Background: The caudal epidural block is one of the most commonly used regional techniques for post-operative pain management in pediatric age group patients undergoing infraumbilical surgeries. Adjuvants use increase the duration of analgesia and decrease local anesthetic dose requirement, thereby decreasing the risk of toxicity. Aims and Objectives: The aim of the present study was to compare the prolongation of the duration of analgesia, hemodynamic parameters, and side effects if any, provided by the addition of dexmedetomidine (DEX) or dexamethasone as an adjuvant to levobupivacaine in the caudal block. Materials and Methods: A total of 60 patients with the American Society of Anesthesiologists grade I/II scheduled for elective infraumbilical surgeries were randomly allocated into two groups of 30 each. Group 1 received levobupivacaine 0.25% 1 mL/kg+DEX 1 mcg/kg and Group 2 received levobupivacaine 0.25% 1 mL/kg+dexamethasone 0.1 mg/kg for caudal block. The duration of analgesia, hemodynamic parameters, and adverse events during the post-operative period were noted. Results: There was no statistically significant difference in hemodynamic parameters between Group 1 and Group 2. Post-operative face, leg, activity, cry, consolability, (FLACC) pain scores were significantly lower in Group 1 when compared with Group 2. The mean duration of analgesia was prolonged in Group 1 at 824.23±53.53 min than in Group 2 with a mean duration of analgesia 480.50±31.66 min which was statistically significant (P0.05). Conclusion: The addition of DEX to levobupivacaine in caudal block significantly prolongs the duration of analgesia in post-operative period in comparison to the addition of dexamethasone with levobupivacaine. It also provides more hemodynamic stability during the intraoperative and post-operative period, lower FLACC pain scores, and is associated with minimal side effects.

DYRK1A, a ubiquitously expressed kinase, is linked to the dominant intellectual developmental disorder, microcephaly, and Down syndrome in humans. It regulates numerous cellular processes such as cell cycle, vesicle trafficking, and microtubule assembly. DYRK1A is a critical regulator of organ growth; however, how it regulates organ growth is not fully understood. Here, we show that the knockdown of DYRK1A in mammalian cells results in reduced cell size, which depends on mTORC1. Using proteomic approaches, we found that DYRK1A interacts with the tuberous sclerosis complex (TSC) proteins, namely TSC1 and TSC2, which negatively regulate mTORC1 activation. Furthermore, we show that DYRK1A phosphorylates TSC2 at T1462, a modification known to inhibit TSC activity and promote mTORC1 activity. We also found that the reduced cell growth upon knockdown of DYRK1A can be rescued by overexpression of RHEB, an activator of mTORC1. Our findings suggest that DYRK1A inhibits TSC complex activity through inhibitory phosphorylation on TSC2, thereby promoting mTORC1 activity. Furthermore, using the Drosophila neuromuscular junction as a model, we show that the mnb, the fly homologs of DYRK1A, is rescued by RHEB overexpression, suggesting a conserved role of DYRK1A in TORC1 regulation.

The extensive practice of DC-DC converter interfacing circuits to maintain DC link voltage in DC microgrid applications introduces unavoidable voltage regulation problems due to its nonlinearity. These unregulated voltages demean the performance and result in an unstable system. This brief proposes an internal model control (IMC) non-linear approach based robust proportional-integral-lead (PI-Lead) controller design for regulating a practical fifth-order DC-DC boost converter. The main innovation of this paper is the developed procedure for designing controller. The prominent feature of IMC filter coefficient is to fulfil the desired level of gain margin and phase margin to show the trade-off between performance and robustness. The proposed controller's performance is evaluated in the presence of uncertain disruptions and its effectiveness is verified by comparing it with the some conventional proportional-integral (PI) as well as with recently published IMC-proportional-integral-derivative (IMC-PID) and IMC-proportional integral derivative double derivative (IMC-PIDD2) controllers. The applicability of the proposed control methodology has been validated under varying supply and load current levels in presence of parametric uncertainty using a MATLAB/Simulink tool along with a low-cost microcontroller DSP F280379D based laboratory prototype.

