Abstract

The number of children requiring prolonged mechanical ventilation (PMV) has increased with the advancement of medical care. We aimed to estimate the prevalence of PMV worldwide, document demographic and clinical characteristics of children requiring PMV in paediatric intensive care units (PICUs), and to understand variation in clinical practice and health-care burden.

Abstract

Background Sepsis in children is characterized by a dysregulated host response to infection and life-threatening organ dysfunction. Pediatric acute respiratory distress syndrome (PARDS) is a severe complication of sepsis. Several known risk factors of PARDS are immunodeficiency, obesity, source of infection, and genetic factors. Objective To describe the potential risk factors of PARDS in septic children at Haji Adam Malik General Hospital, Medan, North Sumatera. Methods This descriptive study using eligible medical record data from sepsis patients aged 1 month - 18 years was conducted at Haji Adam Malik General Hospital, Medan, North Sumatera, in 2020-2022. A total sampling method was used for subject selection. Patients' demographic and clinical characteristics, mechanical ventilation modes and parameters, laboratory results, and outcomes were collected from medical records. Patients were classified as having sepsis with PARDS if Pediatric Acute Lung Injury Consensus Conference (PALICC) criteria of ARDS was met. Results Of 112 pediatric sepsis patients, 33.9% developed PARDS. The mortality rate for PARDS was 73.7%. Of the patients with PARDS, 57.9% had immunodeficiency, 60.5% patients had an intrapulmonary infection, 39.5% had an extrapulmonary infection, and 23.7% of patients had a suspected genetic disorder. In addition, 39.5% had good nutritional status, followed by 21.1% with obese. Conclusion The majority of patients with PARDS have intrapulmonary infection and immunodeficiency. Abstract

Hyperuricemia is a state of uric acid in the body that is higher than the body's tolerance limit. The formation of uric acid will be accompanied by the formation of Reactive Oxygen Species (ROS) by the changes of Xanthine Dehydrogenase to Xanthine Oxide with the help of NAD+ and Oxygen, which can trigger oxidative stress, causing cell damage and apoptotic cell, one of which is through lipid peroxidation followed by the release of Malondialdehyde (MDA) into the extracellular space. This study aims to analyze the effect of giving melinjo peel ethanol extract (Gnetum gnemon) on serum MDA levels in hyperuricemic Wistar strain rats. Rats were grouped into 4 treatment groups, namely K0 (without treatment), Kneg (hyperuricemia and CMC), Kpos (hyperuricemia and allopurinol), and P (hyperuricemia and ethanol extract of melinjo peel). Manufacture a hyperuricemic rat model by giving a high-fructose diet and then measuring uric acid levels to ensure that the rat is already in hyperuricemia. The high-fructose diet was continued until the rats were executed. Serum MDA was measured using the Thiobarbituric acid (TBA) method. The results showed differences in MDA levels between groups of hyperuricemic rats. Based on the examination of lower MDA levels after administration of melinjo peel ethanol extract compared to the CMC group and the allopurinol group, statistically significant differences were obtained (p = 0.001). Based on the analysis results, it was concluded that the ethanolic extract of melinjo peel (Gnetum gnemon) could reduce serum MDA levels in hyperuricemic Wistar rats.

8. Development and validation of a clinical predictive model for severe and critical pediatric COVID-19 infection.

Abstract

Introduction: Children infected with COVID-19 are susceptible to severe manifestations. We aimed to develop and validate a predictive model for severe/ critical pediatric COVID-19 infection utilizing routinely available hospital level data to ascertain the likelihood of developing severe manifestations. Methods: The predictive model was based on an analysis of registry data from COVID-19 positive patients admitted to five tertiary pediatric hospitals across Asia [Singapore, Malaysia, Indonesia (two centers) and Pakistan]. Independent predictors of severe/critical COVID-19 infection were determined using multivariable logistic regression. A training cohort (n = 802, 70%) was used to develop the prediction model which was then validated in a test cohort (n = 345, 30%). The discriminative ability and performance of this model was assessed by calculating the Area Under the Curve (AUC) and 95% confidence interval (CI) from final Receiver Operating Characteristics Curve (ROC). Results: A total of 1147 patients were included in this analysis. In the multivariable model, infant age group, presence of comorbidities, fever, vomiting, seizures and higher absolute neutrophil count were associated with an increased risk of developing severe/critical COVID-19 infection. The presence of coryza at presentation, higher hemoglobin and platelet count were associated with a decreased risk of severe/critical COVID-19 infection. The AUC (95%CI) generated for this model from the training and validation cohort were 0.96 (0.94, 0.98) and 0.92 (0.86, 0.97), respectively. Conclusion: This predictive model using clinical history and commonly used laboratory values was valuable in estimating the risk of developing a severe/critical COVID-19 infection in hospitalized children.
