Objectives: During mandibular third molar (MTM) extraction surgery, preoperative analysis to quantify the proximity of the MTM to the surrounding inferior alveolar nerve (IAN) is essential to minimize the risk of IAN injury. This study aims to propose an automated tool to quantitatively measure the proximity of IAN and MTM in cone-beam computed tomography (CBCT) images., Materials and Methods: Using the dataset including 302 CBCT scans with 546 MTMs, a deep-learning-based network was developed to support the automatic detection of the IAN, MTM, and intersection region IR. To ensure accurate proximity detection, a distance detection algorithm and a volume measurement algorithm were also developed., Results: The deep learning-based model showed encouraging segmentation accuracy of the target structures (Dice similarity coefficient: 0.9531 ± 0.0145, IAN; 0.9832 ± 0.0055, MTM; 0.8336 ± 0.0746, IR). In addition, with the application of the developed algorithms, the distance between the IAN and MTM and the volume of the IR could be equivalently detected (90% confidence interval (CI): - 0.0345-0.0014 mm, distance; - 0.0155-0.0759 mm 3 , volume). The total time for the IAN, MTM, and IR segmentation was 2.96 ± 0.11 s, while the accurate manual segmentation required 39.01 ± 5.89 min., Conclusions: This study presented a novel, fast, and accurate model for the detection and proximity quantification of the IAN and MTM on CBCT., Clinical Relevance: This model illustrates that a deep learning network may assist surgeons in evaluating the risk of MTM extraction surgery by detecting the proximity of the IAN and MTM at a quantitative level that was previously unparalleled., Competing Interests: Declarations. The interforaminal region is considered more favorable for implant placement than the posterior mandible in edentulous patients, mainly because the inferior alveolar nerve can interfere with implant placement in the severely resorbed posterior mandible. However, complications in the interforaminal region may occur due to the presence of the mandibular incisive nerve. This scoping review aims to describe the mandibular incisive nerve anatomy related to the potential interference in implant therapy. A comprehensive literature search was conducted in the following databases: MEDLINE (via PubMed), Web of Science, and Scopus. This scoping review was structured according to the Joanna Briggs Institute method. Thirteen studies were included in the review. All of the studies were observational cohort anatomical studies, carried out mainly by CBCT and on cadavers. A total of 1,471 patients/cadavers were studied. The mandibular incisive nerve was present in 87% to 100% of cases, with an average length of 9.97 mm and an average diameter of 1.97 mm. The mandibular incisive nerve may be damaged during drilling and implant placement, especially when using implant lengths > 12 mm. Damage to the mandibular incisive nerve due to implant placement could be present, but it is necessary to conduct more studies focusing on assessing mandibular incisive nerve damage to understand the clinical relevance of this nerve and its associated morbidities, such as neurosensorial alterations. Due to the different anatomical characteristics of this nerve, CBCT analysis is recommended for implant therapy in the anterior mandible to prevent the described complications.

Important anatomical structures such as mandibular incisive canal, tongue foramen, and mouth floor vessels may be damaged during implant surgery in the mandibular anterior region, which may lead to mouth floor hematoma, asphyxia, pain, paresthesia and other symptoms. In severe cases, this can be life-threatening. The insufficient alveolar bone space and the anatomical variation of blood vessels and nerves in the mandibular anterior region increase the risk of blood vessel and nerve injury during implant surgery. In case of vascular injury, airway control and hemostasis should be performed, and in case of nerve injury, implant removal and early medical treatment should be performed. To avoid vascular and nerve injury during implant surgery in the mandibular anterior region, it is necessary to be familiar with the anatomical structure, take cone-beam computed tomography, design properly before surgery, and use digital technology during surgery to achieve accurate implant placement. This article summarizes the anatomical structure of the mandibular anterior region, discusses the prevention strategies of vascular and nerve injuries in this region, and discusses the treatment methods after the occurrence of vascular and nerve injuries, to provide clinical reference.

The present study aimed to evaluate the effect of Piezosurgery on histopathologic features of the inferior alveolar nerve (IAN) damage after osteotomy of the buccal cortex of the mandible using piezoelectric devices in Hamdani sheep. A total of ten healthy mature female sheep were included. Each side of the mandible underwent two different experiments: the first experiment operated directly on the mental nerve by touching and activating the piezo tip on the nerve for ten seconds for the left side and thirty seconds for the right side. In the second experiment, the inferior alveolar nerve was touched by an activated piezo tip inside the mandibular canal for ten seconds on the left side and thirty seconds on the right side. All the nerve samples underwent histopathological evaluation, and the scoring system was performed to assess the nerve structures. Mental nerves exposed to piezo tip for 10 seconds showed mild abnormality including disruption of the perineurium with the endoneurium remaining intact. Mental nerves exposed for 30 seconds showed moderate injury with destruction of the perineurium and moderate degeneration of nerve fibers, nevertheless, the endoneurium remained continuous with normal node of Ranvier. Severe damage of the inferior alveolar nerve was seen after exposure to piezo tip for 10 seconds, which showed sloughing of the perineurium and severe vacuolar degeneration of nerve fibers, partial disruption of the endoneurium; however, the axons were still intact. Inferior alveolar nerves exposed for 30 seconds revealed destruction of the perineurium, marked vacuolar degeneration of nerve fibers, focal damage of axon and loss of endoneurium (axonotmesis). Piezosurgery devices have the potential to cause severe nerve damage during surgery and should be used very carefully.