Abstract Indoor air quality (IAQ) describes the air quality within buildings and other structures, particularly concerning how it affects the health and comfort of building inhabitants. Your likelihood of experiencing indoor health issues can be decreased by being aware of and controlling common indoor contaminants. In addition to maintaining indoor relative humidity at or below 65%, The American Society of Heating, Refrigerating, and Air-Conditioning Engineers (ASHRAE) recommends temperatures between 21 and 26 °C in the summer and 20–24 °C in the winter. The EPA advises using humidity levels between 30 and 60% to stop mold from growing. Silica gel, lithium chloride, and molecular sieves 5A are excellent for dehumidification due to their strong moisture adsorption capability and potential for regeneration. Choosing the right desiccant depends on factors like adsorption capacity, pore size, and chemical compatibility, which are crucial for dehumidification applications and environments. This study compares the performance of silica gel desiccant wheels with three additional composite materials for air dehumidification, which is unusual in this work because very few studies of these combinations have been provided. For all desiccant wheels, the various operating parameters of the needed dehumidification process have been investigated and compared. For adsorption and regeneration rate, the performance of three different composite desiccant wheels was examined and compared experimentally. This experimental evaluation reveals that the composite desiccant wheel (silica gel-lithium chloride-molecular sieve 5A) has a higher adsorption and regeneration rate than others. When a composite desiccant wheel made from silica gel-lithium chloride, silica gel-molecular sieve 5A, and silica gel-lithium chloride-molecular sieve 5A was compared to the performance of a silica gel desiccant wheel, the percentage improvements in adsorption rate were 34.99%, 43.58%, and 85.5%, and the percentage increases in regeneration rate were 5.65%, 7.08%, and 14%.

Abstract Background Ewing sarcoma (ES) is a highly aggressive malignant tumor most commonly affecting long bones. Extraskeletal Ewing sarcoma (EES) is a malignant tumor with aggressive behavior carrying bad prognosis with pancreas being an extremely rare primary site. We present a case of histopathologically proven EES of the pancreas in a young female who presented with abdominal pain. 18F-fluorodeoxyglucose positron emission tomography (18F FDG PET/CT) is a useful modality for detecting distant metastases in EES. It helps in diagnosis, localizing the primary, its extension, optimal treatment planning and evaluation of response to standard treatments available. Case presentation An 18-year-old female presented with complaints of progressive abdominal pain and distention since 6 weeks. Physical examination was suggestive of a solid large mass in the upper left abdomen and decreased breath sounds with dullness in the left lower lung fields. On Contrast enhanced computed tomography (CECT) imaging, a large heterogeneously enhancing mass was seen arising from pancreas along with retroperitoneal lymphadenopathy. A moderate sized left sided pleural effusion with atelectasis of lower lobe of left lung was also noted. Histopathological analysis was suggestive of pancreatic ES following which the patient underwent five cycles of chemotherapy. Following this, she underwent 18F FDG PET/CT which showed hypermetabolic large mass arising from body and tail of pancreas with areas of internal necrosis along with left adrenal metastasis, retroperitoneal lymphadenopathy, a massive left pleural effusion and compressive atelectasis of left lower lobe. The patient expired within a week following PET/CT. Conclusions EES most often presents in the late stage of the disease with vague symptoms. Timely diagnosis and initiation of treatment is of utmost importance considering the aggressiveness of the tumor. Establishing a diagnosis of Ewing sarcoma is especially difficult when the mass is arising from the pancreas. Imaging, histopathology and immunohistochemistry (IHC) play a key role in accurate diagnosis of such masses. 18F FDG PET/CT can be useful for detecting local and distant spread, operability, treatment planning and evaluation of response to chemotherapy.

