OBJECTIVE: To determine the feasibility, efficacy, and safety of early cold stored platelet transfusion compared with standard care resuscitation in patients with hemorrhagic shock. BACKGROUND: Data demonstrating the safety and efficacy of early cold stored platelet transfusion are lacking following severe injury. METHODS: A phase 2, multicenter, randomized, open label, clinical trial was performed at 5 US trauma centers. Injured patients at risk of large volume blood transfusion and the need for hemorrhage control procedures were enrolled and randomized. The intervention was the early transfusion of a single apheresis cold stored platelet unit, stored for up to 14 days versus standard care resuscitation. The primary outcome was feasibility and the principal clinical outcome for efficacy and safety was 24-hour mortality. RESULTS: Mortality at 24 hours was 5.9% in patients who were randomized to early cold stored platelet transfusion compared with 10.2% in the standard care arm (difference, -4.3%; 95% CI, -12.8% to 3.5%; P =0.26). No significant differences were found for any of the prespecified ancillary outcomes. Rates of arterial and/or venous thromboembolism and adverse events did not differ across treatment groups. CONCLUSIONS AND RELEVANCE: In severely injured patients, early cold stored platelet transfusion is feasible, safe and did not result in a significant lower rate of 24-hour mortality. Early cold stored platelet transfusion did not result in a higher incidence of arterial and/or venous thrombotic complications or adverse events. The storage age of the cold stored platelet product was not associated with significant outcome differences. TRIAL REGISTRATION: ClinicalTrials.gov identifier: NCT04667468.

The ideal method of operative fixation for unstable distal clavicle fractures remains controversial, with particular emphasis on the need to restore the normal coracoclavicular distance. Our preferred method of fixation includes open reduction and internal fixation with a distal clavicle locking plate in combination with a polydioxanone suture wrapped around the plate and coracoid to restore the coracoclavicular distance and offload the bony repair. The purpose of this study is to report clinical, radiographic, and functional outcomes in patients with unstable distal clavicle fractures treated with this method of fixation.

The authors use data on high school students and teachers from Washington state to connect the observable characteristics and preparation of career and technical education (CTE) teachers to various non-test outcomes (absences, disciplinary incidents, grades, grade progression, and on-time graduation) of students with and without disabilities in their classrooms. The authors find that students participating in CTE tend to have better non-test outcomes when they are assigned to a CTE teacher from the state's Business and Industry (B&I) pathway--designed for CTE teachers with 3 years of industry experience but no formal teacher preparation--relative to being assigned to a traditionally prepared CTE teacher. These relationships do not significantly differ for students with and without disabilities, despite survey data suggesting that CTE teachers from the B&I pathway receive little formal training in special education. These results suggest that content knowledge and experience may matter more than traditional preparation for CTE teacher effectiveness.

Here we introduce a novel algorithm designed to isolate individual respiratory cycles on a thoracic respiratory inductance plethysmography signal using the non-invasive signal of the respiratory inductance plethysmography belts.Purpose: The algorithm locates breaths using signal processing and statistical methods on the thoracic respiratory inductance plethysmography belt and enables the analysis of sleep data on an individual breath level.Patients and Methods: The algorithm was evaluated against a cohort of 31 participants, both healthy and diagnosed with obstructive sleep apnea. The dataset consisted of 13 female and 18 male participants between the ages of 20 and 69. The algorithm was evaluated on 7.3 hours of hand-annotated data from the cohort, or 8782 individual breaths in total. The algorithm was specifically evaluated on a dataset containing many sleep-disordered breathing events to confirm that it did not suffer in terms of accuracy when detecting breaths in the presence of sleep-disordered breathing. The algorithm was also evaluated across many participants, and we found that its accuracy was consistent across people. Source code for the algorithm was made public via an open-source Python library.Results: The proposed algorithm achieved an estimated 94% accuracy when detecting breaths in respiratory signals while producing false positives that amount to only 5% of the total number of detections. The accuracy was not affected by the presence of respiratory related events, such as obstructive apneas or snoring.Conclusion: This work presents an automatic respiratory cycle algorithm suitable for use as an analytical tool for research based on individual breaths in sleep recordings that include respiratory inductance plethysmography.Keywords: respiratory analysis, breath detection algorithm, sleep analysis, breath segmentation, respiratory cycle isolation

Abstract The KidGen Collaborative's Policy Implementation Workshop 2023 celebrated the 10th anniversary of Australia's first kidney genetics clinic in Brisbane. This event marked the establishment of a national network now comprising 19 kidney genetics clinics across Australia, all dedicated to providing equitable access to genomic testing for families affected by genetic kidney diseases. The workshop reflected on past progress and outlined future objectives for kidney genetics in Australia, recognising the collaborative efforts of clinical teams, researchers, and patients. Key insights from the workshop are documented in the proceedings.