Introduction: Facial reanimation procedures are used in the treatment of the disorder that impairs mimetic function and jeopardizes physical and psychological health, and one of the most important instruments of these techniques is the masseteric neurovascular bundle (NVB) and proper identification at the mandibular notch level. In the current study, a triangular area (deep masseteric triangle, DMT) on the lateral surface of the masseter muscle that was identified to help reliable determination of the masseteric NVB at the mandibular notch level., Material and Methods: 40 parotideomasseteric region dissections were performed in 10 female and 10 male donated cadavers. Structures lateral to the masseter muscle were removed. The edge length of the masseter muscle on the zygomatic arch side was measured. After the edges of the DMT were measured, the masseteric NVB was found by dissection and its distance (depth) from the skin line was measured., Results: The mean lengths of the superior, posterior, and anterior margins were 17.3 (±4.5) mm, 25.9 (±6.2) mm, and 26.3 (±6.5) mm, respectively. The total length of the upper edge of the masseteric muscle attached to the zygomatic arch averaged 52.7 (±5.2) mm. The masseteric neurovascular bundle was detected at a depth of approximately 17 mm from the skin of the parotideamasseteric region., Discussion: The visualization of the DMT can be used as an important landmark for access to branch-free part of the masseteric nerve. Moreover, an specific approach for masseteric NVB localization can be established by drawing a line between the mandibular angle and the midpoint of the upper edge of the DMT. Background: The purpose of this study was to evaluate the recovery of lingual nerve (LN) neurosensory function in patients undergoing sagittal split osteotomy (SSO) with a low and short medial horizontal cut., Methods: This was a prospective study of patients with mandibular deformities undergoing SSO with a low and short medial horizontal cut over a 4-year period. The outcomes of interest were neurosensory recovery of the LN, as assessed objectively using functional sensory recovery (FSR) and subjectively by patient report., Results: The sample included 123 SSOs in 62 subjects with a mean age of 19.3 ± 3.1 years. Thirty-seven subjects (61.7%) were female. Mandibular advancements were performed in 52 SSOs (42.3%); mandibular setbacks were performed in 71 SSOs (57.7%). One subject underwent revision BSSO. FSR was achieved at 122 LNs (99.1%) within 6 weeks postoperatively, with 120 sites (97.5%) having S4 sensation at 6 weeks. Decreased LN sensation was reported at 10 (8.3%) sites at 1 week postoperatively. At 6 weeks postoperatively, 118 sites (97.5%) had reported normal sensation. By 12 weeks postoperatively, all LN sites had S4 sensation and there were no subjective complaints. Revision sagittal split osteotomy was associated with prolonged (≥6 weeks) time to S4 sensation ( P = 0.02) and subjective complaint of decreased sensation ( P = 0.02)., Conclusions: LN sensory recovery occurs rapidly following the low and short SSO, with 99% of sites achieving FSR and subjectively normal sensation within 6 weeks of surgery and all patients achieving FSR with S4 sensation by 12 weeks postoperatively. Mental nerve paresthesia is a serious postoperative complication of mandibular third molar extraction. It appears that no study has examined the relationship between the surface morphology of the mandibular third molar roots and the possibility of mental nerve paresthesia following tooth extraction. Therefore, the root morphology of the mandibular third molars was examined according to age using dental cone beam computed tomography (CBCT), and the possibility of mental nerve paresthesia following tooth extraction was evaluated. The study included 1216 patients who had undergone mandibular third molar extractions. The root morphology of 1534 teeth in 791 patients who had CBCT performed before surgery was studied. Factors evaluated were age, complete or incomplete formation of the mandibular third molar roots, periodontal ligament atrophy of the mandibular third molar roots, hypercementosis, and mandibular canal deformation. Mandibular third molar root formation was completed between the ages of 19 and 30 years. Complete formation of the mandibular third molar roots (P = 0.002) and deformation of the mandibular canal (P < 0.001) were identified as risk factors for mental nerve paresthesia. This study explored the average length of the incisive branch (IB) of the inferior alveolar nerve on cone-beam computerized tomography (CBCT) with regard to patient demographics in patients with edentulous mandibles. CBCT was used in a retrospective study of edentulous mandibles to assess the presence and anatomical variation for the IB. Three independent observers measured bilateral IB lengths. In addition to demographics, IB length and port of exit data were obtained. A 1-way analysis of variance was used to test whether IB length varied by sex or port of exit, and a standard Pearson correlation was used to test for IB length and age significance, with a significance level of P < .05. Intraclass correlation coefficients showed significant agreement in IB length across all observers. No significant difference was noted between the exit port and IB length. An important effect was reported for sex, indicating women have generally shorter IB lengths (9.43 ± 3.99 vs 10.55 ± 3.92). There was a significant correlation with age, but the relationship was weak. Edentulous mandibles have an altered anatomic landscape, and establishing predictive IB dimensions aids practitioners in surgical planning.

Objective: The aim of the study is reducing the risks of surgical injury to the inferior alveolar nerve, by taking into account individual topographic and anatomical features, improving diagnostic methods, and techniques for removing retinated teeth with a close fit to the mandibular canal., Material and Methods: An examination was conducted in the Department of Surgical Dentistry (CBCT/OPG) and surgical treatment of 223 patients, with a close fit of the roots of the retinated lower third molar to the mandibular canal. Microslips of teeth with roots intact during removal ( n =96) of the main group and the control group ( n =52) were prepared with a Micromet Remet manual petrographic machine. The sections were carried out along the longitudinal axis of the tooth with the capture of the area of close fitting of the nerve, the teeth from the control group were sawed longitudinally along the axis of the root. The measurement of the macroanatomic features of the roots was carried out with a micrometer (MCC-MP-100 0.001 electronic «CHEESE», manufactured in the Russian Federation), measurements of the thickness of dentine and cement tissues on macroglyphs were carried out using a microscope calibration ruler with an accuracy of 0.01 mm., Results: In the main group, three types of attachment of the mandibular canal to the root of the third molars were distinguished: 20 (96) cases of inter-root attachment of the mandibular canal, 42 (96) apical, 34 (96) lateral (buccal and lingual). A number of anomalies in the structure of the roots of the third molars have been revealed, which are a factor in injury to the neurovascular bundle of the mandibular canal during tooth extraction. The surface of the roots, as well as the microscopes of the tooth sections adjacent to the mandibular canal, were studied under a microscope., Conclusion: A number of specific anomalies of the roots of retinated third molars formed by root dilaceration, thinning of cement tissues, hypercementosis, which are formed at the site of the mandibular canal.In the presence of a deep indentation on the root of the tooth, as well as in the presence of areas of apical hypercementosis in the form of a «spike», the probability of nerve injury during tooth extraction increases many times, which must be taken into account when removing retinated third molars.

This study aimed to compare preoperative data relevant to third molar surgery based on radiographic orthopantomography (OPG) and orthopantomogram-like MR images (MR-OPG), using five different MR protocols. X-ray-based OPG and OPG-like MRI reconstructions from DESS, SPACE-STIR, SPACE-SPAIR, T1-VIBE-Dixon, and UTE sequences were acquired in 11 patients undergoing third molar surgery, using a 15-channel mandibular coil. Qualitative (image quality, susceptibility to artifacts, positional relationship, contact/non-contact of the inferior alveolar nerve (IAN), relationship to maxillary sinus, IAN continuity, root morphology) and quantitative (tooth length, retromolar distance, distance to the IAN, and distance to the mandible margin) parameters of the maxillary and mandibular third molars were assessed regarding inter-reader agreement and quantitative discrepancies by three calibrated readers. Radiation-free MR-OPGs generated within clinically tolerable acquisition times, which exhibited high image quality and low susceptibility to artifacts, showed no significant differences compared with X-ray-based OPGs regarding the assessment of quantitative parameters. UTE MR-OPGs provided radiographic-like images and were best suited for assessing qualitative preoperative data (positional relationship, nerve contact/non-contact, and dental root morphology) relevant to third molar surgery. For continuous and focal nerve imaging, DESS MR-OPG was superior. Previous work on the mandibular canal, mental foramen, and mandibular foramen has focused on humans and some other non-primate mammals (with small sample sizes), but little work has investigated the mandibular canal and inferior alveolar nerve (IAN) across primates. However, it is important to understand the relationship between the IAN and mandibular canal due to the IAN's close relationship to the teeth and mastication, and thus dietary adaptations. While it is assumed that most bony canals within the skull grow around and form to pre-existing nervous structures, this relationship has never been validated for the IAN and mandibular canal. MicroCT scans of 273 individuals (131 females, 134 males, and 8 unknown sex) from 68 primate species and three mammalian outgroups, and diceCT scans of 66 individuals (35 females, 23 males, and 8 unknown sex) from 33 primate species and the same mammalian outgroups were used to create 3D models of the IAN and mandibular canal from which cross-sectional areas were taken at various points on the structures. Using qualitative descriptions, phylogenetic generalized least squares analysis, and phylogenetic ANOVAs, we were able to establish three main conclusions: (1) the mandibular canal is most often not a defined canal within the mandible of primates, (2) when the canal can be identified, the IAN does not comprise most of the space within, and (3) there are significant relationships between the IAN and the corresponding canals, with most showing isometry and the mental foramen/nerve showing negative allometry., (© 2023 American Association for Anatomy.)

