Sperm acrosomal membrane proteins, such as Izumo sperm-egg fusion 1 (IZUMO1) and sperm acrosome-associated 6 (SPACA6), play essential roles in mammalian gamete binding or fusion. How their biosynthesis is regulated during spermiogenesis has largely remained elusive. Here, we show that 1700029I15Rik knockout male mice are severely subfertile and their spermatozoa do not fuse with eggs. 1700029I15Rik is a type-II transmembrane protein expressed in early round spermatids but not in mature spermatozoa. It interacts with proteins involved in N-linked glycosylation, disulfide isomerization, and endoplasmic reticulum (ER)-Golgi trafficking, suggesting a potential role in nascent protein processing. The ablation of 1700029I15Rik destabilizes non-catalytic subunits of the oligosaccharyltransferase (OST) complex that are pivotal for N-glycosylation. The knockout testes exhibit normal expression of sperm plasma membrane proteins, but decreased abundance of multiple acrosomal membrane proteins involved in fertilization. The knockout sperm show upregulated chaperones related to ER-associated degradation (ERAD) and elevated protein ubiquitination; strikingly, SPACA6 becomes undetectable. Our results support for a specific, 1700029I15Rik-mediated pathway underpinning the biosynthesis of acrosomal membrane proteins during spermiogenesis. [ABSTRACT FROM AUTHOR]

Objectives: To assess the presence of self‐reactive immune responses to seminal and prostate antigens (PAg), biomarkers of inflammation of the male genital tract, and semen quality parameters in patients with chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). Patients, Subjects and Methods: Peripheral blood and semen samples were collected from patients with CP/CPPS and age‐matched healthy control volunteers. We analysed the lymphoproliferative responses of peripheral blood mononuclear cells (PBMC) to different seminal plasma (SP)‐derived and purified PAg, serum autoantibodies specific to PAg, leucocyte subpopulations, and inflammatory cytokines in semen, sperm apoptosis/necrosis, and semen quality parameters. Results: Significantly greater PBMC proliferative responses specific to PAg, with elevated secretion of interferon (IFN)γ and interleukin (IL)‐17, were detected in the patients with CP/CPPS vs the controls. Moreover, the patients with CP/CPPS had significantly greater serum immunoglobulin G immune reactivity to SP proteins, such as prostate‐specific antigen and prostatic acid phosphatase, than the controls. Inflammation of the male genital tract was exemplified by high levels of IFNγ, IL‐17, IL‐1β and IL‐8, as well as higher counts of leukocytes, mainly CD4 T lymphocytes and macrophages, in the semen. In addition, this local inflammation was associated with an overall diminished semen quality, i.e., reduced sperm concentration, motility and viability; and higher levels of sperm apoptosis/necrosis in patients with CP/CPPS vs controls. Conclusion: Patients with CP/CPPS show T helper type 1 (Th1) and Th17 immune responses specific to PAg associated with chronic inflammation of the male genital tract and reduced semen quality. These immune responses may underlie the induction and development of chronic pelvic pain and inflammation of the male genital tract, which in turn could alter normal prostate functioning and impair semen quality. Oxidation of Sperm thiol during sperm maturation is important for sperm components stabilization, the acquisition of sperm motility and the fertilizing ability. The aim of the present study was to measure the sulfhydryloxidase concentration in serum and semen and to look for the association between seminal plasma sulfhydryl oxidase with some of semen parameters in infertile male. The current study included three groups of Iraqi male, 30 infertile patients (15 with oligospermia, 15 with azoospermia) and 15 fertile healthy as a control group. Serum and semen sulfhydryl oxidase concentration was measured in all studied groups. The results of the comparison between this parameters in fertile and non fertile individuals showed. A significant decrease (p< 0.05) in serum sulfhydryl oxidase concentration of oligospermia, a highly significant increase (p< 0.001) of azoospermia. Meantime highly significant increase (p< 0.001) in semen sulfhydryl oxidase concentration of azoospermia, a highly significant decrease (p< 0.001) of oligospermia group. As well as highly significant variations (p< 0.001) between these two patients groups in serum and semen were observed. The personal correlation revealeda significant positive correlation between serum & semen sulfhydryl oxidase concentration in oligospermia group (r = 0.79*, p<0.05). Meanwhile, the correlation between semen SOx in fertile and infertile group with semen parameters revealed a significant positive correlation between semen sulfhydryl oxidase concentration and sperm progressive motility in oligospermia group (r=o.61*, p<0.05) with no correlation with semen count was found. Objectives: Phthalates are environmental chemicals with reproductive toxicity and estrogenic effects in animals. They are of increasing concern to human health. Aim: To determine whether phthalate levels in semen were associated with infertility. Methods: Using semen samples from 107 infertile and 94 fertile men, the presence and quantity of five phthalate esters were measured using high-performance liquid chromatography (HPLC). Using data collected from questionnaires and clinical examinations, the correlation between phthalate exposure and semen quality was analyzed. Results: The cumulative levels of the measured phthalate esters were significantly higher in the infertility group compared to the control group (P<0.05). Concentrations of the five phthalate esters in men varied by age with older men showing higher cumulative levels. Conclusions: The presence of phthalates may contribute to male infertility in our study population. [ABSTRACT FROM AUTHOR]

