted normal responders treated with rFSH? SUMMARY ANSWER The presence of FSHR SNPs (rs6165, rs6166, rs1394205) has a statistically significant impact in ovarian response, although this effect is of minimal clinical relevance in predicted normal responders treated with a fixed dose of 150 IU rFSH. WHAT IS KNOWN ALREADY Ovarian reserve markers have been a breakthrough in response prediction following ovarian stimulation. However, a significant percentage of patients show a disproportionate lower ovarian response, as compared with their actual ovarian reserve. Studies on pharmacogenetics have demonstrated a relationship between FSHR or FSHB genotyping and drug response, suggesting a potential effect of individual genetic variability on ovarian stimulation. However, evidence from these studies is inconsistent, due to the inclusion of patients with variable ovarian reserve, use of different starting gonadotropin doses, and allowance for dose adjustments during treatment. This highlights the necessity of a well-controlled prospective study in a homogenous population treated with the same fixed protocol. STUDY DESIGN, SIZE, DURATION We conducted a multicenter multinational prospective study, including 368 patients from Vietnam, Belgium, and Spain (168 from Europe and 200 from Asia), from November 2016 until June 2019. All patients underwent ovarian stimulation followed by oocyte retrieval in an antagonist protocol with a fixed daily dose of 150 IU rFSH until triggering. Blood sampling and DNA extraction was performed prior to oocyte retrieval, followed by genotyping of four SNPs from FSHR (rs6165, rs6166, rs1394205) and FSHB (rs10835638). PARTICIPANTS/MATERIALS, SETTING, METHODS Eligible were predicted normal responder women MAIN RESULTS AND THE ROLE OF CHANCE The prevalence of derived allele homozygous SNPs in the FSHR was rs6166 (genotype G/G) 15.8%, rs6165 (genotype G/G) 34.8%, and rs1394205 (genotype A/A) 14.1%, with significant differences between Caucasian and Asian women (P T) was very rare (0.5%). Genetic model analysis revealed that the presence of the G allele in FSHR variant rs6166 resulted in less oocytes retrieved when compared to the AA genotype (13.54 ± 0.46 vs 14.81 ± 0.61, estimated mean difference (EMD) −1.47 (95% CI −2.82 to −0.11)). In FSHR variant rs1394205, a significantly lower number of oocytes was retrieved in patients with an A allele when compared to G/G (13.33 ± 0.41 vs 15.06 ± 0.68, EMD −1.69 (95% CI −3.06 to −0.31)). A significantly higher prevalence of hypo-responders was found in patients with the genotype A/G for FSHR variant rs6166 (55.9%, n = 57) when compared to A/A (28.4%, n = 29), ORadj 1.87 (95% CI 1.08–3.24). No significant differences were found regarding the FORT across the genotypes for FSHR variants rs6166, rs6165, or rs1394205. Regarding the FOI, the presence of the G allele for FSHR variant rs6166 resulted in a lower FOI when compared to the A/A genotype, EMD −13.47 (95% CI −22.69 to −4.24). Regarding FSHR variant rs6165, a lower FOI was reported for genotype A/G (79.75 ± 3.35) when compared to genotype A/A (92.08 ± 6.23), EMD −13.81 (95% CI −25.41 to −2.21). LIMITATIONS, REASONS FOR CAUTION The study was performed in relatively young women with normal ovarian reserve to eliminate biases related to age-related fertility decline; thus, caution is needed when extrapolating results to older populations. In addition, no analysis was performed for FSHB variant rs10835638 due to the very low prevalence of the genotype T/T (n = 2). WIDER IMPLICATIONS OF THE FINDINGS Based on our results, genotyping FSHR SNPs rs6165, rs6166, rs1394205, and FSHB SNP rs10835638 prior to initiating an ovarian stimulation with rFSH in predicted normal responders should not be recommended, taking into account the minimal clinical impact of such information in this population. Future research may focus on other populations and other genes related to folliculogenesis or steroidogenesis. STUDY QUESTION: Does administration of corifollitropin alfa followed by highly purified (hp) HMG result in higher ongoing pregnancy rates compared with daily recombinant