This study aimed to evaluate the prevalence and type of mandibular canal branching in a sample of the Iranian population through cone-beam computed tomography (CBCT) images. This cross-sectional study was performed on CBCT records of 300 patients (112 males and 188 females; mean age 30.14 ± 10.96 y). The presence of mandibular canal branch (MCB) was evaluated by reconstructing multiple panoramic views and buccolingual cross-sections, according to Naitoh's classification. The prevalence of MCB, the type of MCB, and the demographic information of all patients were recorded. The maximum number of branching and the maximum type of branching were also registered for each patient. Mandibular canal branching was observed in 131(43.7%) of 300 CBCT images without side or sex differences ( P > 0.05). The most frequent type was dental, followed by the retromolar, forward, and buccolingual types, respectively. There was a statistically higher frequency of retromolar canal in females than in males (odds ratio: 1.14 ± 4.25; P = 0.013). About 46% of patients with MCB had more than one accessory canal and 29% displayed more than one type of MCB. The coincidence of dental and retromolar canals in a patient was the most prevalent combination. Considering the relatively high prevalence of MCB (43.7%), it is recommended to keep in mind the possibility of anatomical variations of the inferior alveolar nerve during dental procedures to avoid postsurgical complications. Retrieval of the displaced mandibular third molar in the floor of the mouth is challenging as the lingual nerve is always at risk of injury. However, there are no available data to show the incidence of the injury caused by the retrieval. The goal of this review article is to provide the incidence of the iatrogenic lingual nerve impairment/injury caused by the retrieval based on the review of the existing literature. The retrieval cases were collected with the search words below using PubMed, Google Scholar, and CENTRAL Cochrane Library database on October 6, 2021. A total of 38 cases of lingual nerve impairment/injury in 25 studies were eligible and reviewed. Temporary lingual nerve impairment/injury due to retrieval was found in six cases (15.8%) and all recovered between three to six months after retrieval. General anaesthesia and local anaesthesia were used for retrieval in three cases each. The tooth was retrieved using a lingual mucoperiosteal flap in all six cases. The permanent iatrogenic lingual nerve impairment/injury due to retrieval of the displaced mandibular third molar is considered extremely rare as long as the appropriate surgical approach is chosen based on surgeons' clinical experience and anatomical knowledge., (Copyright © 2023 The British Association of Oral and Maxillofacial Surgeons. Objectives: This systematic review aimed to identify the studies that performed cone-beam computed tomography analysis of the prevalence and length of the anterior loop (AL) and caudal loop (CL) of the mental nerve., Materials and Methods: An electronic database search was performed across PubMed, EMBASE, Web of Science, Scopus, the Cochrane library, and Google Scholar. Original studies reporting the frequency and length of the AL and CL were selected. Qualitative synthesis and meta-analysis were then conducted to assess the prevalence and length of the AL and CL and their associations with age, gender, sides, and dentition status., Results: A total of 21 studies were included in this review. The pooled prevalence of the AL (95% confidence interval) of the mental nerve at the patient and side level was 51% (31-71%) and 53% (37-69%) while the mean anterior loop length was 2.08 (1.46-2.70) mm. The pooled prevalence of the CL and mean caudal loop length were 100% and 4.73 (3.44-6.01) mm. No significant associations were found between the prevalence and length of the AL and CL and age, gender, sides, and dentition status., Conclusions: Overall, the pooled prevalence and mean length of AL of the mental nerve varied among different populations while CL seems to be a constant anatomical landmark with a longer CL than AL., Clinical Relevance: Surgeons performing inter-foraminal oral surgeries like dental implant placement and genioplasty should be aware of the possible distance of the AL and CL to avoid iatrogenic mental nerve injury., (© 2022. Objectives: Investigation in Saudi Arabia or the Arab Gulf States to assess the unfavorable impacts of the bilateral sagittal split osteotomy (BSSO) is non-existent, so questions have been raised about the success rate of this operation and the frequency of unwilling outcome. To address these worries, we directed a case series study to evaluate the hypoesthesia, a type of neurosensory deficit (NSD) of the inferior alveolar nerve (IAN) after BSSO, and if the hypoesthesia outcome will improve if the surgeries performed by a single surgeon., Patients and Methods: This was a prospective case series study for the patient who underwent BSSO in a medical complex that is considered one of the largest in Saudi Arabia (Riyadh). The inclusion criteria include patient aged 18-40 years, any gender, and American Society of Anesthesiologists (ASA) class I. They will undergo BSSO for either mandibular, retrognathia, prognathic, or to follow the maxilla. The outcome will be measured after evaluating the neurosensory by four means light touch (LT), pinprick (PP), 2-point discrimination (2PD), and thermal sensations (TT) in four repeated measurements (preoperatively, 1 week, 1 month, 3 months postoperatively) as the primary outcome. Other confounding factors were the secondary outcome (age, gender, visualization of the I.A.N, the type of mandibular movement, split favorability, mandibular canal location, and patient reports about paresthesia or dysesthesia on any given side); these data analyses were carried out using SPSS ver. 25 data processing software., Results: The nerve was visible in 93% of cases. During the operation, none of the nerves was transected. Hypoesthesia on the first follow-up was 94% of cases for LT, 92% for PP, 82% for TT, and 100% for the 2PD. On the last follow-up, the patients still had hypoesthesia for the LT 51%, PP 35%, TT41%, and 2PD 55%; age and sex did not significantly affect hypoesthesia outcomes. Nerve visibility and inferior alveolar nerve canal (IAC) distance did not influence the results. The level of confidence for all tests was set at p < 0.05., Conclusions: The 2PD sensation was the most affected sense on the last visit, and the right side of the chin and lower lip was affected most both on early and long-term follow-up due to several reasons. A 3-month period was enough as a recovery time to restore 100% of neurological sensation for 45% of the sample, which is similar to several studies in the literature. A single surgeon did not show superior result compared to two surgeons' literature papers. Advancement movement was associated with a high percentage of hypoesthesia., (© 2021. Preoperative assessment is essential to prevent inferior alveolar nerve (IAN) injury during surgical extraction of the lower third molar (LM3). Here, we aimed to establish an assessment system to predict IAN injury during surgical extraction of the LM3. We conducted a retrospective cohort study on 115 patients diagnosed as 'high-risk' based on our previous risk assessment method involving three anatomical features of the inferior alveolar canal using computed tomographic (CT) images. We evaluated the occurrence of neurosensory impairment in these high-risk patients, and its association with novel anatomic features based on CT images. Neurosensory impairments were observed in 19 patients (16.5%). The inferior alveolar canal major diameter (p < 0.0001) and lingual bone thickness (p = 0.0039) were significantly associated with the occurrence of neurosensory impairment during LM3 extraction. Receiver operating characteristic curves were used to determine cut-off values of these quantitative factors to specifically predict IAN injury. Purpose: Inferior alveolar neurosensory disturbance (IAND) is the most common complication of bilateral sagittal split osteotomy (BSSO). The aim of the present study was to evaluate IAND with subjective tests postoperatively and assess the relationship between three-dimensional measurements of the mandibular canal (MC) and IAND., Methods: Eighteen patients (Mean age: 24.05 ± 5.85 years) treated with BSSO were retrieved from the archive. Subjective tests (light touch, tactile sensitivity, 2-point discrimination, brush-stroke directional discrimination, sharp/blunt discrimination, dental vitality, questionnaire) and three-dimensional measurements related to MC and fixation screws were done postoperatively (on average 20.43 ± 8.76 months after surgery). Statistical significance was set at P  < 0.05., Results: Subjective test results were found compatible with each other except brush-stroke directional discrimination test. According to the questionnaire, IAND was apparent in all patients immediately after surgery, and recovery after 1 to 2 years was statistically significant ( P  < 0.05). Preoperative ramus width, medial and lateral cancellous bone lengths, the decrease in MC length, and the presence of screw in MC were not related to IAND ( P  < 0.05)., Conclusions: There is a high incidence of IAND following BSSO, and the subjective tests are efficient to evaluate the disturbance. Spontaneous recovery of the nerve occurs during the follow-up periods. The aim of this study was to evaluate the prevalence and length of the anterior loop (AL) of the inferior alveolar nerve, nerve emergence from the mental foramen, and prevalence of sensory disorders after implant placement in the interforaminal region. Four hundred and fifty hemimandibles (225 patients) were evaluated using cone beam computed tomography and panoramic radiographs. Information on the presence of sensory disorders was obtained from the medical records. AL prevalence was 13.6% and mean AL length was 1.25 mm. The false-negative rate for the identification of the AL using panoramic radiography was 58.6%. Straight nerve emergence from the mental foramen was the most prevalent (62.7%), followed by anterior (21.8%) and posterior (15.6%) emergence. The incidence of sensory disorders was 4.4%, and 1.1% were related to the presence of the AL. When implants were placed within the planned distance of the mental foramen or further, 1.2% had sensory problems associated with the presence of the mandibular incisive canal. In cases of distances smaller than planned, 12.9% had sensory alterations. Only five (1.1%) had the AL, with a length between 0 and 4.5 mm. However, in four cases, the planned distance was respected and, even so, there was a sensory disorder. The nerve to mylohyoid muscle supplies the mylohyoid and the anterior belly of the digastric muscles, with terminal sensory branches that might innervate the submental skin and mandibular teeth. The nerve to mylohyoid muscle typically originates from the posterior surface of the inferior alveolar nerve right before entering the mandibular foramen. In rare cases, the nerve to mylohyoid muscle arises from the lingual nerve. The variations of the nerve to mylohyoid muscle might have led to failure of an inferior alveolar nerve blockade. During the routine dissection of a cadaveric head, a rare case was identified where the nerve to mylohyoid muscle had origins from both the inferior alveolar and lingual nerves. This case is reviewed and salient literature reviewed.

