3. Consistency issues in Gaussian Mixture Models reduction algorithms

Abstract

In many contexts Gaussian Mixtures (GM) are used to approximate probability distributions, possibly time-varying. In some applications the number of GM components exponentially increases over time, and reduction procedures are required to keep them reasonably limited. The GM reduction (GMR) problem can be formulated by choosing different measures of the dissimilarity of GMs before and after reduction, like the Kullback-Leibler Divergence (KLD) and the Integral Squared Error (ISE). Since in no case the solution is obtained in closed form, many approximate GMR algorithms have been proposed in the past three decades, although none of them provides optimality guarantees. In this work we discuss the importance of the choice of the dissimilarity measure and the issue of consistency of all steps of a reduction algorithm with the chosen measure. Indeed, most of the existing GMR algorithms are composed by several steps which are not consistent with a unique measure, and for this reason may produce reduced GMs far from optimality. In particular, the use of the KLD, of the ISE and normalized ISE is discussed and compared in this perspective.

20. A short-term food intake model involving glucose, insulin and ghrelin

Abstract

Body weight control is gaining interest since its dysregulation eventually leads to obesity and metabolic disorders. An accurate mathematical description of the behavior of physiological variables in humans after food intake may help in understanding regulation mechanisms and in finding treatments. This work proposes a multi-compartment mathematical model of food intake that accounts for glucose-insulin homeostasis and ghrelin dynamics. The model involves both food volumes and glucose amounts in the two-compartment system describing the gastro-intestinal tract. Food volumes control ghrelin dynamics, whilst glucose amounts clearly impact on the glucose-insulin system. The qualitative behavior analysis shows that the model solutions are mathematically coherent, since they stay positive and provide a unique asymptotically stable equilibrium point. Ghrelin and insulin experimental data have been exploited to fit the model on a daily horizon. The goodness of fit and the physiologically meaningful time courses of all state variables validate the efficacy of the model to capture the main features of the glucose-insulin-ghrelin interplay.

26. Ointments containing Ceratothoa oestroides extract: Evaluation of their healing potential in the treatment of diabetic foot ulcers

Abstract

Diabetic foot ulceration is a common and severe complication of diabetes, causing substantial social, medical, and economic burdens. Treatment of foot ulcers remains challenging, thus requiring increasing awareness and more efficient management. This study investigates the efficacy of ointments, containing as main active ingredient the olive oil extract of the marine isopod Ceratothoa oestroides, in the treatment of patients with diabetic foot ulcers. Fifty-two patients were allocated into four treatment groups either receiving therapy with an ointment containing extract of C. oestroides or extract of C. oestroides and eosin or extract of C. oestroides and cefaclor or no treatment. Patients were monitored for a period of 135 days by evaluation of transepidermal water loss, skin hydration, planimetry, photo-documentation, and clinical condition. Treatment with the extract of C. oestroides demonstrated significant healing properties that became evident after 45 days of treatment and resulted in complete ulcer healing in 61% of the patients. A significant improvement in transepidermal water loss (p < 0.001), skin hydration levels (p < 0.001), and wound area (p < 0.001) was observed in all patients. Similar efficacy was demonstrated for the combination of C. oestroides extract with eosin treatment (p < 0.001). On the contrary, the combination of C. oestroides extract with cefaclor antibiotic agent completely inhibited the healing properties of the isopod extract and did not improve water loss, skin hydration, or wound area. An important factor for C. oestroides extract healing properties is its selective activity against Gram negative bacteria. Ointments containing C. oestroides extract alone or combined with the antimicrobial agent eosin emerges as an effective regimen for the treatment of diabetic foot ulcers. © 2019 by the Wound Healing Society

27. Dryness of Foot Skin Assessed by the Visual Indicator Test and Risk of Diabetic Foot Ulceration: A Prospective Observational Study

Abstract

Research Question: Previous cross-sectional studies have shown an association between sudomotor dysfunction and diabetic foot ulceration (DFU). The aim of this prospective multicenter study was to determine the role of dryness of foot skin and of established neurological modalities in the prediction of risk for foot ulceration in a cohort of individuals with diabetes mellitus (DM). Design: The study was conducted from 2012 to 2017. A total of 308 subjects with DM without history of DFU or critical limb ischemia completed the study. Diabetic neuropathy was assessed using the neuropathy symptom score (NSS) and neuropathy disability score (NDS). In a subset of participants, vibration perception threshold (VPT) was evaluated. Dryness of foot skin was assessed by the visual indicator plaster method (IPM). The diagnostic performance of the above neurological modalities for prediction of DFU was tested by receiver operating characteristic curve (ROC) analysis. Results: During the 6-year follow-up, 55 patients (annual ulceration incidence 2.97%) developed DFU. Multivariate Cox-regression analysis after controlling for the effect of age, gender, and DM duration demonstrated that the risk (hazard ratio, 95% confidence intervals) of DFU increased significantly with either abnormal IPM (3.319, 1.460–7.545, p = 0.004) or high (≥6) NDS (2.782, 1.546–5.007, p = 0.001) or high (≥25 volts) VPT (2.587, 1.277–5.242, p = 0.008). ROC analysis showed that all neurological modalities could discriminate participants who developed DFU (p < 0.001). IPM testing showed high sensitivity (0.86) and low specificity (0.49), while high vs. low NDS and VPT showed low sensitivity (0.40 and 0.39, respectively) and high specificity (0.87 and 0.89, respectively) for identification of patients at risk for DFU. Conclusion: Dryness of foot skin assessed by the IPM predicts the development of DFU. IPM testing has high sensitivity, whereas high NDS and VPT have high specificity in identifying subjects at risk for DFU. Abstract