Abstract Background Patients with Functional Somatic Symptoms (FSS) are frequently encountered within healthcare settings such as Emergency Departments (ED). There is limited research regarding characterisation and frequency of FSS within frequent presenters to ED and no previous Australian evidence. This study aims to fill this gap. Methods A retrospective, single-centre study of frequent ED presenters over a 6-month period was undertaken. Patients with > 3 re-presentations/month were reviewed for the presence of FSS using Stephenson and Price’s (Stephenson DT, Price JR. Medically unexplained physical symptoms in emergency medicine. Emerg Med J. 2006;23(8):595.) categorisation of FSS. Patients were divided into three groups – FSS, possible FSS (pos-FSS) and non-FSS. The characteristics of these groups were compared using descriptive statistics (chi-square tests, Welch’s ANOVA). Person-time at risk during the 6-month study period was estimated for patients in each group and incidence of ED presentation for each group was then calculated. Psychological distress indicators for ED presenters with FSS, as noted by the treating clinician, were also analysed. Results 11% (71/638) of frequent ED presenters were categorised as having FSS and 72% (458/638) as having possible FSS (Pos-FSS). Abstract Hudson Bay has warmed over 1 °C in the last 30 years. Coincident with this warming, seasonal patterns have shifted, with the spring sea ice melting earlier and the fall freeze-up occurring later, leading to a month longer of ice-free conditions. This extended ice-free period presents a significant challenge for polar bears, as it restricts their hunting opportunities for seals and their ability to accumulate the necessary body weight for successful reproduction. Drawing on the latest insights from CMIP6, our updated projections of the ice-free period indicate a more spatially detailed and alarming outlook for polar bear survival. Limiting global warming to 2 °C above pre-industrial levels may prevent the ice-free period from exceeding 183 days in both western and southern Hudson Bay, providing some optimism for adult polar bear survival. However, with longer ice-free periods already substantially impacting recruitment, extirpation for polar bears in this region may already be inevitable.

Background and objective: Radical prostatectomy (RP) is an established treatment for localised prostate cancer that can have a significant impact on urinary and sexual function, with recovery over time. Our aim was to describe functional recovery in the first year after RP, reporting descriptive outcomes alongside validated patient-reported outcome measure scores (Expanded Prostate Cancer Index Composite, EPIC-26). Methods: Men undergoing RP between September 2015 and November 2019 completed EPIC-26 at baseline and 1, 3, 6, and 12 mo. Key findings and limitations: Overall, 2030 men consented to participation, underwent RP, and completed EPIC-26. At baseline, 97% were pad-free (1928/1996; 95% confidence interval [CI] 96–97%) and 77% were leak-free and pad-free (1529/1996; 95% CI 75–78), with a median EPIC-26 incontinence domain score of 100 (interquartile range [IQR] 86–100). At 12 mo, 65% were pad-free (904/1388; 95% CI 63–68%) and 42% were leak-free and pad-free (583/1388; 95% CI 39–45%), with a median EPIC-26 score of 76 (IQR 61–100). While one in three men reported wearing a pad at 12 mo, fewer than one in ten men needed more than 1 pad/d. At baseline, 1.9% reported a “moderate or big problem” with urine leakage, which increased to 9.7% at 12 mo. At baseline, the median sexual domain score among 1880 men was 74 (IQR 43–92) and 52% had erections sufficient for intercourse without medication (975/1880; 95% CI 50–54%). Among these 975 men, 630 responded at 12 mo, of whom 17% reported sufficient erections for intercourse (105/630; 95% CI 14–20%), without medication in 6% (37/630; 95% CI 4–8%) and needing medication in 11% (68/630; 95% CI 9–13%); the median EPIC-26 domain score was 26 (IQR 13–57). Conclusions and clinical implications: Reporting of functional outcomes after RP in terms of easily understood concepts such as pad-free and leak-free status, and erections with and with medication, alongside the classical report using EPIC-26 domain scores, increases the understanding of RP recovery patterns over the first year. Patient summary: At 12 months after surgery for prostate cancer, one in ten men reported a moderate or big problem with urine leakage and one in five men reported sufficient erections.