The general principles of anatomical terminology indicate that the "mandibular canal" should be named the "inferior alveolar canal" as it accommodates the inferior alveolar neurovascular bundles. Therefore, we performed a Delphi study to evaluate the current understanding and use of the terminology in different geographical regions and areas of expertise and to determine the appropriate terminology for this bony canal. A Delphi panel was formed and questions sent and answered via email about: field of expertise (anatomy, oral surgery/oral and maxillofacial (OMF) surgery, oral radiology/OMF radiology, plastic surgery, ENT surgery, or dentistry with the exception of oral/OMF surgery and oral/OMF radiology), years of experience in the field of expertise, country currently working in, "what is the name of the bony canal that contains the inferior alveolar neurovascular bundle," and "what should the structure above be called, in general?" A total of 52 participants responded to the questionnaire. Half or more of the experts in anatomy, oral/OMF surgery, and ENT/plastic surgery considered "mandibular canal" to be the most appropriate name for this bony canal. In contrast, more than half of all experts in oral/OMF radiology and dentistry, that is, most fields of dentistry, considered "either mandibular canal or inferior alveolar canal" to be the appropriate name. Abstract: Nerve injury especially inferior alveolar nerve (IAN) is the one of the complications that occur when the mandibular third molar (M3) is extracted and in case of high risk patients, coronectomy might be an alternative to tooth extraction. The purpose of this retrospective study was to analyze root migration and its influencing factors at 6 months after coronectomy in both 2- and 3-dimensions using periapical view and cone-beam computed tomography (CBCT). We analyzed 33 cases of root remnant after coronectomy and measured the amount of migration in CBCT. The following factors that could possibly affect root migration were also analyzed: age, gender, number of M3 roots, shape of M3s, Pell, and Gregory classification, mesiodistal (MD) angulation, buccolingual (BL) angulation, contact point with the second molar, root curvature, and complete removal of the coronal portion. Migration of greater than 2 mm was found in 64% of the roots in the 2-dimensional (2D) analysis, and the average root migration was 4.11 mm in the 3-dimensional (3D) analysis. The factors affecting migration were the root morphology, complete removal of the coronal portion, impaction depth, and MD angulation in the 2D analysis, and MD and BL angulation in the 3D analysis. Ensuring sufficient space for root migration especially considering angulation, depth and complete removal of the coronal portion might be important factors after coronectomy of the M3. The inferior alveolar nerve (IAN) is a sensitive branch of the mandibular nerve innervating the lower lip, the chin, the buccal mucosa and the teeths. Lesions of the IAN are reported to occur in the 64,4% of maxillo-facial procedures, leading to anesthesia, hypoestesia and/or neurogenic discomfort. An extensive segment of the nerve can be moreover removed during mandibular resection for benign or malignant pathologies. Nervous grafts can be used in these cases to restore the nerve continuity. In order to optimize the procedure and to allow a concomitant mandibular osseous reconstruction, the Authors identified several standardized steps. Background: Mandibular fracture healing is a complex process involving nerves and growth factors. Nerve growth factor (NGF) not only facilitates the maintenance of sympathetic neurite growth but also stimulates other growth factors that can promote the essential osteogenesis and angiogenesis for fracture healing. Therefore, it is necessary to analyze the combined effects of NGF, bone morphogenic protein-9 (BMP-9), and vascular endothelial growth factor (VEGF) to accelerate the healing of mandible fractures., Methods: The models of mandible fracture with local nerve injury established in 48 rabbits were randomly divided into nerve growth factor group (NGF group), gelatin sponge group (GS group), blank group, and intact group. The recovery of nerve reflex was assessed by observing the number of rabbits with lower lip responses to acupuncture. The fracture healing was observed with visual and CBCT, and then callus tissues from the mandibular fracture area were collected for hematoxylin and eosin (HE) staining observation, and the expression of BMP-9 and VEGF in callus at different stages was detected by quantitative real-time PCR (qRT-PCR)., Results: Needling reaction in the lower lip showed the number of animals with nerve reflex recovery was significantly higher in the NGF group than that in the GS and blank groups at the 2nd and 4th weeks after the operation. The combined results of macroscopic observation, CBCT examination, and histological analysis showed that a large number of osteoblasts and some vascular endothelial cells were found around the trabecular bone in the NGF group and the amount of callus formation and reconstruction was better than that in the GS group at the 2nd week after the operation. The qRT-PCR results indicated that the expression levels of BMP-9 and VEGF in the four groups reached the highest values at the 2nd week, while the expression levels of both in the NGF group were significantly higher than that in the GS group., Conclusion: The exogenous NGF could accelerate the healing of mandible fractures. This work will provide a new foundation and theoretical basis for clarifying the mechanism of fracture healing, thereby promoting fracture healing and reducing the disability rate of patients.

