17. A wide next-generation-sequencing panel improves the molecular diagnosis of dyslipidemias

Abstract

Introduction: Dyslipidemias are common clinical conditions associated to cardiovascular diseases. Among these, Familial Hypercholesterolemia (FH) and severe Hypertriglyceridemia (sHTG) are the most frequent ones. The aim of this study is to evaluate the possible use of a wide next-generation-sequencing (NGS) panel of 28 genes involved in lipid metabolism, to improve the molecular diagnosis of dyslipidemias. Methods: A reanalysis of 25 patients (21 FH and 4 sHTG) previously analyzed for a few causative genes has been carried on. Patients bearing different types of variants [single nucleotide variants (SNVs) and copy number variations (CNVs)] in different genes, previously analyzed with Sanger sequencing and multiplex ligation-dependent probe amplification (MLPA) have been selected. DNA libraries have been prepared using Agilent SureSelect target enrichment protocol; the sequencing has been performed using Illumina MiSeq (V2 150x2 Micro). The results of the sequencing have been evaluated by Agilent SureCall and Agilent Alissa Align & Call and Intrepret pipelines. Results: All previously identified SNVs and CNVs have been confirmed by NGS. Additional rare variants, not always associated with dyslipidemias, were found in 23/25 patients. An additional pathogenic variant in the APOB gene has been identified in a sHTG patient carrying only 1 pathogenic variant in the APOA5 gene (causative of sHTG). Conclusions: The NGS-method confirmed all the results obtained with direct sequencing and MLPA methodologies. Additional rare variants were detected, even if most of them turned out to be variant of uncertain significance (VUS). In conclusion, this NGS approach may enhance the molecular diagnosis of different types of dyslipidemias, thereby leading to a better understanding and detection of complex phenotypes.

Abstract

This paper develops a decentralized event-control strategy to avoid deadlock in automated manufacturing systems. On the basis of the characteristics of the part mix that is currently active, the system is partitioned in subsystems that can be controlled locally and independently using graph-theoretic tools. The controller of each subsystem can be chosen as the best compromise between computational costs and flexibility in resource allocation and depending on the particular layout of the subsystem. The decentralized scheme allows the overall system to improve its flexibility and guarantee good performance measures. The approach is particularly suitable for cellular manufacturing systems that do not exclude intercell flow. In this case the method determines the cells that can be controlled independently and the ones that must be controlled jointly. A case study performed by discrete-event simulation confirms the efficiency of the proposed methodology. [ABSTRACT FROM AUTHOR]

Abstract

© 2014 Chinese Association of Automation.This paper introduces an electrical drives control architecture combining a fractional-order controller and a setpoint pre-filter. The former is based on a fractional-order proportional-integral PI unit, with a non-integer order integral action, while the latter can be of integer or non-integer type. To satisfy robustness and dynamic performance specifications, the feedback controller is designed by a loop-shaping technique in the frequency domain. In particular, optimality of the feedback system is pursued to achieve input-output tracking. The setpoint pre-filter is designed by a dynamic inversion technique minimizing the difference between the ideal synthesized command signal i.e., a smooth monotonic response and the prefilter step response. Experimental tests validate the methodology and compare the performance of the proposed architecture with well-established control schemes that employ the classical PIbased symmetrical optimum method with a smoothing pre-filter.

Abstract

Focal nodular hyperplasia (FNH) and adenoma are rare benign hepatic tumors, and the standards for diagnosis and treatment still remain controversial. Usually adenoma is an indication for resection, due to its tendency to bleed and to degenerate; FNH, on the contrary, may be treated conservatively. Preoperation differential diagnosis is, however, difficult, often impossible. Materials and methods. Thirty-eight patients with presumed hepatic adenoma and/or FNH were studied at our department from 1984 to 1996. Preoperative assessment included clinical evaluation and symptoms, laboratory tests, liver biopsy, ultrasound scan, computed tomography scan, magnetic resonance imaging, scintigraphy, and angiography. Thirteen patients had a presumed diagnosis of FNH, 16 of adenoma, and 9 of undetermined benign lesions; 27 had hepatic resections (3 with laparoscopic technique), and 11 were not operated on and are actually under a strict follow-up observation. Results. The final diagnosis was 19 FNH and 19 adenomas (2 of which contained areas of hepatocarcinoma). Presumed diagnosis was confirmed in 71% of cases. Use of oral contraceptives, abdominal symptoms, and pathologic liver test results were more frequent in patients with adenomas. There were no deaths after surgery. All resected patients were tumor free during the follow-up, and in 10 of the 11 nonoperated cases, the size of the nodules remained unchanged. We conclude that precise diagnosis of these benign liver tumors remains difficult and sometimes impossible, despite new imaging techniques. Hepatic resections can be performed under very safe conditions; laparoscopic surgery may play a role in selected cases. Adenomas and uncertain cases are clear indications for surgery. Only when a diagnosis of FNH can be firmly confirmed in asymptomatic patients is strict observation without surgery recommended.

© 2014 IEEE.This paper focuses on a design method for feedback and feedforward fractional order control of electromechanical systems. The architecture combines a fractional order proportional-integral controller and a set-point filter. First, the open-loop frequency response is shaped to obtain robustness specifications and to approximate an optimal feedback system in the input-output tracking, at least in a specified bandwidth. Secondly, the set-point filter is designed by dynamic inversion to minimize the difference between the ideal synthesized command signal, that provides a smooth monotonic response, and the filter step response. Tests on the position/speed control of DC and permanent magnet synchronous motors show the effectiveness of the methodology in comparison with PI controller tuned by symmetrical optimum and coupled with a smoothing filter.