This note investigates the behavior of a population of interconnected irreversible switches, whose switching mechanism is triggered by a chaotic map. Motivation comes from the fact that many emergent properties of biological systems are ruled by the activation of a population of biochemical switches, whose timing and fluctuations may lead to different cell fates. They have been usually investigated according to a stochastic approach, and in this work we show how some properties could be similarly explained in terms of the emergent properties of a chaotic system. With respect to noise, chaos comes out from a deterministic framework, thereby allowing the implementation of experimental procedures directed to investigate the behavior of the system according to different scenarios by means of a rigorous deterministic passage from mathematics to simulation, that cannot be honestly rendered for the stochastic framework, since pseudo-random sequences are usually invoked.

Abstract

This note presents a new approach for the construction of a state observer for nonlinear systems when the output measurements are available for computations after a non negligible time delay. The proposed observer consists of a chain of observation algorithms reconstructing the system state at different delayed time instants (chain observer). Conditions are given ensuring global exponential convergence to zero of the observation error for any given the delay in the measurements. The implementation of the observer is simple and computer simulations demonstrate its effectiveness. Index Terms--Delay systems, nonlinear systems, state observation, state prediction.

38. Improvement of metabolic control after 3-month use of real-time continuous glucose monitoring in patients with type 1 diabetes: a multicenter study in Greece

Abstract

Purpose: To assess the efficacy of a real-time continuous glucose monitoring (RT–CGM) system added to insulin pump therapy for 3 months, in sub-optimally controlled adults with type 1 diabetes mellitus (T1D). Methods: This was a prospective, multicenter, non-randomized, post-market release study. A total of 43 adult patients with T1D on insulin pump therapy and inadequate glycemic control (HbA1c > 7.0%) participated in the study. The primary endpoint was the change from baseline HbA1c levels. Secondary objectives were to evaluate the impact of the RT–CGM system on glucose variability, daily insulin requirements, and the frequency of hypoglycemic and ketoacidosis events. Results: At 3 months, the baseline HbA1c values decreased from 8.0 (7.6, 8.7) to 7.1 (6.7, 8.0) % (p < 0.001). Nineteen participants (44.2%) had a posttreatment HbA1c level ≤ 7%. Average total daily insulin requirements, as well as the average number of insulin boluses per day, increased significantly after the use of the RT–CGM system. The number of hypoglycemic events recorded did not differ between the first week and last week of RT–CGM usage, while no severe hypoglycemic episodes, ketoacidosis events, or hospitalizations related to diabetes occurred during the 3-month follow-up period. Conclusion: Addition of a RT–CGM system to insulin pump therapy for 3 months in inadequately controlled patients with T1D resulted in improved HbA1c levels, without increasing the risk of hypoglycemic events. © 2019, Hellenic Endocrine Society.

41. Noise propagation in chemical reaction networks: Analysis of a molecular subtractor module

Abstract

The realization of embedded molecular control systems is a challenging aim in Synthetic Biology, where a major goal is to design synthetic biological circuits performing specific tasks. In this field, the novel emergent approach is to assemble the circuit in a modular fashion, possibly restraining reciprocal interactions from interconnected modules (zero-retroactivity). Within this framework, recent results have been proposed, dealing with the realization of an embedded subtractor module, with the idea of exploiting it in a more general chemical reaction network that resembles a classical control scheme. So far, this research has been carried out according to the deterministic approach. More sophisticated analysis requires the use of stochastic models, which play a paramount role in investigating noise propagation in chemical reaction networks, especially when the species copy number is low and the intrinsic stochasticity of the phenomena under investigation cannot be neglected. This note deals with a first analysis of the subtractor module, according to the stochastic approach. To this end, Chemical Master Equations are exploited to model one of the possible molecular circuits implementing the subtractor, and moment equations are written in order to evaluate how noise propagates with respect to different values of the inputs and different model parameter settings.

Abstract

International audience; Harmful cyanobacterial blooms are a major threat to freshwater ecosystems globally weighing down the Good Environmental Status of waters and risk for human health. Biosensors are a highly promising technology that can produce near real-time measurements that could be used by early warning systems safeguarding human and freshwater ecosystem health. We developed a rapid, sensitive, simple and economical test to identify and daily monitor the blooms of potentially toxic cyanobacteria in aquatic environments based on their genetic identity. This innovative solution comes in a colorimetric biosensor format to quickly and efficiently monitor and sustainably manage the contamination risks of potentially harmful cyanobacteria. The biosensor is a genus-specific ribosomal RNA/DNA probe-based assay of potentially toxic genus and is calibrated to equivalent cell numbers. Here we present the field validation tests results performed in a recreative bathing area in south Paris concerning the detection of Microcystis at different sampling points during summer 2017 campaign. The validation was performed by taxonomic cyanobacteria cell counting present in the samples.