Background: Osseointegration is the premise of the chewing function of dental implant. Nerve growth factor (NGF), as a neurotrophic factor, can induce bone healing. However, the influence of NGF-chondroitin sulfate (CS)/hydroxyapatite (HA)-coating composite implant on the osseointegration and innervations is still not entirely clear., Materials and Methods: NGF-CS/HA-coating composite implants were prepared using the modified biomimetic method. The characteristics of NGF-CS/HA-coating implants were determined using a scanning electron microscope. After NGF-CS/HA-coating implants were placed in the mandible of Beagle dogs, the early osseointegration and innervation in peri-implant tissues were assessed through X-ray, Micro-CT, maximal pull-out force, double fluorescence staining, toluidine blue staining, DiI neural tracer, immunohistochemistry, and RT-qPCR assays., Results: NGF-CS/HA-coating composite implants were made successfully, which presented porous mesh structures with the main components (Ti and HA). Besides, we revealed that implantation of NGF-CS/HA-coating implants significantly changed the morphology of bone tissues and elevated maximum output, MAR, BIC, and nerve fiber in the mandible of Beagle dogs. Moreover, we proved that the implantation of NGF-CS/HA-coating implants also markedly upregulated the levels of NGF, osteogenesis differentiation, and neurogenic differentiation-related genes in the mandible of Beagle dogs., Conclusion: Implantation of NGF-CS/HA-coating composite implants has significant induction effects on the early osseointegration and nerve regeneration of peri-implant tissues in the mandible of Beagle dogs.

Background: Transoral endoscopic thyroidectomy vestibular approach (TOETVA) is a promising technique for eliminating a neck incision. A new risk of TOETVA is the potential for injury to the mental nerves during placement of three oral endoscopic ports. A better understanding of the variations in mental nerve anatomy is needed to inform safer TOETVA technique., Materials and Methods: We performed 120 dissections of mental nerve branches exiting the mental foramen in 60 human cadavers. Anatomic distances and relationships of the foramen to the midline were evaluated. Mental nerve branching patterns were studied and compared with previously reported classification systems to determine surgical safe zones free of nerve branches., Results: The mean midline-to-mental foramen distance was 29.2 ± 3.3 mm, with high variability across individuals (18.8-36.8 mm). There were differences in this distance between the left and right foramina (29.8 ± 3.2 versus 28.8 ± 3.3 mm, P = 0.03). All mental nerve branches exiting the mental foramen distributed medially. The branching patterns were classified into eight distinct categories, three of which are previously undescribed. One of these novel patterns, occurring in 9.2% of cases, had a dense and wide clustering of branches traveling toward the midline., Conclusions: The location of the mental foramen and mental nerve branching patterns demonstrate high variability. To avoid mental nerve injury in TOETVA, we identify a safe zone for lateral port placement lateral to the plane of the mental foramen. The Mexican salamander, Ambystoma mexicanum, is able to regenerate limbs, tail, and gills when nerves are present. However, the nerve-dependency of tooth regeneration has not been evaluated. Here, we reevaluated tooth regeneration processes in axolotls using a three-dimensional reconstitution method called CoMBI and found that tooth regeneration is nerve-dependent although the dentary bone is independent of nerve presence. The induction and invagination of the dental lamina were delayed by denervation. Exogenous Fgf2, Fgf8, and Bmp7 expression could induce tooth placodes even in the denervated mandible. Our results suggest that the role of nerves is conserved and that Fgf+Bmp signals play key roles in axolotl organ-level regeneration. Objective: The aim of the study was to test the effect of transecting the inferior alveolar nerve on the inorganic bone component of the rat mandible., Methods: 7-9 weeks old, male Wistar rats were used for the study. The animals were divided in 3 groups: control, experimental (nerve was transected) and sham (nerve was only prepared but not transected). After 4 weeks, the animals were killed, their teeth were extracted, and the mandibular bone was divided in 4 parts. Inductively coupled plasma mass spectrometry was used to the levels of 7 elements in the bone., Results: The study results demonstrate that transection of the inferior alveolar nerve caused a decrease in calcium, iron, and strontium, and an increase of zinc. It caused the differences in potassium contents between the sides was significantly lower in the experimental group. The increase in the magnesium content, and decrease of sodium and potassium in the experimental group, as well as differences in the contents of: magnesium, sodium, potassium, iron and zinc between individual locations in the mandible are associated with the surgical approach., Conclusion: The results support our hypothesis - that sensory innervation has an impact on the inorganic component of the mandibular bone., Competing Interests: The authors have no conflict of interest.

