Background and objective: Bronchoscopic resection of endobronchial hamartomas has been reported to have a favourable outcome. This study describes the bronchoscopic features of endobronchial hamartoma and reports the clinical outcome of bronchoscopic intervention. Methods: A retrospective analysis was conducted of patients with histologically proven endobronchial hamartomas, diagnosed in the 10-year period 1999–2009 to elucidate the clinical, radiological and bronchoscopic features of hamartoma and to describe the clinical outcomes. Results: Seventeen of the 135 patients with pulmonary hamartomas were diagnosed as having endobronchial hamartomas. CXR was abnormal in 11 of the 17 patients. On chest CT (n = 16), the median diameter of the lesion was 15.6 mm. Calcification and areas of focal fat in the lesion, the diagnostic CT findings of pulmonary hamartoma, were found in two of 16 (12.5%) patients. At bronchoscopy (n = 16), all tumours had a mass appearance and most were smooth surfaced round masses (50.0%) with 18.8% having a ‘stalk’. Bronchoscopic forceps biopsies were performed in 13 patients, which resulted in five patients (38.5%) being diagnosed with endobronchial hamartoma. Fifteen patients were treated with rigid or flexible bronchoscopic resection, one had lobectomy, and one had no intervention. No procedure-related mortalities or late complications developed. Conclusions: Bronchoscopic intervention appears to be a safe and effective method to resect endobronchial hamartomas.

PURPOSE To assess the correlation among quantitative indexes of computed tomography (CT), spirometric pulmonary function tests (PFTs), and visual scores (VSs) of CT in patients with diffuse interstitial lung disease (DILD) and to prove the estimated value of CT quantification for the prediction of the possibility of pulmonary function impairment. METHODS AND MATERIALS A total of 157 patients (male to female ratio, 96:61; mean age, 63 ± 11 years) with DILD were enrolled in this study. All patients underwent volume thin-section CT in the supine position at full inspiration. During the same period, 23 people (male to female ratio, 10:13; mean age, 55 ± 13 years) with no history of DILD and with normal PFTs and CT findings were used as a control group. Quantitative indexes were obtained using a commercial CAD system (Brilliance Workspace v3.0; Philips Medical Systems). Quantitative indexes included total lung volume (TLV), mean lung attenuation, variation of lung attenuation, emphysema volume ( pixel > -950 HU), and interstitial lung disease volume (>-700 HU). Visual scores were measured semiquantitatively and included the overall extent of pulmonary parenchymal abnormality as well as the extent of consolidation, ground glass opacity, reticulation, and honeycomb opacities. Quantitative indexes were correlated with PFT and VSs using the Pearson correlation test. RESULTS Quantitative indexes, PFT results, and VSs differed significantly between the DILD group and the control group, except for emphysematous parameters (P < 0.05).Pulmonary function test results showed significant correlation with quantitative indexes in the DILD group. Functioning lung volume showed positive correlation with forced vital capacity and forced expiratory volume in 1 second (r = 0.80 and 0.73, P < 0.001). Total lung capacity showed positive correlation with TLV (r = 0.83, P < 0.001).Visual scores were correlated with the ratio of a specific volume to TLV (indicated as ®). Interstitial lung disease volume® showed positive correlation (r = 0.53, P < 0.001), and FLV® showed a negative correlation with the overall extent of ILD (r = -0.52, P < 0.001). variation of lung attenuation showed a positive correlation with honeycombing extent (r = 0.37, P < 0.001), and mean lung attenuation showed a positive correlation with reticulation extent (r = 0.42, P < 0.001). CONCLUSIONS Quantitative indexes measured by a commercial workstation showed good correlation not only with the extent of DILD estimated by visual inspection but also with PFT results. Quantitative indexes can be used as an objective tool for quantitative evaluation of disease extent and for follow-up of the progression or improvement of a DILD.

Invasive pulmonary aspergillosis is a serious infectious complication in immunocompromised patients. Recent reports indicate its favorable clinical outcomes by early diagnosis with chest computed tomography scan. We retrospectively analyzed our experiences with histopathological evaluation by open lung biopsy in 31 patients (32 cases) with hematologic malignancies, suspected of having invasive pulmonary aspergillosis clinically and radiologically. Although the initial computed tomography findings of all cases were highly indicative of invasive pulmonary aspergillosis by demonstrating nodules or masses with a halo sign (16 cases), segmental area of consolidation with ground-glass attenuation (7 cases), both nodules or masses with a halo sign and segmental area of consolidation with ground-glass attenuation (7 cases) and poorly defined centrilobular nodules (2 cases), we could histopathologically confirm invasive fungal infections only in 17 cases (53.1%) by open lung biopsy. There were 13 cases of invasive pulmonary aspergillosis, two cases of aspergilloma, and two cases of mucormycosis. No fungal hyphae were found in the other 15 cases: organizing pneumonia in seven cases, pulmonary hemorrhage in three cases, brochiolitis obliterans with organizing pneumonia in two cases, and CMV pneumonia, pulmonary tuberculosis, candida pneumonia in one case each, respectively. We could perform open lung biopsy without mortality and significant morbidity. In view of the low positive predictive value of chest computed tomography scan and the very low morbidity of open lung biopsy, this procedure is recommendable for the diagnosis of invasive pulmonary aspergillosis and determination of its treatment. Objectives : Since Mountain proposed the new staging system of non-small cell lung cancer in 1986, the indications for operation of NSCLC have been extended. However, operative mortality is from 3 to 6%. Therefore it is important to reduce unnecessary operation and to evaluate unresectability of tumor correctly, preoperatively. The purpose of this study is to find out the causes of unresectability in patients who were initially thought to be resectable preoperatively. Methods : By retrospective analysis, 64 patients out of 291 NSCLC patients who were undergone operation for curative resection in Seoul National University Hospital from Jan. of 1987 to Dec. of 1991, were found to be unresectable at operating room, were selected for this study. Out of 64 patients, 42 were evaluable. The analysis was focused on the change of pre- & post-operative staging and the causes of unresectability of tumors. Results : Among 42 patients with unresectable tumor who could be evaluated, preoperative CT finding showed resectable tumors in 55%(23 patients) and suspicious for unresectable tumors in 45%(19 patients). The causes of unresectability were technically unresectable T3 lesions in 7%(3 patients), T4 lesions in 62%(26 patients), N2 lesions in 17%(7 patients) and N3 lesions in 14%(6 patients). Conclusion : The major causes of unresectability of NSCLC were pulmonary artery invasions. It is suggested that careful evaluation of mediastinal structure, especially great vessels by additional imaging technique other than CT(like MRI) is indicated in selected NSCLC cases.