FSH (rFSH) in young poor responders?SUMMARY ANSWER: Corifollitropin alfa followed by hp-HMG does not increase ongoing pregnancy rates compared with rFSH in young poor responders, although more supernumerary cryopreserved embryos were obtained with corifollitropin alfa and hp-HMG.WHAT IS KNOWN ALREADY: Poor ovarian response remains one of the main therapeutic challenges in women undergoing ovarian stimulation, given that very low live birth rates of 6% have been reported in this particular group of infertile patients. Nevertheless, concerns have been raised that a degree of heterogeneity remains, as the prognostic effect of individual factors is still unclear, particularly for the young poor responder group. The rationale for conducting the current randomized trial was based on the results of a previous pilot study demonstrating promising results with the administration of hp-HMG following corifollitropin alpha in women younger than 40 years of age, fulfilling the 'Bologna' criteria.STUDY DESIGN, SIZE, DURATION: A multicenter, phase III, superiority, randomized trial was conducted using a parallel two-arm design. The study included 152 patients younger than 40 years old and fulfilling the 'Bologna' criteria for poor ovarian response, from one tertiary referral centre in Europe and one tertiary referral centre in Asia. Enrolment was performed from March 2013 to May 2016.PARTICIPANTS/MATERIALS, SETTING, METHODS: Eligible patients were randomized to either administration of 150 μg corifollitropin alfa followed by 300 IU hp-HMG (Group A) or to 300 IU of daily recombinant FSH (Group B) in a fixed GnRH antagonist protocol. The randomization sequence was created using a computer generated randomization list stratified by centre, using 1:1 allocation. The primary outcome was ongoing pregnancy rate (defined as the presence of an intrauterine gestational sac with an embryonic pole demonstrating cardiac activity at 9-10 weeks of gestation). Secondary outcomes included embryo cryopreservation rates, clinical and biochemical pregnancy rates and number of oocytes retrieved.MAIN RESULTS AND THE ROLE OF CHANCE: Overall, 152 poor ovarian responders defined by the 'Bologna' criteria were included in the study. Using an intention-to treat analysis, the ongoing pregnancy rates did not differ significantly between Group A 11/77 (14.3%) and Group B 11/70 (15.7%), absolute difference: -0.4 (-11.5 to 10.8), OR = 0.9 (0.4-2.4). Biochemical and clinical pregnancy rates, live birth rates and the number of oocytes retrieved were also comparable between the two groups. Nevertheless, more patients in the corifollitropin alfa group had cryopreserved embryos compared to the rFSH group [22 (28.6%) versus 10 (14.3%), OR = 2.4 (1.01-5.5)]. Incidentally, Asian patients had significantly lower cancellation rates compared to European poor responders [2/64 (3.1%) versus 17/83 (20.4%), OR = 0.12 (0.03-0.5)]. This discrepancy could be explained by the fact that Asian women were better prognosis patients than European patients, with significantly lower FSH [9.8 (5.3) versus 11.5 (5.4), P = 0.017] and significantly higher AMH [1.1 (0.9) versus 0.4 (0.3), P-value LIMITATIONS, REASONS FOR CAUTION: Ongoing pregnancy rates close to 14% for both treatment groups differ significantly from the hypothesized primary outcome rates used in the power calculation. Therefore, our randomized trial might have been underpowered to detect smaller differences. The use of multiple secondary outcomes and multiple comparisons could have increased a Type 1 error. Finally, although the chance of selection biases remains low given the nature of the infertile population, the open-label design could have been a limitation.WIDER IMPLICATIONS OF THE FINDINGS: Poor ovarian response represents a challenge and although a specific protocol may have increased the number of cryopreserved embryos, no difference was observed in ongoing pregnancy rates. Our study, being one of the largest RCTs in 'Bologna' criteria poor responders, highlights that baseline characteristics may play a crucial role in clinical prognosis of this