Background: The inferior alveolar nerve (IAN) courses through the mandibular foramen (MF) to enter the first segment of the mandibular canal (MC) in the mandibular ramus, to further supply with trigeminal fibers the lower teeth of that hemimandible. As the IAN also supplies the mylohyoid nerve, it is a mixed nerve., Methods: Unusual morphologies of the mandibular ramus were encountered during a retrospective study of archived CBCT files., Results: A previously unreported anatomic variation was found bilaterally in an edentulous mandible, consisting of a lowered position of the MF, with seemingly compensatory lengthening and enlargement of the sulcus colli, thus shortened MCs. Purpose: In bilateral sagittal split osteotomy (BSSO), the risk of lesion of the inferior alveolar nerve (IAN) is important. The aim of this study was to investigate the situation of the IAN in the area of the BSSO in preoperative 158 CBCT., Methods: The situation of the mandibular canal (MC) has been studied in six coronal sections (one section each 4 mm) from the proximal root of the second mandibular molar. The height of the MC related to the height of the mandible, and the horizontal distance between the MC and the lateral cortical plate related to the mandibular width have been determined in each section. The cancellous bone width has been measured allowing to determine a cancellous bone ratio., Results: The variability of the MC was high, depending mainly on the cancellous bone ratio which was higher when the IAN was deep. When a third molar was present, the MC was lower in the area of the third molar., Conclusions: To decrease the risk of injuring the IAN in BSSO, the evaluation of the cancellous bone ratio by a preoperative CBCT may be proposed to adapt the surgical technique to the anatomy.

Purpose: To investigate the relationship between the mandibular foramen (MF) and the inferior occlusal plane (IOP) of anatomic specimens, in order to guide clinicians in the inferior alveolar nerve block (IANB)., Method: Eighty-two dry mandibles (totally or partially toothed) of Brazilian adults were selected, totalizing 66 left and 66 right antimeres. A flat plate of rigid PVC plastic was placed on the occlusal surfaces of posterior teeth and pressure was then manually applied to simulate the IOP. Following, the perpendicular distance from the IOP to the lowest point of the MF was measured by the use of a digital caliper, adopting negative values inferiorly and positive values superiorly., Results: Regarding the antimeres, the majority of the lowest limits of the MF was below the IOP (right: 86.4%, left: 81.8%) with a mean of -4.2mm (±4.8) in the right and -4.1mm (±4.8) in the left ones, without any statistical significance. Considering both sexes, no statistically significant difference was obtained between them., Conclusion: The IOP seems not to be a good anatomical landmark to localize the MF and, its turn, to guide clinicians during the IANB for Brazilian adults, given the marked topographic variations., (Copyright © 2019 Elsevier Masson SAS. Aim: The aim of this retrospective study is to determine the level of visibility of the lingual foramen in panoramic radiography using cone-beam computed tomography (CBCT) as a reference tool., Method: A total of 200 patients (111 females and 89 males) were included in the study. Cross-sectional CBCT images of the mandible in the midline were used to locate and define the orifice and diameter of each lingual canal that was detected. The appearance of the lingual foramen in the anterior mandible in panoramic radiography was assessed using a five-point ordinal scale. Descriptive statistics including tables and graphs were used. The correlation between CBCT and panoramic radiography findings were analyzed using Chi-square tests., Results: The lingual foramen was found to be located most frequently in the mandibular midline, above the genial tubercle (78.5%). A narrow-diameter type was found to occur most frequently (56.5%). A prominent genial tubercle was the most commonly found type (57.5%). The lingual foramen could be definitively identified in panoramic radiographs in only 8 cases (4%), while a high degree of probability was found in 25 cases (12.5%). The lingual foramen appears wider and therefore more distinctive in panoramic radiographs when the angulation of the lingual canal is less than 20° to the ground plane., Conclusions: Radiological evaluation with panoramic radiographs alone may lead to inadequate assessments prior to surgical procedures involving the anterior mandible. Clinicians may consider using three-dimensional imaging for procedures with a risk of hemorrhagic and neurosensory complications such as dental implant placement in the anterior mandible., Competing Interests: None

Purpose: An accessory foramen around the mandibular foramen is called an accessory mandibular foramen (AMF). The clinical anatomy of the AMF has not been well described. The aim of this study was to reveal the clinical anatomy of the AMF for a better understanding of its implication during ramus surgeries., Methods: Twenty-two sides fresh-frozen cadaveric heads with a mean age of 76.2 ± 14.4 years at death underwent dissection. The neurovascular bundles passing through the AMF were observed. Additionally, a hemi-face of a latex injected embalmed cadaver was dissected medially to laterally and the neurovascular bundles of the AMF investigated., Results: A unilateral foramen, bilateral foramina, and absence of foramina were found in 45.4%, 18.2%, and 36.4%, respectively. The origin of the neurovascular bundle entering the AMF was a branch of the maxillary artery in 20% and a tributary of the inferior alveolar vein in 80%. In the latex embalmed cadaver, the AMF was found to contain a branch from the maxillary artery and a tributary of the maxillary vein., Conclusion: Given the practical meaning of the specific AMF located in the operative field of the ramus osteotomy, we suggest these be named "foramina for ramus osteotomy."

Background: The marginal mandibular branch (MMB) of the facial nerve provides lower lip symmetry apparent during human smile or crying and is mandatory for vocal phonation. In treating facial palsy patients, so far, little attention is directed at the MMB in facial reanimation surgery. However, isolated paralysis may occur congenital, in Bell's palsy or iatrogenic during surgery, prone to its anatomical course. A variety of therapies address symmetry with either weakening of the functional side or reconstruction of the paralyzed side. To further clarify the histoanatomic basis of facial reanimation procedures using nerve transfers, we conducted a human cadaver study examining macroanatomical and microanatomical features of the MMB including its axonal capacity., Methods: Nerve biopsies of the MMB were available from 96 facial halves. Histological processing, digitalization, nerve morphometry investigation, and semiautomated axonal quantification were performed. Statistical analysis was conducted with P < 0.05 as level of significance., Results: The main branch of 96 specimens contained an average of 3.72 fascicles 1 to 12, and the axonal capacity was 1603 ± 849 (398-5110, n = 85). Differences were found for sex (P = 0.018), not for facial sides (P = 0.687). Diameters were measured with 1130 ± 327 μm (643-2139, n = 79). A significant difference was noted between sexes (P = 0.029), not for facial sides (P = 0.512.) One millimeter in diameter corresponded to 1480 ± 630 axons (n = 71). A number of 900 axons was correlated with 0.97 mm (specificity, 90%; sensitivity, 72%)., Conclusions: Our morphometric results for the MMB provide basic information for further investigations, among dealing with functional reconstructive procedures such as nerve transfers, nerve grafting for direct neurotization or babysitter procedures, and neurectomies to provide ideal power and authenticity.