Abstract Background Incidental gallbladder carcinoma (IGBC) is identified after cholecystectomy being performed for a presumed to be benign disease, and histopathology turns out as malignant disease. For optimal management planning, it is crucial to know the actual disease status. 18F-fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT) provides local, regional as well as distant disease, i.e., restaging and identifying true burden of disease for optimal treatment planning. The aim of this study was to restage the IGBC patients on 18F FDG PET/CT and find out any change in treatment plan. Methods This retrospective descriptive study was performed between November 2021 and February 2023. All PET/CT scans were analyzed which came for restaging in IGBC. Results PET/CT was performed at a median time of 9 weeks (range 6–12 weeks) from the date of surgery. This study included 17 patients (6 males and 11 females), with a median age of 55 years (range 38–76 years). From total of 17 PET/CT scans, 10 (58.8%) patients were positive and 7 (41.1%) patients were negative on PET/CT. Among the PET/CT positive patients, disease pattern was seen in the form of local/residual disease/liver infiltration, regional lymph nodes and distant metastases. Among the 17 patients, treatment plan in 5 patients (having PET/CT negative) was changed from surgical intervention to no treatment, and in 2 patients (having PET/CT positive), treatment plan was changed to chemotherapy, i.e., total 7 (5 + 2, 41% of total 17 patients) patients’ treatment plans were changed. By reducing the number of patients undergoing re-resection, we can say that it reduces the burden on already overburdened health infrastructure, especially in developing countries like India where incident is high. Conclusions PET/CT provides the actual stage of IGBC. It changes treatment plan and reduces the number of patients undergoing re-resection. It also decreases burden on overburdened health infrastructure.

Background: Polycystic ovary syndrome’s (PCOS) main feature is hyperandrogenism, which is linked to a higher risk of metabolic disorders. Gene expression analyses in adipose tissue and skeletal muscle reveal dysregulated metabolic pathways in women with PCOS, but these differences do not necessarily lead to changes in protein levels and biological function. Methods: To advance our understanding of the molecular alterations in PCOS, we performed global proteomic and phosphorylation site analysis using tandem mass spectrometry, and analyzed gene expression and methylation. Adipose tissue and skeletal muscle were collected at baseline from 10 women with and without PCOS, and in women with PCOS after 5 weeks of treatment with electrical stimulation. Results: Perilipin-1, a protein that typically coats the surface of lipid droplets in adipocytes, was increased whereas proteins involved in muscle contraction and type I muscle fiber function were downregulated in PCOS muscle. Proteins in the thick and thin filaments had many altered phosphorylation sites, indicating differences in protein activity and function. A mouse model was used to corroborate that androgen exposure leads to a shift in muscle fiber type in controls but not in skeletal muscle-specific androgen receptor knockout mice. The upregulated proteins in muscle post treatment were enriched in pathways involved in extracellular matrix organization and wound healing, which may reflect a protective adaptation to repeated contractions and tissue damage due to needling. A similar, albeit less pronounced, upregulation in extracellular matrix organization pathways was also seen in adipose tissue. Conclusions: Our results suggest that hyperandrogenic women with PCOS have higher levels of extra-myocellular lipids and fewer oxidative insulin-sensitive type I muscle fibers. These could be key factors leading to insulin resistance in PCOS muscle while electric stimulation-induced tissue remodeling may be protective. The Bernoulli equation is useful to assess the motility and recovery rate with respect to time in order to measure the COVID-19 outbreak. The homotopy perturbation method was applied in the current article to compute the Bernoulli equation. For the existence and uniqueness of solutions, we also used the Caputo–Fabrizio Integral and differential operators. Additionally, we conducted a corresponding investigation for derivatives of integer and fractional orders on the estimated motility and recovery rate.