population. Given that ovarian stimulation using novel protocols does not seem to significantly increase pregnancy rates even in young women, we suggest that future clinical research should focus on increasing the number of recruitable follicles and on oocyte quality rather than evaluating different stimulation protocols.STUDY FUNDING/COMPETING INTERESTS: No external funding was used for this study. Đặt vấn đề: Thiếu máu tán huyết di truyền (thalassemia) là một bệnh lý huyết học đơn gen. Bệnh nhân cần được điều trị suốt đời và có thể tử vong nếu không tuân thủ điều trị Tại Việt Nam, ước tính hiện nay có khoảng 20.000 người bị thalassemia thể nặng, mỗi năm có thêm khoảng 10 triệu người mang gen bệnh và hàng năm, có thêm trên 2.000 trẻ bị bệnh thalassemia chào đời. Kỹ thuật chẩn đoán đột biến di truyền đơn gen giai đoạn phôi tiền làm tổ (Preimplantation Genetic Testing for Monogenic disorders - PGT-M) ra đời, cho phép các cặp vợ chồng sau khi có phôi từ thụ tinh trong ống nghiệm (TTTON), lựa chọn những phôi không có đột biến gen gây bệnh để chuyển vào buồng tử cung người vợ giúp có thể sinh con không mang bệnh. Mục tiêu: Đánh giá hiệu quả của chuyển phôi sau PGT-M trên bệnh nhân vô sinh mang gen bệnh thalassemia. Phương pháp: Đây là một báo cáo loạt ca, thực hiện trong thời gian từ tháng 02/2017 đến tháng 05/2018 tại Đơn vị Hỗ trợ sinh sản Mỹ Đức (IVFMD), bệnh viện Mỹ Đức. Đối tượng bệnh nhân là những cặp vợ chồng có chỉ định làm TTTON và quá trình sàng lọc cho thấy cả hai có mang gen bệnh. Phôi ngày 5, hình thành sau TTTON, được tiến hành sinh thiết để lấy khoảng 5 tế bào lá nuôi. Các tế bào được gửi phân tích di truyền bằng công nghệ giải trình tự thế hệ mới (Next Generation Sequencing – NGS) qua hai giai đoạn, (1) xác định tình trạng lệch bội và (2) chẩn đoán bất thường đơn gen trên các phôi không lệch bội. Phôi được đông lạnh sau khi sinh thiết và những phôi nào không mang gen bệnh hay ở dạng dị hợp tử mới được sử dụng để chuyển vào buồng tử cung của người vợ. Yếu tố đánh giá kết quả chính là tỷ lệ sinh sống sau một chu kỳ chuyển phôi. Kết quả: Trong thời gian nghiên cứu, có 17 cặp bệnh nhân thỏa tiêu chuẩn nhận loại. Số noãn chọc hút trung bình là 16,7 ± 8,5, với số phôi ngày 5 trung bình là 6,8 ± 4,6. Trong số 79 phôi được sinh thiết, có 50 (63,3%) phôi không bị lệch bội. Trong 50 phôi này, tỷ lệ phôi không mang gen đột biến hay ở dạng dị hợp tử lần lượt là 36% và 56%. Hiện có 7 trường hợp sinh sống được ghi nhận và 1 trường hợp có thai đang ở tuổi thai 33 4/7 tuần. Kết luận: PGT-M có thể được áp dụng thường quy trên các cặp vợ chồng có mang gen gây bệnh thalassemia nhằm giúp có thể sinh con từ những phôi không mang gen bệnh, hạn chế các gánh nặng về kinh tế, tâm lý cho gia đình và xã hội.

Mục tiêu: So sánh các kết quả lâm sàng của việc chuyển phôi ngày 3 hoặc ở giai đoạn phân chia hoặc có dấu hiệu nén sớm khi được nuôi cấy trong môi trường nuôi cấy đơn bước (SSM). Phương pháp: Đoàn hệ tiến cứu được thực hiện tại IVFMD, Bệnh viện Mỹ Đức. Tiêu chuẩn nhận: phụ nữ trải qua chu kỳ điều trị IVF/ICSI thoả (i) ≤2 chu kỳ điều trị, (ii) kích thích buồng trứng bằng GnRH antagonist (iii) phôi được nuôi cấy trong môi trường đơn bước (Life Global®) và (iv) đồng ý chuyển 2 phôi hoặc ở giai đoạn nén sớm hoặc giai đoạn phân cắt, có 7-9 tế bào, phân mảnh ≤20%. Kết cục chính là tỷ lệ trẻ sinh sống. Kết cục phụ là các tỷ lệ: thai lâm sàng, làm tổ, thai diễn tiến, thai ngoài tử cung và sẩy thai. Kết quả: Từ tháng 12/2016 đến tháng 4/2018, có 430 bệnh nhân được nhận, tương ứng 215 bệnh nhân mỗi nhóm. Độ tuổi trung bình ở nhóm được chuyển phôi nén sớm nhỏ hơn đáng kể so với nhóm được chuyển phôi phân chia (32,7 ± 4,5 so với 33,7 ± 4,7 tuổi; P = 0,03). Hiệu chỉnh (OR) tỷ lệ trẻ sinh sống khi chuyển phôi nén sớm so với phôi phân chia vào ngày 3 sau chu kỳ chuyển phôi đầu tiên là 1,02 (95% [CI] 0,69-1,51) và 0,9 (95% CI 0,57-1,41). Tỷ lệ trẻ sinh sống lần lượt là 32,6% và 33,0%. Các kết cục phụ khác không có khác biệt thống kê giữa hai nhóm. Kết luận: Chuyển hai phôi ngày 3 hoặc giai đoạn phân chia hoặc có dấu hiệu nén sớm cho kết cục thai kì giống nhau khi được nuôi cấy trong SSM.

