1. Comparison of Thessaly Test with Joint Line Tenderness and McMurray Test in the Diagnosis of Meniscal Tears

Abstract

INTRODUCTION: Meniscus injuries are the most frequent problem of the knee. The aim of this study was to investigate the accuracy of the Thessaly test and comparing it with those of McMurray and Joint-line tenderness tests for diagnosing meniscal tears. MATERIALS AND METHODS: This study was designed as a prospective observational one done in an outpatient clinic at a university hospital. 106 patients with knee pain and 82 agematched control were included during study period (from February 2014 to January 2015). Each patient was clinically examined with McMurray, Thessaly, and joint line tenderness tests. Then, the findings were matched by MRI and arthroscopic findings. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy were calculated as main outcomes. RESULTS: Based on MRI, Thessaly was the most sensitive for medial meniscus tears (56.2%), while McMurray and jointline tenderness were more specific (89.1% and 88.0%, respectively). For lateral meniscus tears, McMurray was the most sensitive (56.2%) and all were specific (McMurray 89.6%, Thessaly 88.4%, joint-line tenderness 90.2%). With arthroscopy, Thessaly was the most sensitive for medial meniscus (76.6%), while McMurray and joint-line tenderness were more specific (81.0%, and 81.0%). Abstract

Cogan's syndrome is a chronic inflammatory disorder of unknown cause affecting mostly young adults. Two main observation of the disease are bilateral interstitial keratitis and vestibuloauditory dysfunction. Association between Cogan's syndrome and systemic vasculitis as well as aortitis are exist. The diagnosis of the disease is based upon the presence of inflammatory eye disease and vestibuloauditory dysfunction . In this article, the classic Cogan's syndrome has been reported in a 47-year-old woman. Two months prior to admission, the patient had been suffering from headache, vertigo, nausea, vomiting, right leg claudication, musculoskeletal pains, bilateral hearing loss and blindness. Ophthalmologic examination revealed that visual acuity was 0.1 bilaterally and on slit lamp examination, there was a conjunctival hyperemia, bilateral cataract and interstitial keratitis. Pure tone audiogram (PTA) and auditory brain stem response (ABR) showed bilateral sensorineural hearing loss. The patient was initially treated with pulse intravenous methylprednisolone and was followed by oral prednisolone and cyclophosphamide, which in follow-up showed partial improvement.

Abstract

Background/Objective:Enlarged lateral ventricle (LV) volume and decreased volume in the corpus callosum (CC) are hallmarks of schizophrenia (SZ). We previously showed an inverse correlation between LV and CC volumes in SZ, with global functioning decreasing with increased LV volume. This study investigates the relationship between LV volume, CC abnormalities, and the microRNA MIR137 and its regulated genes in SZ, because of MIR137’s essential role in neurodevelopment. Methods:Participants were 1224 SZ probands and 1466 unaffected controls from the GENUS Consortium. Brain MRI scans, genotype, and clinical data were harmonized across cohorts and employed in the analyses. Results:Increased LV volumes and decreased CC central, mid-anterior, and mid-posterior volumes were observed in SZ probands. The MIR137-regulated ephrin pathway was significantly associated with CC:LV ratio, explaining a significant proportion (3.42 %) of CC:LV variance, and more than for LV and CC separately. Other pathways explained variance in either CC or LV, but not both. CC:LV ratio was also positively correlated with Global Assessment of Functioning, supporting previous subsample findings. SNP-based heritability estimates were higher for CC central:LV ratio (0.79) compared to CC or LV separately.Discussion:Our results indicate that the CC:LV ratio is highly heritable, influenced in part by variation in the MIR137-regulated ephrin pathway. Findings suggest that the CC:LV ratio may be a risk indicator in SZ that correlates with global functioning.

Abstract

Cryptocurrencies have attracted significant awareness up to now, and Bitcoin is the most remarkable one. As they have enormous instability in their price, predicting their uctuations is a challenging task. Although several research used traditional statistical and economic methods to discover the driving variables of cryptocurrency prices, progress in developing prediction models for decision-making tools in investing techniques is still in its early stages. Many different cryptocurrency price prediction methods cover purposes, such as forecasting a one-step approach that can be done through time series analysis, neural networks, and machine learning algorithms. However, realizing the trend of a coin in the long run is required. In this paper, we aimed to investigate and forecast Bitcoin prices based on three other well-known coins (i.e., Ethereum, Zcash, and Litecoin), assuming little information about Bitcoin prices using machine learning algorithms. Moreover, we proposed a new method to predict Bitcoin's price considering different cryptocurrencies prices. The results demonstrated that Zcash had the best performance in forecasting Bitcoin price without information on Bitcoin price uctuations among other cryptocurrencies. [ABSTRACT FROM AUTHOR]