Abstract Background Ewing sarcoma and peripheral primary neuroendocrine tumors are aggressive neoplasms which consist of small, round, blue cells of neuroectodermal origin. They usually arise from the skeleton and consist of genetic mutations EWSR1 in chromosome 22 and FL1 gene on chromosome 11. Extraskeletal Ewing sarcomas (EES) are rare entities with most common sites of EES being extremities, head and neck region and retroperitoneum. Posterior mediastinal Ewing sarcoma is rare. For its evaluation, 18F-fluorodeoxyglucose positron emission tomography (18F FDG PET/CT) plays significant role in staging, management planning and prognostication. Case presentation We describe a rare case of EES of posterior mediastinum in a 20-year-old boy who presented with signs of upper motor neuron lesion below D10 level. Contrast-enhanced magnetic resonance imaging (CEMRI) showed a heterogeneously enhancing posterior mediastinal mass in pre- and paravertebral region with intraspinal extension in D2-D4 levels. Fluorodeoxyglucose PET/CT showed a metabolically active mass occupying the superior, middle and posterior mediastinum on the left, displacing the trachea and esophagus toward the right side and causing complete collapse of the left lung. Posteriorly the mass was seen destroying the D2-D5 vertebrae with intraspinal extension at D2-D4 level. Metabolically active metastatic disease was seen in pleura, skull, D12 vertebra, right iliac bone and bilateral proximal femorae. Biopsy obtained from lung and adjacent pleura showed features of a round cell tumor positive for NKX 2.2, weak positive for FLI 1 and negative for PAN CK, LCA, Vimentin and TLE-1, suggestive of Ewing sarcoma. Based on these investigations, a diagnosis of EES of posterior mediastinum was made. Conclusion Extraskeletal Ewing sarcoma of posterior mediastinum is a rare and aggressive entity. Management of metastatic EES comprises radiotherapy and systemic chemotherapy which reduces tumor burden and micrometastasis. However, response to treatment in metastatic EES is poorer than in localized disease with overall 5-year survival rates of less than 30%. Fluorodeoxyglucose PET/CT can be a useful tool to accurately detect the extent of local disease in the presence of atelectasic lung for radiotherapy planning as well as evaluating response to therapy.

Abstract Background Ewing’s sarcoma is categorized as small round cell aggressive tumor which commonly occurs in bone and soft tissue. Primary renal Ewing’s sarcoma is an extremely rare entity and accounts for less than 1% of renal masses. It is characterized by a very aggressive course, with very few reported cases in the literature. Primary renal Ewing’s sarcoma is indistinguishable from renal cell carcinoma on anatomical imaging and therefore need histopathological correlation for confirmation. The positron emission tomography (PET) fused with computed tomography (CT) helps in the evaluation of disease, for staging and metastatic workup. Case presentation We present a case of 48-year-old lady who presented to urology department with difficulty in passing urine for 5 months associated with pain in the right flank region. She had an episode of haematuria 5 months back. Contrast enhanced CT showed a large lobulated heterogeneously enhancing right retroperitoneal mass in renal fossa. Histopathological examination and immunohistochemistry were suggestive of Ewing’s sarcoma. The PET/CT revealed FDG avid right renal mass with lymph nodal and skeletal metastasis. Conclusions Ewing’s sarcoma being aggressive tumour usually present with bone, liver and lung metastasis along with lymph nodal metastasis at time of diagnosis. The 18 F FDG PET/CT helps in diagnosis, staging, appropriate treatment planning, restaging, response evaluation and prognostication of Ewing’s sarcoma.

The study of atomic spectroscopy and collision processes in a dense plasma environment has gained a considerable interest in the past few years due to its several applications in various branches of physics. The multiconfiguration Dirac-Fock (MCDF) method and relativistic configuration interaction (RCI) technique incorporating the uniform electron gas model (UEGM) and analytical plasma screening (APS) potentials have been employed for characterizing the interactions among the charged particles in plasma. The bound and continuum state wavefunctions are determined using the aforementioned potentials within a relativistic Dirac-Coulomb atomic structure framework. The present approach is applied for the calculation of electronic structures, radiative properties, electron impact excitation cross sections and photoionization cross sections of many electron systems confined in a plasma environment. The present study not only extends our knowledge of the plasma-screening effect but also opens the door for the modelling and diagnostics of astrophysical and laboratory plasmas.