Mục tiêu: So sánh hiệu quả nuôi cấy phôi giữa 2 hệ thống tủ cấy benchtop có và không có sử dụng khí trộn sẵn. Đối tượng và phương pháp nghiên cứu: Nghiên cứu chia noãn được thực hiện từ 02/2017 - 04/2017 tại IVFMD, bệnh viện Mỹ Đức. Tiêu chuẩn nhận bệnh: bệnh nhân ≤ 38 tuổi; số chu kỳ điều trị trước đó ≤ 2; nuôi cấy phôi ngày 5. Tiêu chuẩn loại: các chu kỳ xin cho noãn; IVM; tinh trùng phẫu thuật; bất thường tử cung, vùng chậu. Tại thời điểm chọc hút noãn, tiến hành chia cụm COCs thành 2 nhóm (1) nuôi cấy phôi bằng tủ cấy dùng khí trộn sẵn (tủ cấy BT37, Origio, Đan Mạch) và (2) nuôi cấy phôi không sử dụng hệ thống khí trộn sẵn (tủ cấy G185, K-system, Đan Mạch). Nếu số cụm COCs là số lẻ thì cụm COC dư ra sẽ được chia vào nhóm 2. Kết cục chính là tỷ lệ phôi nang tốt. Kết cục phụ là tỷ lệ thụ tinh và tỷ lệ tạo phôi nang. Kết quả: Có 60 bệnh nhân tham gia vào nghiên cứu, trong đó có 735 cụm COCs nuôi cấy ở nhóm 1 và 765 cụm COCs nuôi cấy ở nhóm 2. Tỷ lệ phôi nang tốt không có sự khác biệt thống kê giữa 2 nhóm (39.5% so với 38.2%; p = 0.89). Không có sự khác biệt về tỷ lệ thụ tinh (90.0% so với 92.0%, p = 0.20) cũng như tỷ lệ tạo phôi nang (55.1% so với 56.6%, p = 0.87) giữa 2 nhóm. Kết luận: Hệ thống tủ cấy có sử dụng khí trộn có hiệu quả tương đương hệ thống tủ cấy không sử dụng khí trộn về kết cục nuôi cấy phôi nang. Việc chọn lựa sử dụng hệ thống tủ cấy nào phụ thuộc vào số chu kỳ và quy trình quản lí sử dụng tại từng labo thụ tinh trong ống nghiệm.