연구배경 : 폐 림프관평활근종증은 주로 가임연령의 여성에 발생하는 매우 드문 질환으로 치료에도 불구하고 점차 진행하여 호흡부전에 이르는 무서운 질형이다. 그러나 이 질환의 임상경과는 아직 정확하게 알려져 있지 않고 20여년전부터 항에스트로젠제재가 치료에 사용되고 있으나 그 효과에 대해서도 논란이 있는 실정이다. 저자들이 경험한 4례의 폐 림프관평활근종증 증례를 보고하고 극내 폐 림프관평활근종증의 임상적 고찰을 하여 폐 림프관평활근종증에 대한 이해를 돕고자 본 연구를 시행하였다. 방법 : 국내 림프관평활근종증 12례중 199년 2월 일본 교또에서 열린 림프관평활근조증 심포지움에서 국내 증례의 임상적, 방사선학적, 병리학적 소견에 대해 토의한 결과 최종진단에 이의가 제기된 2례를 제외한 10예의 임상소견, 방사선소견과 병리소견을 검토하였다. 결과 : 모든 환자는 여성이었으며 평균연령은 $33{\pm}7$ 세 였다. 가장 흔한 증상은 노작성 호흡곤란이었고 대부분에서 재발성 기흉의 병력이 관찰되었다. 폐기능검사상 전례에서 폐확산능의 감소가 있었다. 전예에서 고해상도 전산화단층촬영상 특정적인 낭이 전폐야에서 관찰되었다. 대부분 호르몬요법으로 증상의 호전을 보이지 않았다. 결론 : 재발성 기흉의 병력과 노작성 호흡곤란을 호소하는 가임기 여성에서는 폐 림프관평활근종증의 가능성을 반드시 염두에 두어야하고 의심되면 우선 고해상도 전산화단층촬영으로 특징적인 낭 유무를 확인하고 필요하면 개흉폐생검으로 확진해야 할 것으로 판단된다. 【Background: Lymphangioleiomyomatosis(LAM) is a very rare disease occurring in women of reproductive age and leading to progressive respiratory failure despite therapy. But the natural history of this disease is uncertain and although anti-estrogenic agents have been used for more than twenty years, it's efficacy is still in debate. This study was performed to enhance understanding of this fatal disease in Korea by examining clinical, radiological, and pathologic findings of all the previously reported cases of LAM on Korea along with four new cases of LAM whom we report in this paper. Method: Out of twlve cases of LAM previously unpublished and published in domestic papers, two cases whose diagnoses were considered doubtful after review of clinical, radiological, and pathologic findings at "Asian Congress on Lymphangioleiomyomatosis" at Kyoto, Japan in feburary of 1993 were excluded from this study. Six cases which were reported previously and four new cases of LAM whom we report in this paper were analysed for the clinical, radiological, and pathologic characteristics. Results: All ten patients were women with mean age of $33{\pm}7$ . The most common symptom was exertional dyspnea and most patients had history of pneumothoraces. Pulmonary function tests showed decreased diffusing capacity. on high resolution computed tomography(HRCT), all the cases had characteristic cysts. Most of the patients did not respond to hormonal therapy. Conclusion: In women of reproductive age, presenting with dyspnea who has a history of pneumothorax, LAM should always be considered as one of the diagnostic possibilities. If suspected, HRCT should be done to look for characteristic cysts and if needed, open lung biopsy should be done to confirm the diagnosis.】

Background: Bronchiectasis is characterized by chronic sputum production and complications such as hemoptysis and repeated infections. Even though some patients are cured by surgical resection of bronchiectatic lesions, most bronchiectasis patients can not be treated surgically because of multiple site involvement, and they are treated by medical measures such as postura1 drainage and antibiotics when indicated. Recently there have been some reports that low-dose longterm erythromycin treatment is effective on bronchiectsis, and it is well known that low-dose longterm erythromycin treatment is the treatment of choice in diffuse panbronchiolitis which is characterized by chronic sputum production and dyspnea. To evaluate the efficacy of erythromycin, we tried erythromycin on twenty five stable bronchiectasis patients for more than six months. Methods: We tried erythromycin 250 mg b.i.d. for more than 6 months. We checked respiratory symptoms, chest PA, spirometry, and side effects before treatment and after 3 and 6 months of treatment. Results: 1) 32% of the patients showed marked improvement of symptoms and PFT. 32% showed slight improvement of symptoms with little change of PFT, and 36% showed no change of symptoms and signs. 2) Analysis of the patients showing marked improvement revealed that most of them had diffuse bronchiectasis and paranasal sinus involvement. Conclusion: These results suggest that low-dose longterm erythromycin treatment can be tried on diffuse bronchiectasis patients with sinus involvement. And further studies will be followed on the mechanism of erythromycin in bronchiectasis.