Abstract

BACKGROUND: Empagliflozin slowed the progression of CKD in patients with type 2 diabetes and cardiovascular disease in the EMPA-REG OUTCOME Trial. In a prespecified statistical approach, we assessed treatment differences in kidney function by analyzing slopes of eGFR changes.METHODS: Participants (n=7020) were randomized (1:1:1) to empagliflozin 10 mg/d, empagliflozin 25 mg/d, or placebo added to standard of care. We calculated eGFR slopes using random-intercept/random-coefficient models for prespecified study periods: treatment initiation (baseline to week 4), chronic maintenance treatment (week 4 to last value on treatment), and post-treatment (last value on treatment to follow-up).RESULTS: Compared with placebo, empagliflozin was associated with uniform shifts in individual eGFR slopes across all periods. On treatment initiation, adjusted mean slope (eGFR change per week, ml/min per 1.73 m2) decreased with empagliflozin (-0.77; 95% confidence interval, -0.83 to -0.71; placebo: 0.01; 95% confidence interval, -0.08 to 0.10; PCONCLUSIONS: The hemodynamic effects of empagliflozin, associated with reduction in intraglomerular pressure, may contribute to long-term preservation of kidney function.

Abstract

Telomerase is the enzyme that maintains telomere length by adding telomeric repeats after each cell division. Numerous metabolic factors such as obesity, insulin resistance or physical inactivity have been associated with shortened telomeres. In the present study, we assessed telomerase activity in diabetic patients having or not foot ulcer. A total of 90 adult patients with type 2 diabetes mellitus (T2DM) were studied. Patients were allocated into two groups according to the absence or presence of active foot ulcers as follows: Νon-ulcer group (N=58) and ulcer group (N=32). Our data revealed that the patients with diabetic ulcers had significantly greater waist circumference and neuropathy disability score, while exhibiting lower telomerase activity, indicating the possible existence of a common clinical profile among ulcer-bearing diabetic patients. Validation of our findings by extending the study in larger patient groups may contribute to the understanding of T2DM pathophysiology and its main clinical implications. © 2018, Spandidos Publications. 49. Modeling Biological Timing and Synchronization Mechanisms by Means of Interconnections of Stochastic Switches

Abstract

Timing and synchronization mechanisms are ubiquitous in living systems, and in many cases involve switch-like regulators that control complex molecular pathways and cellular functions. The switching of such regulators is often irreversible and controlled by the co-activation of a set of concurrent independent enabling events. Despite the random nature of each individual switch, the timing in the onset of the controlled process has a rather small variability. This note introduces a mathematical framework for the description of the collective behavior of populations of interconnected stochastic switches. The main contribution of this note is to explain how the connecting mode (series/parallel) of switches affects the behavior of the entire switch population and in particular the degree of synchronization. We describe the switch model for the G1/S phase transition in yeast and briefly discuss the general utility of this class of models in systems biology

Supplemental Digital Content is available in the text., Background and Purpose— In the EMPA-REG OUTCOME trial (Empagliflozin Cardiovascular Outcome Event Trial in Type 2 Diabetes Mellitus Patients), empagliflozin added to standard of care in patients with type 2 diabetes mellitus and high cardiovascular risk reduced the risk of 3-point major adverse cardiovascular events, driven by a reduction in cardiovascular mortality, with no significant difference between empagliflozin and placebo in risk of myocardial infarction or stroke. In a modified intent-to-treat analysis, the hazard ratio for stroke was 1.18 (95% confidence interval, 0.89–1.56; P=0.26). We further investigated cerebrovascular events. Methods— Patients were randomized to empagliflozin 10 mg, empagliflozin 25 mg, or placebo; 7020 patients were treated. Median observation time was 3.1 years. Results— The numeric difference in stroke between empagliflozin and placebo in the modified intent-to-treat analysis was primarily because of 18 patients in the empagliflozin group with a first event >90 days after last intake of study drug (versus 3 on placebo). In a sensitivity analysis based on events during treatment or ≤90 days after last dose of drug, the hazard ratio for stroke with empagliflozin versus placebo was 1.08 (95% confidence interval, 0.81–1.45; P=0.60). There were no differences in risk of recurrent, fatal, or disabling strokes, or transient ischemic attack, with empagliflozin versus placebo. Patients with the largest increases in hematocrit or largest decreases in systolic blood pressure did not have an increased risk of stroke. Conclusions— In patients with type 2 diabetes mellitus and high cardiovascular risk, there was no significant difference in the risk of cerebrovascular events with empagliflozin versus placebo.