Study question Does luteinizing hormone (LH) supplementation improve live birth rate after in vitro fertilization (IVF) in patients aged ≥35 years receiving a gonadotrophin-releasing hormone (GnRH) antagonist protocol? Summary answer There was no difference in live birth rate with use of LH during IVF in patients aged ≥35 years undergoing IVF treatment using a GnRH antagonist protocol. What is known already Use of GnRH analogues as part of a controlled ovarian hyperstimulation protocol during IVF treatment cycles decreases the amount of LH available to developing follicles. The role of LH supplementation for improving outcomes in patients undergoing controlled ovarian hyperstimulation as part of assisted reproduction treatments, particularly those involving a GnRH antagonist protocol, is unclear. It has been suggested that higher risk patients (e.g. age ≥35 years, poor ovarian reserve) may benefit from LH supplementation. Study design, size, duration This single-centre, randomized controlled trial was conducted from 1 October 2012 to 30 June 2014. A total of 240 women aged ≥35 years undergoing IVF received ovarian stimulation using a GnRH antagonist protocol, with recombinant follicle-stimulating hormone (r-FSH; Gonal-F(®)) starting from cycle day 2 or 3. GnRH antagonist (Cetrotide(®)) was administered on Day 5 of r-FSH administration. On Day 6, patients in the LH supplementation group were switched to r-FSH/r-LH (Pergoveris(®)) 150/75 IU/day. Randomization to study treatments was performed in blocks of 4 via a computer-generated random number list. Participants/materials, setting, methods Of the 240 patients randomized to treatment, 120 received r-FSH/r-LH and 120 received r-FSH. Patients were recruited from the IVFAS, An Sinh Hospital, Ho Chi Minh City, Vietnam. Main results and the role of chance Live birth rate did not differ significantly (P > 0.05) between r-FSH/r-LH and r-FSH recipients (16.7 versus 17.5%; between-group difference 0.8, 95% confidence interval [CI] -9.5, 11.2). In addition, there were no significant differences between the r-FSH/r-LH and r-FSH groups with respect to the number of oocytes retrieved, implantation rate, miscarriage rate and clinical pregnancy rate. Limitations, reasons for caution The open-label design could have introduced bias, and the relatively small sample size may have allowed detection of only the most common adverse events. In addition, the study was likely to be underpowered based on differences between the response rate assumptions used in the sample size calculation and the actual response rate during the study. Wider implications of the findings The results of this study found no additional benefit from adding LH supplementation to ovarian stimulation with a GnRH antagonist protocol in women aged ≥35 years, and add to the body of evidence in this area. However, findings across studies are still inconsistent and additional research is needed before any clear recommendations for clinical practice can be made. Study question Is corifollitropin alfa 150 μg equivalent to follitropin beta 300 IU/day for controlled ovarian hyperstimulation (COS) in older women weighing ≥50 kg undergoing IVF and/or ICSI in Vietnam? Summary answer Corifollitropin alfa 150 μg was equivalent to follitropin beta 300 IU/day with respect to the number of oocytes retrieved, the ongoing, cumulative and live birth rates and obstetric outcomes. What is known already Corifollitropin alfa is a recombinant FSH (rFSH) preparation with slow absorption and a long half-life allowing administration of a single dose for COS lasting 7 days. Several randomized, controlled clinical trials have reported that COS with corifollitropin alfa is associated with similar outcomes compared with COS using daily rFSH. However, limited data are available in Asian patients. Study design size duration This randomized controlled trial was conducted at a single large IVF centre in Vietnam from June 2015 to August 2016. A total of 400 patients were included, 200 in each treatment group. The primary outcome measure was the number of oocytes retrieved. Patients were followed for 1 year after randomization. Participants /materials setting methods Participants aged 35-42 years with a body weight ≥50 kg who were undergoing an IVF cycle were randomized to undergo COS with a single dose of corifollitropin alfa 150 μg on Day 2 or 3 of the menstrual cycle, or follitropin beta 300 IU/day for 7 days starting on Day 2 or 3 of the menstrual cycle. All underwent ICSI according to standard institutional protocols. A beta hCG test was performed 17 days after ovum pick-up, and positive tests were confirmed on vaginal and/or abdominal ultrasound at 5-6 weeks after embryo transfer (clinical pregnancy) and at ≥10 weeks (ongoing pregnancy). Rates of ovarian hyperstimulation syndrome, and maternal and foetal outcomes after one cycle of ICSI were