13. Physical Activity, Physical Function and Quality of Life in Community-Based Maintenance Cardiac Rehabilitation

Abstract

Objective: This study compared physical activity (PA), physical function and quality of life in elderly individuals with coronary artery disease (CAD) and their non-CAD peers participating in community-based, maintenance cardiac rehabilitation (CR). Design: Cross-sectional study. Setting: Community-based maintenance CR programme. Methods: A total of 39 individuals (71.8% women; age 70.5 ± 5.5 years; 13 CAD, 26 non-CAD) wore an accelerometer for 7 days and completed anthropometry, chair stands, handgrip strength, a Short Physical Performance Battery, a PA questionnaire (IPAQ- SF), a quality of life questionnaire (SF-36), a 10-m shuttle walk test and two 6-minute walk tests. Results: Compared with the non-CAD group, the CAD group accumulated more objectively measured moderate-to-vigorous PA per week (329.7 ± 233.3 vs 160.6 ± 149.5 minutes, p = 0.013), achieved PA guidelines on more days per week (3.8 ± 2.2 vs 2.1 ± 2.6 days/week, p = 0.042), had a lower proportion of body fat (27.5 ± 8.4% vs 36.5 ± 8.7%, p = 0.004) and a higher proportion of muscle mass (72.5 ± 8.4% vs 63.0 ± 9.8%, p = 0.022). Physical function and quality of life were not different between the groups. Conclusion: Elderly CAD patients participating in community-based maintenance CR performed more PA and had more favourable body composition but similar physical function and quality of life compared with their non-CAD peers. Long-term participation in community-based maintenance CR may promote PA in elderly CAD patients and help maintain physical function and quality of life at a level similar to that of their non-CAD peers. However, due to a small sample size, these findings should be interpreted with caution and examined in future larger studies.

Abstract

The study aimed to investigate the ActivityStat hypothesis by examining the presence and the timeframe of the temporal relationships among physical activity (PA) levels and stationary time (ST) in adolescents. A secondary analysis was performed on data involving 356 adolescents in Dunedin, New Zealand. Participants wore a waist-worn accelerometer for several consecutive days to assess PA levels [i.e. light PA (LPA), and moderate-to-vigorous PA (MVPA)] and ST. Bayesian continuous-time structural equation modelling (CT-SEM) was used to examine the relationship between the behaviours over time and the timeframe during which these relationships occur. Increases in LPA, MVPA, and ST were positively associated with their later behaviours until 2.5, 1.7, and 2 days later, respectively. A cross-behavioural reciprocal and negative relationship between LPA and ST was demonstrated 0.4 days later. A positive relationship between ST and MVPA was observed until about 0.4 days later. This is the first study to investigate the ActivityStat hypothesis using Bayesian CT-SEM in adolescents, examining the multivariate relationships among different behaviours and the associated timeframes. To conclude, evidence of activity synergy was suggested for the within-behavioural relationships, while behavioural compensation was noted for ST. Abstract

New Zealand legislation removing school zones radically reshaped school choice, resulting in increased school stratification from parental choice frequently driven by social factors such as ethnic makeup of the school community. This article considers school choice through the eyes of 1,465 adolescents from 12 secondary schools in Dunedin (New Zealand). The most common reasons for school choice included: preference for a coeducational school, school's facilities, positive comments from parents/students, and friends' enrollment. Reasons for school choice differed by who was making the decision. Social factors and school programs/facilities, rather than proximity to home, influenced school choice decisions in Dunedin.

25. A Short Dance-Exercise Intervention as a Strategy for Improving Quality of Life in Inactive Workers

Abstract

Objective: To investigate the effects of a short exercise intervention based on the use of a Zumba Fitness® programme on the quality of life (QoL) in inactive adult workers. Design: Non-experimental pre-test/post-test study involving one experimental group of inactive university workers. Setting: Riobamba in the Andean region of central Ecuador. Methods: A total of 60 inactive adults working at a university (age: 39 ± 1.0 years; 80% women, who used to perform < 150 min of moderate-vigorous physical activity per week) completed a 5-week Zumba Fitness® exercise intervention (three classes per week, 60 minutes per class; outside of work hours). QoL was assessed using the 36-Item Short Form Health Survey (SF-36) administered at baseline, post-intervention and 2 months after the intervention. Data were analysed using a per-protocol analysis. Results: The 5-week intervention improved six out of eight subscales of QoL, including general health (baseline: 63.6±2.51; post-intervention: 68.0±2.5; p = 0.007), physical role (baseline: 82.1±3.8; post-intervention: 90.6±3.3; p =0 0.029), emotional role (baseline: 71.3±5.0; post-intervention: 88.3±3.9; p = 0.001), social functioning (baseline: 76.9±2.6; post-intervention: 83.9 ± 2.6; p = 0.010), vitality (baseline: 60.4±2.8; post-intervention: 69.8±2.4; p < 0.001), mental health (baseline: 72.4±2.5; post-intervention: 80.4±2.3; p < 0.001) and the health transition perception item (baseline: 53.9±3.5; post-intervention: 63.6±3.1; p = 0.001). No statistical differences were found between post-intervention and 2-month follow-up; however, the majority of subscales which improved post-intervention (general health, emotional role, social functioning, vitality and mental health) were maintained at 2-month follow-up showing differences (p < 0.05) compared to baseline. Conclusion: A 5-week exercise intervention based on Zumba Fitness® programme could improve QoL in inactive adult workers and most improvements could be maintained at 2 months post-intervention.

Abstract

School neighbourhood built environment (BE) characteristics are related to active transport to school (ATS) among adolescents. This study examined objectively measured and perceived school neighbourhood BE correlates of ATS in adolescents and compared school neighbourhood BE features in rural versus small-to-medium urban areas. We analysed data from adolescents (n = 95; 68.4% under 16 years of age; 58.9% female) from 11 schools located in small-to-medium urban areas and rural settings in Otago, New Zealand, who were familiar with their school neighbourhood environment and lived within 4.8 km from school. Adolescents reported perceptions of their school neighbourhood using a modified version of Neighbourhood Environment Walkability Scale for Youth (NEWS-Y) questionnaire. Objectively measured BE features were generated using Geographic Information Systems (GIS). In a multivariate analysis, perceived residential density (odds ratio (95% CI): 0.17 (0.04, 0.82)) and traffic safety concerns (0.13(0.02, 0.95)) were negatively associated with adolescents’ ATS rates, but those associations were no longer significant once distance to school was taken into account. Compared to small-to-medium urban area schools, perceived land use mix diversity and recreational facilities and objectively measured intersection density in school neighbourhoods were higher, whereas perceived residential density and land use mix accessibility and objectively measured neighbourhood walkability were lower in rural schools (all p < 0.05). Future initiatives should address both perceived and objectively measured school neighbourhood BE to promote ATS among adolescents. [ABSTRACT FROM AUTHOR]