Background Egr4 is expressed in primary and secondary spermatocytes in adult mouse testes and has a crucial role in regulating germ cell maturation. The functional loss of Egr4 blocks spermatogenesis, significantly reducing the number of spermatozoa that are produced. In this study, we examined whether EGR4 variants are present in Korean men with impaired spermatogenesis. Methods A total 170 Korean men with impaired spermatogenesis and 272 normal controls were screened. The coding regions including exon-intron boundaries of EGR4 were sequenced by PCR-direct sequencing method. Results We identified eight sequence variations in the coding region and 3′-UTR regions of the EGR4 gene. Four were nonsynonymous variants (rs771189047, rs561568849, rs763487015, and rs546250227), three were synonymous variants (rs115948271, rs528939702, and rs7558708), and one variant (rs2229294) was localized in the 3′-UTR. Three nonsynonymous variants [c.65_66InsG (p. Cys23Leufs*37), c.236C > T (p. Pro79Leu), c.1294G > T (p. Val432Leu)] and one synonymous variant [c.1230G > A (p. Thr410)] were not detected in controls. To evaluate the pathogenic effects of nonsynonymous variants, we used seven prediction methods. The c.214C > A (p. Arg72Ser) and c.236C > T (p. Pro79Leu) variants were predicted as “damaging” by SIFT and SNAP2. The c.65_66insG (p. Cys23Leufs*37) variants were predicted as “disease causing” by Mutation Taster, SNPs &GO and SNAP2. The c.867C > G (p. Leu289) variants were predicted as “disease causing” only by Mutation Taster. Conclusion To date, this study is the first to screen the EGR4 gene in relation to male infertility. However, our findings did not clearly explain how nonsynonymous EGR4 variations affect spermatogenesis. Therefore, further studies are required to validate the functional impact of EGR4 variations on spermatogenesis. 8. Clinical Outcomes of Microscopic Subinguinal Varicocelectomy for Male Infertility and Scrotal Pain Patients

【背景・目的】精索静脈瘤は男性不妊症や陰囊痛の原因となり, 手術により造精機能の回復や疼痛の消失が期待できる疾患と認識されている. 近年, 浅鼠径輪より末梢レベルで顕微鏡下に静脈を処理する顕微鏡下低位結紮術が, 低侵襲で再発率の低い手術手技として普及してきている. 今回当院で施行した同手術の手術成績を検討した. 【対象と方法】2004年3月から2013年11月までに当院にて精索静脈瘤が原因と診断した男性不妊および陰囊痛患者に対し顕微鏡下低位結紮術を施行した51例を対象とし, 患者背景, 手術時間, 手術所見,治療効果を検討した.対象症例の年齢は中央値32歳,両側症例は2症例,男性不妊症例は26症例,陰囊痛症例は25症例であった. 【結果】手術は全例で左側のみに対して行われたが, 両側症例でも左側の手術により右側の静脈瘤も消失した. 手術時間は86.1±21.9 分(平均±SD) であった. 動脈温存数は1本が73%, 2本が15.7%, 3本は2%であった. 9.8%で動脈の同定ができず, その80%が疼痛症例であった. 精巣萎縮を来した症例は無かった.疼痛は手術を施行した全症例で改善した.再発は1症例に認め,再手術を要した.合併症は1症例にナットクラッカー症候群による一過性血尿, 60%に一過性軽度陰囊水腫を認めたのみで重篤なものは無かった. 不妊症で手術を行った症例の60%で精子濃度および運動率が改善し, 27%で精子濃度,運動率, 奇形率のいずれかが改善し, 総合的には87%の症例で精液所見の改善が認められた. 【結語】男性不妊症または陰囊痛を主訴とする精索静脈瘤に対する顕微鏡下低位結紮術により, 精液所見の改善を87%に認め, 陰囊痛は全例で改善した. 同術式による精索静脈瘤治療は有効な治療法であると考えられた.
