Abstract

Pancreatic tumor is a deadly disease in which cancerous (malignant) cells form in the tissues of the pancreas. There are numerous types of pancreatic cancers, the most common is pancreatic adenocarcinoma. Almost 90% cases suffer from pancreatic adenocarcinoma. However detecting lesions from medical images is very challenging, given that the shape of the pancreas in abdomen is very irregular, low contrast in edges, variability in location which requires more complex and accurate segmentation of boundary of pancreas from abdomen images. In computer-aided diagnosis system, automatic segmentation of a specific organ and its tumor is very important. The proposed framework provides a way to apply K-means clustering method (unsupervised learning algorithm) on pancreas CT image to detect the area of interest from the background. Our aim is to build an efficient framework to segment tumors from pancreas CT images which is most commonly used in the domain of medical imaging as well as help clinicians in better decision making for surgical planning. It is observed from the results that K-means clustering segmentation algorithm produces more accuracy on CT imaging datasets. Abstract

Gymnema sylvestre (Retz.) R.Br. ex Sm. is a perennial climbing shrub found over the tops of woody trees in tropical forest of India. It is a miracle plant having great anti-diabetic potential. The phytoconstituents of Gymnema sylvestre were isolated and their chemistry and structures were studied and elucidated. An inspection of literature revealed some extraordinary pharmacological activities of this plant. Abstract

Pancreatic cancer is one of the deadliest cancers, with less than 9% survival rates. Pancreatic Ductal Adeno Carcinoma (PDAC) is common with the general public affecting most people older than 45. Early detection of PDAC is often challenging because cancer symptoms will progress only at later stages (advanced stage). One of the earlier symptoms of PDAC is Jaundice. Patients with diabetes, obesity, and alcohol consumption are also at higher risk of having pancreatic cancer. A decision support system is developed to detect pancreatic cancer at an earlier stage to address this challenge. Features such as Mean Hue, Mean Saturation, Mean Value, and Mean Standard Deviation are computed after color space conversion from RGB to HSV. Fuzzy k-Nearest Neighbor (F-kNN) is designed for classification. The system proposed is trained and tested using features extracted from jaundiced eye images. The proposed system results indicate that this model can predict pancreatic cancer as earlier as possible, helping clinicians make better decisions for surgical planning. [ABSTRACT FROM AUTHOR]

Abstract

Systemic sclerosis is a challenging diagnosis for clinicians and pathologists alike due to its protean manifestations and often insidious onset, particularly in cases without significant titres of auto-antibodies. Herein we present a case of a female in her sixties who died rapidly following a clinical diagnosis of pneumatosis intestinalis and respiratory failure of unclear etiology. Recently revised clinical diagnostic criteria were applied to the clinical history and postmortem findings to reach an unexpected diagnosis of systemic sclerosis. The diagnosis of systemic sclerosis at autopsy has important medicolegal implications largely related to premature death due to delayed treatment or poor post-operative outcome. Moreover, familial clustering of this disease underscores the importance of maintaining a high index of suspicion in the postmortem setting.

Managing non functional requirements has been a challenge onto software engineers for a long time. Throughout the years, numerous strategies and methods have been proposed to enhance their elicitation, documentation and approval. Similarly, security requirements emphatically impact the building configuration of complex IT frameworks comparatively as non-functional requirements. Both security building and software engineering furnish strategies to manage such necessities. Nonetheless, there is still a basic crevice concerning the joining of techniques for these two different fields. In this paper we close this crevice as for security prerequisites by proposing a technique that joins software engineering technologies with the best in class security engineering standards. Clinical summary This is the first clinical report of use of a combination of nanocrystalline silver and subatmospheric pressure therapy to treat a resistant wound infection, following tumour removal and radiation therapy, in a difficult-to-manage surgical site in a cat. Practical relevance The therapy was well tolerated and the authors suggest it is a valid treatment protocol for management of non-healing or infected wounds in the cat.

48. Food Environments, Food Security, and Household Food Availability of Circular Migrant Families: A Mixed-Methods Study Among Brick Kiln Laborers in Bihar, India

Abstract

Background: Circular migration is the dominant pattern of movement in India and is a livelihood strategy used by many food insecure rural households. Repeated shifts in food environments have important implications on household food security and dietary patterns but have not been studied.Objective: To explore differences in the food environment, food security, and food availability between home and destination spaces.Methods: Mixed-methods research was conducted among circular migrant families working and residing on brick kilns in the state of Bihar. Utilizing stratified cluster sampling, 2 rounds of cross-sectional data were collected from 2564 families. Additionally, 25 in-depth interviews were conducted with circular migrant parents, kiln owners, and labor contractors. The Food Insecurity Experience Scale was validated for use in our study population. Bivariate analyses were conducted to estimate the association of food insecurity with sociodemographic variables. Qualitative data were analyzed using descriptive thematic methods.Results: Seventy percent of respondents utilized at least one nonmarket source of food at the origin; at the destination, sources of food were limited to the private market. Despite higher food prices at the destination, perceived food affordability was higher during periods of migration, resulting in improved food security. Tubers, rice, and wheat were typically available in the household daily, whereas fruits, eggs, and dairy were typically unavailable during the week.Conclusions: Circular migration can enable short-term food security by improving food affordability. Policy frameworks must address the root causes of chronic food insecurity, especially among rural-to-rural circular migrant families.

Abstract

Aim Little is known about the reasons why tuberculosis (TB) service providers do not follow TB treatment guidelines. A better understanding of their real-world experiences associated with non-compliance with guideline may highlight opportunities for enhanced TB care. Whether the problem is actual or perceived may also affect the type of intervention needed to overcome the barrier. This study aimed to explore TB service providers' reasons for not complying with TB treatment guidelines. MethodA descriptive qualitative design was used. Focus group discussions were held with 39 service providers involved in TB care. The study was carried out in the Tigray region of Ethiopia, in four of the six zonal hospitals. Data were analysed using content analysis. ResultsThe main reasons for non-compliance with TB treatment guidelines were: (1) insufficient diagnostic modalities, limiting the capacity to correctly diagnose TB and provide the right regimen; (2) ambiguity in guideline recommendations especially for specific subgroups of patients, making it difficult to prioritize them to the right regimen; and (3) poor documentation concerning referred patients, with no appropriate examination and difficulty obtaining information from previously treated patients. ConclusionNon-compliance may reflect valid questions about the applicability of guideline recommendations in the local context of health care. Special consideration should be given to previously treated patients, as an important factor influencing guideline compliance. Guideline compliance may also be improved by a better strategy to retest those patients whose initial tests are negative in health care contexts where TB is frequently missed.