Although various animal studies have indicated that sensory nerves played an important role in bone metabolism and nerve injury could impair the process of bone remodeling, the actual effect of sensory nerve rupture on human bones remains unclear. The aim of this preliminary study was to investigate the effect of inferior alveolar nerve (IAN) rupture on mandibular bone remodeling of patients underwent bilateral sagittal split ramus osteotomy (BSSRO).Ten patients with unilateral IAN rupture during BSSRO were involved in this study. Neurosensory examinations were employed to assess the sensory function of bilateral IAN. The remodeling process of the post-operational mandible was evaluated by panoramic radiographs and computed tomography (CT) scans.Neurosensory examinations indicated that nerve rupture resulted in significant hypoesthesia at the IAN-rupture side. Assessment of panoramic radiographs showed no evident alterations of bone structure at the IAN-rupture side of mandible. Evaluation of CT images also indicated no statistical difference in bone density and thickness between IAN-rupture side and contralateral side.Accordingly, our study indicated that IAN rupture may not significantly impair the short-term bone remodeling process of human mandible.

Background: The mandibular incisive canal (MIC) is a neural canal containing one of the lower branches of the inferior alveolar nerve, called the mandibular incisive nerve, which can get damaged and cause complications during the removal of bone from the interforaminal region., Objectives: The aim of this study was to determine the effect of the inversion filter (IF) on improving the visibility of MIC as compared to the original images., Material and Methods: In this retrospective, descriptive, analytical study, 343 samples of digital panoramic radiography were examined. The images were analyzed with and without IF. The frequency and confidence intervals (CIs) of identifying MIC were used to determine its visibility, both with IF and in the original images. Besides, the difference between the maximum and minimum diameters of the canal as well as the distance from MIC to the alveolar crest and to the mental foramen were examined. For statistical analysis, McNemar's test and the paired t-test were used, and the concordance was calculated using the kappa coefficient., Results: No significant differences were found in the prevalence of the incisive canal, or in its unilateral or bilateral visibility between the original and filtered radiography in this study (p = 0.42 and p = 0.67, respectively). The absolute values of the interval difference between MIC and the mental foramen, the maximum and minimum diameters of MIC, and the distance from MIC to the alveolar crest were statistically significant between the filtered and original radiography, although the difference was clinically unimportant., Conclusions: The use of IF produced results similar to the original radiography; its application neither increased the clarity nor improved the visibility of the incisive canal.

Objectives : To investigate the effects of inferior alveolar nerve on new bone formation in rabbit mandibular distraction osteogenesis. Methods : 20 New Zealand White rabbits underwent bilateral distraction osteogenesis with a rate of 1 mm/day. The inferior alveolar nerve of one side was resected under the surgical microscope, with the inferior alveolar vascular intact. The contralateral side received sham operation. The rabbits were sacrificed at consolidation time of 28 days. The regenerate callus underwent radiograph examination, dual-energy X-ray absorptiometry, haematoxylin and eosin staining and histomorphometric analysis. A paired t-test was performed using SPSS 16.0 software package. Results : The BMD of the new bone in the distraction gap on the denervation side of mandibular was significantly lower (P<0.05) than on the control side. The histological investigation showed that the bone trabeculae were dis-arrayed containing dispersed cartilage cells on the denervation side, whereas the bone trabeculae were orderly with rich blood vessels and no cartilage cell on the control side. Both new bone volume and the thickness of new trabeculae were significantly lower on the denervation side than on the control side (P < 0.05). The aim of this study was to compare, in vitro, the mechanical resistance to vertical displacement of the mandible after osteotomy for lateralization of the inferior alveolar nerve and installation of dental implants. One hundred eighty polyurethane mandibles were equally divided into 6 groups: G1-intact hemi-mandibles (control group), G2-hemi-mandibles after osteotomy for lateralization of the inferior alveolar nerve, G3-hemi-mandibles with installation of 3 bicortical dental implants (3.75 × 13 mm), G4-hemi-mandibles with installation of 3 dental implants that did not reach the basal cortical bone (3.75 × 11 mm), G5-hemi-mandibles after osteotomy for lateralization of the inferior alveolar nerve and installation of 3 bicortical dental implants (3.75 × 13 mm) and G6-hemi-mandibles after osteotomy for lateralization of the inferior alveolar nerve and installation of 3 dental implants that did not reach the basal cortical bone (3.75 × 11 mm). The specimens were subjected to linear loading tests. The highest mean value of maximum load was found in G1 (412.36N ± 11.99), followed by G2 (396.87N ± 23.94), G3 (319.63N ± 57.28), G4 (303.34N ± 18.25), G5 (231.75N ± 63.64) and G6 (228.13N ± 20.75). Based on this data, it can be concluded that the bicorticalization (or not) of the implants was not a statistically significant risk factor for the vertical displacement of polyurethane hemi-mandibles.

The lingual nerve is a branch of the mandibular division of the trigeminal nerve. It descends medial and anterior to the inferior alveolar nerve through the pterygomandibular space, runs by the lingual plate and lingual crest at the lower third molar closely, and supplies sensory fibers to the anterior two-thirds of the tongue. Therefore, injury of this nerve is occasionally induced by wisdom tooth extraction and could lead to paralysis of the tongue. The inferior alveolar nerve gives rise to the nerve to mylohyoid just before entering the mandibular foramen, which supplies the mylohyoid and anterior belly of the digastric muscle. We present an extremely rare anatomical variation where the nerve to mylohyoid arose from the lingual nerve near the submandibular duct during routine oral dissection.

After review of the literature, there appears to be no report on the histology of the mandibular nerve fiber distribution. Therefore, using a Wistar rat model, immunohistochemical staining with protein gene product (PGP) and calcitonin gene-related peptide (CGRP) antibody for all nerves and only the pain-sensitive nerves, respectively, was performed. We also statistically compared the nerve distribution density by mandibular region. The section of the mandible from the alveolar crest to the mandibular canal was compartmentalized to several regions. Subsequently, nerve distribution density by region was measured microscopically in both the PGP- and CGRP-positive nerves. Furthermore, the ratio of CGRP- to PGP-positive nerves was measured in each region and statistically compared. In both the PGP- and CGRP-positive nerves, the nerve distribution density significantly increased vertically toward the mandibular canal from the alveolar crest and horizontally toward the periodontal ligament from the periosteum. From the CGRP- to PGP-positive nerve ratio, the pain-sensitive nerve accounted for approximately >70% in each region. Pain would therefore be more likely to develop when surgical invasiveness deepens toward the mandibular canal or periodontal ligament. Therefore, sufficient local anesthetic infiltration and/or combined use of conduction anesthesia or periodontal ligament injection may be required. These results may aid in the development of more effective surgical and anesthetic techniques for mandibular surgery.