18. Measuring atrazine degradation and PRZM-2 testing

Abstract

This study investigated spatial and temporal variation of atrazine concentration and its transportation in the Daneshkadeh soil series (fine, mixed, mesic, Calcixerollic Xerochrepts) at the experimental fields of the Agricultural College, Shiraz University, Iran. It also assessed the risk of soil and water contamination due to applying atrazine, and evaluated the PRZM-2 model for the simulation of atrazine concentration. The experiment had 3 plots of size 209 [m.sup.2] (19 by 11 m). Irrigation water was applied according to corn evapotranspiration by a solid-set sprinkler system. Seven weeks after planting of maize (Zea mays L.) in early summer, atrazine (2-chloro-4-ethylamino-6-isopropylamins-s-triazine) was applied at a rate of 3.5 kg a.i./ha to each plot. During the growing season, soil samples were collected from each plot 7 times to 1 m soil depth at 0.10-m increments. The PRZM-2 model was applied using our field data for atrazine transport simulation. The observed data showed temporal reduction of atrazine concentrations in the soil profile. The maximum depth of atrazine traced was about 0.5 m below the soil surface. Statistical parameters ME, RMSE, EF (model efficiency), and CRM (coefficient of residual mass) were obtained to compare predicted and observed soil residue concentrations, and were 2.78 (mg/kg soil), 12.73, 0.49, and 0.25, respectively. The simulation results were in close agreements with the observed data, especially 1 month after application due to the small effect of ambient and soil temperature on atrazine transformation. This paper analyses Thomas Hardy’s Jude the Obscure in the light of Foucault’s notion of power relations and normalization. Foucault holds that there is a dynamically productive power in every society that imposes its own ideology on people. Through different strategies and procedures, mainly normalization, he insists, power tries to produce docile subjects that can be best controlled. However, since the existence of power presupposes the existence of resistance, he believes, subjects can have their own alternatives if they try their hand at acquiring an ethical self by practicing the strategies of ‘care of the self.’ Relying on such perspective, the present article tries to study Jude, the semi-protagonist of Thomas Hardy’s novel, and examine the way he is capable of resisting normalization and the extent he is successful in achieving a new self. The findings show that Hardy’s rendering of society is not completely in line with Foucault’s notion of power relations because Jude is incapable of effectively resisting power relations working in his society.Keywords: Foucault, power, normalization, Hardy, Jude

Abstract

This study tries to analyze the Terror and Ambivalence of the Human Soul in the selected plays of Eugene O’Neill which does not seem to have received a significant attention by the researchers. O’Neill constantly looked into the mysterious, terrifying Gorgon-faces of reality with a subjective and artistic passion. In his drama we have life coming full circle as it traces the double pattern of experience passing into art, and art flowing back into experience again. He was always searching for the missing elements in life with a restless curiosity and an uneasy consciousness of the penultimate quality of all human discoveries. He swiftly moved from one horizon to another, continually looking beyond thehorizon for a clue to the essence. If as a playwright he dramatized with a fascinating variety and ingenuity, the vision of the human torment, as a man in search of his soul, he also projected the torment of the vision. It is perhaps natural that a cycle of achievement and reaction in the world of criticism. Cargill (1962, p. 2) commented:The plays of O’Neill, it seems touch something fundamental in those who expose themselves to their effect. They reach down to frightening depths; they step on private, social, religious, philosophical, aesthetic toes; they either evoke immoderate enthusiasm or provoke immoderate anger.

Abstract

In spite of much research on Miller's The Death of a Salesman, no significant studies have been carried out on the aspect of ‘search for self’. This study re-justifies the need for a contingent model of the quest for identity, drawing ideas from the multiple manifestations of the threatening of self. It emphasizes the minimization of philosophical treatment and theorizing on self, on the grounds that the focus is on how Miller has dramatized the search for self among the different characters of the same family. The quest for self is expressed through a nostalgic yearning for the past, which has been lost due to various reasons. The process of search for self is spread over two generations, which enhances the effectiveness of Miller in depicting the struggle between the self of an individual and the public self the individual is compelled to put on as a mask. The dramatic strength of the play lies in the fact that it has interwoven the crisis of self in the life of the individuals and the broader changes taking place in the American society. Abstract

Background and purpose: Rational use of drugs remains a problem in Iran. Polypharmacy, overuse of antibiotic, misuse and overuse of injections, short consulting time and poor patient compliance are common patterns of irrational use of drugs in Eastern Azarbydjan - Iran. Concerning the promotion of rational use of drugs, this study aimed the effect of educational intervention as interactive group discussion on prescribing behavior of Tabriz Northwest physicians.Methods: Fifty one general physicians from private and public sectors in Northwest of Tabriz were selected randomly and their prescriptions were analyzed. A questionnaire with 8 close questions was completed for each prescription to investigate all aspects of prescribing patterns. By a professional software the information of the prescriptions was analyzed and drug prescribing indicators such as percentage of patients receiving antibiotics, glucocorticoids and other drug groups were determined.The method of intervention included focus group and interactive group discussions. Focus group was built in group of health professionals to collect training material using pre-intervention results for interactive group discussion. The physicians were divided into two groups of control andintervention groups. Following pre-intervention study the physicians of intervention group were discussed and trained in one-day interactive group discussion course using pre-intervention data and educational materials obtained from the focus group discussion.Results: The results of pre-intervention study showed that the average number of drugs in each prescription was 3.82. The percentage of patients receiving antibiotics, corticosteroids and injections were 40.81, 25.94 and 58.04 %, respectively. The results of this survey show great differences from WHO and international standards. Following the intervention the indicators were similar in both intervened and non-intervened groups and also were same as the pre-intervention results.Conclusion: The results of this study show that a very close and effective method of training program as an interactive group discussion has no beneficial effects on improving rational prescription behavior of general physicians involved in this study.