monitored over 12 months. Main results and the role of chance Patients in the corifollitropin alfa and follitropin beta groups were well matched at baseline (mean age 37.5 ± 1.9 vs 37.7 ± 2.0 years, mean body weight 53.7 ± 5.4 vs 52.5 ± 4.8 kg). There was no significant difference between the corifollitropin alfa and follitropin beta groups in the number of oocytes retrieved (11.4 ± 5.9 vs 10.8 ± 5.8; P = 0.338). The ongoing pregnancy rate (31.5 vs 32.0%; P = 0.99) and live birth rate (30.5 vs 32.0%; P = 0.83) (both per initiated cycle at 12 months after randomization) were also similar in the two treatment groups. Complication rates were low and similar in the corifollitropin alfa and follitropin beta groups, and there were no significant between-group differences in obstetric outcomes. Limitations reasons for caution This study had an open-label design, and therefore, the potential for bias cannot be excluded. The findings are only applicable to patient populations with similar characteristics to those enroled in the study. Wider implications of the findings This study adds to the body of evidence supporting the equivalence of corifollitropin alfa and follitropin beta for COS in a variety of patients undergoing IVF and/or ICSI. The ability to provide COS with corifollitropin alfa has the potential to reduce the burden of treatment for patients. PURPOSE: A recent dose-finding study showed no significant differences in number of mature oocytes, embryos and top-quality embryos when triptorelin doses of 0.2, 0.3 or 0.4 mg were used to trigger final oocyte maturation in oocyte donors co-treated with a gonadotropin-releasing hormone (GnRH) antagonist. This analysis investigated whether triptorelin dosing for triggering final oocyte maturation in oocyte donors induced differences in follicular fluid (FF) hormone levels and granulosa cell gene expression.METHODS: This single-centre, randomised, parallel, investigator-blinded trial was conducted in oocyte donors undergoing a single stimulation cycle at IVFMD, My Duc Hospital, Ho Chi Minh City, Vietnam, from August 2014 to March 2015. A total of 165 women aged 18-35 years with body mass index 1.25 ng/mL, and antral follicle count ≥6 were randomised to three different triptorelin doses for trigger. The main outcome was concentration of steroid hormones in FF collected from the first punctured follicle on each side. Moreover, luteinising hormone receptor (LHR), 3β-hydroxy-steroid-dehydrogenase (3ßHSD) and inhibin-Ba (INHB-A) gene expression in cumulus and mural granulosa cells were investigated in a subset of women from each group.RESULTS: Progesterone and oestradiol levels in FF did not differ significantly by trigger doses; findings were similar for 3βHSD, LHR and INHB-A gene expression in both cumulus and mural granulosa cells.CONCLUSIONS: In women co-treated with a GnRH antagonist, no significant differences in FF steroid levels and granulosa cell gene expression were seen when different triptorelin doses were used to trigger final oocyte maturation.

OBJECTIVE: To determine the optimal GnRH agonist dose for triggering of oocyte maturation in oocyte donors.DESIGN: Single-center, randomized, parallel, investigator-blinded trial.SETTING: IVFMD, My Duc Hospital, Ho Chi Minh City, Vietnam.PATIENT(S): One hundred sixty-five oocyte donors (aged 18-35 years, body mass index [BMI] 1.25 ng/mL, and antral follicle count ≥6).INTERVENTION(S): Ovulation trigger with 0.2, 0.3, or 0.4 mg triptorelin in a GnRH antagonist cycle.MAIN OUTCOME MEASURE(S): The primary end point was number of metaphase II oocytes. Secondary end points were fertilization and cleavage rates, number of embryos and top-quality embryos, steroid levels, ovarian volume, and ongoing pregnancy rate (PR) in recipients.RESULT(S): There were no significant differences between the triptorelin 0.2, 0.3, and 0.4 mg trigger groups with respect to number of metaphase II oocytes (16.0 ± 8.5, 15.9 ± 7.8, and 14.7 ± 8.4, respectively), embryos (13.2 ± 7.8, 11.7 ± 6.9, 11.8 ± 7.0), and number of top-quality embryos (3.8 ± 2.9, 3.6 ± 3.0, 4.1 ± 3.0). Luteinizing hormone levels at 24 hours and 36 hours after trigger was significantly higher with triptorelin 0.4 mg versus 0.2 mg and 0.3 mg (9.8 ± 7.1 IU/L vs. 7.3 ± 4.1 IU/L and 7.2 ± 3.7 IU/L, respectively; 4.6 ± 3.2 IU/L vs. 3.2 ± 2.3 IU/L and 3.3 ± 2.1 IU/L, respectively. Progesterone level at oocyte pick-up +6 days was significantly higher in the 0.4-mg group (2.2 ± 3.7 ng/ml) versus 0.2 mg (1.1 ± 1.0 ng/ml) and 0.3 mg (1.2 ± 1.6 ng/ml). One patient developed early-onset severe ovarian hyperstimulation syndrome (OHSS).CONCLUSION(S): No significant differences between triptorelin doses of 0.2, 0.3, and 0.4 mg used for ovulation trigger in oocyte donors were seen with regard to the number of mature oocytes and top-quality embryos.CLINICAL TRIAL REGISTRATION NUMBER: NCT02208986.