The aim of this study was to find out if juxta-apical radiolucency (JAR) is a reliable risk factor for injury to the inferior alveolar nerve (IAN) during removal of lower third molars. We designed a cohort study of patients whose dental panoramic tomograms (DPT) had shown JAR before complete removal of lower wisdom teeth. The outcome variable was postoperative permanent neurosensory disturbance of the IAN. A total of 39 patients (50 lower third molars) were identified and screened for permanent neurosensory disturbance. None reported any permanently altered sensation 18 months after the operation. Purpose: The aim of this study was to evaluate the frequency and location of accessory mental foramen (AMF) in a Turkish population using cone beam computerized tomography (CBCT)., Method: CBCT images of 1005 patients were retrospectively reviewed from the archive of the Department of Oral and Maxillofacial Radiology, Faculty of Dentistry, Izmir Katip Celebi University. The prevalence, size, and localization of AMFs were assessed according to gender and age. In addition, the vertical and horizontal dimension of the mental foramen (MF) with and without AMF was measured to investigate the relationship between MF dimensions and the presence and absence of AMF. The average distance between the MF and AMF was also calculated. All measurements were then analyzed statistically., Results: A total of 149 AMFs in 123 (12.23%) patients were identified on the CBCT images of the 1005 patients reviewed. There was no significant correlation between presence of AMF and gender (p  = 0.152). The most common localization of an AMF with respect to the MF was posterio-inferior (n = 52; 34.89%). A statistically significant difference was also found between the vertical and horizontal dimensions of the MF with and without AMF (p  < 0.05)., Conclusion: The results of this study demonstrate the high prevalence of AMF in the studied Turkish population. Considering the high prevalence and the structures passing through it, recognition of AMF and its detection using CBCT is important to prevent complications that may occur during and after surgical procedures.

Nerve fibers and vessels play important roles in bone formation, and inadequate innervation in the bone defect area can delay the regeneration process. However, there are few studies aiming to promote innervation to engineer bone formation. Here, we report the development of an injectable thermoresponsive mesoporous silica nanoparticle (MSN)-embedded core-shell structured poly(ethylene glycol)-b-poly(lactic-co-glycolic acid)-b-poly(N-isopropylacrylamide) (PEG-PLGA-PNIPAM) hydrogel for localized and long-term co-delivery of microRNA-222 and aspirin (ASP) (miR222/MSN/ASP hydrogel). ASP was found to stimulate bone formation as previously reported, and miR222 induced human bone mesenchymal stem cell differentiation into neural-like cells through Wnt/β-catenin/Nemo-like kinase signaling. In a rat mandibular bone defect, injection of the co-delivered MSN hydrogel resulted in neurogenesis and enhanced bone formation, indicating that the present injectable miR222- and ASP-co-delivering colloidal hydrogel is a promising material for innervated bone tissue engineering.

The aim of this case report was to describe the inferior alveolar nerve (IAN) lateralization technique using the piezoelectric device for the posterior rehabilitation of an atrophic mandible with implants. The patient presented the absence of elements 35 and 36 associated with a vertical defect impairing the adequate dental implant placement without IAN damage. A full-thickness mucoperiosteal flap was raised, and a bone window was made with a piezoelectric device centralized on the IAN canal position. After dissection, the IAN was moved buccally with a sterile elastic strip. Morse cone 4.0 × 1.5-mm implants were inserted while the IAN remained retracted. At the 4-month follow-up, the screw-retained prosthesis was installed guaranteeing the recovering masticatory function. In conclusion, the case report showed that the IAN lateralization performed with an adequate surgical technique can be successfully indicated for longer implant placement in edentulous atrophic posterior mandible with no permanent neural damage.

Purpose: To introduce a modified protocol for mandibular reconstruction and evaluate the protocol using a standardized assessment method., Method: This retrospective study involved a case series of nine patients who underwent mandibular reconstruction between 2015 and 2017. The modular protocol comprised three novel modifications in terms of computer-assisted surgical simulation (CASS); surgical template (ST), and surgical procedure. The standardized postoperative evaluation consisted of operation time, part comparison analysis (PCA), facial symmetry, and mechanical quantitative sensory testing., Results: The surgery successfully removed the affected mandible and preserved the inferior alveolar neurovascular bundle (IANB). PCA revealed that the mean error and standard deviation were 0.92 and 0.96 mm, respectively, for all mandibular surface sites. Follow-up results showed good facial symmetry, existence of sensation in lower lip, and no significant differences in pulp vitality between both sides (p = 0.181). Also, the results showed a reduction in the overall operating time., Conclusion: The modified mandibular reconstruction method used in this study could repair lateral mandibular defects and preserve the sensory function of the chin and lower lip., (Copyright © 2018 European Association for Cranio-Maxillo-Facial Surgery. A major component of tooth innervation is made of capsaicin-sensitive primary afferents (CSPA). These fibers play a key role in tooth pain and inflammation; little is known, however, about the role of CSPA in tooth eruption. The aim of this study was to examine the role of the capsaicin-sensitive afferents in the process of eruption of intact rat incisors. CSPA fibers in several rat groups, were subjected to one of the following experimental procedures: systemic chemical ablation, systemic ablation followed by chemical sympathectomy and localized activation. The observed effects on incisor eruption were compared to those made on controls. The total amount of eruption in control/naïve rats, measured over a total period of 144 h, was 3.18 ± 0.07 mm and decreased to 2.43 ± 0.08 mm (n = 7; p < 0.001) following systemic ablation of CSPA. Further decrease to 2.24 ± 0.08 mm (n = 7; p < 0.001) was noticed when chemical sympathectomy was added to CSPA ablation. The average rate of eruption was 1.7 ± 0.25 mm following CSPA activation, compared to an average of 0.8 ± 0.07 mm for controls (n = 7; p < 0.001). Capsaicin sensitive fibers play an important role in tooth homeostasis, and intact neural supply is required for tooth growth under normal conditions.

It is challenging to achieve high implant accuracy in dental implant placement, because high risk tissues need to be avoided. In this study, we present an augmented reality (AR) surgical navigation with an accurate cone beam computed tomography (CBCT)-patient registration method to provide clinically desired dental implant accuracy. A registration device is used for registration between preoperative data and patient outside the patient's mouth. After registration, the registration device is worn on the patient's teeth for tracking the patient. Naked-eye 3D images of the planning path and the mandibular nerve are superimposed onto the patient in situ to form an AR scene. Simultaneously, a 3D image of the drill is overlaid accurately on the real one to guide the implant procedure. Finally, implant accuracy is evaluated postoperatively. A model experiment was performed by an experienced dentist. Totally, ten parallel pins were inserted into five 3D-printed mandible models guided by our AR navigation method and through the dentist's experience, respectively. AR-guided dental implant placement showed better results than the dentist's experience (mean target error = 1.25 mm vs. 1.63 mm; mean angle error = 4.03° vs. 6.10°).