Abstract Despite increasing awareness of genetic kidney disease prevalence, there is limited population-level information about long term outcomes of people with genetic kidney disease receiving kidney replacement therapy. This analysis included people who commenced kidney replacement therapy between 1989 and 2020 as recorded in the Australian and New Zealand Dialysis and Transplant registry. Genetic kidney diseases were subclassified as majority and minority monogenic. Non-genetic kidney diseases were included as the comparator group. Primary outcome measures were 10-year mortality and 10-year graft failure. Cox proportional hazard regression were used to calculate unadjusted and adjusted hazard ratios (AHRs) for primary outcomes. There were 59,231 people in the dialysis subgroup and 21,860 people in the transplant subgroup. People on dialysis with genetic kidney diseases had reduced 10-year mortality risk (majority monogenic AHR: 0.70, 95% CI 0.66–0.76; minority monogenic AHR 0.86, 95% CI 0.80–0.92). This reduced 10-year mortality risk continued after kidney transplantation (majority monogenic AHR: 0.82, 95% CI 0.71–0.93; minority monogenic AHR 0.80, 95% CI 0.68–0.95). Majority monogenic genetic kidney diseases were associated with reduced 10-year graft failure compared to minority monogenic genetic kidney diseases and other kidney diseases (majority monogenic AHR 0.69, 95% CI 0.59–0.79). This binational registry analysis identified that people with genetic kidney disease have different mortality and graft failure risks compared to people with other kidney diseases.

One-minute papers allow students to process what they learned during class and improve student performance. However, this activity can become monotonous and takes significantly longer than the name implies. The research described here tests the effectiveness of a briefer, more flexible version of this technique to increase the perceived relevance of the material and, in turn, performance. Students were randomly assigned to either self-generate a reflective response or answer a content-specific question generated by the instructor at the end of class. Students positively evaluated the brief reflective response activity. Moreover, answering the self-generated reflective responses was associated with greater perceived relevancy of course material to students' daily lives compared to answering the content-specific questions. Perceived relevancy, in turn, was associated with higher examination grades. Thus, brief reflective responses are a useful tool to increase meaningful connections to course material which enhances examination performance.

In this paper, I review literature that can build foundation for integrating allocative and productive efficiency research of transit provision in the United States. Both frameworks concern transit performance but use distinct methods and talk past each other. Productive efficiency research uses economic cost models to measure the economic structure of transit service — economies of scale, economies of scope, etc. — to inform aggregate system size and output. This research shows that bus transit reaches constant scale economies between 300 and 500 vehicles but says little about when and where to allot these resources. Allocative efficiency research uses cost allocation models that disaggregate transit systems into parts — generally spatial areas or times of service — to measure service output and pricing variability. This literature suggests that transit agencies underprice or over-allocate resources to suburban areas and peak travel times. Yet, allocative efficiency scholarship ignores aggregate economic factors. This isolation in research approach and scale of focus leaves unclear the union between how much transit to produce, and when and where to produce it. Both bodies of literature can benefit from representation of transit modes other than bus transit and methodological integration. Allocative efficiency research can benefit from the use of more granular data and spatiotemporally focused studies.

Background. Transplantation of human-induced pluripotent stem cell (hiPSC)-derived islet organoids is a promising cell replacement therapy for type 1 diabetes (T1D). It is important to improve the efficacy of islet organoids transplantation by identifying new transplantation sites with high vascularization and sufficient accommodation to support graft survival with a high capacity for oxygen delivery. Methods. A human-induced pluripotent stem cell line (hiPSCs-L1) was generated constitutively expressing luciferase. Luciferase-expressing hiPSCs were differentiated into islet organoids. The islet organoids were transplanted into the scapular brown adipose tissue (BAT) of nonobese diabetic/severe combined immunodeficiency disease (NOD/SCID) mice as the BAT group and under the left kidney capsule (KC) of NOD/SCID mice as a control group, respectively. Bioluminescence imaging (BLI) of the organoid grafts was performed on days 1, 7, 14, 28, 35, 42, 49, 56, and 63 posttransplantation. Results. BLI signals were detected in all recipients, including both the BAT and control groups. The BLI signal gradually decreased in both BAT and KC groups. However, the graft BLI signal intensity under the left KC decreased substantially faster than that of the BAT. Furthermore, our data show that islet organoids transplanted into streptozotocin-induced diabetic mice restored normoglycemia. Positron emission tomography/MRI verified that the islet organoids were transplanted at the intended location in these diabetic mice. Immunofluorescence staining revealed the presence of functional organoid grafts, as confirmed by insulin and glucagon staining. Conclusions. Our results demonstrate that BAT is a potentially desirable site for islet organoid transplantation for T1D therapy.
